Objectives: To quantify the patient-reported effect of oral rehabilitation and investigate aspects associated with this., Methods: The patient-reported effect in participants treated with fixed dental prosthesis (FDP; n = 72) or removable dental prosthesis (RDP; n = 58) was measured using the Oral Health Impact Profile 14 (OHIP-14) and global oral ratings (GOR) of aesthetics, chewing and comfort before and after treatment and global transition judgements (GTJ) of aesthetics, chewing and comfort after treatment. Explanatory variables included gender, age, if a prosthetic replacement was present before treatment, type of prosthetic treatment performed, number of teeth replaced and zone of the teeth replaced., Results: The RDP and FDP treatments both significantly improved the OHIP-14 score with a moderate to large effect and most participants had good patient-reported effect. The effect varied depending on the method used. In the bivariate analyses, the number of participants with good effect was higher in the RDP group than the FDP group when using the OHIP-14 and GOR whereas the effect was higher in the FDP group when using the GTJ. Multiple regression analyses showed that poor chewing ability before treatment was associated with good effect measured by the OHIP-14 and GOR and that poor effect in chewing ability and RDP treatment was associated with poor effect in comfort measured by GTJ. Functional problems after treatment were associated with no/poor effect., Conclusions: Oral rehabilitation with fixed or removable prostheses in general yields good patient-reported effect. Objective: To investigate the status of tooth loss and denture restoration in Chinese adults, analyse the changing trend and provide fundamental data for oral health policy., Methods: According to the protocol of the 4th National Oral Health Survey, a multistage stratified random cluster-sampling method was used to enrol adult subjects aged 35 to 44, 55 to 64 and 65 to 74 years in all 31 provinces, municipalities and autonomous regions of the mainland of China. The status of tooth loss and denture restoration was investigated. SPSS20.0 software was used for statistics analysis., Results: Among the 13,464 subjects investigated, 13.8% had complete dentition, 84.4% had dentition defects, and 1.8% was edentulous. Urban subjects showed a significantly higher proportion of complete dentition than those in rural (P = 0.02), and males showed the statistically higher proportion of complete dentition than females (P = 0.01). The mean of remaining teeth was 26.1 ± 6.90, which in urban areas was significantly higher than in rural areas (P < 0.01). The means of remaining teeth were 29.6 ± 2.3, 26.3 ± 6.1, and 22.5 ± 8.7 in the 35 to 44, 55 to 64 and 65 to 74 age groups, respectively. The detection rate of fixed partial dentures (FPD) was statistically higher in urban than in rural areas and in males than that in females (P < 0.01). The detection rate of removable partial dentures (RPD) was statistically higher in urban areas than in rural locations (P < 0.01). However, the detection rates of irregular denture and unrepair of tooth loss were both significantly lower in urban than in rural areas (P < 0.01). The rate of restoration of tooth loss was 41.6% in Chinese adults., Conclusion: Although the tooth loss and denture restoration status recorded in the survey was improved compared with the results of 10 years ago, more efforts need to be made on strengthening oral health promotion, particularly for elderly people and those living in rural areas.

Purpose: To evaluate the current status of all-ceramic inlay-retained fixed dental prostheses (CIR-FDPs) for the replacement of posterior teeth., Study Selection: Screening of titles and abstracts, full-text analysis for inclusion eligibility, quality assessment, data extraction and evaluation of the scientific evidence were performed independently by two reviewers. The electronic databases MEDLINE/PubMed, EMBASE, Cochrane Central Register of Controlled Trials, and Compludoc were searched with no restriction to publication date or language. The quality of the studies was evaluated through: the original 'QDP' ('Questionnaire for selecting articles on Dental Prostheses') (for research papers); the 'Guidelines for managing overviews' of the Evidence-Based Medicine Working Group (for reviews); the Cochrane risk of bias tool; and the GRADE scale for grading scientific evidence., Results: This review started with 4942 articles, which were narrowed down to 23 according to the selection criteria. The data was not statistically treated because of the heterogeneity of the studies. Zirconia-based CIR-FDPs may be recommended for restoring posterior single missing teeth, although the prosthesis/tooth bonded interface has yet to be improved. The addition of lateral wings to the classical inlay preparation seems promising. The weakest parts of CIR-FDPs are the connectors and retainers, while caries and endodontic problems are the most common biological complications. The fabrication of CIR-FDPs with monolithic zirconia may eliminate chipping problems., Conclusions: A three-unit CIR-FDP is a viable treatment option for replacing a posterior missing tooth. Objectives: This clinical study evaluated the long-term outcome and survival rate of all-ceramic cantilever resin-bonded fixed dental prostheses (RBFDPs) made of zirconia-ceramic with a single-retainer design to replace missing incisors. In addition, whether the reason for missing incisors has an influence on the longevity of RBFDPs was analyzed., Materials and Methods: One hundred and eight zirconia ceramic cantilever RBFDPs were provided for 87 patients. Seventy-five RBFDPs replaced maxillary incisors and 33 mandibular incisors. The restorations were subsequently categorized into 3 groups according to the reasons for missing teeth (congenitally missing, trauma and other reasons). The patients were followed up annually, and the restorations were assessed for function and aesthetics. The mean observation time of the RBFDPs was 92.2±33months., Results: Six debondings and 1 loss of restoration were recorded. The lost restoration was removed at the patient's request after a small chip occurred on the mesial edge of the pontic. Three of the 6 observed debondings were caused by traumatic incidents. All debonded RBFDPs were rebonded successfully with no further complications. Zirconia ceramic RBFDPs yielded a 10-year survival rate of 98.2% and a success rate of 92.0%., Conclusions: Anterior zirconia ceramic cantilever RBFDPs provided excellent clinical longevity. Purpose: Many studies have investigated the impact of prosthetic treatment on oral health-related quality of life (OHRQoL). However, most of these have been performed among fully or partially edentulous patients. Studies involving patients with a single missing tooth are limited. The purpose of this study was to compare the OHRQoL between patients treated by single-tooth implants and three-unit fixed partial dentures (FPDs) for single missing tooth restoration., Materials and Methods: A cross-sectional survey was conducted in Korea with patients drawn by stratified purposive sampling based on age. OHRQoL was measured using the Korean version of the 14-item Oral Health Impact Profile (OHIP-14K) questionnaire. Pre- and posttreatment OHIP-14K scores were compared by paired t test. Single-tooth implants and three-unit FPDs were compared by two-sample t test. In addition, multiple regression analysis was used to evaluate the impact of treatment mode on OHIP-14K total score after adjusting the effect of demographics and clinical factors., Results: Thirty-five patients with single-tooth implants and 36 patients with three-unit FPDs were included. All participants had a significant improvement in OHRQoL compared with before the treatment (P < .0001). However, there was no statistically significant difference in the change of OHIP-14K score between the two treatment modes. In addition, the treatment mode had no significant impact on the change of OHIP-14K total score after adjusting the influence of covariates (P = .170)., Conclusion: Both single-tooth implants and three-unit FPDs for single missing tooth replacement resulted in significant and similar improvement of OHRQoL.

Aim: The conventional treatment of a single missing tooth is most frequently based on the provision of a fixed dental prosthesis (FDPs). A variety of designs and restorative materials are available which have an impact on the treatment outcome. Consequently, it was the aim of this review to compare resin-bonded, all-ceramic and metal-ceramic FDPs based on existing evidence., Materials and Methods: An electronic literature search using "metal-ceramic" AND "fixed dental prosthesis" AND "clinical, all-ceramic" AND "fixed dental prosthesis" AND "clinical, resin-bonded" AND "fixed dental prosthesis" AND "clinical, fiber reinforced composite" AND "clinical, monolithic" AND "zirconia" AND "clinical" was conducted and supplemented by the manual searching of bibliographies from articles already included., Results: A total of 258 relevant articles were identified. Metal-ceramic FDPs still show the highest survival rates of all tooth-supported restorations. Depending on the ceramic system used, all-ceramic restorations may reach comparable survival rates while the technical complications, i.e. chipping fractures of veneering materials in particular, are more frequent. Resin-bonded FDPs can be seen as long-term provisional restorations with the survival rate being higher in anterior locations and when a cantilever design is applied. Inlay-retained FDPs and the use of fiber-reinforced composites overall results in a compromised long-term prognosis. Recently advocated monolithic zirconia restorations bear the risk of low temperature degradation., Conclusions: Several variables affect treatment planning for a given patient situation, with survival and success rates of different restorative options representing only one factor. The broad variety of designs and materials available for conventional tooth-supported restorations should still be considered as a viable treatment option for single tooth replacement.

Prosthodontic treatment is a common procedure for the elderly as tooth loss is a reality in old age. Dentists take care of increasingly older patients with physiological age manifesting as cognitive impairment, frailty or multiple chronic diseases or who have side effects of medicines. We evaluated how patients' age affects prosthodontic treatment choice and whether we could identify the age when a change in practice occurs. In addition, we determined how common the treatment method of fixed prostheses is among patients aged 60 years or over in Public Dental Services (PDS) and how common rehabilitation of dentition with new dentures is compared with repair of existing dentures. Our data cover all patients aged 60 years and older (n = 130,060) treated in Helsinki PDS in 2007-2012. Data were aggregated into seven groups: 60-64, 65-69, 70-74, 75-79, 80-84, 85-89, and 90 years and over. During the 6-year period, the mean annual number of the population was about 114,000 and the mean annual number of patients treated with prosthodontics 1700. Prosthodontic treatment choices (repair, removable prosthodontics, fixed prostheses, fibre-reinforced composite fixed prostheses) vary by age; the older the patient, the rarer fixed or fibre-reinforced composite fixed prostheses and removable prostheses and the more frequent repairs (P < 0.001). Denture repair was virtually the only treatment that patients over 90 years received. Based on our results, the age at which prosthodontic treatment practices in PDS change is around 70 years. Design: Randomised controlled trial., Intervention: All patients received treatment to render them dentally fit. Patients were randomly allocated to either the removable dental prostheses (RPD) or the shortened dental arch (SDA) group. Patients in the RDP group were restored to complete arches with RDP using cobalt-chromium frameworks according to a standardised protocol. For the SDA group, patients were restored to a shortened arch of ten occluding pairs of natural and replacement teeth using resin-bonded bridgework (RBB)., Outcome Measure: Treatment effect was measured using the change in oral health-related quality of life (OHrQOL). For each patient the costs of delivering treatment were recorded by a research nurse during the intervention period. Laboratory costs were recorded as part of normal hospital policy for all patients. All of the dental materials used were recorded and given a unit price. The cost of professional time per patient was estimated using the highest point of the salary scale for the Community Dental Service in Ireland., Results: One hundred and thirty-two patients were randomised; 65 to the RPD group and 67 to the SDA group. Ninety-two patients (69.7%) completed the study (46 in RPD group; 46in SDA group). There was no difference in the success rates of the two treatments over the period. Five pieces of resin-bonded bridgework (RBB) debonded and were recemented over the 12-month period giving a success rate of 95.5% for the RBB. Four patients discontinued wearing their RPDs; all four RPDS were fitted in the lower arch and included bilateral free end saddles, a success rate of 95.9%. Both RPD and SDA groups demonstrated statistically significant improvements in OHrQoL scores after 12 months.The total cost of achieving the minimally important clinical difference (MID) in OHrQOL for an average patient in the RDP group was [euro ]464.64. For the SDA group, the cost of achieving the MID for an average patient was [euro ]252.00. The cost-effectiveness ratio was therefore 1:1.84 in favour of SDA treatment., Conclusions: With an increasingly ageing population, many patients will continue to benefit from removable prostheses to replace their missing natural teeth. From a purely economic standpoint, the results from this analysis suggest that the treatment of partially dentate older adults should be focused on functionally orientated treatment because it is simply more cost-effective.

Traumatic injuries or congenital malformations may cause soft and hard tissue defects resulting in the loss of alveolar bone and attached mucosa. Restoring the defective area presents a challenge for clinicians. The prosthetic rehabilitation of 2 patients with maxillary dentoalveolar defects with 2 different prosthetic designs is presented. Objective: To conduct a cost-effectiveness analysis comparing two different tooth replacement strategies for partially dentate older patients, namely partial removable dental prostheses (RDP) and functionally orientated treatment based on the shortened dental arch concept (SDA)., Methods: Ninety-two partially dentate older patients completed a randomized controlled clinical trial. Patients were randomly allocated to two treatment groups: the RDP group and the SDA group. Treatment effect was measured using impact on oral health-related quality of life (OHrQOL), and the costs involved in providing and maintaining care were recorded for all patients. Patients were followed for 12 months after treatment intervention. All treatment was provided by a single operator., Results: The total cost of achieving the minimally important clinical difference (MID) in OHrQOL for an average patient in the RDP group was €464.64. For the SDA group, the cost of achieving the MID for an average patient was €252.00. The cost-effectiveness ratio was therefore 1:1.84 in favour of SDA treatment., Conclusion: With an increasingly ageing population, many patients will continue to benefit from removable prostheses to replace their missing natural teeth. Purpose: The objective of the present study was to compare inter-proximal fill and papilla height between different embrasures., Material & Methods: One hundred and fifty non-smoking consecutive patients (mean age 54, range 32-73; 63 males and 87 females) without periodontal disease were selected in a multidisciplinary practice during regular supportive care. All had been treated for multiple tooth loss in the anterior maxilla at least 1 year earlier by means of a fixed restoration on teeth (n = 50) or implants (n = 100) using straightforward procedures (without hard and/or soft tissue augmentation). Embrasure fill was assessed by means of Jemt's papilla index and papilla height was registered following local anaesthesia by means of bone sounding by one clinician., Results: Tooth-pontic and tooth-implant embrasures demonstrated comparable inter-proximal fill and papilla height (≥58% Jemt's score 3; mean papilla height ≥4.1 mm). Between missing teeth, embrasure fill and papilla height were lower regardless of the embrasure type. The implant-implant and implant-pontic embrasure demonstrated comparable outcome (≤42% Jemt's score 3; mean papilla height ≤ 3.3 mm; p ≥ 0.416), which was significantly poorer when compared to the pontic-pontic embrasure (82% Jemt's score 3; mean papilla height 3.7 mm; p ≤ 0.019). Overall, papilla index and papilla height demonstrated a weak correlation (Spearman's correlation coefficient: 0.198; p = 0.002)., Conclusions: The re-establishment of a papilla is difficult when there is no tooth involved. In that scenario a short papilla should be expected and implant-borne restorations demonstrate the poorest outcome. With the advent of new adhesive technologies, fiber-reinforced composite fixed partial dentures (FRC-FPDs) have become an inexpensive, esthetic and useful restorative option for patients missing a single tooth. This article describes a case involving an inlay-retained FRC-FPD with a prefabricated fiber-reinforced pontic substructure. In addition, a 6-year follow-up is presented. The FRC-FPD fully restored the single missing molar with simple and conservative intracoronal preparations in the proximal teeth and was deemed clinically acceptable after the evaluation period. The clinical procedures to build a FRC-FPD with a prefabricated fiber-reinforced pontic substructure typically are finished by the clinician in 1 or 2 visits without laboratory assistance, making it a viable alternative for patients who cannot afford implant-based treatment or a conventional metal-ceramic FPD.

Introduction: Ever since the introduction of the minimal preparation fixed partial dentures, the major concern of the clinicians has been its longevity., Aims and Objective: The objective of the present study was to compare the clinical performance of resin bonded cantilever bridges with conventional cantilever bridges for a period of 1 year and to evaluate the periodontal changes of abutment teeth using radio visio graphy., Materials and Methods: Fourteen patients with missing first or second premolar were selected for the study. The patients were divided into 2 groups, conventional cantilever group and resin bonded cantilever group., Results: The results were analyzed using Student's t test and Mann-Whitney U test. The clinical success rate for both the groups was 100%. The amount of bone loss at 6 months for conventional group was 0.11 mm and for resin bonded bridge group was 0.10 mm. The amount of bone loss at 1 year for conventional cantilever group was 0.14 mm and for resin bonded fixed partial denture group was 0.16 mm. There was no statistical difference between both the groups. The results showed that the resin bonded cantilever bridges can be used for the treatment of missing posterior tooth.

The loss of a permanent first molar in adolescent patient creates a need for early space maintenance and restoration of function. To ascertain function and esthetics, immediate treatments include interim restorative approaches. This paper describes a conservative, functional and cost-effective bridge as an interim restoration after the loss of permanent first molar in an early adolescent dentition.

Background: The choice between several treatment options for replacing a single missing tooth is influenced by clinical, dentist- and patient-immanent factors. This study aimed to determine the patient factors that would affect the treatment decision to replace a single missing tooth and to assess the satisfaction with several options., Method: 200 volunteers involved (121 females and 79 males) divided into four groups, Group A: consisted of patients with conventional fixed partial dentures or patients with resin bonded fixed partial dentures. Group B: consisted of patients who received removable partial dentures while Group C: consisted of patients who received a single implant supported crown, and a control group D: consisted of patients who received no treatment. Data were collected using a questionnaire., Results: The highest percentage of males within groups (58%) was within the removable prostheses category. The majority of the subjects in the study reported that the main reason for replacing a missing tooth was for esthetic and function. Most important factor affecting the choice between treatment modalities was damaging the neighboring teeth. Pain, post operative sensitivity and dental phobia were important factors in choosing the prosthesis type and affected the control group patients not to have any treatment. The highest satisfaction percentage among groups studied was recorded for dental implants then FPD groups, while the least percentage were in both the control and RPD groups, for all aspects of function, esthetic and speech efficiency., Conclusions: The final choice between FPD, RPD and implant depended on several factors which affected the decision making; among these is cost and patients' awareness of the different treatment options.

Purpose: The aim of this retrospective study was to evaluate implant survival/success rates and peri-implant parameters as well as patient satisfaction for uniformly designed implant-supported anterior maxillary or mandibular fixed partial dentures (FPDs)., Materials and Methods: A retrospective study was conducted on patients with maxillary or mandibular anterior partial edentulism (all incisors missing) treated between 2002 and 2006 with a two-implant-supported four-unit FPD. All FPDs were of the same design: two implant abutments in the lateral incisor positions and two ovate pontics in the central incisor positions. Cumulative implant survival rates and peri-implant conditions (marginal bone loss, pocket depth, Plaque Index, Gingival Index, Bleeding Index, Periotest values) as well as the incidence and type of prosthodontic maintenance were evaluated. The patients' subjective satisfaction rate was surveyed using multiple questionnaires with a 10-point scoring system (0 = not satisfied to 10 = highly satisfied)., Results: Thirty-six of 38 patients (dropout: 5%) with 72 implants (50 maxillary and 22 mandibular implants, 25 maxillary and 11 mandibular FPDs) were available for follow-up after a mean observation period of 56.2 ± 10.3 months. High cumulative implant survival and success rates (100%) and healthy peri-implant parameters (mean marginal bone resorption: 1.8 ± 0.3 mm; mean pocket depth: 2.5 ± 1.0 mm; Periotest value: -4.5 ± 1.1; and Plaque/Bleeding/Gingival indices of 0 in 70% of sites [with significantly better results in the maxilla than in the mandible]) were achieved. The most frequent required prosthodontic maintenance efforts were maxillary recementation (3/25; 12%) and mandibular rebasing (2/11; 18.2%). A high score for satisfaction was obtained, although slightly better overall results were seen for maxillary than for mandibular FPDs., Conclusion: For all parameters evaluated, these FPDs proved to be a viable treatment procedure for anterior partial edentulism with good clinical and esthetic outcomes.

The contour of edentulous ridge should be carefully evaluated before a fixed partial denture is undertaken. The ideal ridge height and width allows placement of a natural looking pontic which facilitates maintenance of plaque-free environment. The localized alveolar ridge defect refers to the volumetric deficit of the limited extent of bone and soft tissue within the alveolar process. Such type of ridge defects can be corrected by surgical ridge augmentation that can be accomplished by the addition of either soft or hard tissues. This article describes a procedure of surgical connective tissue augmentation of a localized deficient alveolar ridge in the maxilla, followed by fixed partial denture, enhancing the esthetics, function and health.

Objectives: To analyse the possible effects of the socioeconomic status (SES) on the prosthetic replacement of missing teeth in working-age people and to explore the role of potential confounders., Methods: Cross-sectional data were collected from 2310 German adults aged 30-59 years. The relationship between each of the three SES indicators (education, income, and occupational status) and dental prostheses were examined by multinomial logistic regression analyses. For that, partially dentate participants with suboptimal and no replacement of missing teeth were compared with partially dentate participants having optimal replacement. Potential confounders (age, sex, dental status, social network and social support) were entered if their inclusion in the model led to >or= 10% change in the coefficient of interest., Results: Social network and social support did not meet the criterion for confounding. In the maxilla, having no replacement was positively associated with lower categories for each of the three SES indicators [Odds ratios (OR) between 1.6 and 2.1; 95% confidence intervals (CI) between 1.1 and 3.4]. Low occupational status was the single predictor for suboptimal dental prostheses (OR=3.2; 95% CI: 1.6-6.2). In the mandible, occupational status showed no association with the prosthetic status, whereas low educational level and low household income were determinants for having no replacement (OR=1.9 and 1.9, 95% CI: 1.0-3.5 and 1.1-3.0, respectively). Low household income was the single determinant for suboptimal replacement of missing teeth (OR=2.4, 95% CI=1.1-5.2)., Conclusion: The findings may indicate the relevance of the financing of prosthodontic treatment. The strong association between various forms of upper dentures and occupational prestige can be seen as key contributing concept to how individuals' characteristics affect the outcome in prosthodontic care.

This report describes a prosthetic approach that uses fixed partial dentures to establish stable posterior occlusion. Four fixed partial dentures were made of a silver-palladium-copper-gold alloy; the pontics were veneered with a light-curing indirect composite material. They were prepared using a thiouracil metal conditioner and seated with an adhesive luting cement. The clinical follow-up indicated that the prosthetic procedure reported here is applicable for severe anterior and posterior open bite malocclusion.

Severe periodontal disease often leads to tooth loss, necessitating prosthetic rehabilitation to restore function and aesthetics. The concept of perio-prosthetic treatment using extensive bridges of cross-arch design was introduced approximately 30 years ago. Long term follow-up studies have shown that teeth with reduced periodontal support can be used as abutments for extensive fixed prostheses, provided periodontal disease has been treated successfully, and an effective recall programme has been instituted to prevent periodontal disease recurrence. Low complication rates have been reported with these extensive constructions.

In 1990, 364 elderly (76-86 years) inhabitants of Helsinki, Finland, attended a dental and oral examination study that was conducted as part of the Helsinki Aging Study. In spring 1996, these subjects were recalled for a 5-year follow-up. Between the baseline and follow-up examinations, 114 (31%) subjects had deceased (86 women and 28 men), whereas 134 had either moved, were too ill, or refused to participate in the follow-up. Follow-up examination was conducted for 113 subjects (79 women and 34 men), with the participating rate being 46%. Five subjects became edentulous during the follow-up. Of the subjects, 61% had 1-32 teeth at follow-up. In these subjects, the mean number of teeth decreased from 14.9 (+/-8.3) to 13.5 (+/-8.6) (P < 0.0001). Prosthetic status changed in 40% of the elderly dentate people: 25% received new prostheses whereas 15% lost prostheses that were not replaced. New fixed partial dentures were made in five maxillae and in nine mandibles during the follow-up. Acrylic removable partial dentures (ARPD) were most frequently used: 35% of dentate subjects had an ARPD. Subjects with removable prostheses had higher levels of salivary microbes and higher root caries incidence than those with natural teeth. Furthermore, the presence of removable prostheses at baseline, together with the male gender, was clearly associated with tooth loss during follow-up. This study indicates that fixed rather than removable prostheses should be used in elderly patients. The need for a removable denture ought to be carefully considered.

This article explores the various treatment options available to replace missing teeth with a fixed prosthesis. It discusses the requirements for each treatment modality and its associated advantages and disadvantages.

The absence of the temporary incisors could be to a genetic illness or to some multiple premature extractions. These extractions are the aftermaths of the carious lesions or some traumatisms underwent by the temporary incisors. Beyond measure the loss of the space, the premature loss some temporary incisors very often assign the relational development of the child and disturb its psychological development and the aesthetic function. Across some cases clinics, the authors show that the replacement of the temporary incisors is the therapeutic ideal solution. Indeed, the child prosthesis, replacing the temporary absent incisors, solves the psychological, aesthetic and relational problems of the child.

Inlay fixed partial dentures luted by use of adhesive procedures offer a clinical alternative for the restoration of single missing posterior teeth. The introduction of ceromers and fiber-reinforced composites and the continuous improvement of adhesive systems and luting agents make this type of restoration possible, offering good aesthetic and functional results. The procedure is minimally invasive and conservative. This clinical report reviews the factors influencing the diagnosis and the clinical indications for an inlay fixed partial denture. In addition, a patient treatment is presented to illustrate the clinical procedures involved.

The concept of minimally invasive preparation protocols has resulted in reduced loss of critical tooth structures and maintenance of optimal strength, form, and aesthetics. While various treatment options have been described for single-tooth replacement, fiber-reinforced composite (FRC) fixed partial dentures (FPDs) provide a viable treatment alternative with proven mechanical properties, aesthetics, and function. This article presents several clinical scenarios in which minimally invasive adhesive FRC FPDs are provided to deliver enhanced predictability, strength, and durability.

The psychological effects of tooth loss in the permanent dentition are relatively unknown. Complete edentulousness is a serious life event in terms of readjustment. The aim of the study was to describe the process patients with deteriorating dental status had gone through before treatment with a fixed prosthesis (Brånemark System, Novum), and to describe what living with a fixed prosthesis means to the patients themselves. In-depth interviews were carried out with 18 patients, and the interviews were transcribed verbatim and analysed in open, axial and selective coding processes according to Grounded Theory. In the analysis, four categories were developed and labelled: 'alterations in self-image', 'becoming a deviating person', 'becoming an uncertain person' and 'becoming the person I once was'. 'Alterations in self-image' was identified as the core category and was related to the other three categories. The core category describes the changes in self-image starting with the subjects' increasingly worsened dental status, followed by a period of them having to live and cope with a denture and, finally, their living with a fixed prosthesis. The motive power for the decision to undergo treatment with a fixed prosthesis seems to be a desire to restore dental status and also to recapture attractiveness, self-esteem and a positive self-image.

This article describes why it is important to replace the maxillary primary incisors when they are prematurely lost in children. The consequences of not replacing these teeth in relation to speech, esthetics, and self-image considerations are provided. A step-by-step description is presented for the fabrication of an esthetic appliance, which can be easily made to replace missing maxillary primary incisors. In addition, information is presented on when and how to remove the esthetic appliance.

This paper describes an appliance to replace prematurely lost maxillary anterior teeth. The prosthesis is different from previous appliance designs, since it uses double stainless steel crowns on abutment teeth, and uses a donor cast to fabricate artificial teeth. A case is presented describing laboratory and office procedures.

This clinical report highlights some of the problems in providing a suitable prosthesis for a patient who suffers from systemic sclerosis. A conventional removable partial denture may not have been suitable because of the lack of denture bearing area, changing peripheral seal, and insufficient teeth to support and retain the denture. An implant-retained FPD overcame these difficulties. One would hope that, with regular maintenance and monitoring, quoted prosthesis success of 15 years can be achieved.

Occasionally, clinicians are faced with a situation in which a solitary tooth between two bridges requires extraction. When this type of situation occurs, we must ponder the dilemma of how to replace the missing tooth. Options include placing an implant where the tooth was extracted or remaking one large multiunit bridge. Implants are not always possible or practical, and remaking a multiunit bridge that involves the abutments of the two existing bridges can be challenging and becomes much more expensive to the patients. The overcasting technique is a treatment modality that offers a solution in these particular situations.

The correct treatment of a single missing maxillary anterior tooth in the aesthetically prominent area has become more challenging. The missing tooth can today be replaced by one of three prosthodontic treatment modalities--conventional fixed bridge, resin-bonded bridge, and single-tooth implant. The most important factors to be considered are the predictability of aesthetics, the preservation of the enamel shield and the dentinal and pulpal tissue, and the preservation of the periodontium and the alveolar bone. The learning objective of this article is a critical discussion of these three modalities as a replacement of a missing anterior single tooth to aid in selection of the most appropriate treatment in each individual clinical case.

Dentitions seriously compromised by periodontal disease are often in need not only of cause-related periodontal therapy of high quality but also of relevant prosthetic rehabilitation. Contrary to traditional claims, clinical investigations published during the last 2 decades demonstrate that, if adequately treated and controlled, such dentitions can carry fixed, cross-arch bridges on an extremely reduced amount of periodontium, with a good long-range prognosis. It has also been shown that a markedly reduced but healthy and favourably distributed periodontium supporting such constructions can withstand occlusal forces of considerable magnitude. Another controversial topic, related to fixed bridges, involves indications and contra-indications for cantilever segments. Some clinical investigations demonstrate a markedly increased risk of failure if the fixed bridge is provided with cantilever units, while other controlled studies with defined specifications on the design of the constructions exhibit a high success rate after 8 years or more also for bridgework where 2 or 3 cantilever units are included. The force pattern along cantilever segments of both tooth-supported and implant-supported bridges has been extensively studied. The results show that the force distribution depends not only on the occlusal contact pattern and the dimensioning of the cantilever beam, but also on the type of prosthetic construction in the opposite jaw occluding with the cantilever segment. The free-standing, implant-supported bridge has recently become an important treatment modality for rehabilitation of the partially edentulous jaw, and follow-up studies demonstrate a high success rate.(ABSTRACT TRUNCATED AT 250 WORDS)
