Background/aim: Authors widely use pulse oximetry in clinical monitoring of heart rate (HR) and peripheral oxygen saturation (SpO2) by attachment to the fingers; however, there can be a need for an alternative attachment site, especially for burned patients. We investigate the availability of a pulse oximeter probe attached to the penile shaft as an alternative site in pediatric male patients if all extremities became unavailable for pulse oximetry measurement due to severe burn and/or trauma. Materials and methods: We designed a prospective comparative study in a training and research hospital. After local ethical committee approval, pediatric male cases eligible for penile and extremity pulse measurements were evaluated during general anesthesia for medical dressing and/or grafting due to severe burns. One probe was attached to the fingers of the unburned extremity, and the other was to the penile shaft. Furthermore, we recorded SpO2 and HR values at 5-min intervals; 0th (baseline), 5th, 10th and 15th minutes. We compared HR and SpO2 values measured by the finger probe with those measured by the penile probe. Results: Data of 51 patients (median age, 2.9 years (interquartile range, 2.0-5.0 years)) in whom the duration of dressing was at least 15 min were analyzed. There was no significant difference either in comparisons of hemodynamic measurements (HR and SpO2) obtained by finger probe and by a penile probe for each measurement time. The Bland-Altman plot analysis reveals agreement for penile and finger probes with a mean bias value between 0.20 and 0.37 on HR and between 0.43 and -0.20 on SpO2. Conclusion: This clinical trial demonstrated that pulse oximetry measurement under nonhypoxic conditions we could perform confidently using penile probes in pediatric male patients whose extremities are unavailable for measurement. Objective: This study aims to evaluate outcomes of redo orchiopexy and the effect of redo surgery timing upon testicular volume. Materials and Methods: This prospective study involved children receiving redo orchiopexy for recurrent undescended testis. Patients were recruited to assess testicular position, volume, blood flow and presence of microlithiasis. Testis volume was measured by ultrasound and compared with recently developed normative values for testicular size. Results: A total of 38 patients (40 testes) required redo orchiopexy were reviewed in the study. Thirty three of invited boys could be investigated as long term participation. As a result of long term follow up; 28 of the testes were at scrotum, 2 of them were at inguinal canal and 3 of them were non-palpable, with a 15% failure rate of redo orchiopexy. For all patients evaluated in the control visit mean testis volume was 1.23 mL, at 24 of whom were significantly smaller than the normative values for the same age (p<0.001). Eleven of the testes (33.3%) had microlithiasis. The average of duration between primary and redo orchiopexy was 13.5 months in the group of normal volume testes, 23.3 months in the group of significantly smaller testes (p=0.056). Conclusion: The long-term volumes of testes after redo orchiopexy were significantly less than the normative values. Frequent and long time follow up of operated undescended testes and early intervention of recurrent cases may improve outcomes of surgery. [ABSTRACT FROM AUTHOR]

Objective: Percutaneous insertion of permanent tunneled catheters, accompanied by ultrasonography (USG) guidance, has established itself as practical, safe, and widely used procedure. The purpose of our study; To examine and compare the complications associated with permanent tunneled catheters placed by percutaneous or open surgical method in pediatric hemato-oncology patients. Methods: Medical records of 101 pediatric patients from the ASA 3-4 group who were placed in a Hickman-type tunneled central venous catheter for bone marrow transplantation between 2013 and 2015 in our clinic were retrospectively reviewed. Demographic data of the patients, catheter insertion location, size, complications encountered during and after the intervention, and reasons for catheter removal were recorded. Results: One hundred and one tunneled central venous catheters were placed percutaneously under USG guidance in 54 patients, and using open technique in 47 patients. Patients in both groups were similar in terms of age, height, weight, size of the inserted catheter, duration of catheterization, and reasons for removal. No difference was found between percutaneous and surgical groups in terms of intraoperative and postoperative to technique, and infective complications related to catheter. Conclusion: In pediatric hemato-oncology patients, there is no difference in terms of intraoperative, and postoperative complications between percutaneous technique and open technique for permanent tunneled central catheter insertion. Both techniques can be used with low complication rates, and the percutaneous technique may be preferred because it is less invasive. [ABSTRACT FROM AUTHOR]
