1. Ozone therapy improves early stages of osseointegration in ovariectomized rats.

Abstract

Objective: the aim of this study was to analyze the influence of ozone therapy (OZN) on peri-implant bone repair in critical bones by installing osseointegrated implants in the tibia of ovariectomized rats., Methodology: ovariectomy was performed on 30 Wistar rats, aged six months (Rattus novergicus), and, after 90 days, osseointegrated implants were installed in each tibial metaphysis. The study groups were divided into the animals that received intraperitoneal ozone at a concentration of 700 mcg/kg - OZ Group (n=15) - and a control group that received an intraperitoneal saline solution and, for this reason, was named the SAL group (n=15). The applications for both groups occurred during the immediate post-operative period on the 2nd, 4th, 6th, 8th, 10th, and 12th day post-surgery. At various stages (14, 42, and 60 days), the animals were euthanized, and tests were performed on their tibiae. These tests include histomorphometric and immunohistochemical analyses, computerized microtomography, sampling in light-cured resin for calcified sections, and confocal microscopy. The obtained data were then analyzed using One-way ANOVA and the Shapiro-Wilk, Kruskal-Wallis, and student t-tests (P<0.05)., Results: our findings indicate that the OZ group (3.26±0.20 mm) showed better cellular organization and bone neoformation at 14 days (SAL group, 0.90±1.42 mm) (P=0.001). Immunohistochemistry revealed that osteocalcin labeling was moderate in the OZ group and mild in the SAL group at 14 and 42 days post-surgery. The data from the analysis of calcified tissues (microtomography, histometric, and bone dynamism analysis) at 60 days showed no statistically significant differences between the groups (P=0.32)., Conclusion: it was concluded that ozone therapy anticipated the initial phases of the peri-implant bone repair process.

Abstract

This study investigated the impact of adding hydroxyapatite nanoparticles to implant surfaces treated with zirconia blasting and acid etching (ZiHa), focusing on structural changes and bone healing parameters in low-density bone sites. The topographical characterization of titanium discs with a ZiHa surface and a commercially modified zirconia-blasted and acid-etched surface (Zi) was performed using scanning electron microscopy, profilometry, and surface-free energy. For the in vivo assessment, 22 female rats were ovariectomized and kept for 90 days, after which one implant from each group was randomly placed in each tibial metaphysis of the animals. Histological and immunohistochemical analyses were performed at 14 and 28 days postoperatively (decalcified lab processing), reverse torque testing was performed at 28 days, and histometry from calcified lab processing was performed at 60 days The group ZiHa promoted changes in surface morphology, forming evenly distributed pores. For bone healing, ZiHa showed a greater reverse torque, newly formed bone area, and bone/implant contact values compared to group Zi (p < 0.05; t-test). Qualitative histological and immunohistochemical analyses showed higher features of bone maturation for ZiHa on days 14 and 28. This preclinical study demonstrated that adding hydroxyapatite to zirconia-blasted and acid-etched surfaces enhanced peri-implant bone healing in ovariectomized rats. Abstract

Osteoporosis is a skeletal disorder that affects bone tissue density and predisposes to pathological fractures due to increased fragility. The treatment of this condition is through the use of drugs that can induce osteonecrosis of the jaws due to their interference in osteoclastogenesis, sometimes with exposure and sequestration of this necrotic bone. Thus, the objective of this work is a case report of a 63-year-old patient, who was being treated for osteoporosis with alendronate sodium for approximately 10 years, who presented loss of dental implants due to the occurrence of osteonecrosis of the jaws, the which was discovered after attempted rehabilitation. The patient underwent removal of implants, curettage of bone sequestration and removal of friable soft tissue. After 60 days, she presented with closed and normal colored mucosa, with no exposure of bone tissue and pain. This evidences that sequestrectomy can be used to effectively treat MRONJ and shows the importance of recognizing the factors related to the development of this alteration in bone metabolism and its implications for carrying out rehabilitative therapies. However, it is imperative that professionals be aware of possible changes in the bone tissue and soft tissue covering, considering the long half-life of these drugs. La osteoporosis es un trastorno esquelético que afecta la densidad del tejido óseo y predispone a fracturas patológicas debido al aumento de la fragilidad. El tratamiento de esta afección es mediante el uso de medicamentos que pueden inducir osteonecrosis de los maxilares por su interferencia en la osteoclastogénesis, a veces con exposición y secuestro de este hueso necrótico. Así, el objetivo de este estudio es el reporte de un caso de un paciente de 63 años, quien estuvo en tratamiento por osteoporosis con alendronato sódico durante aproximadamente 10 años, quien presentó pérdida de implantes dentales por ocurrencia de osteonecrosis de los maxilares, el cual fue descubierto después de un intento de rehabilitación. El paciente se sometió a la extracción de implantes, legrado del secuestro óseo y extracción de tejido blando friable. A los 60 días presentó mucosa cerrada y de color normal, sin exposición de tejido óseo y dolor. Esto evidencia que la secuestrectomía se puede utilizar para tratar eficazmente la MRONJ y muestra la importancia de reconocer los factores relacionados con el desarrollo de esta alteración en el metabolismo óseo y sus implicaciones para la realización de terapias rehabilitadoras. Sin embargo, es imperativo que los profesionales estén al tanto de posibles cambios en el tejido óseo y el recubrimiento de tejidos blandos, considerando la larga vida media de estos fármacos. A osteoporose é um distúrbio esquelético que afeta a densidade do tecido ósseo e predispõe a fraturas patológicas devido ao aumento da fragilidade. O tratamento desta condição se dá através do uso de medicamentos que podem induzir a osteonecrose dos maxilares devido a sua interferência na osteoclastogênese, por vezes com exposição e sequestro desse osso necrótico. Dessa forma, o objetivo desse trabalho é um relato de caso de uma paciente, de 63 anos de idade, sob tratamento de osteoporose com alendronato de sódio por aproximadamente 10 anos, que apresentou perda dos implantes dentários devido a ocorrência de osteonecrose dos maxilares, a qual foi descoberta após tentativa de reabilitação. A paciente passou por remoção dos implantes, curetagem dos sequestros ósseos e remoção dos tecidos moles friáveis. Após 60 dias, a mesma apresentou-se com a mucosa fechada e normocorada, com ausência de exposição de tecido ósseo e dor. O que evidencia que a sequestrectomia pode ser utilizada para tratamento de MRONJ de modo efetivo e mostra a importância do reconhecimento dos fatores relacionados ao desenvolvimento desta alteração do metabolismo ósseo e suas implicações para realização de terapias reabilitadoras. Entretanto, é imperativo que os profissionais estejam atentos as possíveis alterações do tecido ósseo e do tecido mole de recobrimento, tendo em vista a meia vida longa desses medicamentos.
