Objectives: Currently, anterior-only (AO), posterior-only, and combined anterior-posterior spinal fusions are common strategies for treating cervical kyphosis in patients with neurofibromatosis-1 NF-1. Nevertheless, the choice of surgical strategy remains a topic of controversy. The aim of our study is to evaluate the safety and effectiveness of anterior decompression and spinal reconstruction for the treatment of cervical kyphosis in patients with NF-1., Methods: Twelve patients with NF-1-associated cervical kyphotic deformity were reviewed retrospectively between January 2010 and April 2020. All patients underwent AO correction and reconstruction. The X-ray was followed up in all these patients to assess the preoperative and postoperative local kyphosis angle (LKA), the global kyphosis angle (GKA), the sagittal vertical axis, and the T1 slope. The visual analog scale score, Japanese Orthopedic Association (JOA) score, and neck disability index (NDI) score were used to evaluate the improvement inclinical symptoms. The results of the difference in improvement from preoperatively to the final follow-up assessment were assessed using a paired t-test or Mann-Whitney U-test., Results: The LKA and GKA decreased from the preoperative average of 64.42 (range, 38-86) and 35.50 (range, 10-81) to an average of 16.83 (range, -2 to 46) and 4.25 (range, -22 to 39) postoperatively, respectively. The average correction rates of the LKA and GKA were 76.11% and 111.97%, respectively. All patients had achieved satisfactory relief of neurological symptoms (p < 0.01). JOA scores were improved from 10.42 (range, 8-16) preoperatively to 15.25 (range, 11-18) at final follow-up (p < 0.01). NDI scores were decreased from an average of 23.25 (range, 16-34) preoperatively to an average of 7.08 (range, 3-15) at the final follow-up (p < 0.01)., Conclusion: Anterior-only correction and reconstruction is a safe and effective method for correcting cervical kyphosis in NF-1 patients. In fixed cervical kyphosis cases, preoperative skull traction should also be considered., (© 2024 The Authors. Objective: This is the first study to evaluate the predictive value of the cervical vertebral bone quality (VBQ) score on zero-profile cage (ZPC)subsidence after anterior cervical discectomy and fusion (ACDF) using the Hounsfield units (HU) value of computed tomography as the reference., Methods: A total of 89 patients with at least 1 year of follow-up who underwent single-level ACDF with ZPC were retrospectively and consecutively included. VBQ and HU value were determined from preoperative T1-weighted magnetic resonance imaging and computed tomography. Subsidence was defined as ≥2 mm of migration of the cage into the superior or inferior endplate or both using lateral cervical spine radiography. The results were subjected to statistical analysis., Results: Subsidence was observed among 16 of the 89 study patients (Subsidence rate: 18.0%). The mean VBQ score was 2.94 ± 0.820 for patients with subsidence and 2.33 ± 0.814 for patients without subsidence. The multivariable analysis demonstrated that only an increased VBQ score (odds ratio: 1.823, 95% confidence interval : 0.918,3.620, P = 0.001) was associated with an increased rate of cage subsidence. There was a significant and moderate correlation between HU and VBQ (r = -0.507, P < 0.001). Using receiver operating characteristic curves, the area under the curve was 0.785, and the most appropriate threshold of VBQ was 2.68 (sensitivity 72.7%, specificity 82.1%)., Conclusions: The VBQ score may be a valuable tool for independently predicting ZPC subsidence after single-level ACDF., (Copyright © 2023. Objective: The nano-hydroxyapatite/polyamide66 (n-HA/PA66) cage is a novel biomimetic nonmetal cage, whereas the polyetheretherketone (PEEK) cage has been widely used for decades with excellent clinical outcomes. However, there has been no long-term comparison of these 2 cages in anterior cervical reconstruction., Methods: This retrospective study included 98 patients who underwent single-level anterior cervical decompression and fusion (ACDF) from January 2009 to June 2011 with at least 7 years of follow-up; the PEEK cage was used in 51 patients (PEEK group), whereas the n-HA/PA66 cage was used in 47 (n-HA/PA66 group). The groups were compared regarding radiographic (cage subsidence, fusion status, cervical lordosis, and segmental sagittal alignment) and clinical (10-point visual analog scale and Japanese Orthopedic Association scores) parameters preoperatively, postoperatively, and at the final follow-up., Results: The n-HA/PA66 and PEEK cages had similar fusion rates at the final follow-up (97.9% vs. 98.0%). The respective cage subsidence rates in the PEEK and n-HA/PA66 cage groups were 9.8% and 10.6%. There were no significant differences between the 2 groups in mean cervical lordosis, mean interbody height, segmental sagittal alignment, cervical lordosis, and 10-point visual analog scale and Japanese Orthopedic Association scores at the final follow-up. The n-HA/PA66 cages resulted in high fusion and low subsidence rates during long-term follow-up., Conclusions: After single-level ACDF, the n-HA/PA66 cage showed similar radiographic fusion, subsidence rate, and clinical results compared with the PEEK cage during 7 years of follow-up. Background: Several studies have explored cervical kyphosis (CK) in adolescent idiopathic scoliosis (AIS) patients. However, few studies have evaluated the cervical alignment in these patients according to their coronal curve type. The aim of this study was to analyze the radiological features of cervical sagittal alignment in Lenke 1 AIS patients before and after surgery., Methods: This is a retrospective study enrolled 50 patients. Preoperative and postoperative standing full-length radiographs (at last follow-up after operation) were used to measure the coronal and sagittal parameters. Main sagittal parameters included C2-C7 angle, thoracic inlet angle (TIA), T1 slope, proximal thoracic kyphosis (PTK, T1-5 kyphosis) and thoracic kyphosis (TK, T5-12 kyphosis)., Results: The TIA of patients with CK was significantly smaller than that of patients with CL (63.0° vs. 76.3°, p < 0.05) and the cutoff value was 71°. The TIA of patients with CK after surgery was significantly smaller than that of patients with CL postoperatively (62.5° vs. 74.6°, p < 0.05) and the cutoff value was 62°. In patients with postoperative CL, there was a significant increase in their PTK and a reduction in their TK, regardless of preoperative CL or CK. In patients whose CL deteriorated to CK after surgery, both their PTK and TK significantly decreased after surgery., Conclusions: Patients with TIA less than 71° were more likely to have CK. And patients with TIA less than 62° would lead to the postoperative uncorrected or new onset of CK. The increased PTK after operation could have a beneficial effect on the improvement of CL.

Purpose: Compared with anterior cervical discectomy and fusion (ACDF), cervical disc replacement (CDR) has provided satisfactory clinical results. The incidence of post-operative dysphagia between ACDF with a traditional anterior plate and CDR remains controversial. Considering the limited studies and knowledge in this area, a retrospective study focusing on post-operative dysphagia was conducted., Methods: The Bazaz grading system was used to assess the severity of dysphagia at post-operative intervals including 1 week, 1 month, 3 months, 6 months, 12 months and 24 months respectively. The Chi-square test, Student t-test, Mann-Whitney U tests and Ordinal Logistic regression were used for data analysis when appropriate. Statistical significance was accepted at a probability value of <0.05., Results: Two hundred and thirty-one patients in the CDR group and one hundred and fifty-eight patients in Plate group were included in this study. The total incidences of dysphagia in the CDR and plate group were 36.58% and 60.43% at one week, 29.27% and 38.85% at one month, 21.95% and 31.65% at three months, 6.83% and 17.99% at six months, 5.85% and 14.39% at 12 months, and 4.39% and 10.07% at the final follow-up respectively (All P<0.05, Mann-Whitney U test). Ordinal Logistic regression analysis showed that female patients, two-level surgery, C4/5 surgery, and anterior cervical plating were significant risk factors for post-operative dysphagia (all P<0.05)., Conclusion: Comparing ACDF with a plate, CDR with a Prestige LP can significantly reduce both transient and persistent post-operative dysphagia. Female patients, two-level surgery, C4/5 surgery and anterior cervical plating were associated with a higher incidence of dysphagia. Study Design: This was a retrospective study., Objective: The aim of this study was to investigate the effect of Centerpiece mini-plate fixation on the complete fracture and bony fusion of the hinge side in unilateral open-door cervical expansive laminoplasty., Summary of Background Data: Cervical laminoplasty is an effective and safe surgery for cervical canal stenosis. The Centerpiece mini-plate is an instrument used to secure the laminae and maintain the cervical canal expansion. Stability of the new laminae is largely dependent on healing of the hinge side bone fracture and the degree of bony fusion. To date, few studies have reported on the effects of mini-plate fixation on these 2 important factors., Materials and Methods: Between September 2009 and March 2011, 58 patients received unilateral open-door cervical expansive laminoplasty at the authors' hospital. The group included 47 male and 11 female patients, with a mean age of 61 (range, 35-81) years. Two hundred twenty-five laminae were fixed using the Centerpiece mini-plate (group A), whereas 62 laminae were fixed using suture suspension (group B). The rates of fracture and bony fusion of the hinge were observed using computed tomography scan and compared between the 2 groups. The complete fractures were subdivided into 4 groups based on the degree of displacement of the fractured ends: type I (no displacement), type II (mild to moderate displacement), type III (complete displacement or separation), or type IV (the hinge had collapsed into the cervical canal)., Results: The number of incomplete fractures and type I to IV fractures in group A were 95, 93, 25, 8, and 4 and 29, 25, 4, 2, 2 in group B, respectively. There were no significant differences between the 2 groups in terms of complete fracture rates (P=0.309) and complete fracture type distribution (P=0.694). Group A had a significantly higher rate of bony fusion of the hinge 3 months after surgery (82% vs. 70%, P=0.042); however, this rate was not statistically significant 6 months after surgery (P=0.141). For type I complete hinge fracture, group A had higher bony fusion rates, both 3 months (86% vs. 57%, P=0.004) and 6 months (92% vs. 85%, P=0.048) postoperatively. The rates of bony fusion were also significantly different among all complete fracture types 3 months (P<0.001) and 6 months (P<0.001) postoperatively., Conclusions: Centerpiece mini-plate fixation in unilateral open-door cervical expansive laminoplasty might not increase the complete fracture rate compared with suture suspension and might promote bony fusion of type I complete hinge fractures.

Surgical treatment is one of the effective methods of treatment in cervical spondylosis. The traditional method of operation is decompression fusion; however, this surgery results in restricted movement of cervical vertebra and adjacent segment degeneration. Due to the deficiency of traditional surgery, scholars have widely carried out artificial cervical disk replacement surgery and have achieved good clinical effects. Comparing to the characteristics of the common artificial cervical disk which is used frequently, we developed a new artificial cervical intervertebral disk prosthesis. The purpose of this study was to determine the wear behavior in a cervical total disk replacement system. The total disk replacement system tested consists of a ultra-high-molecular-weight polyethylene inlay articulating between a Ti6Al4V alloy superior plate and an inferior plate, using a spine wear simulator, per the ISO 18192-1:2011 standard test methods. Three rotations and axial force were applied on each station. The specimens were removed at 5 × 10(5) and 10(6) cycles and at intervals of 10(6) cycles thereafter to determine the actual mass loss. The serum was replaced every 5 × 10(5) cycles. The specimens were changed periodically among the different stations. A mean ultrahigh molecular weight polyethylene inlay wear rate of 0.53 mg per million cycles (standard = 0.13 mg per 10(6) cycles) was found after 10(7) cycles. All inferior plates showed slight scratching after 10(7) cycles. The impingement wear simulation introduced here proved to be suitable to predict in vivo impingement behavior in regard to the contact pattern seen on retrieved devices of the Pretic-I disk arthroplasty design in a preclinical test., (© IMechE 2015.)

Background Context: Isolated tuberculosis (TB) of posterior elements of the cervical spine is extremely rare. Only few cases are reported in literature, but none describe isolated spinous process and laminae involvement without TB in any other part of the body., Purpose: To report a case of isolated TB involvement of the spinous process and right laminae of C2 combined with atlantoaxial luxation., Study Design: A case report of isolated spinous process and right laminae TB of C2 combined with atlantoaxial luxation., Patient Sample: A 20-year-old male farmer complained about a 2-month history of aggravating neck pain, a 1-month history of a slowly growing lump at the back of the neck, and numbness and weakness of the right arm that had gradually progressed to involve all the limbs for 2 weeks before presentation., Outcome Measures: X-ray and computed tomography of cervical spine and Frankel grading of neural function are included to evaluate the therapeutic efficiency., Methods: Computed tomography scans showed flake-like bony destruction located in the spinous process and at the right laminae of C2. Magnetic resonance imaging (MRI) showed a large swelling posterior to C2 and atlantoaxial luxation-induced distinct compression of the spinal cord. The patient underwent 2 weeks of skull traction and quadruple anti-TB treatment before surgery., Results: The patient underwent surgical treatment that included posterior radical debridement, decompression, internal fixation with a screw-rod system from C1 to C4, autologous bone grafting, and fusion. The patient gained normal neural function and returned to work 1 year after surgery., Conclusions: Tuberculosis of the posterior elements of the upper cervical spine is very rare and potentially dangerous. Computed tomography and MRI are very useful and important for correct diagnosis. Purpose: The titanium mesh cage (TMC) is a typical metal cage device which has been widely used in cervical reconstruction for decades. Nano-hydroxyapatite/polyamide-66 (n-HA/PA66) cage is a novel biomimetic non-metal cage device growing in popularity in many medical centres in recent years. There has been no comparison of the efficacy between these two anterior reconstructing cages. The purpose of this study was to compare the radiographic and clinical outcomes of these two different devices., Methods: Sixty-seven eligible patients with single-level ACCF using TMC or n-HA/PA66 cage for cervical degenerative diseases, with four-year minimum follow-up, were included in this prospective non-randomised comparative study. Their radiographic (cage subsidence, fusion status, segmental sagittal alignment [SSA]) and clinical (VAS and JOA scales) data before surgery and at each follow-up was recorded completely., Results: The fusion rate of the n-HA/PA66 group was higher than TMC at one year after surgery (94% vs. 84%) though their finial fusion rates were similar (97% vs. 94%). Finial n-HA/PA66 cage subsidence was 1.5 mm with 6% of severe subsidence over three millimetres, which was significantly lower than the respective 2.9 mm and 22% of TMC (P < 0.0001). Lastly, SSA, VAS and JOA in TMC group were worse than in the n-HA/PA66 group (P = 0.235, 0.034 and 0.007, respectively)., Conclusions: The n-HA/PA66 cage is associated with earlier radiographic fusion, less subsidence and better clinical results than TMC within four years after one-level ACCF. With the added benefit of radiolucency, the n-HA/PA66 cage may be superior to TMC in anterior cervical construction.

Objective: To investigate the effect of the penetration of mini-plate mass screws into facet joint on axial symptoms in cervical laminoplasty., Methods: A retrospective analysis was made on the clinical data of 52 patients who underwent unilateral open-door cervical expansive laminoplasty fixed with Centerpiece mini-plate between September 2009 and December 2011. There were 42 males and 10 females, with a mean age of 61.2 years (range, 34-83 years). Seventeen patients exhibited simple degeneration cervical canal stenosis, 25 patients had multilevel cervical disc protrusion, and 10 patients had ossification of posterior longitudinal ligaments. Disease duration ranged 1-120 months (median, 11 months). The Japanese Orthopedic Association (JOA) score was used to assess neurological function, and JOA recovery rates were calculated. The visual analogue score (VAS) and the neck disability index (NDI) were used to evaluate the axial pain and neck daily activities. The axial symptoms and other complications were recorded. The cervical canal diameter, cervical curvature, cervical canal cross area, and open angle were measured according to the X-ray films, CT scans, and MRI scans. The postoperative CT three dimensional (3-D) reconstruction images were used to identify whether the screws penetrated into the facet joints. All the patients were divided into 3 groups according to involved facet joints: no joint penetrating group (no penetrated facet joint), oligo-joint penetrating group (one or two penetrated facet joints), and multi-joint penetrating group (three or more penetrated facet joints)., Results: Five patients suffered from C5 nerve palsy, and 2 patients had cerebrospinal fluid leakage. The follow-up time ranged 3-35 months (mean, 15.7 months). At the final follow-up, the JOA scores, NDI, cervical canal diameter, and cervical canal cross area were significantly improved when compared with preoperative ones (P < 0.05). At 1 week after operation, CT 3-D reconstruction showed that 16 patients had no penetrated facet joint, 23 patients had one or two penetrated facet joints, and 13 patients had three or more penetrated facet joints. There was no significant difference in age, gender, disease duration, operation time, intraoperative blood loss, and follow-up time among 3 groups (P > 0.05). And at the final follow-up, there was no significant difference in JOA score, VAS score, cervical curvature, cervical canal diameter, cervical canal cross area, the JOA recovery rates, and lamiae open angle among 3 groups (P > 0.05). The NDI of the multi-joint penetrated group was significantly higher than that of other 2 groups (P < 0.05). Axial pain occurred in 1 case of no penetrating group, in 4 cases of oligo-joint penetrating group, and in 5 cases of multi-joint penetrating group, showing no significant difference among 3 groups (Chi(2)=4.881, P=0.087)., Conclusion: The penetrations of lateral mass screws into articular surface of facet joint may contribute to the axial symptoms after cervical laminoplasty. The risk of axial symptom raises accompany with increased penetrated facet joints.

Objective: To assess the mid-term effectiveness of anterior decompression and fusion with nanohydroxyapatite/polyamide 66 (n-HA/PA66) cage in treatment of cervical spondylotic myelopathy., Methods: A retrospective study was made on 48 patients with cervical spondylotic myelopathy who underwent anterior decompression and fusion with n-HA/PA66 cage between August 2008 and January 2010. There were 33 males and 15 females with an average age of 54.5 years (range, 42-72 years). The disease duration was 3-12 months (mean, 6 months). The affected segments included 35 cases of single segment (C3, 4 in 7, C4, 5 in 18, and C5, 6 in 10) and 13 cases of double segments (C3-5 in 7 and C46 in 6). Of 48 patients, 28 was diagnosed as having intervertebral disc protrusion, 12 as having ossification of posterior longitudinal ligament, and 8 as having vertebral osteophyte; 35 patients underwent single segmental anterior corpectomy and fusion, and 13 patients underwent single segmental anterior discectomy and fusion. The pre- and post-operative radiographs (cervical anteroposterior and lateral X-ray films and three-dimensional CT scans) were taken to measure the segmental height and lordosis angle. Brantigan et al assessment standard and visual analogue scale (VAS) and Japanese Orthopaedic Association (JOA) scores were used to evaluate the graft fusion and the improvement of clinical symptoms, respectively., Results: All patients were followed up for 46 months on average (range, 36-54 months). No cage breaking, displacement, or sliding was found. At last follow-up, 36 cases were rated as Brantigan grade E, 10 cases as grade D, and 2 cases as grade C; the fusion rate was 96%. Both segmental height and lordosis angle were corrected significantly at immediate and 6 months after operation and last follow-up than those before operation (P < 0.05), but no significant difference was found among different time points after operation (P > 0.05). At last follow-up, the cage subsidence was (1.3 +/- 1.0) mm. The VAS and JOA scores at 6 months after operation and last follow-up were significantly improved when compared with preoperative scores (P < 0.05), and the scores at last follow-up were superior to ones at 6 months after operation (P < 0.05)., Conclusion: The mid-term effectiveness of anterior decompression and fusion with the n-HA/PA66 cage in patients with cervical spondylotic myelopathy is satisfactory because it can effectively restore and maintain segmental height and lordosis angle and promote osseous fusion.

Objective: To evaluate the application of artificial lamina of multi-amino-acid copolymer (MAACP)/nano-hydroxyapatite (n-HA) in prevention of epidural adhesion and compression of scar tissue after posterior cervical laminectomy., Methods: Fifteen 2-year-old male goats [weighing, (30 +/- 2) kg] were randomly divided into experimental group (n=9) and control group (n=6). In the experimental group, C4 laminectomy was performed, followed by MAACP/n-HA artificial lamina implantations; in the control group, only C4 laminectomy was performed. At 4, 12, and 24 weeks after operation, 2, 2, and 5 goats in the experimental group and 2, 2, and 2 goats in the control group were selected for observation of wound infection, artificial laminar fragmentation and displacement, and its shape; Rydell's degree of adhesion criteria was used to evaluate the adhesion degree between 2 groups. X-ray and CT images were observed; at 24 weeks after operation, CT scan was used to measure the spinal canal area and the sagittal diameter of C3, C4, and C5 vertebrea, 2 normal goats served as normal group; and MRI was used to assess adhesion and compression of scar tissue on the dura and the nerve root. Then goats were sacrificed and histological observation was carried out., Results: After operation, the wound healed well; no toxicity or elimination reaction was observed. According to Rydell's degree of adhesion criteria, adhesion in the experimental group was significantly slighter than that in the control group (Z= -2.52, P=0.00). X-ray and CT scan showed that no dislocation of artificial lamina occurred, new cervical bone formed in the defect, and bony spinal canal was rebuilt in the experimental group. Defects of C4 vertebral plate and spinous process were observed in the control group. At 24 weeks, the spinal canal area and sagittal diameter of C4 in the experimental group and normal group were significantly larger than those in the control group (P < 0.05), but no significant difference was found between experimental group and normal group (P > 0.05). MRI showed cerebrospinal fluid signal was unobstructed and no soft tissue projected into the spinal canal in the experimental group; scar tissue projected into the spinal canal and the dura were compressed by scar tissue in the control group. HE staining and Masson trichrome staining showed that artificial lamina had no obvious degradation with high integrity, some new bone formed at interface between the artificial material and bone in the experimental group; fibrous tissue grew into defect in the control group., Conclusion: The MAACP/n-HA artificial lamina could maintaine good biomechanical properties for a long time in vivo and could effectively prevent the epidural scar from growing in the lamina defect area.

Study Design: A series study of patients with lower cervical facet dislocation accompanied by traumatic disc herniation treated with anterior decompression and nonstructural bone grafting and posterior fixation., Objective: To describe a surgical technique of anterior decompression and nonstructural bone grafting and posterior fixation and its clinical outcome in a group of patients with lower cervical facet dislocation accompanied by traumatic disc herniation., Summary of Background Data: The optimal treatment for lower cervical facet dislocation with a prolapsed disc is still controversial., Methods: After discectomy and endplate preparation, a layer of morselized cancellous bone grafts from the iliac crest was placed in the interspace, and held in appropriate sagittal position by 2 layers of gelatin sponge and carefully sutured longus colli muscle. The anterior wound was then closed. The posterior elements were exposed and the reduction was performed. Fluoroscopy was used during reduction maneuver to ensure that the graft was still in the appropriate position. A posterior fusion was performed and the posterior wound was closed., Results: Between January 2006 and February 2010, 21 patients with cervical facet dislocation accompanied by traumatic disc herniation (13 unilateral dislocations and 8 bilateral dislocations) were recruited for this study. All the patients completed at least 1-year follow-up. Average follow-up duration was 29 ± 3.5 months. Average Frankel scales were significantly improved at the end of follow-up, visual analogue scale decreased from 7.8 ± 1.2 before the operation to less than 1.6 ± 0.5 (P < 0.05) 6 months later. Kyposis was corrected from 17.7° ± 6.3° to 6.5° ± 4.1° (P < 0.05) and remained at 5.9° ± 4.2° (P > 0.05) 1 year later. The average subsidence of bone graft was 1.28 ± 0.16 mm at 12 months after the operation and remained 1.34 ± 0.20 mm at 36 months after the operation. All patients had consolidation of both anterior and posterior fusions. No cases of instrument failure occurred and no complications were attributed to the use of this technique., Conclusion: Although further study based on injury types as well as long-term follow-up is still needed, anterior decompression and nonstructural bone grafting and posterior fixation provides a promising surgical option for treating cervical facet dislocation with traumatic disc herniation.

This research was to design and prepare interbody fusion cages using composite materials of multi-amino acid copolymer/tri-calcium phosphate (MAACP/TCP) and to test compressive strength of the cages. 16 specimens of C3-4 segments from female adult goats were scanned by X-ray to exclude disease of cervical spine, and then anatomical data were measured, i. e. disc space height of C3-4 segment (DSH), sagittal diameter of C3 lower endplate (SDLE3), sagittal diameter of C4 upper endplate (SDUE4), coronary diameter of C3 lower endplate (CDLE3), and coronary diameter of C4 upper endplate (CDUE4). According to the anatomical data, we designed and prepared the interbody fusion cage using the composite of MAACP/TCP and titanium with the same sizes. The MAACP/TCP Cages were made with the method of injection molding and finish machining, and titanium Cages were made with machining. In the testing of compressive strength of Cages, the specimens were divided into three groups, tricortical iliac crest bone group (isolated from goats), MAACP/TCP Cage group and titanium Cage group. There were 8 specimens in every group with the same sizes, the length of 12 mm, the width of 10 mm, and the height of 6 mm. The compressive strength of all specimens was tested on a universal testing machine. The values of DSH, SDLE3, SDUE4, CDLE3 and CDUE4 were (4.78 +/- 0.17) mm, (15.06 +/- 0.53) mm, (12.46 +/- 0.44) mm, (14.47 +/- 0.51) mm and (12.15 +/- 0.65) mm, respectively. MAACP/TCP Cage was successfully designed and made with a compressive strength of 76.34 MPa, which was much higher than that of tricortical iliac crest bone (18.41 MPa). The maximal loading of universal testing machine was 50 000 N, so the compressive strength of titanium Cages, whose value should be more than 541.35 MPa, could not be tested precisely. It is feasible to make cages with MAACP/TCP composite, and the compressive strength of MAACP/TCP Cages was much higher than that of tricortical iliac crest bone isolated from goats.

Study Design: A retrospective study to analyze the effect of decompressive extent on the posterior shift of spinal cord after expansive open-door laminoplasty (ELAP)., Objective: To investigate the effect of decompressive extent on cord shift distance after ELAP, and determine the morphologic limitations of posterior approach when the cervical alignment is lordotic or straight., Summary of Background Data: It is still controversial on the effect of space available for spinal cord at the level cephalad to the decompression with cord shift. Moreover, there is less understanding regarding the significance of decompressive extent of laminoplasty in relation to spinal cord shift and clinical outcome., Methods: Preoperative and postoperative MRIs of 76 patients with a straight or lordotic cervical spine who had undergone cervical laminoplasty were reviewed and evaluated retrospectively. Radiographic parameters including cervical sagittal alignment, space available at the level cephalad, the thickness of compressive mass, and the average anterior subarachnoid space were measured. Laminoplasty was performed from C1 to C7 in 11 cases (CI group), C2 to C7 in 30 cases (CII group), and C3 to C7 in 35 cases (CIII group). According to whether the anterior indirect decompression was adequate or not, CII and CIII groups were further divided into two subgroups, the noncontact group in which the spinal cord was completely separated from the anterior compressive mass after laminoplasty, and the contact group in which there was residual cord compression after laminoplasty. The recovery rate that based on the Japanese Orthopedic Association score was calculated for each patient., Results: There were statistically significant differences in the average anterior subarachnoid space among CI, CII, and CIII groups (P < 0.05);the average anterior subarachnoid space was the largest in CI group, and the smallest in CIII group. The space available at the level cephalad had strong sigmoidal correlation with cord postoperative shift in CIII group (R = 0.91). A higher neurologic recovery rate (69% ± 20% vs. 29% ± 11%; P < 0.05) in the noncontact group after surgery than in the contact group, with a similar follow-up period., Conclusion: The posterior decompression extent is a main factor affecting cord shift distance after laminoplasty in the context of a straightened or lordotic cervical curvature. The space available at the levels cephalad is a key factor to predict cord shift distance in laminoplasty from C3 to C7. Neurologic recovery rate after ELAP is affected by whether the anterior indirect decompression was adequate or not.

Background: A novel nanohydroxyapatite/polyamide-66 cage (n-HA/PA66 cage) with a horseshoe shape was designed to lower the subsidence rate of the traditional hollow cylindrical n-HA/PA66 cage. However, no studies have compared the incidence of subsidence in the two cages. The purpose of this study was to compare the long-term clinical and radiological outcomes of the novel n-HA/PA66 cage with the hollow cylindrical n-HA/PA66 cage after anterior cervical discectomy and fusion (ACDF) to treat single-level cervical degenerative disk disease (CDDD). Methods: Fifty-two patients with novel n-HA/PA66 cages (Group A) and fifty-five patients with hollow cylindrical n-HA/PA66 cages (Group B) were included. The radiological parameters included intervertebral height (IH), C2-7 angle (C2-7a), segmental alignment (SA), subsidence rate, and fusion rate. The clinical outcomes were visual analog scale (VAS) scores, Japanese Orthopedic Association (JOA) scores, and patient satisfaction rates. Results: The pre- and postoperative SA, C2-7a, and fusion rates of the patients in Groups A and B were similar. The preoperative and 6-month postoperative IHs in both groups were comparable. However, the final follow-up IH in Group B was significantly smaller than that in Group A (35.9 mm vs. 36.7 mm). The difference in the subsidence rates at the final follow-up between Group A (5.8%, 3/52) and Group B (18.2%, 10/55) was significant. The VAS score, JOA score, and patient satisfaction rate were not significantly different. Conclusions: The novel n-HA/PA66 cage had similar favorable SA, C2-7a, fusion rate, and clinical outcomes compared to the hollow cylindrical n-HA/PA66 cage for treating single-level ACDF. Moreover, the novel n-HA/PA66 cage achieved a lower subsidence rate and higher IH than the hollow cylindrical n-HA/PA66 cage at the final follow-up. [ABSTRACT FROM AUTHOR]

Background: Several studies have explored cervical kyphosis (CK) in adolescent idiopathic scoliosis (AIS) patients. However, few studies have evaluated the cervical alignment in these patients according to their coronal curve type. The aim of this study was to analyze the radiological features of cervical sagittal alignment in Lenke 1 AIS patients before and after surgery.Methods: This is a retrospective study enrolled 50 patients. Preoperative and postoperative standing full-length radiographs (at last follow-up after operation) were used to measure the coronal and sagittal parameters. Main sagittal parameters included C2-C7 angle, thoracic inlet angle (TIA), T1 slope, proximal thoracic kyphosis (PTK, T1-5 kyphosis) and thoracic kyphosis (TK, T5-12 kyphosis).Results: The TIA of patients with CK was significantly smaller than that of patients with CL (63.0° vs. 76.3°, p < 0.05) and the cutoff value was 71°. The TIA of patients with CK after surgery was significantly smaller than that of patients with CL postoperatively (62.5° vs. 74.6°, p < 0.05) and the cutoff value was 62°. In patients with postoperative CL, there was a significant increase in their PTK and a reduction in their TK, regardless of preoperative CL or CK. In patients whose CL deteriorated to CK after surgery, both their PTK and TK significantly decreased after surgery.Conclusions: Patients with TIA less than 71° were more likely to have CK. And patients with TIA less than 62° would lead to the postoperative uncorrected or new onset of CK. The increased PTK after operation could have a beneficial effect on the improvement of CL. Study Design: Retrospective case series.Objective: To introduce a new type of atlantoaxial rotatory fixation (AARF) with a locked C1-C2 lateral facet and evaluate its therapeutic strategy.Summary Of Background Data: AARF presenting with torticollis and limited neck motion is commonly seen in teenagers. Fielding and Hawkins' classification is generally applied to AARF. Although conservative treatment is effective for acute AARF, it is often unsuccessful for chronic AARF, which ultimately requires surgery. We introduce a new type of chronic AARF with a locked C1-C2 lateral facet that does not fit into Fielding and Hawkins' classification and describe the appropriate treatment.Methods: Eight patients who had chronic AARF with a unilaterally locked C1-C2 lateral facet were referred to our clinic. Reduction had failed after traction for 3 to 4 weeks. After open release and reduction using the anterior retropharyngeal approach, we applied posterior C1-C2 transpedicular screw fixation with an autologous iliac bone graft for stage I or II.Results: The anterior retropharyngeal approach provided direct access to the C1-C2 locked lateral facet. The patient in an overall poor condition (stage II) underwent delayed posterior C1-C2 arthrodesis. The others (stage I) underwent immediate posterior C1-C2 arthrodesis. All patients were followed up for an average of 14.8 months (5-37 mo). Three-dimensional computed tomography revealed C1-C-2 arthrodesis bone graft fusion after an average of 3.1 months (2-4 mo). There was no recurrence of symptoms and no dislocations or internal fixation device loosening or breakage.Conclusion: AARF with a locked C1-C2 lateral facet is a new type of AARF that cannot be classified using Fielding and Hawkins' classification. The anterior retropharyngeal approach for the release and reduction of AARF, followed by posterior C1-C2 arthrodesis is an effective therapeutic strategy for AARF with a locked C1-C2 lateral facet.Level Of Evidence: 4. Background Choosing the fusion level for posterior fusion in patients with Lenke 5C adolescent idiopathic scoliosis (AIS) is highly associated with coronal balance. Previous studies indicated that in patients with lowest end vertebra tilt >25°, surgeons could extend distal fusion to avoid coronal imbalance (CIB). This study aimed to assess the risk factors for CIB in Lenke 5C scoliosis and to discuss how to select fusion level. Methods We reviewed 59 patients with Lenke 5C AIS in 1 institution with at least 2 years follow-up from 2010 to 2015. Preoperative and 3-month and 2-year postoperative follow-up radiographs were measured using several specific measurements related to coronal balance. Patients were categorized into an LEV (lower end vertebra) group and an LEV+1 group. Results CIB was found in 6/31 patients in the LEV+1 group at final follow-up and not found in the LEV group at the first or final follow-up. The C7 plumb line shifted to the convex side of the central sacral vertical line in 47/59 patients, including all of the 19 patients with CIB after surgery. Patients who underwent fusion at LEV+1 with >25° LEV tilt also showed poor results regarding CIB. Statistically, coronal balance at the final follow-up was correlated with preoperative bending lumbosacral hemicurve ( P = 0.002) and all 6 patients with CIB had bending lumbosacral hemicurve >15°. No significant difference was found in Scoliosis Research Society–22 questionnaire scores between the 2 groups at 2-year-follow-up. Conclusions Distal fusion extension at LEV+1 is more likely to result in CIB at the first and final follow-up, especially when the bending lumbosacral hemicurve is >15°.
