3. Women’s Experiences in the Childbirth Care Process: An Integrative Review.

Introduction: Childbirth is recognized as a physiological process essential to procreation and societal development. Nevertheless, it often lacks adequate discussion on humanized care practices. Therefore, it is necessary to identify women’s experiences during vaginal birth care to create practices that ensure better quality care. Objective: To characterize scientific evidence related to women’s experiences during vaginal birth care. Materials and Methods: An integrative review was conducted following Cooper’s methodology; six databases were reviewed: Cochrane, PubMed, Science Direct, Springer, Scopus, and Cinahl, using the descriptors: Normal Birth; Humanized Birth; Humanization of Care; Obstetric Violence; Qualitative Research, in Spanish, English, and Portuguese, limited to the 2010-2023 period, with the following inclusion/exclusion criteria: Qualitative full-text articles on vaginal births with a score of 8 or higher according to CASPe. Results: Six thematic units emerged: microaggressions and macro-impacts: childbirth: between pain and fear; acting with respect: childbirth as a natural process; silencing, enduring, and bearing; my birth, my choice; making the invisible visible: normalizing violence during childbirth; and the ritual of childbirth: women’s dignity. Conclusions: Women’s childbirth experiences are influenced by the cultural contexts in which they live; the biomedical model and traditional practices clash and impact women’s lives. Women must know their reproductive rights to contribute to a humanized childbirth. [ABSTRACT FROM AUTHOR]

The objective was to analyze the experiences and conditions that enhanced the violation of obstetric violence in a group of Nahua women. Qualitative study in which ten women who experienced childbirth in the last three years were interviewed. The interviews were transcribed and analyzed using Strauss and Corbin's proposal. Women faced higher levels of vulnerability because of their gender status, and also because they live with high levels of poverty and marginalization, they do not understand Spanish well and they have a worldview that makes them experience the disowning of their bodies more painfully, within sexist, patriarchal and lacking-ofinterculturality institutions. In Brazil obstetric violence has been researched since the 1980s. However, in the nineties the phenomenon started to have a more prominent position. The form of violence analysed in this study refers to a covert violence, called "perfect violence". This essay reflects about the occurrence of perfect violence in obstetrics, especially regarding the subtleties of medical discourse, which can disguise this aggression as a form of care. This type of violence may sound as if it was a concern with the patient's health on behalf of the doctor, but it can make the patient submit herself to the medical recommendations in a passive manner, believing it is the best for her and her baby. In this way, violence is disguised as good practice, allegedly aiming at what is best for the patient. In practicing perfect violence, the doctor can interfere in the delivery outcome. The caesarean "epidemic" in Brazil has been justified by doctors as the woman's preference, but research undermines this hypothesis and raises the reflection: who is in fact choosing the type of childbirth? This research article attempts to study the phenomenon of obstetric violence as a form of gender-based violence by trying to identify those elements that would typify this practice in relation to its social and structural causes. To this end, the international normative and jurisprudential framework will be examined in order to identify international standards of reference conducive to preventing and eventually sanctioning such behavior. It is also suggested that the implementation of a human rights approach in attempting to prevent such misconduct has contributed to the invisibility and normalization of such a disvaluable practice. In order to address the issue of obstetric violence from a theoretical perspective, this article reviews the existing literature, using a qualitative socio-legal analysis through a review of norms and jurisprudence. To characterize OV through the documentary "The rebirth of childbirth". Exploratory research with a qualitative approach based on the reports present in the trilogy "The Renaissance of Childbirth". Verbal violence, disrespect for female protagonism and manipulation were the frequent situations of obstetric violence in the reports, neglecting women's rights and transforming childbirth into a procedure motorized and disrespectful for both the mother and the baby, causing diverse and often irreversible consequences, such as behavioral disorders, confusions, traumas, insecurities, transforming a potentially unforgettable, biological and natural event into something terrifying, traumatic and inhumane. Nursing professionals, along with their team, need to provide an environment of total welcome, from the moment of prenatal care to the time of delivery and postpartum follow-up. The human look and holistic vision should predominate and be cultivated by them, respecting and prioritizing women's emotions and feelings. [ABSTRACT FROM AUTHOR]

This research aims to offer a rationale on the subjection of women to the medical-hospital authority during pregnancy and childbirth. The study analyzes the current obstetric system in Brazil from a qualitative approach, based on narratives of twenty-four women who tell stories about how they felt objectified at the time of childbirth. The analysis shows that this system has been constituted in a colonized and violent way. The relationship of these women with the obstetric system is ruled by every form of body objectification, obstetric violence, and non-attendance to the woman's wishes as the protagonist of childbirth. Professional tutelage prevails to the detriment of the body's knowledge, of sensitiveness, of what is natural. The alternative to excessive medical/hospital authority in the traditional process has been the search for humanized teams, dismantling the hegemonic procedure from the desire to live childbirth as an experience of protagonism. Despite being a relatively new term, obstetric violence is an old problem. In 2014, the World Health Organization declared: "Many women experience disrespectful and abusive treatment during childbirth in facilities worldwide. Such treatment not only violates the rights of women to respectful care, but can also threaten their rights to life, health, bodily integrity, and freedom from discrimination". This problem, named as "abuse", "disrespect" and/or "mistreatment" during childbirth, has been addressed in several studies. However, there has been no consensus on how to properly name this problem, although its typology has been well described. Considering the magnitude of this problem, it is essential to give the correct terminology to this important health and human rights issue. Naming it as obstetric violence and understanding it as gender-based violence will ensure appropriate interventions to avert this violation of women's rights. [ABSTRACT FROM AUTHOR]

Objective: The aim is to identify the discursive positions of different actors about obstetric violence and its relationship with the social field of pregnancy, delivery and puerperium care, produced by newspapers from Argentina, Chile, Colombia, Mexico and Venezuela from 2010 to March 2017. Methodology: Qualitative study, of an exploratory nature using the discourse analysis approach of 120 journalistic notes. Results: Discursive positions of professionals and women were identified, both traditional and open visions, in reference to healthcare. Conclusions: The interventions on this violence are not isolated, but framed in a context of discussion, where the discursive positions of the actors can be defined and concretized. El parto en Chile ha tenido grandes cambios históricos, políticos y económicos. Este artículo analiza el parto como un proceso socioeconómico basándose en dos investigaciones de ciencias sociales: la primera sociológica, con una revisión de la literatura sobre el consumo en Chile; y la segunda antropológica, con entrevistas a personal de salud obstétrica en Santiago de Chile. Las reflexiones que proponemos con este cruce interdisciplinario dan cuenta de cómo el parto en Santiago de Chile pasa de ser un proceso manejado entre la comunidad y luego profesionalizado en el siglo XX, a convertirse hoy en un objeto de consumo, ofertado especialmente por instituciones privadas. Desde los recientes modelos de atención obstétrica veremos que, dentro de una cultura neoliberal, la producción de un objeto-parto con atrayentes características, promete mejoras tanto en la experiencia de las mujeres (previamente violentadas) como en sus recién nacidos, aquí llamadas guaguas optimizadas. Siempre y cuando esto pueda pagarse. Estas investigaciones nos proponen poner atención en el intercambio gestado por las instituciones, quienes ofrecen métodos diferenciados del parir tomando las demandas sociales contra la violencia obstétrica para empaquetarlas en programas que parecen mejorar el producto humano y la experiencia materna. Queda abierta la cuestión vivida dentro de estos partos e invitamos a investigar tales posibilidades., Childbirth in Chile has had great historical, political, and economic changes. This article analyzes childbirth as a socio-economical process, based on two social science investigations: a sociological one that produces a literature review of Chilean consumption; and a anthropological one, based on interviews to obstetric staff in Santiago, Chile. The reflections we propose with this interdisciplinary work address how childbirth in Santiago de Chile goes from being a process managed by the community and then professionalized in the 20th century, to become today as an object of consumption, offered mainly by private institutions. From new models of obstetric care, we will see that within a neoliberal culture, that the production of a childbirth-object with seductive characteristics, promises improvements, both in the experience of women (previously mistreated) and in their newborns or -as we call them- optimized babies. As long as they can pay for it. The mixture of these two investigations proposes to pay attention to the exchange that the institutions are advertising, which offer differentiated methods of childbirth care, taking social demands against obstetric violence to package them into programs that seem to improve the human product and the mother’s experience. The question within the experimentation of these deliveries remains open and we extend the invite to investigate such possibilities., O parto no Chile passou por grandes mudanças históricas, políticas e econômicas. Este artigo analisa o parto como um processo sócio-econômico baseado em duas investigações das ciências sociais: a primeira sociológica, com uma revisão da literatura sobre consumo no Chile; e a segunda antropológica, com entrevistas com pessoal de saúde obstétrica em Santiago do Chile. As reflexões que propomos com esta encruzilhada interdisciplinar mostram como o parto em Santiago do Chile passou de um processo gerenciado pela comunidade e depois profissionalizado no século XX, para se tornar hoje um objeto de consumo, oferecido especialmente por instituições privadas. A partir de modelos recentes de atendimento obstétrico veremos que, dentro de uma cultura neoliberal, a produção de um projeto de nascimento com características atraentes promete melhorias tanto na experiência das mulheres (anteriormente violadas) quanto em seus recém-nascidos, aqui chamados de bebês otimizados. Desde que isso seja possível. Esta pesquisa sugere que prestemos atenção ao intercâmbio gerado pelas instituições que oferecem métodos diferenciados de parto, levando as demandas sociais contra a violência obstétrica e embalando-as em programas que parecem melhorar o produto humano e a experiência maternal. A questão da experiência vivida dentro destes nascimentos permanece em aberto e convidamos você a investigar tais possibilidades.

Resumo: O artigo tem o objetivo de analisar a violência obstétrica e suas diversas expressões sob a ótica de puérperas. Captadas através de entrevistas semiestruturadas com 12 puérperas acompanhadas por uma Unidade Básica de Saúde em um município do nordeste brasileiro, as questões versaram sobre o perfil socioeconômico, a experiência vivenciada no parto e a percepção que as mesmas detinham sobre a violência obstétrica. Identificamos que a violência obstétrica é recorrente na assistência hospitalar e expressa desigualdades e opressões nas relações de gênero e entre profissionais e usuárias dos serviços de saúde. O enfrentamento da violência obstétrica demanda atribuir maior visibilidade a esta problemática, incluindo-a nos processos de formação e nos espaços de trabalho destes profissionais, bem como entre as mulheres, na busca de propiciar a identificação e o enfrentamento desta forma de violência. Abstract: The study aims to analyze obstetric violence and its various expressions from the perspective of postpartum women. Captured through semi-structured interviews with 12 postpartum women accompanied by a Basic Health Unit in a municipality in northeastern Brazil, the questions were about the socio-economic profile, the experience of childbirth, and the perception they had about obstetric violence. We identified that obstetric violence is recurrent in-hospital care and expresses inequalities and oppression in gender relations and between professionals and users of health services. The confrontation of obstetric violence demands to give greater visibility to this problem, including it in the training processes and the workspaces of these professionals, as well as among women, in an attempt to provide the identification and confrontation of this form of violence. Resumen: El estudio tiene como objetivo analizar la violencia obstétrica y sus diversas expresiones desde la perspectiva de la mujer posparto. Capturadas a través de entrevistas semiestructuradas con 12 puérperas acompañadas de una Unidad Básica de Salud en un municipio del noreste de Brasil, las preguntas fueron sobre el perfil socioeconómico, la experiencia del parto y la percepción que tenían sobre la violencia obstétrica. Identificamos que la violencia obstétrica es recurrente en la atención hospitalaria y expresa desigualdades y opresión en las relaciones de género y entre profesionales y usuarios de los servicios de salud. El enfrentamiento a la violencia obstétrica demanda dar mayor visibilidad a esta problemática, incluyéndola en los procesos de formación y en los espacios de trabajo de estas profesionales, así como entre las mujeres, en un intento por brindar la identificación y confrontación de esta forma de violencia.

RESUMO O presente trabalho tem o objetivo de construir uma inteligibilidade sobre a sujeição da mulher à autoridade médico-hospitalar nos momentos de gestação e parto. O sistema obstétrico vigente no Brasil, estudado a partir de uma abordagem qualitativa, envolvendo narrativas de 24 mulheres que contam histórias sobre como elas se sentiram despersonificadas no momento do parto, denota que este tem se constituído de modo colonizado e violento. Na relação dessas mulheres com o sistema obstétrico, impera toda forma de objetificação do corpo, de violência obstétrica e de não atendimento às vontades da mulher enquanto ser protagonista do parto. Impera a tutela profissional em detrimento da atuação do saber do corpo, do sensível, do comum. A alternativa à excessiva autoridade médica/hospitalar no processo tradicional têm sido a busca por equipes humanizadas, desarticulando o procedimento hegemônico a partir do desejo de viver o parto como uma experiência de protagonismo. RESUMEN El presente trabajo tiene como objetivo construir una inteligibilidad sobre el tema de la mujer a la autoridad médico-hospitalaria en los momentos del embarazo y parto. El sistema obstétrico vigente en Brasil, estudiado desde un enfoque cualitativo, involucrando narrativas de veinticuatro mujeres que cuentan historias sobre cómo se sintieron despersoficadas en el momento del parto, denota que se ha constituido de manera colonizada y violenta. En la relación de estas mujeres con el sistema obstétrico, prevalece toda forma de objetivación del cuerpo, violencia obstétrica e incumplimiento de la voluntad de la mujer siendo protagonista del parto. La tutela profesional prevalece a costa de actuar sobre el conocimiento del cuerpo, lo sensible, lo común. La alternativa a la excesiva autoridad médico/hospitalaria en el proceso tradicional ha sido la búsqueda de equipos humanizados, desmantelando el procedimiento hegemónico del deseo de vivir el parto como una experiencia de protagonismo. ABSTRACT This research aims to offer a rationale on the subjection of women to the medical-hospital authority during pregnancy and childbirth. The study analyzes the current obstetric system in Brazil from a qualitative approach, based on narratives of twenty-four women who tell stories about how they felt objectified at the time of childbirth. The analysis shows that this system has been constituted in a colonized and violent way. The relationship of these women with the obstetric system is ruled by every form of body objectification, obstetric violence, and non-attendance to the woman’s wishes as the protagonist of childbirth. Professional tutelage prevails to the detriment of the body’s knowledge, of sensitiveness, of what is natural. The alternative to excessive medical/hospital authority in the traditional process has been the search for humanized teams, dismantling the hegemonic procedure from the desire to live childbirth as an experience of protagonism.

31. Ausencia de percepción de violencia obstétrica en mujeres indígenas del centro norte de México.

- Abstract
Introduction: Indigenous women are the population that least complains in the institutions of medical arbitration and human rights defense. Objective: To know in indigenous women the facts perceived as acts of obstetric violence in the attention of their births. Methodology: A qualitative-ethnographic study was carried. Group interviews were conducted and critical analysis of discourse was applied. Results: Women only identify verbal abuse as violence, excessive medicalization processes are considered necessary, inevitable and even desirable, with the exception of vaginal tact. Women reported experiences of abuse of power, omission and neglect that women tolerate because they are naturalized by the doctor-patient relationship. Conclusions: It is necessary to enable women to identify violent actions and sensitize them to assume the rights to care. It is also necessary to promote changes in the processes of training human resources in health, to mainstream gender perspective, interculturality and human rights. [ABSTRACT FROM AUTHOR]

Un factor importante durante la etapa del embarazo es disponer de un estado emocional adecuado que permita la evolución sin obstrucción reflejada por algún tipo de violencia por ello, esta investigación propone caracterizar la violencia obstétrica en la etapa de embarazo, parto y puerperio en el hospital regional Dr. Verdi Cevallos Balda periodo 2019 – 2020. Para ello se utilizó un tipo de estudio no experimental, con un enfoque cuali - cuantitativo; apoyados con los métodos, estadístico, bibliográfico, analítico, descriptivo, se utilizó técnicas como la entrevista y la encuesta, con los instrumentos: la guía de entrevista y cuestionario, en una población y muestra de 210 mujeres atendidas en el área de ginecología del hospital. Como resultados se obtuvo que, con el 74% las mujeres en el proceso de puerperio, se han sentido deprimidas, mientras que con el 5% se han sentido intimidadas por algún profesional del hospital. Para concluir, este trabajo pone de manifiesto que existe violencia durante la etapa del embarazo, parto y puerperio, si bien esta no se atribuye a factores físicos, si implica características psicológicas, reflejadas en el hospital por parte del personal de salud, quienes están inmersos en la atención que se brinda a usuarios. An important factor during the pregnancy stage is having an adequate emotional state that allows the evolution without obstruction reflected by some type of violence, therefore, this research proposes to characterize obstetric violence in the pregnancy, delivery and postpartum stage in the regional hospital. Dr. Verdi Cevallos Balda period 2019-2020. For this, a non-experimental type of study was used, with a qualitative and quantitative approach; supported by the statistical, bibliographic, analytical and descriptive methods, techniques such as the interview and the survey were used, with the instruments: the interview guide and questionnaire, in a population and sample of 210 women attended in the gynecology area of the hospital . As results, it was obtained that, with 74% women in the postpartum process, they have felt depressed, while with 5% they have felt intimidated by a hospital professional. To conclude, this work shows that there is violence during the stage of pregnancy, childbirth and the puerperium, although this is not attributed to physical factors, but does involve psychological characteristics, reflected in the hospital by health personnel who are immersed in the care provided to users.

Trabajo fin de grado. Defendido en junio de 2022. La situación actual a efectos legales y a efectos de la práctica clínica, en torno a la violencia obstétrica es algo difusa. El estudio propuesto pretende obtener una prevalencia de la violencia obstétrica en España, a través de determinados cuestionarios que relacionen de manera objetiva la percepción de las mujeres sobre la violencia obstétrica durante el embarazo, parto y puerperio con las secuelas físicas, psicológicas y de relaciones inter-familiares, tratando de establecer la relación existente entre determinadas técnicas clínicas y el aumento de las consecuencias posteriores. Para conseguir esta información se plantea un estudio descriptivo, observacional, longitudinal, multicéntrico y prospectivo. Abstract: The current legal and clinical practice situation regarding obstetric violence is somewhat diffuse. Therefore, the proposed study aims to obtain a prevalence of obstetric violence in Spain, through certain questionnaires that objectively relate women's perception of obstetric violence during pregnancy, childbirth and puerperium with the physical, psychological and inter-family relationship sequelae, trying to establish the relationship between certain clinical techniques and the increase in subsequent consequences. La violencia obstétrica, definida como un conjunto de prácticas y conductas realizadas por profesionales de la salud a las mujeres durante el embarazo, el parto y el puerperio, que por acción u omisión son violentas o pueden ser percibidas como violentas, ha generado un debate tanto profesional como social en los últimos años. La propuesta del Ministerio de Igualdad de incluir la violencia obstétrica dentro de la reforma de la Ley de salud reproductiva y sexual (2010) propició la aparición de diferentes comunicados de ginecólogos y matronas, que se posicionaban en contra y a favor respectivamente. Para conocer la realidad de la violencia obstétrica, se presenta este Trabajo de Fin de Grado en formato de reportaje multimedia con el objetivo de acercar este término a los lectores, así como los diferentes puntos de vista que suscita. Para ello, se usa una metodología de revisión bibliográfica, entrevistas e historias de vida. De este modo, se logra el testimonio de una experta en la investigación de la violencia obstétrica, dos ginecólogas con visiones enfrentadas, una matrona y una madre que sufrió este tipo de violencia en su primer parto. Todo ello da como resultado un reportaje multimedia que, a través de una narrativa con vídeos, audios y visualizaciones interactivas, consigue reflejar la realidad de una violencia que está extendida e invisibilizada., Obstetric violence, defined as a set of practices and behaviors carried out by health professionals on women during pregnancy, childbirth and the puerperium, which by action or omission are violent or may be perceived as violent, has generated a debate both professional and social in recent years. The proposal of the Ministry of Equality to include obstetric violence within the reform of the reproductive and sexual health law led to the appearance of different communiqués from gynecologists and midwives, who positioned themselves against and in favor, respectively. In order to learn about the reality of obstetric violence, this Final Degree Project is presented in a multimedia report format with the aim of bringing readers closer to this term, as well as the different points of view it raises. To achieve this, a methodology of bibliographic review, interviews and life stories is used. In this way, the testimony of an expert in the investigation of obstetric violence, two gynecologists with conflicting views, a midwife and a mother who suffered this type of violence in her first childbirth is obtained. Violence against women in childbirth is a recurring theme in Brazilian society. Explicit or veiled, obstetric violence against pregnant women is presented in this study as a result of the excessive medicalization of childbirth process, which submits the pregnant woman an absolute invisibility process as a result of medical knowledge-power that is imposed in the mold Foucault’s philosophy. In this sense, we try to understand, from the phenomenological method, the extent to which women victims of such violence can be considered a “naked life” under the category developed in the work of the Italian philosopher Giorgio Agamben. The birth process has always been seen as one of the most special moments in a woman's life. However, situations of obstetric violence during the pregnancy and puerperal period are very present in public and private institutions, causing a painful event both physically and psychologically. Thus, this article aimed to describe obstetric violence and its implications for the well-being of parturients and to investigate the current scientific production on the subject, and demonstrate methods that enable the quality of care. The study consists of an integrative literature review with a theoretical approach. 43 articles were initially selected that addressed the descriptors, which after analyzing the object of study and the inclusion criteria, remained 21 articles, in the language: Portuguese, published between 2014 and 2020. The exclusion criteria were thesis publications, dissertation, monograph and duplicate materials and articles not directly related to the research theme. Women have the right to choose obstetric care, therefore, they must have their dignity assured with regard to the basic principles of sexual and reproductive rights. Changes in current obstetric care practices are considered necessary, in order to reduce unnecessary interventions, and to promote actions of humanization of care. El proceso del parto siempre se há considerado uno de los momentos más especiales en la vida de una mujer. Sin embargo, las situaciones de violencia obstétrica durante el embarazo y el puerperio están muy presentes en las instituciones públicas y privadas, provocando un evento doloroso tanto física como psicológicamente. Así, este artículo tuvo como objetivo describir la violencia obstétrica y sus implicaciones para el bienestar de las parturientas e investigar la producción científica actual sobre el tema, y demostrar métodos que posibiliten la calidad de la atención. El estudio consiste en una revisión integradora de la literatura con un enfoque teórico. Se seleccionaron inicialmente 43 artículos que abordaron los descriptores, que luego de analizar el objeto de estudio y los criterios de inclusión, quedaron 21 artículos, en el idioma: portugués, publicados entre 2014 y 2020. Los criterios de exclusión fueron publicaciones de tesis, disertación, monografía y duplicado. materiales y artículos no relacionados directamente con el tema de investigación. Las mujeres tienen derecho a elegir la atención obstétrica, por lo tanto, deben tener asegurada su dignidad con respecto a los principios básicos de los derechos sexuales y reproductivos. Los cambios en las prácticas actuales de atención obstétrica se consideran necesarios para reducir las intervenciones innecesarias y promover acciones de humanización de la atención. O processo de parto sempre foi encarado como um dos momentos mais especiais da vida de uma mulher. Entretanto, as situações de violência obstétrica durante o período gravídico puerperal são muito presentes em instituições públicas e privadas, ocasionando um acontecimento doloroso tanto de forma física ou psicológica. Sendo assim, este artigo teve como objetivo descrever a violência obstétrica e suas implicações no bem-estar da parturiente e investigar a produção cientifica atual acerca do tema, e demostrar métodos que possibilitem a qualidade da assistência. O estudo constitui-se de uma revisão integrativa da literatura, com abordagem teórica. Foram selecionados inicialmente 43 artigos que abordavam os descritores, que após análise do objeto de estudo e os critérios de inclusão, restaram 21 artigos, no idioma: português, publicados entre os anos de 2014 e 2020. Os critérios de exclusão foram publicações de tese, dissertação, monografia e materiais duplicados e artigos não relacionados diretamente à temática da pesquisa. A mulher tem direito de escolha da assistência obstétrica, deve, portanto, ter a sua dignidade assegurada no que concerne aos princípios básicos dos direitos sexuais e reprodutivos. Consideram-se necessárias mudanças nas práticas assistenciais obstétricas vigentes, visando a reduzir as intervenções desnecessárias, é promover ações de humanização do cuidado.

Resumen Introducción: Se analizan, desde una perspectiva feminista, algunas formas de maltrato y violación de los derechos humanos, sexuales y reproductivos de las mujeres durante la atención del embarazo y parto, fenómeno conocido como violencia obstétrica. Desarrollo: Se analizan las historias de embarazo/parto de seis mujeres que parieron sus hijos/as en instituciones hospitalarias, tanto públicas como privadas, así como la autoetnografía de los dos embarazos/partos de la autora, para analizar las maneras en las que las mujeres describen y dan sentido a sus experiencias de violencia obstétrica durante la atención de su parto. En el artículo, se visibilizan las diversas formas de violencia que componen la violencia obstétrica, como institucional, física, verbal, psicológica y simbólica. Conclusiones: Se aportan algunos elementos que, a juicio de las mujeres y de la Organización Mundial de la Salud, deben transformarse para garantizar un trato digno y el derecho a un parto y nacimiento en condiciones de respeto y seguridad. Abstract Introduction: This article analyzes some forms of mistreatment and violation of the human, sexual and reproductive rights of women during the pregnancy, delivery and childbirth care, phenomenon known as obstetric violence. Development: It analyzes the pregnancy/childbirth histories of six women who gave birth their children in public and private hospitals, as well as the auto-ethnography of the two pregnancies/delivery of the author; to analyze the ways in which women describe and give meaning to their experiences of obstetric violence during their delivery care. In the article, the different forms of violence that make up obstetric violence are visualized, such as institutional, physical, verbal, psychological and symbolic violence. Conclusions: Analyzing the elements that, in the opinion of women and the World Health Organization, must be transformed to guarantee a dignified care and the right to a delivery and birth in conditions of respect and security. Resumo Introdução: Analisam-se, desde uma perspectiva feminista, algumas formas de abuso e violação dos direitos humanos, sexuais e reprodutivos, das mulheres durante a atenção da gravidez e parto, fenômeno conhecido como violência obstétrica. Desenvolvimento: Se analisam as histórias de gravidez/parto de seis mulheres que pariram aos seus filhos/as em instituições hospitalares tanto públicas quanto privadas, assim como a etnografia das duas gravidezes/partos da autora; para analisar as maneiras como as mulheres descrevem e são sentido às suas experiências de violência obstétrica durante a atenção de seu parto. No artigo, se visibilizam as diversas formas de violência que compõem a violência obstétrica, como institucional, física, verbal, psicológica e simbólica. Conclusões: Aportam-se alguns elementos que, ao juízo das mulheres e a Organização Mundial da Saúde, devem transformar-se para garantir um tato digno e o direito a um parto e nascimento em condições de respeito e segurança.

The biomedical discourse illustrates the existence of a strong biologistic vision and reinforces the belief that childbirth is a pathological, risky and uncontrollable event. The witnesses show a chain of abuses, mistreatments and violence that lead to the systematic violation of the human rights of women. Conclusions. In Mexico, the practice of unjustified cesareans is a naturalized exercise of violence, linked to other forms of abuse. Biomedical care of childbirth is experiencing a moment of profound questioning as the violence that women suffer during childbirth has increased. It is urgent to expose this type of violence and promote health policies that transform, dignify and humanize the public services of childbirth healthcare., Introducción. El discurso biomédico ilustra la existencia de una marcada visión biologicista y refuerza la creencia del parto como un evento patológico, de riesgo e incontrolable. Los testimonios muestran una cadena de abusos, maltratos y violencia que se traduce en la vulneración sistemática de los derechos humanos de las mujeres. Conclusiones. La atención hospitalaria al nacimiento en nuestro país, se caracteriza por ser altamente medicalizada, intervencionista y por elidir los aspectos subjetivos de los protagonistas. Reflexionaremos sobre las razones que posibilitan dicho escenario, a pesar de existir un extenso marco legal y vasta evidencia científica que refleja los efectos negativos de este modelo atencional. Estas prácticas son el reflejo de lógicas patriarcales que subyacen a los entramados relacionales de nuestra sociedad y una de las tantas formas que adopta la violencia de género en la actualidad. Considerando al nacimiento como un evento profundamente subjetivo, significativo y singular, puntualizaremos acerca de las posibles consecuencias emocionales y trazas subjetivas que devienen de la atención dominante. In this paper, we analyze the reasons that make this scenario possible, despite the existence of an extensive legal framework and vast scientific evidence that show the negative effects of this model. These practices reflect the patriarchal logics which underlie the relational social fabric of our society and one of the many forms that gender violence adopts today. Considering birth as a deeply subjective, significant and singular event, we draw attention to the possible emotional consequences and subjective traces that result from the dominant attention model. Over the last several years, a new legal construct has emerged in Latin America that encompasses elements of quality of obstetric care and mistreatment of women during childbirth - both issues of global maternal health import. Termed "obstetric violence," this legal construct refers to disrespectful and abusive treatment that women may experience from health care providers during pregnancy, childbirth, and the postpartum period, as well as other elements of poor quality care, such as failure to adhere to evidence-based best practices. This new legal term emerged out of concerted efforts by women's groups and networks, feminists, professional organizations, international and regional bodies, and public health agents and researchers to improve the quality of care that women receive across the region. Resumo: A “humanização” da assistência ao parto vem se difundindo no Brasil nas últimas décadas, tendo se disseminado principalmente nas camadas médias urbanas. Através de sua incorporação ao sistema público de saúde, a proposta foi estendida também a mulheres de camadas populares, o que vem colocando alguns desafios e gerado questionamentos, sobretudo ao formato que tem assumido a assistência “humanizada” nas instituições públicas. A partir da análise de duas situações vivenciadas por mulheres de diferentes classes sociais (uma no setor público e outra no privado), o artigo busca refletir acerca das noções de parto “natural” e de parto “humanizado”, apontando como se configuram, em cada um desses contextos, diferentes percepções sobre “humanização” e, em decorrência, também sobre o que seria seu oposto: a “violência obstétrica”. Abstract: The proposal of “humanization” of childbirth care has gained ground in the last decades in Brazil, having spread mainly in the urban middle strata. However, through its incorporation into the public health system, the proposal was also extended to women from the lower classes. This reality has posed some challenges and has raised questions about the format that “humanized” assistance has assumed in public institutions. Based on the analysis of two specific situations experienced by women of different social classes (one assisted in the public sector and the other in the private sector), the article seeks to reflect on the notions of “natural” childbirth and “humanized” childbirth, pointing out, in each of these contexts, different perceptions about “humanization” and, consequently, considerations on what could be viewed as its opposite: “obstetric violence”.

Diante de um contexto social em que predominam os partos instrumentais, via cirurgia cesariana, nos hospitais particulares e a "cascata de intervenções" nos partos vaginais dos hospitais públicos brasileiros, o movimento de mulheres marcou o início do século XXI com a nomeação da "violência obstétrica" (VO). A compreensão da VO abarca desde agressões físicas, psicológicas, verbais, simbólicas, sexuais até negligências nas assistências, discriminação, medicalização excessiva e inapropriada, adesão a práticas obstétricas desaconselhadas, dolorosas, prejudiciais e sem embasamento em evidências científicas - vividas no momento da gestação, parto, nascimento e pós parto. Diante deste cenário, o objetivo geral da tese é analisar os aspectos desta violência - com imposição de dor, sofrimentos e mortes evitáveis, legitimada pela ciência obstétrica e autorizada pelo Estado como assistência à saúde sexual e reprodutiva. A hipótese que norteou a pesquisa é a de que a VO é um ato misógino de punição às mulheres, resultado de séculos de negação de sua sexualidade e capacidade de decidir. A pergunta central a qual a pesquisa buscou responder é como se deu a autorização política para violentar os corpos de mulheres, assim como sua naturalização e invisibilização justificadas pela ciência obstétrica e praticadas por sua assistência à saúde sexual e reprodutiva. Utilizando de uma abordagem qualitativa, a pesquisa se desenvolveu em três dimensões: 1) Reconstrução genealógica do termo e análise da conjuntura da VO a partir de casos denunciados, instrumentos normativos, documentações nacionais e internacionais, atuações civis, conceituações e tipificações; 2) Análise de dados secundários de um inquérito nacional sistematizado pela Fiocruz em 2014 e 3) Relatos de observação participante em partos e rodas de parto na capital e em uma cidade do interior de Goiás, além de um relato autobiográfico situado e focalizado. As análises se fundamentaram na teoria social de cunho construcionista e nas teorias feministas, com destaque para as contribuições da interseccionalidade. Com este acervo de conhecimentos foi possível refletir sobre como as relações intrínsecas entre Estado e Parto e via as diferentes assistências - pública e privada - coordenam a disciplina sobre o corpo expropriado das mulheres conforme seus marcadores sociais de cor, classe, raça/etnia e gênero. Desse modo, há importantes indícios de que os processos de usurpação científica operados pela atual ciência obstétrica sobre a antiga parteria tradicional, a utilização política da atuação das parteiras nas comunidades fora do alcance/interesse do Estado, a desigualdade entre as violências conforme as mulheres e as assistências, o imperativo de uma obstetrícia sem evidências científicas e o próprio engodo dos programas políticos da "humanização", estejam todos a serviço do controle deste Estado ocupado em manter a punição sexual às mulheres. Faced with a social context in which instrumental deliveries predominate through cesarean surgery in private hospitals and the "cascade of interventions" in the vaginal deliveries of Brazilian public hospitals, a women's movement marked the beginning of the 21st century with the appointment of "obstetric violence" (OV). The understanding of OV ranges from physical, psychological, verbal, symbolic, sexual aggressions to negligence in attendance, discrimination, excessive and inappropriate medication, consent to obstetric practices not recommended, painful, harmful and unfounded in scientific evidence - lived during the gestation, childbirth, birth and postpartum. Given this scenario, the aim of the thesis is to analyze the aspects of this violence - with imposition of pain, suffering and avoidable deaths, legitimized by obstetric science and authorized by the State as sexual and reproductive health care. The hypothesis that guided the research is that OV is a misogynist act of punishment to women, the result of centuries of denial of their sexuality and ability to decide. The central question that the research sought to answer is how political authorization was given to violate women's bodies, as well as its naturalization and justified invisibility in obstetric science and practiced in sexual and reproductive health care. Using a qualitative approach, the research developed in three dimensions: 1) Genealogical reconstruction of the term and analysis of the OV conjuncture from reported cases, normative instruments, national and international documentation, civil actions, conceptualizations and typifications; 2) Analysis of secondary data from a national survey systematized by FIOCRUZ in 2014 and (3) Reports of participant observation in labor and delivery in the capital and in a rural city of the state of Goiás, as well as an autobiographical narrative situated and focused. The analyzes were based on social theory of constructionist and feminist theories, with emphasis on the contributions of intersectionality. With this body of knowledge, it was possible to reflect on how the intrinsic relations between the State, childbirth and the different health care - public and private - coordinate the discipline on the expropriated body of women, according to their social markers of color, class, race/ethnicity and genre. Thus, there are important indications that the processes of scientific usurpation operated by the current obstetrical science on the old traditional midwife, the policies of the state against the performance of midwives in the communities, the inequality between the violence of women according to women and assertions, the imperative of obstetrics without scientific evidence, and the deception of the political programs of "humanization", are all at the service of the control of this State engaged in maintaining sexual punishment of women. Metodología: Estudio cualitativo, de carácter exploratorio, tipo análisis del discurso de 120 notas periodísticas. Resultados: Se identificaron posiciones discursivas de los profesionales y de las mujeres, de tipo tanto tradicionales como abiertas en referencia al campo de la atención. Conclusiones: Las intervenciones sobre esta violencia no son aisladas, sino enmarcadas en un contexto de discusión, donde las posiciones discursivas de los actores pueden definirlas y concretarlas. Abstract Objective: The aim is to identify the discursive positions of different actors about obstetric violence and its relationship with the social field of pregnancy, delivery and puerperium care, produced by newspapers from Argentina, Chile, Colombia, Mexico and Venezuela from 2010 to March 2017. Methodology: Qualitative study, of an exploratory nature using the discourse analysis approach of 120 journalistic notes. Results: Discursive positions of professionals and women were identified, both traditional and open visions, in reference to healthcare. Conclusions: The interventions on this violence are not isolated, but framed in a context of discussion, where the discursive positions of the actors can be defined and concretized. Resumo Objetivo: O objetivo é identificar as posições discursivas de diferentes atores sobre a violência obstétrica e sua relação com o campo social da gravidez, parto e puerpério, produzido por jornais da Argentina, Chile, Colômbia, México e Venezuela entre os 2010 e março de 2017. Metodologia: Estudo qualitativo, de caráter exploratório, tipo análise discursiva de 120 notas jornalísticas. Resultados: Foram identificadas posições discursivas de profissionais e mulheres, tradicionais e abertas em referência ao campo do cuidado. Conclusões: As intervenções sobre essa violência não são isoladas, mas enquadradas em um contexto de discussão, onde as posições discursivas dos atores podem defini-las e concretizá-las.

Al mismo tiempo que la gineco-obstetricia se feminiza, aumenta la problematización de la violencia obstétrica. Es a partir de estos dos fenómenos que el artículo se propone describir las diferentes posturas que los actores han tomado en torno de la introducción de prácticas de ‘parto humanizado’ – clasificadas como ‘abiertas’, ‘intermedias’ y de ‘resistencia’–, en tanto y en cuanto éstas buscan disminuir la violencia obstétrica. En especial, se subrayan las explicaciones y reacciones de las mujeres gineco-obstetras ante la violencia obstétrica. El análisis busca mostrar cómo el género como construcción y la antropología como método y enfoque contribuyen a entender el fenómeno y hacer inteligible esta aparente paradoja. Como conclusión, se plantea que establecer relaciones entre la feminización de la ob-gyn y la problematización de la violencia obstétrica nos permiten una entrada a la comprensión de los alcances y limitaciones del proceso de feminización de la profesión médica., While the ob-gyn becomes feminized, the problematization of obstetric violence increases. From these two phenomena, the article aims to describe the different positions the actors have taken on the introduction of humanized birth. These positions are 'open', 'intermediate' and 'resistance' and aim to reduce the obstetrics violence. In particular, we emphasize the explanations and reactions of the ob-gyn women when they have to face violence obstetric. The analysis seeks to show how gender and anthropology construction as a method and approach contribute to understand the phenomenon and to make intelligible this apparent paradox. In conclusion, we propose that to establish relations between the feminization of the ob-gyn and the problematization of the obstetric violence allow us an entry to understand the scope and limitations of the process of feminization of the medical profession., Facultad de Psicología

En el presente artículo se presentan dos casos de violencia obstétrica ocurridos en Barcelona entre los años 2016-2017 en el mismo hospital: un centro de primer nivel concertado en la comunidad autónoma de Cataluña. Los casos presentados resultan instrumentales porque ilustran claramente el fenómeno de la violencia obstétrica padecida dentro de condicionantes de género y colectivos, en tanto que abordan más de un modelo y con ello se desea aportar un entendimiento profundo de casos similares ocurridos en los últimos años.El objetivo del presente artículo consiste en analizar los casos referidos para poder destacar los derechos de autonomía y consentimiento informado en el embarazo y el parto desde una perspectiva de interseccionalidad de género. A lo largo del texto, se reflexiona sobre cómo la trivialización producida por el modelo ontológico individualista actual ha generado una profunda confusión entre el deseo y el derecho, reproduciendo procesos tayloristas del parto. En dichos procesos se confunde la violencia con altruismo, la posibilidad de tener hijas/os con la adquisición de ellas/os, la salud con la mercancía. Por esto, se quiere remarcar la importancia de los condicionantes de género y sus intersecciones, para adoptar una mirada desde la complejidad y la profundidad, así como definir y comprender la violencia obstétrica para transformar la situación actual., In this article we present two cases of obstetric violence that occurred in Barcelona between the years 2016-2017 in the same hospital: a first level subsidised center in the autonomous community of Catalonia. The cases presented are instrumental because they clearly illustrate the phenomenon of obstetric violence suffered within gender and collective conditions as they address more than one model and, because with them, we might be able to provide an in-depth understanding of similar cases that have occurred in recent years. Thus, the aim of this article is to analyze said cases in order to highlight the rights of autonomy and informed consent in pregnancy and childbirth from a gender intersectional perspective. Throughout the text we reflect on how the trivialization produced by the current individualistic ontological model has generated a deep confusion between desire and right, reproducing taylorist processes of childbirth. In these processes violence is confused with altruism, the possibility of having children with the acquisition of them, health with merchandise. We want to highlight the importance of gender determinants and intersections to adopt a complex view and to define and understand obstetric violence in order to transform the current situation.

IntroducciónLa violencia obstétrica (VO), es el tema central de este trabajo. Se considera VO a las diferentes formas de control y opresión sobre las mujeres que están embarazadas o que se encuentran en trabajo de parto. Dado que la VO se ejerce con el fin de controlar y oprimir a las mujeres, podemos afirmar que es otra forma de violencia de género y también se considera violencia institucional y simbólica.ObjetivoEl objetivo de este estudio es conocer si las mujeres que paren en los hospitales de la Isla de Tenerife sufren alguna forma de violencia obstétrica y cómo viven esa experiencia.MetodologíaSe trata de un estudio observacional descriptivo. Como instrumento de recogida de datos se utilizó una encuesta de elaboración propia basada en un Test de VO de diversas organizaciones argentinas. Distribuido por redes sociales (Facebook y WhatsApp), fue contestado por un total de 282 mujeres cuyos partos habían tenido lugar entre enero de 2008 y febrero de 2016.ResultadosUn 56,4% de la muestra sufrió VO durante el parto, siendo los indicadores más frecuentes la participación de alumnado en prácticas sin el consentimiento de la mujer y el cambio de ritmo en el parto por conveniencia del personal sanitario.ConclusionesMuchas veces la VO se genera debido a la alta tecnificación y la concepción del parto como un proceso en cadena, que lo despersonaliza y desnaturaliza. Esto provoca que las mujeres no sean tratadas en su globalidad sino como enfermas a las que hay que someter a procedimientos médico-quirúrgicos., IntroductionThe subject of this study is Obstetric Violence (OV). OV is understood as the different ways in which pregnant women, or those who are going into labor, are controlled and oppressed. OV may be considered as another type of gender-based violence because its aim is to control and oppress women. Additionally, OV could be considered as institutional and symbolic violence.ObjectivesThe aim of this study is to discover whether new mothers in Tenerife are being subject to Obstetric Violence and to bring to light the way in which they experience it.Materials and methodsThis is a descriptive study. The instrument for collecting data was a survey based on an OV test from several Argentinian organizations. Facebook and WhatsApp were used for collecting the data. The survey was completed by 282 women who had given birth between January 2008 and February 2016.ResultsThe results show that 56.4% of the sample was subject to OV during childbirth. The most important indicators were the involvement of students without the mother’s consent and the change of the pace in childbirth for the convenience of the healthcare staff.ConclusionsOV often occurs because childbirth is technologically-oriented and understood as an assembly line-like process, which depersonalizes and denaturalizes childbirth. This results in women not being viewed holistically but treated as patients in need of medical and surgical treatment.
