Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES A pathological condition with a high level of severity, caused by drug resistance, is one of the major public health problems worldwide and has worrying consequences. This scenario becomes more complex when we mention the infections caused by the pathogens ESKAPE - Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa and Enterobacter spp. - (because they are microorganisms with high multiresistance and virulence). Thus, repositioning has been a trending topic in the literature and consists of finding new uses for drugs approved for clinical use. In this sense, the present study aimed, in the published articles, to present to the scientific community the current research on treatment alternatives for infections caused by ESKAPE pathogens and perspectives on the anti-infective activities of proton pump inhibitors (PPIs). In the manuscripts, the biological activities of the benzodiazepines clonazepam and diazepam and PPIs omeprazole and esomeprazole as candidate drugs for redirection were verified. Scientific database searches were carried out on treatment alternatives for ESKAPE pathogens and anti-infective activities of PPIs. In addition, the in vitro antibacterial activities of clonazepam, diazepam, omeprazole and esomeprazole against American Type Culture Collection (ATCC) standard bacterial strains and clinical isolates of ESKAPE pathogens were analyzed, determining the minimum inhibitory concentration (MIC), minimum bactericidal concentration (MBC), its interactions with the standard antibacterial ciprofloxacin by the checkerboard method and determination of the fractional inhibitory concentration index (FICI). For clonazepam and diazepam, the clinical impact of the use of these drugs in repositioning was analyzed, and for omeprazole and esomeprazole, the ability to cleave plasmid DNA was verified, and molecules capable of cleavage can act as antibacterials. In the treatment alternatives for infections caused by ESKAPE pathogens, drugs from the pharmacological classes of antineoplastic, anti-inflammatory, antidepressant and antialcoholic have been reported. In the study covering the anti-infective activities of PPIs, activities of these drugs against 17 infectious agents were described, some of them Pseudomonas aeruginosa, Mycobacterium tuberculosis, Enterococcus faecalis, Staphylococcus aureus, Candida albicans and SARS-CoV-2. Drug repositioning, defined as the process of investigating new uses for drugs that have already been clinically approved, has offered a number of advantages in the face of the current lack of therapeutic innovations. As it is a faster, safer and more economical technique than the conventional development of active substances, repositioning has shown advantages. Also, synergism resulting from the combination of two or more drugs can improve drug efficacy, reduce toxicity, and provide an expanded spectrum of activity compared to monotherapeutic regimens. Thus, this study presents candidate drugs for redirection in bacterial infections and in the face of the pandemic caused by the new coronavirus (SARS-CoV-2). The present study investigated the biological activity of antidepressants belonging to the class of selective serotonin reuptake inhibitors (SSRIs) fluoxetine (FX) and paroxetine (PX). The analysis of antibacterial activity was performed in vitro using American Type Culture Collection (ATCC) standard bacterial strains and MDR clinical isolates, determining the minimum inhibitory concentration (MIC), minimum bactericidal concentration (MBC), interaction with the antibacterial ceftriaxone (CTX) using the Checkerboard method and determination of the fractional inhibitory concentration index (FICI). It was observed that, against five ATCC standard strains and 72 MDR clinical isolates collected in 2019 and 2021 in a university hospital in southern Brazil, FX presented MIC values between 4 and 256 µg/ml, while PX ranged from 1 to 256 µg /ml. CTX, on the other hand, presented MIC ≥4096 µg/ml against most strains evaluated. The association of CTX with FX/PX using the Checkerboard method caused CTX to reduce the MIC by up to 32,000 times. FICI values < 0.5 against all strains tested ratified the synergism of the combination of drugs. FX and PX were also able to promote the cleavage of double-stranded plasmid DNA through a hydrolytic mechanism at the three concentrations tested, showing greater efficiency at pH 7,4 at 37 °C. Faced with SARSCoV-2, different studies have found that antidepressant drugs belonging to the class of SSRIs were able to reduce the risk of intubation or death related to Covid-19, reduce the need for intensive medical care, in addition to inhibiting viral titers in up to 99%. This means that, after additional studies, the repositioning of this class of drugs could be a promising alternative in the treatment of patients with Covid-19, especially fluoxetine and fluvoxamine. The antimicrobial activity of several non-antibiotic drugs belonging to different pharmacological classes was also systematically evaluated, in order to present probable candidates for repositioning in infectious diseases, especially antidepressant, antihypertensive, anti-inflammatory, antineoplastic, hypoglycemic drugs, among others. We conclude that drug repositioning is currently a promising approach against bacterial infections and in the fight against Covid-19, and that the antidepressants fluoxetine and paroxetine prove to be promising alternatives that have significant antibacterial and antiviral properties. O aumento substancial das bactérias multidroga resistentes (MDR) tornam urgentes os esforços na descoberta por terapias antimicrobianas mais eficazes. O reposicionamento de medicamentos, definido como o processo de investigar novos usos para medicamentos, já foi aprovado clinicamente e tem oferecido uma série de vantagens diante da atual carência de inovações terapêuticas. Por ser uma técnica mais rápida, segura e econômica que o desenvolvimento convencional de substâncias ativas, o reposicionamento tem apresentado vantagens. Também, o sinergismo resultante da combinação de dois ou mais medicamentos pode melhorar a eficácia dos fármacos, reduzir a toxicidade e fornecer um espectro de atividade ampliado em comparação aos regimes monoterapêuticos. Assim, esse estudo apresenta fármacos candidatos ao redirecionamento em infecções bacterianas e frente à infecção ocasionada pelo novo coronavírus (SARS-CoV-2). O presente estudo investigou a atividade biológica dos antidepressivos pertencentes à classe dos inibidores seletivos da recaptação de serotonina (ISRS) fluoxetina (FX) e paroxetina (PX). A análise da atividade antibacteriana foi realizada in vitro utilizando cepas bacterianas padrão American Type Culture Collection (ATCC) e isolados clínicos MDR, determinando-se a concentração inibitória mínima (CIM), concentração bactericida mínima (CBM), interação com o antibacteriano ceftriaxona (CTX) utilizando o método Checkerboard e determinação do índice de concentração inibitória fracionada (FICI). Foi observado que, frente a cinco cepas padrão ATCC e 72 isolados clínicos MDR coletados em 2019 e 2021 em um hospital universitário no Sul do Brasil, FX apresentou valores de CIM entre 4 e 256 µg/ml, enquanto PX variou de 1 a 256 µg/ml. Já CTX apresentou CIM ≥4096 µg/ml frente a maioria das cepas avaliadas. A associação de CTX com FX/PX utilizando o método Checkerboard fez com que CTX reduzisse a CIM em cerca de até 32 mil vezes. Valores de FICI < 0,5 frente a todas as cepas testadas ratificaram o sinergismo da associação dos medicamentos. FX e PX também foram capazes de promover a clivagem do DNA plasmidial fita dupla por meio de mecanismo hidrolítico nas três concentrações testadas, apresentando maior eficiência em pH 7,4 a 37 °C. Frente a SARS-CoV-2, diferentes estudos constataram que medicamentos antidepressivos pertencentes à classe dos ISRS foram capazes de diminuir o risco de intubação ou morte relacionada à Covid-19, reduzir a necessidade de cuidados médicos intensivos, além de inibir os títulos virais em até 99%. Isso faz com que, após estudos adicionais, o reposicionamento dessa classe de medicamentos possa ser uma alternativa promissora no tratamento de pacientes com Covid-19, especialmente FX e fluvoxamina. Também foi avaliado de modo sistemático a atividade antimicrobiana de diversos fármacos não-antibióticos pertencentes a diferentes classes farmacológicas, a fim de apresentar prováveis candidatos ao reposicionamento em doenças infecciosas, destacando-se fármacos antidepressivos, anti-hipertensivos, anti-inflamatórios, antineoplásicos, hipoglicemiantes, entre outos. Concluímos que o reposicionamento de fármacos é atualmente uma abordagem promissora frente a infecções bacterianas e no combate à Covid-19, e que os antidepressivos FX e PX demonstram ser alternativas promissoras que apresentam propriedades antibacterianas significativas.

Orientador: Rafael Elias Marques Pereira Silva Dissertação (mestrado) - Universidade Estadual de Campinas, Instituto de Biologia Resumo: Arbovírus (arthropod-borne virus) são organismos de grande importância médica que causaram impactos significativos à saúde humana no Brasil e no mundo nas últimas décadas. Entre eles, o gênero Flavivirus destaca-se por compreender importantes patógenos humanos, tais quais o vírus da Dengue, Febre Amarela e Zika. Além destes, há outros com potencial risco de emergência, como o vírus da encefalite de St. Louis (SLEV), capaz de causar neuropatias que podem levar a morte ou causar sequelas. Ainda não há vacinas ou tratamentos específicos para a encefalite causada por SLEV. Como o processo de desenvolvimento de novas drogas é demorado e oneroso, o reposicionamento de fármacos figura como uma boa estratégia para a descoberta de potenciais tratamentos contra SLEV visto que, com os testes toxicológicos dos compostos já realizados, o tempo e o custo das pesquisas é reduzido. A partir disso, foi padronizado e validado um ensaio fenotípico de High Throughput Screening através da biblioteca Covid Box para a triagem de compostos protetores contra a infecção por SLEV. Dos oitenta compostos triados em três concentrações diferentes, nove deles apresentaram efeito protetor contra a infecção, em diferentes níveis. Uma vez que a classe farmacológica dos hits foi distinta, o estudo sugere que pode haver diferentes vias moleculares relevantes na infecção e, portanto, possibilidade de diferentes alvos terapêuticos no desenvolvimento de tratamentos. Sendo assim, busca-se melhor compreender os mecanismos patogênicos do vírus para o desenvolvimento futuro de estratégias viáveis de tratamento desta e outras arboviroses relacionadas Abstract: Arthropod-borne viruses are organisms of great medical importance that have caused significant impacts on human health in the last decades. The Flavivirus genus stands out among them for comprising important human pathogens such as Dengue, Yellow fever, and Zika virus. In addition, there are others with the potential to emerge, such as St. Louis encephalitis virus (SLEV), which is present in the Americas and may cause a neurological disease that leads to death or sequelae. There is no specific treatment or vaccine against SLEV. As drug discovery is an expensive and time-consuming process that can take several years to be accomplished, drug repurposing figures as a viable strategy in the searching for new treatments for diseases once it requires less time and money because the compounds have already been tested and approved for human use. Hence, a phenotypic High-Throughput Screening assay was developed and validated using the MMV Covid-Box compound library. Of eighty compounds screened at three different concentrations, nine showed a protective effect against infection at different levels. Since they belong to distinct pharmacological classes, our study suggests there may be different relevant molecular pathways in SLEV infection, and thus multiple possibilities of therapeutic targets to be explored in the development of treatments. Neurodegenerative disorders, such as Alzheimer's and Parkinson's disease, are characterized by the progressive loss of neuronal cells, resulting in different clinical symptoms according to the affected brain region. Although there are drugs available for the treatment of these diseases, they present relatively low efficacy and are not capable of modifying the course of the disease or stopping its progression. In the field of drug development, drug repurposing could be an interesting strategy to search new therapeutic options against neurodegenerative diseases, since it involves lower costs and time for development. In this review, we discuss the search of new treatments for Alzheimer's and Parkinson's disease through drug repurposing. A focus was given to drugs that modulate neuroinflammation, since it represents a common point among neurodegenerative diseases and has been explored as a target for drug action.

Objective: To present an overview of the pathophysiology of Covid-19 and drug treatments used in Brazil. Methodology: This is a narrative bibliographic review. Searches were performed on official websites (Ministry of Health of Brazil; Butantan); Cochrane, Google Scholar, Pubmed and Scielo databases and supplementary texts. Searches took place between February and June 2021 under the terms 'Covid-19', 'Coronavirus' 'Pandemia', 'Covid-19 vaccines', 'SARS-CoV-2' and 'Covid-19 drug therapies'. Texts in English, Portuguese and Spanish, published from 2020, were included and texts that were not related to the theme were excluded. Results: Azithromycin, corticosteroids, antiretrovirals and anticoagulants exemplify drugs used in the treatment of Covid-19, in addition to prevention with vaccines. Conclusion: During the period of this review, the effort of the scientific community in the repositioning of drugs to treat the disease was noted, but no evidence was found to prove the effectiveness of a specific treatment. Vaccines become a great ally in preventing the disease. Objetivo: Presentar una visión general de la fisiopatología del Covid-19 y los tratamientos farmacológicos utilizados en Brasil. Metodología: Se trata de una revisión bibliográfica narrativa. Las búsquedas se realizaron en sitios web oficiales (Ministerio de Salud de Brasil; Butantan); Bases de datos Cochrane, Google Scholar, Pubmed y Scielo y textos complementarios. Las búsquedas se realizaron entre febrero y junio de 2021 bajo los términos 'Covid-19', 'Coronavirus', 'Pandemia', 'Vacunas Covid-19', 'SARS-CoV-2' y 'Terapias farmacológicas Covid-19'. Se incluyeron los textos en inglés, portugués y español, publicados a partir de 2020 y se excluyeron los textos que no estaban relacionados con el tema. Resultados: La azitromicina, los corticosteroides, los antirretrovirales y los anticoagulantes ejemplifican los fármacos utilizados en el tratamiento de Covid-19, además de la prevención con vacunas. Conclusión: Durante el período de esta revisión se notó el esfuerzo de la comunidad científica en el reposicionamiento de fármacos para tratar la enfermedad, pero no se encontró evidencia que demuestre la efectividad de un tratamiento específico. Las vacunas se convierten en un gran aliado para prevenir la enfermedad. Objetivo: Apresentar um panorama sobre a fisiopatologia da Covid-19 e tratamentos medicamentosos utilizados no Brasil. Orientador: Karina Cogo Müller Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: Introdução: A sinvastatina é um fármaco hipolipemiante associado a melhorias nas condições cardiovasculares de pacientes afetados por dislipidemias. Em células humanas, atua inibindo a enzima 3-Hidroxi-3-MetilGlutaril Coenzima-A Redutase (HMG-CoA Redutase), reduzindo a síntese de colesterol, em uma via conhecida como "via do mevalonato". Nos últimos anos, têm sido reportados vários efeitos não-lipídeo relacionados, associados à sinvastatina, os chamados efeitos pleiotrópicos como a atividade antimicrobiana, sendo o Staphylococcus aureus uma das bactérias mais sensíveis ao fármaco. Objetivo: Investigar o efeito da sinvastatina sobrea via de produção de mevalonato e sobre a expressão de genes relacionados à síntese de peptidoglicanos e à produção de fatores de virulência em resposta ao estresse de parede celular em cepas de S. aureus. Material e Métodos: S. aureus ATCC 29213 e ATCC 33591 foram expostos a concentrações inibitórias e subinibitórias de sinvastatina na presença de mevalonato exógeno em diferentes concentrações (25 µM, 100 µM, 200 µM, 500 µM e 1 mM) e o crescimento bacteriano foi acompanhado durante 24 horas. Ainda, S. aureus ATCC 29213 foi submetido ao tratamento com uma concentração subinibitória de sinvastatina (7,8 µg / mL) durante 6 horas e ensaios de análise de expressão gênica relativa foram conduzidos para avaliar a influência do tratamento nos genes envolvidos na síntese de peptidoglicano (uppS, uppP e murG), bem como na resposta ao estresse de parede celular (vraX, sgtB e tcaA). Resultados: A adição de mevalonato conseguiu reverter de maneira parcial ou total a inibição causada pela sinvastatina em cepas de S. aureus ATCC 29213 e ATCC 33591 (p < 0,05. ANOVA 2 fatores – Bonferroni). Para as concentrações inibitórias de sinvastatina (31,25 µg / mL para S. aureus ATCC 29213 e 62,5 µg / mL para ATCC 33591), as maiores concentrações de mevalonato (1 mM, 500 µM e 200 µM na cepa 29213 e 1 mM, 500 µM, 100 µM e 25 µM para 33591) reverteram de forma parcial o crescimento bacteriano. Para as concentrações subinibitórias de sinvastatina (15,62 µg / mL e 7,8 µg / mL para ATCC 29213, e 31,25 µg / mL para ATCC 33591), a adição de menores concentrações de mevalonato (500 µM, 200 µM, 100 µM e 25 µM) promoveu reversão total ou parcial do crescimento bacteriano. A concentração de sinvastatina a 15,65 µg / mL, não promoveu efeito inibitório na cepa 33591. Adicionalmente, foi observado aumento de 28 vezes na expressão do gene vraX (p < 0,001. Teste t de student) e uma redução de 30% na expressão de murG (p < 0,05. Teste t). Foi observada, ainda, uma redução de 10% na expressão do gene tcaA e um aumento de 2 vezes em uppS, porém sem significância estatística, não havendo alteração na expressão de sgtB e uppP. Conclusão: A inibição causada pela sinvastatina foi revertida total ou parcialmente pela adição de diferentes concentrações de mevalonato. Ainda, sinvastatina promoveu aumento na expressão do gene vraX e redução em murG. Os resultados do presente estudo fornecem evidências adicionais para o entendimento do mecanismo de ação antimicrobiano da sinvastatina Abstract: Background: Simvastatin is a lipid-lowering drug associated with improvements in cardiovascular conditions in patients affected by dyslipidemia. In human cells, it acts by inhibiting the enzyme 3-Hydroxy-3-MethylGlutaryl Coenzyme-A Reductase (HMG-CoA reductase), reducing cholesterol synthesis, in a pathway known as the "mevalonate pathway". In recent years, several non-lipid related effects associated with simvastatin have been reported, the so-called pleiotropic effects such as antimicrobial activity, with Staphylococcus aureus being one of the most sensitive bacteria to the drug. Objective: To investigate the effect of simvastatin on the mevalonate pathway and on the expression of genes related to peptidoglycan synthesis and the production of virulence factors in response to cell wall stress in S. aureus strains. Material and Methods: S. aureus ATCC 29213 and ATCC 33591 were exposed to inhibitory and subinhibitory concentrations of simvastatin in the presence of exogenous mevalonate at different concentrations (25 µM, 100 µM, 200 µM, 500 µM and 1 mM) and growth bacterial was followed for 24 hours. Furthermore, S. aureus ATCC 29213 was subjected to treatment with a sub-inhibitory concentration of simvastatin (7.8 µg / mL) for 6 hours and relative gene expression analysis assays were conducted to evaluate the influence of the treatment on genes involved in the synthesis of peptidoglycan (uppS, uppP and murG) as well as in response to cell wall stress (vraX, sgtB and tcaA). Results: The addition of mevalonate was able to partially or completely reverse the inhibition caused by simvastatin in S. aureus ATCC 29213 and ATCC 33591 strains (p < 0.05. 2-way ANOVA – Bonferroni). For inhibitory concentrations of simvastatin (31.25 µg / mL for S. aureus ATCC 29213 and 62.5 µg / mL for ATCC 33591), the highest concentrations of mevalonate (1 mM, 500 µM and 200 µM in strain 29213 and 1 mM, 500 µM, 100 µM and 25 µM for 33591) partially reversed bacterial growth. For subinhibitory concentrations of simvastatin (15.62 µg / mL and 7.8 µg / mL for ATCC 29213, and 31.25 µg / mL for ATCC 33591), the addition of lower concentrations of mevalonate (500 µM, 200 µM, 100 µM and 25 µM) promoted total or partial reversion of bacterial growth. Simvastatin concentration at 15.65 µg / mL did not promote an inhibitory effect on strain 33591. Additionally, a 28-fold increase in vraX gene expression was observed (p < 0.001. Student t test, unpaired, two-tailed) and a 30% reduction in murG expression (p < 0.05. t-test). It was also observed a 10% reduction in tcaA gene expression and a 2-fold increase in uppS, but without statistical significance, with no change in sgtB and uppP expression. Conclusion: The inhibition caused by simvastatin was totally or partially reversed by the addition of different concentrations of mevalonate. Furthermore, simvastatin promoted an increase in the expression of the vraX gene and a reduction in murG. Orientador: Catarina Raposo Dias Carneiro Dissertação (mestrado) - Universidade Estadual de Campinas, Instituto de Biologia Resumo: DCs com função defeituosa encontradas em pacientes com câncer possuem relação direta com o pobre prognóstico da doença. Com o desenvolvimento de novas imunoterapias, percebeu-se que alguns fármacos podem reduzir a ação supressora do microambiente tumoral (TME) e serem úteis como imunoadjuvantes no tratamento do câncer. O sildenafil (Viagra®?) é um inibidor de fosfodiesterase 5 (PDE5-I) que aumenta a resposta antitumoral de linfócitos T, quando administrado sistemicamente. Porém, os efeitos desse medicamento nas DCs, células que determinam o fenótipo dos linfócitos T, ainda não são conhecidos. Portanto o objetivo geral dessa pesquisa foi avaliar o efeito do sildenafil como um modulador de DCs in vitro, visando o uso desse fármaco como um adjuvante em vacina de DCs contra adenocarcinoma mamário (EO771); foi objetivo paralelo padronizar um modelo experimental mais adequado, caracterizando o efeito do GM-CSF e do sobrenadante de L929 na diferenciação das DCs. DCs geradas da medula óssea de camundongos foram expostas ao lisado tumoral de EO771 e/ou ao sidenafil (0,1, 1 e 10 µM) por 24 h in vitro, e posteriormente foi avaliada a expressão de marcadores de superfície (MHCII, CD11c e CD80) e a liberação de citocinas (IL1-?, IL-10 e TNF-?). Também foram analisadas as populações de linfócitos (CD4+ e CD8+) mantidas em cocultura com as DCs moduladas e, por fim, a capacidade desses linfócitos em matar a linhagem tumoral (EO771), quando colocados em cocultura, para avaliar a eficácia in vitro da modulação das DCs. O sildenafil aumentou a expressão de MHCII e de CD80, a secreção de IL-1? e de TNF-?, enquanto diminuiu a expressão de de IL-10, indicando a indução de um perfil ativado dessas células. O tratamento aumentou o percentual das populações de linfócitos duplo-positivos CD4+CD8+. Entretanto, não houve aumento da morte celular de EO771 em 24 h de cocultura com linfócitos diferenciados pelas DCs tratadas com o sildenafil. Como resultado paralelo, a concentração de 10 ng/mL de GM-CSF foi considerada a mais eficaz para diferenciar populações CD11c+ MHCII+ F4/80-. Nosso trabalho sugere que o sildenafil modulou as DCs para um perfil fenotípico e funcional ativado, porém não melhorou a capacidade dessas células em induzir uma reposta antitumoral in vitro, nas condições de tempo, linhagem tumoral e concentração estudadas. With the development of new immunotherapies, it was realized that some drugs can reduce the suppressive action of the tumor microenvironment (TME) and be useful as immunoadjuvants in the treatment of cancer. Sildenafil (Viagra®) is a phosphodiesterase 5 (PDE5-I) inhibitor that increases the T lymphocyte antitumor response when administered systemically. However, the effects of this drug on DCs, cells that determine the phenotype of T lymphocytes, are not yet known. Therefore, the general objective of this research was to evaluate the effect of sildenafil as a DC modulator in vitro, aiming at the use of this drug as an adjuvant in a DC vaccine against breast adenocarcinoma (EO771); it was a parallel objective to standardize a more adequate experimental model, characterizing the effect of GM-CSF and L929 supernatant on the differentiation of DCs. DCs generated from the bone marrow of mice were exposed to the tumor lysate of EO771 and/or to sidenafil (0.1, 1 and 10 µM) for 24 h in vitro, and subsequently the expression of surface markers (MHCII, CD11c and CD80) and the release of cytokines (IL1-?, IL-10 and TNF-?). Lymphocyte populations (CD4 and CD8) maintained in co-culture with modulated DCs were also analyzed, and finally, the capacity of these lymphocytes to kill the tumor line (EO771), when placed in co-culture, to evaluate the in vitro efficacy of the modulation of the DCs. Sildenafil was able to increase the expression of MHCII in 24 h of treatment and CD80 in 1 h, the secretion of IL-1? in 1 h, that of TNF-? in 24 h and the percentage of CD4 + CD8 + populations. It also decreased the expression of CD11c in 1h, the secretion of IL-10 and was not related to cell death of EO771 in coculture with DCs and lymphocytes in 24 h of treatment. Our work suggests that sildenafil acts positively on the phenotype and function of DCs and has no chemotherapeutic action. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES Infections caused by multiresistant microrganisms have increased considerably in recent years, reaching approximately one fifth of worldwide mortality, becoming a public health problem. Another worrying factor is the resistance to antitumor drugs that lead to therapeutic failure in cancer patients. Thus, the search for more potent and selective drugs with antibacterial and antitumor action is essential, through faster and more effective alternatives, such as drug repositioning, in order to overcome these challenges and limitations. Different non-antibiotics are reported in the literature with potential antibacterial and antitumor activity, among them the antihypertensive class. Amlodipine and valsartan are non-antibiotic drugs commonly used in the medical clinic to treat high blood pressure. Thus, the aim of this study, in the first article, was to present to the scientific community a selection of studies that address the repositioning of the non-antibiotic amlodipine and its synergistic effects in the treatment of infectious diseases. The aim of the manuscript was to evaluate the antibacterial activity in vitro alone and in association with ciprofloxacin of the amlodipine and valsartan drugs, as well as to evaluate the cleavage activity of the plasmid DNA of the studied drugs, as well as their probable mechanism of action involved. In the first article, we present sixteen studies involving anlodipine repositioning as an alternative in the treatment of microbial and neoplastic infections, demonstrating significant synergistic activity when associated with ten different drugs. In the manuscript, antibacterial activity was evaluated against ten standard American Type Culture Collection (ATCC) reference bacterial strains and thirty-one multiresistant clinical isolates (MDR) from patients admitted to the University Hospital of Santa Maria, where anlodipine alone was active against all bacteria, especially against Gram-positive as the Coagulase-negative methicillin-resistant Staphylococcus (MRSCoN). Unlike with valsartan, which was active only against two strains and one isolate. When combined with ciprofloxacin, amlodipine resulted in nine synergistic isolates, whereas valsartan did not yield significant results. Also, both drugs were able to cleave the plasmid DNA under the defined conditions of pH and temperature, and in the investigation of the probable mechanism of action involved, it is suggested that both act through the hydrolytic mechanism because they were unable to inhibit the activity of cleavage in the presence of free radical scavengers. Thus, we highlight the repositioning of these drugs as an effective way to treat infections caused by multiresistant microrganisms. Infecções causadas por microrganismos multirresistentes têm aumentado consideravelmente nos últimos anos, chegando aproximadamente um quinto da mortalidade mundial, tornando-se um problema de saúde pública. Outro fator preocupante é a resistência a medicamentos antitumorais que levam ao fracasso terapêutico em pacientes com câncer. Assim, é essencial a busca por medicamentos mais potentes e seletivos com ação antibacteriana e antitumoral, através de alternativas mais rápidas e eficazes, como o reposicionamento de medicamentos, a fim de contornar estes desafios e limitações. Diferentes não antibióticos são relatados na literatura com potencial atividade antibacteriana e antitumoral, dentre eles a classe dos anti-hipertensivos. O anlodipino e valsartana são medicamentos não antibióticos comumente utilizados na clínica médica para tratamento da hipertensão arterial. Sendo assim, o objetivo deste estudo, no primeiro artigo, foi apresentar à comunidade científica uma seleção de estudos que abordem o reposicionamento do não antibiótico anlodipino e seus efeitos sinérgicos no tratamento de doenças infecciosas e neoplásicas. No manuscrito, objetivou-se avaliar a atividade antibacteriana in vitro isoladamente e em associação ao ciprofloxacino dos fármacos anlodipino e valsartana, e ainda, avaliar a atividade de clivagem do DNA plasmidial dos medicamentos em estudo, bem como seu provável mecanismo de ação envolvido. No primeiro artigo, apresentamos dezesseis pesquisas envolvendo reposicionamento do anlodipino como alternativa na terapêutica de infecções microbianas e neoplásicas, demonstrando atividade sinérgica significativa quando associado a dez diferentes fármacos. No manuscrito, a atividade antibacteriana foi avaliada frente a dez cepas bacterianas padrão de referência American Type Culture Collection (ATCC) e trinta e um isolados clínicos multirresistentes (MDR) provenientes de pacientes admitidos no Hospital Universitário de Santa Maria, onde anlodipino sozinho foi ativo frente a todas as bactérias, especialmente frente as Gram-positivas como os Staphylococcus coagulase negativos resistentes à meticilina (MRSCoN). Diferente do que ocorre com valsartana, que foi ativa apenas frente a duas cepas e um isolado. Quando associados ao ciprofloxacino, anlodipino resultou em nove isolados sinérgicos, enquanto que valsartana não obteve resultados significativos. Ainda, ambos os medicamentos foram aptos a clivar o DNA plasmidial nas condições definidas de pH e temperatura, sendo que na investigação do provável mecanismo de ação envolvido, sugere-se que ambos atuem através do mecanismo hidrolítico pois não foram capazes de inibir a atividade de clivagem na presença de sequestradores de radicais livres. Sendo assim, evidenciamos o reposicionamento destes fármacos como forma eficaz na terapêutica de infecções ocasionadas por microrganismos multirresistentes.

Concomitant with the increase in infections caused by multidrugresistant microorganisms (MDR), there has been a drastic reduction of new antibacterials in the market, as well as investments for their creation. In addition to antimicrobial resistance, resistance to antitumor drugs is already a reality. Thus, it is essential to search for new substances and / or compounds that exhibit antibacterial and / or antitumor activity. With this, drug repositioning has emerged as an alternative approach for the faster identification of effective drugs. In this sense, the present study aimed, in the first article, to present to the scientific community the current research on the repositioning of antidepressants for the treatment of infections caused by microorganisms. In the second manuscript, the individual antibacterial activity and in combination with antibacterials of the non-antibiotics escitalopram oxalate and clonazepam were analyzed against standard strains and clinical isolates multiresistant. In addition, the chemical nuclease ability of these drugs was analyzed. In the first article, we report that antidepressant drugs, highlighting the class of selective serotonin reuptake inhibitors (SSRIs), have significant activity against several microorganisms. We present 14 studies covering the repositioning of antidepressant drugs to treat infections. Among the antidepressants, escitalopram oxalate was reported with activity reported in vitro against standard bacterial strains. In the second manuscript, we report the antibacterial and chemical nuclease activity of the drugs oxalate of escitalopram and clonazepam. The first was active against a standard Gram-negative strain and eight Grampositive MDR clinical isolates. The second drug showed activity against both Gram-positive and Gram-negative, with four standard Gram-positive strains and all Gram-positive and Gram-negative MDR clinical isolates. Escitalopram oxalate and clonazepam showed activity in vitro when individually associated with ciprofloxacin and sulfamethoxazole-trimethoprim against standard strains and clinical MDR Gram-positive isolates. Clonazepam was also active against a gram-negative bacterium. Clonazepam was able to cleave the plasmidial DNA. Further studies should be performed by our research group, in order to verify the activity of clonazepam against tumor cell lines. Therefore, in this work we show that these two drugs constitute a promising alternative to redirection in the treatment of infectious diseases and also for neoplasias. Further studies, using different techniques, are needed to elucidate the mechanisms of action involved. Concomitante ao aumento das infecções ocasionadas por microrganismos resistentes a múltiplas drogas (MDR) ocorreu uma redução drástica de novos antibacterianos no mercado, bem como, investimentos para sua criação. Além da resistência aos antimicrobianos, a resistência aos medicamentos antitumorais também já é uma realidade. Assim, é essencial a pesquisa de novas substâncias e/ou compostos que apresentem atividade antibacteriana e/ou antitumoral. Com isso, o reposicionamento de fármacos surgiu como uma abordagem alternativa para a identificação mais rápida de medicamentos eficazes. Nesse sentido, o presente estudo objetivou, no primeiro artigo, apresentar à comunidade científica a pesquisa atual do reposicionamento de antidepressivos para o tratamento de infecções ocasionadas por microrganismos. No segundo manuscrito, analisou-se a atividade antibacteriana individual e em combinação com antibacterianos dos não-antibióticos oxalato de escitalopram e clonazepam frente a cepas padrão e isolados clínicos multirresistentes. Além disso, foi analisada a capacidade de nuclease química desses medicamentos. No primeiro artigo, relatamos que medicamentos antidepressivos, destacando a classe dos inibidores seletivos da recaptação da serotonina (ISRSs), apresentam atividade significativa contra vários microrganismos. Foram apresentados 14 estudos abrangendo o reposicionamento de medicamentos antidepressivos para tratamento de infecções. Dentre os antidepressivos, foi citado o oxalato de escitalopram com atividade relatada in vitro frente a cepas bacterianas padrão. No segundo manuscrito, relatamos a atividade antibacteriana e de nuclease química dos medicamentos oxalato de escitalopram e clonazepam. O primeiro foi ativo frente a uma cepa padrão Gram-negativa e oito isolados clínicos MDR Gram-positivos. O segundo medicamento apresentou atividade tanto frente a Gram-positivas quanto a Gram-negativas, sendo quatro cepas padrão Gram-positivas e todos os isolados clínicos MDR Gram-positivos e Gram-negativos. Oxalato de escitalopram e clonazepam apresentaram atividade in vitro quando associados, individualmente, com ciprofloxacino e sulfametoxazol-trimetoprima frente a cepas-padrão e isolados clínicos MDR Gram-positivos. O clonazepam também foi ativo frente a uma bactéria Gram-negativa. Clonazepam foi capaz de clivar o DNA plasmidial. Mais estudos devem ser realizados pelo nosso grupo de pesquisa, com objetivo de verificar a atividade do clonazepam frente a linhagens celulares tumorais. Portanto, nesse trabalho evidenciamos que estes dois medicamentos constituem uma alternativa promissora ao redirecionamento no tratamento de doenças infecciosas e também para neoplasias. São necessários estudos adicionais, utilizando diferentes técnicas, para a elucidação dos mecanismos de ação envolvidos.

Background: Alzheimer’s disease (AD) is a multifactorial and complex neuropathology that involves impairment of many intricate molecular mechanisms. Despite recent advances, AD pathophysiological characterization remains incomplete, which hampers the development of effective treatments. In fact, currently, there are no effective pharmacological treatments for AD. Integrative strategies such as transcription regulatory network and master regulator analyses exemplify promising new approaches to study complex diseases and may help in the identification of potential pharmacological targets. Methods: In this study, we used transcription regulatory network and master regulator analyses on transcriptomic data of human hippocampus to identify transcription factors (TFs) that can potentially act as master regulators in AD. All expression profiles were obtained from the Gene Expression Omnibus database using the GEOquery package. A normal hippocampus transcription factor-centered regulatory network was reconstructed using the ARACNe algorithm. Master regulator analysis and two-tail gene set enrichment analysis were employed to evaluate the inferred regulatory units in AD case-control studies. Finally, we used a connectivity map adaptation to prospect new potential therapeutic interventions by drug repurposing. Results: We identified TFs with already reported involvement in AD, such as ATF2 and PARK2, as well as possible new targets for future investigations, such as CNOT7, CSRNP2, SLC30A9, and TSC22D1. Furthermore, Connectivity Map Analysis adaptation suggested the repositioning of six FDA-approved drugs that can potentially modulate master regulator candidate regulatory units (Cefuroxime, Cyproterone, Dydrogesterone, Metrizamide, Trimethadione, and Vorinostat). Conclusions: Using a transcription factor-centered regulatory network reconstruction we were able to identify several potential molecular targets and six drug candidates for repositioning in AD. Our study provides further support for the use of bioinformatics tools as exploratory strategies in neurodegenerative diseases research, and also provides new perspectives on molecular targets and drug therapies for future investigation and validation in AD.

Drug discovery is a very expensive and time-consuming endeavor. Fortunately, recent omics technologies and Systems Biology approaches introduced interesting new tools to achieve this task, facilitating the repurposing of already known drugs to new therapeutic assignments using gene expression data and bioinformatics. The inherent role of transcription factors in gene expression modulation makes them strong candidates for master regulators of phenotypic transitions. However, transcription factors expression itself usually does not reflect its activity changes due to post-transcriptional modifications and other complications. In this aspect, the use of high-throughput transcriptomic data may be employed to infer transcription factors-targets interactions and assess their activity through co-expression networks, which can be further used to search for drugs capable of reverting the gene expression profile of pathological phenotypes employing the connectivity maps paradigm. Following this idea, we argue that a module-oriented connectivity map approach using transcription factors-centered networks would aid the query for new repositioning candidates. Through a brief case study, we explored this idea in bipolar disorder, retrieving known drugs used in the usual clinical scenario as well as new candidates with potential therapeutic application in this disease. Indeed, the results of the case study indicate just how promising our approach may be to drug repositioning.
