Background Spigelian hernias (SpH) belong to the group of eponymous abdominal wall hernias. Major reasons for diagnostic difficulties are its low incidence reaching maximum 2% of abdominal wall hernias, a specific anatomical localization with intact external oblique aponeurosis covering the hernia sac and non-constant clinical presentation. Methods A literature review was completed to summarize current knowledge on surgical treatment options and results. Results SpH presents a high incarceration risk and therefore should be operated upon even if the patient is asymptomatic. Both laparoscopic and open repair approaches are validated by current guidelines with lesser postoperative complications and shorter hospital stay in favour of minimally invasive surgery, regardless of the technique used. Overall recurrence rate is very low. Conclusion All diagnosed SpH should be planned for elective operation to prevent strangulated hernia and, therefore emergency surgery. Both open and laparoscopic SpH treatment can be safely performed, depending on surgeon’s experience. In most cases, a mesh repair is generally advised.

Background: Any protrusion through the anterior abdominal wall with the exception of hernia through the inguino-femoral region is defined as ventral hernia. Incisional hernia and primary defects in the abdominal fascia, which can cause umbilical hernia, epigastric hernia, para-umbilical hernia and spigelian hernia are grouped under the definition of ventral hernia. Objective: To evaluate the recurrence rate of laparoscopic ventral hernia repair with closure and non-closure of the hernia defect in non-complicated ventral hernia cases. Patients and Methods: This prospective randomized study was conducted from March, 2015 till December, 2019 in General Surgery Department, Mansoura University Hospital. It involved 50 patients with ventral hernia, who were classified randomly into 2 groups by computer generated sampling technique after retrieval of the calculated sample size according to inclusion and exclusion criteria. Results: The mean age of the non-closure group and of the closure group was 39.96 ± 6.52 and 39.84 ± 7.48 years respectively. The males were 36.0% and females were 64.0% in non-closure group, while in the closure group males were 24.0%, and the females were 76.0%. The mean BMI in the non-closure group was 31.44 ± 3.76 kg/m2, while in the closure group was 30.88 ± 3.70 kg/m2. In the non-closure group, the most common hernia sites were epigastric (60.0%), followed by umbilical (40.0%) while in the closure group, the most common hernia sites were umbilical (96.0%). Conclusion: Laparoscopic ventral hernia repair is a safe and feasible technique. The great advance in abdominal laparoscopic surgery and advance in the equipments and instruments as well as individual skills make closure of the hernia defect represents a good alternative to conventional laparoscopic ventral hernia repair (LVHR) with mesh only.

We introduce a novel approach to the surgical repair of Morgagni hernias (MHs) utilizing the robotic transabdominal preperitoneal repair (rTAPP) approach. Borrowed from our previous and robust experience with rTAPP repairs for hernias of the anterior abdominal wall, this technique boasts the benefits of hernia sac reduction, the use of an uncoated mesh in an extraperitoneal plane, and minimal fixation leading to lower postoperative pain relative to other approaches. To evaluate the effectiveness of this novel approach, five consecutive symptomatic Morgagni hernias (MHs) were repaired with the rTAPP approach. The size of the defect, mesh size, length of stay, follow-up imaging, and follow-up complications were documented for comparison. The size of the MH defects ranged from 4 × 6 cm to 5 × 10 cm. LOS was an average of 1.2 days. Two out of the five patients underwent concomitant repair of a lower abdominal hernias (one Spigelian hernia, and one indirect inguinal hernia). Outpatient follow-up from surgery ranged anywhere from 6 months to 4 years, with most patients receiving follow-up after 1 year. Four out of the five patients received follow-up CT scans to confirm the absence of hernia recurrence. One patient experienced an incisional hernia from the midline 12-mm port site which was repaired 1 year after. We propose a new technique for a minimally invasive strategy to treat these complex hernias utilizing an rTAPP technique resulting in minimal length of stay and a durable result in long-term follow-up. The benefits of repair, which include minimal postoperative pain, minimal length of stay, and cost-effective prosthetic mesh hidden from the visceral contents, are consistent with the author’s experience for rTAPP repairs for hernias of the anterior abdominal wall.

Objective Spigelian hernia is an uncommon congenital or acquired defect in the transversus abdominis aponeurosis with non-specific symptoms posing a diagnostic challenge. There is a paucity of radiology literature on imaging findings of Spigelian hernia. The objective of this study is to explore the role of MDCT in evaluating Spigelian hernia along with clinical and surgical implications. Materials and methods In this IRB approved, HIPAA compliant retrospective observational analysis MDCT imaging findings of 43 Spigelian hernias were evaluated by two fellowship-trained radiologists. Imaging features evaluated were: presence of Spigelian hernia, laterality, relation to “hernia belt” (between 0 and 6 cm cranial to an imaginary axial line between both anterior superior iliac spines), the hernia neck and sac sizes, hernia content, and other coexistent hernias (umbilical, incisional, inguinal). Patient's demographics (age, gender, BMI, conditions with increased intra-abdominal pressure) were also recorded for any correlation. Results 60% (26/43) of Spigelian hernias were located below the hernia belt while 33% (14/43) within the hernia belt and 7% (3/43) above the hernia belt. The most common subtype of Spigelian hernia encountered was interparietal (84%). The mean hernia neck diameter was 3.4 cm, mean hernia sac volume was 329 cc. Hernia content included: fat (43/43) bowel (23/43), fluid (3/43). 3 patients had no clinical history provided, the remaining 37 patients' clinical presentation was asymptomatic in 73% (27/37), acute abdominal pain in 5% (2/37) and chronic abdominal pain in 22% (8/37). None of the hernia were incarcerated and none of the patients underwent emergent surgery. No significant correlation was noted between Spigelian hernia and causes of increased intra-abdominal pressure. 90% of our patients had other abdominal hernias. 30.9 was the mean BMI (20.8–69.1). Conclusion Most of the Spigelian hernia occurred below the traditionally described hernia belt and the majority are of interparietal subtype that can be best diagnosed with MDCT in contrast to physical examination.

A spigelian hernia is a type of ventral hernia where aponeurotic fascia pushes through a holeto create a bulge . It is the protuberance of omentum , adipose tissue or bowel in that weak space between rectus muscles and ventral abdominal obliques that ultimately pushes intestines or superficial fatty tissue through a hole creting a bulge.Spigelian hernias are rare as they are located not under abdominal layers of fat but between fascia interconnecting two muscles. We hereby present a case report on this rare hernia presenting as a bulge in left inguinal fossa of a 66 yr old woman scheduled for total laparoscopic hysterectomy and was repaired subsequently . &nbsp

Laparoscopic ventral hernia repair with intraperitoneal onlay mesh reinforcement is often performed in clinical practice. We herein describe a patient who developed a Spigelian hernia at the edge of the mesh due to rupture of the muscular layer in the abdominal wall. A 69-year-old woman developed a left-sided abdominal bulge 15 months after laparoscopic ventral hernia repair. CT showed a 33-mm defect in the abdominal wall at the lateral edge of the left abdominal rectus muscle with an intestinal prolapse through the defect. She was diagnosed with a Spigelian hernia and underwent operation. The hernia orifice was located at the aponeurosis of the transverse abdominal muscle where the thread had been used to fix the mesh through all layers of the abdominal wall. This report details a case of a Spigelian hernia after laparoscopic ventral hernia repair.

Introduction Spigelian hernia is a rare lateral ventral hernia traditionally repaired through open incision with extensive dissection. Three laparoscopic techniques have been reported in the literature: intraperitoneal onlay mesh (IPOM), transabdominal preperitoneal (TAPP), and total extraperitoneal (TEP). TEP is less popular than the other approaches. We evaluated TEP's safety and effectiveness and compared different laparoscopic techniques. Methods All patients with Spigelian hernia who had undergone extended TEP (eTEP) repair with mesh in our center from January 2007 to February 2020 were studied. A three-port technique with a preperitoneal space created by telescope at the midline was adopted. A systematic review on laparoscopic mesh repair was performed by searching for "Spigelian hernia" and "laparoscopic" from 1999 to 2019 in the MEDLINE database. Results Seven patients underwent eTEP repair for Spigelian hernia. Five presented with abdominal mass and underwent preoperative imaging. Two were diagnosed incidentally during TEP for inguinal hernia. The mean operative duration was 65 minutes (range, 40-93 minutes). There were no open conversions or intraoperative complications. The mean length of hospital stay was 1.4 days (range, 1-3 days). The mean follow-up period was 44.3 months. One patient developed seroma. There was no recurrence or chronic pain. We identified 197 laparoscopic mesh repairs reported in 41 articles. IPOM (n = 91) was the most popular approach, followed by TAPP (n = 70) and TEP (n = 36). Laparoscopic mesh repair of Spigelian hernia is safe and offers excellent outcomes. Conclusion We found the eTEP approach safe and effective for Spigelian hernia repair. IPOM, TAPP, and TEP are comparable.

La hernia de Spiegel es un defecto infrecuente de la pared abdominal producido por una alteración en la unión del borde lateral del músculo recto del abdomen y el borde medial del músculo transverso del abdomen en la línea semilunar. Representan apenas del 0,12 al 2% de todas las hernias. La presente investigación se enfocó en describir las manifestaciones clínicas, el diagnóstico, la incidencia, el procedimiento quirúrgico y su evolución en dos casos reportados. Caso 1: Femenina de 33 años, multípara, obesa, quien consulta por dolor y masa palpable en el cuadrante superior izquierdo del abdomen. Al examen físico, se evidenció dolor a la palpación en mesogastrio e hipocondrio izquierdo, exhibiendo protrusión del contenido abdominal, en la línea de Spiegel. Intervención quirúrgica: hernioplastia protésica de Spiegel con colocación de malla supraaponeurótica. Caso 2: Masculino de 89 años, con diagnóstico de enfermedad pulmonar obstructiva crónica, acudió a consulta presentando dolor abdominal en fosa iliaca y en la región inguino–escrotal izquierda. Al examen físico se evidenció saco herniario en la región inguino–escrotal por lo que se decide su resolución quirúrgica. Como hallazgo transoperatorio se evidenció defecto aponeurótico del recto anterior del abdomen izquierdo en su tercio inferior. Intervención quirúrgica: hernioplastia protésica de Spiegel con colocación de malla supraaponeurótica. Conclusión: La hernia de Spiegel es una entidad poco frecuente por lo que se debe concientizar al cirujano para sospechar este tipo de hernia en aquellos pacientes con presencia de aumento de volumen en la línea de Spiegel.

BACKGROUND Malignant peritoneal mesothelioma is an unusual disease process characterized radiologically by ascites and infiltration of the peritoneum by multiple small tumor nodules. Both parietal and visceral peritoneum are involved by the multiple malignant tumor nodules. Computed tomography (CT) has been used to identify the anatomic pathology induced by the progression of this malignant process. AIM To identify and then describe unusual CT images in patients with malignant peritoneal mesothelioma. Recognition of these unusual radiologic findings can cause the radiologist to be suspicious of this rare malignant process. METHODS In 100 patients who were to undergo definitive treatment of malignant peritoneal mesothelioma, the findings on preoperative CT scans were catalogued. Many of these changes were repeatedly noted on the CT scans. Other pathologic CT images were less common. These unusual radiologic presentations were specially studied in this manuscript. RESULTS Eight unusual radiologic presentations of malignant peritoneal mesothelioma were selected for study. These unusual findings included a mass occurring within a Spigelian hernia, infiltration of the splenic parenchyma by spherical mesothelioma masses, infiltration of the lower mediastinum by tumor, a mesothelioma mass within a left inguinal canal, enlarged cardiophrenic angle lymph nodes, pleural plaques associated with the progression of malignant peritoneal mesothelioma, splenic notches caused by disease surrounding the spleen, and a mass greater than 5 cm associated with the proximal jejunum and directly adjacent to the anatomic location of the Treitz ligament. CONCLUSION There are unusual radiologic presentations of malignant peritoneal mesothelioma that are important to recognize in order to accurately diagnose this disease by CT.

A Spigelian hernia is a rare hernia, making up approximately 0.1% of all abdominal wall hernias. This hernia goes through a defect in the Spigelian fascia which is the part of the transversus abdominis aponeurosis lateral to the rectus muscle, often at the level of the arcuate line, where the fascia is widest and weakest. Clinical diagnosis is difficult in patients without obvious abdominal mass but imaging can be a valuable adjunct in diagnosis. We report the case of a 64-year-old male who presented to our hospital with small bowel obstruction secondary to an incarcerated Spigelian hernia who was pre-operatively diagnosed with ultrasound and computed tomography. At ultrasound and computed tomography a closed loop obstruction in a Spigelian Hernia was detected, resembling on both imaging modalities a "bulb-like" appearance.

Spigelian hernia is an uncommon form of abdominal wall hernia. The symptoms can be insidious but the commonest are abdominal pain or a lump on the anterior abdominal wall. Diagnosis can be challenging, but the use of ultrasonography and contrast enhanced CT (CECT) scan is essential to avoid diagnostic delays. In view of a high risk of incarceration and strangulation anticipated, urgent operative intervention is necessary. In this case report, a 42-year old multiparous woman presented with bilateral lumps on the abdominal wall and abdominal pain with no symptoms or signs of obstruction or strangulation. The clinical evaluation was suspicious of bilateral spigelian hernia. This was confirmed by CECT scan of abdomen as ultrasonography was inconclusive. This case report illustrates the diagnostic challenges of spigelian hernia and how early operative intervention is crucial to preventing strangulation.

The operative technique for Spigelian hernia (SH) continues to evolve over the last few decades. The primary endpoint of this systematic review was to evaluate the evolution of surgical technique for the management of SH in adults, and to identify the optimum technique for easy identification of anatomical landmarks. The secondary outcome was to evaluate the operative management of bilateral SH. A systematic search using Cochrane, Medline, and Google Scholar databases was performed to identify all studies from 1989 to 2019 investigating the management of SH. Of 535 articles identified, 29 studies reporting on 619 patients (389 open repair and 230 laparoscopic repair) were included, out of which 25 studies (469 patients) described complication and recurrence rate. Five additional studies reporting on five patients with bilateral SH were identified for a total of 19 cases (3.1%) of bilateral SH. There has been a gradual transition from open to laparoscopic SH repair over the last three decades. Overall complication and recurrence rates after SH repair were 5.5% and 2.8% respectively, with higher rates in open repair (complication 7.7% and recurrence 4.2%). The laparoscopic transabdominal preperitoneal repair (TAPP) has the lowest recurrence and complication rate reported in the literature. The choice of SH repair should be based on the need for abdominal wall reconstruction, any associated intra-abdominal pathology, and the experience of the surgeon. Laparoscopic TAPP with mesh appears to have the lowest complication and recurrence rate.

Highlights • Hernia repair is associated with higher morbidity in chronic liver disease. • Chronic abdominal pain following transplantation is a diagnostic challenge. • Incidental appendectomy in the immunosuppressed carries additional risk. • Mesh repair can be safely performed following liver transplantation. • Uncommon ventral hernias can be the source of chronic abdominal pain following liver transplantation., Introduction Abdominal ventral hernias are common in chronic liver disease due to increased abdominal pressure and sarcopenia. Following liver transplantation, diagnosis of chronic abdominal pain is challenging because it may relate to immunosuppression, scaring or opportunistic infections. Presentation of case A 62 years-old male presented with chronic abdominal pain one year following liver transplantation due to hepatocellular carcinoma. After work-up he was diagnosed with a Spigelian hernia containing the appendix. We did hernia repair with mesh but appendectomy was not performed since it showed no signs of inflammation. On follow-up the patient had complete resolution of the pain. Discussion This is the first case of spigelian hernia containing the appendix following liver transplantation. Mesh repair can be safely performed in this setting but incidental appendectomy is controversial due to higher morbidity and mortality. In this case report we discuss the relationship between liver transplantation, abdominal hernias and the pitfalls of incidental appendectomy. Conclusion Uncommon ventral hernias are a possible cause for chronic abdominal pain after surgery and should be investigated with imaging studies. Mesh repair is safe but incidental appendectomy in the immunosuppressed is not encouraged due to increased morbidity.

Introduction: There is the obscurity of published data on surgical management of external abdominal wall hernias in our environment. This study was, therefore, to describe the pattern, outcome, and experience in the surgical management of anterior abdominal wall hernias in Shisong, Cameroon. Methods: This was a descriptive retrospective study conducted at St. Elizabeth Catholic General Hospital, Shisong. The various cases of abdominal wall hernias performed in the hospital during the study period of three years covering January 2017 to December 2019 by the surgical teams were included. We ensured that Ethical clearance and written informed consent were obtained before the start of the project. Information includes detailed sociodemographic data collected and analyzed by using SPSS 22 statistical software. Results: 465 cases of various abdominal wall hernias were included in this study. The most common type of hernia was indirect inguinal hernia (86.5%), and one rare Spigelian hernia was also there. The sub-arachnoid block was the most prevalent form of anesthesia. The various surgical procedures performed, including herniotomy (8.7%), hernioplasty (38.7%), and herniorrhaphy (28.6%), which were done for various inguinal hernias. In contrast, only mesh repair (9%) was performed for epigastric, lumbar, umbilical, Spigelian, and incisional hernia. Amongst the inguinal hernias, 55.2% were right-sided, 40.6% were left-sided, and 4.2% were bilateral, whereas 50% each of femoral hernia was left and right-sided. Overall, the annual recurrence rate for groin hernia was 1.9%. Conclusion: Inguinal Hernia was the most common type of hernia (86.3%). Among the inguinal hernia, the right side was more common (58.42%). The adoption of newer modalities of care should be considered standard. However, in resource-constrained settings like ours in Cameroon, the goal should be to perform a skillful and technically effective technique. Spigelian hernia is an uncommon surgical condition in children and accounts for only 1% of all abdominal hernias. Its traumatic origin has been described sporadically in the literature.We report a case of a 9-year-old boy, brought to the emergency department 2 hours after abdominal trauma caused by a bicycle's handlebar grip at the lower abdomen, complaining of abdominal pain. Apart from edema, bruising and tenderness of the right of lower abdomen, physical examination showed a 1.5 * 2 cm reducible swelling lateral to the rectus muscle and a 1-cm facial defect of the anterior abdominal wall at the same site. Ultrasonography concluded to diastasis between the rectus abdominis muscle and the broad abdominal muscles with evidence of a hernia sac. The diagnosis of traumatic Spigelian hernia was retained and the patient was operated on. He had primary repair of the defect in layers using interrupted sutures. He was discharged in the same day and the aftermath of surgery was uneventful.In conclusion, traumatic Spigelian hernias are rare entities in children which diagnosis should be considered following blunt abdominal trauma. We recommend careful abdominal examination after trauma to highlight swelling and/or aponeurotic defect.

Only 0.12% to 2% of diagnosed hernias are Spigelian type. Even less frequently encountered—Grynfeltt-Lesshaft hernias—hernias have unknown incidence. A Spigelian hernia is encountered along the Spigelian fascia and Grynfeltt-Lesshaft hernias are bounded by the superior lumbar triangle. These unique hernias can both be intermuscular, given their anatomical borders which allow concealment and preclusion of accurate diagnosis. Here, an 86-year-old male presented with symptoms consistent with small bowel obstruction. On physical exam, a right lower quadrant hernia and right posterior flank mass were appreciated. Computed tomography revealed obstruction secondary to bowel incarceration within Spigelian hernia and additional Grynfeltt-Lesshaft hernia. The patient underwent reduction and repair of Spigelian hernia with synthetic mesh, while repair of asymptomatic hernia was deferred. These unusual hernias are difficult to distinguish, given their negligible occurrence and unreliable exam findings. Clinicians must remain cognizant of their features to aid in diagnosis and mitigate potential sequelae.

Aim “Spigelian hernia is uncommon and accounts for only 0.12–2% of all abdominal hernias. It is mandatory to perform surgical correction and in recent years the laparoscopic approach is gaining ground. The authors pretend to demonstrate a video of an outpatient laparoscopic repair of Spigelian hernia.” Material and Methods “53-year-old woman with a left Spigelian hernia referred to laparoscopic repair via intraperitoneal approach.” Results “The patient was submitted to laparoscopic correction with a Ventralex® mesh. The surgery went without any complication and under 30 minutes. A few hours after the surgery, the patient was discharged. In the follow-up appointment, the patient had no complaints or evidence of recurrence.” Conclusions “Spigelian hernias are rare and have a mandatory surgical indication. Traditionally, open surgical repair is most commonly used. However, laparoscopic approach is becoming increasingly popular since it allows faster recovery, shorter hospital stay, and less pain, with no commitment to recurrence. Currently, there are no studies that demonstrate the superiority of a laparoscopic technique (intraperitoneal, TAPP or TEP). The intraperitoneal route is a simple, faster, and easily reproducible approach.”

Abdominal wall hernias are a common imaging finding. Their most common complications include bowel obstruction, incarceration, strangulation and trauma. We report an exceptional case of spigelian hernia in 55 years old women complicated by perforation and massive subcutaneous emphysema which was correctly diagnosed by CT scan. This case showed the interest of CT scan with multiplanar reconstructions in the confirmation of the diagnosis.

Background: Spigelian hernia (SH) is a rare entity characterized by a defect of the anterior abdominal wall located along the Spigelian line, it may be congenital or acquired.Association with other anomalies is worth reporting. Case Presentation: A 22-day-old male newborn was admitted with strangulated right inguinal hernia and operated on emergently. Clinical examination also revealed a Spigelian hernia in the left lower abdominal quadrant with bilateral cryptorchidism, associated with polydactyly of the small right finger and anal stenosis. At surgery, the SH contained a part of the small intestine and the ipsilateral undescended testis. Conclusion: Pediatric SH is rare, but its association with undescended ipsilateral testis is frequent. Other abnormalities can be concomitant to this association.

A 66-year-old man with abdominal pain had a 5-year-old mass subsequently identified as a Spigelian hernia. Exploratory laparotomy revealed a strangulated and gangrenous appendix contained within the hernia, necessitating an appendectomy. This case highlights the importance of early identification and exploration of this rare hernia, so that early management can prevent the development of more serious and dangerous symptoms.

PURPOSE- Spigelian Hernia[SH] constitutes 0.12% to 2% of total abdominal wall hernia. The diagnosis of SH poses great difficulty because of non-specific presentation, and non obvious finding on examination. Also, Surgeons are less experienced with this entity and have low index of suspicion further contributing to underdiagnosis. Thus, whether SH is a rare entity or an underdiagnosed condition is debatable. The aim is share our experience and discuss different clinical aspects and outcomes of SH with a literature review regarding diagnostic difficulties. PATIENTS AND METHODS- This is a case series of 18 patients diagnosed with 20 SH, were operated in a single surgical unit of Sir Sunderlal Hospital from January 2016 to July 2019. Historical patient’s records including demographic profiles, clinical presentation, and management were analysed. RESULTS- Total 18 patients were admitted with women preponderance. The mean age of presentation was 39.23years. The most common presentation is pain followed by swelling. Most patients don’t have any clinical finding. Defect is identified only in 3 patients. Ultrasound, Contrast-Enhanced Computed Tomography and Magnetic Resonance Imaging were used to diagnose the hernia. All the patients were managed operatively with pre-peritoneal fat as the most frequent content. CONCLUSION- SH is a diagnostic challenge with rather simple management. A high index of clinical suspicion is the key to diagnosis. SH seems more of an under-diagnosed rather than a rare entity. Also the author suggests to scan whole length of spigelian aponeurosis whenever there is a suspicion or the failure to reach an otherwise diagnosis.

Background Rare locations of hernias, as well as primary ventral hernias under certain circumstances (cirrhosis, dialysis, rectus diastasis, subsequent pregnancy), might be technically challenging. The aim was to identify situations where the treatment strategy might deviate from routine management. Methods The guideline group consisted of surgeons from the European and Americas Hernia Societies. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used in formulating the recommendations. The Scottish Intercollegiate Guidelines Network (SIGN) critical appraisal checklists were used to evaluate the quality of full‐text papers. A systematic literature search was performed on 1 May 2018 and updated 1 February 2019. The Appraisal of Guidelines for Research and Evaluation (AGREE) instrument was followed. Results Literature was limited in quantity and quality. A majority of the recommendations were graded as weak, based on low quality of evidence. In patients with cirrhosis or on dialysis, a preperitoneal mesh repair is suggested. Subsequent pregnancy is a risk factor for recurrence. Repair should be postponed until after the last pregnancy. For patients with a concomitant rectus diastasis or those with a Spigelian or lumbar hernia, no recommendation could be made for treatment strategy owing to lack of evidence. Conclusion This is the first European and American guideline on the treatment of umbilical and epigastric hernias in patients with special conditions, including Spigelian and lumbar hernias. All recommendations were weak owing to a lack of evidence. Further studies are needed on patients with rectus diastasis, Spigelian and lumbar hernias., This guideline addresses the treatment of umbilical and epigastric hernias in clinically challenging situations (cirrhosis, dialysis, subsequent pregnancy, rectus diastasis), as well as the treatment of Spigelian and primary lumbar hernias. Spigelian hernias constitute a minute fraction of all abdominal hernias. In this monography, we present a case report of this relatively seldom seen phenomenon which some general surgeons never get to see during their medical career.

In this study, we aimed to present that incarcerated Spigelian hernia is an important cause of acute abdomen although it is rare and accounts for 1-2% of all abdominal wall hernias. Spigelian hernia arises from a defect in the aponeurosis of the transversus abdominis muscle, also known as the Spiegel fascia. This case analysis aims to present a Spigelian hernia case in which the sigmoid colon is incarcerated. The patient was referred to our emergency surgery clinic complaining of severe abdominal pain and a palpable mass in the left quadrant of the abdomen, presenting tenderness on the front abdominal wall. The symptoms suddenly emerged approximately eight hours ago before the patient was admitted to the hospital. The patient was taken into surgery after the ultrasonography (US) and computed tomography (CT) results suggested a preliminary diagnosis of incarcerated Spigelian hernia for which polypropylene mesh repair was performed. No recurrence was identified in the patient's control examination performed 22 months later. Incarcerated Speigel hernia should be considered as a cause for patients developing sudden stomach ache and mass, causing tenderness on the front abdominal wall for which mesh repair should be performed.

Spigelian hernia is one of the most uncommon hernia of the abdominal wall. Authors present 53 years old women with left sided spigelian hernia containing: cae-cum with appendix and ileum, which was a uncommon content of hernial sac. Past medical history of urinary bladder operation suggested postoperative hernia. However the correct diagnosis was made during the operation. Hernioplasty was made with mesh onlay method. The postoperative course was unevenful.

Post-traumatic spigelian hernia is a rare entity and requires a high index of suspicion for its detection. Mostly the cases are diagnosed at the time of first presentation when a swelling is seen at the site of injury. We report a case of missed post-traumatic abdominal wall hernia at the left spigelian point in a 3-year child missed during initial exploration for traumatic jejunal perforation which led to re-exploration for incarceration of hernia. The rarity of presentation as a post-operative complication has not been reported in literature.

Background Misdiagnosis or failure to intraoperatively detect occult hernia in the inguinal region can lead to the recurrence of postoperative hernia and the appearance of local pain symptoms, which affect the patient’s quality of life and make it difficult to reperform hernia repair. Methods This study included 1066 inguinal hernia patients who underwent surgical treatment at Shanghai Tongren Hospital between January 2016 and October 2018 to investigate ipsilateral occult hernia epidemiology, to analyze the characteristics of ipsilateral occult hernias with regards to patient age, gender, classification and anatomical site, and to explore the superiority and inferiority of the expert hernia surgeons/ non-expert hernia surgeons group and of operation methods in finding occult inguinal hernias. Results The incidence of ipsilateral occult hernia in the surgical population was 8.26%. Ipsilateral occult hernia included indirect inguinal hernia, direct inguinal hernia, femoral hernia, obturator hernia, and spigelian hernia, among which the highest incidence was direct inguinal hernia (4.11%), followed by indirect inguinal hernia (2.45%). There was no difference in the incidence of ipsilateral occult hernia between males and females, but there were significant differences in the incidence of ipsilateral occult hernia, which decreased gradually with increasing age in patients younger than 70 years-old; there was no difference in incidence in patients over 70 years-old. There were significant differences in the incidence of ipsilateral occult hernia in the bilateral inguinal region between direct and femoral hernia, with the higher incidence found on the right side; in contrast, there was no difference in the incidence of indirect inguinal hernia in the bilateral inguinal region. There was no difference in the ability of experienced physicians to detect ipsilateral occult inguinal hernias, either professionally or by surgery. Conclusions Ipsilateral occult inguinal hernia has a higher incidence in patients with inguinal hernia, especially older patients; therefore, it is necessary for experienced surgeons to carefully detect for possible occult hernia during the operation and in elderly patients.

Spigelian hernia (SH) occurs due to the protrusion through a congenital or acquired defect or weakness in the Spigelian aponeurosis. SH accounts for only 0.1–0.4% of occurrence and a 17–24% risk of strangulation. We hereby report a case of a 34-year-old gentleman presented with concomitant incarceration of the omentum with small intestine and testis in Spigelian hernia sac. We have successfully operated on this patient via a transperitoneal approach with a small incision over the hernia site. This incision could be an alternative to midline laparotomy as a safe and effective method in managing incarcerated SH in an emergency setting.

Spigelian hernia-undescended testes (SH-UDT) syndrome is a rare disorder, with only 57 cases reported in the literature. The presentation can be asymptomatic or symptomatic in the form of pain, tenderness, or a lump. We present a case of a 50-day-old boy with SH-UDT syndrome. The patient presented with signs and symptoms of acute intestinal obstruction. Exploration confirmed a Spigelian hernia containing small bowel loops and right undescended testis. Orchidopexy was done after ligation of the hernial sac. A follow-up visit after two years revealed right testicular atrophy.

Rare forms of hernias can cause diagnostic difficulties that require special modifications of operating methods. Due to their rarity, they may lead to a serious threat to the health and life of patients. We present three patients with unique sudden hernias that may be problematic in clinical practice. We have chosen cases that were urgently operated in our department. The described cases of obturator hernia, Spiegel's hernia and strangulated inguinal hernia containing a bladder in the hernial sac require more attention not only because of their rarity, but above all because of unusual symptoms leading to delayed diagnosis. This may result in a delayed start of treatment and, consequently, an increase in mortality. The surgeon often finds out about the difference of hernia only during the operation, therefore everyone should know the techniques of managing such cases. Medical emergencies included in this paper demonstrate that unique pathologies must remain in the mind of the surgeons and offer a comprehensive review.

The anterior abdominal wall is an exceptionally rare variant site for ectopic testis. The purpose of this case report is to highlight the importance of being aware of this rare clinical entity, thereby increasing the chance of preserving an ectopic testis. In addition, this case report shows how a laparoscopic approach is advantageous for better localizing and treating such rare cases, as this is the first reported case of such a condition to be managed with a laparoscopic approach. Case Report: This is a rare case of testicular ectopia in an unclassified abdominal wall defect mimicking a Spigelian hernia that occurred in a 1.5-year-old boy. He presented with congenital swelling of the right lower abdominal wall and an empty right hemiscrotum. Diagnostic laparoscopy was performed and the intraoperative findings suggested right testicular ectopia in the anterior abdominal wall defect. The right testis was herniated through an abnormal, unclassified, abdominal wall defect with both a closed inguinal ring and no defect in the semilunar line. Using a laparoscopic approach, the ectopic testis and hernia were managed successfully with primary hernia repair and single-stage orchidopexy. The patient’s postoperative recovery was uneventful. At his 10-month follow-up appointment, he had no evidence of hernia recurrence. Conclusions: As demonstrated in this case, laparoscopic surgery, which has not been used in previously reported cases of ec-topic testis, aids in proper localization and repair of an anterior abdominal wall defect along with orchidopexy.

History of cryptorchidism is present in about 10% of infertile patients seeking medical help, whereas 20% of them are azoospermic. Most of the patients with bilateral cryptorchidism have a low testicular volume and high serum FSH level. Ectopic testes are present only in 5% of the patients with cryptorchidism. The anterior abdominal wall is a rare site for ectopic testis where Spigelian hernia is usually accompanied. We present a case of bilateral ectopic anterior abdominal wall testes associated with Spigelian hernia on the left side.

Spigelian hernia is a rare form of abdominal wall hernias. We report an unusual case of a strangulated spigelian hernia with necrotic contents of the caecum, appendix and terminal ileum in the elderly. An 84-year-old woman, with a history of eight pregnancies, was admitted for bowel obstruction evolving for 7 days. The computed tomography scan showed a strangulated right spigelian hernia. Surgical exploration with an incision centered on the mass confirmed the diagnosis with extensive necrosis of the caecum, appendix and 10 cm of the terminal ileum. A right hemi-colectomy with an ileo-colic anastomosis was performed. The post-operative course was marked by the death of the patient in intensive care due to multi-visceral failure. The occurrence of strangulation with necrosis of the caecum, appendix and terminal ileum in spigelian hernia is extremely rare. It requires early diagnosis and treatment to reduce post-operative morbidity and mortality.

Spigelian hernias are generally considered to occur through solitary defects in the fascial layers of the anterior abdominal wall in the ‘Spigelian hernia belt’ but can be found anywhere along the line of the linea semilunaris. They are uncommon in children and in adults thought to be acquired and associated with obesity. We describe an unusual case of Spigelian herniation previously unreported and possibly previously unrecognised.

It rarely presents as Small Bowel Obstruction or reaches dimensions that becomes clinically remarkable. Presentation of case 84-year-old woman entered our Surgical Department for Small Bowel Obstruction due to a giant (8 × 7 cm) abdominal wall hernia, which was intraoperatively identified as Spigelian Hernia. We performed a minilaparotomy with reduction of viable small bowel and preperitoneal positioning of polypropilene mesh. Postoperative course was uneventfull. Discussion Due to its small dimensions and infrequence, the diagnosis could be challenging even if the patient undergoes a CT scan. The presentation with clear signs of small bowel obstruction associated with a large abdominal hernia is rare and suggests a urgent surgical approach with mesh repair to avoid recurrences. Conclusion Even if rarely symptomatic, the Spigelian Hernia is an entity to consider in the differential diagnosis of small bowel obstruction in a virgin abdomen. Preoperative diagnosis, when available, is mandatory to guide a correct surgical approach.

Background: Is to describe the pathological, clinical and radiological features of Spigelian hernia, and to compare between open and laparoscopic methods of its surgical treatment. Patients and methods: We describe the clinical management of 20 cases of Spigelian hernias as regard patient complaint, clinical examination, radiological investigations, and the surgical approach that was randomly performed either open or laparoscopic intraperitoneal only mesh. Results: In all cases, the suspicion aroused during the clinical examination of the patients, and it was confirmed via imaging techniques. 10 cases were treated through open surgical approach, no intraoperative or postoperative complications have occurred, 3 patients had postoperative small wound seroma which resolved spontaneously, Other10 cases were treated through laparoscopic intraperitoneal onlay mesh repair with no intraoperative or postoperative complications. No recurrences had been observed for both groups in 24 months period of the study. Conclusion: Spigelian hernia is a rare entity; its diagnosis is depending on a thorough clinical examination and must be confirmed by imaging studies especially CT scan which is the modality of choice for its diagnosis. Open surgical repair is considered the standard method for treatment due to its rarity but laparoscopic intraperitoneal onlay mesh repair is also safe and effective with shorter postoperative hospital stay.

A Spigelian hernia is a very rare hernia, making up approximately 0.1% of all abdominal wall hernias. This hernia goes through a defect in the Spigelian fascia which is the part of the transversus abdominis aponeurosis lateral to the rectus muscle, often at the level of the arcuate line, where the fascia is widest and weakest. We present the case of a 77-year-old female with no past surgical history who presented to our teaching hospital with high-grade small bowel obstruction secondary to an incarcerated Spigelian hernia. She was taken to the operating room for a laparotomy and a portion of the small bowel mesentery was found to be strangulated. The hernia was reduced, and the defect was repaired primarily. The diagnosis of a Spigelian hernia can often be difficult to diagnose on history and physical examination alone, but computed tomography (CT) imaging can be a valuable adjunct in diagnosis.
