This report presents a 13-year-old girl with an aggressive skeletal lesion involving the posterior elements of the cervicothoracic junction. The lesion was determined to be a Musculoskeletal Tumor Society grade 3 aneurysmal bone cyst based on imaging and pathologic findings. The cyst was treated with endovascular n-butyl cyanoacrylate embolization and percutaneous cryoablation with sensory and motor-evoked potential monitoring. Follow-up imaging at 16 months demonstrated significant reduction in the size of the lesion, with no evidence of recurrence., (Copyright © 2015 SIR. Published by Elsevier Inc. All rights reserved.)

Introduction: Despite numerous descriptive publications, the nature, character, differential diagnosis and optimal treatment of aneurysmal bone cysts (ABCs), remain obscure. The authors report a case of the solid variant of aneurysmal bone cyst (S-ABC) occurring in the posterior components and body of C7 vertebra focusing on the differential diagnosis and surgical treatment rationale., Case Report: Right shoulder and neck pain were the presenting symptoms of 9-year-old boy. Torticollis developed later on but no neurological deficit was found. Imaging revealed an osteolytic lesion with significant extraosseous extension. Although diagnosis favoured an ABC, imaging studies did not provide clear diagnostic criteria. CT guided biopsy performed preoperatively was also not directly diagnostic. Given that differential diagnoses included S-ABC but also giant cell tumor (GCT) of bone, decision was made to proceed with a staged, back and front, complete resection of the affected bony elements of C7. Preoperative spinal angiography showed supply to the tumor from the right ascending and deep cervical artery territories. Particle embolization was not performed due to the presence of ipsilateral supply to the anterior spinal artery at the C6 level and contralateral supply at the C7 level. Intraoperatively, histology taken from posterior elements, although again not clearly diagnostic, favoured S-ABC variant rather than GCT. Thus, initial plan was revised and anterior surgery was postponed as the extent of the dissection would have been dependent on the presumed diagnosis. The final histological report confirmed the diagnosis of an S-ABC. In view of this, it was decided to embolize the lesion to avoid a second stage anterior surgery. At embolization, repeat spinal angiography showed reduced tumor blush following the surgery. Distal branches of the deep cervical artery were occluded with platinum coils (avoiding the risks associated with the use of particles or liquid embolic agents). No further procedure was planned., Conclusion: Imaging and histological pattern of this specific type of ABC, differential diagnosis from GCT and the surgical protocol followed with the patient consist of an interesting case of revising the initial plan, according to the upcoming histological reports.

Aneurysmal bone cyst is a benign tumor-like bony lesion with a propensity to develop in the pediatric population. It generally involves one vertebral level when localized to the spine. The degree of resection correlates highly with fewer recurrences. En bloc spondylectomy is the procedure of choice for this reason, but its high complication rate has led to the development of alternative surgical methods. This paper presents a three-level aneurysmal bone cyst that was excised totally in two stages, and the safety and efficacy of this method especially in the pediatric population are emphasized. This paper also states that anterior and posterior instrumented fusions are necessary in spite of the growing spine.

Study Design: Case report., Objective: To present 2 cases of aneurysmal bone cyst in the cervical spine of children who were aggressively treated with resection, fusion, and stabilization with hardware to achieve complete cure and future stability., Summary of Background Data: Aneurysmal bone cyst is a rapidly expanding and destructive bone lesion of unknown etiology. Current approaches to treatment include surgical en bloc excision, complete curettage, selective arterial embolization, radiotherapy, and intralesional injection with a sclerosing agent. Fusion and stabilization with instrumentation have often been avoided in children due to concerns over instability following future axial skeleton growth., Methods: The authors present 2 case reports of aneurysmal bone cyst in the cervical spine of children, both demonstrating deeply involved lesions with extensive bone destruction. Both were treated aggressively with resection, fusion, and stabilization with instrumentation. Additionally, the relevant background literature is briefly discussed., Results: Both patients had an uncomplicated postoperative course. At 36- and 18-month follow-up, both patients have no cervical spine instability or recurrence of tumor., Conclusion: Treatment of aneurysmal bone cyst in the cervical spine is challenging when it occurs in close proximity to neural and vascular structures. Future stability of the cervical spine is a concern, especially when there is significant involvement of the vertebral body and posterior elements. At 36- and 18-month follow-up, the surgical approach to aneurysmal bone cyst presented here provided these patients with complete curative treatment and preservation of cervical spine stability.

An aneurysmal bone cyst (ABC) is a rare skeletal tumor that accounts for approximately 1% of all bone tumors. A spinal location for an ABC is very rare. Methods for treatment of an ABC include resection, curettage, embolization, and intralesional injection of a variety of agents. The patient in this case was a 9-year-old girl presenting with neck pain who was diagnosed with an ABC involving the C-2 spinal level. Percutaneous intralesional injections of calcitonin and methylprednisolone were performed. Two years and 7 months after the initial injection, a CT scan showed massive bone formation and cortical thickening without a change in the size of the lesion. To the authors' knowledge, this is the third reported case of intralesional injection of calcitonin and methylprednisolone for treatment of an ABC. This method is safe and effective, and is an important alternative to surgery, especially when a surgical procedure is technically difficult or unsafe in high-risk patients.

Aneurysmal bone cysts (ABC) are rare, benign, but locally destructive bone tumors. They occur in the spine in 14% of cases, but only 2% are found in the cervical spine. This case report presents a 16-year-old female with an expansive aneurysmatic bone cyst with extensive bone destruction and instability in the cervical segments C1 and C2. In CT and MRI, there was a soft-tissue tumor mass with multiple cysts and fluid-fluid levels within these cysts, as well as contrast enhancement of the cyst wall and the tissue among the cysts. The typical macroscopic and histological findings were present. The tumor was resected en bloc, after which the patient underwent adjuvant radiation therapy. The patient made a complete recovery.

Aneurysmal bone cysts are not counted among the classic malignant tumors, although they are destructive locally as blood filled reactive bone lesions. Typically, they are found in the metaphysis of the long bones, while localizations on the spine are rare. A 16-year-old female patient presented with unspecific, progressive neck pain which had been present for half a year. The initial x-ray showed no noticeable pathology whatsoever. Subsequently, the complete destruction of the first cervical vertebrae was found. The tumor had completely infiltrated and completely surrounded the spinal chord. A combined approach was used as therapy: resection of the dorsal tumor portion with occipitocervical spondylodesis (C0-C4) and postoperative radiation of the remaining ventral portions. Currently, the patient is free of complaints and recurrence. The differential diagnosis of an aneurysmal bone cyst should also be considered in cases of unspecific cervical vertebral complaints in adolescents that are not otherwise explainable.

Study Design: An aneurysmal bone cyst in the neural arch of the fourth cervical vertebra of a 10-year-old girl is reported, along with a brief review of the literature on the topic., Objective: To report the presentation and diagnosis of this disorder along with a discussion of the major pitfalls of treatment., Summary of Background Data: An aneurysmal bone cyst occurs commonly in the second decade, with a predilection for the lumbar spine. With occurrence in the neural arch of a cervical vertebra, the potential for instability following surgical excision is high., Methods: A 10-year-old white female presented with neck pain of 3 months' duration. Diagnostic imaging revealed an expansile lytic lesion in the spinous process and lamina of the fourth cervical vertebra. Surgical treatment consisted of excisional biopsy and a segmental instrumented posterior fusion from C3-C5. The histopathology was consistent with an aneurysmal bone cyst., Results: Surgical excision consisting of laminectomy and instrumented segmental fusion provided a good clinical result, and minimized the risk and degree of the 2 most common complications: recurrence of the tumor; and postlaminectomy kyphosis, a frequent occurrence in the pediatric population., Conclusions: In pediatric patients who develop a bone tumor of the posterior elements of the cervical spine, careful clinical and radiologic evaluation is necessary to narrow the differential diagnosis. In most cases, a complete excision should be performed if possible. The risk of postlaminectomy kyphosis is high in the pediatric age population. As such, a fusion should be considered whenever a laminectomy is performed in the immature cervical spine. Risk factors for kyphosis include a high cervical level, multiple laminectomy levels, and postoperative irradiation.

Purpose of the Study: Aneurysmal cyst is an uncommon benign primitive bone tumor generally observed in young subjects, in a spinal localization in 10% of the cases. We report the clinical, radiological, and therapeutic aspects of seven cases of aneurysmal cyst of the spine., Material and Methods: This series included seven children, mean age 11.7 years (range 8-14) who were followed for 9 years on the average (range 2-24 years). Findings were compared with reports in the literature., Results: Four cysts were located in the cervical spine (C1, C3, C4, C7), two in the thoracic spine (T4, T7) and one in the sacrum. Biopsy-resection was performed in six patients. The sacral cyst was treated by selective arterial embolization. Two complications were observed after surgical treatment. Bony filling of the lesion, evaluated by CT scan six months after surgery, was achieved in five cases, incomplete in two without recurrence of cystic cavities., Discussion and Conclusion: Aneurysmal bone cysts account for 15% of all primitive tumors of the spine. They are more frequent in the cervical and thoracic spine than in a lumbar or sacral localization. One vertebra is involved in most cases, generally the vertebral body and the posterior arch with an asymmetrical extension to one pedicle in some cases. Neurological complications of variable severity are frequent and can be improved by surgical removal of the tumor. CT scan and MRI are the best exams to analyze the spinal cyst, to show bony walls and liquid-liquid levels suggestive of aneurysmal bone cyst, to evaluate the tumor extension and to search for cord compression. When the CT scan and MRI show characteristic signs of aneurysmal bone cyst of the spine, we do not perform a biopsy systematically if complete removal of the tumor appears possible. Surgical removal with biopsy is the treatment of choice. Reconstruction and stabilization of the spine must be complete using bone graft with or without instrumentation. In case of medullary involvement, decompression must be performed rapidly with the most complete tumor removal as possible. Selective arterial embolization can be used in some localizations or as preparation to surgery. Radiotherapy is contraindicated in children because of the risk of radio-induced tumors.

The authors describe the case of an 18-year-old man who presented with complaints of weakness and paresis in his arms following an injury. Radiological examination demonstrated an aneurysmal bone cyst of C-6. The patient underwent a two-stage operation. Satisfactory results were obtained after complete resection of the lesion, laminoplasty, and anterior fusion without placement of instrumentation. The authors consider a two-stage operation supplemented by fusion without instrumentation to be the best treatment for young patients with aneurysmal bone cysts occurring at C-6.

A series of tumour cases of the upper cervical spine who we had treated was analysed retrospectively with respect to surgical complications, quality of life and survival, 13 patients (mean age 56 years, metastases 6, plasmocytomas 3, chordomas 2, histiocytosis 1, aneurysmal bone cyst 1) with tumours of the upper cervical spine (C2 n = 7, C2 and C3 n = 4, C3 n = 2) were treated during an 8-year period. A total of 16 operations were carried out with ten one-step procedures and three two step-procedures. Eight extra-oral, one transoral, three dorsal and, in one case, a combined dorsal and extra-oral tumour removal were performed. Four dorsal stabilisation's, four ventral platings and two combined ventral platings plus dorsal fixations and eight vertebral body replacements were carried out. The neurological status and the quality of life were analysed preoperatively and during the follow-up examinations. Flexionextension radiographs were taken during follow-up. There was no operative mortality. The transient morbidity was 8%. The operative intervention significantly improved the quality of life in all patients during the follow-up visits (mean: 20 months). No instability was seen. The average survival time of all patients was 23 months. Six patients died following the operation after a mean time of 28 (4-64) months. Tumour removal in the upper cervical spine using individually modified surgical strategies based on an approach combining tumour location, stabilisation and vertebral body replacement significantly increases the time of survival and quality of life with an acceptable surgical risk for complications.

The authors describe the surgical anatomy of lateral approach to the upper cervical spine. The approach is retrovascular retropharyngeal extraoral allowing for anterolateral exposure of the upper cervical column, and it may be enlarged posteriorly on the homolateral posterior hemiarch of C1 and C2, and/or inferiorly on the lower cervical column. Absolute indications to this approach are anterolateral fusion with transarticular screws C1-C2 and excision of skeletal neoplasms adjacent to the vertebral artery. For other indications such as the treatment of anterior neoplasms and spinal cord decompression it is preferable to use pre-vascular extraoral approach or transoral approach.

Aneurysmal bone cysts are uncommon bony lesions of the spine. Two such patients are reported in the study. The first patient had involvement of first and second thoracic vertebrae, and a preoperative diagnosis was made possible due to the typical magnetic resonance imaging appearances. A total removal was achieved in two stages. In the second patient the C2 spinous process was involved, and this patient presented as atantoaxial dislocation. Removal of the abnormal tissue and posterior fusion was done. Both the patients showed neurologic improvement. The relevant literature is reviewed.

A 19-year-old girl was admitted with a history of difficulty in moving her neck for several years and a sudden onset of neck pain three months before. Plain radiographs of the cervical spine revealed destruction of the left half of the 6th cervical body with an expansive soap-bubble appearance. Neurological examination on admission was within normal limits. The angiography and bone scintigraphy revealed no abnormality. MRI of T1-weighted image showed a cystic lesion with various signal intensities. T2-weighted image demonstrated a hyperintense balloon-like lesion in the vertebral body and left lamina. At surgery, a cystic tumor was fully extirpated by the posterior approach and the bony defect was packed with apatite granules. She was discharged without any neurological deficits. This disease should be considered as one of the etiologies when a patient with difficulty in neck movement is encountered in young generation.
