Amaç: Glomerüler hastalıkların tanısında, ışık mikroskobik inceleme ile birlikte kullanılan immunfloresan inceleme (İF) yöntemi tüm dünyada halen altın standart olarak kabul edilmektedir. Buna rağmen bazı dezavantajlarının bulunması alternatif tanısal inceleme yöntemlerinin araştırılmasına yol açmıştır. Bizim bu çalışmadaki amacımız, bazı merkezlerde rutin uygulanmaya başlanan immunhistokimyasal (İHK) inceleme yönteminin tanısal olup olmadığını araştırmak ve İF inceleme ile kıyaslamaktır.Gereç ve Yöntemler: Çalışmaya Haziran- Aralık 2012 tarihleri arasında Ege Üniversitesi Tıp Fakültesi'nde glomerülonefrit ön tanısı ile nativ böbrek biyopsi uygulanan 95 erişkin olgu dahil edildi. Olgular immunbirikim özellikleri nedeniyle IgA nefropatisi, membranöz glomerülonefrit (MGN), lupus nefriti (LN), nonspesifik grup ve negatif grup olarak beş gruba ayrılarak değerlendirildi. Tüm olguların formalin fikse parafine gömülü dokularından hazırlanan kesitlere primer antikor olarak IgG, IgM, IgA, Kappa ve Lambda antikorları uygulandı ve daha önce rutin inceleme için uygulanan C4d incelemeleri ile birlikte iki patolog tarafından İF inceleme sonuçları ve tanıları bilinmeden kör olarak değerlendirildi.Bulgular: Uygulanan İHK'sal antikorların hepsi, negatif vaka grubu ile diğer gruplar arasında fark gözetmeksizin nonspesifik bir boyanma paterni oluşturduğu için tanısal olarak anlamlı kabul edilebilecek bir bulgu saptanmadı. C4d glomerül bazal membran boyanması, MGN grubunda %100 oranında, lupus nefriti grubunda %66,7 oranında, IgA nefropatisi grubunda %12,5 oranında pozitif saptandı ve istatistiksel olarak anlamlı bulundu. C4d glomerül mezengium boyanması MGN grubunda %12,5 oranında, lupus nefritinde %66,7 oranında, IgA nefropatisinde %25,0 oranında pozitif saptandı ve gruplar arasında istatistiksel olarak anlamlı fark bulundu.Tartışma ve Sonuç: İmmunhistokimyasal inceleme yöntemi teknik olarak uygulanması zor bir yöntem olup, bu yöntemin başarılı olabilmesi için kesit kalınlığı, formaldehit fiksasyon süresi ve enzimatik sindirim ile antijen açığa çıkarma basamağı dikkatle uygulanmalıdır. İHK'sal incelemenin teknik sorunları giderildikten sonra İF inceleme ile korele şekilde uygulanmalı ve İF inceleme yönteminin uygulanamadığı durumlarda, başvurulacak alternatif bir yöntem olarak İHK'sal incelemenin rutin olarak uygulanması büyük bir avantaj sağlayacaktır. C4d GBM boyanması ise İF inceleme olanağının olmadığı durumlarda özellikle MGN olgularında tanısal açıdan önemlidir. Purpose: In the diagnosis of glomerular diseases, the immunofluorescence (IF) examination method that is used with light microscope examination is still globally accepted as the golden standard. Nevertheless, some disadvantages of the method lead to investigation of alternative diagnosis examination methods. In this study, our purpose is to investigate whether the immunohistochemistry examination method that has become routine clinic practice in some centers is diagnostic or not, and to compare it with the IF.Material and Methods: The study involves 95 adult cases to whom native kidney biopsy is applied in Ege University Medicine Faculty between June and December 2012, with pre-diagnosis of glomerulonephritis. Based on their immune deposit features, the cases are separated and evaluated in five groups which are IgA nephropathy, membranous glomerulonephritis (MGN), lupus nephritis (LN), nonspecific group and negative group. For all the cases, IgG, IgM, IgA, Kappa and Lambda antibodies are applied to fractions prepared from tissues embedded with formalin-fixed paraffin embedded, and together with C4d examinations applied previously for routine examinations, they have been evaluated by two pathologists who do not know IF examination results and the diagnoses. Results: Since all applied İHK antibodies indiscriminately constitute a non-specific coloration pattern between negative case group and other groups, no diagnostically meaningful finding is determined. C4d glomerulus basement membrane staining is found positive in the ratio of %100 in MGN group, %66,7 in lupus nephritis group, %12,5 in IgA nephropathy group, and they are statistically found meaningful. The staining of C4d glomerulus mesangium is found positive in the ratio of %12,5 in MGN group, %66,7 in lupus nephritis group, %25,0 in IgA nephropathy group, and a statistically meaningful difference is found between the groups.Discussion and Conclusion: İmmunohistochemistry examination is a method whose application is technically difficult, and in order to succeed section thickness, formaldehyde fixation and the revelation of antigens with enzymatic digestion step should be applied carefully. After the technical difficulties of IHK examination are resolved, it should be applied in correlation with IF, and in the cases when IF examination method cannot be applied, the routine application of IHK examination as an alternative method will provide a great advantage. Especially in MGN cases, C4d GBM staining is important for diagnosis if there is no opportunity for IF examination. 105
