Objective: The aim of this double-blind, randomized trial was to evaluate the influence of the occlusogingival distance (OGD) in noncarious cervical lesions (NCCLs) on the clinical performance of a regular bulk-fill resin composite and a regular nanofilled resin composite., Materials and Methods: One hundred and forty NCCLs in 77 participants were randomly divided into four groups (n = 35), according to OGD (1.5 mm ± 10% or 3 mm ± 10%) and resin composites (Filtek Bulk Fill Posterior [B] or Filtek Z350 XT [C]), namely: 1.5 mm-B, 1.5 mm-C, 3 mm-B, and 3 mm-C. The restorations were bonded using a two-step self-etch adhesive (Clearfil SE Bond), applied following the manufacturer's instructions. Restorations were polished 1 week after placement. Two experienced and calibrated examiners evaluated the restorations using modified US Public Health Service criteria at baseline (7 days) and after 6, 12, 18, 24, and 30 months. Statistical analyses were carried out using Kruskal-Wallis, Friedman's repeated measures analysis of variance, and the Wilcoxon signed rank test (α = 0.05)., Results: After 30 months, the recall rate was 94.2%. Eight restorations were lost (3 for 1.5 mm-C, 2 for 1.5 mm-B, 1 for 3 mm-C, and 2 for 3 mm-B). All groups resulted in a significantly worse marginal discoloration and surface texture at 30 months in comparison with the baseline (1 week). No significant difference was found for the other parameters., Conclusion: The restorations performed with both resin composites produced clinically acceptable restorations. The OGD of NCCLs did not influence the clinical performance of restorations., Clinical Significance: The OGD of NCCLs did not have a significant effect on the clinical performance of a regular bulk-fill and nanofilled restorations. Purpose: The purpose was to perform a systematic review and meta-analysis based on the following research question: do tooth- and cavity-related aspects of noncarious cervical lesions (NCCLs) affect the retention of composite restorations?, Methods: Randomized clinical trials (RCTs) that evaluated the retention rate of resin restorations in NCCLs were included for the identification and comparison of their characteristics. The search was conducted in PubMed and adapted for Scopus, Web of Science, Latin American and Caribbean Health Sciences Literature database (LILACS), Brazilian Library in Dentistry (BBO), Cochrane Library, and System for Information on Grey Literature in Europe (SIGLE) without restrictions until July 2018. Unpublished and ongoing trial registries were also searched. The Cochrane Collaboration tool was used for assessing risk of bias. The quality of the evidence was graded using the Grading of Recommendations: Assessment, Development and Evaluation. Using the random effects model, a meta-analysis was conducted for each aspect (arch distribution, tooth location, wear facets, dentin sclerosis, shape, size, depth, occluso-gingival distance, and margin location)., Results: We retrieved 6738 articles. After removal of duplicates and nonrelevant articles, 24 RCTs remained. The anterior tooth location favored the retention rates of restoration of NCCLs (relative risk [RR], 1.08; 95% confidence interval [CI], 1.00-1.16). The presence of wear facets is a risk factor for the retention of restorations (RR, 0.91; 95% CI, 0.83-0.99). The evidence was moderate for arch distribution and low or very low for all other factors because of heterogeneity, imprecision, and inconsistency., Conclusion: The tooth location and the presence of wear facets can affect the retention of composite resins in NCCLs.

Objectives: The purpose of this study was to evaluate the 2-year success of resin composite restorations in non-carious cervical lesions (NCCLs) using the direct or semi-direct techniques., Materials and Methods: Thirty volunteers presenting with at least two NCCLs were included. Each participant received one restoration using the direct technique and the other using the semi-direct technique, totaling 60 restorations. Time for completing the treatment was computed. Assessments at baseline, 7 days, and 6, 12, and 24 months were performed using the modified United States Public Health Service criteria. Descriptive analysis was reported as a percentage of successful treatments. For inferential analysis, the Student t test was used to evaluate the differences between extension, depth, and time. The chi-square/Fisher tests were used to compare treatment success after each period (α = 0.05). The results were evaluated by using the Kaplan-Meier survival analysis., Results: Differences were detected regarding mean ± standard deviation time, in which direct and semi-direct procedures were accomplished in 21.8 (± 14.5) and 35.3 (± 19.9) min, respectively. Of the 60 restorations placed, 7 failed in the direct group while 8 failed in the semi-direct group up to 2 years. No differences were detected between restorative protocols. The cumulative survival was 88.5% and 88.4% for the direct technique and semi-direct techniques after 24 months, respectively., Conclusion: The tested restorative protocols present similar results for NCCLs within the studied periods., Clinical Relevance: The semi-direct technique exhibited clinical performance similar to direct technique for NCCL, demonstrating an alternative for restorations of these lesions.

Purpose: This randomized clinical trial evaluated the influence of the occlusogingival distance (OGD) of noncarious cervical lesions (NCCLs) on the clinical performance of a regular bulk-fill resin composite and a regular nanofilled resin composite., Methods and Materials: A total of 140 restorations were randomly placed in 77 participants by one operator. NCCLs were divided into four groups (n=35) according to OGD (1.5 mm±10% or 3 mm±10%) and resin composites (Filtek Bulk Fill Posterior [B] or Filtek Z350 XT [C]) used: 1.5 mm-B, 1.5 mm-C, 3 mm-B, and 3 mm-C. A two-step self-etch adhesive (Clearfil SE Bond) was applied following manufacturer instructions in all restorative procedures. Restorations were polished 1 week after placement. Clinical evaluation was performed at baseline (7 days), 6 months, and 1 year by two calibrated examiners, according to the modified US Public Health Service criteria evaluating fractures/retention, marginal staining, marginal adaptation, recurrence of caries, anatomic form, postoperative sensitivity, and surface texture. The Kruskal-Wallis test was used for intergroup comparison in each follow-up; the Friedman analysis of variance, followed by the least significant difference test (multiple comparisons) was used for intragroup comparison between baseline and follow-up times ( α =0.05)., Results: Two restorations were lost at 12 months (1 for 1.5 mm-B and 1 for 3 mm-B). The retention rates at 12 months were 100% for 1.5 mm-C, 97% for 1.5 mm-B, 100% for 3 mm-C; and 97% for 3 mm-B, with no statistical difference among the groups ( p =0.570). At 12 months, a statistically significant difference was found among the follow-up times for the same group (1.5 mm-B, 1.5 mm-C, and 3 mm-B) regarding the marginal staining criterion; moreover, the 3 mm-C group showed a significant difference from 6 months. No significant difference was found for the other parameters., Conclusion: Both resin composites showed acceptable clinical performance, and the OGD of NCCLs did not influence the clinical performance of resin composite restorations after 12 months.

The study aimed to evaluate the influence of cavosurface angle on stress concentration and gap formation in class V restorations. Cylindrical cavities 3 mm in diameter were prepared in forty-five bovine incisors, changing only the angle of the bur in relation to the flat surface of the tooth. The cavities maintained the same volume (17.67 mm³). The samples were divided according to the cavosurface angle, into three groups (n = 15): 90°, 120°, 135°. After adhesive application (Futurabond U, VOCO), the cavity was filled with bulk placement of a resin composite (GrandioSO, VOCO). The teeth were analyzed with stereomicroscopy. Data of marginal gap formation were statistically analyzed with a one-way analysis of variance (ANOVA) followed by Tukey tests (significance level: α = 0.05). Finite element analysis (FEA) was used to study residual stress in these geometries and to correlate those stresses with experimentally measured gap formation. The elastic modulus and polymerization shrinkage were determined for FEA. Residual shrinkage stresses were expressed in maximum principal stress (MPS). There was a significant difference in the gap formation among the groups (p = 0.001). A significantly lower marginal gap formation was found for 120° and 135° angles, with no significant difference between them. The cavosurface angle at 90° caused substantially higher stresses, in the restoration interface, with greater marginal gap. For the 120° and 135° angles, the stress concentrations were smaller and were located in the dental structure. Objectives: This study aimed to evaluate the bonding performance of a universal adhesive used according to self-etching or etch-and-rinse protocols in noncarious cervical lesions (NCCLs) and to compare the two protocols with their respective gold standard techniques., Methods and Materials: This randomized, double-blind clinical study enrolled 34 participants who met the inclusion criteria, 29 of whom returned after two years. They received 152 restorations bonded with one of the three adhesives (Scotchbond Universal Adhesive, Adper Single Bond 2, or Clearfil SE Bond) and one of the two bonding techniques tested. The NCCLs were restored with nanocomposite resin (Filtek Supreme). Final contours were done with a fine diamond rotary instrument and polished with rubber points. The restorations were evaluated using the FDI World Dental Federation criteria at baseline (seven days after the restoration procedure), 6, 12, and 24 months. Descriptive statistics, Kruskal-Wallis, Friedman analysis of variance, and least significant difference tests were performed., Results: No statistically significant differences were found in esthetics or the functional and biological evaluated criteria among the adhesive systems and techniques., Conclusions: The Scotchbond Universal system behaved similarly to the conventional etch-and-rinse or self-etching systems in all the adhesion strategies in the evaluated periods.

This double-blind randomized controlled clinical trial evaluated the effectiveness of dentin pretreatment with 100% ethanol (EWBT - ethanol wet bonding technique) and different adhesive protocols in noncarious cervical lesions (NCCL) after 6 months. Patients presenting at least one NCCL were included. NCCLs (n=148) were randomly assigned to 4 groups: NE (Non-EWBT + three-step etch-and-rinse (Scotchbond Multi Purpose, 3M ESPE [MP]), E (EWBT + MP); EB (EWBT + [Bond - third step of MP]), and EU (EWBT + universal adhesive (Single Bond Universal, 3M ESPE). Conventional acid-etching (Condac 37%, FGM) and nanohybrid resin composite (Z350, 3M ESPE) were used. Trained and calibrated examiners (Kappa = 0.61) evaluated the restorations at baseline (7 days) and 6-month recall using the USPHS modified criteria. Data were subjected to Chi square (α = 0.05). Differences in the success rate were found for the treatments (p = 0.003). EB presented the lowest success rate compared with the other groups (p < 0.02). No significant differences were detected among NE, E, and EU (p > 0.49). The survival rates were 97.23%, 97.30%, 78.95%, and 97.30% for NE, E, EB, and EU, respectively. Regarding postoperative sensitivity, a significant reduction was found for groups E (p = 0.027) and EU (p < 0.01) after 6 months. After 6 months, EWBT associated to the hydrophobic adhesive system had the highest failure rate.

Objective: The goal of this study was to evaluate the stress distribution in a tooth/restoration system according to the factors "amount of dental remnant" (3 levels) and "restorative material" (2 levels)., Methods: Three endodontically treated maxillary molars were modeled with CAD software for conducting non-linear finite element analysis (FEA), each with a determined amount of dental remnant of 1.5, 3, or 4.5mm. Models were duplicated, and half received restorations in lithium disilicate (IPS e.max CAD), while the other half received leucite ceramic restorations (IPS Empress CAD), both from Ivoclar Vivadent (Schaan, Liechtenstein). The solids were imported to analysis software (ANSYS 17.2, ANSYS Inc., Houston, TX, USA) in STEP format. All contacts involving the resin cement were considered no-separation, whereas between teeth and fixation cylinder, the contact was considered perfectly bonded. The mechanical properties of each structure were reported, and the materials were considered isotropic, linearly elastic, and homogeneous. An axial load (300N) was applied at the occlusal surface (triploidism area). Results were determined by colorimetric graphs of maximum principal stress (MPS) on tooth remnant, cement line, and restoration., Results: MPS revealed that both factors influenced the stress distribution for all structures; the higher the material's elastic modulus, the higher the stress concentration on the restoration and the lower the stress concentration on the cement line. Moreover, the greater the dental crown remnant, the higher the stress concentration on the restoration. Thus, the remaining dental tissue should always be preserved., Significance: In situations in which few dental remnants are available, the thicker the restoration, the higher the concentration of stresses in its structure, protecting the adhesive interface from potential adhesive failures. The purpose of this case series is to describe a restorative and surgical approach to treat the combined defect (CD)-gingival recession associated with noncarious cervical lesions (NCCL)-through connective tissue graft in combination with partial resin composite filling, restoring the crown zone plus 1 mm of the NCCL. A total of 10 patients were included. Bleeding on probing, probing depth, relative gingival recession, clinical attachment level, CD height, and dentin hypersensitivity were measured. The treatment provided statistically significant gain in clinical attachment level and shallow probing depths. The percentage of the combined defect coverage was 70% ± 20.2%. It can be concluded that this treatment protocol, in the presence of a partial resin composite restoration, can be used successfully to treat CD.

This study evaluated the influence of liner agents placed under resin composite restorations on shear bond strength (SBS) to dentin. A total of 40 extracted bovine incisors were used. Enamel surfaces were ground to expose a flat dentin area of 7 mm in diameter. Teeth were divided into four groups according to tested liners: Group 1 (control) used no liner; Group 2 used a resin-modified glass-ionomer cement liner (Vitrebond Plus, 3M ESPE); Group 3 used a light-activated calcium hydroxide resin liner (Ultra-Blend Plus, Ultradent); and Group 4 used a polyacrylic acid surface treatment + conventional glass-ionomer cement liner (Vidrion F, SSWhite). All groups were subjected to bonding procedures with Single Bond (3M ESPE) and 4-mm-thick buildups of resin composite were fabricated. Artificial aging was performed, and the specimens were submitted to mechanical shear testing. One-way analysis of variance (ANOVA) revealed significant differences among liners (P = .00). Group 4 showed the highest SBS. Group 1 showed significantly higher mean SBS compared with Group 3. Results showed that SBS is improved when using polyacrylic acid surface treatment + conventional glass-ionomer cement and decreased when using light-activated calcium hydroxide when compared with no liner. Polyacrylic acid surface treatment + conventional glass-ionomer cement as a liner placed under adhesion surface can improve the shear bond strength between dentin and resin, while light-activated calcium hydroxide resin should be used restrictedly in very deep cavities, because they may reduce the bond strength of the restoration submitted to masticatory forces and temperature changes in the oral environment.

Purpose: The purpose of this study was to comparatively assess the seven-year clinical performance of a one-bottle etch-and-rinse adhesive with resin composite (RC) and resin-modified glass ionomer (RMGI) restorations in noncarious cervical lesions., Methods and Materials: One operator placed 70 restorations (35 restorations in each group) in 30 patients under rubber dam isolation without mechanical preparation. The restorations were directly assessed by two independent examiners, using modified US Public Health Service criteria at baseline and 6, 12, 24, 60, and 84 months. The obtained data were tabulated and statistically analyzed using the Fisher and McNemar tests. A difference was significant if p<0.05., Results: Twenty patients were available for recall after seven years (66.6%), and 25 RC and 26 RMGI restorations out of 70 restorations were evaluated. Excellent agreement was registered for all criteria between examiners (κ≥0.85). Alfa and bravo scores were classified as clinically acceptable. The McNemar test detected significant differences within RC restorations between baseline and seven-year evaluations for anatomic form, marginal integrity, and retention (p<0.05). For RMGI restorations, a significant difference was identified for marginal integrity (p<0.05). As to material comparison, the Fisher exact showed a better retention performance for RMGI restorations than for RC restorations (p<0.05). Twelve composite restorations were dislodged (52.0% retention) and three ionomer restorations were lost (88.5% retention). The cumulative success rate for RC and RMGI was 30% and 58.1%, respectively., Conclusions: After seven years of service, the clinical performance of RMGI restorations was superior to that of the adhesive system/resin composite restorations in this study.

Purpose: The purpose of this study was to assess restorative treatment outcomes in the mixed dentition of amelogenesis imperfecta (AI) patients and determine the postrehabilitation oral health status and satisfaction of the patients., Methods: Clinical and radiographic examinations were performed on eight AI patients, who had 74 restorations placed in permanent incisors and molars, to allow evaluation of the integrity of the restorations and periodontal status post-treatment. Subjects completed a survey regarding esthetics, function, and sensitivity., Results: Among the 74 restorations evaluated, seven were lost; of the remaining restorations, 31 were posterior, and 36 were anterior. Ten were rated clinically unacceptable. Teeth with stainless steel crowns had a moderate gingival index (mean=2.3) and plaque index (mean=2.0) scores. Widening of the periodontal ligament and pulp canal obliteration were common radiographic findings. Subject's recall of satisfaction regarding esthetics (P=.002) and sensitivity (brushing-P=.03; eating-P=.01) showed a statically significant difference before and after treatment., Conclusions: During mixed dentition, teeth with amelogenesis imperfecta may be restored with conventional treatment modalities. Direct restorations should be considered "interim" with multiple repairs anticipated. Post-treatment, gingival inflammation and plaque accumulation were observed. Subjects were satisfied with their appearance and reported a decrease of hypersensitivity.

Minimally invasive caries-removal procedures remove only caries-infected dentin and preserve caries-affected dentin that becomes remineralized. Dental cements containing calcium phosphate promote remineralization. This study evaluated the in vivo remineralization capacity of resin-based calcium-phosphate cement (Ca-P) used for indirect pulp-capping. Carious and sound teeth indicated for extraction were randomly restored with the Ca-P base or without base (control), followed by adhesive restoration. Study teeth were extracted after three months, followed by elemental analysis of the cavity floor. Mineral content of affected or sound dentin at the cavity floor was quantified by electron probe micro-analysis to 100-mum depth. After three months, caries-affected dentin underneath the Ca-P base showed significantly increased calcium and phosphorus content to a depth of 30 mum. Mineral content of treated caries-affected dentin was in the range of healthy dentin, revealing the capacity of Ca-P base to promote remineralization of caries-affected dentin.

Purpose: To investigate in vitro the effect of retentive grooves, GIC type and insertion method on the fracture resistance of Class II glass-ionomer cement (GIC) restorations., Methods: Premolars were divided into 12 groups (n = 10) according to three variables: retentive grooves [presence (PR) or absence AR)], GICs type [Ketac-Molar (KM), Fuji VIII (F8) and RelyX Luting (RX)], and insertion method [syringe injector (SI) or spoon excavator (SE)]. The specimens were subjected to fracture resistance test. Data were submitted to three-way ANOVA and multiple comparisons were performed using a Tukey test (P < 0.05)., Results: Mean fracture resistance values (Kgf) +/- standard deviations (SD) were: KM (PR+SI) = 65.66 +/- 2.5; KM (PR+SE) = 62.58 +/- 2.1; KM (AR+SI) = 57.11 +/- 1.9; KM (AR+SE) = 51.94 +/- 2.3; F8 (PR+SI) = 63.05 +/- 2.1; F8 (PR+SE) = 60.12 +/- 2.3; F8 (AR+SI) = 55.11 +/- 1.9; F8(AR+SE) = 49.20 +/- 1.6; RX (PR+SI) = 50.99 +/- 2.4; RX (PR+SE) = 48.81 +/- 2.5; RX (AR+SI) = 45.53 +/- 2.6; RX (AR+SE) = 41.88 +/- 3.0. Statistically significant differences were observed among all the groups tested (P = 0.001). There was significant difference when pooled means for GIC type were compared with retentive grooves (P = 0.01) and when pooled means for retentive grooves were compared with insertion method (P = 0.01).

The purpose of this study was to evaluate the clinical performance of glass ionomer cement (GIC) restorations comparing two minimally invasive methods in permanent teeth after 12 months. Fifty pregnant women (second trimester of pregnancy), mean age 22 +/- 5.30 years, were treated by two previously trained operators. The treatment approaches tested were: chemomechanical method (Carisolv; MediTeam) and atraumatic restorative treatment (ART). A split-mouth study design was used in which the two treatments were randomly placed in 50 matched pairs of permanent teeth. The chemomechanical method (CM) was the test group and the ART was the control group. The treatments were performed in Public Health Centers. The tested restorative material was a high-strength GIC (Ketac Molar; 3M/ESPE). The restorations were placed according to the ART guidelines. Two calibrated independent examiners evaluated the restorations in accordance with ART criteria. The inter-examiner kappa was 0.97. Data were analyzed using 95% confidence interval on the binomial distribution and Fisher's exact test at 5% significance level. In a 12-month follow-up, 86% of the restorations were evaluated. In the test group (CM), 100% (CI=93.3-100%) of the restorations were considered successful. In the control group (ART) 97.6% (CI=87.4-99.9%) of the restorations were considered successful and 2.4% unsuccessful (marginal defect >0.5 mm). There was no statistically significant difference between the 12-month success rate for both groups (Fisher's exact test: P=0.49) and between the two operators (Fisher's exact test: P=1.00). Both minimally invasive methods, chemomechanical method and ART, showed a similar clinical performance after 12 months of follow up.

Purpose: The purpose of this study was to evaluate the performance of multiple-surface restorations employing 2 different glass ionomer cements (GICs) and the Atraumatic Restorative Treatment (ART) approach in permanent molar teeth., Methods: This study examined 60 restorations--36 Class I restorations involving 2 or more tooth surfaces and 24 Class II restorations--that were placed in 46 schoolchildren (9-16 years of age) by 2 dentists using the ART approach. The restorations were randomly divided into 2 groups: (a) 30 cavities were filled with high strength GIC (Ketac Molar-3M ESPE), and (b) 30 cavities were filled with resin-modified GIC (Fuji VIII-GC Corp). Two independent calibrated examiners carried out the evaluations according to ART criteria. The interexaminer kappa was 0.92. Data were submitted to chi-square, McNemar, and Fisher's tests. A difference was statistically significant if P<.05., Results: In a 12-month follow-up, 59 restorations were evaluated. The success rates of the restorations were 100% and 93% for Fuji VIII and Ketac Molar, respectively. There was no statistically significant difference between GICs, cavity types, or operators., Conclusions: Based on a 12-month follow-up evaluation, the clinical performance of the multiple-surface atraumatic restorative treatment restorations of both glass ionomer cements (high-strength and resin-modified) was considered satisfactory with a high success rate.

The clinical performance of two packable posterior composites, Alert (A)-Jeneric/Pentron and SureFil (S)-Dentsply, was evaluated in 33 patients. Each patient received one A and one S restoration, resulting in a total of 66 restorations. The restorations were placed by one operator according to the manufacturer's specifications and were finished and polished after 1 week. Photographs were taken at baseline and after 2 years. Two independent evaluators conducted the clinical evaluation by using modified United States Public Health Service criteria. After 2 years, 60 restorations (30 A and 30 S), 27 class I (16 A and 11 S) and 33 class II (14 A and 19 S) were evaluated in 30 patients. Criterion A for recurrent caries, vitality, and retention was applicable to all 60 restorations. Criterion B was distributed among 40 restorations as follows: surface texture (15 A; 2 S), color (5 A; 6 S), postoperative sensitivity (1 S), marginal discoloration (8 A), marginal adaptation (3 A), and wear resistance (2 A). Data were analyzed using the Exact Fisher and McNemar tests. After 2 years, S showed a significantly better performance than A with respect to surface texture and marginal discoloration. The clinical performance of both materials was considered acceptable over the 2-year period. Further evaluations are necessary for a more in-depth analysis.

Atraumatic Restorative Treatment (ART) has been adopted around the world to avoid unnecessary extractions, especially in non-industrialized countries. The development of specific glass ionomer cements marketed for the ART technique has contributed to the technical success rate. In this study, Ketac-Molar (3M ESPE, Dental Medzin, Germany) was used to restore 150 Class I cavities in 118 Brazilian public school children, aged from 7-12 years. At baseline and at subsequent recalls, CPI probes with a ball-end of 0.5 millimeters (mm) were used to assess loss of restorative material, and photographic color transparencies of restorations were made. After six months, 83 patients returned for follow-up examinations, with 71.8% of their restorations designated as acceptable. After three years, 49 patients with 57 ART-restorations were evaluated, with 21.0% of these restorations graded as acceptable. Another 29.8% of their restorations had been replaced by more permanent materials. The main objective of the ART technique is tooth retention; this was achieved for 94.7% of the restored teeth in a high caries risk population who returned for recalls.

Pulp exposure due to traumatic injuries in the anterior permanent teeth of adolescents is a common occurrence. A vital permanent maxillary incisor with complex crown fracture and pulp exposure was treated by partial pulpotomy and assessed clinically through pulpal sensitivity tests and radiographically for periapical healing. Partial pulpotomy consisted of pulp tissue removal to a depth of only 1 to 2 mm, then capping the pulpal wound with calcium hydroxide, lining with resin-modified glass-ionomer cement, and restoration with resin composite. At each recall (7, 15, 21, 50, 90, 150, and 250 days), no spontaneous pain was observed; the pulp showed signs of vitality and absence of periapical radiolucency after 90 days. For long-term success, partial pulpotomy is recommended as an option for cases of traumatic pulp exposure in permanent incisors with crown fractures.
