To analyse clinical studies investigating coating agents such as sealants and other bonding materials to prevent the initiation or inhibit the progress of white spot lesions (WSL) during orthodontic treatment with fixed appliances. Electronic databases (Pubmed, CENTRAL, EMBASE) were screened for studies. No language restrictions were applied. Study selection, data extraction and quality assessment were done in duplicate. Primary outcome included assessment of WSL with visual-tactile assessment and/or laser fluorescence measurements. Twenty-four studies with 1117 patients (age: 11-40 years) and 12,809 teeth were included. Overall, 34 different sealants or bonding materials were analysed. Fourteen studies analysed fluoride and 14 studies non-fluoride releasing materials. Meta-analysis for visual tactile assessment revealed that sealants significantly decreased the initiation of WSL compared to untreated control (RR [95%CI] = 0.70 [0.53; 0.93]; very low level of evidence). Materials releasing fluoride did not decrease initiation of WSL compared to those with no fluoride release (RR [95%CI] = 0.84 [0.70; 1.01]; very low level of evidence). For laser fluorescence measurements no meta-analysis could be performed. The use of sealants seems to be effective in preventing the initiation of post-orthodontic WSL. Furthermore, there is no evidence supporting that fluoride-releasing sealants or bonding materials are more effective than those without fluoride release. No gold standard prevention strategy to prevent WSL during treatment with fixed orthodontic appliances has been established yet. Methods: The current study includes 80 extracted premolars of human from the patient visiting for orthodontic treatment of Coorg Institute of Dental Sciences, Karnataka, India. The brackets were debonded using four different methods. The enamel surface damage after the procedure was assessed with the Enamel Surface Index (ESI); similarly, the Adhesive Remnant Index (ARI) score was used to determine the adhesive residual deposit. Scanning electron microscopy (SEM) was used to visualize better microporosities and micromechanical retention of adhesive remnants on the enamel surface. The normality of the data was tested using the Kolmogorov-Smirnov test. Depending upon the normality test result, the one-way ANOVA test or Kruskal-Wallis test was used to test the mean ESI and mean ARI differences among different debonding methods along with the appropriate post hoc tests. The necessary ethical clearance was obtained from the Ethics Committee of the institute., Results: The ultrasonic scaler (US) technique led to more significant enamel surface damage, with 13 (65%) samples in the ESI scores III and IV against the satisfactory surface in 2 (10%) samples with the ligature cutter (LC) technique (ESI-I) reflecting LC as a better technique. The ESI scores (III and IV) for debonding plier (DP) and thermal method (TM) reflected a higher value in 12 (60%) and 10 (50%) samples and caused more damage to the enamel surface as compared to the LC technique. The ARI score was highest (ARI-1 = 40%) with the LC technique, followed by the US (ARI-1 = 20%), TM (ARI-1 = 15%), and DP (ARI-1 = 5%) methods. We have observed a significant association ( p value <0.05) of the ARI score among four different debonding ways in terms of each tooth's residual adhesive after the bracket removal., Conclusion: The result establishes the LC technique as a more acceptable one as it causes minimal harm to the debonded surface. The adhesive left on the debonded area is also minimum as compared to the other three methods tested. Objectives: The purpose of this study was to determine if shear bond resistance of orthodontic brackets bonded to enamel is associated with genes implicated in the enamel mineralization process., Methods: Ninety-two permanent, caries-free premolars extracted for orthodontic purposes and their associated saliva samples were obtained. Eighteen single nucleotide polymorphisms (SNPs) were studied for association with shear bond resistance. The genes of interest in this study were those previously associated with dental caries by our group. All tooth samples were bonded on the buccal surface with metallic lower lateral brackets, and then subjected to physical debonding. The force required to debond the bracket was recorded in Newtons (N) and converted to a shear bond resistance value in Megapascals (N/mm 2 ). The data were analyzed for statistical significance as compared with the mean shear bond resistance value via PLINK whole genome analysis software., Results: Associations were found between the SNPs for tuftelin (rs7526319, P = 0.004) and amelogenin (rs17878486, P = 0.04) and a higher shear bond resistance., Conclusion: The collected data support the proposed hypothesis that genes involved in the mineralization process affect the bonding of orthodontic brackets, and such an association is of value for the field of orthodontics, particularly in evaluating the efficacy of enamel-resin bond strength for patients receiving treatment., (Copyright © 2020 World Federation of Orthodontists. Background: Adhesion strength of orthodontic attachments to enamel should be within optimal range to resist occlusal forces and to allow debonding without enamel damage., Objective: The present study compared the effect of non-thermal plasma (NTP) and conventional surface treatment on the adhesion strength of orthodontic bracket to enamel., Methods: A total of 100 premolar teeth were allocated into 5 groups according to the bonding procedure followed: Group 1 (Etch, prime and composite adhesive); Group 2 (Prime and composite adhesive); Group 3 (Glass ionomer cement); Group 4 (NTP, prime and composite adhesive) and Group 5 (NTP and glass ionomer cement). Ten specimens in each group were subjected to artificial aging and the remaining ten specimens served as baseline specimens. Adhesion strength values were recorded after debonding and bond failure types were scored. Water contact angles of the NTP treated and untreated enamel surface were measured., Results: Group 1 specimen demonstrated highest bond strength at baseline (21.5 ± 3.01) and thermocycling (15.8 ± 2.87) and the least values were in Group 2 specimens at baseline (3.9 ± 1.01) and thermocycling (1.6 ± 0.7). Among the experimental (NTP) groups, Group 4 specimens exhibited high adhesion strength at baseline (10.2 ± 1.76) and after thermocycling (9.8 ± 2.15) compared to Group 5 specimens at baseline (10.1 ± 1.05) and thermocycling (6.5 ± 2.19). The water contact angle on untreated enamel surface was 53.1° ± 2.1° as compared to 1.4° ± 0.7° in treated surface., Conclusion: Non-thermal plasma (NTP) treatment in conjunction with composite adhesives demonstrated clinically acceptable adhesion strength and was well within the optimal range (7-14 MPa) for enamel bonding.

Objectives: To evaluate and compare the enamel microcracks (EMCs) characteristics (qualitative and quantitative) in the form of tooth damage before and after debonding from human teeth of in vitro studies., Eligibility Criteria: Laboratorial studies evaluating EMCs characteristics before and after debonding metal and ceramic brackets from human teeth with intact buccal enamel., Information Sources: An electronic search of four databases (all databases of the Cochrane Library, CA Web of Science, MEDLINE via PubMed, and Google Scholar) and additional manual searches were carried out, without language restrictions. Studies published between 2000 and 2017 years were selected. Reference lists of the included articles were screened, and authors were contacted when necessary., Risk of Bias: The following six parameters were analyzed: blinding of examiner and outcome assessment, incomplete outcome data before bonding and after debonding, selective outcome reporting, and incomplete reporting of EMCs assessment., Included Studies: Out of 430 potentially eligible studies, 259 were screened by title and abstract, 180 were selected for full-text analysis, 14 were included in the systematic review. Seven studies were selected for the meta-analysis., Synthesis of Results: The results for EMCs characteristics were expressed as mean differences (MDs) with their 95 per cent confidence intervals (CIs), and calculated from random-effects meta-analyses. Debonding was associated with the increase in number (three studies, MD = 3.50, 95% CI, 2.13 to 4.87, P < 0.00001), length (seven studies, MD = 3.09 mm, 95% CI, 0.75-5.43, P < 0.00001), and width (three studies, MD = 0.39 µm, 95% CI, -0.01 to 0.79, P = 0.06) of EMCs. Considerable statistical heterogeneity was found for two forest plots evaluating the changes of number and length characteristics during debonding., Conclusions: There is weak evidence indicating length and width of EMCs increase following bracket removal and the scientific evidence concerning quantitative evaluation of the number parameter before and after debonding is insufficient. However, there is a strong evidence that after debonding the number of EMCs is likely to increase., Registration: No registration was performed.

Background: The present study was conducted to evaluate the effect of different topical agents utilized for prevention of enamel decalcification around orthodontic brackets bonded to bleached and non-bleached enamel., Methods: Human maxillary premolars (n = 120) were divided into two equal groups. Teeth in group I were left without bleaching while those in group II were bleached with Vivastyle gel. Metal brackets were bonded to all the teeth using light-cured adhesive. Each group was divided into six equal subgroups (A, B, C, D, E, and F). In subgroup A, no material was applied (control). In subgroups B, C, D, E, and F, the following materials were applied respectively: Profluorid varnish, Enamel Pro Varnish, Ortho-Choice Ortho-Coat, GC Tooth Mousse, and GC MI Paste Plus. All teeth were cycled in a demineralization solution/artificial saliva for 15 days. Laser fluorescence was used to measure the level of enamel mineralization. The data were statistically analyzed., Results: Regarding the non-bleaching subgroups, all studied material revealed significant demineralization reduction in comparison to the control subgroup (P < 0.05). Ortho-Choice Ortho-Coat revealed the highest significant effect while GC Tooth Mousse showed the least effect. In bleached subgroups, Profluorid varnish, Enamel Pro Varnish, and Ortho-Choice Ortho-Coat significantly reduced demineralization (P < 0.05) while either GC MI Paste Plus or GC Tooth Mousse had no significant effects (P > 0.05)., Conclusions: Ortho-Choice Ortho-Coat, and Profluorid and Enamel Pro varnishes could be utilized successfully to reduce enamel demineralization around brackets bonded to either bleached or non-bleached enamel. GC MI Paste Plus and GC Tooth Mousse were effective only in non-bleached enamel.

Introduction: Iatrogenic damage to the tooth surface in the form of enamel tearouts can occur during removal of fixed orthodontic appliances. The aim of this study was to assess debonded metal and ceramic brackets attached with a variety of bonding materials to determine how frequently this type of damage occurs., Methods: Eighty-one patients close to finishing fixed orthodontic treatment were recruited. They had metal brackets bonded with composite resin and a 2-step etch-and-bond technique or ceramic brackets bonded with composite resin and a 2-step etch-and- bond technique, and composite resin with a self-etching primer or resin-modified glass ionomer cement. Debonded brackets were examined by backscattered scanning electron microscopy with energy dispersive x-ray spectroscopy to determine the presence and area of enamel on the base pad., Results: Of the 486 brackets collected, 26.1% exhibited enamel on the bonding material on the bracket base pad. The incidences of enamel tearouts for each group were metal brackets, 13.3%; ceramic brackets, 30.2%; composite resin with self-etching primer, 38.2%; and resin-modified glass ionomer cement, 21.2%. The percentage of the bracket base pad covered in enamel was highly variable, ranging from 0% to 46.1%., Conclusions: Enamel damage regularly occurred during the debonding process with the degree of damage being highly variable. Damage occurred more frequently when ceramic brackets were used (31.9%) compared with metal brackets (13.3%). Removal of ceramic brackets bonded with resin-modified glass ionomer cement resulted in less damage compared with the resin bonding systems., (Copyright © 2017 American Association of Orthodontists. Objectives: White spot lesions due to biofilm acid-induced enamel demineralization are prevalent in orthodontic treatments. The aim of this study was to develop a novel bioactive multifunctional cement with protein-repellent, antibacterial and remineralizing capabilities, and investigate the effects on enamel hardness and lesion depth in vitro for the first time., Materials and Methods: 2-Methacryloyloxyethyl phosphorylcholine (MPC), dimethylaminohexadecyl methacrylate (DMAHDM), and nanoparticles of amorphous calcium phosphate (NACP) were incorporated into a resin-modified glass ionomer (RMGI). Extracted human premolars had brackets bonded via four groups: (1) Transbond XT (TB), (2) RMGI (GC Ortho LC), (3) RMGI+MPC+DMAHDM, (4) RMGI+MPC+DMAHDM+NACP. Demineralization was induced via a dental plaque microcosm biofilm model. Samples were tested using polarized light microscopy (PLM) for lesion depth. Enamel hardness was tested for different groups., Results: Incorporating MPC, DMAHDM and NACP did not affect enamel bond strength. "RMGI+MPC+DMAHDM+NACP" group had the least lesion depth in enamel (p<0.05). Groups with NACP had the highest enamel hardness (p<0.05). Mineral loss (ΔS) in enamel for NACP group was about one third that for RMGI control. "RMGI+MPC+DMAHDM" had greater effect on demineralization-inhibition, compared to RMGI and TB controls. "RMGI+MPC+DMAHDM+NACP" was more effective in protecting enamel prisms from dissolution by biofilm acids, compared to RMGI and TB control groups., Conclusion: The Novel "RMGI+MPC+DMAHDM+NACP" cement substantially reduced enamel demineralization adjacent to orthodontic brackets, yielding much less lesion depth and greater enamel hardness under biofilm acid attacks than commercial controls. The clinical significance is that the novel multi-agent (RMGI+MPC+DMAHDM+NACP) method is promising for a wide range of preventive and restorative applications to combat caries., (Copyright © 2017 Elsevier Ltd. The aim of the study was to evaluate ultrastructural changes of dental enamel after fixation of orthodontic appliances, initial influence of orthodontic forces and removal of braces. Five intact permanent tooth extracted for orthodontic reasons were included in the experimental study. Scanning probe microscopy was conducted in 4 random enamel points in each tooth (20 points overall) in semi-contact mode with standard 10 nm probes. The study showed ultrastructural enamel changes such as nanofractures up to 1 mm along the braces locks. The changes correlated with surface morphological features and teeth anatomy and may play an important role in dental decay and non-carious lesions occurring in the course of orthodontic treatment.

Aim: The aim of this prospective study was to test the efficacy of casein phosphopeptide amorphous calcium phosphate (CPP-ACP) paste applied in-office to prevent white spot lesions (WSL) in patients undergoing fixed orthodontic treatment., Materials and Methods: Study design: this study was designed as a randomised controlled clinical trial. Fifty-seven patients undergoing nonextraction fixed orthodontic treatment were enrolled and divided randomly into two groups (control n = 28, experimental n = 29). The pretreatment plaque, gingival and bleeding indices, and oral hygiene habits were recorded. One group received CPP-ACP paste (GC Tooth Mousse) at each monthly orthodontic follow-up examination; the control group received routine orthodontic treatment. The mean patient age was 16.9 years in the experimental group and 17.1 years in the control group. The periodontal indices, decayed, missing, and filled teeth (DMFT), decayed, missing, and filled surfaces (DMFS), and the WSL prevalence pre- and post-treatment were measured and compared between the groups., Statistics: differences between groups in normally distributed data between groups were assessed by the paired-t test, and the Wilcoxon Signed Rank test was used to compare variables that were not normally distributed., Results: There was a lower incidence of WSL in the experimental group compared to the control group. Most of the WSL occurred at the maxillary incisors, mandibular canines and premolars., Conclusion: The in-office application of CPP-ACP paste did not prevent WSL development completely; however, it did significantly decrease the number of WSL compared to the control patients.

Braces as foreign bodies in the mouth carry a risk of side effects and toxicity to the human body. This article presents the results indicating the possible toxic effects of tools used for cleaning the enamel after the completion of orthodontic treatment. The studies were carried out in vitro . The procedure of enamel etching, bonding orthodontic metal brackets, and enamel cleaning after their removal was performed under laboratory conditions. The enamel microstructure and elements present on its surface were evaluated using the scanning electron microscope (SEM). Silicon and aluminium were found in addition to the tooth building elements.

Aim: The purpose of this work is to evaluate the effectiveness of one self-etching and two filled orthodontic primers on enamel demineralization around orthodontic brackets., Methods: Brackets were bonded to 84 bovine teeth and the vestibular enamel surfaces covered with acid-resistant nail varnish exposing 1 mm of space on each side of the bracket base. The teeth were allocated to four groups, using either Transbond XT conventional primer on etched enamel (group 1), Transbond Plus Self-Etching Primer on untreated enamel (group 2), Pro Seal filled resin primer on etched enamel (group 3), or Opal Seal filled resin primer on etched enamel (group 4). Each tooth was subjected to 15,000 strokes of brushing followed by exposure to an acid challenge. Calcium-ion release from each sample was calculated using atomic absorption spectrophotometry. Data were analyzed using one-way ANOVA and a post hoc Tukey test. Differences were considered statistically significant at p ≤ 0.05., Results: Statistically significant differences were observed between the four groups (p < 0.001). No significant difference was found between the controls (group 1) and the Opal Seal group. Higher calcium release was observed in the Pro Seal group and the self-etching primer group compared to the controls. The highest calcium release was recorded in the self-etching primer group., Conclusion: Filled sealants may not have a protective effect against enamel demineralization. Transbond Plus Self-Etching Primer should be used cautiously, considering the risk of demineralization involved in its application.

One possible undesirable consequence of orthodontic therapy is the development of incipient caries lesions of enamel around brackets. The aim of this study was to compare the effects of CO2 (λ = 10.6 μm) and Nd:YAG (λ = 1,064 nm) lasers associated or not with topical fluoride application on the prevention of caries lesions around brackets. Brackets were bonded to the enamel of 65 premolars. The experimental groups (n = 13) were: G1--application of 1.23% acidulated fluoride phosphate gel (AFP, control); G2--Nd:YAG laser irradiation (0.6 W, 84.9 J/cm(2), 10 Hz, 110 μs, contact mode); G3--Nd:YAG laser irradiation associated with AFP; G4--CO2 laser irradiation (0.5 W, 28.6 J/cm(2), 50 Hz, 5 μs, and 10 mm focal distance); and G5--CO2 laser irradiation associated with AFP. Quantitative light-induced fluorescence was used to assess enamel demineralization. The data were statistically compared (α = 5%). The highest demineralization occurred in the Nd:YAG laser group (G2, 26.15% ± 1.94). The demineralization of all other groups was similar to that of the control group. In conclusion, CO2 laser alone was able to control enamel demineralization around brackets at the same level as that obtained with topical fluoride application.

Unlabelled: This study evaluated the effect of high fluoride dentifrice on the bond strength of brackets after erosive challenge. Eighty-four enamel specimens were divided into seven groups (n = 12): WN (distilled water/no acid challenge), W3C (distilled water/3 cycles of acid challenge), and W6C (distilled water/6 cycles of acid challenge) were not submitted to dentifrice treatment. Groups RF3C (regular fluoride dentifrice/3 cycles of acid challenge) and RF6C (regular fluoride dentifrice/6 cycles of acid challenge) were treated with dentifrices containing 1450 μg F(-)/g and HF3C (high fluoride dentifrice/3 cycles of acid challenge) and HF6C (high fluoride dentifrice/6 cycles of acid challenge) were with 5000 μg F(-)/g. Acid challenges were performed for seven days. After bond strength test, there was no significant difference among groups submitted to 3 cycles of acid challenge (P > 0.05). Statistically significant difference was found between the regular and high fluoride dentifrices after 6 cycles of acid challenge (<0.05). Similar areas of adhesive remaining were found among control groups and among groups W6C, RF3C, RF6C, HF3C, and HF6C. The high fluoride dentifrice was able to prevent the reduction of bond strength values of brackets submitted to acid challenge., Clinical Relevance: the high fluoride toothpaste prevents debonded brackets on erosive enamel.

The presence of enamel cracks is not only due to the removal process. It is all the stages before that we need to consider. 1. The presence of risks factors before the treatment (cracks, trauma...) have to be explained to the patient and recorded. 2. The choice of bracket placement: the buccal side is responsible for less cracks than the lingual one. 3. Ceramic brackets need higher debonding forces than metal ones; metal reinforced orthodontic ceramic brackets could be an interesting approach. 4. CVIMR could be better than classic bonds. 5. In case of multiple rebondings, it is better not to etch enamel again but only to level the resin left on tooth surface. 6. Objective: In this randomized study, the caries-protective effect on vestibular enamel of two fluoride-containing sealants (Protecto® and Light Bond®) during multibracket treatment was investigated., Materials and Methods: In all, 40 orthodontic patients about to receive a multibracket appliance with the brackets bonded to the vestibular tooth surfaces were randomly included in this study. Each one was randomly assigned to one of four groups. A crossover design was selected in which a sealed quadrant was contralateral to an unsealed quadrant, then choosing the reverse configuration in the opposite jaw. Two sealants were, thus, tested on vestibular enamel on left and right anterior teeth and premolars in both jaws of each patient over 6 months of multibracket treatment. A DIAGNOdent® pen measuring laser fluorescence was used to analyze the relevant enamel surfaces both at baseline and after 6 months., Results: Neither the incidence nor the characteristics of the demineralization we observed during the study differed between the 4 groups., Conclusion: Single application of smooth-surface sealants did not protect enamel around brackets from incipient carious lesions during the first 6 months of multibracket treatment.

Objective: To evaluate whether the debonding procedure leads to restitutio ad integrum of the enamel surface by investigating the presence of enamel within the bracket base remnants after debonding., Materials and Methods: Sixty patients who completed orthodontic treatment with fixed appliances were included. A total of 1068 brackets were microphotographed; the brackets presenting some remnants on the base (n = 818) were selected and analyzed with ImageJ software to measure the remnant area. From this population a statistically significant sample (n = 100) was observed under a scanning electron microscope to check for the presence of enamel within the remnants. Energy dispersive x-ray spectrometry was also performed to obtain quantitative data., Results: Statistically significant differences in the remnant percentage between arches were observed for incisor and canine brackets (P < .0001 and P = .022, respectively). From a morphologic analysis of the scanning electron micrographs the bracket bases were categorized in 3 groups: group A, bases presenting a thin enamel coat (83%); group B, bases showing sizable enamel fragments (7%); group C, bases with no morphologic evidence of enamel presence (10%). Calcium presence was noted on all evaluated brackets under energy dispersive x-ray spectrometry. No significant difference was observed in the Ca/Si ratio between group A (16.21%) and group B (18.77%), whereas the Ca/Si ratio in group C (5.40%) was significantly lower than that of the other groups (P < .323 and P = .0001, respectively)., Conclusion: The objective of an atraumatic debonding is not achieved yet; in some cases the damage could be clinically relevant.

The purpose of this study was to evaluate and compare enamel micro-crack characteristics of adult patients before and after removal of metal brackets. After the examination with scanning electron microscopy (SEM), 45 extracted human teeth were divided into three groups of equal size: group 1, the teeth having enamel micro-cracks, group 2, the teeth without initial enamel micro-cracks, and group 3, control group to study the effect of dehydration on existing micro-cracks or formation of new ones. For all the teeth in groups 1 and 2, the same bonding and debonding procedures of metal brackets were conducted. The length and width of the longest enamel micro-crack were measured for all the teeth before and after removal of metal brackets. The changes in the location of the micro-cracks were also evaluated. In group 3, teeth were subjected to the same analysis but not bonded. The mean overall width of micro-cracks after removal of metal brackets was 3.82 μm greater than before bonding procedure (P < 0.05). Also, a significant difference was noticed between the width of micro-cracks in first zone (cervical third) and third zone (occlusal third) after debonding procedure (P < 0.05). New enamel micro-cracks were found in 6 of 15 (40 per cent) examined teeth. Greatest changes in the width of enamel micro-cracks after debonding procedure appear in the cervical third of the tooth. On the basis of this result, the dentist must pay extra care and attention to this specific area of enamel during removal of metal brackets in adult patients.

Purpose: The decalcification of enamel is a serious clinical problem in orthodontic patients and is usually observed as white spot lesions surrounding brackets. This study's purpose was to evaluate the effect of professionally applied 1.23 percent fluoride foam on reducing the formation of white spot lesions (WSLs) in patients with fixed orthodontic appliances., Methods: In a randomized, double-blind, placebo-controlled trial, 100 participants were randomly divided into two groups. The two groups received fluoride foam and placebo foam, respectively, every two months during the treatment. The examinations before bonding and after debonding were performed by one examiner and included the presence and severity of WSLs on incisors, canines, and premolars., Results: The incidence of WSLs was approximately 13 percent in the fluoride foam group and 51 percent in the placebo group (P<.001). The mean increment of WSLs score was significantly lower in the fluoride foam group (0.71 ± 2.80) than in the placebo group (4.36 ± 5.41; P<.001). The preventive fraction was approximately 76 percent, and the number needed to treat was calculated as 2.6., Conclusions: Professional application of 1.23 percent fluoride foam during orthodontic treatment effectively reduced the development of white spot lesions. A prophylactic regimen based on the routine use of fluoride foam during orthodontic treatment is recommended.

Objective: To clinically evaluate the effectiveness of icon infiltration resin on masking post-orthodontic white spots., Methods: Eight post-orthodontic patients with 6 maxillary anterior teeth showing signs of decalcification (total 48 teeth) were enrolled in this study. All teeth were treated with icon resin infiltration according to manufacturer's recommendation. Standardized digital photographs were taken before, immediately after and 1 week, 6 and 12 months after treatment. Before taking pictures the assigned teeth were cleaned using pumice and rubber polishing cups. The results were classified into three groups: completely masked, partially masked, and unchanged. Pictures of partially masked teeth were analyzed using image analysis software (Image-pro plus 6.0), size of the white spot lesion (W) and the whole tooth facial surface (T) were measured, then W/T ratio (in %) was calculated. The images were imported into image analysis software (Photoshop) which presented the images into histograms of gray scale from (0 to 255)., Results: Among the 48 teeth, 11 teeth (22.9%, 11/48) were classified as completely masked, whereas 37 teeth (77.1 %, 37/48) were classified as partially masked and no tooth unchanged. For partially masked teeth, W/T ratio decreased significantly after treatment from 31.37% to 7.99% (by Wilcoxon's signed rank test, P<0.05). The means at gray scale for the initial and 1 week photographs after treatment were 188.07± 5.62 and 143.20± 7.03 respectively, and there was significant difference by Wilcoxon,s signed rank test (P<0.05). The data of 6 and 12 months after treatment were 136.33± 4.54 and 139.57± 3.70 respectively, there were no significant differences (P>0.05) in comparison to 1 week after treatment., Conclusion: Resin infiltration was proven to be an effective treatment for masking white spot lesions. The surface color of infiltrated lesions remained stable after 12 months.

Objective: To compare changes in enamel microhardness adjacent to orthodontic brackets after using bonding agents containing various compositions of bioactive glass compared to a traditional resin adhesive following a simulated caries challenge., Materials and Methods: Extracted human third molars (n  =  10 per group) had orthodontic brackets bonded using one of four novel bioactive glass (BAG)-containing orthodontic bonding agents (BAG-Bonds) or commercially available Transbond-XT. The four new adhesives contained BAG in varying percentages incorporated into a traditional resin monomer mixture. Teeth were cycled through low-pH demineralizing and physiologic-pH remineralizing solutions once each day over 14 days. Microhardness was measured on longitudinal sections of the teeth 100, 200, and 300 µm from the bracket edge and beneath the brackets, at depths of 25 to 200 µm from the enamel surface. Normalized hardness values were compared using three-way analysis of variance., Results: Significantly less reduction in enamel microhardness was found with the experimental adhesives at depths of 25 and 50 µm at all distances from the bracket edge. In all groups, there were no significant changes in enamel microhardness past 125-µm depth. Results varied with the different BAG-Bonds, with 81BAG-Bond showing the smallest decrease in enamel microhardness., Conclusions: The BAG-Bonds tested in this study showed a reduction in the amount of superficial enamel softening surrounding orthodontic brackets compared to a traditional bonding agent. The results indicate that clinically, BAG-Bonds may aid in maintaining enamel surface hardness, therefore helping prevent white spot lesions adjacent to orthodontic brackets.

Objective: To compare the effects of sodium fluoride mouth rinse, casein phosphopeptide-amorphous calcium phosphate (CPP-ACP), and the microabrasion technique in treating white spot lesions., Materials and Methods: The study population consisted of 80 patients (46 females, 34 males; 966 affected teeth) who had developed multiple decalcified enamel lesions after fixed orthodontic therapy. The study population was divided into four groups of 20 patients each. The control group (group I) participants were to just brush their teeth, the fluoride group (group II) participants were instructed to use 20 ml of neutral 0.025% sodium fluoride rinse, the participants in the CPP-ACP group (group III) were instructed to use tooth mousse twice a day in addition to fluoride toothpaste for 6 months, and the participants in the microabrasion group (group IV) were to undergo treatment by the microabrasion technique, which is a commonly used mixture of 18% hydrochloric acid. Data were analyzed with the generalized linear mixed model and Tukey HSD at the P < .05 level., Results: The area of the white spot lesions decreased significantly in all groups. Inter group differences in the treatment success rates were significant. The highest success rate was observed for group IV (97%). The success rate of group III (58%) was significantly higher than that of groups II (48%) and I (45%)., Conclusions: The use of CPP-ACP can be more beneficial than fluoride rinse for postorthodontic remineralization. Microabrasion is an effective treatment for cosmetic improvement of long-standing white spot lesions.

Objective: To determine if a new experimental resin-based material containing Portland cement (PC) can help prevent enamel caries while providing adequate shear bond strength (SBS)., Materials and Methods: Brackets were bonded to human premolars with experimental resin-based adhesive pastes composed of three weight rations of resin and PC powder (PC 30, 7:3; PC 50, 5:5; PC 70, 3:7; n  =  7). Self-etching primer (SEP) adhesive (Transbond Plus) and resin-modified glass ionomer cement (RMGIC) adhesive (Fuji Ortho FC Automix) were used for comparison. All of the bonded teeth were subjected to alternating immersion in demineralizing (pH 4.55) and remineralizing (pH 6.8) solutions for 14 days. The SBS for each sample was examined, and the Adhesive Remnant Index (ARI) score was calculated. The hardness and elastic modulus of the enamel were determined by a nanoindenter at 20 equidistant depths from the external surface at 100 µm from the bracket edge. Data were compared by one-way analysis of variance and a chi-square test., Results: PC 50 and PC 70 showed significantly greater SBS than Fuji Ortho FC Automix, although Transbond Plus showed significantly greater SBS than other bonding systems. No significant difference in the ARI category was observed among the five groups. For specimens bonded with PC 50 and PC 70, the hardness and elastic modulus values in most locations were equivalent to those of Fuji Ortho FC Automix., Conclusions: Experimental resin-based bonding material containing PC provides adequate SBS and a caries-preventive effect equivalent to that of the RMGIC adhesive system.

This study evaluated the enamel loss and composite remnants after debonding and clean-up. The tested null hypothesis is that there are no differences between different polishing systems regarding removing composite remnants without damaging the tooth surface. Brackets were bonded to 75 extracted human molars and removed after a storage period of 100 hours. The adhesive remnant index (ARI) was evaluated. The clean-up was carried out with five different procedures: 1. carbide bur; 2. carbide bur and Brownie and Greenie silicone polishers; 3. carbide bur and Astropol polishers; 4. carbide bur and Renew polishers; and 5. carbide bur, Brownie, Greenie and PoGo polishers. Silicone impressions were made at baseline (T0) and after debonding (T1) and polishing (T2) to produce plaster replicas. The replicas were analysed with a three-dimensional laser scanner and measured with analytical software. Statistical analysis was performed with the Kruskal-Wallis test and pairwise Wilcoxon tests with Bonferroni-Holm adjustment (α = 0.05). Enamel breakouts after debonding were detectable in 27 per cent of all cases, with a mean volume loss of 0.02 mm(3) (±0.03 mm(3)) and depth of 44.9 μm (±48.3 μm). The overall ARI scores was 3 with a few scores of 1 and 2. The composite remnants after debonding had a mean volume of 2.48 mm(3) (±0.92 mm(3)). Mean volume loss due to polishing was 0.05 mm(3) (±0.26 mm(3)) and the composite remnants had a mean volume of 0.22 mm(3) (±0.32 mm(3)). There were no statistically significant differences in volumetric changes after polishing (P = 0.054) between the different clean-up methods. However, sufficient clean-up without enamel loss was difficult to achieve.

Objective: The purpose of this study was to compare the shear bond strength and incidence of enamel fractures of the ceramic brackets Fascination® and Fascination®2., Materials and Methods: A total of 360 teeth (180 first upper bicuspids and 180 lower incisors) were stored in 96% ethanol, while 360 other teeth (180 first upper bicuspids and 180 lower incisors) were stored in 0.1% thymol. All 720 teeth were bonded one-half each with Fascination® and Fascination®2 brackets using three different adhesives and three different light curing units. The teeth were debonded with a debonding-device according to DIN EN ISO 10477 using a universal testing machine with a crosshead speed of 1 mm per minute. The enamel surface was then examined stereomicroscopically (10x and 40x magnification). The non-parametric Mann-Whitney U test was used, since the data were not normally distributed., Results: The Fascination®2 brackets provided significantly lower shear bond strength than Fascination® brackets (p = 0.003). Fascination® brackets demonstrated significantly fewer, smaller enamel fractures than Fascination®2 brackets (p = 0.012)., Conclusion: The lower shear bond strength of the Fascination®2 brackets is clinically acceptable, but our study's experimental design did not enable us to prove whether this is clinically associated with a lower risk of enamel fracture.

The aim of the present study was to evaluate the effect of fluoride in prevention of tooth erosion around orthodontic brackets under erosive challenge. Edgewise brackets were bonded with Transbond™ XT composite on vestibular surface of 40 bovine incisors. The teeth were assigned to 4 groups (n=10): G1: Remineralizing saliva; G2: Erosive challenge; G3: Experimental group submitted to topical application of neutral fluoride gel (2% NaF) before erosive challenge; G4: Experimental group submitted to three daily applications of fluoride dentifrice (PFM 1500 ppmF) during erosive challenge. After 14 days of erosive challenge, direct visual and tactile examination were performed by two calibrated and trained examiners (Kappa = 0.867). The following scores were used: 0 = Intact enamel, 1 = Demineralized enamel without cavity, 2 = Demineralized enamel with cavity, 3 = Remineralized enamel without cavity, 4 = Remineralized enamel with cavity. Kruskal-Wallis and Mann-Whitney tests were applied to determine erosion levels, establishing a confidence interval of 95% (p<0.05). G2 and G3 presented 100% of score 2, with large cavities, presenting rough and opaque surface. G4 showed 50% of score 3 and 50% of score 4. Considering the studied conditions, it was found a significant difference between G2 and G4 and between G3 and G4 (p<0.01). By contrast to single application of neutral fluoride gel, the high frequency of use of fluoride at low concentration had a great influence during the dynamics of erosion.

Objective: To determine the relative effects that phosphoric acid etching, metal brackets, composite resin adhesives, and filled resin sealants have on enamel decalcification., Materials and Methods: One hundred and fifty teeth were randomly divided into six groups to test the effects of acid etching, brackets, and a composite resin adhesive. The right side of each tooth was sealed. The teeth were submitted to a simulated carious challenge by Streptococcus mutans in a nutritive media for 42 days. Scanning electron photomicrographs (SEMs) were used to qualitatively evaluate the tooth surfaces. DIAGNOdent laser fluorescence was used to quantify the decalcification changes that occurred., Results: SEMs showed normal enamel on the control teeth, etching patterns on the acid-etched teeth, surface degradation on the teeth that had resin, and sealant tags on the sealed side. The untreated control group showed significantly less decalcification (P ≤ .05) than did all groups except the no etch+bracket group. The no etch+bracket group showed significantly less decalcification than did the etch+bracket, no etch+resin, and the etch+resin groups. Acid-etched teeth demonstrated significantly more decalcification (34%) than did those that were not etched. The teeth with resin showed significantly more decalcification (24%) than did those without resin. There was no significant difference between teeth that did and did not have brackets. The unsealed side of the teeth showed small amounts of--but significantly more--decalcification (5%) compared to the sealed side., Conclusions: Acid etching and composite resin adhesives increase enamel decalcification in vitro, while brackets do not. Filled resin sealants protect the teeth from decalcification.

Introduction: White-spot lesions (WSL) might be susceptible to mechanical damage during orthodontic bracket and adhesive removal. The aims of this in-vitro study were to investigate enamel loss on bracket and adhesive removal when the brackets were surrounded by WSL and to determine the effect of remineralizing these lesions with a 1% (w/v) casein phosphopeptide amorphous calcium fluoride phosphate (CPP-ACFP) solution before bracket and adhesive removal., Methods: Precoated metal mandibular incisor brackets were centrally bonded onto polished third molars and WSL produced by exposure to a demineralization buffer for 4, 12, and 30 days (n = 20 per group). Half of the demineralized window was covered with acid-resistant nail varnish, and the specimens were then subjected to remineralization with 1% CPP-ACFP. Brackets and residual adhesive were removed, and enamel damage was assessed by digital photography, profilometry, and scanning electron microscopy. Lesion depth, mineral loss, and remineralization were measured by transverse microradiography., Results: WSL enamel around the bracket was more susceptible to iatrogenic damage at adhesive removal compared with sound enamel. Remineralization of lesions with 1% CPP-ACFP before adhesive removal significantly (P <0.002) reduced the area and depth of damage., Conclusions: Remineralizing WSLs with CPP-ACFP before adhesive removal reduced iatrogenic enamel damage., (Copyright © 2011 American Association of Orthodontists. Introduction: The objective of this study was to evaluate the surface enamel after bracket debonding and residual resin removal., Methods: Thirty patients (female, 20; male, 10; mean age, 18.4 years) who completed orthodontic treatment with fixed appliances (Twin Brackets, 3M Unitek, Monrovia, Calif) (n = 525) were included. The amounts of adhesive left on the tooth surfaces and the bracket bases were evaluated with the adhesive remnant index (ARI). ARI(tooth) (n = 498) was assessed on digital photographs by 2 operators. After resin removal and polishing, epoxy replicas were made from the maxillary anterior teeth (n = 62), and enamel surfaces were scored again with the enamel surface index. Elemental analysis was performed on the debonded bracket bases by using energy dispersive x-ray spectrometry mean area scanning analysis. The percentages of calcium and silicon were summed up to 100%. Tooth damage was estimated based on the incidence of calcium from enamel in relation to silicon from adhesive (Ca%) and the correlation between the ARI(bracket) and Ca%., Results and Conclusions: While ARI(tooth) results showed score 3 as the most frequent (41%) (P<0.05), followed by scores 0, 1, and 2 (28.7%, 17.9%, and 12.4%, respectively), ARI(bracket) results showed score 0 more often (40.6%) than the other scores (P<0.05). Maxillary anterior teeth had significantly more scores of 3 (49%) than the other groups of teeth (10%-25%) (chi-square; P<0.001). There were no enamel surface index scores of 0, 3, or 4. No correlation between the enamel surface index and ARI(tooth) scores was found (Spearman rho = 0.014, P = 0.91). The incidence of Ca% from the scanned brackets showed significant differences between the maxillary and mandibular teeth (14% ± 8.7% and 11.2% ± 6.5%, respectively; P <0.05), especially for the canines and second premolars (Kruskal-Wallis test, P<0.01). With more remnants on the bracket base, the Ca% was higher (Jonckheere Terpstra test, P<0.05). Iatrogenic damage to the enamel surface after bracket debonding was inevitable. Whether elemental loss from enamel has clinical significance is yet to be determined in a long-term clinical follow-up of the studied patient population., (Copyright © 2010 American Association of Orthodontists. Orthodontic treatment with fixed appliances involves a significant risk of enamel demineralization. The aim of this study was three-fold: to gain insight into a) the preventive measures which are applied as standard practice in The Netherlands in orthodontics, b) when the use of fluoride rinses is initiated, and c) which concentrations and frequencies of use are recommended. A questionnaire was sent to all orthodontists in private practice in The Netherlands. Completed questionnaires were returned by 154 of the orthodontists (response rate 81%). A basic practice protocol for preventing demineralization was used at the start of treatment by 93%. The majority (90%) recommended to use fluoride rinse once daily. They generally (85%) recommended to do this in the evening immediately after toothbrushing. The authors recommend rinsing at another moment than after tooth brushing, in order to increase the number of fluoride moments during the day, which enhances the effectiveness of this measure in preventing caries.

Background: Unsightly colour changes may occur in the enamel around bonded orthodontic brackets., Aim: To determine the enamel colour changes around orthodontic brackets bonded with different adhesives., Methods: One hundred extracted upper first premolars were divided into five equal groups. Fiducial holes were drilled in the buccal and palatal surfaces of the teeth and metal orthodontic brackets were bonded to the buccal surfaces with different combinations of adhesives and primers. The teeth were immersed in a tea-coffee solution for one week. The brackets were then debonded and the teeth polished. The colour of the enamel surface was measured around each fiducial hole before bonding (baseline), after debonding and after polishing. The colour differences (deltaE) were compared using one-way ANOVA and the paired t-test., Results: There were no significant differences between the five adhesive groups in baseline-debonding deltaE and baseline-polishing deltaE, but statistically significant differences were found between the baseline-debonding deltaE on the buccal and palatal surfaces and the baseline-debonding deltaE versus baseline-polishing deltaE on the buccal surfaces. Although statistically significant, the colour changes would not be visible to many observers. Approximately 75 per cent of the staining was attributed to the enamel components., Conclusions: The type of adhesive and the methods of application had no effect on enamel colour change. Most of the colour change was attributed to uptake of stain by the components of the enamel, not the resin tags.

Introduction: The purpose of this study was to evaluate 3-dimensionally the changes on tooth surfaces after debonding orthodontic brackets and after removing residual adhesive and finishing., Methods: Sixty premolars were randomly divided into 2 groups, and brackets were bonded according to the manufacturers' instructions. Two types of orthodontic adhesives were used: resin-modified glass ionomer cement (group 1) and resin-coated adhesive precoated brackets (group 2). The brackets were debonded on a testing machine at a cross-head speed of 1 mm per minute. Models were made of each tooth before bonding, after debonding, and after removal of residual adhesive. The models were scanned with a 3-dimensional laser scanning machine, and the scanned images were analyzed by using modified analytical software., Results: The Mann-Whitney test showed significant differences in adhesive thickness and enamel loss between the 2 groups (P <.001). The mean (+/- SD) thickness for group 1 was 31.2 microm (+/- 26.5 microm), and the mean (+/- SD) thickness for group 2 was 102.7 microm (+/- 79.71 microm). The means (+/- SD) for enamel losses after cleaning and finishing the enamel surfaces were 22.8 microm (+/- 17.67 microm) for group 1 and 50.5 microm (+/- 31.27 microm) for group 2., Conclusions: Adhesive thickness and enamel loss due to orthodontic procedures can successfully be measured in vitro by using 3-dimensional laser scanning technology.

Enamel decalcification is a significant problem in orthodontic patients. The argon laser has been shown to reduce decalcification during an acidic challenge in vitro. The purpose of this study was to investigate the in vivo effects of argon laser irradiation on enamel decalcification during orthodontic treatment. Nine volunteers whose treatment plans included 4 first premolar extractions were enrolled in the study. The 36 extracted premolars were assigned to 1 of the following 4 groups: group 1, control group with no treatment; group 2 (pumice-laser), teeth were pumiced for 3 seconds and treated with a 325 mW, 5-mm diameter laser beam for 60 seconds; group 3 (pumice-etch-laser), teeth were pumiced for 3 seconds, acid-etched with 30% phosphoric acid for 30 seconds, and treated for 60 seconds with laser; and group 4 (laser only), teeth were treated for 60 seconds with laser. A specially designed (oversized) orthodontic band was fitted on each of the premolars to create a pocket for decalcification. The bands were cemented in place for 5 weeks. After extraction, the teeth were sectioned and examined under polarized light microscopy. Images of lesions were digitally analyzed and measured. Average lesion depths were calculated from 3 depth measurements recorded 10 microm apart. Average lesion area was calculated with the aid of imaging analysis software. Data were analyzed with analysis of variance (P <.05) and Student t tests. Significant differences were found in lesion depth (P <.001) and lesion area (P <.01) among the 4 test groups. The average lesion depths were 15.93 +/- 9.31 microm (control), 6.45 +/- 8.70 microm (pumice-laser), 1.71 +/- 4.82 microm (pumice-etch-laser), and 1.34 +/- 3.80 microm (laser only). The average lesion areas were 1028.67 +/- 725.68 microm (2) (control), 555.49 +/- 948.20 microm (2) (pumice-laser), 79.91 +/- 226.03 microm (2) (pumice-etch-laser), and 55.71 +/- 157.59 microm (2) (laser only). The average lesion depth in the laser-only group was reduced by 94.1% and the average lesion area was reduced by 94.4% when compared with the control group. In the pumice-etch-laser group, the average lesion depth was reduced by 89.1% and the average lesion area was reduced by 92.2% when compared with the control group. There were no significant differences in lesion depth and lesion area between maxillary and mandibular teeth (P <.06 and P <.08, respectively) and between the teeth on the right and left sides (P <.68 and P <.55, respectively). These results show that argon laser irradiation is effective in reducing enamel decalcification during orthodontic treatment. Pumicing and etching do not appear to reduce the effect of laser on enamel solubility.

A mechanical computerized three-dimensional scanner with a resolution of 1 micron was used to assess loss of enamel caused by orthodontic bonding and debonding. A total of 2646 measurements was performed on six human premolars. The results showed an average loss of enamel of 7.4 microns. The range was between 1 and 52 microns, which may account for discrepancies with earlier studies that measured only a few points per tooth surface.

Orthodontic treatment may in some cases initiate damage to teeth and supporting tissues (decalcification and gingivitis) or root resorption, pulp damage, allergic reactions, or craniomandibular dysfunction. This review updates some interesting findings regarding these problems.

Direct bonding of brackets onto the tooth enamel has become a routine procedure in orthodontics. Some detrimental side effects of this technique are described. Most iatrogenic effects are restricted to the enamel and mostly when white spots exist before bonding the brackets. The risk of damage is limited when the etching agent is applied carefully.
