1. A CASE REPORT OF ACUTE APPENDICITIS PRESENTING WITH DIARRHEA.

Abstract

A 26-year-old female patient presented to the family medicine outpatient clinic with complaints of diarrhea, widespread abdominal pain, nausea, and vomiting, which had been going on for five days. In the first physical examination findings of the patient, there was widespread sensitivity in the abdomen, but there were no findings of defense or rebound. The patient was re-evaluated with the results of conducted tests, her abdominal examination was repeated, and rebound positivity was detected in the right lower quadrant. The patient was referred to the Emergency Department, and she was taken into operation by the general surgery team with the diagnosis of acute appendicitis. The most important factor in delaying the diagnosis of acute appendicitis is the presence of gastroenteritis. Cases of acute appendicitis presenting with diarrhea have been reported in the medical literature. Since acute pathologies should always be kept in mind in the differential diagnosis of patients who present to primary care with abdominal pain and diarrhea, it is deemed appropriate to present the case with literature. Abstract

Objectives: The family physicians are the individuals who constitute the first contact point with patients and take position in primary care. On behalf of family physicians are dominating by a wide range of disease groups, reveal the awareness of the anticoagulant therapy and monitoring necessary. Materials and Methods: Research was planned as an observational, cross sectional survey. Surveys were hand out to family physician residents in Ankara. 143 participants answering the questions of survey without any reserve from assistant family physician, who were invited to our study date between 15/10/2014 and 12/01/2014, were incorporated. Results: Only 8.4% of the residents stated that they attended a training on anticoagulant therapy and follow‐up. We pick out that participants who attend training have more accurate answer to questions which intended to measure awareness and knowledge. Those constituted the majority who has not completed 10 years in their medical profession. The correct answer rates of the group that we call young physicians were higher. Conclusion: With this research, the awareness about anticoagulant therapy and monitoring and lack of education are reveal. With the researches supporting this study, identifying the lacking issues that residents of family physicians' awareness then making education plans intended to these issues and better equipped family physician residents giving medical service will be provided. 3. The Assessment of Knowledge Levels and Practices of Family Physicians Practicing in Family Health Centers in Ankara with Regard to Well Child Follow-up.

Abstract

Objectives: The purpose of our study is to call attention to responsibilities of family physicians regarding well child follow-up, which is very important to improve and support pediatric health, to refresh family physicians' knowledge, and to emphasize the significance and vitality of well child followup. Materials and Methods: The study was planned as an observational and cross-sectional survey study. The survey questions were prepared based on the guideline titled "Periodical Health Examinations and Screening Tests Recommended in Family Medicine Practice", which was produced by the Turkish Ministry of Health and distributed to all family physicians. All family physicians practicing in Family Health Centers (FHC) located in Ankara were sent 4 e-mails containing the link to the survey between December 1st, 2016 and February 2nd, 2017, and invited to participate in the study. Among the family physicians who agreed to participate in the study, 191 physicians who answered the section measuring their knowledge levels in full were included in the study. The overall knowledge level was calculated by scoring each correct answer as 1 point. The maximum score possible was 19 points. Results: The average total score of family physicians was found to be 9.73 ± 2.94 which was relatively low. A positive, weak, and significant correlation was found between knowledge level score and total months worked in FHC. Also, a positive and significant correlation was found between frequency of reading the guideline on well child follow-up and knowledge level score. Further, it was revealed that family physicians mostly performed their daily practices related to well child follow-up. However, it was seen that some non-mandatory follow-up practices were not sufficiently performed. Conclusion: This study showed that it was necessary to provide trainings for family physicians and to encourage them to use guidelines on the subject in order to ensure well child follow-up is at the desired level in primary health care to raise a healthy generation. Abstract

Tezin Amacı: Anne sütü ile beslenme süresine etki eden faktörlerin araştırılması.Materyal ve metot: Aralık 2006-Aralık 2007 tarihleri arasında Fatih Üniversitesi Tıp Fakültesi Hastaneleri Çocuk Polikliniklerine başvuran, 2-4 yaşları arasında çocuğu olan annelere demografik sorular ve emzirme tutumu ile ilgili toplam 32 soru içeren anket uygulandı. Uygun şekilde doldurulan 182 anket değerlendirilmeye alındı.Bulgular: Toplam anne sütü verilme süresinin annenin geceleri çocuğuyla aynı odada kalma süresinin artışıyla ve babanın emzirmeye desteğiyle doğru orantılı, annenin çalışıyor olması ile ters orantılı olduğu bulundu. Toplam anne sütü verilme süresinin sağlık personelinden alınan emzirme eğitiminden, annenin eğitim düzeyinden, çocuğun cinsiyetinden, düzenli gebelik takibinden, bebeğin emzik kullanmasından, doğumdan sonra anne sütüne başlanma süresinden, bebeğin doğum haftasından, doğum şeklinden ve bebeğin doğum ağırlığından etkilenmediği gözlendi.Sonuçlar: Anne sütü verme süresinin artırılmasında annelerin yanında babalara da eğitim verilmesi faydalı olabilir. Anne ve bebeğin en az 2 yaşına kadar geceleri aynı odada kalması teşvik edilebilir, ayrıca çalışan anneler için mesai saatlerinde de bebekleriyle beraber olma süresini artırıcı düzenlemeler yapılabilir. Annelerin yalnızca emzirme yoluyla anne sütü vermelerine yönelik olarak geceleri daha sık emzirmesi önerilebilir. Purpose of the thesis: Factors influencing duration of breastfeeding are investigated.Material and methods: A questionnaire consisting of 32 questions about demographic data and breastfeeding attitudes is performed in mothers with children between 2 to 4 years of age who applied to outpatient pediatrics clinics of the Hospitals of Fatih University, School of Medicine. This study was performed between December 2006 - December 2007. Appropriately answered 182 questionnaires are evaluated.Findings: Total duration of breastfeeding is found to be correlated with the duration of the stay of the child in the same room with mother at night and father?s support for the breastfeeding. It is also found to be inversely correlated with employment of the mother. Total duration of the breastfeeding was independent from breastfeeding education by health staff, mother?s education level, gender of the children, regular follow-up during pregnancy, pacifier using, interval between the birth and starting breastfeeding, gestation week, mode of delivery and birth weight of the children.Results: Our findings suggest that in order to increase the duration of the breastfeeding, educating fathers during education of mothers and promoting the stay of the children with mother in the same room during the first 2 years at nights can help, and regulatory measures allowing mothers with their children in working hours would be helpful. Mothers can be recommended for more frequent breastfeeding at night. 53

Abstract

Gebelik döneminde kaliteli ve düzenli beslenme biçimini öğrenmek çokönemlidir. Anne karnındaki bebeğin gelişimi tamamen anne tarafından karşılandığıiçin anne adayının yedikleri çok değerlidir. Bu tanımlayıcı çalışmada bu kadarönemli ve değerli bir dönem olan gebelik ve gebelikte beslenme alışkanlıklarınındeğerlendirilmesi açısından Mart - Kasım 2011 tarihleri arasında Ankara NumuneEğitim Araştırma Hastanesi Kadın Hastalıkları ve Doğum Kliniği Gebe Polikliniğinebaşvuran gebelere gönüllülük ilkesine dayanarak anket uygulanmıştır.Gebelere genel bilgileri, gebelikle ilişkili bilgileri, anne sütü ile beslenmehakkındaki düşünceleri, beslenme alışkanlıkları ve besin tüketim sıklığı ile ilgilitutumlarını değerlendiren ayrıntılı bir form verilmiştir. Her gebeye anketi nasıldolduracağı anlatılmıştır.Yapılan analizler sonucunda gebelik öncesi BKİ ile gebelik sayısı arasında,eğitim seviyesi ile gelir düzeyi arasında, önceki gebeliğinde yaşadığı sağlık sorunuile kilo alamaması arasında, çocuk sayısı ile ek hastalıklardan sezeryan arasındaistatiksel olarak anlamlı ilişki bulundu.Bulgularımız beslenmenin hayatın her aşamasında önemli olduğu bilincinihastalarımıza aşılayarak, farkındalık seviyesi artmış nesillerin gelişmesininsağlanması gerekliliği yönündedir.Anahtar Kelimeler: Gebelik, Beslenme, Farkındalık Knowledge about the quality and regular diet during pregnancy is veryimportant for mothers-to-be. Because of an unborn baby's developmental needs arefully met by the mother, nutrıtıon during pregnancy is very valuable. This descriptivestudy was performed in the Obstetrics and Gynecology Clinic, in Ankara NumuneTraining and Research Hospital, between March-November 2011. Pregnant womenthat have agreed to participate in this survey were asked to complete a questionnaireabout their nutritional habits.The questionnaire was structured in order to obtain general information,pregnancy-related information, ideas about breastfeeding, nutritional habits andattitudes related to assessing the frequency of food consumption. Each participantwas informed about how to fill in the questionnaire.The results of the study yielded a statistically significant relation between prepregnancy BMI and number of pregnancies, educational level and income level, thepresence of any health care problem during previous pregnancies and inability to getweight, number of children and caesarean section with additional diseases.The pregnant women should be informed about the importance of nutrition inevery stage of life.
