Background: Bulimia nervosa is an eating disorder resulting in an intended weight loss due to decreased food intake, induced vomiting, or hyperactivity, and is observed frequently between 12 and 25 years of age. One of the complications is early tooth loss. Moreover, since bulimia nervosa patients suffer from increased atrophy of the alveolar processes, oral rehabilitation even with short dental implants may be impossible. In these cases, lateralization or transposition of the inferior alveolar nerve (IAN) followed by implant placement can be useful., Case Presentation: A 40-year-old woman with a long-lasting history of bulimia nervosa requested a fixed rehabilitation of her partially edentulous mandible. In 2012, a bilateral IAN transposition approach was performed using piezosurgery, and without any postoperative neurosensory alterations. Two years later, bilateral insertion of each two implants was followed by an inflammatory destabilization of the lower left mandible; subsequent to the implant removal, a fracture occurred, and the latter was stabilized by osteosynthesis plates. In 2017, three additional implants were placed, finally providing the patient with a fixed restoration on five implants., Conclusion: Eating disorders may have a tremendous impact on both physical condition and oral health, resulting in early tooth loss and severe bone atrophy. IAN transposition is a viable treatment option to enable installing fixed prostheses via dental implants, but the latter will clearly increase the risk of inflammation and interruption of mandibular continuity. Close clinical and radiologic monitoring is mandatory to adequately respond to complications such as peri-implant mucositis, peri-implantitis, osteomyelitis, or concomitant fractures.

Purpose: To compare long-term survival and marginal bone loss of immediate interantral implants in the nonaugmented maxilla subjected to immediate vs delayed loading., Materials and Methods: Graftless maxillary cross-arch rehabilitation was performed in a total of 362 patients in the years 2004 to 2013 (1,797 implants). Of the 240 patients with immediate implants replacing their failing maxillary dentition, 81% were subjected to immediate loading and 19% to delayed loading of their 4 to 6 interantral implants (980 and 235 implants, respectively). Kaplan-Meier survival estimates were computed and marginal bone loss was evaluated in a stratified random sample of 20 patients per group., Results: Thirty-one of 1,215 implants failed within the mean observation period of 3.9 years, and no difference in 8-year survival estimates could be seen between immediate (97.6% [95% CI: 96.7 to 98.6]) and delayed (96.6% [95% CI: 94.3 to 98.9]) loading protocols (P = .359). Mean marginal bone resorption following implant insertion did not differ significantly between the groups (1.5 ± 1.7 mm vs 0.7 ± 1.1 mm, P = .379); however, it was significantly associated with a reduced number of implants (P = .017) and patient history of periodontal disease (P < .001)., Conclusion: Immediate loading of interantral implants yields satisfactory results in the transition of patients from a failing maxillary dentition to full-arch implant rehabilitation and thus may be favored over delayed loading concepts.

This clinical report demonstrated the use of an implant-supported fixed dental prosthesis fabricated with a contemporary digital approach. The digital diagnostic data acquisition was completed with a digital diagnostic impression with an intraoral scanner and cone-beam computed tomography with a prefabricated universal radiographic template to design a virtual prosthetically driven implant surgical plan. A surgical template fabricated with computer-aided design and computer-aided manufacturing (CAD/CAM) was used to perform computer-guided implant surgery. Rare disorders are conditions defined as either fatal or causing chronic functional impairment. There is limited evidence of the behavior of oral implants in patients suffering from such diseases, and the existing data are merely based on cases reports. This paper describes the outcome of implant treatment after up to 15 years of function in three patients afflicted with X-linked hypophosphatemia, two patients with Langerhans cell histiocytosis, and one patient with osteogenesis/dentinogenesis imperfecta.

To prove the efficiency of dental implantation in complex treatment of patients with class III malocclusion and partial teeth loss. Nine patients aged 18-35 years with class III malocclusion and partial teeth loss were included in the study. All patients received two-step treatment involving orthodontic correction with two-step implantation and provisional restorations placement followed by orthognatic surgery procedure. The patients were followed-up for three years with no recurrent malocclusion or implant failure revealed, thus proving dental implantation to be useful tool in complex oral rehabilitation of patients with class III malocclusion and partial teeth loss.

The paper presents the results of 20 years clinical experience in dental prosthetics with the use of porous titanium nickelide dental implants. Implants were placed in 565 patients aged 15-17 years. Long-term results analysis showed restorations to function properly after mean follow-up period of 8 years in 435 patients (78.2%) proving this material to be clinically successful.

Purpose: To present results of a survey on the status of an implantology amongst implant-practicing dentist across the world in 2009., Materials and Methods: A questionnaire was sent to the members of EAO (European Association of Osseointegration), ICOI (International Congress of Osseointegrated Implants), ISOI (Indian Society of Oral Implantologists), Asian Academy of Osseointegration (AAO), Deutsche Gasellschaft Fur Orale Implantologie (DGOI), Philippines Implant Organization, Korean Society of Oral Implantologist, Japanese Association of OralIimplantologists, Chinese Dental Association, Pakistan Dental Association, asking for the personal (anonymous) background data and their implantology concepts. Specific questions dealt with level of recognition of implants, use of implants, superstructures, techniques followed, and materials used., Results: A total of 1500 (63.6%) of the 2358 questionnaires were answered. Dental implants were the most preferred treatment modality for restoring the missing teeth. Threaded implants were the most preferred. Cement retained implant prosthesis was the most preferred restoration procedure. Dentists believe that the general dentist should practice dental implant treatment modality, preferably teamwork. Immediate loading was the much-accepted concept among the dentists of the developed nations., Conclusion: Dental implants were much accepted treatment modality for the replacement of missing teeth. Most the dentists follow the well documented technique and proven materials, which have been documented in the literature, an evidenced based practice, thus, delivering the best to their patients. Dentists from the developing nations agreed to have standardization in implants.

Loss of an anterior tooth as the result of trauma is relatively common, especially in the young. Orthodontic treatment is not always the definitive therapy of choice; instead, an implant can be placed to replace the missing tooth. This article describes the case of a 19-year-old woman whose maxillary left central incisor was replaced by an immediately restored dental implant, placed using flapless surgery. The implant achieved excellent primary stability, as determined by resonance frequency analysis, so it was immediately restored with a provisional acrylic resin crown in no centric occlusion. This case report supports the use of single implants for replacing a missing anterior tooth where esthetics are a priority.

Bone volume is limited by the presence of the maxillary sinus together with loss of alveolar bone height. Sinus lift procedures increase bone volume by augmenting the sinus cavity with autogenous bone and/or commercially available biomaterials., Objectives: To test whether and when augmentation of the maxillary sinus is necessary and which are the most effective augmentation techniques for rehabilitating patients with implant-supported prostheses., Search Methods: The Cochrane Oral Health Group's Trials Register, the Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE and EMBASE were searched. Several dental journals were hand searched. The bibliographies of review articles were checked, and personal references were searched. More than 55 implant manufacturing companies were also contacted. The last electronic search was conducted on 7th January 2010., Selection Criteria: Randomised controlled trials (RCTs) of different techniques and materials for augmenting the maxillary sinus for rehabilitation with dental implants reporting the outcome of implant therapy at least to abutment connection., Data Collection and Analysis: Screening of eligible studies, assessment of the methodological quality of the trials and data extraction were conducted independently and in duplicate. Authors were contacted for any missing information. Results were expressed as random-effects models using mean differences for continuous outcomes and odds ratios for dichotomous outcomes with 95% confidence intervals. The statistical unit of the analysis was the patient., Results: Ten RCTs out of 29 potentially eligible trials were suitable for inclusion. One trial including 15 patients, evaluated whether 5-mm-long implants with a diameter of 6 mm could be an alternative to sinus lift in bone having a residual height of 4 to 6 mm. Nine trials with 235 patients compared different sinus lift techniques and, of these, four trials (114 patients) evaluated the efficacy of platelet-rich plasma (PRP). Since different techniques were evaluated in different trials, only two meta-analyses evaluating the efficacy of PRP could be performed for implant failures (two trials) and complications (three trials). No statistically significant difference was observed for any of the evaluated interventions., Conclusions: Conclusions are based on few trials, usually underpowered, having short follow-ups, and often judged to be at high risk of bias, therefore they should be viewed as preliminary and interpreted with great caution. It is still unclear when sinus lift procedures are needed. Short implants (5 mm) can be successfully loaded in maxillary bone with a residual height of 4 to 6 mm, but their long-term prognosis is unknown. Elevating the sinus lining in the presence of 1 to 5 mm of residual bone height without the addition of a bone graft may be sufficient to regenerate new bone to allow rehabilitation with implant-supported prostheses. Bone substitutes might be successfully used as replacements for autogenous bone. If the residual alveolar bone height is 3 to 6 mm, a crestal approach to lifting the sinus lining and placing 8 mm implants may lead to less complications than a lateral window approach and placing implants at least 10 mm long. PRP treatment does not seem to improve the clinical outcome of sinus lift procedures with autogenous bone or bone substitutes.

The principal reason for providing periodontal therapy is to achieve periodontal health and retain the dentition. Patients with a history of periodontitis represent a unique group of individuals who previously succumbed to a bacterial challenge. Therefore, it is important to address the management and survival rate of implants in these patients. Systematic reviews often are cited in this article, because they provide a high level of evidence and facilitate reviewing a vast amount of information in a succinct manner.

Purpose: This retrospective study assessed implant and prosthodontic treatment outcomes of patients suffering from rheumatic disorders such as rheumatoid arthritis (RA) and connective tissue diseases (CTDs)., Materials and Methods: This study included 22 patients (all women) suffering from autoimmune rheumatic disorders such as isolated RA (n = 16), RA with concomitant CTDs (n = 5), or isolated CTDs (n = 1). Overall, 89 implants were placed for rehabilitations such as single-tooth replacement (n = 8), fixed partial dentures (n = 14), complete dentures (n = 5), and overdentures (n = 2), and were evaluated after a mean of 42.6 +/- 25.2 months. The cumulative implant survival and success rates and peri-implant conditions (marginal bone loss, pocket depth, Plaque Index, Gingival Index, Bleeding Index, and Calculus Index) were evaluated with a special focus on RA and CTDs. In addition, incidence and type of prosthodontic maintenance were evaluated., Results: A high implant survival rate was noted during follow-up with a cumulative 3-year implant success rate of 96.1%. Patients with RA demonstrated acceptable marginal bone resorption (mean: 2.1 +/- 0.5 mm) and good soft tissue conditions, while CTD patients showed increased bone resorption (mean: 3.1 +/- 0.7 mm). This was especially noted in scleroderma patients, as were major peri-implant soft tissue alterations (Bleeding Index) in patients suffering from Sjogren syndrome., Conclusions: A high implant and prothodontic success rate can be anticipated even for patients suffering from autoimmune rheumatic disorders such as RA and CTDs. A scrupulous maintenance program that includes optimal oral hygiene could assist in ensuring stable long-term results for CTD patients with more vulnerable soft tissue conditions. The presented case report describes a systematic treatment concept, the treatment sequence and the maintenance care of a sixty-seven-year old female patient suffering from generalized chronic periodontitis with advanced attachment loss. Due to a pretherapeutic risk assessment, several teeth were classified as "irrational to treat" or "doubtful". Therefore, a comprehensive reconstructive rehabilitation after active periodontal therapy was necessary. The restoration of a periodontally compromised dentition represents various difficulties. The patient wished to have fixed reconstructions. Depending upon anatomical conditions several different reconstructions were used: conventional bridges, partly using a resected molar root, a solely implant-retained bridge and a combined implant-tooth-retained bridge.

The treatment sequel of post-traumatic teeth for the use of dental implants in the anterior maxillary region to rehabilitate anterior maxillary missing teeth was evaluated. Files of 53 healthy patients reporting anterior dental trauma were reviewed. All patients had an anterior maxillary dental implant because of tooth loss after trauma. At initial examination, 18 patients (34%) had root canal treatment and an inflammatory lesion, 15 (28.3%) had a missing tooth on admission, 12 (22.6%) had a prior operation (i.e. root-end surgery or crown lengthening), 4 (7.5%) presented an ankylotic root, and 4 (7.5%) had a root remnant not suitable for rehabilitation, with no inflammatory periapical lesion. Treatment sequences and complications were recorded. Augmentation procedure (i.e. onlay bone graft or guided bone regeneration) was performed in 43 patients (81.1%), and 2 patients (3.8%) had orthodontic extrusion prior to tooth extraction and implantation. Implants were placed immediately in 25 patients (47.2%) and 4 (7.5%) had immediate loading at the time of implantation. Complications and postoperative incidents (fistula, inflammation, swelling hematoma, etc.) were observed in 24 patients (45.3%). There was no difference in complication and postoperative incident rates with regards to the implantation technique. Complications were found at the prosthetic phase in seven patients (13.2%; six fistula and one implant failure). When patients were divided into two groups, with and without an inflammatory lesion, a significantly lower complication and postoperative incident rate were found in the non-inflammatory group (P = 0.057). This study reaffirmed the necessity for scrupulous diagnosis of teeth and alveolar bone after a traumatic injury. Treatment is multidisciplinary, requiring surgical, orthodontic, endodontic, operative, and prosthetic compliance. A specially designed treatment plan for each patient is necessary. General rules do not apply.

The anterior maxilla is the most traumatized region during childhood. Posttraumatic complications occasionally lead to tooth loss as well as the need for future implants. Unfortunately, it is contraindicated to place dental implants during childhood. A waiting period of approximately 8 to 10 years before completion of growth is required. For this patient to become a candidate for future dental implants, it is necessary to ensure the continuous growth and to preserve the dimensions of the alveolar process until growth has ceased from time of injury until joint maturation. To achieve these goals, it is essential to coordinate the treatment sequence at the time of trauma. After loss of a traumatized anterior permanent maxillary incisor in young adults, treatment options are limited: orthodontic closure of the gap and reshaping the adjacent teeth, or tooth extraction and maintaining the gap with a temporary restoration. Orthodontic space closure has limited indications and requires prosthetic restoration of at least 2 teeth. Extraction and temporary restoration will usually lead to bone augmentation before implantation. Other possible treatment options include orthodontic extrusion of the root remnant (in cases of untreatable root fracture or complicated crown-root fracture) and a temporary crown to serve the patient until the completion of growth and development, autogenous tooth transplantation, intentional extraction and immediate tooth replantation, distraction osteogenesis, and decoronation. Because general rules do not apply, individual treatment plans are necessary.

The aim of this study was to evaluate the results of single-tooth implant treatment in patients where teeth have been lost as a result of trauma. Also, the patients' and professionals' opinions regarding the final outcome of treatment were assessed. Thirty-four patients with 42 lost teeth were evaluated by clinical and radiographic examinations and interviews 2-5 years after treatment. A professional who had not taken part in the treatment evaluated the implant crowns. Central maxillary incisors were the most frequently lost and replaced teeth after trauma (75%) followed by lateral incisors (21%). In patients with incomplete growth, implant treatment was generally postponed until completion of growth. Lack of space was treated by presurgical orthodontics (7%) or by selecting an implant with a reduced diameter (5%). Deficiency of bone was seen in 17% and was treated by bone grafting or local augmentation prior to implant surgery. Patients who had lost two or more teeth after trauma were all subjected to bone grafting. Preservation of roots in the alveolar process seemed to maintain the bone volume enabling better conditions for later implant placement. Forty-one implants (97.6%) were integrated successfully. Complications were few and of minor importance (9.5% before and 12% after cementation of crowns) and could all be managed. No or minimal bone loss was seen. In general, the patients felt that they received good care and that they were well informed about their treatment. Some patients reported that the local anesthesia procedure was not pain-free, but 71% of the patients experienced the treatment as pain-free. For each of the variables (color, shape, height, and size of the crowns), the highest degree of satisfaction was noted in 93-98% of the patients and 91-95% of the single evaluating professional. Given that the patients have finished growth and a careful treatment planning and timing are performed, the functional and esthetical outcome of single-tooth implant treatment today is excellent and can be recommended for replacing tooth losses after trauma in the anterior region of the maxilla.

Background: It is difficult to determine the effectiveness or potential harm of dental therapies. Thus, any tools able to condense reliable scientific information would be of benefit., Purpose: To discuss methods for the assessment of the scientific literature and, in particular, of systematic reviews for evaluating the effectiveness of oral implant rehabilitation procedures., Materials and Methods: Various study designs and methods to identify scientific evidence are described, discussed, and ranked. Issues on how to critically appraise randomized controlled clinical trials (RCTs) and systematic reviews are presented., Results: Properly conducted RCTs and structured critical systematic reviews are the gold standard of clinical research for assessing whether a therapeutic intervention is effective. In the field of oral implantology, there is an urgent need to implement more RCTs and to summarize their results in systematic reviews., Conclusions: Searching of the truth requires training and critical skill. Dentists should be trained on how to integrate their clinical experience with evidence-based research and on how to discriminate between clinically useful scientific information and less useful research.

This article describes the possibilities and difficulties of reconstruction in the front regio by means of implants and bone transplants in five patients. First of all, planning is the most important step. Sometimes the class III relationship is a real challenge. Different bone transplants, the inlay and onlay technique, the number of the implants, the importance of the way the implants are uncovered in the second stage are discussed. In the front region, we aim for functional and esthetic reconstruction.

Replacing small, single incisors with implants can be esthetically challenging and difficult because of the limited amount of bone. In this investigation, 3.0-mm-diameter implants were used to support 30 single maxillary and mandibular incisors in 21 patients. The implants have been in function for 3 to 7 years, and 29 are still stable. Only 2 complications in the mandibular incisor region have occurred; 1 implant fractured (after 5 years of function) and 1 prosthesis was replaced. The overall success rate is 96.7%. The favorable results and esthetic appearance achieved suggest that replacing small incisors where light occlusal forces are present with narrower implants is a feasible treatment option.

The loss of teeth is an extremely traumatic and upsetting experience. The accompanying feelings of grief and loss are compounded by the inherent inadequacies of complete dentures. It is hardly surprising that many edentulous people and their dentists have searched long and hard for a viable alternative. This article places in perspective the shift that has occurred in the therapeutic paradigm of treatment as a result of osseointegrated implant therapy.

This study reports the results of implant therapy involving a sinus membrane lift as well as conventional implant therapy in 24 periodontally compromised patients who were treated during the period between June 1990 and May 1995. Patients were included on the basis of being in need of at least 2 implants, 1 of which was to be placed in the maxillary sinus region, and at least 3 mm bone height was required in the sinus region. Following fenestration of the lateral sinus wall and lifting of the sinus membrane sinus implants were inserted as described for conventional implants. Annual follow-up visits included recording of plaque, probing pocket depth and bleeding on probing, and recording of the radiographic distance from the implant shoulder to the alveolar crest in mm. The Astra, Astra sinus, ITI, and ITI sinus were observed for an average of 30.8, 29.9, 29.4 and 25.3 months, respectively. Of the 80 implants inserted 1 Astra and 2 ITI had failed at 11-12 months and 1 ITI had failed at 42 months. The Kaplan-Meier estimate of the proportion of non-failed implants at 36 months were 100%, 95%, 91%, and 86% for the Astra, Astra sinus, ITI, and ITI sinus implants, respectively. Of the Astra, Astra sinus, and ITI implants, 71-82% remained free from bone-loss > or = 1.5 mm at 36 months, whereas this was the case for only 29% of the ITI sinus implants. About 80-90% of the implants remained free from plaque; 65-86% remained free from bleeding; and 44-80% remained free from pockets > or = 4 mm. These results indicate that the sinus lift technique can be used successfully in periodontally compromised patients.

Statement of Problem: Treatment of tooth loss in the anterior maxilla can involve difficult functional, esthetic, and psychologic problems, especially in young patients with otherwise good dentition., Purpose: The purpose of this study was to provide a preliminary comparative evaluation of two implants (ITI and Astra) in single-tooth restorations., Material and Methods: This prospective study of 102 single-tooth replacements with 56 ITI and 46 Astra dental implants was performed in 82 patients at the Finnish Student Health Service Foundation. One Astra implant was lost before loading. The overall survival rate of the implants was 97.8% for Astra implants and 100% for the ITI system. After the initial healing period of at least 6 months, the remaining 101 implants (56 ITI, 45 Astra) were free of periimplant infection and revealed no detectable mobility. Radiographs did not reveal signs of periimplant radiolucencies. All 101 implants received single-tooth crowns., Results: Periimplant parameters and acceptable implant function were examined and demonstrated satisfactory results with preestablished clinical parameters and radiographs at 1 year. During the observation time the mean marginal bone loss was 0.13 mm with Astra implants and 0.11 mm with ITI implants. Subjectively all patients were satisfied with their single-tooth restorations supported by either ITI or Astra dental implants., Conclusion: The favorable results of this short-term study support the application of the two implant systems for single-tooth restorations, especially in the anterior region of the maxilla.

Edentulism is often reported after treatment for malignancy of oral cavity. The resulting deformities from loss of teeth and bone create a "social invalid" with difficulties in articulation and mastication. Prosthetic rehabilitation was difficult because of two problems: the risk of osteoradionecrosis development and local anatomic modifications. The introduction of osseointegrated implants improved results of oromandibular reconstruction. A preliminary report of nineteen Branemark implants of seven patients is reported here. Implants have been in place and functioning for a period of one to four years. No major complication occurred and osseointegration permitted good oral rehabilitation. Placement of osseointegrated implants in osteomyocutaneous flaps and in irradiated bone is discussed.

The general aim of the thesis was to study single-tooth replacements with implants using a recently developed prosthodontic concept, CeraOne. More specifically, the aims were to present prosthodontic procedures and early clinical experiences of CeraOne, and to study the following: the acceptance and the rejection of referred patients, the mechanical strength of the complete implant post and of superstructures on the CeraOne abutment, the treatment outcome from an on-going prospective clinical investigation, the effect of guided tissue regeneration when using e-PTFE membranes in narrow alveolar ridges, and finally, the outcome from treatments by general practitioners (GPs) who had performed the complete care of the single-tooth patient. The acceptance and the rejection of patients were studied on 184 patients, and the conclusions were based on data from referral, case history, clinical examination, and therapy planning. External trauma, root fractures, and congenitally missing teeth were the main reasons for lost or missing teeth in referred patients, while anatomical limitations were the most frequent reasons for the rejections. The mechanical strength was determined after testing in an Instron universal testing machine. The abutment screw was shown to be the weakest link, while a satisfactory safety margin against fractures was observed for the metal-ceramic and the all-ceramic crowns. It was also shown that the all-ceramic ¿ crown could resist bending moments up to a level that was more than three times as high as the one at which the porcelain crown fractured. In the clinical prospective study, the results from treatments of 57 patients (65 implants) were reported after two years and from 34 patients (37 implants) after three years. Favourable survival rates were shown for the implants (97.3% at 3-4 years) and, as a mean, only a small bone loss around the implants and the adjacent teeth was observed. The conical implants, however, showed a higher degree of bone los in comparison to the other implants. The CeraOne prosthodontic concept was simple to sue because of prefabricated components, and favourable aesthetic and clinical short-term results were achieved. Problems of screw loosening and fistulas were avoided and the cementation of the crown and the subgingival placement of the abutment shoulder did not cause any soft tissue recession. Fifteen patients (20 implants) with a suspected need of guided tissue regeneration were selected for the study using e-PTFE membranes. After treatment and at the second stage surgery, it was shown that most of the primarily exposed implant surfaces at this time were covered by a newly formed tissue with signs of osteoid activity.(ABSTRACT TRUNCATED AT 400 WORDS)

The use of osseointegrated implants is an accepted procedure for the treatment of total and partial edentulism and offers good predictability of long-term success. For the last several years, this procedure has also been used for single-tooth replacement following loss of teeth for traumatic, periodontal, and carious lesions. This paper reports the clinical experience of 47 patients treated for a single-tooth replacement. Sixty-seven screw-type standard, self-tapping implants and mini-implants were placed. The results were similar to those for complete and partial osseointegrated prostheses. The total implant survival rate was 94.4%. Two implants were lost as a result of suppuration caused by membrane exposure, while another was lost after loading. The results suggest that single-tooth replacement using osseointegrated implants provides predictable success.

The functional and aesthetic rehabilitation of a single missing anterior tooth presents a surgical and prosthetic challenge. This article describes reconstructive and restorative techniques applied to the rehabilitation of such a case, using an osseointegrated implant, guided bone regeneration, mucogingival preprosthetic plastic surgery, and an all-ceramic restoration. The learning objective of this article is to enhance the knowledge of single-tooth implant restorative approaches and procedures.

After dental loss, occlusal reconstruction is required to prevent dental migration, to reestablish masticatory function and to protect the temporo-mandibular joint. Maxillary implants using osteointegrated biocompatible materials (Titanium especially) and scrupulously respecting the Branemark principles, allows this aim to be achieved in a high percentage of cases.
