Purpose: We examined the effect of intracameral administration of cefuroxime on oxidative stress and endothelial apoptosis in rat corneal tissue., Methods: In total, 30 rats were divided into 3 groups of 10 rats each (intracameral administration of cefuroxime 0.1 mg/0.01 mL (cefuroxime group); intracameral administration of balanced salt solution 0.01 mL (control group); or absence of intracameral injection (sham group). Corneal endothelial apoptosis was assessed by immunohistochemical analysis using caspase-3 and caspase-8. Total oxidant status, total antioxidant status, oxidative stress index, and paraoxonase and arylesterase levels were examined in corneal endothelial tissue and serum., Results: Paraoxonase levels in the serum were significantly different between the sham and cefuroxime groups (p=0.027). A significant difference was also observed in total oxidant status levels between the cefuroxime and balanced salt solution groups (p=0.023). In addition, there were significant differences in total antioxidant status levels in corneal tissue between the cefuroxime and sham groups (p<0.001) and between the cefuroxime and balanced salt solution groups (p<0.001). Furthermore, significant differences were also observed in oxidative stress index levels between the cefuroxime and balanced salt solution groups (p=0.001) and between the cefuroxime and sham groups (p=0.026). According to the immunohistochemical staining results, a significant association with caspase-3 activity existed between the cefuroxime and balanced salt solution groups (p=0.007), while no significant difference was found with caspase-8 activity (p=0.541). Caspase-3 activity exhibited a significant relationship between the sham and balanced salt solution groups (p=0.018), but no relationship was found with caspase-8 activity (p=0.623)., Conclusion: Immunohistochemical examination revealed that intracameral cefuroxime increased apoptosis when compared to the sham and balanced salt solution groups. Moreover, intracameral cefuroxime increased oxidative stress in the cornea and simultaneously induced apoptosis.

Accurate and repeatable measurement of the central cornea thickness (CCT) is important in terms of follow-up of glaucoma patients. The aim of this study was to compare CCT measurements made with an ultrasonic pachymeter (USP) and with the Spectralis Anterior Segment Module optical coherence tomography (OCT) device. The study included 52 eyes of 26 primary open-angle glaucoma patients. CCT measurements were taken with the USP device and then with the OCT by imaging cornea slices. The mean CCT was measured as 555.71 ± 47.76 μm with USP and 569.46 ± 49.5 μm with OCT. Despite the differences between the measurements of the two methods, a very strong degree of consistency was seen between the CCT measurement with USP and OCT (ICC = 0.93, p < 0.001).

Purpose: To compare postoperative corneal nerve fiber regeneration after small incision lenticule extraction (SMILE) and femtosecond laser-assisted LASIK (FS-LASIK)., Methods: The medical records of patients who underwent SMILE were retrospectively reviewed. Patients with SMILE in one eye (the SMILE group) and FS-LASIK in the fellow eye (the FS-LASIK group) and who had undergone a confocal microscopy study were retrospectively identified. Subbasal nerve fiber density was measured and compared between the groups preoperatively and at 1 week and 1, 3, and 6 months postoperatively., Results: Thirty eyes of 15 patients were included in the study. Preoperative characteristics of the eyes were similar between the groups. However, there was a statistically significant difference in subbasal nerve fiber density between the groups at 1 month (SMILE group: 1,505 ± 627 µm/mm(2) [range: 625 to 2,540 µm/mm(2)], FS-LASIK group: 1,107 ± 509 µm/mm(2) [range: 595 to 2,313 µm/mm(2)], P = .003) and 3 months (SMILE group: 1,534 ± 503 µm/mm(2) [range: 650 to 2,255 µm/mm(2)], FS-LASIK group: 1,194 ± 485 µm/mm(2) [range: 530 to 1,695 µm/mm(2)], P = .004) postoperatively. No statistically significant difference was detected at 1 week and 6 months postoperatively., Conclusion: Eyes treated with SMILE have a higher density of subbasal nerve fibers than eyes treated with FS-LASIK in the early postoperative period (1 week to 3 months) but the two procedures are equivalent by 6 months postoperatively., (Copyright 2015, SLACK Incorporated.)

Purpose: To compare the postoperative corneal hysteresis (CH) and corneal resistance factor (CRF) of eyes implanted with a small aperture corneal inlay versus fellow eyes., Methods: Medical records of patients who underwent small aperture corneal inlay (KAMRA; AcuFocus, Inc., Irvine, CA) implantation were retrospectively reviewed. There were two groups: the implanted and non-implanted. Main outcome measures were CH, CRF, Goldmann-correlated intraocular pressure (IOPg), corneal-compensated IOP (IOPcc), and Goldmann applanation tonometry measurements performed preoperatively and at postoperative week 1 and months 1, 3, and 6., Results: The study included 68 eyes of 34 patients. CH was higher in the implanted group when compared with the non-implanted group at postoperative week 1 (12.2 ± 3.1 vs 10.9 ± 1.7 mm Hg; P = .007) and month 1 (12.3 ± 2.5 vs 10.9 ± 1.8 mm Hg; P = .001). CRF was higher in the implanted group when compared with the non-implanted group at postoperative week 1 (11.9 ± 2.9 vs 10.7 ± 1.6 mm Hg; P = .003) and month 1 (12.5 ± 2.5 vs 10.4 ± 1.8 mm Hg; P < .001). IOPcc was higher in the implanted group when compared with the non-implanted group at only the first postoperative month (17.1 ± 3.5 vs 14.3 ± 2.9 mm Hg; P < .001). There were no statistically significant differences in Goldmann applanation tonometry or IOPg measurements between the two groups (P > .05). At the 3-month postoperative visit, all parameters had returned to baseline and there was no change at the 6-month visit., Conclusions: Implantation of the KAMRA corneal inlay does not induce a permanent change in CH or CRF. Purpose: To compare corneal hysteresis (CH) and corneal resistance factor (CRF) between eyes treated with small incision lenticule extraction (SMILE) and femtosecond laser-assisted laser in situ keratomileusis (femto-LASIK)., Setting: Beyoğlu Eye Training and Research Hospital., Design: Prospective comparative case series., Methods: Sixty eyes from 30 patients with bilateral myopia or myopic astigmatism were studied. Inclusion criteria were spherical equivalent of subjective manifest refraction (SE) <10diopters (D) and a difference ≤0.50D between the SEs of both eyes. One eye of each patient was treated with SMILE, and the fellow eye underwent femto-LASIK. Randomization was performed using a sealed envelope system. The main outcome measures were CH and CRF measured preoperatively and postoperatively (1 and 6 months)., Results: Preoperative SE was similar in both groups (p=0.852). CH and CRF values were reduced postoperatively in both groups compared to their corresponding preoperative values (p<0.001). At the 6-month follow-up visit, the mean CH values in the SMILE and femto-LASIK groups were 8.95±1.47 and 9.02±1.27, respectively (p=0.852), and the mean CRF values were 7.77±1.37 and 8.07±1.26, respectively (p=0.380)., Conclusion: CH and CRF decreased after SMILE. Purpose: To compare effects of small-incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (F-LASIK) on corneal sensation and dry eye parameters., Methods: In this prospective, randomized, contralateral-eye study, 28 patients with myopia or myopic astigmatism in both eyes were enrolled. One eye of each patient was treated by SMILE, and the fellow eye was treated by F-LASIK. Mean outcome measures Cochet-Bonnet esthesiometry, Schirmer test with anesthesia, tear breakup time, and tear film osmolarity were evaluated preoperatively as well as 1 week, 1 month, 3 months, and 6 months after surgery., Results: Corneal sensation was reduced after both SMILE and F-LASIK surgeries 1 week, 1 month, and 3 months after surgery (p < 0.05 in both groups). Corneal sensation was significantly lower after F-LASIK than after SMILE at postoperative 1 week, 1 month, and 3 months (p < 0.01 for all points). Dry eye parameters such as tear breakup time, Schirmer test, and tear film osmolarity did not change significantly in any of the procedures and were not significantly different between the procedures at any follow-up visits., Conclusions: The findings of the study revealed that, although both F-LASIK and SMILE procedures changed the corneal sensation 1 week, 1 month, and 3 months after surgery, they did not affect the dry eye parameters at any point.

Purpose: To assess the safety, predictability, and stability of a new microwave thermokeratoplasty procedure to correct myopia., Setting: Cornea and refractive surgery subspecialty., Design: Prospective clinical trial., Method: Thermokeratoplasty was performed in myopic eyes at a single center in Turkey from June 2009 to June 2010. The attempted corrections ranged from -1.25 to -5.75 diopters (D). The main outcome measures were changes in logMAR uncorrected distance visual acuity (UDVA) and in keratometry (K) values., Results: The procedure was performed in 33 eyes (patients aged 20 to 45 years). The mean preoperative logMAR UDVA (0.76 ± 0.24 [SD]) significantly improved to 0.19 ± 0.20 at 1 month, postoperatively. By 3 months, the mean UDVA had markedly regressed to 0.59 ± 0.29; however, the residual improvement remained statistically significant. At 12 months, the mean logMAR UDVA was 0.72 ± 0.26. The mean K values were 43.9 ± 1.36 D preoperatively, 41.25 ± 2.63 D at 1 month, 43.4 ± 1.69 D at 3 months, and 44.1 ± 1.09 D at 12 months. The mean endothelial cell density was 2836 ± 342 cells/mm(2) preoperatively and were statistically unchanged 12 months postoperatively (2732 ± 353 cell/mm(2)). No patient lost lines of corrected distance visual acuity by 12 months postoperatively., Conclusions: The new thermokeratoplasty procedure produced the desired reduction in myopia and improvement in postoperative UDVA 1 month postoperatively without significant side effects. Aims: To compare the biomechanical properties of the cornea and intraocular pressure (IOP) between patients with nanophthalmos and age-matched controls., Methods: In this prospective, cross-sectional and comparative study, 27 eyes of 27 healthy individuals (control group) and 27 eyes of 27 patients with nanophthalmos (study group) were enrolled. Corneal hysteresis (CH), corneal resistance factor (CRF), corneal compensated intraocular pressure (IOPcc) and Goldmann correlated intraocular pressure (IOPg) were recorded for the right eye of each participant using Reichert Ocular Response Analyser measurements. Also, all participants in this study underwent a standardised ocular examination including IOP measurement with Goldmann applanation tonometry (IOP(GAT)), central corneal thickness and axial length (AL) assessments., Results: Mean CH in the nanophthalmic eyes and in the control eyes were 13.3 ± 2.4 mm Hg and 11.6 ± 1.7 mm Hg, respectively (p=0.003); mean CRF values in the nanophthalmic and the control eyes were 13.2 ± 1.8 mm Hg and 11.4 ± 1.9 mm Hg, respectively (p=0.001). Mean IOP(GAT) was 15.2 ± 3.3 mm Hg in the nanophthalmic eyes and 13.4 ± 2.7 mm Hg in the control group (p=0.031); mean IOPg values for the nanophthalmic and the control groups were 17.1 ± 5.3 mm Hg and 14.7 ± 3.5 mm Hg, respectively (p=0.042). Mean IOPcc values in the nanophthalmic and the control group were 13.6 ± 6.1 mm Hg and 14.8 ± 3.2 mm Hg, respectively (p=0.365)., Conclusion: The CH, CRF, IOPg and IOP(GAT) were significantly higher in the nanophthalmic eyes, whereas no significant differences in IOPcc were observed. These findings may be taken into account when measuring IOP values in patients with nanophthalmos.

Purpose: To investigate biomechanical parameters of the cornea measured with ocular response analyzer (ORA) in myopic eyes with high axial length and the relationship between these parameters and axial length (AL)., Methods: A total of 165 eyes of 165 consecutive patients were included. Eyes with AL greater than 26 mm were named group 1 and eyes with AL shorter than 26 mm were named group 2. Axial length and keratometric values were measured by intraocular lens (IOL) Master optical biometry. Metrics of corneal biomechanical properties, including corneal hysteresis (CH) and corneal resistance factor (CRF), were measured with the ORA. The ORA also determined the values of intraocular pressure (IOPg) and corneal compensated IOP (IOPcc)., Results: The mean age of total subjects was 43.0 ± 15.6 years. Eighty-three eyes were included in group 1; 82 eyes were included in group 2. The CH and CRF of group 1 were significantly lower than group 2. The IOPcc was significantly higher in group 1 than group 2. When group 1 and group 2 were combined for analysis, CH was negatively correlated with age. Both CH and CRF were significantly correlated with SE. However, CH and CRF were negatively correlated with AL. Also, there was significant correlation between AL and IOPcc (p<0.05)., Conclusions: Highly myopic eyes showed decreased CH and CRF. As the AL increased the IOPcc also increased and the CH decreased. We conclude that the biomechanical properties of the cornea change with elongation of the eye and this may have an impact on IOP measurement.

Purpose: To evaluate the agreement among three different optical methods in measuring anterior chamber depth (ACD), central corneal thickness (CCT), and pupil diameter., Methods: One hundred eyes of 50 healthy patients (25 men, 25 women) were enrolled in the study. Mean patient age was 25 years (range: 21 to 32 years). Exclusion criteria were history of any intraocular or corneal surgery, contact lens wear, corneal anomalies, and spherical refraction >5.00 diopters (D) or cylindrical refraction >2.00 D. All measurements were done by the same operator under mesopic light conditions and repeated using three different optical methods: Visante optical coherence tomography (OCT) (Carl Zeiss Meditec), Orbscan (Bausch & Lomb), and Pentacam (Oculus Optikgeräte GmbH)., Results: Mean CCT as measured by Visante OCT, Orbscan, and Pentacam was 529+/-30.5 microm, 554+/-32.7 microm, and 552+/-29.3 microm, respectively. Mean ACD values were 2.94+/-0.34 mm, 2.84+/-0.33 mm, and 2.98+/-0.33 mm, respectively. Mean pupil diameter measurements were 4.87+/-1.09 mm, 4.0+/-0.67 mm, and 3.05+/-0.59 mm, respectively. The Visante OCT measured CCT thinner and Orbscan measured ACD shallower than the other two methods. All three methods measured pupil diameters significantly different., Conclusions: This study found some statistically significant but clinically insignificant differences among the optical methods assessed. The differences are small and do not influence decisions for refractive surgery in clinical practice.

Purpose: To evaluate the corneal topographic characteristics of first-degree relatives of patients with keratoconus with corneal topography to determine the incidence of clinical keratoconus and topographic abnormalities., Methods: Between February and August 2006, Orbscan II analysis was done in 144 eyes of 72 cases who were first-degree relatives of patients diagnosed with clinical keratoconus. The findings were compared with preoperative Orbscan analyses of 52 clinically normal individuals who underwent laser in situ keratomileusis surgery and did not develop corneal ectasia in 3 years of follow-up., Results: In 8 of the 72 first-degree relatives of patients with keratoconus, clinical keratoconus was diagnosed by the topographic pattern in Orbscan and clinical examination (group 1). The remaining 64 subjects (group 2) were compared with the control group (group 3). The central corneal thickness was 523.7 +/- 40.4 microm in group 2, whereas it was 546.3 +/- 33.1 microm in group 3 (P < 0.05). The central corneal thickness, thinnest pachymetric reading, posterior elevation value, distance between the greatest anterior/posterior elevation points, and corneal center, posterior best fit sphere (BFS) values, posterior BFS:anterior BFS ratio, and irregularity values were significantly different between group 2 and group 3 (P < 0.05)., Conclusions: The keratoconus incidence was found to be 11% in first-degree relatives of patients with keratoconus as opposed to a reported incidence of keratoconus of 0.05% in the general population. In first-degree relatives of patients with keratoconus who did not have a topographic keratoconus pattern, abnormal corneal topographic values were detected. The asymptomatic relatives of patients with keratoconus should undergo a thorough preoperative analysis for subtle topographic abnormalities before any keratorefractive surgery.

Purpose: : To analyze the differences in corneal topography in keratoconus patients with and without atopy., Methods: : In the Beyoglu Eye Research and Training Hospital Refractive Surgery Department, corneal examination with the Orbscan II device (Bausch & Lomb) was done to all patients between May and June 2004 who were diagnosed as keratoconus with corneal topography findings and who did not have any surgical procedure. The atopy grading was done according to the criteria of the UK Atopy Study Group (1994). Patients were divided into 2 groups according to the presence of atopy. The Orbscan findings of the 2 groups were compared., Results: : Among 70 keratoconus patients, 33 patients had partial or full atopy. Sixty-six eyes of the 33 patients were classified as the atopic group, whereas 74 eyes of the remaining 37 patients were classified as the control group. The mean age of the patients was 27.7 +/- 9.5 years in the atopic group and 26.9 +/- 9.5 years in the control group. The central corneal thickness was 428.5 +/- 37.1 mum in the atopic group and 491.8 +/- 56.3 mum in the control group. There was a statistically significant difference between the mean central and thinnest corneal pachymetric readings, the distance of thinnest point to corneal center, anterior and posterior elevation values, distances of the anterior and posterior elevation to the central cornea, and irregularity indexes in the 2 groups (P < 0.05)., Conclusions: : This study shows that corneal topographic and pachymetric characteristics of keratoconus may be different in atopic eyes from those in nonatopic eyes. Atopic keratoconus patients could possibly be evaluated as a separate entity in keratoconus disease.

Purpose: To compare visual and refractive results, contrast sensitivity, and tear stability after laser in situ keratomileusis (LASIK) in one eye and laser epithelial keratomileusis (LASEK) in the fellow eye for low myopia., Methods: Patients diagnosed with low myopia, with a maximum difference of 1 D between their two eyes, were randomly assigned to receive LASEK on one eye and LASIK on the other eye. A total of 64 eyes of 32 patients with a mean age of 26.83 +/- 5.33 years were included in the study. Preoperative myopia ranged from -1.00 to -6.00 D. Follow-up was 6 to 12 months. Uncorrected (UCVA) and best spectacle-corrected visual acuity (BSCVA), Schirmer test results, tear break-up time, corneal asphericity, corneal uniformity index, predicted corneal acuity, and contrast sensitivity values were compared with preoperative values. A Wilcoxon test was used for statistical comparisons and a P-value less than .05 was considered significant., Results: At 6 months after surgery, there was no statistically significant difference in UCVA, BSCVA, spherical and cylindrical refractive error, Schirmer test, or tear break-up time between groups. Contrast sensitivity values in the LASIK eyes were lower in comparison to preoperative values, but there was no change in the LASEK group., Conclusions: Based on 6-month results, LASEK for low myopia was safe and effective with predictable results, offered early refractive stability, and may be considered an alternative for LASIK.

Purpose: To investigate the influence of uneventful phacoemulsification on anterior chamber depth (ACD), iridocorneal angle (ICA) width, and intraocular pressure (IOP) in nonglaucomatous eyes with open ICA preoperatively., Setting: Beyoglu Eye Education and Research Hospital, Istanbul, Turkey., Methods: Fifty-three eyes of 49 patients were evaluated for 6 months postoperatively. The nonparametric Wilcoxon signed rank test was used to compare preoperative and postoperative IOP, ACD, and ICA width. Univariate and multivariate regression analyses were used to evaluate the relationships between IOP, ACD, and ICA width and between preoperative patient characteristics., Results: The mean preoperative IOP of 15.1 mm Hg +/- 2.8 (SD) dropped postoperatively to 13.4 +/- 3.4 mm Hg at 1 day, 13.3 +/- 2.6 mm Hg at 1 week, 13.2 +/- 3.1 mm Hg at 1 month, 13.3 +/- 2.7 mm Hg at 3 months, and 14.1 +/- 2.5 mm Hg at 6 months (P<.05). The mean preoperative ICA grade of 2.97 +/- 0.72 increased to 3.55 +/- 0.48 at 1 week and 3.68 +/- 0.45 at 1 month (P<.05). The mean preoperative ACD of 3.06 +/- 0.49 mm increased to 3.57 +/- 0.47 mm at 4 weeks, 3.69 +/- 0.32 mm at 1 month, and 3.70 +/- 0.36 mm at 3 months (P<.05). The IOP decrease was not correlated with the changes in ICA width or ACD. Multiple regression analysis showed preoperative IOP was the single predictor of the postoperative IOP drop (P<.001)., Conclusions: In nonglaucomatous eyes with an open ICA preoperatively, uneventful phacoemulsification reduced IOP, increased ACD, and widened the ICA. The changes were statistically significant over 6 months.

This study aimed to investigate the effects of commercial tattoo inks used in corneal tattooing on conjunctival microbiota. This prospective case control study consisted of 125 participants divided in the following three groups: 35 patients with corneal tattoos, 40 patients with corneal leukoma, and 50 healthy subjects. Corneal tattooing was performed in all the cases in this study using a tattoo pen machine and commercial tattoo ink. A total of 500 cultures were taken from 250 eyes of 125 individuals on chocolate and sheep blood agar. Bacteriological samples were taken from the inferior eyelid conjunctiva using a sterile cotton swab. Without any contact elsewhere, the swabs were smeared on bedside chocolate agars and 5% sheep blood agar. In tattooed eyes, bacterial growth was detected in 42.9% of the chocolate and sheep blood agar samples. In other healthy eyes of patients with corneal tattoos, 54.5% bacterial growth on chocolate agar and 57.1% on sheep blood agar were detected. No statistical difference was detected in the conjunctival microbiota of chocolate and sheep blood agar (p = 0.254, p = 0.134, respectively) in the tattooed eyes compared to the other eye of the individual. No statistically significant difference was found in terms of bacterial growth in tattooed, leukoma, or healthy eyes on chocolate and sheep blood agar (p = 0.408, p = 0.349). The growth rate of Staphylococcus epidermidis decreased by 33.3% (from 12 to 8) on chocolate agar in 35 tattooed eyes, and it decreased by 28.5% (from 14 to 10) on sheep blood agar, while gram-negative bacteria Brevundimonas diminuta, Acinetobacter lwoffii, and Psychrobacter faecalis were detected in three patients. Corneal tattooing using commercial dye does not affect conjunctival microbiota. In the past 3 years, 120 patients have been tattooed with commercial tattoo ink in Istanbul Medeniyet University Göztepe Training and Research Hospital. No complications related to infection were found in the 3-year follow-up. The gram-negative bacteria detected in the healthy control group and tattooed eyes were bacteria found on normal skin or in the respiratory tract. Although some gram-negative bacteria do not cause infection, careful eye examination, follow-up, and culture are required in suspicious cases. [ABSTRACT FROM AUTHOR]

Purpose: This article aims to present the corneal tattooing method and how using a tattoo pen machine can improve aesthetic appearance in patients with corneal leukoma. Methods: In this study, 42 patients were evaluated who had no visual potential and who had undergone colored corneal tattooing using an automatic tattoo pen machine for aesthetic purposes. The procedure was conducted according to the principles of the Declaration of Helsinki. The commercially available tattoo ink that has traditionally been used on human skin (brown, green, and black) for years was used for all the patients in this study, and 252 corneal photographs (with a Topcon slit lamp imaging device at 16 magnifications, i.e., 16 ×) taken within the last 2 years were evaluated retrospectively. Red, green, and blue (RGB) and hue, saturation, and lightness (HSL) values of the tattooed areas, such as pupils and iris, in corneal photographs were determined online using the Color Code Finder program. The RGB and HSL values of the pupil and iris were compared before surgery on the first day and first week, first month, third month, and twelfth month after surgery. Results: In the first postoperative month, the mean pupil lightness (L) and iris L values were found to have increased by 10.7% and 5.7%, respectively. Between the first month and the first year, the L value of the mean pupil and that of the iris increased by 1.7% and 5.2%, respectively. The increase in the RGB value of the mean pupil in the first month was statistically significant (p = 0.02). The highest increase in RGB values of the iris was observed in the first week and first month (p = 0.113). This result shows that the majority of fading occurred in the first month. After the first month, the increase in the L value in the black-colored pupil was less than that in the brown- or green-colored iris. These results show that light colors fade faster and more. Conclusion: Esthetically, corneal leukoma causes severe psychological problems. Many patients are unable to use prosthetic contact lenses. Evisceration surgery has many complications, and limbal stem cells are used in evisceration surgery. Corneal tattooing using a tattoo pen machine is an easy, practical, and repeatable method used for aesthetic purposes. Successful results require the use of appropriate methods, ink, and ophthalmologist's experience. All patients in this study had a more aesthetic appearance than the preoperative white eye. Purpose: Our study aims to investigate the effects of intracameral carbachol on oxidative stress and apoptosis in rat corneas. Methods: In this experimental study, 28 Wistar albino male rats were used. There were 9 rats in the carbachol group, 9 in the balanced salt solution (BSS) group, and 10 in the sham group. The carbachol group was injected with 0.01 cc of carbachol, and 0.01 cc of BSS was injected intracamerally into the BSS group. No drug was injected into the sham group, but the anterior chamber was entered with an empty syringe. Blood serum samples and corneas of rats were taken 1 week later. Caspase-3 and caspase-8 were immunohistochemically examined in rat corneal endothelial to investigate apoptosis. To determine oxidative stress in corneal endothelial tissue and serum, total antioxidant status (TAS), total oxidant stress (TOS), oxidative stress index (OSI), arylesterase (ARES), and paraoxonase (PON) levels were measured. Results: The mean OSI values in the rat serum of the carbachol group were significantly lower than those of the sham and BSS groups (P = 0.05 and P = 0.004, respectively). The mean TOS value of the carbachol group’s rat serum was significantly lower than the mean of the BSS group (P = 0.001). There was no significant difference in the mean TAS, TOS, OSI, ARES, and PON levels from the rat cornea of the carbachol, BSS, and sham groups (P > 0.05). All corneas in the carbachol group were caspase-3 negative, and a statistically significant difference was found among the sham, BSS, and carbachol groups (P = 0.002). No significant difference was observed among the sham, BSS, and carbachol corneas in caspase-8 staining (P = 0.094). Conclusion: Our study demonstrates that intracameral carbachol is a safe intracameral drug. Carbachol reduced oxidative stress in rat serum compared to rats injected with BSS and the sham group. In addition, carbachol did not increase oxidative stress in rat corneas compared to the BSS and sham groups. Purpose To compare the efficacy, safety and postoperative pain of mechanical versus transepithelial photorefractive keratectomy (PRK) techniques. Setting Cornea and refractive surgery subspecialty. Design Prospective clinical trial. Methods This prospective comparative study included 84 eyes of 42 patients with myopia who received mechanical PRK (m-PRK) in 1 eye and transepithelial PRK (t-PRK) in the contralateral eye. The mean patient age was 28.5 ± 6.3 years (range 20–46 years). Postoperative uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), manifest refractions, postoperative epithelial healing time, surgical time, postoperative pain rating and corneal haze were recorded. Results At week 1, statistically the UDVA was significantly better in the t-PRK eyes; however, at 3 months, similar refractive stability was achieved in both groups. The mean spherical equivalent (SE) decreased from −2.44 ± 1.00 D (m-PRK eyes) and −2.88 ± 1.24 D (t-PRK eyes) at baseline to −0.19 ± 0.38 D and −0.30 ± 0.40 D, respectively, after 1 year. Surgical time was 98.6 ± 9.8 s in m-PRK eyes and 58.0 ± 6.4 s in t-PRK eyes. On postoperative days 1 and 3, using the global assessment rating, 81% of mPRK eyes that had pain, reported more pain than that reported for the tPRK eyes. In addition, m-PRK treated eyes demonstrated higher mean pain scores based on the 11-point numerical rating scale and Visual Analogue Scale (VAS). The mean time to complete epithelial healing was 2.19 ± 0.39 days (t-PRK) and 3.76 ± 0.43 days (m-PRK). Conclusion t-PRK for mild-to-moderate myopia was more comfortable than conventional m-PRK; patients had less pain, and faster healing time. [ABSTRACT FROM AUTHOR]
