Although disasters can affect anyone in a given community or region, those with access and functional needs have the highest rates of morbidity and mortality during an emergency or disaster. There are many unique and complex issues that should be considered when dealing with these individuals in a disaster situation. Who are these individuals, what specific issues should be addressed when considering these members of the population, and what recommendations can be made in order to address their unique needs? How can we include them as part of the all-hazard, comprehensive approach to disaster management? The first part of this three-part series identifies who is included in this population and what are the legal considerations that arise in caring for not only this unique group but also all of the members of the community in a disaster. The second part considers evacuation, sheltering, sheltering in place, communication, medical needs, independence, supervision, and transportation (CMIST) with a focus on mental health. The third part deals with the medical aspect of CMIST and with recommendations that may aid disaster responders and planners in caring for these high-risk individuals in a disaster.

We discuss key components of disaster planning and management for pediatric patients including emergency medical services, hospital/facility issues, evacuation centers, family separation/reunification, children with special healthcare needs, mental health issues, and overcrowding/surge capacity. Specific policy recommendations and an appendix with detailed practical information and algorithms are included. Pediatric patients are likely victims in a disaster and are more vulnerable in a disaster than adults, yet they have been essentially overlooked in disaster management according to the Pediatric Institute of Medicine Report. We did a needs assessment of Disaster Medical Assistance Teams regarding pediatric issues. Results were as follows: pediatric patients comprise a significant percentage of disaster victims (up to 85% in one disaster), and deficiencies were noted in the curriculum/training/resources. The percentage of time pediatric topics were missing from the curriculum was as follows: airway, 16%; trauma, 33%; disaster triage, 36%; burns, 42%; pain management, 42%; mental health, 45%; patient scenarios, 45%. The percentage of time pediatric equipment was missing was as follows: airway, 16%; intravenous lines, 37%; cervical collars, 38%; medicines, 38%; Broselow tape, 46%; backboards, 62%. Pediatric patients were included in disaster drills 63% of the time. Only 33% had pediatric protocols other than JumpSTART. A need to improve the pediatric components of Disaster Medical Assistance Teams was identified.

The unique characteristics of the pediatric population expose them to errors in the emergency department (ED) with few standard practices for the safety of care. Young children and high-acuity patients are at increased risk of adverse events both in the prehospital and ED settings. We provide an overview of the problems and possible solutions to the threats to pediatric patient safety in the ED. Endorsing a culture of safety and training to work in a team are discussed. Medication errors can be reduced by using organizational systems, and manufacturing and regulatory systems, by educating health care providers, and by providing caregivers tools to monitor prescribing. The consensus is that a safe environment with a high quality of care will reduce morbidity and mortality in ED pediatric patients.
