This study aimed to verify marginal fit and the effect of cement film thickness standardization on retention of provisional crowns made with prefabricated acrylic cylinders on abutments, using two temporary luting agents subjected or not to mechanical cycling. Provisional crowns were made from bis-acryl (Luxatemp Fluorescence) or methyl methacrylate (Duralay) resins on acrylic cylinders and marginal fit and cement film thickness were evaluated. For retention evaluation, crowns were cemented with two temporary luting agents: non-eugenol zinc oxide (Tempbond NE) or calcium hydroxide-based (Hydcal) cements and subjected to tensile strength in a universal testing machine. After cleaning, debonded crowns were cemented again, subjected to mechanical cycling and retention was reassessed. The results of marginal fit and cement film thickness were analyzed by Student's t-test while retention of cements before and after mechanical cycling was analyzed using a mixed linear model. Methyl methacrylate crowns presented greater marginal misfit (p=0.001) and occlusal cement film thickness (p=0.003) than the bis-acryl ones. No difference was observed at axial cement film thickness (p=0.606). Resins (p=0.281) did not affect crown retention, but luting agents (p=0.029) and mechanical cycling (p=0.027) showed significant effects. The only significant interaction was mechanical cycling*luting agents, which means that luting agents were differently affected by mechanical cycling (p=0.002). In conclusion, the results showed that bis-acryl resin associated to calcium-hydroxide luting agent provided the best retention and lower cement thickness.

Purpose: This study compared the loosening torque of experimental conical-head abutment screws to that of conventional flat-head screws of implants with external-hex (EH) and internal tri-channel (IT) connections before and after mechanical loading., Materials and Methods: Forty-four implant/screw/abutment assemblies were divided into four groups: EH/flat-head screw (EH), IT/flat-head screw (IT), EH/conical-head screw (EHCS), and IT/conical-head screw (ITCS). Three assemblies from each group were analyzed in a stereoscopic magnifier and then returned to their respective groups. One assembly was removed from each group and transversely sectioned for optical microscopy analysis. Abutments were tightened at 32 Ncm of torque; after 10 minutes, loosening torque was measured. The same abutments were then retightened with 32 Ncm of torque; after 10 minutes, they were mechanically loaded for 300,000 cycles and loosening torque was again measured. Data were collected and an exploratory analysis was performed. Comparisons were made by orthogonal contrasts using a linear mixed effects model (random and fixed effects)., Results: Comparisons before mechanical cycling showed significant differences between groups except for EH vs IT and EHCS vs ITCS. After mechanical cycling, only the comparisons EH vs IT and EHCS vs ITCS showed no significant differences. Considering the same group, before and after mechanical cycling, all comparisons showed significant differences., Conclusions: The shape of the abutment screw head significantly influenced loosening torque: conical-head screws showed higher loosening torque values than conventional flat-head screws before and after loading. The implant/abutment connection design exerted no significant influence on loosening torque.

Objective: This study aimed to evaluate the importance of a distal proximal contact on the load transfer to the posterior region of the mandible by non-splinted adjacent implant-supported crowns using photoelastic stress analysis., Material and Methods: A rectangular model (68x30x15 mm) was made of polymethylmethacrylate resin to simulate half of the mandibular arch. One model was completed with resin replicas representing the first premolar and second molar and with two 3.75 mm dia.x11 mm internal hexagon threaded implants replacing the second premolar and first molar. The other model was manufactured in the same way but without the second molar. Both models were duplicated using photoelastic resin. The roots of the teeth replicas were covered with a layer of polyether impression material to simulate the periodontal ligament. Two different vertical loads were applied to the crowns as follows: 1 - single static point load alternately applied to the crowns replacing the second premolar and first molar (50 N); 2 - simultaneous static point loads applied to both of the crowns replacing the second premolar and first molar (100 N). The resulting isochromatic fringe pattern in the photoelastic model was monitored and photographed., Results: All loading conditions studied showed that the presence of the second molar has changed the load transmission and the pattern of stresses., Conclusion: Results showed that the presence of a second molar proximal contact can help minimize the stresses around the implants.

Objectives: The aim of this study was to compare the stresses resulting from cemented or screw-retained dental prostheses using universal cast to long abutment by the photoelastic method., Material and Methods: A photoelastic resin model was fabricated to simulate a segment of the mandibular arch with resin replicas of a first premolar and a second molar, and two 3.75 × 11-mm implants replacing the second premolar and the first molar. Vertical static loads were individually applied on the occlusal surface of the premolar and the molar (5 kgf) and simultaneously on both crowns (10 kgf)., Results: Results found variable stress patterns around each implant and concentrated stresses in the individually loaded crowns. Force transfer characteristics of each situation and quantification of the isochromatic fringes were evaluated., Conclusions: Within the limitations of this study and based on the results obtained for implant-supported prosthesis with internal hexagon connection/universal cast to long abutment, it could be concluded that single adjacent screw-retained prosthetic restorations lead to higher stresses around the implants when compared with cement-retained restorations.

This study compared vertical and passive fit of one-piece cast frameworks made with 3 different materials: commercially pure titanium (CP Ti - G1), cobalt-chromium alloy (Co-Cr - G2) and nickel-chromium-titanium alloy (Ni-Cr-Ti - G3). Fifteen frameworks were obtained simulating bars for fixed prosthesis in a model with 5 implants. The passive and vertical fit of the framework interface was measured using an optical microscope at x30 magnification. Data were statistically analyzed by ANOVA and LSD tests (α=0.05). Mean and standard deviation values for passive fit and vertical fit were, respectively: G1 [472.49 (109.88) µm and 29.9 (13.24) µm], G2 [584.84 (120.20) µm and 27.05 (10.30) µm], and G3 [462.70 (179.18) µm and 24.95 (11.14) µm]. For vertical fit, there were no significant differences among G1, G2 and G3 (p=0.285). There were no significant differences for passive fit between G1 and G3 (p=0.844), but both differed significantly from G2 (p=0.028 and p=0.035, respectively), which showed the highest misfit values. It may be concluded that the vertical fit of frameworks was not affected by the tested materials, and that one-piece cast frameworks resulted in inadequate passive fit. The Co-Cr alloy presented the worst values for passive fit.

Objective: The purpose of this study was to evaluate the retention force of an O-ring attachment system in different inclinations to the ideal path of insertion, using devices to compensate angulations., Material and Methods: Two implants were inserted into an aluminum base, and ball attachments were screwed to implants. Cylinders with O-rings were placed on ball attachments and connected to the test device using positioners to compensate implant angulations (0 degrees , 7 degrees , and 14 degrees ). Plexiglass bases were used to simulate implant angulations. The base and the test device were positioned in a testing apparatus, which simulated insertion/removal of an overdenture. A total of 2900 cycles, simulating 2 years of overdenture use, were performed and 36 O-rings were tested. The force required for each cycle was recorded with computer software. Longitudinal sections of ball attachment-positioner-cylinder with O-rings of each angulation were obtained to analyze the relationship among them, and O-ring sections tested in each angulation were compared with an unused counterpart. A mixed linear model was used to analyze the data, and the comparison was performed by orthogonal contrasts (alpha=0.05)., Results: At 0 degrees , the retention force decreased significantly over time, and the retention force was significantly different in all comparisons, except from 12 to 18 months. When the implants were positioned at 7 degrees , the retention force was statistically different at 0 and 24 months. At 14 degrees , significant differences were found from 6 and 12 to 24 months., Conclusions: Within the limitations of this study, it was concluded that O-rings for implant/attachments perpendicular to the occlusal plane were adequately retentive over the first year and that the retentive capacity of O-ring was affected by implant inclinations despite the proposed positioners.

Purpose: The aim of this study was to compare the accuracy of fit of three types of implant-supported frameworks cast in Ni-Cr alloy: specifically, a framework cast as one piece compared to frameworks cast separately in sections to the transverse or the diagonal axis, and later laser welded., Materials and Methods: Three sets of similar implant-supported frameworks were constructed. The first group of six 3-unit implant-supported frameworks were cast as one piece, the second group of six were sectioned in the transverse axis of the pontic region prior to casting, and the last group of six were sectioned in the diagonal axis of the pontic region prior to casting. The sectioned frameworks were positioned in the matrix (10 N.cm torque) and laser welded. To evaluate passive fit, readings were made with an optical microscope with both screws tightened and with only one-screw tightened. Data were submitted to ANOVA and Tukey-Kramer's test (p < 0.05)., Results: When both screws were tightened, no differences were found between the three groups (p > 0.05). In the single-screw-tightened test, with readings made opposite to the tightened side, the group cast as one piece (57.02 +/- 33.48 mum) was significantly different (p < 0.05) from the group sectioned diagonally (18.92 +/- 4.75 microm) but no different (p > 0.05) from the group transversally sectioned (31.42 +/- 20.68 microm). On the tightened side, no significant differences were found between the groups (p > 0.05)., Conclusions: Results of this study showed that casting diagonally sectioned frameworks lowers misfit levels of prosthetic implant-supported frameworks and also improves the levels of passivity to the same frameworks when compared to structures cast as one piece.

This study compared the vertical misfit of 3-unit implant-supported nickel-chromium (Ni-Cr) and cobalt-chromium (Co-Cr) alloy and commercially pure titanium (cpTi) frameworks after casting as 1 piece, after sectioning and laser welding, and after simulated porcelain firings. The results on the tightened side showed no statistically significant differences. On the opposite side, statistically significant differences were found for Co-Cr alloy (118.64 microm [SD: 91.48] to 39.90 microm [SD: 27.13]) and cpTi (118.56 microm [51.35] to 27.87 microm [12.71]) when comparing 1-piece to laser-welded frameworks. With both sides tightened, only Co-Cr alloy showed statistically significant differences after laser welding. Ni-Cr alloy showed the lowest misfit values, though the differences were not statistically significantly different. Simulated porcelain firings revealed no significant differences.

Several formulations of alternative alloys have been proposed for the substitution of gold-based alloys used in Dentistry. Recently, a Ni-Cr-Ti-based alloy has been introduced. The purpose of this work was to verify the marginal adaptation obtained with one-piece superstructures for implant-supported prostheses obtained in Ni-Cr-Ti alloy, compared to a semi-noble alloy Pd-Ag. Eight superstructures for each alloy were produced over 4 implants in the anterior region of the mandible. The superstructures were placed in a torquemeter and the fixation screw of implant #1 was tightened with a 20 Ncm load with the others loosened (Sheffield test). The unfitness (in mm) was measured using a three-dimensional optical measurer in each implant, in the buccal and lingual aspects. The obtained data were submitted to statistical analysis by the analysis of variance and Tukey's test at 5% level. Significant differences were found for the factors material (p< or =0.05), with Ni-Cr-Ti pieces better than Pd-Ag ones, and implants (p< or =0.01). There were no significant differences for the factor position and interactions among factors (p>0.05). Based on the analysis of the data, it is possible to conclude that the Ni-Cr-Ti alloy makes possible the obtainment of one-piece implant-supported superstructures with a smaller misfit compared to the one obtained in Pd-Ag alloy, traditionally indicated for this situation. Additional tests may verify the superiority of the Ni-Cr-Ti alloy.
