Infection by pathogenic microbes is widely hypothesized to be a risk factor for the development of neurocognitive disorders and dementia, but evidence remains limited. We analyzed the association of seropositivity to 11 common pathogens and cumulative infection burden with neurocognitive disorder (mild cognitive impairment and dementia) in a population-based cohort of 475 older individuals (mean age = 67.6 y) followed up over 3-5 years for the risk of MCI-dementia. Specific seropositivities showed a preponderance of positive trends of association with MCI-dementia, including for Plasmodium, H. pylori, and RSV (p < 0.05), as well as Chickungunya, HSV-2, CMV and EBV (p > 0.05), while HSV-1 and HHV-6 showed equivocal or no associations, and Dengue and VZV showed negative associations (p < 0.05) with MCI-dementia. High infection burden (5 + cumulated infections) was significantly associated with an increased MCI-dementia risk in comparison with low infection burden (1-3 cumulative infections), adjusted for age, sex, and education. Intriguingly, for a majority (8 of 11) of pathogens, levels of antibody titers were significantly lower in those with MCI-dementia compared to cognitive normal individuals. Based on our observations, we postulate that individuals who are unable to mount strong immunological responses to infection by diverse microorganisms, and therefore more vulnerable to infection by greater numbers of different microbial pathogens or repeated infections to the same pathogen in the course of their lifetime are more likely to develop MCI or dementia. Background: By the time Alzheimer's disease and related disorders (ADRD) are diagnosed, efficacy of treatments is limited. Preventive interventions are urgently needed.Objective: To design a randomized controlled trial to assess a novel intervention that aims to prevent ADRD in high-risk groups.Methods: We report on the rationale and describe the design of a multisite randomized controlled trial that aims to prevent ADRD in older persons with: (1) mild cognitive impairment (MCI); (2) remitted major depressive disorder (MDD) without MCI; or (3) remitted MDD with MCI.Results: PACt-MD (Prevention of Alzheimer's dementia with Cognitive remediation plus transcranial direct current stimulation in Mild cognitive impairment and Depression) is a trial that randomized 375 older participants with MCI, MDD, or MCI + MDD to cognitive remediation (CR) plus transcranial direct current stimulation (tDCS) or sham-CR + sham-tDCS for 5 days/week for 8 weeks followed by boosters for 5 days/week once every 6 months until participants progress to MCI or ADRD, or the end of the study. Between boosters, participants are asked to train on CR daily. At baseline, end of 8 weeks, and yearly from baseline, participants undergo clinical, cognitive, and functional assessments. The primary aims are to compare the efficacy of CR + tDCS versus sham + sham in preventing: 1) long-term cognitive decline; and 2) incidence of ADRD or MCI. The secondary aim is to assess for cognitive improvement after the 8-week course. We will also explore the moderating and mediating effects of several biomarkers collected from the participants.Conclusion: PACt-MD is unique in combining brain stimulation and a psychosocial intervention to prevent ADRD. Cognitive disorder is one of the most disabling complications of ischemic stroke significantly impairing the patient’s commitment to rehabilitation and a life long treatment, reducing the quality of life of the patient and his relatives. This article discusses the epidemiology, etiology, pathophysiology of post-stroke cognitive impairment. Risk factors, classification, treatment and prevention of post-stroke cognitive disorders are discussed. The data of the study ARTEMIDA devoted to the possibility of therapy of post-stroke cognitive impairment and prevention of dementia after stroke with the use of the drug Actovegin are provided.

Introduction: Alzheimer disease is presented with different neuropsychological phenotypes and it is considered that the memory disorder is its most important one; however, recent studies suggest that the dysfunctions in the executive functions can constitute very early markers of the disease. Objectives: To characterize the clinical and neuropsychological profiles that constitutes phenotypical markers of the neurocognitive disorders possible Alzheimer subtype. Methods: A descriptive and cross-sectional study of 39 sick persons with ages between 57-85 years, classified according to the DSM-5 criteria, with cognitive mild and major disorder, classified from the etiological point of view in the possible Alzheimer subtype was carried out. Multiple cognitive domains were evaluated (complex attention, memory and learning, executive functions, language, motor and perceptual functions) and a detailed evaluation of the executive functions was made by means of a group of specific neuropsychological tests. Results: The statistical analysis demonstrated that comparing the two study groups with cognitive disorder, significant differences exist as for the level of cognitive performance in all the explored domains, except in the perception and motor functions. Likewise, the results demonstrated that the yielding level for each function evaluated within the domain of the executive functions is significantly different between both groups with cognitive disorder, except for the space orientation. Conclusions: The neuropsychological profile of the cognitive disorder Alzheimer possible subtype was characterized by a disorder in multiple domains, with predominance of the focused executive dysfunction in the areas of the mental flexibility and inhibition of automatisms. Background: Dementia impairs the coping with routine daily tasks and social relationships due to an increasing degeneration of cognitive abilities. An appropriate treatment must adequately consider the effects of declined cognitive abilities on patients and their environment. Therefore, in recent times, integrative procedures for cognitive rehabilitation (CR) have become increasingly important for the therapy of patients with mild cognitive impairment (MCI) and mild dementia (MD). CR approaches provide compensatory possibilities for clearly defined routine challenges and the individual needs of those affected. Material and methods: This overview article in the form of a selective review elaborates factors for the effectiveness of CR on the basis of the currently available literature: 1) individuality - consideration of personal needs and targets, 2) compensation - mediation of skills and strategies to compensate for cognitive impairments, 3) interaction - inclusion of relatives and environmental conditions and 4) integration - integration of various therapeutic disciplines and methods. Results and discussion: On the basis of this assessment with regards to the content, a critical analysis of the methods of short and long-term therapeutic effects on MCD and MD was carried out. Although the resulting factors were of high long-term relevance for the improvement of depression and quality of life, effects on cognition were more pronounced for MCI than for MD, which emphasizes the importance of beginning therapy as early as possible. The results show that future studies on effectiveness must employ endpoints relevant for routine daily life, and that the possibility of an implementation of therapeutic concepts in a healthcare system should be considered as an essential criterion. Objective. The aims were to (i) report the outcome of mild cognitive disorder (MCD) 3.6 years after initial interview and diagnosis; (ii) identify predictors of new cases of MCD. The hypotheses were that (i) persons with MCD are more likely to develop dementia than those without MCD; (ii) symptoms of anxiety or depression predict MCD caseness at follow-up. Design . Longitudinal cohort study. Setting . Community of elderly people (age 70–97 years). Participants . 612 of 897 elderly subjects (mean 76 years) were reinterviewed. Of the 36 MCD cases originally identified, 25 were available at follow-up. 24 incident cases of MCD were identified. Main outcome measures . ICD-10 dementia, DSM-III-R dementia, ICD-10 mild cognitive disorder diagnoses made by the Canberra Interview for the Elderly, tests of anxiety, depression, neuroticism and cognitive performance. Main results .Of the original 25 MCD cases available at follow-up, two had a diagnosis of MCD, and three had a diagnosis of both ICD-10 and DSM-III-R dementia. The prevalence of MCD and DSM-III-R dementia at follow-up was no greater for MCD cases diagnosed at initial interview than in normal subjects at initial interview. There was, however, an increased prevalence of ICD-10 dementia among original MCD cases. At initial interview and at follow-up MCD cases were more anxious and depressed but had similar cognitive performance to normals. For incident cases of MCD the only significant predictor was age. Conclusions . MCD cannot be seen to be a specific forerunner of dementia.
