In Paraguay, 49% of the population depends on biomass (wood and charcoal) for cooking. Residential biomass burning is a major source of fine particulate matter (PM 2.5 ) and carbon monoxide (CO) in and around the household environment. In July 2016, cross-sectional household air pollution sampling was conducted in 80 households in rural Paraguay. Time-integrated samples (24 hours) of PM 2.5 and continuous CO concentrations were measured in kitchens that used wood, charcoal, liquefied petroleum gas (LPG), or electricity to cook. Qualitative and quantitative household-level variables were captured using questionnaires. The average PM 2.5 concentration (μg/m 3 ) was higher in kitchens that burned wood (741.7 ± 546.4) and charcoal (107.0 ± 68.6) than in kitchens where LPG (52.3 ± 18.9) or electricity (52.0 ± 14.8) was used. Likewise, the average CO concentration (ppm) was higher in kitchens that used wood (19.4 ± 12.6) and charcoal (7.6 ± 6.5) than in those that used LPG (0.5 ± 0.6) or electricity (0.4 ± 0.6). Multivariable linear regression was conducted to generate predictive models for indoor PM 2.5 and CO concentrations (predicted R 2  = 0.837 and 0.822, respectively). Background: Household air pollution from solid fuel burning is a leading contributor to disease burden globally. Fine particulate matter (PM 2.5 ) is thought to be responsible for many of these health impacts. A co-pollutant, carbon monoxide (CO) has been widely used as a surrogate measure of PM 2.5 in studies of household air pollution., Objective: The goal was to evaluate the validity of exposure to CO as a surrogate of exposure to PM 2.5 in studies of household air pollution and the consistency of the PM 2.5 -CO relationship across different study settings and conditions., Methods: We conducted a systematic review of studies with exposure and/or cooking area PM 2.5 and CO measurements and assembled 2,048 PM 2.5 and CO measurements from a subset of studies (18 cooking area studies and 9 personal exposure studies) retained in the systematic review. We conducted pooled multivariate analyses of PM 2.5 -CO associations, evaluating fuels, urbanicity, season, study, and CO methods as covariates and effect modifiers., Results: We retained 61 of 70 studies for review, representing 27 countries. Reported PM 2.5 -CO correlations ( r ) were lower for personal exposure (range: 0.22-0.97; median=0.57) than for cooking areas (range: 0.10-0.96; median=0.71). In the pooled analyses of personal exposure and cooking area concentrations, the variation in ln(CO) explained 13% and 48% of the variation in ln(PM 2.5 ), respectively., Conclusions: Our results suggest that exposure to CO is not a consistently valid surrogate measure of exposure to PM 2.5 . Studies measuring CO exposure as a surrogate measure of PM exposure should conduct local validation studies for different stove/fuel types and seasons. https://doi.org/10.1289/EHP767.

Rationale: Household air pollution (HAP) from solid fuel combustion is a major contributor to the global burden of disease, with considerable impact from respiratory infections in children. The impact of HAP on lung function is unknown., Objectives: The Childhood Exposure to Respirable Particulate Matter (CRECER) prospective cohort study followed Guatemalan children who participated in the Randomised Exposure Study of Pollution Indoors and Respiratory Effects (RESPIRE) trial of a chimney stove intervention to determine the effect of early childhood HAP exposure on growth of lung function., Methods: RESPIRE households with pregnant women or infant children were randomised to receive a chimney stove at the beginning or at the end of the 18-month trial. During CRECER, a subset of these children, as well as children from households with newly installed stoves, were followed with spirometry beginning at age 5. Biomass smoke exposure was measured using personal carbon monoxide tubes. Two-stage regression models were employed to analyse associations with lung function growth., Measurements and Main Results: Longitudinal peak expiratory flow (PEF) and FEV1 data were available for 443 and 437 children, respectively, aged 5-8 (mean follow-up 1.3 years). Decreases in PEF growth of 173 mL/min/year (95% CI -341 to -7) and FEV1 of 44 mL/year (95% CI -91 to 4) were observed with stove installation at 18 months compared with stove installation at birth in analyses adjusted for multiple covariates. No statistically significant associations were observed between personal HAP exposure and lung function., Conclusions: A significant decrease in PEF growth and a large non-significant decrease in FEV1 growth were observed with later stove installation. Background: COPD is the third most frequent cause of death globally, with much of this burden attributable to household biomass smoke exposure in developing countries. As biomass smoke exposure is also associated with cardiovascular disease, lower respiratory infection, lung cancer, and cataracts, it presents an important target for public health intervention., Methods: Lung function in Guatemalan women exposed to wood smoke from open fires was measured throughout the Randomized Exposure Study of Pollution Indoors and Respiratory Effects (RESPIRE) stove intervention trial and continued during the Chronic Respiratory Effects of Early Childhood Exposure to Respirable Particulate Matter (CRECER) cohort study. In RESPIRE, early stove households received a chimney woodstove at the beginning of the 18-month trial, and delayed stove households received a stove at trial completion. Personal exposure to wood smoke was assessed with exhaled breath carbon monoxide (CO) and personal CO tubes. Change in lung function between intervention groups and as a function of wood smoke exposure was assessed using random effects models., Results: Of 306 women participating in both studies, acceptable spirometry was collected in 129 early stove and 136 delayed stove households (n = 265), with a mean follow-up of 5.6 years. Despite reduced wood smoke exposures in early stove households, there were no significant differences in any of the measured spirometric variables during the study period (FEV1, FVC, FEV1/FVC ratio, and annual change) after adjustment for confounding., Conclusions: In these young Guatemalan women, there was no association between lung function and early randomization to a chimney stove or personal wood smoke exposure. Future stove intervention trials should incorporate cleaner stoves, longer follow-up, or potentially susceptible groups to identify meaningful differences in lung function.

Background: Household air pollution (HAP) resulting from the use of solid cooking fuels is a leading contributor to the burden of disease in India. Advanced combustion cookstoves that reduce emissions from biomass fuels have been considered potential interventions to reduce this burden. Relatively little effort has been directed, however, to assessing the concentration and exposure changes associated with the introduction of such devices in households., Objectives: The aim of this study was to describe HAP exposure patterns in pregnant women receiving a forced-draft advanced combustion cookstove (Philips model HD 4012) in the SOMAARTH Demographic Development & Environmental Surveillance Site (DDESS) Palwal District, Haryana, India. The monitoring was performed as part of a feasibility study to inform a potential large-scale HAP intervention (Newborn Stove trial) directed at pregnant women and newborns., Methods: This was a paired comparison exercise study with measurements of 24-hour personal exposures and kitchen area concentrations of carbon monoxide (CO) and particulate matter less than 2.5 μm in aerodynamic diameter (PM2.5), before and after the cookstove intervention. Women (N = 65) were recruited from 4 villages of SOMAARTH DDESS. Measurements were performed between December 2011 and March 2013. Ambient measurements of PM2.5 were also performed throughout the study period., Findings: Measurements showed modest improvements in 24-hour average concentrations and exposures for PM2.5 and CO (ranging from 16% to 57%) with the use of the new stoves. Only those for CO showed statistically significant reductions., Conclusion: Results from the present study did not support the widespread use of this type of stove in this population as a means to reliably provide health-relevant reductions in HAP exposures for pregnant women compared with open biomass cookstoves. The feasibility assessment identified multiple factors related to user requirements and scale of adoption within communities that affect the field efficacy of advanced combustion cookstoves as well as their potential performance in HAP intervention studies., (Copyright © 2015 The Authors. Household air pollution (HAP) due to solid fuel use is a major public health threat in low-income countries. Most health effects are thought to be related to exposure to the fine particulate matter (PM) component of HAP, but it is currently impractical to measure personal exposure to PM in large studies. Carbon monoxide (CO) has been shown in cross-sectional analyses to be a reliable surrogate for particles<2.5 µm in diameter (PM2.5) in kitchens where wood-burning cookfires are a dominant source, but it is unknown whether a similar PM2.5-CO relationship exists for personal exposures longitudinally. We repeatedly measured (216 measures, 116 women) 24-hour personal PM2.5 (median [IQR] = 0.11 [0.05, 0.21] mg/m(3)) and CO (median [IQR] = 1.18 [0.50, 2.37] mg/m(3)) among women cooking over open woodfires or chimney woodstoves in Guatemala. Pollution measures were natural-log transformed for analyses. In linear mixed effects models with random subject intercepts, we found that personal CO explained 78% of between-subject variance in personal PM2.5. We did not see a difference in slope by stove type. This work provides evidence that in settings where there is a dominant source of biomass combustion, repeated measures of personal CO can be used as a reliable surrogate for an individual's PM2.5 exposure. This finding has important implications for the feasibility of reliably estimating long-term (months to years) PM2.5 exposure in large-scale epidemiological and intervention studies of HAP.

Background: In India, approximately 66% of households rely on dung or woody biomass as fuels for cooking. These fuels are burned under inefficient conditions, leading to household air pollution (HAP) and exposure to smoke containing toxic substances. Large-scale intervention efforts need to be informed by careful piloting to address multiple methodological and sociocultural issues. This exploratory study provides preliminary data for such an exercise from Palwal District, Haryana, India., Methods: Traditional cooking practices were assessed through semi-structured interviews in participating households. Philips and Oorja, two brands of commercially available advanced cookstoves with small blowers to improve combustion, were deployed in these households. Concentrations of particulate matter (PM) with a diameter <2.5 μm (PM2.5) and carbon monoxide (CO) related to traditional stove use were measured using real-time and integrated personal, microenvironmental samplers for optimizing protocols to evaluate exposure reduction. Qualitative data on acceptability of advanced stoves and objective measures of stove usage were also collected., Results: Twenty-eight of the thirty-two participating households had outdoor primary cooking spaces. Twenty households had liquefied petroleum gas (LPG) but preferred traditional stoves as the cost of LPG was higher and because meals cooked on traditional stoves were perceived to taste better. Kitchen area concentrations and kitchen personal concentrations assessed during cooking events were very high, with respective mean PM2.5 concentrations of 468 and 718 µg/m3. Twenty-four hour outdoor concentrations averaged 400 µg/m3. Twenty-four hour personal CO concentrations ranged between 0.82 and 5.27 ppm. The Philips stove was used more often and for more hours than the Oorja., Conclusions: The high PM and CO concentrations reinforce the need for interventions that reduce HAP exposure in the aforementioned community. Of the two stoves tested, participants expressed satisfaction with the Philips brand as it met the local criteria for usability. Further understanding of how the introduction of an advanced stove influences patterns of household energy use is needed. The preliminary data provided here would be useful for designing feasibility and/or pilot studies aimed at intervention efforts locally and nationally.

We investigated whether early life chronic exposure to woodsmoke, using personal passive 48-h carbon monoxide (CO) as an indicator, is associated with children's neurodevelopmental and behavioral performance. CO measures were collected every 3 months from 2002 to 2005 among mother-child dyads during the Randomized Exposure Study of Pollution Indoors and Respiratory Effects (RESPIRE) stove intervention trial in San Marcos, Guatemala. From March to June, 2010, study children of age 6-7 years, performed a follow-up non-verbal, culturally adapted neurodevelopmental assessment. We found inverse associations between CO exposure of pregnant mothers during their 3rd trimesters (m=3.8ppm ± 3.0ppm; range: 0.6-12.5 ppm) and child neuropsychological performance. Scores on 4 out of 11 neuropsychological tests were significantly associated with mothers' 3rd trimester CO exposures, including visuo-spatial integration (p<0.05), short-term memory recall (p<0.05), long-term memory recall (p<0.05), and fine motor performance (p<0.01) measured using the Bender Gestalt-II's Copy, Immediate Recall, and an adapted version of a Delayed Recall Figures drawing, and the Reitan-Indiana's Finger Tapping Tests, respectively. These 4 significant finding persisted with adjustment for child sex, age, visual acuity, and household assets (socio-economic status). Summary performance scores were also significantly associated with maternal 3rd trimester CO when adjusted for these covariates. Other variables accounting for variance but were excluded in our final multiple regression models included the following: HOME environment stimulation score, child examiner, WHO height-for-age percentile, and age that the infant stopped breastfeeding. This seems to be the first study on woodsmoke exposure and neurodevelopment, and the first longitudinal birth cohort study on chronic early life CO exposures, determined by high quality measures of mothers' and infants' personal CO exposures, and using well-established, reliable child neuropsychological tests. The use of wood-fired steam baths, or temazcales, is a potentially dangerous source of CO exposure in Guatemalan Highland communities where adults and children use them regularly for bathing, relaxation, and healing purposes. Physical characteristics of children predispose them to absorb CO faster than adults, placing them at greater exposure and health risks. Efforts to quantify temazcal exposures across all age groups, however, have been hampered by the limitations in exposure measurement methods. In this pilot study we measured COHb levels in children and adults following use of the temazcal using three field-based, non-invasive CO measurement methods: CO-oximetry, exhaled breath, and by estimation of COHb using micro-environmental concentrations and time diaries. We then performed a brief comparison of methods. Average CO concentrations measured during temazcal use were 661 ± 503 ppm, approximately 10 times the 15 min WHO guideline. Average COHb levels for all participants ranged from 12-14% (max of 30%, min 2%), depending on the method. COHb levels measured in children were not significantly different from adults despite the fact that they spent 66% less time exposed. COHb measured by CO-oximetry and exhaled breath had good agreement, but precision of the former was affected substantially by random instrument error. The version of the field CO-oximeter device used in this pilot could be useful in screening for acute CO exposure events in children but may lack the precision for monitoring the burden from less extreme, but more day-to-day CO exposures (e.g. indoor solid fuel use). In urban settings, health effects in children and adults have been associated with chronic exposure to ambient CO concentrations much lower than measured in this study. Future research should focus on reducing exposure from temazcales through culturally appropriate modifications to their design and practices, and targeted efforts to educate communities on the health risks they pose and actions they can take to reduce this risk.

The temazcal is a wood-fired steam bath used in the rural highlands of Guatemala for bathing and healing. We measured carbon monoxide (CO) among 288 participants in 72 temazcales. Participants were drawn from communities who participated in the RESPIRE (Randomized Exposure Study of Pollution Indoors and Respiratory Effects) chimney stove intervention trial. Temazcal CO exposures were extremely high, averaging 431 parts per million (time-weighted average). Compared to kitchen wood-smoke exposures, the temazcal contributes significantly to weekly exposures, despite the fact that the population spends less time in the temazcal than in the kitchen. This report 1) describes temazcal use patterns; 2) reports participants' signs and symptoms during temazcal use; 3) models the distribution of temazcal CO concentrations; 4) assesses reliability of exhaled breath CO as a biomarker of CO exposure; and 5) provides a proportional analysis of CO concentrations from temazcal use, as compared to kitchen concentrations.

During the first randomized intervention trial (RESPIRE: Randomized Exposure Study of Pollution Indoors and Respiratory Effects) in air pollution epidemiology, we pioneered application of passive carbon monoxide (CO) diffusion tubes to measure long-term personal exposures to woodsmoke. Here we report on the protocols and validations of the method, trends in personal exposure for mothers and their young children, and the efficacy of the introduced improved chimney stove in reducing personal exposures and kitchen concentrations. Passive diffusion tubes originally developed for industrial hygiene applications were deployed on a quarterly basis to measure 48-hour integrated personal carbon monoxide exposures among 515 children 0-18 months of age and 532 mothers aged 15-55 years and area samples in a subsample of 77 kitchens, in households randomized into control and intervention groups. Instrument comparisons among types of passive diffusion tubes and against a continuous electrochemical CO monitor indicated that tubes responded nonlinearly to CO, and regression calibration was used to reduce this bias. Before stove introduction, the baseline arithmetic (geometric) mean 48-h child (n=270), mother (n=529) and kitchen (n=65) levels were, respectively, 3.4 (2.8), 3.4 (2.8) and 10.2 (8.4) p.p.m. The between-group analysis of the 3355 post-baseline measurements found CO levels to be significantly lower among the intervention group during the trial period: kitchen levels: -90%; mothers: -61%; and children: -52% in geometric means. No significant deterioration in stove effect was observed over the 18 months of surveillance. The reliability of these findings is strengthened by the large sample size made feasible by these unobtrusive and inexpensive tubes, measurement error reduction through instrument calibration, and a randomized, longitudinal study design. These results from the first randomized trial of improved household energy technology in a developing country and demonstrate that a simple chimney stove can substantially reduce chronic exposures to harmful indoor air pollutants among women and infants.

As a part of a longitudinal study in the highlands of Guatemala to elicit the chronic health effects of wood smoke from cooking, mean area and personal 48 h concentrations of 2.5 microm particulate matter (PM2.5) and carbon monoxide (CO) were measured every 3 months over 19 months. Monitoring was conducted in 63 households, 28 using traditional open wood fires and 35 using wood cookstoves with chimneys. The goal of this paper is to estimate personal exposure concentrations to PM2.5 using the measurements from CO diffusion tubes as a proxy. CO tubes are cheaper and easier to use than PM-monitoring devices, and can be worn by all family members, even infants. The relationship of PM2.5 and CO was determined by comparing measurements from both co-located instruments. CO measurements in ppm were corrected for temperature and pressure to mass concentrations. PM2.5 exposure was modeled with the following linear regression created using measured concentrations: PM2.5 (mg m(-3)) = 0.10 (0.093, 0.12) x CO (mg m(-3)) + 0.067 (0.0069, 0.13), R(2) = 0.76. No significant difference was found between the separate regressions for open fires and cookstoves. No significant improvement was obtained by applying a mixed statistical model. The equation was used to estimate personal exposures of PM2.5 using personal CO measurements from CO tubes worn by women, infants under 18 months, and children 48-72 months. Estimated 48 h mean personal PM2.5 concentrations for mother, infants, and children in open-fire homes were 0.27 +/- 0.02, 0.20 +/- 0.02, and 0.16 +/- 0.02 mg m(-3) respectively. In chimney-stove homes, mothers and children experienced PM2.5 personal concentrations of 0.22 +/- 0.03 and 0.14 +/- 0.03 mg m(-3), respectively.

The RESPIRE and CRECER studies measured the effects of reduced household air pollution (HAP) from wood-fired cookstoves on respiratory health in rural highland Guatemala. This article examines behavior change and leadership skill development in local community members who were hired as fieldworkers to assist with research. Fieldworkers administered household questionnaires, shared functions similar to community health workers, and bridged health resources to communities. A mixed-methods design for data collection (in-depth interviews, focus groups, impact drawings, knowledge questionnaire, and retrospective pre-test) was used. Purposive sampling included 10 fieldworkers and 13 local service providers. Fieldworkers showed an increase in knowledge, positive attitudes, and practices around HAP. They developed new technical, interpersonal, and leadership skills. Fieldworkers played a crucial role in building confianza (trust) with the community, bridging resources, and improving outside researchers' relationships with locals. Recommendations for future researchers include inclusion of additional training courses and adoption of community participatory approaches.

Background: Epidemiology frequently relies on surrogates of long-term exposures, often either individual-level short-term measurements or group-level based on long-term characteristics of subjects and their environment. Whereas individual-level measures are often imprecise due to within-subject variability, group-level measures tend to be inaccurate due to residual between-subject variability within groups. Rather than choose between these error-prone estimates, we borrow strength from each by use of mixed-model prediction and we compare the predictive validity., Methods: We compared alternative measures of long-term exposure to carbon monoxide (CO) among children in the RESPIRE woodstove randomized control trial during years 2003 and 2004. The main study included 1932 repeated 48-hour-average personal CO measures among 509 children from 0-18 months of age. We used a validation study with additional CO measures among a random subsample of 70 of the children to compare the predictive validity of individual-level estimates (based on observed short-term exposures), group-level estimates (based on stove type and other residential characteristics), and mixed-model predictions that combine these 2 sources of information., Results: The estimated error variance for mixed-model prediction was 63% lower than the individual-level measure based on the exposure data and 58% lower than the corresponding group-level measure., Conclusions: When both individual- and group-level estimates are available but imperfect, mixed-model prediction may provide substantially better measures of long-term exposure, potentially increasing the sensitivity of epidemiologic studies to underlying causal relations.

Unlabelled: In 2001-2003, a team of researchers from the United States and China performed an independent, multidisciplinary review of China's National Improved Stove Program carried out since the 1980s. As part of a 3500-household survey, a subsample of 396 rural households were monitored for particulate matter less than 4 microm (PM(4)) in kitchens and living rooms over 24 h, of which 159 were measured in both summer and winter. Carbon monoxide was measured in a 40% subsample. The results of this indoor air quality (IAQ) component indicate that for nearly all household stove or fuel groupings, PM(4) levels were higher than - and sometimes more than twice as high as - the national PM(10) standard for indoor air (150 microg PM(10)/m(3)). If these results are typical, then a large fraction of China's rural population is now chronically exposed to levels of pollution far higher than those determined by the Chinese government to harm human health. Further, we observed highly diverse fuel usage patterns in these regions in China, supporting the observations in the household survey of multiple stoves being present in many kitchens. Improved stoves resulted in reduced PM(4) from biomass fuel combinations, but still not at levels that meet standards, and little improvement was observed in indoor pollution levels when other unimproved stoves were present in the same kitchen. As many households change fuels according to daily and seasonal factors, resulting in different seasonal concentrations in living rooms and kitchens, assessing health implications from fuel use requires longitudinal evaluation of fuel use and IAQ levels, combined with accurate time-activity information., Practical Implications: Leaving aside the difficult issue of enforcement, it is uncertain whether Chinese household IAQ standards represent realistic objectives for current attainment given current patterns of energy consumption in rural China, which rely so heavily on unprocessed solid fuels. Even when used with chimneys, these fuels emit substantial pollution into the household environment. It is probable that low-emission technologies involving gaseous/liquid fuels or high combustion - efficiency biomass stoves need to be promoted in order to achieve these standards for the greater part of the population.

Urinary methoxyphenols have been proposed as biomarkers for woodsmoke exposure, but few field studies have been undertaken. We evaluated these biomarkers for assessing the exposure to woodsmoke of householders in rural Guatemala. The study population was a subset (10 female cooks, 2 female non-cooks, and 8 male non-cooks ranging in age from 7 to 60) drawn from those participating in a longterm randomized intervention trial (RESPIRE) in the highlands. All households rely solely on woodburning for cooking and home heating. Approximately half of the homes in the trial used open woodfires in the home, while the intervention group used cookstoves, called "planchas, "that vent most of the woodsmoke outdoors through a chimney. Corrected for creatinine levels, 16 of the 19 methoxyphenols measured were lower in the urine of cooks using the plancha; and 11 of the 19 compounds were lower in the urine of non-cooks from homes using the plancha. Furthermore, the 4 low-molecular-weight syringyl methoxyphenols (syringol, methysyringol, ethylsyringol, propylsyringol) were each moderately correlated (r2 = 0.71,0.64, 0.68, 0.53 respectively, with all p < 0.05) with personal exposure measurements determined by carbon monoxide (CO) passive diffusion tubes, but not with CO in exhaled breath. 48-Hour kitchen area measurements of PM2.5 mass, PM2.5 levoglucosan, and CO were highly correlated (>0.89) with each other and moderately correlated (0.54-0.78) with personal CO measurements. Although based on relatively few measurements, this study demonstrates that the urinary concentrations of specific methoxyphenols may be effective biomarkers of short-term exposures to inhaled woodsmoke in field conditions.

Background: Indoor air pollution (IAP) from combustion of biomass fuels represents a global health problem, estimated to cause 1.6 million premature deaths annually., Aims: RESPIRE (Randomised Exposure Study of Pollution Indoors and Respiratory Effects) Guatemala is the first randomised controlled trial ever performed on health effects from solid fuel use. Its goal is to assess the effect of improved stoves (planchas) on exposure and health outcomes in a rural population reliant on wood fuel., Methods: Questions about symptoms were asked at baseline and periodically after the intervention, to an initial group of 504 women (259 randomly assigned to planchas (mean (standard deviation) age 27.4 (7.2) years) and 245 using traditional open fires (28.1 (7.1) years)). Levels of carbon monoxide (CO) in exhaled breath, a biomarker of recent exposure to air pollution from biomass combustion, were measured at each visit. In addition to reducing IAP levels, the plancha may also have a positive health effect by changing the working posture to an upright position., Results: A high prevalence of eye discomfort, headache and backache was found. The odds of having sore eyes and headache were substantially reduced in the plancha group relative to the group using open fires for the follow-up period (odds ratio (OR) 0.18, 95% confidence interval (CI) 0.11 to 0.29 and (OR) 0.63, 95% CI 0.42 to 0.94, respectively). Median CO in breath among women in the intervention trial was significantly lower than controls., Conclusion: In addition to reducing discomfort for women, tangible improvements in symptoms experienced by a substantial proportion of women may help to gain acceptance and wider use of planchas.

Kitchen-area 22-h gravimetric PM2.5 and passive diffusion stain-tube carbon monoxide (CO) concentrations were measured in homes with open fire and improved wood cookstoves in two studies. In the first study (Guat-2), which also studied homes with gas cookstoves, three samples were collected per stove condition from each of three test houses. In the second study (Guat-3), one sample was collected per house from 15 open fire and 25 improved-stove houses. CO personal samples were also taken for mother and child in both studies. Spearman correlation coefficients (R) between kitchen-area CO and PM2.5 levels in homes using open fires or impoved wood cookstoves were high ranging from 0.92 (Guat-2) to 0.94 (Guat-3), as were those between the personal samples for mother and child ranging from 0.85 (Guat-3) to 0.96 (Guat-2). In general, the correlations were lower for less-polluted conditions. The study found that CO is a good proxy for PM2.5 in homes using open fires or planchas (improved wood cookstove with chimney) but not under gas stove use conditions. It also determined that mother personal CO is a good proxy for child's (under 2 years of age) personal CO and that area CO measurements are not strongly representative of personal CO measurements. These results generally support the use of Draeger CO passive diffusion tubes as a proxy for PM2.5 in such cases where a single type of emission source is the predominant source for CO and PM2.5.

Continuous particles less than 2.5 microm in diameter (PM2.5) and carbon monoxide (CO) were monitored during breakfast, lunch, and dinner in three high-density and four low-density villages near Quetzaltenango, Guatemala to help assess the viability of this region for a proposed respiratory health and stove intervention study. Approximately 15 homes were visited during each mealtime in each of the seven villages; in all, 98 homes were visited, with a sampling duration of 2-3 min per home per meal. For each village, a line (transect) was drawn on a village map along existing roads from one end of the village to the other; homes and between-home outside locations along the transect were monitored. Although the predominant stove type was the open fire, several other stoves, in various levels of disrepair, were observed frequently. The highest indoor concentrations of PM2.5 were observed in homes using the open fire (avg. = 5.31 mg/m3; SD=4.75 mg/m3) or equivalent, although homes using the plancha--indigenous wood-burning stove with chimney--also had measurements > 13.8 mg/m3, PM2.5 limit of detection. The highest indoor concentrations of CO were also observed in homes using the open fire (avg. = 22.9 ppm; SD = 28.1 ppm), with a maximum measurement of > 250 ppm. For both PM2.5 and CO, levels measured in homes with plancha, lorena, or open fire were significantly higher than levels taken in the street or in homes using a gas stove. The Spearman correlation coefficient between PM2.5 and CO for all data combined was 0.81, and ranged from 0.30 for the lorena to 0.68 for the plancha in homes using wood-fueled stoves. Although indoor PM2.5 and CO levels were not significantly different between high- and low-density villages, street-level PM2.5 (p = 0.002) and CO (p= 0.002), were significantly higher in the high - density villages. These data provide a useful picture of the pollution levels coming from a range of cooking stoves in various levels ofdisrepair, as well as a representation of how outdoor particle mass and CO levels vary from high- versus low-density villages.

Area 22-h average carbon monoxide (CO), total suspended particulates (TSP), particles less than 10 microns in diameter (PM10), and particles less than 2.5 microns in diameter (PM2.5) measurements were made in three test homes of highland rural Guatemala in kitchens, bedrooms, and outdoors on a longitudinal basis, i.e. before and after introduction of potential exposure-reducing interventions. Four cookstove conditions were studied sequentially: background (no stove in use); traditional open woodstove, improved woodstove with flue (plancha), and bottled-gas (LPG) stove. With nine observations each, kitchen PM2.5 levels were 56 micrograms/m3 under background conditions, 528 micrograms/m3 for open fire conditions, 96 micrograms/m3 for plancha conditions, and 57 micrograms/m3 for gas stove conditions. Corresponding PM10/TSP levels were 173/174, 717/836, 210/276, 186/218 micrograms/m3. Corresponding CO levels were 0.2, 5.9, 1.4, 1.2 ppm. Comparisons with other studies in the area indicate that the reductions in indoor concentrations achieved by improved wood-burning stoves deteriorate with stove age. Mother and child personal CO and PM2.5 measurements for each stove condition demonstrate the same trend as area measurements, but with less differentiation.
