Background: Spiritual leadership emphasizes that nurses encounter situations that require compassion, empathy, and a deep understanding of the human experience. Passion can drive nurses' motivation, engagement, and commitment to patient care. Altruism is a core value in nursing that involves selfless concern for the well-being of others. Spiritual leadership might be an essential organizational resource in enhancing followers' inspiring vision, compassion for others, and passion for their work. Aim of the study: To investigate the role of spiritual leadership in the relationship between altruism and passion for work among nurse colleagues. Subjects & method: A multicenter descriptive, cross-sectional research study was conducted at six Nursing Egyptian faculties affiliated with Alexandria, Port-Said, Fayoum, Damanhur, Assiut, and Sohag Universities, with 391 nurse colleagues. Data collection tools: Personal and Job-related Data Questionnaire, Spiritual Leadership Questionnaire (SLQ), Passion Scale, and 9- Self-Report Altruism Scale (9- SRA) were utilized to collect data. Results: The current study verified that nurse colleagues had high levels of perceived spiritual leadership, passion, and altruism (50.9, 47.6, and 40.2, respectively). The results revealed noteworthy positive correlations between spiritual leadership, passion, and altruism (r = 0.644, 0.519, & 0.509). Furthermore, mediation analysis highlighted that spiritual leadership could mediate the effect of altruism on the passion level. Conclusion: This study delivers valued insights into the crucial role of spiritual leadership in augmenting nurse colleagues' altruistic tendencies and passion. Recommendations: Cultivating workplace spirituality on a daily agenda is a key component of management that requires spiritual competencies from nurse leaders. Background: Monkeypox has emerged as a global health concern, necessitating preventive measures and awareness among healthcare students. Understanding nursing students' fears, concerns, and preventive practices regarding Monkeypox can help improve preparedness and response strategies. Aim: This study aimed to assess the levels of fear, concerns, and preventive practices related to monkeypox among nursing students at Beni Suef University, Egypt, during the 2024–2025 academic year. Methods: A cross-sectional descriptive design was employed, involving 505 nursing students systematically randomized recruited. Data collection utilized three validated tools: the Monkeypox Fear Scale, Monkeypox Concern Scale, and Monkeypox Preventive Practices Scale. Reliability was confirmed with Cronbach's alpha values ranging from 0.87 to 0.94. Data analysis included descriptive statistics, Spearman correlation, and multiple linear regression using SPSS Version 23. Results: Most (56.2%) students rarely worried about Monkeypox infection, though 49.2% reported anxiety influenced by media coverage. High preventive practices were noted, with 60.6% frequently practicing hygiene measures. A strong positive correlation between Monkeypox concern and fear (r = 0.646, p < 0.001) and a moderate positive correlation between Monkeypox concern and preventive practices (r = 0.229, p = 0.001). Fear was also significantly correlated with preventive practices (r = 0.432, p < 0.001). Multiple linear regression analysis (R² = 0.216, F = 22.633, p < 0.001) revealed that fear was a strong positive predictor of preventive practices, while Monkeypox concern had a positive but non-significant effect (B = 0.138, p = 0.156). Age was not a significant predictor (B = 0.251, p = 0.637), whereas family income showed a significant negative association (B = -1.885, p = 0.010). Conclusion: The study revealed moderate fear and concerns among nursing students, with generally high adherence to preventive practices. Implication: Findings suggest the need for targeted health education programs to address monkeypox-related concerns and enhance preventive measures, thus improving nursing students' readiness to respond to emerging health threats. Introduction: Smartphones have significantly increased digital engagement among young people due to their ease of use and constant internet access. Nomophobia and the fear of missing out are associated with mobile and internet use, potentially impacting students' mental health and academic performance. Objectives: To provide the prevalence of nomophobia and fear of missing out while shedding light on the role of psychological alienation between them. Methods: A multisite descriptive correlational study was conducted among 1,273 undergraduate nursing students at six Egyptian universities: North Sinai, South Sinai, Port-Said, Suez Canal, Suez, and Damanhur University, Egypt. From June 2023 to November 15, 2023, the students were surveyed using questionnaires on nomophobia, fear of missing out, and psychological alienation. Results: Nursing students experienced moderate to severe levels of nomophobia (37.4%–45.3%) and psychological alienation (45.8%–55.4%). There was a significant positive correlation between nomophobia and fear of missing out (r =.908, p <.001), as well as between nomophobia and psychological alienation (r =.377, p <.001). Psychological alienation was also found to mediate the relationship between fear of missing out and nomophobia, with the indirect effect being statistically significant (indirect effect = 1.000; p <.001). Conclusion: The study highlights the significant prevalence of nomophobia and psychological alienation among nursing students. The findings underscore the complex interplay between digital connectivity issues, psychological disconnection, and the fear of missing out. Caregiving for children with autism spectrum disorder (ASD) poses significant stress for parents, necessitating an exploration of mitigating factors. This study investigates the interplay between Family Sense of Coherence, Psychological Capital and caregiver stress in this context. A total of 205 caregivers of children with ASD participated in this cross‐sectional study. Data were collected on Family Sense of coherence, Psychological Capital (encompassing hope, resilience, optimism and self‐efficacy) and caregiver stress. Structural equation modelling was employed to test the mediation effect of psychological capital between family sense of coherence and caregiver stress. The results indicated a strong positive correlation between family sense of coherence and all subdomains of psychological capital, with coefficients ranging from 0.541 to 0.610. Conversely, psychological capital demonstrated significant negative correlations with various domains of the Kingstone Caregiver Stress Scale, including caregiving, family issues and financial issues (coefficients from −0.443 to −0.427). Furthermore, family sense of coherence showed a direct negative effect on stress (β = −0.384, p < 0.001). Notably, the study revealed a significant mediating role of psychological capital in the relationship between family sense of coherence and caregiver stress, with an indirect effect of family sense of coherence on stress through psychological capital (β = −0.127). The findings underscore the crucial role of family sense of coherence and psychological capital in enhancing psychological resources and mitigating stress among caregivers of children with ASD. These results suggest that interventions aimed at strengthening family coherence and building psychological capital could be effective strategies in alleviating stress among caregivers of children with ASD. Introduction: Academic achievement in nursing students is significantly influenced by their self-esteem and goal orientation. Defensive pessimism, a strategy where students set low expectations to prepare for potential failure, helps them stay motivated and focused. While this cognitive strategy can boost effort and performance, its relationship with self-esteem and goal orientation remains underexplored among nursing students, revealing a gap in understanding how these factors interact to influence academic success. Objective: Assess the level and relationship between defensive pessimism, goal orientation, and self-esteem among nursing students at the Faculty of Nursing. Method: Four hundred students were included in the research using a stratified sampling method followed by a simple randomization sampling technique to select 100 students from each academic year. Tools: The Defensive Pessimism Questionnaire, the Goal Orientation Scale, and the Self-Esteem Scale were used. Results: 56.28% of participants had a low level of defensive pessimism, about two-thirds (62.30%) had a low level of goal orientation, and 70.0% of the students had a mild level of self-esteem. In addition, higher levels of defensive pessimism are significantly associated with stronger goal orientation (r =.291, p <.001) but lower self-esteem (r = −.246, p <.001). Additionally, self-esteem is significantly positively correlated with mastery-performance approach goals (r =.335, p <.001; r =.142, p <.001) and significantly negatively correlated with mastery-performance avoidance goals (r = −.351, p <.001; r = −.041, p <.001). Conclusion: These findings highlight the complex interplay between defensive pessimism, goal orientation, and self-esteem in nursing students. Background: Depressive disorders are prevalent mental health conditions that significantly impair individuals' social functioning and communication competence. These challenges can exacerbate depressive symptoms, creating a vicious cycle that hinders recovery and overall well‐being. Despite the growing recognition of the impact of depressive disorders on social functioning and communication competence, there remains a significant gap in effective interventions addressing these specific areas. Aims: This research aims to investigate the effect of interpersonal effectiveness skill training on social functioning and communication competence among clients with depressive disorder. Design: A quasiexperimental research design. Setting: The research procedure was carried out in the outpatient clinic of Alexandria University Hospital. Subjects: The study group, which included 42 males and 18 females, and the control group had the same number of patients. Tools: After the preliminary assessment of depression, three instruments were used to collect data: a structured interview schedule that gathered sociodemographic and clinical information, the Scale of Communication Competence, and the Social Functioning Scale. Results: The study group showed a positive difference in their social functioning and communication competence score compared with the control group. Implication: The findings of this study have significant implications for the treatment and support of clients with depressive disorder by incorporating structured skill‐building programs into therapeutic regimens for individuals with depressive disorders. Background: Menstrual pain, or dysmenorrhea, can cause physical discomfort and mood swings, potentially impacting the attitudes of female nursing students toward marriage. Effective emotion regulation strategies are essential for managing stress and shaping their perspectives on marriage. This study sought to investigate the relationship between dysmenorrhea, emotional regulation skills, and attitudes toward marriage among female nursing students. Methods: A cross-sectional survey was conducted using a stratified sample of 504 female nursing students from four academic years. The study used a combination of the Working Ability, Location, Intensity, Days of Pain, Dysmenorrhea (WaLIDD) Questionnaire, the Marital Attitude Scale, and the Emotion Regulation Questionnaire to assess various factors among the participants. The Pearson correlation coefficient was utilized to examine the relationships among the three primary variables. Additionally, linear regression analysis was employed to forecast attitudes toward marriage based on factors such as dysmenorrhea, residence, family type, regularity of the menstrual cycle, and age at menarche. Results: The findings of the study are significant, revealing a negative correlation between dysmenorrhea and marital attitudes (r = -0.105, p = 0.019). Dysmenorrhea exhibited minimal and non-significant correlations with emotion regulation strategies, including cognitive reappraisal (r = -0.006, p = 0.898) and expressive suppression (r = 0.013, p = 0.774). In contrast, marital attitudes significantly influenced emotion regulation (β = -0.169, p < 0.001). Moderation analyses indicated significant effects of various factors on marital attitudes, including residence (β = -0.5136, p = 0.0478), family type (β = 5.9923, p = 0.0060), regularity of the menstrual cycle (β = 6.1262, p = 0.0014), and age at menarche (β = 1.5521, p = 0.0491). Conclusion: A significant negative correlation between dysmenorrhea and certain attitudes, specifically optimistic and realistic marital attitudes, was found. This indicates that higher levels of dysmenorrhea are associated with less favorable perspectives in these domains. Additionally, the study demonstrates that negative marital attitudes were linked to poorer emotional regulation. This suggests that individuals with more pessimistic views about marriage may face challenges in effectively managing their emotions. Background. Novel educational strategies are essential to cultivate a cadre of nurses with the requisite skills, critical thinking abilities, and adaptability demanded by contemporary healthcare contexts. The shift towards various learning modalities in nursing education, including online, hybrid, and in‐person formats, raises questions about their impact on student engagement and academic self‐concept. Objectives. This study aimed to assess and compare the levels of student engagement and academic self‐concept among nursing students engaged in different study modalities (online, hybrid, and in‐person). Methods. In this descriptive comparative study, 203 nursing students were evaluated using the university student engagement inventory (USEI) and academic self‐concept questionnaire (ASCQ). Participants were divided into groups based on their involvement in online, hybrid, or in‐person study modes. Differences and correlational analysis within variables were performed to evaluate the comparison between groups. Results. Significant differences were observed in the USEI scores among the study modalities: online (mean = 45.7, SD = 5.6), hybrid (mean = 48.8, SD = 6.1), and in‐person (mean = 50.4, SD = 6.2), with in‐person students displaying the highest levels of engagement (F = 10.663, P < 0.001). Academic self‐concept scores also varied notably (F = 9.422, P < 0.001) between modalities, particularly between online (mean = 54.8, SD = 5.3) and in‐person students (mean = 58.8, SD = 7.7). Correlational analysis within each modality revealed significant relationships among the subdomains of USEI and ASCQ. Conclusion. The study demonstrates that the mode of study substantially influences nursing students' engagement and academic self‐concept, with in‐person learners showing higher scores in both domains. Background: Climate changes have led to health and environmental risks, so it has become essential to measure climate change literacy among the entire population, especially nursing students. The significant role of nursing students in raising public awareness and future healthcare roles emphasizes assessing the predictors of climate change literacy among nursing students. Aims: This study seeks to identify the predictors of climate change literacy among nursing students in A Multi-Site Survey. Design: A multi-site descriptive cross-sectional study adheres to the guidelines outlined in A Consensus-Based Checklist for Reporting Survey Studies collected for five months, from the 1st of July 2023 to November 2023. The study participants comprise 10,084 nursing students from all 27 governments in Egypt. The researcher used the Predictors of Nursing Students' Climate Change Literacy scale in this study. Data was collected, with 25 min average time to complete. Backward multiple linear regression was used to identify these predictors. Results: In the current study, nursing students demonstrated a moderate understanding of climate science (mean score 14.38), communication and advocacy skills (mean score 14.41), and knowledge of adaptation and mitigation strategies (mean score 13.33). Climate health impacts (mean score 17.72) emerged as the domain with the highest level of knowledge. No significant differences in climate literacy were observed across diverse student backgrounds (all p-values were > 0.05). Perceived faculty knowledge of climate change positively correlated with all four domains of climate literacy and emerged as a significant predictor in multiple linear regression analyses (all p-values were < 0.001). Implication. While our findings highlight significant predictors of climate literacy, it is essential to recognize that these results identify associations rather than causal relationships. Based on these associations, it is recommended that nursing professionals be equipped with comprehensive knowledge of climate adaptation strategies to better advocate for and implement effective public health measures. Background: Toxic leadership is abusive, destructive, and detrimental to nurses, can decrease nurse morale, increase stress levels, diminish organizational performance, and cause employee emotional exhaustion. Emotional exhaustion, a key component of burnout, has been linked to decreased job performance and increased likelihood of engaging in deviant behaviors at work. Organizational cynicism, as a negative attitude or distrust towards the organization and its leadership, may exacerbate the relationship between toxic leadership and workplace deviance. This study aims to explore how toxic leadership, workplace deviance, emotional exhaustion, and organizational cynicism are interrelated in the nursing profession. Method: A multicenter descriptive, cross-sectional research design was conducted at three university hospitals affiliated with three Egyptian governorates: Zagazig, El-Fayoum, and Alexandria. Two hundred forty-three nurses completed the Personal and Job-related Data Questionnaire, Toxic Leadership Scale, Emotional Exhaustion Scale, Workplace Deviance Scale, and Organizational Cynicism Scale. Mediation analysis using SPSS PROCESS revealed that emotional exhaustion indirectly influences the relationship between toxic leadership and workplace deviance, while hierarchical multiple regression showed that organizational cynicism moderates this relationship, with linear regression confirming the predictive impact of these variables on workplace deviance. Results: Toxic leadership exhibits a substantial positive influence on workplace deviance (B = 16.132, p < 0.001), as does emotional exhaustion (B = 8.760, p < 0.001) and organizational cynicism (B = 5.376, p = 0.036). Furthermore, the interaction terms of toxic leadership, organizational cynicism, and emotional exhaustion are significant (B = -3.012, p = 0.036). The negative coefficient of the interaction term implies a mitigating impact, indicating that the combined presence of high toxic leadership, high emotional exhaustion, and high organizational cynicism may counterintuitively reduce workplace deviance (t = -2.110). Conclusion: These results suggest that while toxic leadership, emotional exhaustion, and organizational cynicism each independently increase workplace deviance, their combined presence may unexpectedly reduce deviant behaviors. Implication for nursing: These results highlight the significance of tackling toxic leadership practices and cultivating a positive organizational culture to enhance a healthier work environment and reduce instances of workplace deviance. Background: Modern undergraduate nursing students face unique challenges as digital natives balancing internet activities with the substantial academic demands of nursing studies. Given the detrimental effects of internet addiction on students' academic performance and well-being, having time management skills is crucial. Aims: To assess the prevalence and levels of internet addiction and time management and their association among undergraduate nursing students. Design: A cross-sectional, survey-based research design was used. Setting: The Faculty of Nursing at Alexandria University in Egypt. Subjects: A stratified random sample consisting of 825 undergraduate nursing students. Tools: The internet addiction test and time management questionnaire were utilized to collect data. Findings: Internet addiction was prevalent among 98.8% of students, with 56.0% exhibiting mild levels, 40.0% showing moderate levels, and 2.8% having severe levels. A statistically significant negative correlation was found between students' internet addiction and overall time management (r= − 0.387, p < 0.001). Conclusion: A considerable level of internet addiction was revealed among the great majority of undergraduate nursing students; however, many students also demonstrated strong time management skills. Furthermore, internet addiction and overall time management were negatively associated, indicating that students with higher levels of internet addiction tend to have poorer time management abilities. Background: Kinesiophobia could act as a significant barrier against physical activity following cardiac procedures worsening cardiovascular health problems and potentially leading to conditions like hospital-acquired anxiety and depression among patients with cardiovascular disease (CVD). Nurses are the vanguard health care team who can aid patients in taking proactive steps to overcome fear of movement following cardiac procedures. Aim: The overarching aim is to investigate the relationship between kinesiophobia, anxiety and depression, and patients' perception of nursing care. Method: A descriptive correlational research design in two rural hospitals, conducted at cardiac intensive care units of Kafr Eldawar Hospital and Damanhur Medical National Institute. Data were collected from 265 nurses, using the following patient-reported outcome measures, the Tampa Scale for Kinesiophobia (TSK), the Hospital Anxiety and Depression Scale (HADS), the Person-Centered Critical Care Nursing Questionnaire (PCCNP) and the patients' demographic and clinical profile. Result: A significant negative correlation was found between HADS and PCCNP (r: -0.510, p < 0.001) however, Kinesiophobia was significantly and positively correlated (r: 0.271, p < 0.001). A direct effect of PCCNP in the presence of the mediator was found to be not statistically significant (-0.015, CR = 0.302, p = 0.763). Nonetheless, PCCNP indirectly affects kinesiophobia through HADS (p=-0.099). Implication for nursing practice: Customizing individualized cardiac rehabilitation (CR) programs based on the emotional experience of cardiac patients will be conducive to rehabilitation and prognosis for patients, thereby lessening the physical burden and improving their quality of life. Background: Climate change, a pervasive global phenomenon, exerts discernible impacts on the physical, social, and psychological dimensions of well-being. The apprehension surrounding this complex environmental issue has reached a critical juncture, with over 76,000 individuals across more than thirty nations expressing profound levels of concern, characterizing their anxiety as either "very" or "extremely" pronounced. This surge in awareness regarding the potential consequences of climate change has given rise to an emergent and escalating challenge known as climate anxiety. This distinctive form of anxiety manifests through profound feelings of fear, helplessness, and despair elicited by the impending repercussions of climate change. Notably, the intersection of climate anxiety with occupational domains, particularly within the context of Nursing University Colleagues, suggests a nuanced relationship with job engagement, wherein the psychological responses to climate change may influence professional commitment and involvement. Aim of the study: To examine the correlation among Climate Anxiety, Environmental Attitude, and Job Engagement among Nursing University Colleagues comprising eight distinct nursing faculties. Design: A multicenter descriptive, cross-sectional research design study followed. Subject: Three hundred fifty-nine participants from the Centre, Delta, West, Suez Canal, and Upper regions of Egypt using a stratified random cluster sampling technique. Measurements: Social and health related to climate data structured questionnaire, climate anxiety scale, environmental attitude inventory, and job engagement scale. Results: The influence of demographics on climate anxiety, environmental attitude, and job involvement was not observed. Nevertheless, geographical variations emerged as a noteworthy factor. A statistically significant inverse correlation was identified between climate anxiety, job engagement dimensions, and the overall score of environmental attitudes. Conclusion: Climate anxiety was strongly associated with environmental attitudes and job engagement among nursing university colleagues. Higher climate anxiety is associated with a lower attitude towards the environment and decreased job engagement. Additionally, a higher attitude towards the environment is associated with decreased overall engagement in participants' jobs. Implications: The study's patterns make it clear how important it is to provide targeted psycho-educational interventions to help reduce climate anxiety among the group of nursing university colleagues. The imperative lies not only in alleviating the immediate psychological distress associated with heightened climate anxiety but also in fostering adaptive coping mechanisms. Background: Carpal tunnel syndrome (CTS) is a prevalent condition characterized by hand pain, tingling, and numbness. The severity of symptoms and functional status in CTS patients may be influenced by perceived stress and how individuals cope with it. However, scarce knowledge exists about the role of coping strategies as moderators in this relationship. Unfolding the role of perceived stress and coping strategies for CTS management will help the nurse to provide comprehensive and tailored nursing care. This will ultimately improve patient comfort, functionality, and quality of life. Purposes: This study aimed to examine the role of coping strategies (adaptive and maladaptive) in the relationship between perceived stress and both symptoms severity and function status among those patients. Method: We employed a multisite, correlational study design with moderation analysis. The study included 215 patients with CTS from neurosurgery outpatient clinics at three hospitals in Egypt. After obtaining their consent to participate, eligible participants completed anonymous, self-reported measures of perceived stress, the brief COPE inventory, and the Boston Carpal Tunnel Questionnaire. Demographic and biomedical data were also collected. The questionnaire took about 20 min to be completed. The data was collected over six months, starting in February 2023. Results: The results showed that perceived stress, adaptive coping, and maladaptive coping were significant predictors of symptoms severity and functional status. Adaptive coping moderated the relationships between perceived stress and both symptoms severity and function status, while maladaptive coping did not. The interaction between perceived stress and adaptive coping explained a moderate effect on symptoms severity and function status after controlling for the main effects and the covariates. Conclusion: This study explored the relationship between perceived stress, coping strategies, and outcomes in patients with CTS. The results indicate that nurses play a vital role in assessing and assisting patients to adopt effective coping strategies to manage perceived stress and alleviate symptoms and functional impairment. Moreover, the findings support the need for psychological interventions that address both perceived stress and coping strategies as a way to enhance the functioning status and quality of life of patients with CTS. Background: Nomophobia, the crippling fear of being disconnected from mobile devices, is a burgeoning global concern. Given the critical nature of the profession of nursing students, understanding nomophobia's prevalence and potential impacts on patient care and professional conductors becomes even more crucial. Aim: This study explores the relationship between nomophobia and impulsive sensation-seeking among nursing students in Egypt. Design and methods: A multicenter cross-sectional survey was conducted with 1626 nursing students randomly selected from three universities across Egypt. Tools: The Arabic versions of the Nomophobia and Impulsive Sensation Seeking Questionnaires were employed to gather data. Results: Our findings revealed that 40.3% of the surveyed nursing students exhibited severe nomophobia, indicating a substantial dependence on their mobile devices. Notably, this high prevalence was accompanied by a strong tendency towards impulsive sensation-seeking behaviors. Furthermore, a stepwise regression analysis identified several significant predictors of nomophobia (p < 0.001). Impulsive sensation-seeking, year of study, average daily smartphone usage, and age emerged as key factors, explaining 27.5% of the variability in nomophobia scores. Conclusion: The prevalence of nomophobia among Egyptian nursing students is undeniable, highlighting their substantial reliance on mobile devices. A significant association with impulsive sensation-seeking behavior further compounds this dependence. Factors such as impulsive sensation seeking, year of study, average daily smartphone usage, and age were identified as significant predictors of nomophobia. Recognizing these factors as key predictors of nomophobia is crucial for designing effective interventions and psychotherapies. Background: Cardiopulmonary resuscitation (CPR) is considered one of the most stressful experiences in critical care nursing; it directly and indirectly leads to compassion fatigue and burnout. Aim: Determine the levels of and relationship between postcode stress and compassion fatigue. Design and methods: A descriptive-correlational study using 300 critical care nurses from five intensive care units in two hospitals was conducted. Tools: Demographic and work-related data, the Postcode Stress Scale, and the Professional Quality of Life Scale: Compassion Fatigue subscale. Results: Nurses had moderate to high postcode stress and compassion fatigue (67.98 ± 16.39 and 65.40±14.34, respectively). Moreover, there was a significant positive correlation between postcode stress, burnout (r=0.350, p=<0.001), secondary traumatic stress (r=0.518, p=<0.001), and subsequently, compassion fatigue (r=0.449, p=<0.001). In addition, higher levels of postcode stress were associated with higher levels of compassion fatigue with its subscales: burnout and secondary traumatic stress, with a coefficient of determination for compassion fatigue (0.199), burnout subscale (0.121), and secondary traumatic stress (0.266). Conclusion: Critical care nurses involved in resuscitation experiences are susceptible to postcode stress, burnout, secondary traumatic stress, and compassion fatigue. There is a significant relationship between these factors, with higher levels of postcode stress contributing to higher levels of compassion fatigue and its subscales: burnout and secondary traumatic stress. These results highlight the importance of addressing and managing the psychological well-being of nurses in resuscitation settings to mitigate the adverse effects of stress and promote their overall resilience and well-being. Objective Delayed detection of LN associates with worse outcomes. There are conflicting recommendations regarding a threshold level of proteinuria at which biopsy will likely yield actionable management. This study addressed the association of urine protein:creatinine ratios (UPCR) with clinical characteristics and investigated the incidence of proliferative and membranous histology in patients with a UPCR between 0.5 and 1. Methods A total of 275 SLE patients (113 first biopsy, 162 repeat) were enrolled in the multicentre multi-ethnic/racial Accelerating Medicines Partnership across 15 US sites at the time of a clinically indicated renal biopsy. Patients were followed for 1 year. Results At biopsy, 54 patients had UPCR <1 and 221 had UPCR ≥1. Independent of UPCR or biopsy number, a majority (92%) of patients had class III, IV, V or mixed histology. Moreover, patients with UPCR <1 and class III, IV, V, or mixed had a median activity index of 4.5 and chronicity index of 3, yet 39% of these patients had an inactive sediment. Neither anti-dsDNA nor low complement distinguished class I or II from III, IV, V or mixed in patients with UPCR <1. Of 29 patients with baseline UPCR <1 and class III, IV, V or mixed, 23 (79%) had a UPCR <0.5 at 1 year. Conclusion In this prospective study, three-quarters of patients with UPCR <1 had histology showing class III, IV, V or mixed with accompanying activity and chronicity despite an inactive sediment or normal serologies. Objectives Pure membranous (class V) LN is considered a less aggressive phenotype, but tissue fibrosis and chronic kidney disease may still develop. This study aimed to elucidate the prognostic value of a history of class switch in pure membranous LN. Methods We included LN patients with at least two clinically indicated kidney biopsies. New onset of end stage kidney disease (ESKD) was defined as estimated glomerular filtration rate <15 ml/min/1.73 m2, initiation of dialysis or kidney transplantation. Results Among 220 patients (542 biopsies), 199 (90%) were female, and 118 (54%) were African American, 59 (27%) Caucasian, with median age of 28 years at the first kidney biopsy. Patients with pure class V in a first biopsy converted to proliferative LN in 41% of cases. Pure class V in a repeat biopsy was preceded by proliferative LN in 52%. Trajectory analysis of up to four repeat biopsies revealed that ISN class switch may happen at any time, even after multiple biopsies with the same class. New onset ESKD was observed within 2 years in 5/56 (9%) patients with pure class V in a repeat biopsy. All five patients had proliferative LN in the first biopsy (log rank P  = 0.024). Conclusions The conversion from proliferative to membranous (and vice-versa) is frequent in SLE. It can occur at any time in the course of disease, limiting the prognostic value of the first biopsy.
