Background Percutaneous renal sympathetic denervation was developed as an adjunct method to treat clinical conditions associated to sympathetic hyperactivity. Percutaneous renal sympathetic denervation increases the renal blood flow and reduces vasoconstriction. The effects of percutaneous renal sympathetic denervation in renal artery diameter have not been reported. Our objective was to evaluate such effects by quantitative angiography. Methods Prospective, observational, study including consecutive patients undergoing percutaneous renal sympathetic denervation. Results Thirty-one patients were selected, 21 were submitted to percutaneous renal sympathetic denervation to control resistant arterial hypertension and 10 to control refractory ventricular arrhythmias. Seventeen patients did not perform renal arteriography in the follow-up due to clinical contraindications or because they did not complete the 6-month period established by the protocol. In addition, one patient performed a unilateral percutaneous renal sympathetic denervation and was also excluded from this analysis. Therefore, 52 renal angiographies (26 pairs) of 13 patients were analyzed. Mean maximal diameter of the right renal artery before the procedure was 4.54 ± 0.21 mm and increased to 5.2 ± 0.44 mm at 6 months ( p = 0.01). Likewise, there was a significant increase in the diameter of the left renal artery at 6 months of follow-up, increasing from 4.37 ± 0.42 to 5.23 ± 0.77 mm ( p = 0.02). Conclusions The results of this analysis illustrate the significant increment in renal artery diameter after percutaneous renal sympathetic denervation. Randomized controlled clinical trials are required to consolidate our observations. RESUMO Efeitos da Denervacao Simpatica Renal no Diâmetro da Arteria Renal Avaliados Pela Angiografia Quantitativa Introducao A denervacao simpatica renal percutânea surgiu como metodo adjunto no controle de condicoes clinicas as- sociadas a hiperatividade simpatica. Ela resulta em aumento do fluxo sanguineo renal e em reducao da vasoconstricao. Os efeitos da denervacao simpatica renal percutânea no diâmetro da arteria renal ainda nao foram descritos. Nosso objetivo foi avaliar tais efeitos por meio da angiografia quantitativa. Metodos: Estudo prospectivo e observacional que incluiu pacientes consecutivos submetidos a denervacao simpatica renal percutânea. Resultados Selecionamos 31 pacientes, sendo 21 submetidos a denervacao simpatica renal percutânea para controle da hipertensao arterial resistente e 10 para controle de arritmias ventriculares refratarias. Dezessete pacientes nao realizaram arteriografia renal no seguimento por nao completarem o periodo protocolar de 6 meses, ou por contraindicacao clinica. Adicionalmente, uma paciente realizou denervacao simpatica renal percutânea unilateral, sendo tambem excluida desta analise. Assim, 52 angiografias renais (26 pares) de 13 pacientes foram analisadas. A media do diâmetro maximo da arteria renal direita, antes do procedimento, foi de 4,54 ± 0,21 mm e aumentou para 5,2 ± 0,44 mm aos 6 meses ( p = 0,01). Da mesma forma, observou-se aumento significativo do diâmetro da arteria renal esquerda aos 6 meses de seguimento, ampliando de 4,37 ± 0,42 para 5,23 ± 0,77 mm ( p = 0,02). Conclusoes Os resultados desta analise ilustram o incremento significativo dos diametros das arterias renais apos denervacao simpatica renal percutânea. Ensaios clinicos randomizados e controlados sao necessarios para consolidar nossas observacoes.

Background: Systemic hypertension is an important public health problem and a significant cause of cardiovascular mortality. Its high prevalence and the low rates of blood pressure control have resulted in the search for alternative therapeutic strategies. Percutaneous renal sympathetic denervation emerged as a perspective in the treatment of patients with resistant hypertension. Objective: To evaluate the feasibility and safety of renal denervation using an irrigated catheter. Methods: Ten patients with resistant hypertension underwent the procedure. The primary endpoint was safety, as assessed by periprocedural adverse events, renal function and renal vascular abnormalities at 6 months. The secondary endpoints were changes in blood pressure levels (office and ambulatory monitoring) and in the number of antihypertensive drugs at 6 months. Results: The mean age was 47.3 (± 12) years, and 90% of patients were women. In the first case, renal artery dissection occurred as a result of trauma due to the long sheath; no further cases were observed after technical adjustments, thus showing an effect of the learning curve. No cases of thrombosis/renal infarction or death were reported. Elevation of serum creatinine levels was not observed during follow-up. At 6 months, one case of significant renal artery stenosis with no clinical consequences was diagnosed. Renal denervation reduced office blood pressure levels by 14.6/6.6 mmHg, on average (p = 0.4 both for systolic and diastolic blood pressure). Blood pressure levels on ambulatory monitoring decreased by 28/17.6 mmHg (p = 0.02 and p = 0.07 for systolic and diastolic blood pressure, respectively). A mean reduction of 2.1 antihypertensive drugs was observed. Conclusion: Renal denervation is feasible and safe in the treatment of resistant systemic arterial hypertension. Larger studies are required to confirm our findings.

BACKGROUND: Aging and atherosclerosis are related to renovascular hypertension in elderly individuals. Regardless of comorbidities, renal artery stenosis is itself an important cause of cardiovascular morbidity and mortality. OBJECTIVE: To define the sensitivity, specificity, positive predictive value, and negative predictive value of noninvasive imaging tests used in the diagnosis of renal artery stenosis. METHODS: In a group of 61 patients recruited, 122 arteries were analized, thus permitting the definition of sensitivity, specificity, and the relative contribution of each imaging study performed (Doppler, scintigraphy and computed tomographic angiography in comparison to renal arteriography). RESULTS: The mean age was 65.43 years (standard deviation: 8.7). Of the variables related to the study population that were compared to arteriography, two correlated with renal artery stenosis, renal dysfunction and triglycerides. The median glomerular filtration rate was 52.8 mL/min/m2. Doppler showed sensitivity of 82.90%, specificity of 70%, a positive predictive value of 85% and negative predictive value of 66.70%. For tomography, sensitivity was 66.70%, specificity 80%, positive predictive value 87.50% and negative predictive value 55.20%. With these findings, we could identify the imaging tests that best detected stenosis. CONCLUSION: Tomography and Doppler showed good quality and efficacy in the diagnosis of renal artery stenosis, with Doppler having the advantage of not requiring the use of contrast medium for the assessment of a disease that is common in diabetics and is associated with renal dysfunction and severe left ventricular dysfunction.

OBJETIVOS: Comparar a morbi-mortalidade de pacientes com e sem síndrome metabólica (SM) (de acordo com a I Diretriz Brasileira de Diagnóstico e Tratamento da SM) submetidos à cirurgia de revascularização miocárdica (RM), na fase hospitalar e após 30 dias. MÉTODOS: Foram avaliados 107 pacientes submetidos à RM sem procedimentos associados, no período de agosto a outubro de 2005, sendo 74 (69,2%) portadores de SM. Os critérios de evolução intra-hospitalar foram: fibrilação atrial aguda (FA), ventilação mecânica prolongada (VM), necessidade de transfusão de hemoderivados, número de horas na UTI, período de internação (em dias), infecção respiratória e de ferida operatória, e óbito. Em 30 dias avaliou-se a combinação de eventos como a necessidade de re-internação, infecção de ferida operatória e óbito. RESULTADOS: A média de idade foi 60,6±9,7 e o grupo com SM apresentou maior índice de massa e superfície corpórea e maior prevalência de diabete. Não houve diferença estatisticamente significante na ocorrência dos eventos intra-hospitalares como FA (15,2% x 16,2%), VM (3,0% x 2,7%), quantidade de unidades de hemoderivados utilizados (3,2±2,7 x 2,6±2,9), tempo de permanência na UTI (53,7±27,3 x 58,9±56,5), tempo de permanência hospitalar (9,2±8,7 x 8,5±8,5), infecção respiratória (6,1% x 2,7%), infecção de ferida operatória (3,0% x 5,4%) e taxa de mortalidade (3,0% x 0,0%). A ocorrência de eventos combinados em 30 dias foi semelhante nos dois grupos (12,2% x 20,3%, p = NS). CONCLUSÃO: Portadores de SM não apresentaram aumento da morbimortalidade quando submetidos à cirurgia de RM, porém apresentam tendência à maior ocorrência de infecção em ferida operatória. OBJECTIVES: To compare the morbidity and mortality of patients with and without metabolic syndrome (MS) (according to the First Brazilian Guidelines for Diagnosis and Treatment of MS) undergoing coronary artery bypass graft (CABG) during hospitalization and after 30 days. METHODS: A total of 107 patients who underwent CABG with no associated procedures were assessed between August and October 2005; 74 (69.2%) of them had MS. Criteria of in-hospital outcome were: acute atrial fibrillation (AF), prolonged mechanical ventilation (MV), need for transfusion of blood products, number of hours spent in the ICU, length of hospital stay (days), respiratory and surgical wound infection, and death. For 30 days, the combination of events such as the need for readmission to hospital, surgical wound infection and death was assessed. RESULTS: The mean age was 60.6±9.7 years, and the group with MS presented a higher body mass index and body surface, and a higher prevalence of diabetes. No statistically significant difference was observed in the occurrence of in-hospital events such as AF (15.2% vs. 16.2%), MV (3.0% vs. 2.7%), number of units of blood products used (3.2±2.7 vs. 2.6±2.9), length of ICU stay (53.7±27.3 vs. 58.9±56.5), length of hospital stay (9.2±8.7 x 8.5±8.5), respiratory infection (6.1% vs. 2.7%), surgical wound infection (3.0% vs. 5.4%), and mortality rate (3.0% vs. 0.0%). The occurrence of combined events in 30 days was similar in both groups (12.2% vs. 20.3%, p = NS). CONCLUSION: Patients with MS did not have increased morbidity and mortality when undergoing CABG, however they had a higher trend of occurrence of surgical wound infection.

Background : The 3-year results of the Symplicity HTN-1 trial have shown the superiority of renal sympathetic denervation over drug treatment in patients with resistant hypertension. However, the eficacy of renal sympathetic denervation was not conirmed by Symplicity HTN-3, the irst sham-controlled study. Our objective was to report the safety and eficacy of renal sympathetic denervation using the irrigated rad iofrequency ablation catheter in patients with resistant hypertension. Methods: Twenty-one patients were submitted to the procedure since July 2012, nine of them had a follow-up  12 months and were included in this analysis. The primary endpoint was safety, evaluated by periprocedural adverse events, renal func tion and renal vascular abnormalities at 6 months. Secondary endpoints included changes in blood pressure obtained in ofice and in the number of anti-hyperten sive drugs at 12 months. Results: Mean age was 48.8  11.7 years. In the irst case, there was a renal artery dissection caused by the vascular sheath. There were no cases of thrombosis, renal infarction or death. Serum creatinine levels did not increase during the follow-up period. At 6 months, one case of signiicant renal stenosis without clinical impact was diagnosed. The average reduction in ofice blood pressure was 41.1  33.2/18.6  15.2 mmHg (p = 0.04 for systolic blood pressure and p = 0.08 for diastolic blood pressure) and there was a mean reduction of 2.0  2.3 in the number of anti-hyperten sive drugs at 12 months (p = 0.03). Conclusions: Renal sympathetic denervation using the irrigated radiofrequency ablation catheter proved to be feasible, safe and effective.
