Limited data exist regarding drug-coated balloon (DCB) treatment in de novo large coronary arteries. We sought to demonstrate procedural characteristics, residual stenosis, and clinical outcomes following DCB angioplasty for de novo lesions in large versus small coronary arteries. The study included 184 consecutive patients with 223 de novo coronary lesions undergoing paclitaxel DCB angioplasty between January 2019 and August 2020, who were divided according to whether the DCB diameter was ≥ 3.0 mm (large group, n = 58) or < 3.0 mm (small group, n = 125). The large group had a higher proportion of acute coronary syndrome more commonly with ostial, bifurcation, and calcified lesions in large vessels and received lesion preparation with more frequent use of scoring or cutting balloons and atherectomy devices compared to the small group. Postprocedural angiographic diameter stenosis was smaller in the large group compared to the small group (31% [22-37] vs. 35% [26-42], p = 0.032), and intravascular ultrasound revealed no significant difference in postprocedural area stenosis between the groups (66.2 ± 7.7% vs. 67.9 ± 7.8%; p = 0.26). The median follow-up duration was 995 days. The incidence of a composite of all-cause death, myocardial infarction, stroke, or target lesion revascularization was similar between the groups (log-rank p = 0.41) and was influenced by the presence of acute coronary syndrome and anemia but not by DCB diameter. The rate of cardiovascular outcomes after DCB treatment was comparable in de novo large and small coronary arteries. Background: There is a paucity of data regarding the optimal duration of drug-coated balloon (DCB) inflation for coronary lesions. We sought to explore the effect of DCB angioplasty with versus without long inflation time on residual stenosis and clinical outcomes in patients with coronary artery disease., Methods: This study included 314 consecutive patients with 445 lesions undergoing paclitaxel DCB angioplasty using different inflation time, divided according to whether the total inflation time of the DCB was ≥180 s (prolonged group) or <180 s (standard group). The primary clinical endpoint, defined as a composite of all-cause death, myocardial infarction, stroke, or target lesion revascularization, was examined in 92 propensity score matched pairs., Results: In the matched cohort, the median clinical follow-up period was 947 days. Postprocedural angiographic diameter stenosis was smaller in the prolonged group than in the standard group (30.0% [22.0-37.0] vs. 33.5% [25.5-40.5]; p = 0.042). Intravascular ultrasound measurements revealed that longer DCB inflation time resulted in smaller area stenosis (66.6 ± 7.8% vs. 69.4 ± 7.0%; p = 0.044) and a less mean increase in percent atheroma volume (-11.2 ± 7.1% vs. -7.4 ± 5.9%; p = 0.004) after angioplasty. The rate of the primary endpoint was lower in the prolonged group than in the standard group (log-rank p = 0.025). The efficacy of prolonged DCB inflation was prominent in patients with in-stent restenosis and longer lesions., Conclusion: Prolonged DCB inflation was associated with reduced residual stenosis and improved clinical outcomes in patients with coronary artery disease undergoing percutaneous coronary intervention. Background: Although real-time three-dimensional echocardiography (RT3DE) is useful for delineating the extent and severity of stress-induced wall motion abnormalities during dobutamine stress echocardiography (DSE), it also provides simultaneous multiple two-dimensional cut planes, which may potentially improve the detection of stress-induced wall motion abnormalities. The aim of this study was to determine the comparative diagnostic accuracy of RT3DE in multiplane and multislice modes for the diagnosis of coronary artery disease (CAD) during DSE against coronary angiography reference., Methods: Multiplane and multislice cut planes (3 V, GE Healthcare, Milwaukee, WI) at rest and peak dose of dobutamine were acquired in 71 patients with known or suspected CAD. In multiplane mode, matrix array transducer allowed the simultaneous visualization of parasternal long and short-axis views or apical 4-, 2-, and 3-chamber views. From full-volume datasets, 9 equidistant 2-dimensional short-axis images from LV base to apex were extracted and simultaneously displayed (multislice mode). Visual assessment of regional wall motion was performed. Coronary angiography was performed within 48 hours and used as a reference., Results: Abnormal findings (new or worsened wall motion abnormalities or fixed wall motion abnormalities) were noted in 36 patients by multiplane mode and 28 patients by multislice mode. Coronary angiography showed significant stenosis in 32 of 71 patients in 49 of 213 coronary arteries. On a patient basis, sensitivity was not different, but specificity was significantly higher in multislice mode (95%) compared with multiplane mode (77%, P < .05). Diagnostic accuracy for detecting right CAD was also significantly higher in multislice mode (93% vs 80%, P < .05)., Conclusion: Assessment of LV wall motion from multiple short-axis slices extracted from full-volume RT3DE datasets improves the diagnosis of CAD and is thus a useful addition to DSE tools.

Although the application of intravenous contrast agents during stress echocardiography has been shown to improve diagnostic accuracy for detecting coronary artery disease, less information exists regarding its prognostic value. The aim of this study was to determine the role of contrast-enhanced dobutamine stress echocardiography (DSE) for predicting future cardiac events in patients with coronary artery disease (CAD). We studied 893 patients (mean age: 66, 581 men) with known or suspected CAD undergoing contrast-enhanced DSE. Positivity was defined as new/worsened wall motion abnormality or fixed abnormality during stress. All patients were followed for 15 +/- 10 months to evaluate hard cardiac events (cardiac death and nonfatal myocardial infarction) and total cardiac events (hard cardiac events, congestive heart failure, unstable angina, and late revascularization). Three patients were lost to follow-up, and 128 patients developed cardiac events, including 21 hard cardiac events. The 3-year event free survival rate was significantly lower in patients with positive DSE results than in those with negative DSE results. Stepwise Cox multivariate analysis revealed that positivity of DSE (P < 0.0001, Hazard ratio (HR): 2.48) and peak wall motion score index (WMSI) >1.5 (P < 0.0001, HR: 2.41) were independent predictors for total cardiac events. Considering hard cardiac events, the independent predictors were peak WMSI > 1.5 (P < 0.0001, HR: 6.65) and age > 70 years (P < 0.005, HR: 3.27). We conclude that contrast-enhanced DSE provides important prognostic information for future cardiac events.

Atherosclerosis is a diffuse process and can be evaluated in the distal part of the left anterior descending coronary artery (LAD). We evaluated whether 2-dimensional high-frequency transthoracic echocardiography could detect changes in coronary wall thickness and size associated with coronary risk factors. In all, 312 patients without a history of coronary artery disease and 40 patients with LAD disease proven by coronary angiography underwent transthoracic echocardiography evaluation of the distal LAD using high-frequency transducer. LAD wall thickness, luminal diameter, external diameter, and coronary flow velocity were measured, and these values were compared according to the number of coronary risk factors (hypertension, hypercholesterolemia, diabetes, smoking, and family history). The feasibility of measuring coronary thickness and coronary flow velocity was 86% and 90%, respectively. LAD wall thickness and external diameter significantly increased according to the increase in the number of risk factors. Patients with 3 or more coronary risk factors showed similar LAD wall thickness (0.82 +/- 0.2 vs 0.89 +/- 0.2 mm) and external diameter (3.4 +/- 0.8 vs 3.3 +/- 0.9 mm) compared with those obtained in patients with LAD disease. High-frequency transthoracic echocardiography is feasible to measure coronary artery wall thickness and external diameter in the distal LAD. This approach has a potential for noninvasive evaluation of the arterial remodeling and the changes in these variables after risk factor modification and intensive drug therapy.

Objectives: The study was done to determine potential utility of measuring coronary flow velocity (CFV) by transthoracic Doppler echocardiography (TTDE) during intra-aortic balloon pumping (IABP)., Background: Use of IABP has been shown to increase CFV assessed by an invasive technique. The CFV in the left anterior descending coronary artery (LAD) can be measured by TTDE., Methods: Coronary flow velocity in the distal LAD by TTDE was measured in 40 critically ill patients requiring IABP. All patients received emergency coronary angiography. Both CFV and pressure data were obtained during 1:2 balloon pumping., Results: Adequate diastolic CFV recording was obtained in all patients. The IABP decreased systolic pressure and increased diastolic pressure. Average peak diastolic flow velocity and diastolic velocity time integral was 19 +/- 11 cm/s and 7.7 +/- 4.4 cm with non-augmented beat. These values were increased significantly (61 +/- 38%, 59 +/- 35%, p < 0.001) with augmented beat. Significant correlation was noted between % diastolic pressure augmentation and % increase in diastolic CFV (r = 0.62 to 0.69, p < 0.001). There was no significant difference in flow enhancement during IABP, irrespective to the proximal LAD stenosis severity (severe stenosis: 73 +/- 70%; intermediate stenosis: 61 +/- 29%; no significant stenosis: 58 +/- 29%; p = NS, analysis of variance). By continuous recording of CFV, the optimal timing of balloon control could be adjusted to maximize flow velocity during augmentation., Conclusions: Use of TTDE can be employed in monitoring CFV augmentation during IABP. The IABP produced significant distal flow enhancement even in patients with critical proximal stenosis. This totally noninvasive approach may help to optimize the benefits of IABP for coronary flow augmentation.
