BACKGROUND Adoption and outcomes for conduction system pacing (CSP), which includes His bundle pacing (HBP) or left bundle branch area pacing (LBBAP), in real-world settings are incompletely understood. We sought to describe real-world adoption of CSP lead implantation and subsequent outcomes. METHODS We performed an online cross-sectional survey on the implantation and outcomes associated with CSP, between November 15, 2020, and February 15, 2021. We described survey responses and reported HBP and LBBAP outcomes for bradycardia pacing and cardiac resynchronization CRT indications, separately. RESULTS The analysis cohort included 140 institutions, located on 5 continents, who contributed data to the worldwide survey on CSP. Of these, 127 institutions (90.7%) reported experience implanting CSP leads. CSP and overall device implantation volumes were reported by 84 institutions. In 2019, the median proportion of device implants with CSP, HBP, and/or LBBAP leads attempted were 4.4% (interquartile range [IQR], 1.9-12.5%; range, 0.4-100%), 3.3% (IQR, 1.3-7.1%; range, 0.2-87.0%), and 2.5% (IQR, 0.5-24.0%; range, 0.1-55.6%), respectively. For bradycardia pacing indications, HBP leads, as compared to LBBAP leads, had higher reported implant threshold (median [IQR]: 1.5 V [1.3-2.0 V] vs 0.8 V [0.6-1.0 V], p = 0.0008) and lower ventricular sensing (median [IQR]: 4.0 mV [3.0-5.0 mV] vs. 10.0 mV [7.0-12.0 mV], p

Aims Ablation index (AI) is a marker of lesion quality during catheter ablation that incorporates contact force, time, and power in a weighted formula. This index was originally developed for pulmonary vein isolation as well as other left atrial procedures. The aim of our study is to evaluate the feasibility and efficacy of the AI for the ablation of the cavotricuspid isthmus (CTI) in patients presenting with typical atrial flutter (AFL). Methods and results This prospective multicentre non-randomized study enrolled 412 consecutive patients with typical AFL undergoing AI-guided cavotricuspid isthmus ablation. The procedure was performed targeting an AI of 500 and an inter-lesion distance measurement of ≤6 mm. The primary endpoints were CTI ‘first-pass’ block and persistent block after a 20-min waiting time. Secondary endpoints included procedural and radiofrequency duration and fluoroscopic time. A total of 412 consecutive patients were enrolled in 31 centres (mean age 64.9 ± 9.8; 72.1% males and 27.7% with structural heart disease). The CTI bidirectional ‘first-pass’ block was reached in 355 patients (88.3%), whereas CTI block at the end of the waiting time was achieved in 405 patients (98.3%). Mean procedural, radiofrequency, and fluoroscopic time were 56.5 ± 28.1, 7.8 ± 4.8, and 1.9 ± 4.8 min, respectively. There were no major procedural complications. There was no significant inter-operator variability in the ability to achieve any of the primary endpoints. Conclusion AI-guided ablation with an inter-lesion distance ≤6 mm represents an effective, safe, and highly reproducible strategy to achieve bidirectional block in the treatment of typical AFL.

Aims Permanent pacemaker implantation after transcatheter aortic valve implantation (TAVI) has emerged as a relevant issue, being more frequent than after surgery and the progressive shift towards low-risk patients stressed the importance to reduce the risk of complications that could impact patient’s long-term prognosis. Long-term right ventricular pacing has been related to an increased risk of electromechanical asynchrony, negative left-ventricular remodelling, atrial fibrillation and heart failure, but there is a lack of evidence regarding the prognostic impact on TAVI patients. The aim of this international multicentre study is to assess the impact of right ventricular pacing on prognosis of TAVI patients undergone pacemaker implantation after the procedure due to conduction disorders. Methods and results All the consecutive patients with severe aortic stenosis treated with TAVI and subsequently underwent pacemaker implantation in each participating centre were enrolled. Patients were divided into two subgroups according to the percentage of ventricular pacing (VP cut-off: 40%) at pacemaker interrogation. The primary endpoint was the composite of cardiovascular mortality and hospitalization for heart failure in subgroups based on the percentage of ventricular stimulation. All cause and cardiovascular mortality in the subgroups according to the percentage of ventricular pacing were the secondary endpoints. In total, 427 patients were enrolled, 153 patients with VP Conclusions TAVI Patients underwent permanent pacemaker implantation after the procedure due to conduction disorders and with a VP ≥ 40% at follow-up are at increased risk of cardiovascular death and HF hospitalizations and of all-cause mortality compared to patients with a VP

5. Abstract 10623: Validation of Embolic and Bleeding Risk Scores in Patients with Atrial Fibrillation and Cancer

Introduction: Atrial fibrillation (AF) is common among cancer patients. Anticoagulation can reduce the risk of stroke and systemic embolism in AF patients. However, anticoagulation in patients with cancer can be difficult given unpredictable changes in thrombosis and bleeding risk. Although CHA 2 DS 2 -VASc and HAS-BLED are useful, their predictive performance in patients with cancer is unknown. Hypothesis: CHA 2 DS 2 -VASc and HAS-BLED scores in AF patients with cancer could lead to misclassification. Methods: Overall, 16,056 AF patients between 2014 and 2018 were followed during a median follow-up of 4.9 years, including 1,137 with cancer history. We used C statistic and Brier score for assessing the performance of both scores. Results: Discrimination, assessed with C statistics (assuming death as a competing risk), was similar between cancer and non-cancer anticoagulated patients. However, in non-anticoagulated patients, c-statistic of CHA 2 DS 2 -VASc was poor and significantly lower in non-cancer patients. The overall precision of the CHA 2 DS 2 -VASc score was good throughout the follow-up (Brier score 2 DS 2 -VASc score = 1 was similar to CHA 2 DS 2 -VASc score ≥ 2. Only AF patients with cancer and CHA 2 DS 2 -VASc score = 0 presented a truly low risk of embolic events (negative predictive value 100%). A HAS-BLED score > 3 did not identify AF patients with cancer at higher bleeding risk. Conclusions: In AF patients with cancer, neither the CHA2DS2-VASC score nor the HASBLED score were useful for predicting embolic and hemorrhagic events, respectively. However, a CHA 2 DS 2 -VASc score 0 is useful to identify low embolic risk patients with AF and cancer.

The purpose of this study was to assess the incidence, predictors, and prognostic clinical impact of atrial fibrillation (AF) over time after cavotricuspid isthmus (CTI) ablation of typical atrial flutter (AFL). This was a follow-up observational study using 408 patients who underwent CTI AFL ablation between 1998 and 2010. The relationships between the different predictors and the outcomes (AF, stroke, and death) were modeled by means of multistate Cox model analyses. The incident rate of AF per 100 person-years during follow-up was 10.2 (95 % CI 8.7–11.8). Prior AF and chronic obstructive pulmonary disease (COPD) were the only independent variables to predict AF occurrence in the population. Their hazard ratios (HRs) were 2.55 (95 % CI 1.84–3.52) and 1.56 (95 % CI 1.08–2.27), respectively. Patients who transitioned to AF had an increased risk of death by an HR of 2.82 (95 % CI 1.88–4.70) and an increased risk of stroke by an HR of 2.93 (95 % CI 1.12–8.90). Age, COPD, and heart failure (HF) were predictive factors of death by HRs of 1.05 (95 % CI 1.00–1.08), 2.85 (95 % CI 1.39–5.83), and 2.72 (95 % CI 1.15–6.40), respectively. Age, smoking, COPD, and HF were predictive factors of death in the group of patients with AF during follow-up. HRs were 1.07 (95 % CI 1.02–1.12), 2.55 (95 % CI 1.55–4.21), 7.60 (95 % CI 3.01–19.16), and 3.07 (95 % CI 1.18–7.95), respectively. The transition to AF after CTI AFL ablation was high during a long-term follow-up period and maintained over time. Prior AF and COPD were the primary predictors of transition to AF after CTI AFL ablation. Patients who transitioned to AF had an increased risk of stroke and a more than twofold mortality rate. These clinical implications make it necessary to investigate AF after CTI ablation.

Tras un periodo de lavado de 2 semanas libre de farmacos y otras 2 semanas de administracion de un placebo se han incluido un total de 114 pacientes, mayores de 18 anos, diagnosticados de hipertension arterial (HTA) leve a moderada (presion arterial diastolica [PAD]: 90-119 mmHg). El diseno del estudio fue simple ciego y se comparo la eficacia y tolerancia de amlodipino (AML), con un rango de 5-10 mg en dosis unica diaria, frente a nifedipino GITS (NGITS), con un rango de 30-60 mg tambien en dosis unica diaria, durante 2 meses de tratamiento. Se realizaron 2 monitorizaciones de la presion arterial durante 24 horas (MAPA) a cada paciente, una basal, al finalizar la fase placebo, y otra final, transcurridos los 2 meses de tratamiento activo; asimismo para la MAPA final se consideraron 2 subgrupos de pacientes distribuidos aleatoriamente: un subgrupo control o de referencia (incluyendo finalmente 39 MAPA validos) que tomaron la medicacion el dia en que se les colocaba la grabadora, y un segundo subgrupo que omitieron la dosis del farmaco correspondiente el dia que les era colocado el dispositivo grabador de la MAPA (con 41 MAPA validos analizados). A cada paciente se le efectuaron controles semanales de la presion arterial (PA), frecuencia cardiaca (FC) y acontecimientos adversos, asi como electrocardiograma y analitica (bioquimica y hematimetria) al comienzo y a la finalizacion del estudio. Se considero hipertenso, mediante el criterio de la MAPA, todo paciente con una PA media (PAM) en el registro de 24 horas igual o superior a 135 mmHg de PA sistolica (PAS) y 85 mmHg de PAD. El analisis estadistico de los datos se llevo a cabo mediante el protocolo por “intencion de tratar” (ITT). Una vez obtenida la MAPA basal, 94 pacientes pasaron a la fase de tratamiento activo, siendo asignados aleatoriamente a alguno de los 2 grupos de tratamiento: 47 fueron incluidos en el grupo de pacientes tratados con AML y 47 fueron incluidos en el grupo tratado con NGITS. La PA, mediante la toma casual, se redujo de 163,3/100,3 a 138,7/87,4 mmHg en el grupo tratado con AML (p No hubo diferencias significativas entre ambos tratamientos en las determinaciones analiticas. Comparando los resultados de la MAPA, para ambos tratamientos y para los subgrupos con y sin omision de la ultima dosis se obtuvo que unicamente en el caso de un perfecto cumplimiento terapeutico ambos farmacos reducian de manera similar la PA de los pacientes en el estudio. Sin embargo, cuando se omitio una dosis, el descenso de PAS, PAD y PAM fue superior, en terminos absolutos, en los pacientes tratados con AML, siendo, ademas, estadisticamente significativa la diferencia entre ambos tratamientos en el descenso conseguido de la PAS durante el periodo nocturno (p = 0,046).

Introduction: Contrast-induced nephropathy (CIN) is an usual complication of percutaneous coronary interventions (PCI). Here we report the validation of the new cystatin C (CyC)-based glomerular filtrate rate GFR equations (CKD-EPI\_CyC and CKD-EPI\_Cr&CyC) to predict CIN in patients with ACS. We evaluated their performance, as compared with existing creatinine (Cr)-based GFR equations (CG, MDRD-4 and CKD-EPI_Cr), and tested their usefulness in improving the classification of patients in risk of CIN. Methods: 497 subjects admitted consecutively to our hospital (2008-2010) with a diagnosis of ACS and underwent PCI were included. Discriminative power and calibration index of each eGFR equation were calculated and compared, together with the predictive values (sensitivity, specificity). The incremental value that the new CyC-based eGFR equations brings over the sCr-based eGFR equations was assessed by the Net Reclassification Improvement (NRI).
