Polymethylmethacrylate (PMMA) is the most commonly used bone void filler for vertebral augmentation in osteoporotic fracture. It provides mechanical stability and immediate pain relief; however, PMMA is not osteointegrated and is separated from the surrounding bone tissue by a thin fibrous layer. The aim of this study was to investigate the effect of nanocrystalline hydroxyapatite (HA) on osteointegration of PMMA in a sheep model. A composite material, consisting of PMMA and nanocrystalline HA (70:30, v/v), was implanted in one distal femur, with pure PMMA in the other femur as a control. Three and 6 months after implantation, the distal femora were histologically investigated. All composite implants exhibited a tight junction to the surrounding bone tissue, with minimal bone ingrowth into the outer surface of the implant. In comparison, with use of the control implants, we observed an overall bone resorption around pure PMMA, with fibrous connective tissue encapsulating the implant. These results suggest that nanocrystalline HA enables osteointegration of PMMA in bone tissue, which might alter the biomechanical characteristics of the osteoporotic vertebral body after augmentation., (Copyright © 2012 Orthopaedic Research Society.)

Background: The purpose of this study was to evaluate the performance a newly developed nanocrystalline hydroxyapatite, OSTIM following functional implantation in femoral sites in thirty-eight sheep for 1, 2 or 3 months. Ostim 35 was compared to an established calcium phosphate, Alpha BSM., Methods: Biomechanical testing, micro-CT analysis, histological and histomorphological analyses were conducted to compare the treatments including evaluation of bone regeneration level, material degradation, implant biomechanical characteristics., Results: The micro-computed tomography (microCT) analysis and macroscopic observations showed that Ostim seemed to diffuse easily particularly when the defects were created in a cancellous bone area. Alpha BSM remained in the defect.The performance of Ostim was good in terms of mechanical properties that were similar to Alpha BSM and the histological analysis showed that the bone regeneration was better with Ostim than with Alpha BSM. The histomorphometric analysis confirmed the qualitative analysis and showed more bone ingrowth inside the implanted material with Ostim when compared to Alpha BSM at all time points., Conclusions: The successful bone healing with osseous consolidation verifies the importance of the nanocrystalline hydroxyapatite in the treatment of metaphyseal osseous volume defects in the metaphyseal spongiosa.

The microvascularization of metaphyseal bone defects filled with nanoparticulate, biodegradable hydroxyapatite biomaterial with and without platelet factors enrichment was investigated in a minipig model. Results from morphological analysis and PECAM-1 immunohistochemistry showed the formation of new blood vessels into the bone defects by sprouting and intussusception of pre-existing ones. However, no significant differences were observed in the microvascularization of the different biomaterials applied (pure versus platelet factors-enriched hydroxyapatite), concerning the number of vessels and their morphological structure at day 20 after operation. The appearance of VEGFR-2 positive endothelial progenitor cells in the connective tissue between hydroxyapatite particles was also found to be independent from platelet factors enrichment of the hydroxyapatite bone substitute. In both groups formation of lymphatic vessels was detected with a podoplanin antibody. No differences were noted between HA/PLF- and HA/PLF+ implants with respect to the podoplanin expression level, the staining pattern or number of lymphatic vessels. In conclusion, the present study demonstrates different mechanisms of blood and lymphatic vessel formation in hydroxyapatite implants in minipigs.

There are no studies on the cellular activity in the early phase of biodegradation and bone healing of bone substitutes loaded with platelet factors (PLF). The purpose of this study was to evaluate the cellular effects of PLF in combination with nanoparticulate hydroxyapatite (HA) on the biodegradation and bone formation after 20 days. Autogenous PLFs were obtained by centrifugation of miniature pig blood samples and subsequent degranulation of platelets by calcium and thrombin. A cylindrical bone defect with a diameter of 8.9 mm was created in the distal femoral condyle of 20 miniature pigs. Four of the defects were left empty, 8 were filled with HA with loading and 8 with HA loaded with PLF. The distal femur was harvested after 20 days and TRAP-staining, cathepsin-K and CD44 staining and scanning electron microscopy were performed for cellular assessment of biodegradation was done. Histomorphometry of new bone formation and of biodegradation of HA material was performed. PLF loading of HA led to statistically significant more TRAP-positive cells with enhanced biodegradation of the nanoparticulate HA but no statistically enhanced new bone formation compared to unloaded HA. Furthermore, there was a higher number of CD44 and cathepsin-K positive cells by PLF-loading. In summary, PLF led to stimulation of the cellular process of the biodegradation of HA.

Vascular endothelial growth factor (VEGF) plays a key role for the interaction of osteoblasts and endothelial cells and, therefore, is an important factor for the osteointegration of bone substitutes. The aim of the current work was to study the effects of platelet growth factors (PLF) on new blood vessel formation and VEGF-receptors expression pattern in bone defects filled with nanocrystalline hydroxyapatite (HA) paste in miniature-pigs. Conventional histology, RT-PCR for VEGF and receptors mRNA, Western blot analysis, immunohistochemical staining and quantitative assessment of newly formed vessels was performed. HA enriched with platelet growth factor (HA/PLF+) led to an up-regulation of VEGF-R1 synthesis, a slightly enhanced number of newly formed vessels with higher sprouting activity compared with HA without PLF (HA/PLF-) filling defects. These observation are most likely attributable to a stimulating effect of TGF-ss from the platelet factor on VEGF expression in osteoblasts.

The use of polymethylmetacrylate beads for local delivery of antibiotics requires a second surgical procedure for their removal and resorbable calcium sulphate exhibits cytotoxic effects. In this work, a bioresorbable composite of calcium sulphate and nanoparticulate hydroxyapatite (PerOssal was studied regarding its antibiotic release properties and biocompatibility. Material characteristics of plain PerOssal and pure calcium sulphate pellets were studied using scanning and electron microscopy and X-ray methods. Pellets were soaked with gentamicin and vancomycin, respectively. Release properties of both antibiotics from both materials were investigated over 10 days. Quantitative and qualitative cytotoxic assays were performed for biocompatibility testing. Specific surface was 106 m(2)/g for PerOssal and 2.2 m(2)/g for pure calcium sulphate. Almost complete elution of gentamicin was found for both carrier materials (94.7% for PerOssal vs. 95.8% for calcium sulphate) within 10 days, whereas vancomycin release was higher for PerOssal (96.3% vs. 74.8%). PerOssal showed higher initial and lower release after approximately 5 days compared to calcium sulphate. No significant in vitro cytotoxic differences were found between PerOssal and nontoxic cell culture medium. Calcium sulphate showed cytotoxic effects in two out of four tests. PerOssal exhibits excellent properties regarding resorption, biocompatibility, and antibiotic release.
