3. Efectos de la violencia en la salud de víctimas del conflicto armado en el municipio de Tumaco, Colombia: una revisión narrativa.

- Abstract
Objective: To review the existing literature on the effects of violence on the health of Colombian armed conflict victims in the municipality of Tumaco, Colombia. Methodology: A narrative review of literature was conducted between 2012 and 2022. Potentially relevant qualitative studies on the municipality of Tumaco were included. Quality assessment was supported by the CASPe critical reading program. The analysis considered the theoretical model of the social determinants of health. Results: Fourteen qualitative studies were included. In these, 93% referred to murders, 50% to the deficit in the health system, 43% to physical health problems, 36% to the repercussion on mental health, 14% to the consumption of psychoactive substances, 14% to the presence of disabilities, and 7% to the limited access to mental health care as consequences of the Colombian armed conflict's violence. Conclusion: The dynamics of the Colombian armed conflict in Tumaco exacerbate the vulnerability conditions found in political and economic determinants (poor quality and limited access to public services, employment, and health), sociocultural determinants (fragmentation of social environments), and environmental determinants (presence of contaminants that affect natural resources). The findings provide decision-makers in health matters with elements to perform risk assessments oriented to prevent and mitigate harm to victims of the Colombian armed conflict. 4. Salud global y pandemia de covid-19: un análisis holístico.

- Abstract
There are various interpretations and exegeses on the COVID-19 pandemic, among which are those that attribute the structural causes to the current world development model, its relationship with nature and the capitalist system, the political-economic development of the phenomenon and the socio-health behavior of SARS-CoV-2. The purpose of this review essay is to analyze in a holistic manner the political and socio-health context of the pandemic, both global and regional (Latin America), and its areas of intervention (alternatives), by defining some conceptual and political links between the pandemic and global health (thematic axes). To this end, a review of pandemic and global health in academic databases (PubMed®, LILACS and SciELO) and in academic Google over the last 5 years is carried out, and a thematic definition is made (3 thematic axes): 1) the role of the political factor, globalization and governments in resolving issues related to the pandemic; 2) the socio-health analysis of the pandemic, and 3) the interventions to deal with it. It is concluded that the impact of the pandemic on global health practice has been great, accentuating structural shortcomings in public health, both technical and political, some attributable to the State and others to health professionals. 5. Lesiones proliferativas ginecológicas y victimización sexual en Córdoba, Argentina.

- Abstract
Objective: To explore the prevalence of sexual victimization suffered in childhood or by a partner in adulthood, and the association of both variables with a history of gynecological proliferative lesions (breast dysplasia, cervical dysplasia, breast cancer, and cervical cancer) in a sample of women from Córdoba, Argentina. Methodology: 115 women over 18 years of age were surveyed in 14 cities in the province of Córdoba during 2017 and 2018. Their sociodemographic and health data, history of gynecological proliferative lesions, sexual victimization in childhood or by a partner in adulthood were assessed. Bivariate and binary logistic regression analyses were performed to calculate associations between variables. Results: 40.8 % of the participants reported a history of proliferative lesions. About 30 % of the total sample indicated having suffered at least one experience of sexual victimization during their lifetime. Being forced to have unprotected sexual intercourse by a partner was significantly more frequent in women with a history of gynecological proliferative lesions (12.8 %; OR = 9.80, 95 % CI: 1.14-84.38). Sexually transmitted infections (STIs) were the best predictor of a history of gynecological proliferative lesions (OR = 26.52, 95 % CI: 5.15-136.65), followed by partner-caused sexual violence in adulthood (OR = 13.84, 95 % CI: 1.29-148.78). Conclusions: The association between STIs and gynecologic proliferative lesions could be influenced by behaviors such as partner-caused sexual violence. The results highlight the need to identify potential social determinants of women's sexual and reproductive health in Argentina and the region. [ABSTRACT FROM AUTHOR]

10. Distribución espacial de la mortalidad por COVID-19 y su asociación con condiciones sociodemográficas en Antioquia-Colombia.

- Abstract
Antioquia has shown wide subregional and municipal variations in the risk of mortality due to COVID-19. However, relevant factors to explain the geographical pattern of mortality, other than individual conditions, are unknown. Objective: To explore the possible influence of municipal characteristics on the risk of mortality due to Covid-19 in Antioquia, adjusted by individual conditions. A cross-sectional secondary data analytical study was used, using data from all positive cases of Covid-19 identified between March 9, 2020 and October 29, 2021 in Antioquia-Colombia. Methods: A multilevel logistic model was adjusted to analyze the association between mortality due to Covid-19 and socioeconomic and demographic predictors of the municipalities, independent of age, gender, and ethnicity. Results: After controlling for indivisual variables, 12 out of the 16 municipal-level variables were shown to be independently associated with Covid-19 mortality. The results also suggest a negative gradient of mortality due to Covid-19 where, municipalities with precarious conditions, showed higher risks. Conclusions: These results suggest the need to take into account not only the immediate environment but also the broader environment to which people belong to prevent the spread of the virus and its serious consequences. Objective: to compare the indicators of access to and use of health services by community elderly people according to different levels of social vulnerability. Methods: cross-sectional study carried out with 805 elderly people. A questionnaire from the National Household Sample Survey was used to measure indicators of access to and use of health services; and Social Vulnerability Index to assess social vulnerability. Results: elderly people living in areas of high/very high vulnerability had a higher proportion of negative outcomes in terms of bond with the service (p <0.001), access to medication (p <0.001) and access to the dentist (p < 0.001). Most elderly people living in more vulnerable areas used the health center (54.2%); most residents in low vulnerability areas sought private care (47.2%) (p <0.001). Conclusion: elderly people living in areas of greater social vulnerability had worse results related to indicators of access to health services, although they use the public service more often. Objective: To analyze social determinants of health of homeless population vulnerable to tuberculosis. Methods: Descriptive, exploratory study with a quantitative approach. The collection occurred between July and December 2017, with homeless people from two municipalities of the metropolitan region of Cariri, using an instrument of its own elaboration. The analysis was designed considering the model of Dahlgren and Wintehead for the social determinants of health. Results: 70 people were interviewed, predominantly male, aged between 18 and 32 years, mixed race/color and low education level. Most of them said they were smokers, made use of alcohol, did not use medications, did not maintain contact with their families and had poor living and working conditions and pointed all of those as motives to be homeless. They declared that they seek mainly the primary health care as a service and did not use government services, in spite of having an income below one salary. The ones who reported cough underwent bacilloscopy, but all of them had a negative result for tuberculosis. Conclusion: The identification of the social determinants of health is important to know the health demands of the homeless population and enables the strengthening of policies to combat the weaknesses these people are exposed to. Objective. Analyze inequalities in self-perceived health among population groups located at the intersections of gender identity, ethnicity, and education level in countries of the Americas, classified by income level. Methods. Panel data from the World Values Survey were used for the period 1990--2022. The study sample included 58 790 people between 16 and 65 years of age from 14 countries in the Americas. The dependent variable was poor self-perceived health, and the independent variables were gender, education level, and ethnicity. A multi-categorical variable with 12 strata was created for the intercategorical intersectionality analysis. An analysis of individual heterogeneity and diagnostic accuracy was performed using five logistic regression models, adjusted by age and by survey wave. Results. A clear and persistent intersectional gradient for poor self-perceived health was observed in all country disaggregations by income. Compared to the category with the most advantage (men of majority ethnicity and higher education), the other groups had increased risk of poor health, with the highest risk among women of minority ethnicity and in Indigenous peoples with less than secondary education (three to four times higher). In addition, women had a higher risk of poor health than men in each pair of intersectional strata. Conclusions. The intersectional analysis demonstrated a persistent social gradient of self-perceived ill health in the Americas. [ABSTRACT FROM AUTHOR]

Equity has become a key objective for the Pan American Health Organization (PAHO). This is the result of multidimensional movements with a history that can only be understood in the light of social relations that structure the processes we refer to as "health". From an initial concern for maritime trade to a current interest in the social determinants of health, PAHO has strengthened its efforts to make the concept of health equity the ultimate goal of its actions. The idea of health focused on the individual has given way to the idea of health for all people, which considers the differences between being and doing in the world. These changes have produced policies, analyses, and studies in which gender and cultural diversity are cross-cutting themes that must be considered in all planning and implementation of health policies in the Region of the Americas. 17. Factors associated with COVID-19 length of hospitalization and mortality during four epidemic waves, March 2020--November 2021, Suriname.

Objective. To determine the sociodemographic risk factors associated with coronavirus disease 2019 (COVID- 19) mortality in Suriname. Methods. This was a retrospective cohort study. All registered deaths from COVID-19 in Suriname (n=1112) between March 13, 2020 and November 11, 2021 were included. Data were collected from medical records and included demographic variables and hospitalization duration of patients who died. Descriptive statistics, chi-squared tests, ANOVA models, and logistic regression analyses were used to determine associations between sociodemographic variables, length of hospitalization, and mortality during four epidemic waves. Results. The case fatality rate over the study period was 22 per 1 000 population. The first epidemic wave was from July to August 2020, the second from December 2020 to January 2021, the third from May to June 2021, and the fourth from August to September 2021. Significant differences were found in the number of deaths and hospitalization duration by wave (p<0.001). Patients were more likely to have a longer hospitalization during the first (OR 1.66; 95% CI: 0.98, 2.82) and third waves (OR 2.37; 95% CI: 1.71, 3.28) compared with the fourth wave. Significant differences in mortality were also seen between ethnicities by wave (p=0.010). Compared with the mixed and other group, people of Creole ethnicity (OR 2.7; 95% CI: 1.33, 5.29) and Tribal people (OR 2.8; 95% CI: 1.12, 7.02) were more likely to die during the fourth wave than the third wave. Conclusions. Tailored interventions are needed for males, people of Creole descent, Tribal and Indigenous peoples, and people older than 65 years. [ABSTRACT FROM AUTHOR]

18. Homicide among young people in the countries of the Americas.

Objective. To examine the homicide trends among young people (10--24 years), adolescents (10--19 years), and young adults (20--24 years) in 33 countries in the Americas between 2000 and 2019, with a focus on inequalities between countries in the burden of homicides. Methods. An ecological study was performed using estimated deaths from 33 countries. Age-adjusted rates, percentage change (PC), average annual percentage change (AAPC), and relative risk (RR) were estimated; besides, analysis on social inequalities was performed. Results. In the Americas between 2000 and 2019, homicide has been the leading cause of death with 54 515 deaths on average each year and an age-adjusted rate of 23.6 per 100 000 among young people. The highest rate was found in the Andean subregion (41.1 per 100 000 young people), which also produced the highest decrease (PC = --37.1% and AAPC = --2.4%) in the study period. The risk of homicide in young men is 8.1 times the risk in young women, and the risk in young adults is 2.5 times the risk in adolescents. The three countries with highest risk of homicide for young people are Venezuela (relative risk [RR] = 35.1), El Salvador (RR = 28.1), and Colombia (RR = 26.7). The estimated excess mortality was 26.8 homicides per 100 000 in the poorest 20% of countries compared to the richest 20% of countries in the period 2000--2009, and it decreased to 13.9 in the period 2010--2019. Conclusions. The results of this study add to the knowledge of homicide among young people and can be used to inform policy and programming in countries. Objective: to know the discourses attributed by students entering the Bachelor's course in Nursing to the epistemic units health and disease. Method: exploratory-descriptive, qualitative approach. The research was conducted with 29 beginners of the Bachelor's degree in Nursing at a public university located in the city of Boa Vista, Roraima state. The strategy for data production was the Dramatic Play using the image inducer. The raw data were recorded by scribes during class and analyzed according to the speech. Results: known discourses involved health, social determinants of health, diseases, nursing performance in health-disease processes and expanded discourses on the Unified Health System. Final considerations: through the dramatic play, the nursing freshmen highlighted discourses on health promotion, disease prevention, social elements in health interface, nursing know-how and expanded reflections on health system. Objective. Establish the disease burden of malignant mesothelioma (MM) in Colombia between 2015 and 2020, and its association with the subnational sociodemographic development index (SDI) and with asbestos sites. Methods. Mixed ecological study of the Colombian population diagnosed with MM (according to ICD-10) from 2015 to 2020. The global burden of disease (GBD) was estimated using the methodology proposed by Murray and Lopez, based on prevalence and mortality data obtained from official sources. The subnational (departmental level) SDI was estimated as a measure of socioeconomic development. Linear regressions were established with the GBD, SDI, and documented asbestos sites. Results. The estimated GBD of MM in Colombia during 2015-2020 was 51.71 disability-adjusted life years (DALYs) per 1 000 000 inhabitants (15 375.79 total DALYs), with predominance in people over 50 years of age (91.1%) and males (66.4%). Bogotá and Valle del Cauca were the departments with the highest number of adjusted DALYs. Bogotá had the highest SDI and Guainía and Cesar had the lowest. There was evidence of an association between DALYs and SDI, explaining 22.8% of DALYs. Conclusion. Malignant mesothelioma is the cause of a large number of DALYs, predominantly in the departments with greater socioeconomic development and with companies that used to use asbestos. Abortion is one of the main causes of maternal mortality, because health inequalities and the nonexistence of public health policies that protect women and allowing them to perform abortion on adequate sanitary conditions. The induced and unsafe abortion is a product of several social determinants, like gender, sociodemographic, ethnic-racial and socioeconomic factors, with direct results on women's health, like comorbidities and mortality. This theoretical reflection aims to understand the abortion-related Social Determinants of Health and the existing social protection policies in Brazil. The impact of the unsafe abortion as a public health problem and the maintenance of punitive laws leads to underreporting and lack of the real set of the abortion problem and that makes the creation of effective public policies unfeasible. The researches about abortion from Social Determinants of Health perspective are fundamental to identify what influences this phenomenon on order to create, implement and evaluate equanimous and effective health policies. Objective: to analyze the epidemiological data of Severe Acute Respiratory Syndrome (SARS), and the access to health of black people in Brazil during the pandemic, comparing them with previous years. Method: epidemiological, descriptive, and quantitative study of SARS cases in black patients, reported in the information system on epidemiological surveillance of influenza of the Ministry of Health, between 2017 and 2020. Results: it was observed that, in 2020, there was an increase in the prevalence of SARS cases in blacks, in addition to a significant increase in lethality when compared to the general population. Furthermore, it was found that mortality was significantly higher in individuals with SARS who required ventilatory support. Conclusion: skin color is a determinant of several social and health vulnerabilities in Brazil and worldwide, having a direct impact on the increased morbidity and mortality of the black population during the pandemic. Objective. To identify the prevalence of self-reporting of symptoms and access to testing and diagnosis of coronavirus-19 disease (COVID-19), as well as its association with social determinants of health (SDH). Methods. Cross-sectional study with a sample of 11 728 men and 12 612 women over the age of 17, based on the National Household Survey 2020. The dependent variables were the self-reporting of symptoms, access to testing, and a positive COVID-19 test. The independent variables were age, educational level, area of residence and geographic area, ethnicity, type of household, income per capita, occupation, and health insurance. Prevalences, bivariate associations, and binomial logistical regression models (odds ratio (OR), and 95% confidence interval (CI95%) were calculated. Results. Of the total individuals included, 16% reported symptoms, 10% a test, and 4.2% a positive COVID-19 test. Inequalities were observed in the reporting of COVID--19 symptoms, with a higher probability in women whose income had fallen (OR: 1.7; CI95%: 1.2-2.4) and unemployed persons (OR: 1.2; CI95%: 1.1-1.4 for men and OR: 1.3; CI95%: 1.5-1.5 for women). In contrast, with respect to access to diagnostic tests, the highest probability was observed in people with higher education (OR: 2.4; CI95%: 1.9-2.9 for men and OR: 2.7; CI95%: 2.2-3.4 for women), whose income was maintained (OR: 1.5; CI95%: 1.3-1.9 for men and OR: 1.7; CI95%: 1.4-2.0 for women) and those in the highest quartile of per capita household income (OR: 2.0; CI95%: 1.6-2.5 for men and OR: 1.6; CI95%: 1.3-2.0 for women). The probability of reporting symptoms and getting tested, and being diagnosed with COVID-19 increased with age for people with health insurance and those living in the llanos region; however, it decreased for residents of rural areas. We analyzed the type of health insurance (contributory, subsidized, or none) and its relationship to ethnic-racial status and predisposing variables (sex, age, marital status), demographic variables (area and region of residence), and socioeconomic variables (education, type of employment, income, and unmet basic needs) through simple and multivariate regression analyses. Association between ethnic-racial status and type of health insurance was estimated using odds ratios (OR) and their 95% confidence intervals, through a multinomial logistic model. Results. A statistically significant association was found between ethnic-racial status and type of health insurance. In comparison with the contributory system, the probabilities of being a member of the subsidized system were 1.8 and 1.4 times greater in the indigenous population (OR = 1.891; 95%CI: 1.600-2.236) and people of African descent (OR = 1.415; 95%CI: 1.236-1.620), respectively (p <0.01) than in the population group that did not identify as belonging to one of those ethnic-racial groups. Conclusions. There is an association between ethnic-racial status and type of insurance in the contributory and subsidized health systems in Colombia. Ethnic-racial status is a structural component of inequity in access to health services and heightens the disadvantages of people and population groups with low socioeconomic status. A central debate in contemporary public health revolves around the social determinants of health in the model of the World Health Organization (who) and Latin American social medicine. Considering that in the health literature there are multiple criticisms of the who approach such as its positivist emphasis, reductionism of the ontology of the social and linearity of its methods; at the same time that there is a low amount of health texts about "critical realism" as the epistemological foundation of the Latin American current; The present work exposes some historical concepts of the Latin American proposal and tries to deepen the development of its epistemological positioning from the "critical realism". Critical realism demonstrates a sort of epistemological "naivety" in the who model, demonstrates that the difference of both approaches is not reducible to a semantics or social approach of the health, and highlights operational challenges for the model Latin American. Objective: To present the findings of a study about social inequalities based on experiences of care and treatment of patients affected by type 2 Diabetes Mellitus and to provide theoretical and methodological elements to analyze social inequalities in health. Methodology: Qualitative study based on biographical stories of patients diagnosed for more than one year having different socioeconomic and sociocultural features was carried out. In an attempt to have representation of diverse social classes, genders and ethnic-racial conditions, cases were selected from different healthcare facilities of the Santiago de Cali municipality (Colombia). Results: Three interrelated domains which constitute conditions of inequality throughout life and in experiences of the care and treatment of diabetes were identified. They were health domain, which comprises the access and use of health services and satisfaction of health needs, health care and health results; socioeconomic domain, which includes social spaces, social positions, resources, sociocultural frames of social hierarchy formation; and finally, context domain, which involves policies and economic, social and cultural features of the patient's environment. Conclusions: In order to analyze health inequalities, not only understanding their manifestations in organisms or access to health services is necessary but also the comprehension of their social development. Four key elements for the development of inequalities were identified: the territory, the link between education and employment, gender, and various forms of violence. [ABSTRACT FROM AUTHOR]

Objective: To analyze the social determinants influencing child mental suffering. Method: This is an integrative review with searches conducted in April and May 2019, through three electronic databases. In the initial selection, 70 articles were found, which after applying the inclusion and exclusion criteria were restricted to nine primary articles that were considered for research purposes. Results: It was noticed that the social determinants corroborate the child mental suffering, among them the traumatic experiences and family disorders. Such determinants negatively influence the lives of these children as adults, if they are not properly perceived and treated. Conclusion: It is also observed that such theme is little discussed in the scientific environment in the emphasis that is placed in this study, but it is noteworthy that over the last years have gained worldwide notoriety for its importance in the social and health environment. Objective. To assess the association between intersectional disadvantage and clinically significant depressive symptoms (CSDS), describing the magnitude of social inequalities in the prevalence of symptoms among adult women in Tijuana, Mexico. Methods. This was a cross-sectional study. CSDS were assessed using the Centers for Epidemiological Studies Depression Scale among a probability sample of 2 345 women from 18 - 65 years of age in 2014. CSDS prevalence was calculated according to categories of three social stratifiers: socioeconomic status (SES), educational attainment, and fertility (number of children). Social inequality was measured with the slope index of inequality (SII) and the concentration index (CIx). Intersectionality among stratifiers was explored descriptively and with multivariable regression analysis. Results. CSDS prevalence was 17.7% (95%CI: 15.1% - 21.0%). The SII and CIx showed inequity in all social stratifiers. The absolute difference in CSDS prevalence between the lowest and highest ends of the SES gradient was 21.9% (95%CI: 21.5% - 22.4%). Among the most disadvantaged women, i.e., those at the intersection of lowest SES, lowest educational attainment, and highest fertility, the CSDS prevalence was 39.5% (95% CI: 26.0% -52.9%). Conclusions. Disadvantage along multiple axes was associated with CSDS. Efforts to improve the mental health of women should include equity-oriented policies that address its social determinants. Introduction: Family visits become a significant learning scenario for healthcare team members, being at the same time a social impact strategy when approached following social determinants of health as a contribution to primary healthcare. Objective: To analyze family visits from the approach of social determinants of health as a contribution to primary healthcare. Materials and Methods: The collective case study was carried out with 3 families and 13 members of the dispersed rural area of the municipality of Villamaría. Fieldwork was conducted between September 2017 and May 2018 using quantitative and qualitative methods. Family records were used as instruments with a focus on determinants and a guide to questions. Bardin's technique was used for the analysis. Results: Sociodemographic characteristics were identified where the most affected social determinants of health were described and interprofessional intervention and education were conducted based on the social determinants of health (43 activities), which influenced the risk reduction in those 3 families, placing them at medium risk. Discussion: It is underlined the role of nursing in the analysis of social determinants of health during family visits and the need to address the most affected ones. Conclusions: Family visits with a focus on social determinants of health allow understanding family dynamics in their context, taking into account family realities, history, lifestyles and opportunities for change and entrepreneurship of each of the families that are comprehensively accompanied. 49. Discapacidad y determinantes sociales de la salud en personas con enfermedad cerebrovascular, San Juan de Pasto (Colombia).

- Abstract
Objective: To establish the relationship between the Social Determinants of Health and the human functioning, considering the activity and participation components of people with Stroke at San Juan de Pasto city. Methods: The cohort study with an intentional sample of 98 people with Stroke, over 18 years, with a minimum of 6 months of evolution of the lesion, without cognitive or language deficit, users of health institutions and specialized centers in the city of Pasto. It was used the instrument of Social Determinants of Health and tool of evaluation activity limitations and participation restrictions. Results: the highest percentage of the population is male (54,1%), over 65 years of age, from strata 1 and 2 (78,6%), with a low level of education (they attended until preschool and incomplete primary -17,1 y 27,1%- respectively) they do not report current labor activity (84.6%), nor economic income (51.1%). Significant correlations were found between the socioeconomic stratum, the housing condition and the consumption of medications, with the limitation in activity and restriction in participation (p < 0.05). Conclusions: The results provide evidence regarding activity limitations, participation restrictions and social inequities in the population studied with Stroke at San Juan de Pasto city. This information can be used to advance initiatives and public actions at the departmental level, emphasizing social conditions and multidimensional aspects of disability. [ABSTRACT FROM AUTHOR]
