Acantholytic dermatophytosis is a rarely reported condition of dogs that clinically and histopathologically mimics superficial pemphigus (erythematosus, foliaceus). Histologically, periodic acid-Schiff (PAS) and Grocott's methenamine-silver (GMS) are often necessary to show the fungus. A retrospective histopathological study was conducted on 190 canine skin biopsy specimens: 95 each with the diagnosis of canine dermatophytosis or of superficial pemphigus. All specimens were stained with haematoxylin and eosin, PAS, and GMS. Dermatophytes were not seen in any superficial pemphigus cases. Acantholytic keratinocytes were noted in 14% of the dermatophytosis cases, none of which had clinical signs consistent with superficial pemphigus. Among cases with acantholytic keratinocytes, superficial pemphigus had significantly more acantholytic cells than dermatophytosis (P = 0.02). When comparing face and nonface cases, there was no difference in prevalence of acantholytic keratinocytes in dermatophytosis or number of acantholytic keratinocytes in superficial pemphigus. All dermatophyte cases were both GMS and PAS positive with neither stain being visually superior. No dermatophyte cases where acantholytic keratinocytes were noted had a history, clinical signs and histopathological features compatible with acantholytic dermatophytosis.

Orbifloxacin tablets were administered orally to 23 dogs with superficial and/or deep staphylococcal pyoderma. Response to therapy was excellent in 95.6% of the dogs. Duration of therapy varied from 21 to 40 days (average 29 days) for dogs having only superficial infections, and from 25 to 150 days (average 72 days) for dogs having deep infections. Relapses occurred in 18% of the dogs within a 3-month period. One dog developed a presumed adverse cutaneous drug reaction. Under the conditions of this study, orbifloxacin was an effective, safe, and convenient antibiotic for the treatment of superficial and deep staphylococcal pyoderma in dogs.

Cetirizine and placebo were administered orally as individual agents to 23 dogs with atopic dermatitis. The pruritus was satisfactorily reduced in 4/22 (18%) dogs that completed the trial with cetirizine. Two dogs vomited after administration of the active drug.

Medullary trichomalacia is the name proposed for a hair shaft abnormality that was recognized in 6 German shepherd dogs. Affected dogs had multifocal areas of broken hairs, especially on the dorsolateral trunk. Microscopic examination of hair shafts revealed focal areas of loss of architecture, swelling, and apparent softening of the medulla, followed by longitudinal (length-wise) splitting and breakage of the hair shaft. No cause could be found. Affected dogs were otherwise healthy, and apparent spontaneous recovery was the usual outcome. Relapses may occur.

A commercial homeopathic remedy and a placebo were administered orally as individual agents to 18 dogs with atopic dermatitis. The pruritus was reduced by less than 50% in only 2/18 dogs; 1 of these dogs was receiving the homeopathic remedy, the other was receiving the placebo. One dog vomited after administration of the homeopathic remedy.

Zafirlukast and placebo were administered orally as individual agents to 20 dogs with atopic dermatitis. The pruritus was effectively reduced by at least 50% in 2/18 (11%) dogs that completed the trial with zafirlukast. Two dogs vomited after administration of the active drug.

Two recent case reports described a congenital keratinization defect (congenital follicular parakeratosis; CFP) in Rottweiler and Siberian Husky dogs. Skin biopsy specimens revealed marked parakeratosis targeting the hair follicle and numerous intracorneal vacuoles. A retrospective histopathological study was conducted on skin biopsy specimens from 111 dogs with diseases associated with parakeratotic hyperkeratosis to determine whether intracorneal vacuoles were present. Additional criteria evaluated were the size and location of the vacuoles and the degree of parakeratosis. Cases examined included dogs with primary idiopathic seborrhoea, necrolytic migratory erythema (NME), Malassezia dermatitis, zinc-responsive dermatosis, hereditary nasal hyperkeratosis of Labrador Retriever dogs, thallotoxicosis and CFP. Thirty-seven cases (37/111, 33%) had intracorneal vacuoles, including nine cases of primary idiopathic seborrhoea (9/29, 31%), 10 cases of NME (10/18, 56%), five cases of Malassezia dermatitis (5/19, 26%), five cases of zinc-responsive dermatosis (5/36, 14%), five cases of hereditary nasal hyperkeratosis (5/5, 100%) and three cases of CFP (3/3, 100%). If present, intracorneal vacuoles were found throughout all layers of the parakeratin. The sizes of intracorneal vacuoles varied among diseases, but large (> 5 microm) vacuoles only were present in CFP. Biopsies with a larger degree of parakeratosis were significantly more likely to have intracorneal vacuoles (P = < 0.001). Based on this study, intracorneal vacuoles are a common finding in many parakeratotic skin diseases of the dog, but large (> 5 microm) vacuoles are found only in CFP.

A retrospective study was conducted to evaluate the importance of insect hypersensitivity in atopic dogs in the northeastern United States. Fifty (63%) of 79 dogs tested with 7 insect allergens, other than flea, had positive reactions to one or more insects. No dog had positive reactions to insects only. Forty-four dogs underwent immunotherapy. Thirty-one had insect antigens in their prescription mixture and 13 had only conventional environmental allergens. There was no statistical difference in the response rate between the 2 groups. Thus, testing with insect allergens did not decrease the number of dogs with negative skin tests, and including insect allergens in immunotherapy mixtures did not improve the response rate.

Tetracycline and niacinamide were administered in combination to 19 atopic dogs to determine their effectiveness in controlling pruritus. The pruritus was controlled successfully in only one dog. One dog experienced diarrhea that was severe enough to warrant stopping the medication.

Clindamycin hydrochloride capsules (11 mg/kg body weight, q24 h) were administered orally to 20 dogs with deep staphylococcal pyoderma. Response to therapy was excellent in 100% of the dogs. Duration of therapy varied from 21 to 91 d, with an average duration of 45 d. Relapses occurred in 25% of the dogs within a 3-month period. One dog vomited when the clindamycin was given on an empty stomach. Under the conditions of the study, clindamycin was an effective, safe, and convenient antibiotic for the treatment of deep staphylococcal pyoderma in dogs.

A commercial, lamb and rice, dog food with an omega-6:omega-3 fatty acid ratio of 5.5:1 was fed in a single-blinded, self-controlled clinical trial to 18 atopic dogs. The pruritus in 8 of these dogs (44.4%) was satisfactorily controlled within 7 to 21 d, returned within 3 to 14 d after the diet was withdrawn, and was again controlled when the diet was reinstated. Plasma and skin levels of examined fatty acids changed in all 18 dogs when their diet was switched to the test diet. Dogs responding to the test diet had a different pattern of fatty acid change as compared to the dogs which failed to respond to the diet, suggesting that there are subsets of atopic dogs with different fatty acid metabolism capabilities.

The purpose of this study was to determine the efficacy of orally administered milbemycin oxime in the treatment of canine scabies. Forty dogs were treated. Mean drug dosage for all dogs was approximately 2 mg/kg body weight. Twenty-seven dogs received 3 doses separated by 7 d, and 13 dogs received 2 doses separated by 14 d. All dogs were clinically normal following treatment and no adverse reactions were detected.

Objective: To determine the efficacy of increased dosages of milbemycin oxime in the treatment of generalized demodicosis., Design: Prospective clinical trial., Animals: 26 adult dogs with chronic generalized demodicosis., Procedure: In phase 1, milbemycin was administered daily to 13 dogs at an approximate mean dosage of 1 mg/kg of body weight (PO) until 30 days after skin scrapings failed to detect mites. If the mite count had not decreased by 25% from the prior month's examination, the drug dosage was increased to approximately 2 mg/kg. Treatment was considered a failure if the mite count had not changed on 2 successive examinations. In phase 2 involving 13 other dogs, an approximate mean dosage of 2 mg/kg was used. If mite counts had not been reduced to 0 by 180 days, treatment was considered to have failed., Results: In phase 1, when milbemycin was administered at the initial low dosage, 6 dogs were considered to have been cleared of mites. One of these relapsed 2 months after discontinuation of treatment. For the 7 dogs not cleared of mites, the dosage was doubled. Two of these were never cleared of mites. In phase 2, 12 of 13 dogs were cleared of mites after 60 to 180 days of treatment., Clinical Implications: High-dose milbemycin was effective in the treatment of generalized demodicosis.

A unique, symmetrical onychodystrophy is described in 18 dogs. A rather sudden onset of onychomadesis is followed by chronic onychodystrophy affecting all claws. Pain and lameness are recognized in half of the patients, but the dogs are healthy otherwise. Histopathologically, this disorder is characterized by hydropic and lichenoid interface dermatitis. Nine dogs were treated with a commercial, fatty-acid supplement and had good-to-excellent responses. Due to the clinicopathological characteristics of this disorder, the authors propose the name "symmetrical lupoid onychodystrophy."

In 100 dogs with 4 inflammatory dermatologic diseases, buffy coat preparations from EDTA-treated blood samples were examined cytologically. Fifty-four dogs had atopy, 26 had flea-bite hypersensitivity, 17 had sarcoptic mange, and 3 had food allergy. Twenty-eight dogs had 2 or more concurrent skin diseases; most of these had secondary pyoderma. Dogs did not have mast cell tumors. Thirteen samples contained 1 or more mast cells/4 slides reviewed. This study revealed that dogs with inflammatory skin diseases can have a few to many mast cells evident on cytologic examination of buffy coat preparations.

Tylosin tablets (20 mg/kg, q12h) were administered orally to 21 dogs with superficial or deep staphylococcal pyodermas. Response to therapy was excellent in 90.5% of the dogs, and in vitro susceptibility testing correlated perfectly with therapeutic response. Duration of therapy varied from 17 to 91 days, with an average of 33 days. Relapses occurred in 28.6% of the dogs within a three-month period. No side effects were reported. Under the conditions of the study, tylosin was an effective and safe antibiotic for the treatment of staphylococcal pyoderma in dogs.

Terfenadine (5 mg/kg body weight, q12h) and placebo (0.5 grain/dog q12h) were both administered orally as individual agents to 18 dogs with atopy in a double-blinded study. No dog improved. Hyperactivity, polyphagia, lethargy, anorexia, increased pruritus, or ocular discharge were seen in three dogs treated with terfenadine. Under the conditions of the study, terfenadine was not a useful antipruritic agent for the atopic dog.

Thirty adult dogs with chronic generalized demodicosis were treated with oral administration of milbemycin oxime (0.52 to 3.8 mg/kg of body weight, q 24 h). Results of skin scrapings were used to determine whether administration of milbemycin should be continued or discontinued. Dogs that were free of clinical signs of demodicosis 12 months after administration of milbemycin was discontinued were considered cured. Sixteen dogs were cured, 5 dogs were never cleared of mites, and 9 dogs were cleared of mites, but relapsed after administration of milbemycin was discontinued. Adverse effects of milbemycin were rare and transient.

Pruritus associated with allergic skin diseases is an important reason for consultation in canine medicine. The pathophysiology of pruritus is poorly understood in the dog, but histamine, leukotrienes, and prostaglandins appear to be important mediators. The medical management of allergic pruritus in dogs with nonsteroidal anti-inflammatory agents is discussed. Antihistamines, tricyclic antidepressants, and omega-3/omega-6 fatty acid-containing products are often useful, either as single agents, in combinations, or for reducing glucocorticoid requirements.

Cyproheptadine hydrochloride was administered orally at 0.1 to 0.2 mg/kg/day to 16 dogs with allergic pruritus. No dog improved. Polyphagia was observed in 4 dogs (25%).

Twenty dogs with atopy or idiopathic pruritus were treated in a double-blinded clinical trial with computer-randomized and computer-generated sequences of 4 fatty acid-containing products: evening primrose oil, cold water marine fish oil, DVM Derm Caps, and EfaVet. Each dog received each product for a 2-week period. Five of 20 dogs (25%) had a good-to-excellent reduction in their level of pruritus with at least 1 of the products: evening primrose oil (2 dogs), DVM Derm Caps (1), EfaVet (1), DVM Derm Caps and cold water marine fish oil (1). Only 1 dog experienced a side effect (loose stools). Clinical response to fatty acid supplements appeared to be quite individualized, and independent of age, breed, sex, weight, duration of disease, specific diagnosis, or number of positive intradermal test reactions.

An in-office allergy screening kit was evaluated for its repeatability, sensitivity, and specificity in 21 atopic dogs. Results of this testing were compared with results of a full regionalized allergy test offered by the same manufacturer. Results of the screening kit were repeatable and had high specificity but poor sensitivity. The kit as presently marketed is of questionable value as a diagnostic aid for use in dogs with atopy.

A retrospective study on the skin diseases of Chinese Shar Peis was conducted over a 9-year period. Skin disease was found in 58 (49.2%) of the 118 dogs studied. Folliculitis was the most common clinical finding (43 of 58 dogs). In 6 dogs, there was no apparent reason for the folliculitis; however, it was secondary to allergic dermatitis, demodicosis, IgA deficiency, or hypothyroidism in the other dogs. Approximately 20% of the dogs had more than one of these disorders. Serum IgA concentration was measured in 7 dogs and was low in all 7.

Thirty-one dogs with nonlesional pruritus were treated with amitriptyline (1 mg/kg orally q12h) to determine its efficacy in the management of canine allergic pruritus. Seventeen dogs were known to be atopic, 8 were presumed to be atopic, and 6 had pruritus of undetermined etiology. Pruritus was virtually eliminated in 5 (16.1%) dogs, reduced by approximately 50% in another 5 (16.1%) dogs, and not changed in the remaining 21 dogs. Side effects were uncommon.

Twenty dogs with wintertime dry skin were treated with 2 commercial veterinary emollients. All dogs received weekly treatment for 4 weeks according to manufacturers' recommendations. In one-half of the dogs, treatment was initiated with MicroPearls (2 weeks) and followed with Humilac (2 weeks). This sequence was reversed for the other one-half of the dogs. MicroPearls was the superior product in 16 of the 20 dogs (80%). Side effects were rare with either product.

Thirty dogs with a clinical diagnosis of atopy were skin tested with 58 allergens including an aqueous house dust mite extract and a crude house dust extract. Sixty percent of the dogs had positive skin reactions to both the house dust mite and house dust antigens. In atopic dogs, house dust mites appear to be an important allergen source in house dust extracts but are not the only major source.

A 15-year-old dog was evaluated for a nonresponsive generalized pruritic condition of 5 months duration. Routine diagnostic testing, including intradermal testing with 63 inhaled allergens and the feeding of a home-cooked hypoallergenic diet, failed to define the cause of the pruritus. An intradermal skin test with a staphylococcal cell wall/toxoid mixture and a skin biopsy of the skin test site suggested that the dog had a bacterial hypersensitivity. Antibiotic therapy eliminated the pruritus and the dog's pruritus was successfully managed for 3 years with the combined use of subtherapeutic dosages of antibiotics and a commercial staphylococcal vaccine.

A dog with constitutional and cutaneous signs of hyperadrenocorticism had central visual impairment caused by an intracranial mass. Irradiation of the presumed pituitary mass did not improve the dog's condition. At necropsy, the intracranial mass was not of pituitary origin, but rather, was a parapituitary meningioma.

Forty-three dogs having pruritus associated with atopy, flea bite hypersensitivity, and idiopathy were randomly assigned to 1 of 2 treatment protocols. Twenty-three dogs received chlorpheniramine in combination with a fatty acid supplement (DVM Derm Caps). Twenty dogs received the fatty acid supplement at twice the manufacturer's recommended dosage. All 43 dogs were known to be unresponsive to chlorpheniramine and the manufacturer's recommended dosage of the fatty acid supplement when either drug was used alone. Pruritus was satisfactorily controlled in 34.8% of the dogs in the chlorpheniramine--DVM Derm Caps protocol. No dog in the double DVM Derm Caps protocol showed a beneficial response. Side effects were uncommon and mild with either protocol.

Hypothyroidism was documented in an overweight dog with bilateral entropion, blepharoptosis, and multiple, non-inflammatory papular and vesicular lesions on the head. Histologic evaluation of skin biopsy specimens confirmed the diagnosis of mucinous vesiculation. All skin and eyelid abnormalities resolved in response to thyroid hormone supplementation.

Malignant histiocytosis was diagnosed in a 4-year-old-dog. Major clinicopathologic findings included multiple cutaneous tumors, peripheral lymphadenopathy, hepatomegaly, lytic bone lesions, anemia, thrombocytopenia, and monoclonal IgA gammopathy. Pathologic findings included multi-systemic infiltration of erythrophagocytic malignant histiocytes. Chemotherapy was unsuccessful.

Aside from food hypersensitivities, nutritional skin diseases in pets fed commercial foods appear to be very uncommon. Nutritional deficiencies all cause seborrheic skin signs and most of the clinical presentations are not pathognomonic for a specific nutrient deficiency. Careful consideration of the history and the results of skin biopsies usually suggest which nutrient is deficient. Because nutritional supplements can unbalance an animal's diet, they should not be used indiscriminately but only when indicated. Balanced nutrition should minimize the need for supplements.

Abstract A review of case material from the Cornell University College of Veterinary Medicine from 1988 to 1996 identified 20 dogs and one cat with definitive or presumed erythema multiforme. An additional 24 dogs and five cats with definitive or presumed erythema multiforme were found in the veterinary literature. Erythema multiforme accounted for only 0.40% and 0.11%, respectively, of all the canine and feline dermatology cases examined over a 9-year period. German Shepherd dogs and Pembroke Welsh Corgis appeared predisposed. The condition was manifested as a vesiculobullous and/or ulcerative dermatosis in the majority of dogs and cats. In dogs, the most commonly affected body sites included the ventrum (especially axillae and groin), mucocutaneous junctions, oral cavity, pinnae, and footpads. Histopathological findings in cutaneous and mucocutaneous biopsy specimens were consistent with previously published criteria. In dogs, erythema multiforme occurred more frequently in patients receiving drug therapy. In cats, all cases of erythema multiforme were presumed to be drug related. Elimination of the associated trigger factor and supportive care usually resulted in resolution of the erythema multiforme within 1-2 weeks. A retrospective histopathological study was conducted on skin biopsy specimens from 70 dogs with follicular dysplasia syndromes and 102 dogs with endocrine skin disease. Criteria evaluated included dysplastic hair follicles, dysplastic hair shafts, and irregularities in melanization of the follicular epithelium, pilar canal, hair shaft, and perifollicular areas. Dysplastic hair follicles were found in 76% of the dogs with follicular dysplasia syndromes and 46% of the dogs with endocrine skin disease. There was a significantly higher frequency of dysplastic hair follicles and abnormalities in the melanization of the follicular epithelium, pilar canal, and perifollicular areas in the follicular dysplasia syndromes group. Although dysplastic hair shafts were almost exclusively seen only in specimens from dogs with follicular dysplasia syndromes, unique to the follicular dysplasia syndromes group was the clumping of melanin within hair shafts and the presence of dysplastic hair shafts. Histological criteria other than the mere presence of occasional dysplastic hair follicles must be used to differentiate follicular dysplasia syndromes from endocrine skin disease.
