Myriad conditions may affect both the neurologic system and the thorax, while other diseases primarily affecting the thorax may manifest with neurologic abnormalities. Correlation of signs, symptoms, and imaging findings in the neurological system with those in the thorax can help diagnose certain conditions and/or guide further diagnostic work-up and treatment. We will review and illustrate the imaging appearance of several systemic/neurological diseases with thoracic manifestations as well as discuss conditions in the thorax that can lead to neurologic symptoms., (Copyright © 2020 The Association of University Radiologists. Radiation therapy using conventional fractionated external-beam or high-precision dose techniques including three-dimensional conformal radiotherapy, stereotactic body radiation therapy, intensity-modulated radiation therapy, and proton therapy, is a key component in the treatment of patients with lung cancer. Knowledge of the radiation technique used, radiation treatment plan, expected temporal evolution of radiation-induced lung injury and patient-specific parameters, such as previous radiotherapy, concurrent chemoradiotherapy, and/or immunotherapy, is important in imaging interpretation. Most of the complications following lung cancer surgery occur in the early postoperative period and can result in significant morbidity and mortality. Delayed complications can also occur. Diagnosing these complications can be challenging because clinical manifestations are non-specific. Imaging plays an important role in detecting these complications in a timely manner and facilitates prompt interventions. Hence, it is important to have knowledge of the expected anatomical alterations following lung cancer surgeries, and the spectrum of post-surgical complications and their respective imaging findings to avoid misinterpretations or delay in diagnosis., (Copyright © 2021 The Royal College of Radiologists. By boosting the immune system, immunotherapy with immune checkpoint inhibitors (ICIs) has altered the management of patients with various cancers including those with metastatic non-small cell lung cancer (NSCLC). As a result of immune system activation, ICIs are associated with unique response patterns (that are not addressed by traditional response criteria) and inflammatory side effects termed immune-related adverse events. A variety of surgical procedures are utilized to treat a spectrum of cardiopulmonary diseases. In the imaging of patients in the post-operative period, it is important to have an understanding of surgical techniques including invasive and minimally invasive procedures and the expected postsurgical findings. Knowledge of certain patient risk factors, various complications associated with specific surgical procedures, and a keen attention to detail are essential to avoid misinterpretation and delay diagnosis. Chest computed tomography (CT) is the modality of choice for mediastinal imaging. The high-resolution images provided by multi-detector CT result in routine visualization of normal anatomic structures, which can be confused with pathology. In addition, many mediastinal abnormalities are discovered incidentally, with a routine chest CT protocol which may be insufficient for definite diagnosis. Awareness of the spectrum of potential pitfalls of mediastinal imaging, artifacts related to flow, motion, and solutions to mitigate these problematic issues is important in accurate interpretation. Non-small cell lung cancer (NSCLC) is usually diagnosed when it is not amenable to curative surgery or radiation. Many of these patients are candidates for systemic therapy. Median survival is only approximately 10 months, and, accordingly, treatment in advanced NSCLC is evolving toward a more personalized approach with the identification of genetic abnormalities based on biomarkers. For example, gene mutations in EGFR (epidermal growth factor receptor) and ALK (anaplastic lymphoma kinase) lead to a cascade of pathways resulting in uncontrolled growth, proliferation, and survival of tumor cells. Targeted therapies are aimed at the products of these mutated genes and include agents such as erlotinib and gefitinib (in EGFR-mutant NSCLC) or crizotinib (in ALK-positive NSCLC). Antiangiogenesis agents such as bevacizumab are another category of targeted therapy that inhibits vascular endothelial growth factors. The imaging characteristics of advanced NSCLC with genetic abnormalities, the evolution of targeted therapies and their imaging manifestations will be discussed.

Potential pitfalls in the interpretation of diseases involving the mediastinum are seen when imaging with computed tomography and [18F]-fluoro-2-deoxy-d-glucose positron emission tomography. These pitfalls can involve any mediastinal structure, including the mediastinal vessels, heart, lymph nodes, thymus, trachea, esophagus, and fat. Misinterpretation of normal variants or benign conditions as pathology can affect staging and alter treatment. Objectives/hypothesis: Although several imaging characteristics of human papillomavirus (HPV)-related head and neck squamous cell carcinoma (HNSCC) have been reported, imaging features of nodal metastasis and influence to outcomes have not been well studied thus far. The purpose of the study was to investigate the imaging characteristics of nodal metastasis by HPV status in HNSCC and to clarify whether those findings influence the outcomes., Study Design: Retrospective review., Methods: Computed tomography and magnetic resonance imaging for initial staging on 139 patients of HNSCC with known HPV status were retrospectively reviewed. We investigated imaging characteristics of the nodal metastasis including the presence of extracapsular spread (ECS), and also investigated the influence of nodal metastasis characteristics to outcomes by HPV status. Two-year actuarial control and survival rates were estimated using the Kaplan-Meier product-limit method (P < 0.05)., Results: Eighty-eight patients with nodal metastasis were identified and outcome information was available for 78 patients. Nodal metastasis was significantly more common in HPV-positive patients compared to HPV-negative patients (75% vs. 54%, P = 0.009). HPV-positive patients showed a higher prevalence of ECS compared to HPV-negative patients (77% vs. 56%, P = 0.041). The prevalence of disease recurrence was more common in HPV-negative patients (67% vs. 13%, P < 0.0001), and it was independent of the presence of ECS in nodal metastasis., Conclusions: Nodal metastases were significantly more common in HPV-positive HNSCC, whereas the prevalence of disease recurrence was greater in HPV-negative HNSCC. Pedicle muscle flaps that are used to enhance surgical site integrity after thoracic cancer resections may simulate a recurrent mass at postoperative imaging. However, such flaps have a characteristic appearance and location that should allow their differentiation from a solid mass or fluid collection. The location and appearance of a flap depend on the type of muscle used (whether intercostal, serratus anterior, or latissimus dorsi) and the extent of resection. The appearance also varies according to the presence and amount of fat and calcification in the flap. Most flaps have fat strands along the flap axis that increase in size over time. Linear areas of calcification also may occur because of the inclusion of periosteum in a flap, or more extensive calcification may lead to ossification over time. Uncomplicated flaps do not show substantial radionuclide uptake at positron emission tomography (PET), but areas of extensive calcification may show increased uptake similar to that in bone. Vessels also may be seen within a flap, particularly if the flap has a high fat content; and enhancing vessels frequently are evident at contrast material-enhanced computed tomography (CT). The magnetic resonance (MR) imaging characteristics of flaps are similar to those of normal fat and muscle. Although CT is the modality most often used for follow-up imaging, PET or MR imaging may be helpful in cases in which the CT findings are questionable., (RSNA, 2007)

Bronchioalveolar cell carcinomas (BACs), a subset of primary lung adenocarcinomas, are uncommon. Histologically, they are a diverse group of malignancies. The diagnosis is restricted to adenocarcinomas that grow in a lepidic manner and that have no stromal, vascular, or pleural invasion. Their histologic diversity leads to varied radiologic manifestations that are often indistinguishable from those of other primary non-small-cell lung cancers (NSCLC). However, typical manifestations, many of which can be attributed to lepidic growth, have been reported. Radiologic manifestations include a solitary peripheral pulmonary nodule, airspace disease, and multiple nodules and a combination of these findings can be present in a single patient. The most common manifestation, a solitary pulmonary nodule, is usually indistinguishable from other primary NSCLC. However, pseudocavitation and air bronchograms within the nodule can be useful in suggesting the correct diagnosis. In addition to aiding in the diagnosis of BAC, radiologic imaging is an important component in the evaluation of the therapeutic efficacy of treatment; serial measurements of tumor size before and after treatment are commonly used to assess response. However, BACs that are consolidative or ground-glass in nature present challenges in tumor-response determination. Other imaging modalities, such as positron emission tomography scanning, may prove helpful in assessing the metabolic response to therapy but have yet to be proven effective.
