Diffusion-weighted magnetic resonance imaging (DW-MRI) has evolved to provide increasingly sophisticated investigations of the human brain's structural connectome in vivo. Restriction spectrum imaging (RSI) is a method that reconstructs the orientation distribution of diffusion within tissues over a range of length scales. In its original formulation, RSI represented the signal as consisting of a spectrum of Gaussian diffusion response functions. Recent technological advances have enabled the use of ultra-high b-values on human MRI scanners, providing higher sensitivity to intracellular water diffusion in the living human brain. To capture the complex diffusion time dependence of the signal within restricted water compartments, we expand upon the RSI approach to represent restricted water compartments with non-Gaussian response functions, in an extended analysis framework called linear multi-scale modeling (LMM). The LMM approach is designed to resolve length scale and orientation-specific information with greater specificity to tissue microstructure in the restricted and hindered compartments, while retaining the advantages of the RSI approach in its implementation as a linear inverse problem. Using multi-shell, multi-diffusion time DW-MRI data acquired with a state-of-the-art 3 T MRI scanner equipped with 300 mT/m gradients, we demonstrate the ability of the LMM approach to distinguish different anatomical structures in the human brain and the potential to advance mapping of the human connectome through joint estimation of the fiber orientation distributions and compartment size characteristics., (© 2022 The Authors. Background: To evaluate simplified intravoxel incoherent motion (IVIM) diffusion-weighted imaging (DWI) for differentiating malignant versus benign breast lesions as (i) stand-alone tool and (ii) add-on to dynamic contrast-enhanced magnetic resonance imaging., Methods: 1.5-T DWI data (b = 0, 50, 250, 800 s/mm 2 ) were retrospectively analysed for 126 patients with malignant or benign breast lesions. Apparent diffusion coefficient (ADC) ADC (0, 800) and IVIM-based parameters D 1 ' = ADC (50, 800), D 2 ' = ADC (250, 800), f 1 ' = f (0, 50, 800), f 2 ' = f (0, 250, 800) and D*' = D* (0, 50, 250, 800) were voxel-wise calculated without fitting procedures. Regions of interest were analysed in vital tumour and perfusion hot spots. Beside the single parameters, the combined use of D 1 ' with f 1 ' and D 2 ' with f 2 ' was evaluated. Lesion differentiation was investigated for lesions (i) with hyperintensity on DWI with b = 800 s/mm 2 (n = 191) and (ii) with suspicious contrast-enhancement (n = 135)., Results: All lesions with suspicious contrast-enhancement appeared also hyperintense on DWI with b = 800 s/mm 2 . For task (i), best discrimination was reached for the combination of D 1 ' and f 1 ' using perfusion hot spot regions-of-interest (accuracy 93.7%), which was higher than that of ADC (86.9%, p = 0.003) and single IVIM parameters D 1 ' (88.0%) and f 1 ' (87.4%). For task (ii), best discrimination was reached for single parameter D 1 ' using perfusion hot spot regions-of-interest (92.6%), which were slightly but not significantly better than that of ADC (91.1%) and D 2 ' (88.1%). Adding f 1 ' to D 1 ' did not improve discrimination., Conclusions: IVIM analysis yielded a higher accuracy than ADC. If stand-alone DWI is used, perfusion analysis is of special relevance., (© 2022. This study investigated the impact of different ROI placement and analysis methods on the diagnostic performance of simplified IVIM-DWI for differentiating liver lesions. 1.5/3.0-T DWI data from a respiratory-gated MRI sequence (b = 0, 50, 250, 800 s/mm 2 ) were analyzed in patients with malignant (n = 74/54) and benign (n = 35/19) lesions. Apparent diffusion coefficient ADC = ADC(0,800) and IVIM parameters D 1 ' = ADC(50,800), D 2 ' = ADC(250,800), f 1 ' = f(0,50,800), f 2 ' = f(0,250,800), and D*' = D*(0,50,250,800) were calculated voxel-wise. For each lesion, a representative 2D-ROI, a 3D-ROI whole lesion, and a 3D-ROI from "good" slices were placed, including and excluding centrally deviating areas (CDA) if present, and analyzed with various histogram metrics. The diagnostic performance of 2D- and 3D-ROIs was not significantly different; e.g. AUC (ADC/D 1 '/f 1 ') were 0.958/0.902/0.622 for 2D- and 0.942/0.892/0.712 for whole lesion 3D-ROIs excluding CDA at 1.5 T (p > 0.05). For 2D- and 3D-ROIs, AUC (ADC/D 1 '/D 2 ') were significantly higher, when CDA were excluded. With CDA included, AUC (ADC/D 1 '/D 2 '/f 1 '/D*') improved when low percentiles were used instead of averages, and was then comparable to the results of average ROI analysis excluding CDA. For lesion differentiation the use of a representative 2D-ROI is sufficient. CDA should be excluded from ROIs by hand or automatically using low percentiles of diffusion coefficients., (© 2021. Background: To evaluate the feasibility of two-colour index maps containing combined diffusion and perfusion information from simplified intravoxel incoherent motion (IVIM) for liver lesion malignancy assessment., Methods: Diffusion-weighted data from a respiratory-gated 1.5-T magnetic resonance sequence were analysed in 109 patients with liver lesions. With three b values (0, 50, 800 s/mm 2 ) estimated diffusion coefficient D', perfusion fraction f', and apparent diffusion coefficient (ADC) maps were calculated and analysed for regions of interest (ROIs). D' and f' cutoff values were determined by differentiating haemangiomas from other lesions and focal nodular hyperplasias from other lesions, respectively. Combined I Df index maps were generated with a voxel value set to 100, if both D' and f' voxel values were lower than their cutoff values (1,529.4 × 10 -6 mm 2 /s and 114.4 × 10 -3 , respectively), otherwise to 0. Moreover, I ADC index maps were generated from ADC cutoff value (1,338.5 × 10 -6 mm 2 /s) obtained by differentiating benign from malignant lesions. Discriminatory power was assessed for both I Df and I ADC . Index maps were displayed as two-colour overlays to b-800 images and visually assessed within the translucent hyperintense areas., Results: For I Df , the same diagnostic accuracy was achieved as for the combined use of parameters D' and f' (93.6%). Compared to I ADC , I Df showed a higher diagnostic accuracy. Visual judgment of I Df yielded an accuracy (95.4%) similar to that of quantitative analysis (93.6%)., Conclusion: Voxel-wise combined two-colour index maps I Df provide similar diagnostic accuracy as ROI-based combination of estimated IVIM parameters D' and f' and are suitable for visual assessment of liver lesion malignancy., (© 2021. Objectives: The aim of this study was to retrospectively compare optimized respiratory-triggered diffusion-weighted imaging with simultaneous multislice acceleration (SMS-RT-DWI) of the liver with a standard free-breathing echo-planar DWI (s-DWI) protocol at 3 T with respect to the imaging artifacts inherent to DWI., Materials and Methods: Fifty-two patients who underwent a magnetic resonance imaging study of the liver were included in this retrospective study. Examinations were performed on a 3 T whole-body magnetic resonance system (MAGNETOM Skyra; Siemens Healthcare, Erlangen, Germany). In all patients, both s-DWI and SMS-RT-DWI of the liver were obtained. Images were qualitatively evaluated by 2 independent radiologists with regard to overall image quality, liver edge sharpness, sequence-related artifacts, and overall scan preference. For quantitative evaluation, signal-to-noise ratio was measured from signal-to-noise ratio maps. The mean apparent diffusion coefficient (ADC) was measured in each liver quadrant. The Wilcoxon rank-sum test was used for analysis of the qualitative parameters and the paired Student t test for quantitative parameters., Results: Overall image quality, liver edge sharpness, and sequence-related artifacts of SMS-RT-DWI received significantly better ratings compared with s-DWI (P < 0.05 for all). For 90.4% of the examinations, both readers overall preferred SMS-RT-DWI to s-DWI. Acquisition time for SMS-RT-DWI was 34% faster than s-DWI. Signal-to-noise ratio values were significantly higher for s-DWI at b50 but did not statistically differ at b800, and they were more homogenous for SMS-RT-DWI, with a significantly lower standard deviation at b50. Mean ADC values decreased from the left to right hepatic lobe as well as from cranial to caudal for s-DWI. With SMS-RT-DWI, mean ADC values were homogeneous throughout the liver., Conclusions: Optimized, multislice, respiratory-triggered DWI of the liver at 3 T substantially improves image quality with a reduced scan acquisition time compared with s-DWI.

Aim: The purpose of this IRB-approved, retrospective study was to compare image quality between 2D and high-resolution 3D, T2-weighted (T2WI) magnetic resonance imaging (MRI) sequences and to investigate the additional value of ultra-high b-value diffusion-weighted imaging (DWI; b=2,000 mm/s 2 ) for both rectal cancer staging and evaluating treatment response., Materials and Methods: From 12 February to 24 August 2016, 26 consecutive patients (22 males, four females; mean age: 61.9±14.0 years) with histologically-proven rectal cancer. In total 31 examinations [12 prior to and 19 after chemoradiation (CRT)] were included. The patients underwent pelvic MRI on a 3.0-T scanner (Magnetom Skyra, Erlangen, Germany). Three radiologists (3, 4, and 5 years of experience in MRI, respectively) independently assessed all images and rated the image quality of DWI (b=800 mm/s 2 ), apparent diffusion coefficient map, DWI (b=2,000 mm/s 2 ), 3D sagittal T2WI, 3D axial T2WI, 2D sagittal T2WI, and 2D axial T2WI of each patient, respectively. In addition, signal intensity ratios (SIR) were calculated between rectal cancer and obturator internus muscle (background) in all patients after CRT on DWI (b=2,000 mm/s 2 ) and correlated with histopathological regression grade (RG)., Results: Tumor delineation was significantly better by 2D T2WI than 3D T2WI both before and after CRT (before CRT: Z=-3.2, p=0.02; after CRT: Z=-4.408, p<0.001; all: Z=-5.192; p<0.001) and was the preferred method, although image quality ratings were not significantly different (3D sagittal: 4.00±0.48; 2D sagittal: 4.03±0.34, p=0.713; 3D axial: 3.85±0.61, 2D axial: 3.78±0.64, p=0.537). Independent t-test showed significantly higher SIR between those with RG 1 or 2 (moderate response: mean score=2.02) and those with RG 3+4 (good response: mean score=0.8) (t=3.044, p=0.011). In those with RG 4 (complete response), SIR of b2000 was 0.946 compared to a 1.41 average of the whole cohort. In two patients, tumor was invisible on b2000 following CRT (RG 3 and 4, respectively). Interobserver agreement was mostly good (κ≥0.6) regarding image quality assessment, except for poor agreement (κ=0.4) in DWI (b2000) between the two less-experienced readers., Conclusion: In conclusion, 3D T2WI might be useful for evaluating response to neoadjuvant therapy in a comprehensive, cost-effective protocol, where 2D imaging seems to be preferable. In addition, DWI (b2000) may be beneficial in assessing both the primary and the residual tumor after CRT in rectal cancer and SIR may be helpful in assessing response to CRT., (Copyright© 2018, International Institute of Anticancer Research (Dr. George J. Background/aim: Exctracapsular extension (ECE) in prostate cancer has a high impact on treatment decision. MRI might predict presence of ECE non-invasively., Patients and Methods: Triplanar T2w-sequences, DWI (diffusion weighted imaging) and DCE (dynamic contrast-enhanced imaging) of 34 patients with PCa were analyzed to prior prostatectomy. Sensitivity (SS) and specificity (SP) of T2w, apparent diffusion coefficient (ADC), plasma flow (PF) and mean transit time (MTT) normalized by PCa/normal tissue ratio for prediction of CI (capsular infiltration)/ECE were determined by area-under-the-receiver-operating-characteristics analysis., Results: SS/SP for detecting ECE was 29/85. AUC (area under the curve) of ECE cases was 0.98/0.92/0.69 (cut-off-ratios 3.2/0.51/0.46), SS 93/100/86% and SP 95/80/50% for PF-/MTT-/ADC-ratios, respectively. PF- and MTT-ratios between CI and without CI/ECE differed significantly (PF, p<0.0001; MTT, p=0.0134) with SS/SP 84/89% for PF and SS/SP 52/100% for MTT-ratios. No significant differences regarding ADC-ratios were identified., Conclusion: ECE/CI can be assessed by quantitative DCE analysis with great diagnostic confidence and higher specificity than ADC., (Copyright© 2018, International Institute of Anticancer Research (Dr. George J. Background Functional techniques like diffusion-weighted imaging (DWI) are gaining more and more importance in liver magnetic resonance imaging (MRI). Diffusion kurtosis imaging (DKI) is an advanced technique that might help to overcome current limitations of DWI. Purpose To evaluate DKI for the differentiation of hepatic lesions in comparison to conventional DWI at 3 Tesla. Material and Methods Fifty-six consecutive patients were examined using a routine abdominal MR protocol at 3 Tesla which included DWI with b-values of 50, 400, 800, and 1000 s/mm 2 . Apparent diffusion coefficient maps were calculated applying a standard mono-exponential fit, while a non-Gaussian kurtosis fit was used to obtain DKI maps. ADC as well as Kurtosis-corrected diffusion ( D) values were quantified by region of interest analysis and compared between lesions. Results Sixty-eight hepatic lesions (hepatocellular carcinoma [HCC] [n = 25]; hepatic adenoma [n = 4], cysts [n = 18]; hepatic hemangioma [HH] [n = 18]; and focal nodular hyperplasia [n = 3]) were identified. Differentiation of malignant and benign lesions was possible based on both DWI ADC as well as DKI D-values ( P values were in the range of 0.04 to < 0.0001). Conclusion In vivo abdominal DKI calculated using standard b-values is feasible and enables quantitative differentiation between malignant and benign liver lesions. Assessment of conventional ADC values leads to similar results when using b-values below 1000 s/mm 2 for DKI calculation.

Purpose: To evaluate the feasibility and clinical robustness of zoomed diffusion-weighted echo planar imaging (z-EPI) relative to conventional single-shot EPI (c-EPI) for DWI of the kidneys., Materials and Methods: This retrospective study was approved by the institutional research ethics board. 66 patients (median age 58.5 years±13.4, range 23-83 years, 45 men, 21 women) undergoing 3T (Magnetom Skyra(®), Siemens Healthcare, Erlangen, Germany) using a dynamic parallel transmit array (TimTX TrueShape, Siemens Healthcare, Erlangen, Germany) for renal MRI were included in this study. Both c-EPI and z-EPI images were obtained. For z-EPI, a two-dimensional spatially-selective radiofrequency (RF) pulse was applied for echo planar imaging with the FOV reduced by a factor of 3. Two radiologists, blinded to clinical data and scan parameters evaluated the images with respect to their diagnostic confidence, overall preference, overall image quality, delineation of the kidney, spatial distortion, and image blur. Sequences were compared using a paired Wilcoxon test. ADC values for the upper pole, mid-zone, lower pole of the normal kidneys were compared between sequences as well as ADC values for renal lesions, using a paired t-test., Results: With z-EPI, the kidney was significantly better delineated with sharper boundaries, less image blur and distortion, and overall better image quality relative to c-EPI (all p<0.001). The z-EPI technique led to greater diagnostic confidence than c-EPI (p=0.020). z-EPI was preferred to c-EPI in 60 cases (90.9%, 60/66). No statistically significant differences in the ADC values of renal parenchyma or of renal lesions were observed between the two sequences (all p>0.05)., Conclusion: Image quality, distortion, and susceptibility artifacts might be improved by using z-EPI rather than c-EPI for DWI of the kidney., (Copyright © 2016 Elsevier Ireland Ltd. Aim: To evaluate volume-based apparent diffusion coefficient (ADCVOL) values for early prediction of therapy response after radioembolization (RE) of primary and secondary liver malignancies using a semiautomatic, image analysis software prototype., Patients and Methods: In a retrospective analysis, 74 target lesions were assessed in terms of therapy response 1 month after RE using magnetic resonance imaging. Changes of the diameter of the target lesions, the mean ADC value in a representative single-slice region of interest (ADCROI) and mean ADCVOL were correlated., Results: The median progression-free interval (PFI) of patients overall was 3.5±5.9 months. PFI in patients with an ADCVOL increase was statistically significantly longer than in patients with an ADCVOL decrease (mean PFI=6.5 vs. 2.5 months; p=0.0374). No correlation between PFI and early changes in lesion diameter or ADCROI was found., Conclusion: Semiautomatic, software-based ADC volume measurements seem to offer a clinically valuable parameter for early prediction of therapy response in patients after RE., (Copyright© 2016 International Institute of Anticancer Research (Dr. John G. Purpose: Spatially-tailored (RF) excitation pulses in echo-planar imaging (EPI), combined with a decreased FOV in the phase-encoding direction, enable a reduction of k-space acquisition lines, which shortens the echo train length (ETL) and reduces susceptibility artifacts. The purpose of this study was to evaluate the image quality of a zoomed EPI (z-EPI) sequence in diffusion-weighted imaging (DWI) of the prostate in comparison to a conventional single-shot EPI using single-channel (c-EPI1) and multi-channel (c-EPI2) RF excitation, with and without use of an endorectal coil., Materials and Methods: 33 consecutive patients (mean age: 61 +/- 9 years; mean PSA: 8.67±6.23 ng/ml) with examinations between 10/2012 and 02/2014 were analyzed in this retrospective study. In 26 of 33 patients the initial multiparametric (mp)-MRI was performed on a whole-body 3T scanner (Magnetom Trio, Siemens, Erlangen, Germany) using an endorectal coil (c (conventional)-EPI1). Zoomed-EPI (Z-EPI) examinations of these patients and a complete mp-MRI protocol including c-EPI2 of 7 additional patients were carried out on another 3T wb MR scanner with two-channel dynamic parallel transmit capability (Magnetom Skyra with TimTX TrueShape, Siemens). For z-EPI, the one-dimensional spatially selective RF excitation pulse was replaced by a two-dimensional RF pulse. Degree of image blur and susceptibility artifacts (0=not present to 3= non-diagnostic), maximum image distortion (mm), apparent diffusion coefficient (ADC) values, as well as overall scan preference were evaluated. SNR maps were generated to compare c-EPI2 and z-EPI., Results: Overall image quality of z-EPI was preferred by both readers in all examinations with a single exception. Susceptibility artifacts were rated significantly lower on z-EPI compared to both other methods (z-EPI vs c-EPI1: p<0.01; z-EPI vs c-EPI2: p<0.01) as well as image blur (z-EPI vs c-EPI1: p<0.01; z-EPI vs c-EPI2: p<0.01). Image distortion was not statistically significantly reduced with z-EPI (z-EPI vs c-EPI1: p=0.12; z-EPI vs c-EPI2: p=0.42). Interobserver agreement for ratings of susceptibility artifacts, image blur and overall scan preference was good. SNR was higher for z-EPI than for c-EPI1 (n=1)., Conclusion: Z-EPI leads to significant improvements in image quality and artifacts as well as image blur reduction improving prostate DWI and enabling accurate fusion with conventional sequences. The improved fusion could lead to advantages in the field of MRI-guided biopsy suspicous lesions and performance of locally ablative procedures for prostate cancer., (Copyright © 2015. Objectives: To evaluate the feasibility of zoomed diffusion-weighted EPI (z-EPI) in the head and neck in a healthy volunteer population and to compare to conventional single-shot EPI (c-EPI)., Material and Methods: Nine volunteers were included in this prospective, IRB-approved study. Examinations were performed on a 3 T-MR system equipped with a two-channel, fully-dynamic parallel transmit array. The acquired sequences consisted of a T2w-TSE, a c-EPI, and two z-EPI acquisitions. For quantitative assessment of distortion artefacts, DW images were fused with T2-TSE images. Misregistration of DW images with T2-TSE images was assessed in the cervical spine. For qualitative assessment, two readers ranked c-EPI and z-EPI sequences in terms of susceptibility artefacts, image blur, and overall imaging preference. ADC values of several anatomical regions were calculated and compared between sequences., Results: Mean maximum distortion with the c-EPI was 5.9 mm ± 1.6 mm versus 2.4 mm ± 1 mm (p < 0.05) with z-EPI. Both readers found more blur and susceptibility artefacts in every case with c-EPI. No statistically significant differences in calculated ADC values were observed., Conclusion: z-EPI of the head and neck leads to substantial image quality improvements relative to c-EPI due to a reduction in susceptibility artefacts and image blur., Key Points: • Zoomed DWI is feasible in the head and neck. • Image quality improves substantially with zoomed DWI of the neck. • Zoomed DWI exhibits markedly reduced susceptibility artefacts.

Purpose: To assess whether functional MR imaging using blood-oxygenation level-dependent (BOLD) imaging and diffusion-weighted imaging demonstrate changes in renal oxygenation and apparent diffusion coefficient (ADC) in a pig model., Materials and Methods: After administration of either 1-g iodine/kg body weight low-osmolar iopromide or iso-osmolar iodixanol, 8 mini pigs underwent a series of repeated BOLD measurements (TR/TE 106/5.9-48.7 ms, slice thickness 4 mm) and diffusion-weighted imaging measurements (TR/TE 3900/79 ms, slice thickness 4 mm) for 1 hour at 1.5 T. In this intraindividual cross over study, the second contrast agent injection with the other iodinated contrast agent was performed at least 24 hours after the initial contrast agent injection. BOLD-based R2* values as indirect measures of the renal oxygenation were determined for the cortex, the inner medulla, and the outer medulla. ADC values were measured for the cortex and the whole kidney., Results: For both contrast agents, a drop in R2* was found in the cortex, which normalized after 55 minutes. In the outer medulla and particularly in the inner medulla, a decreased initial drop of R2* was encountered with both contrast agents, with a slow increase toward the baseline R2*. In the inner medulla, elevated R2* values were found with the iso-osmolar contrast agent only. The ADC revealed an initial increase, which slowly decayed over the measurement period. This finding was more pronounced for the cortex compared with the whole-kidney analysis., Conclusion: Functional MR imaging of the kidneys demonstrates increased R2* in the inner medulla only after the administration of the iodixanol potentially indicating hypoxia and thus a pathomechanism of contrast-induced nephropathy.

Renal magnetic resonance (MR) imaging has undergone major improvements in the past several years. This review focuses on the technical basics and clinical applications of MR angiography (MRA) with the goal of enabling readers to acquire high-resolution, high quality renal artery MRA. The current role of contrast agents and their safe use in patients with renal impairment is discussed. In addition, an overview of promising techniques on the horizon for renal MR is provided. The clinical value and specific applications of renal MR are critically discussed.

Purpose: To evaluate the signal-to-noise ratio (SNR) and diagnostic quality of diffusion weighted imaging (DWI) using a fast spin echo (FSE) sequence with BLADE k-space trajectory at 3 T in combination with a 32-channel head coil. The scan was compared with a standard spin echo (SE) echo-planar imaging (EPI) DWI and a high resolution SE EPI DWI sequence., Material and Methods: Fourteen patients with acute brain ischemia were included in this Institutional Review Board approved study. All patients were evaluated with 3 different image sequences, using a 3 T scanner and a 32-channel head coil: (a) a standard SE EPI DWI (matrix size 192 x 192), (b) a high resolution SE EPI DWI (matrix size of 256 x 256) and (c) a FSE DWI BLADE (matrix size 192 x 192). The SNR of the 3 scans was compared in 10 healthy volunteers by a paired student t test. Image quality was evaluated with 4 dedicated questions in a blinded read: (1) The scans were ranked in terms of bulk susceptibility artifact. (2) The scan preference for diagnosis of any diffusion abnormality that might occur and (3) the preference for visualization of the diffusion abnormality actually present was determined. (4) The influence of bulk susceptibility on image evaluation for the diffusion abnormality present was assessed., Results: For visualization of the diffusion abnormality present, BLADE DWI was the scan sequence preferred most by both readers (reader 1: 41.7%, reader 2: 35.7%). For visualization of any diffusion abnormality present, BLADE DWI was the preferred scan in 13 of 14 cases for reader 1 (93%) and in 11 of 14 cases for reader 2 (78.6%). No high resolution SE EPI DWI scan was rated best by reader 1. Reader 2 rated the high resolution SE EPI DWI scan superior in only 1 of 56 judgments. The standard EPI DWI sequence (21.8 +/- 5.3) had in comparison to the high resolution EPI DWI (11.9 +/- 2.6) and the BLADE DWI scans (11.3 +/- 3.8) significantly higher SNR mean values., Conclusion: Our preliminary data demonstrates the feasibility of a FSE EPI DWI scan with radial-like k-space sampling, using a 32-channel coil at 3 T in acute brain ischemia. The BLADE DWI was the preferred scan for the detection of acute diffusion abnormalities because of the lack of bulk susceptibility artifacts.

Objective: To evaluate the clinical feasibility of diffusion tensor imaging (DTI) of the kidney in volunteers and patients with renal diseases., Material and Methods: Ten volunteers and 22 patients (mean age, 56 +/- 14.3) with renal masses and renal artery stenosis underwent breath-hold coronal fat-saturated echo-planar DTI (as provided by the manufacturer, 6 diffusion directions, diffusion weightings b = 0 and 300 s/mm2, repetition time 730 ms/echo time 72 ms; 5 slices; slice thickness, 6 mm; inplane resolution, 2.1 x 2.1 mm2; acquisition time, 26 seconds) of the kidneys at 1.5 T (MAGNETOM Avanto, Siemens Medical Solutions, Erlangen, Germany). The parallel imaging technique, generalized autocalibrating partially parallel acquisitions with an acceleration factor 2, was applied. Using the commercially available Syngo DTI task card software, regions of interests were placed in the cortex, medulla, and in renal masses if present. Fractional anisotropy (FA) and apparent diffusion coefficients (ADC) were determined, and tractography was used to visualize the renal diffusion properties. Statistical analysis was performed using the Wilcoxon signed-rank sum test and paired t tests., Results: In all volunteers, FA was significantly (P < 0.01) higher in the medulla (0.36 +/- 0.03) than in the cortex (0.21 +/- 0.02), whereas the ADC was significantly (P < 0.01) higher in the cortex (2.43 +/- 0.19) than in the medulla (2.16 +/- 0.22). Tractography typically revealed a radial preferred direction of medullary diffusion basically reflecting medullary flow.FA/ADC of simple renal cysts (n = 8) was 0.14 +/- 0.05/2.86 +/- 0.15. Renal cell carcinoma (n = 10) showed a wide FA range from 0.11 to 0.56. Using tractography, the structural organization of renal cell carcinoma such as pseudocapsules could be visualized.In 1 patient with unilateral high-grade renal artery stenosis, the cortical ADC of the affected kidney was lower than on the contralateral side (1.77/2.27) and the FA was increased (0.33/0.18). The FA of the medulla was increased (0.70/0.41) and the ADC decreased (1.43/1.90)., Conclusions: Using parallel imaging, DTI measurements of the kidneys are feasible within a single breath-hold with good discrimination between cortex and medulla. Parallel imaging allows more slices and a superior resolution. DTI measurements of the kidney allows visualization of medullary flow, in pathology ADC and FA were altered. Further investigations will be required to evaluate the role of DTI for studying and monitoring renal ultrastructure.

Background: To analyse the influence of whole body (wb)-MRI on patient management compared to routine diagnostic tests in patients with fever of unknown origin (FUO). Methods: Twenty-four patients with FUO, defined as illness of more than three weeks with fever greater than 38.3 °C, underwent wb-MRI at a 1.5 T MR-system. The MR-protocol consisted of the following sequences: axial T1 VIBE, coronal T2-TIRM and a coronal echoplanar diffusion weighted sequence (overall acquisition time 29:39 min:s). Furthermore, laboratory findings, chest-x-ray, abdominal ultrasound, CT-scans and/or PET-CT scans were evaluated and compared to the wb-MRI findings in regard to treatment changes. Results: Wb-MRI yielded a correct diagnosis in 70% of the patients. In 46% the inflammatory focus was exclusively detected by wb-MRI. Focus detection by wb-MRI led to a subsequent change of the clinical management in 92% of the patients. In 6 patients both a wb-MRI and a PET-CT were performed yielding the correct diagnosis in the same 4 of 6 patients for both imaging modalities. Conclusions: Wb-MRI appears to be of value in the evaluation of FUO patients, allowing for optimized treatment by increasing diagnostic certainty. Due to its lack of nephrotoxicity and ionizing radiation it may be preferred over standard imaging techniques and PET-CT in the future. Background: Implementation of DWI in the abdomen is challenging due to artifacts, particularly those arising from differences in tissue susceptibility. Two-dimensional, spatially-selective radiofrequency (RF) excitation pulses for single-shot echo-planar imaging (EPI) combined with a reduction in the FOV in the phase-encoding direction (i.e. zooming) leads to a decreased number of k-space acquisition lines, significantly shortening the EPI echo train and potentially susceptibility artifacts. Purpose: To assess the feasibility and image quality of a zoomed diffusion-weighted EPI (z-EPI) sequence in MR imaging of the pancreas. The approach is compared to conventional single-shot EPI (c-EPI). Material and Methods: 23 patients who had undergone an MRI study of the abdomen were included in this retrospective study. Examinations were performed on a 3T whole-body MR system (Magnetom Skyra, Siemens) equipped with a two-channel fully dynamic parallel transmit array (TimTX TrueShape, Siemens). The acquired sequences consisted of a conventional EPI DWI of the abdomen and a zoomed EPI DWI of the pancreas. For z-EPI, the standard sinc excitation was replaced with a two-dimensional spatially-selective RF pulse using an echo-planar transmit trajectory. Images were evaluated with regard to image blur, respiratory motion artifacts, diagnostic confidence, delineation of the pancreas, and overall scan preference. Additionally ADC values of the pancreatic head, body, and tail were calculated and compared between sequences. Results: The pancreas was better delineated in every case (23/23) with z-EPI versus c-EPI. In every case (23/23), both readers preferred z-EPI overall to c-EPI. With z-EPI there was statistically significantly less image blur (p<0.0001) and respiratory motion artifact compared to c-EPI (p<0.0001). Diagnostic confidence was statistically significantly better with z-EPI (p<0.0001). No statistically significant differences in calculated ADC values were observed between the two sequences. Conclusion: Zoomed diffusion-weighted EPI leads to substantial image quality improvements with reduction of susceptibility artifacts in pancreatic DWI. [ABSTRACT FROM AUTHOR]
