Background: Comprehension deficits are more pervasive in aphasic syndromes than initially believed. They affect differentially distinct levels of auditory-verbal comprehension. Current evidence from functional imaging studies in healthy subjects indicates that distinct levels of auditory-verbal analysis involve specific networks. Aims: The aim of this study is (1) to assess the different levels of auditory-verbal analysis with real-time monitoring tasks in patients with aphasia, (2) to compare the performance profiles across aphasia types, (3) to analyse patterns of dissociations vs. co-occurrence at specific levels, and (4) to establish correlations between disturbances at specific levels of auditory-verbal analysis and sites of lesions. Methods & Procedures: Forty-two right-handed patients with aphasia associated with a first unilateral left-hemispheric lesion underwent tests monitoring (1) phonetic-phonological, (2) lexical, (3) morphosyntactic, (4) semantic-pragmatic (at sentence level), and (5) linguistic prosody processing. Anatomo-clinical correlations were established by means of voxel-based lesion-symptom mapping. Outcomes & Results: Widespread deficits at multiple levels occurred across aphasic syndromes. In a given patient, more levels tended to be impaired in Wernicke’s and global aphasia than in Broca’s or conduction aphasia. Syllable and word processing double-dissociated behaviourally and partially anatomically. Morphosyntactic deficits were always accompanied by semantic-pragmatic deficits. Anatomo-clinical correlations implicated the dorsal auditory stream in syllable discrimination, the ventral stream in semantic processing at lexical level and in linguistic prosody and both streams in lexical frequency effect. Basal ganglia were implicated in syntactic and semantic processing at sentence level. Conclusions: At prelexical and lexical levels, syllable and word processing appear to be independent of each other. At sentence level, parsing of syntactic structure appears to be necessary for successful semantic-pragmatic analysis. Thus, the fine-grained evaluation of auditory-verbal processing capacities and of the integrity of specialised processing networks in brain-damaged patients provides a sensitive diagnostic tool. Purpose: Cognitive deficits that are present in the acute stage of a focal hemispheric lesion tend to be greater and more general than residual deficits, which persist into the chronic stage. We have investigated the patterns of recovery and the relationship between deficits and damage to specialized networks taking as model auditory cognitive functions. Evidence from human psychophysical, activation and neuropsychological studies suggests that sound recognition and sound localization are processed in anatomically and functionally distinct cortical networks, the auditory "What" and "Where" processing streams, that are each present in both hemispheres. Focal left or right hemispheric lesions centred on these networks were found to be associated, in the chronic stage, with the corresponding deficits in sound recognition and/or sound localization. Methods: We report here on recovery patterns in 24 patients who sustained focal hemispheric lesions and were deficient in sound recognition, sound localization and/or sound motion perception at a first evaluation in the acute (n=9), subacute (n=6) or early chronic stages (n=9). Results: All 24 patients had initially a deficit in sound localization and/or sound motion perception. In the acute stage this deficit occurred without damage to the auditory "Where" stream in almost half of the patients, a situation which was never observed in the early chronic stage. Lack of recovery tended to be associated with damage to the specialized stream plus the persistence of deficits beyond the acute stage, and was only loosely related to the size of the lesion and to the extent of damage to a specialized network. Abstract: Fragile X-associated tremor/ataxia syndrome (FXTAS), a late-onset movement disorder affecting FMR1 premutation carriers, is associated with cerebral and cerebellar lesions. The aim of this study was to test whether computational anatomy can detect similar patterns in asymptomatic FMR1 premutation carriers (mean age 46.7 years) with qualitatively normal -appearing grey and white matter on brain MRI. We used a multimodal imaging protocol to characterize brain anatomy by automated assessment of gray matter volume and white matter properties. Structural changes in the hippocampus and in the cerebellar motor network with decreased gray matter volume in lobule VI and white matter alterations of the corresponding afferent projections through the middle cerebellar peduncles are demonstrated. Diffuse subcortical white matter changes in both hemispheres, without corresponding gray matter alterations, are only identified through age × group interactions. We interpret the hippocampal fimbria and cerebellar changes as early alterations with a possible neurodevelopmental origin. In contrast, progression of the diffuse cerebral hemispheric white matter changes suggests a neurodegenerative process, leading to late-onset lesions, which may mark the imminent onset of FXTAS. Sound localization relies on the analysis of interaural time and intensity differences, as well as attenuation patterns by the outer ear. We investigated the relative contributions of interaural time and intensity difference cues to sound localization by testing 60 healthy subjects: 25 with focal left and 25 with focal right hemispheric brain damage. Group and single-case behavioural analyses, as well as anatomo-clinical correlations, confirmed that deficits were more frequent and much more severe after right than left hemispheric lesions and for the processing of interaural time than intensity difference cues. For spatial processing based on interaural time difference cues, different error types were evident in the individual data. Deficits in discriminating between neighbouring positions occurred in both hemispaces after focal right hemispheric brain damage, but were restricted to the contralesional hemispace after focal left hemispheric brain damage. Alloacusis (perceptual shifts across the midline) occurred only after focal right hemispheric brain damage and was associated with minor or severe deficits in position discrimination. During spatial processing based on interaural intensity cues, deficits were less severe in the right hemispheric brain damage than left hemispheric brain damage group and no alloacusis occurred. These results, matched to anatomical data, suggest the existence of a binaural sound localization system predominantly based on interaural time difference cues and primarily supported by the right hemisphere. More generally, our data suggest that two distinct mechanisms contribute to: (i) the precise computation of spatial coordinates allowing spatial comparison within the contralateral hemispace for the left hemisphere and the whole space for the right hemisphere; and (ii) the building up of global auditory spatial representations in right temporo-parietal cortices. Right hemispheric stroke aphasia (RHSA) rarely occurs in right- or left-handed patients with their language representation in right hemisphere (RH). For right-handers, the term crossed aphasia is used. Single cases, multiple cases reports, and reviews suggest more variable anatomo-clinical correlations. We included retrospectively from our stroke data bank 16 patients (right- and left-handed, and ambidextrous) with aphasia after a single first-ever ischemic RH stroke. A control group was composed of 25 successive patients with left hemispheric stroke and aphasia (LHSA). For each patient, we analyzed four modalities of language (spontaneous fluency, naming, repetition, and comprehension) and recorded eventual impairment: (1) on admission (hyperacute) and (2) between day 3 and 14 (acute). Lesion volume and location as measured on computed tomography (CT) and magnetic resonance imaging (MRI) were transformed into Talairach stereotaxic space. Nonparametric statistics were used to compare impaired/nonimpaired patients. Comprehension and repetition were less frequently impaired after RHSA (respectively, 56% and 50%) than after LHSA (respectively, 84% and 80%, P = 0.05 and 0.04) only at hyperacute phase. Among RHSA, fewer left-handers/ambidextrous than right-handers had comprehension disorders at second evaluation ( P = 0.013). Mean infarct size was similar in RHSA and LHSA with less posterior RHSA lesions (caudal to the posterior commissure). Comprehension and repetition impairments were more often associated with anterior lesions in RHSA (Fisher’s exact test, P < 0.05). Despite the small size of the cohort, our findings suggest increased atypical anatomo-functional correlations of RH language representation, particularly in non-right-handed patients. Evidence from activation studies suggests that sound recognition and localization are processed in two distinct cortical networks that are each present in both hemispheres. Sound recognition and/or localization may, however, be disrupted by purely unilateral damage, suggesting that processing within one hemisphere may not be sufficient or may be disturbed by the contralateral lesion. Sound recognition and localization were investigated psychophysically and using fMRI in patients with unilateral right hemisphere lesions. Two patients had a combined deficit in sound recognition and sound localization, two a selective deficit in sound localization, one a selective deficit in sound recognition, and two normal performance in both tasks. The overall level of activation in the intact left hemisphere of the patients was smaller than in normal control subjects, irrespective of whether the patient''s performance in the psychophysical tasks was impaired. Despite this overall decrease in activation strength, patients with normal performance still exhibited activation patterns similar to those of the control subjects in the recognition and localization tasks, indicating that the specialized brain networks subserving sound recognition and sound localization in normal subjects were also activated in the patients with normal performance, albeit to an altogether lesser degree. In patients with deficient performance, on the other hand, the activation patterns during the sound recognition and localization tasks were severely reduced, comprising fewer and partly atypical activation foci compared to the normal subjects. This indicates that impaired psychophysical performance correlates with a breakdown of parallel processing within specialized networks in the contralesional hemisphere. [Copyright &y& Elsevier]
