Multiple myeloma (MM) patients complain of pain and stiffness limiting motility. To determine if patients can benefit from vertebroplasty, we assessed muscle activation and co-activation before and after surgery. Five patients with MM and five healthy controls performed sitting-to-standing and lifting tasks. Patients performed the task before and one month after surgery. Surface electromyography (sEMG) was recorded bilaterally over the erector spinae longissimus and rectus abdominis superior muscles to evaluate the trunk muscle activation and co-activation and their mean, maximum, and full width at half maximum were evaluated. Statistical analyses were performed to compare MM patients before and after the surgery, MM and healthy controls and to investigate any correlations between the muscle's parameters and the severity of pain in patients. The results reveal increased activations and co-activations after vertebroplasty as well as in comparison with healthy controls suggesting how MM patients try to control the trunk before and after vertebroplasty surgery. The findings confirm the beneficial effects of vertebral consolidation on the pain experienced by the patient, despite an overall increase in trunk muscle activation and co-activation. Therefore, it is important to provide patients with rehabilitation treatment early after surgery to facilitate the CNS to correctly stabilize the spine without overloading it with excessive co-activations.

Workers often develop low back pain due to manually lifting heavy loads. Instrumental-based assessment tools are used to quantitatively assess the biomechanical risk in lifting activities. This study aims to verify the hypothesis that high-density surface electromyography (HDsEMG) allows an optimized discrimination of risk levels associated with different fatiguing lifting conditions compared to traditional bipolar sEMG. 15 participants performed three lifting tasks with a progressively increasing lifting index (LI) each lasting 15 min. Erector spinae (ES) activity was recorded using both bipolar and HDsEMG systems. The amplitude of both bipolar and HDsEMG can significantly discriminate each pair of LI. HDsEMG data could discriminate across the different LIs starting from the fourth minute of the task while bipolar sEMG could only do so towards the end. The higher discriminative power of HDsEMG data across the lifting tasks makes such methodology a valuable tool to be used to monitor fatigue while lifting and could extend the possibilities offered by currently available instrumental-based tools., (Copyright © 2021. Upper limb work-related musculoskeletal disorders have a 12-month prevalence ranging from 12 to 41% worldwide and can be partly caused by handling low loads at high frequency. The association between the myoelectric manifestation of elbow flexor muscle fatigue and occupational physical demand has never been investigated. It was hypothesized that an elbow flexor muscle fatigue index could be a valid risk indicator in handling low loads at high frequency. This study aims to measure the myoelectric manifestation of muscle fatigue of the three elbow flexor muscles during the execution of the work tasks in different risk conditions. Fifteen right-handed healthy adults were screened using a movement analysis laboratory consisting of optoelectronic, dynamometer and surface electromyographic systems. The main result indicates that the fatigue index calculated from the brachioradialis is sensitive to the interaction among risk classes, session and gender, and above all it is sensitive to the risk classes.

This study aims at evaluating upper limb muscle coordination and activation in workers performing an actual use-case manual material handling (MMH). The study relies on the comparison of the workers' muscular activity while they perform the task, with and without the help of a dual-arm cobot (BAZAR). Eleven participants performed the task and the flexors and extensors muscles of the shoulder, elbow, wrist, and trunk joints were recorded using bipolar electromyography. The results showed that, when the particular MMH was carried out with BAZAR, both upper limb and trunk muscular co-activation and activation were decreased. Therefore, technologies that enable human-robot collaboration (HRC), which share a workspace with employees, relieve employees of external loads and enhance the effectiveness and calibre of task completion. Additionally, these technologies improve the worker's coordination, lessen the physical effort required to interact with the robot, and have a favourable impact on his or her physiological motor strategy. Low back pain (LBP) is a leading cause of disability in the workplace, often caused by manually lifting of heavy loads. Instrumental-based assessment tools are used to quantitatively assess the biomechanical risk of lifting activities. This study aims to verify that, during the execution of fatiguing frequency-dependent lifting, high-density surface electromyography (HDsEMG) allows the discrimination of healthy controls (HC) versus people with LBP and biomechanical risk levels. Fifteen HC and eight people with LBP performed three lifting tasks with a progressively increasing lifting index, each lasting 15 min. Erector spinae (ES) activity was recorded using HDsEMG and amplitude parameters were calculated to characterize the spatial distribution of muscle activity. LBP group showed a less ES activity than HC (lower root mean square across the grid and of the activation region) and an involvement of the same muscular area across the task (lower coefficient of variation of the center of gravity of muscle activity). The results indicate the usefulness of HDsEMG parameters to classify risk levels for both HC and LBP groups and to determine differences between them. The findings suggest that the use of HDsEMG could expand the capabilities of existing instrumental-based tools for biomechanical risk classification during lifting activities. [ABSTRACT FROM AUTHOR]

Background The aim of this study was to investigate the lower limb muscle coactivation and its relationship with muscles spasticity, gait performance, and metabolic cost in patients with hereditary spastic paraparesis. Methods Kinematic, kinetic, electromyographic and energetic parameters of 23 patients and 23 controls were evaluated by computerized gait analysis system. We computed ankle and knee antagonist muscle coactivation indexes throughout the gait cycle and during the subphases of gait. Energy consumption and energy recovery were measured as well. In addition to the correlation analysis between coactivation indexes and clinical variables, correlations between coactivation indexes and time–distance, kinematic, kinetic, and energetic parameters were estimated. Findings Increased coactivity indexes of both knee and ankle muscles throughout the gait cycle and during the subphases of gait were observed in patients compared with controls. Energetic parameters were significantly higher in patients than in controls. Both knee and ankle muscle coactivation indexes were positively correlated with knee and ankle spasticity (Ashworth score), respectively. Knee and ankle muscle coactivation indexes were both positively correlated with energy consumption and both negatively correlated with energy recovery. Interpretation Positive correlations between the Ashworth score and lower limb muscle coactivation suggest that abnormal lower limb muscle coactivation in patients with hereditary spastic paraparesis reflects a primary deficit linked to lower limb spasticity. Furthermore, these abnormalities influence the energetic mechanisms during walking. Identifying excessive muscle coactivation may be helpful in individuating the rehabilitative treatments and designing specific orthosis to restrain spasticity. [ABSTRACT FROM AUTHOR]

BACKGROUND: There are few epidemiological data regarding musculoskeletal disorders (MSDs) in retail industry. Biomechanical risk assessment in ergonomics is commonly performed in retail sector using standardized protocols. However, such protocols have numerous limitations, such as the lack of objectivity or applicability and restrictive conditions. OBJECTIVE: The aim of this study was to analyze one of the most commonly used shelves in vegetable and fruit departments in order to investigate the effect of different shelf levels (i.e. with variations in height and horizontal distance) and load weights on the workers' biomechanical load. METHODS: We investigated trunk, shoulder, elbow, hip, knee and ankle joint ROMs, as well as the mean and peak EMG values of the upper limb, trunk and lower limb muscles. RESULTS: We found that shelf level has a significant effect on most of the parameters examined, whereas within this limited range of 6 and 8 kg, weight does not affect the biomechanical load. We also identified the shelf levels that place the least and most strain on the musculoskeletal system. CONCLUSIONS: We therefore recommend that the height and horizontal distance be carefully considered when shelves are being designed. Low-back disorders (LBDs) are the most common and costly musculoskeletal problem. Muscle co-activation, a mechanism that stabilises the spine, is adopted by the central nervous system to provide added protection and avoid LBDs. However, during high-risk lifting tasks, the compressive load on the spine grows owing to increased co-activation. The aim of this study was to develop a method for the sample-by-sample monitoring of the co-activation of more than two muscles, and to compare this method with agonist–antagonist methods. We propose a time-varying multi-muscle co-activation function that considers electromyographic (EMG) signals as input. EMG data of 10 healthy subjects were recorded while they manually lifted loads at three progressively heavier conditions. The repeated measures ANOVA revealed a significant effect of lifting condition on our co-activation index. Heavier conditions resulted in higher muscle co-activation values. Significant correlations were found between the time-varying multi-muscle co-activation index and other agonist–antagonist methods. Increased antagonist muscle co-activation, seen in motor-impaired individuals, is an attempt by the neuromuscular system to provide mechanical stability by stiffening joints. The aim of this study was to investigate the co-activation pattern of the antagonist muscles of the ankle and knee joints during walking in patients with cerebellar ataxia, a neurological disease that strongly affects stability. Kinematic and electromyographic parameters of gait were recorded in 17 patients and 17 controls. Ankle and knee antagonist muscle co-activation indexes were measured throughout the gait cycle and during the sub-phases of gait. The indexes of ataxic patients were compared with those of controls and correlated with clinical and gait variables. Patients showed increased co-activity indexes of both ankle and knee muscles during the gait cycle as well as during the gait sub-phases. Both knee and ankle muscle co-activation indexes were positively correlated with disease severity, while ankle muscle co-activation was also positively correlated with stance and swing duration variability. Significant negative correlations were observed between the number of self-reported falls per year and knee muscle co-activation. The increased co-activation observed in these cerebellar ataxia patients may represent a compensatory strategy serving to reduce gait instability. Indeed, this mechanism allows patients to reduce the occurrence of falls. The need for this strategy, which results in excessive muscle co-contraction, increased metabolic costs and cartilage degeneration processes, could conceivably be overcome through the use of supportive braces specially designed to provide greater joint stability. An experiment was carried out on supermarket cashiers to evaluate the time, kinematic and electromyographic changes, in both sitting and standing positions, following the redesign of a checkout counter. The novelty of the prototype checkout counter is a disk wheel placed in the bagging area, which is designed to avoid the cashier having to manually push products along the bagging area. The kinematic evaluation was based on the upper limb and trunk range of motions (RoM). The electromyographic parameters assessed were mean and maximum muscular activations. Three factors were taken into account: design (before and after redesign), posture (standing or sitting) and bagging area (anterior or posterior). The results show that the RoM values are lowest after the intervention and in the standing position. Mean and maximum muscular activation patterns are similar. Differences related to the bagging area in which the goods were released also emerged. The disk wheel represents a valid aid for reducing biomechanical overload in cashiers; the standing position is biomechanically more advantageous. Background: Modulation of nociceptive withdrawal reflex (NWR) excitability was evaluated during gait initiation in 10 healthy subjects to investigate how load- and movement-related joint inputs activate lower spinal centres in the transition from quiet stance to walking. A motion analysis system integrated with a surface EMG device was used to acquire kinematic, kinetic and EMG variables. Starting from a quiet stance, subjects were asked to walk forward, at their natural speed. The sural nerve was stimulated and EMG responses were recorded from major hip, knee and ankle muscles. Gait initiation was divided into four subphases based on centre of pressure and centre of mass behaviours, while joint displacements were used to categorise joint motion as flexion or extension. The reflex parameters were measured and compared between subphases and in relation to the joint kinematics. Results: The NWR was found to be subphase-dependent. NWR excitability was increased in the hip and knee flexor muscles of the starting leg, just prior to the occurrence of any movement, and in the knee flexor muscles of the same leg as soon as it was unloaded. The NWR was hip joint kinematics-dependent in a crossed manner. The excitability of the reflex was enhanced in the extensor muscles of the standing leg during the hip flexion of the starting leg, and in the hip flexors of the standing leg during the hip extension of the starting leg. No notable reflex modulation was observed in the ankle muscles. Conclusions: Our findings show that the NWR is modulated during the gait initiation phase. Leg unloading and hip joint motion are the main sources of the observed modulation and work in concert to prepare and assist the starting leg in the first step while supporting the contralateral leg, thereby possibly predisposing the lower limbs to the cyclical pattern of walking. Background: The above-knee amputation of a lower limb is a severe impairment that affects significantly the ability to walk; considering this, a complex adaptation strategy at the neuromuscular level is needed in order to be able to move safely with a prosthetic knee. In literature, it has been demonstrated that muscle activity during walking can be described via the activation of a small set of muscle synergies. The analysis of the composition and the time activation profiles of such synergies have been found to be a valid tool for the description of the motor control schemes in pathological subjects.Methods: In this study, we used muscle synergy analysis techniques to characterize the differences in the modular motor control schemes between a population of 14 people with trans-femoral amputation and 12 healthy subjects walking at two different (slow and normal self-selected) speeds. Muscle synergies were extracted from a 12 lower-limb muscles sEMG recording via non-negative matrix factorization. Equivalence of the synergy vectors was quantified by a cross-validation procedure, while differences in terms of time activation coefficients were evaluated through the analysis of the activity in the different gait sub-phases.Results: Four synergies were able to reconstruct the muscle activity in all subjects. The spatial component of the synergy vectors did not change in all the analysed populations, while differences were present in the activity during the sound limb's stance phase. Main features of people with trans-femoral amputation's muscle synergy recruitment are a prolonged activation of the module composed of calf muscles and an additional activity of the hamstrings' module before and after the prosthetic heel strike.Conclusions: Synergy-based results highlight how, although the complexity and the spatial organization of motor control schemes are the same found in healthy subjects, substantial differences are present in the synergies' recruitment of people with trans femoral amputation. In particular, the most critical task during the gait cycle is the weight transfer from the sound limb to the prosthetic one. Future studies will integrate these results with the dynamics of movement, aiming to a complete neuro-mechanical characterization of people with trans-femoral amputation's walking strategies that can be used to improve the rehabilitation therapies. Abstract: The purposes of this study were to assess: (i) the effects of 8-week training programs with constrained-path and unconstrained-path chest press machines on 1-RM; (ii) the different activity patterns of selected arm and shoulder girdle muscles during push movement performed on the different machines; (iii) the transfer of the training effects from one machine to the other. Twenty healthy, sedentary women (mean±SD age, 24.8±1.0yrs), randomized to either the FM or CM strength training protocols were evaluated before and after the strength training program. Muscular activity signals were recorded by surface electromyography (sEMG) from eight muscles while each subject performed the exercise on each machine. Muscle strength was defined by a 1 repetition maximum (1-RM) test for each subject on each machine. Both machines were effective in improving 1-RM, but the 1-RM increased more in the FM than the CM. Adaptive change in the sEMG was observed in all muscles after training on the FM machine, but not within the stabilizers when training on the CM machine. The results suggest that training in an unconstrained condition provides a more effective method for improving inter-muscular coordination via adaptation of the motor strategy aimed at optimising muscular efforts. Objective To explore whether the trigeminocervical reflexes (TCRs) show a reflex receptive field organization in the brainstem. Methods The facial skin of 16 healthy subjects was electrically stimulated at nine sites reflecting the distribution of the three branches of the trigeminal nerve. The reflex-evoked EMG responses were measured bilaterally from the neck muscles and the head and neck kinematic reactions were detected. Results TCRs are site dependent. There was a vertical gradient in the magnitude of the reflex responses. EMG and kinematic reflexes were larger when evoked from ophthalmic and maxillary sites than from mandibular ones. The reflex responses exhibited a crossed right–left behavior. Stimulation of the lateral sites evoked larger reflex responses in the contralateral trapezium muscle as well as head rotation and neck bending away from the stimulated side. Conclusion This modular arrangement of the TCRs seems to be related to withdrawal strategies aimed at protecting the face from injuries, in accordance with the functional role that each group of muscles plays in head and neck motion. Significance It is likely that the CNS may exploit the neck muscle synergies revealed by the painful stimulation of the skin face in order to control the head and neck movements. Abstract: Objective: To describe the temporal, kinetic, kinematic, electromyographic and energetic aspects of gait in Charcot-Marie-Tooth patients with foot drop and plantar flexion failure. Methods: A sample of 21 patients fulfilling clinical, electrodiagnostic and genetic criteria for Charcot-Marie-Tooth disease were evaluated by computerized gait analysis system and compared to a group of matched healthy subjects. Patients were classified as having isolate foot drop (group 1) and association of foot drop and plantar flexion failure (group 2). Results: While it was impossible to detect a reliable gait pattern when the group of patients was considered as a whole and compared to healthy subjects, we observed two distinctive gait patterns when patients were subdivided as group 1 or 2. Group 1 showed a gait pattern with some characteristics of the “steppage pattern”. The complex motor strategy adopted by this group leads to reduce the swing velocity and to preserve the step length in spite of a high energy consumption. Group 2 displayed a “clumsy pattern” characterized by very slow gait with reduced step length, a broader support area and great reduction in the cadence. This group of patients is characterized by a low energy consumption and greater energy recovery, due above all to the primary deficit and the various compensatory mechanisms. Conclusions: Such between-group differences in gait pattern can be related to both primary motor deficits and secondary compensatory mechanisms. Foot drop and plantar flexion failure affect the overall gait strategy in Charcot-Marie-Tooth patients. [Copyright &y& Elsevier]
