Objective The aim of the present study was to evaluate the 10‐year outcomes of cementless total hip arthroplasty (THA) in adult patients with the late sequelae of septic arthritis of the hip. Methods We followed 166 consecutive patients (168 hips) who underwent cementless THA between March 2001 and December 2011. There were 79 men and 87 women, with a mean age of 50.4 years (range 21–76 years) at the time of index THA, all of whom had hip osteoarthritis secondary to hip pyogenic infection. The average duration of follow up was 10.6 years (range 6.9–17.2 years). Preoperative and postoperative clinical ratings were evaluated, including the hip dysfunction and osteoarthritis outcome score (HOOS), the Harris hip score (HHS), range of motion, a 100‐point visual analog scale for hip pain, and the severity of limp and limb length discrepancy (LLD). The anteroposterior and lateral radiographs of the hip and full‐length view of the lower extremities were obtained to assess the position of the components, radiolucent lines, osteolysis, loosening of components, and heterotopic ossification. The intraoperative and postoperative complications were also recorded. Results The mean HSS and hip pain score were 44.2 points (range 29–66 points) and 42.5 points (range 32–64 points), respectively, before the index surgery and significantly improved to 88.1 points (range 78–96 points) and 15.1 points (range 10–26 points), respectively, at final follow‐up examination. The HOOS and range of motion also improved significantly. The mean limb length discrepancy was reduced from 2.6 to 0.8 cm. The limp at last follow‐up examination was moderate in 3 cases because of hip osteoarthritis in the other limb, slight in 26, and absent in 137. A radiolucent line was observed in 12 hips (7.1%) around the acetabular or femoral components. A progressive radiolucent line around the undersized femoral stem in all zones was seen in 1 hip, resulting in aseptic loosening and breakage of the femoral component. There were 8 cases of intraoperative fracture, 7 cases of dislocation, and 7 cases of transient nerve palsy. Recurrence of infection occurred in 2 hips. Revision surgery was conducted in 2 hips because of isolated loosening of the acetabular cup and the femoral stem, respectively. Kaplan–Meier survival was 97% at 10 years, with revision for any reason with any component as an end‐point. Conclusion Improved surgical techniques and development of components with various sizes provided favorable results for cementless THA conducted for late sequelae of sepsis in these young and active patients. Although the incidence of complications was relatively high, the complications were treated successfully., Favorable results can be obtained from cementless THA for late sequelae of pyogenic arthritis. Although the incidence of complications is relatively high, the complications can be treated successfully.

To assess if the educational level of patients in Southwestern China will affect the functional recovery after total knee arthroplasty (TKA).This retrospective study included a total of 334 patients (48 males, 286 females, with an average age of 68 years, range from 51 to 84 years) who had undergone primary unilateral TKA from March 2017 to April 2018. Patients were screened for enrollment and classified into four groups (illiterate group, the primary school group, high school group, and university group) according to their educational attainment. All patients were monitored for at least 2 years after TKA. The primary outcome was determined using the Hospital for Special Surgery knee (HSS) score at the time of follow-up. The secondary outcomes were determined using the 12-Item Short Form Health Survey (SF-12) and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores, the satisfaction level, and complications of the surgery.Three hundred and thirty-four patients were divided into four groups based on their highest educational level: 83 patients in the illiteracy group, 84 in the primary school group, 91 in the high school group, and 76 in the university group. They were followed up for at least 2 years. For the primary outcome, patients with high school and university education had noteworthy better HSS scores on the surgical-side knee than those in the primary school and illiterate groups (illiteracy group 86.71 ± 5.94 vs primary school group 85.36 ± 5.88 vs high school group 89.48 ± 3.66 vs university group 88.95 ± 3.55; P 0.05). For secondary outcomes, the mental component summary (MCS) in the university group was significantly lower than the other three groups (P 0.05). The results of WOMAC scores were consistent with the results of the HSS score: patients in the university group and the high school group had better results when compared with the other two groups (P 0.05). There were no statistical differences in the comparison of additional indicators and complications among the four groups, but more patients (12 peoples, 15.8%) in the university group were dissatisfied with knee function after TKA.In Southwest China, patients with high school education or above can achieve better joint function after TKA but do not get better postoperative satisfaction, which may be related to the patients' higher surgical expectations for social and mental needs.

Primary pulmonary artery sarcoma is an extremely rare and highly aggressive malignant tumor of cardiovascular system. It is usually misdiagnosed as pulmonary thromboembolism due to its atypical clinical features and similar clinical symptoms. Different from published reports, our case received both enhanced CT and 18F-FDG PET/CT examination before the pathologic result, and lung metastases had already occurred at the time of diagnosis. we herein reported a case of 41-year-old female patient who suffered from cough and chest tightness for more than a month. Laboratory examination indicated that both blood routine and tumor markers were within the normal range, and only the D-dimer slightly elevated. contrast-enhanced chest computed tomography showed right pulmonary artery lesion and multiple nodular located right upper lung, the lesion was mild heterogeneous enhancement. no obvious abnormalities were found in deep vein of bilateral lower extremities on ultrasonography. In order to confirm the nature of these lesions, PET/CT scan was performed, which revealed stripe hypermetabolism in right pulmonary artery and nodular hypermetabolism in right upper lung, and the rest of the whole-body PET imaging were negative, a diagnosis of primary pulmonary artery malignancy with pulmonary metastases was made, and pulmonary thromboembolism was ruled out. biopsy of right pulmonary lesions was performed and histopathological examination indicated pulmonary artery sarcoma. She only received palliative conservative medical treatment because the disease was late stage according to the tumor-node-metastasis (TNM) staging system, and did not acceptable surgical treatment, and was in good health during recent follow-up. Our study suggested that 18F-FDG PET/CT image is a good approach for the diagnosis of pulmonary artery sarcoma and could provide adjunct value for further treatment.

Background: The aim of this study was to compare the analgesic effect on the lateral and overall knee and early rehabilitation between adductor canal block (ACB) and femoral nerve block (FNB) after total-knee arthroplasty. Method: Two hundred patients randomly participated in the study and were divided into the ACB group and FNB group in a randomized manner. All patients received standardized anesthesia and analgesia upon hospitalization. Outcome evaluations included visual analog scale (VAS) scores at rest and during activity, quadriceps strength, range of motion (ROM), total opioid consumption and complication occurrence, sleep interruptions caused by pain, postoperative nausea and vomiting (PONV), and postoperative length of stay (PLOS) before discharge in all groups. In the 90-day postoperative follow-up, we also observed the acute deep periprosthetic joint infection, wound breakdown, readmission, reoperations, inpatient falls, ROM, and patient satisfaction score. Results: The lateral knee VAS scores are lower in the FNB group at rest and during activity (2–24 hours postoperatively) compared with those in the ACB group. However, the overall knee VAS score, total opioid consumption and complication occurrence, sleep interruptions caused by pain, and PONV are similar between the FNB and ACB groups. When evaluating early rehabilitation, the quadriceps strength in the ACB group is superior to that in the FNB group 24 hours postoperatively. At 24, 48, and 72 hours postoperatively, ROM in the ACB group is significantly better than that in the FNB group. Furthermore, the ACB group has a shorter PLOS (4.5 ± 0.60 days) than the FNB group (5.3 ± 0.7 days). However, patient satisfaction score, readmission rate, inpatient falls, acute deep periprosthetic joint infection, and wound breakdown are not statistically significantly different between the 2 groups. Conclusion: The ACB does not relieve lateral knee pain in the early stage but provides similar analgesic effect and better effectiveness of early rehabilitation compared with FNB in patients undergoing TKA.

The cause of hip osteoarthritis (OA) remains unclear, morphologic abnormality of hip was thought to be a contributing factor to hip OA. The hypothesis was that there were subtle anatomical morphology differences of the hip between normal and OA subjects; the objective of this study was to explore these anatomical differences which are predisposing to hip OA based on CT 3D reconstruction. Ninety-three normal subjects (186 hips) and 66 mild-to-moderate hip OA subjects (132 hips) were recruited in this study. Three parameters of the head-neck relationship were assessed: translation, rotation and concavity. Translation was the potential translational movements of femoral head related to the neck’s axis. Rotation was described by the physeal scar to evaluate the rotation tendency of femoral head related to the neck at the head-neck junction. Concavity was used to assess the sphericity of the head as it joins the neck. The femoral neck anteversion angle and some parameters of the acetabulum: anteversion, inclination and CE angle were measured too. By comparison, it was found that OA subjects had less femoral head sphericity, head-neck junction concavity, acetabular and femoral neck anteversion angle; but greater acetabular coverage. These characteristics increased the risk of hip OA in OA subjects.

OBJECTIVE: To determine the rate of curing the infection and mid‐ to long‐term outcomes of using extensively coated non‐modular stems in two‐stage revision for infected total hip arthroplasty (THA). METHODS: The clinical data of 33 patients (33 hips) in whom extensively coated non‐modular stems had been used in two‐stage revision THA for deep infection were retrospectively analyzed. All operations received two‐stage reimplantation, which included resection arthroplasty, thorough debridement, insertion of a hand‐molded antibiotic‐impregnated cement spacer with stainless steel reinforcement, a course of intravenous antibiotics, and delayed reimplantation. Microorganism‐specific antibiotics had been chosen according to the results of microbiological studies performed postoperatively. All patients received i.v. antimicrobial therapy for 4 weeks and oral antibiotics to which their organisms were sensitive for a further 6 weeks. Harris hip score (HHS) and plain X‐ray films were used to perform clinical and radiological evaluations. RESULTS: During follow‐up for a minimum of 5 years, no reinfection or loosening were found. Cultures of samples taken during the second stage were all negative for infection. The mean HHS improved from 42 preoperative to 89 at the final follow‐up. All granular bones had fused well with the host bones by 12 months after the surgery. CONCLUSION: Using extensively coated non‐modular stems combined with intramedullary allografts in two‐stage revision for treating infected THAs can achieve satisfactory outcomes.

To evaluate the outcomes of open high tibial osteotomy through small incision with annular external fixator for treating genu varum deformity.We reviewed 8 cases of open high proximal tibia osteotomy through small incision with annular external fixator for patients (10 knees) with genu varum due to high tibia varum. The patients were followed up 1, 2, 3, 6, 12, 24 and 36 months after the operations. Data about infection, healing, varus angle, knee distance and Insall-Salvati index were retrieved and analysed.The patients had a mean varus angle of 15. 3 degrees +/- 2. 3 degrees (range, 10 degrees - 28 degrees) before the operations. The operations were all recorded as successful following a (29. 3 +/- 3. 1) month (range, 24-36 months) post-operation follow-up, with external fixator remaining stable and no instrument failures. The bones were healed at an average of (11. 2 +/- 1. 5) weeks (range, 8-12 weeks) and there was no nonunion or delayed union. The operations reduced varus angle of the knees to - 1. 1 degrees +/- 0. 6 degrees (range, - 7 degrees - 3 degrees ), indicating a significant improvement compared to that of the preoperations. There were 2 cases of superficial pin infections.Open high tibial osteotomy through small incision with annular external fixator has good fixation stability and satisfactory clinical outcomes with little complications for treating patients with genu varum deformity.

To observe stress distributions around the acetabular prosthesis and the bones of a patient who underwent total hip arthroplasty (THA).Finite element analysis (FEA) was performed with an osteoarthritis patient who underwent THA for her secondary hip high dislocations: Scenario A--deepened acetabulum at the true acetabulum with a small 44 mm cup; Scenario B--structural bone graft at lateral acetabular with a 48 mm cup; Scenario C--place tantalum metal acetabular reconstruction at the lateral acetabular with a 48 mm cup; Scenario D--the normal side of the hip. According to the Wasielewski methods, acetabular was divided into four zones, in the same way on the lining surface. Ten points were taken in each zone for measuring the Von Mises stress values.Scenario A generated significantly greater stress values in the bones in zone one than the other three scenarios. Significantly greater stress was also found in the inner surface of polyethylene over all of the four zones under scenario A compared with those of the scenario B and C, especially in zone one and two. The cup initial micro-mobility for scenario A was 49. 18 microm, 19 times of that of scenario B and 8 times of that of scenario C.(1) Deepened acetabulum with small cup can cause stress concentration in the acetabular bones and liner, leading to large cup initial micro-mobility. (2) Acetabular lateral structural bone grafting and placement of tantalum metal reconstruction have better biomechanical properties, which can enable the use of bigger cups.

OBJECTIVE: To assess the difference between drainage and non‐drainage after total hip arthroplasty (THA) in Chinese subjects by evaluating post‐operative complications and joint function. METHODS: One hundred and sixty‐eight patients undergoing THA were randomly allocated into drainage (83 patients) and non‐drainage groups (85 patients). All surgeries were performed by one surgical team using the same pre‐, intra‐, and postoperative techniques. Measured items included: hemoglobin (Hb), superficial and deep wound infection, volume of blood transfusion, wound hematoma thickness, range of motion (ROM) of the hip, wound healing time, ecchymosis and tension vesicles around the wound. RESULTS: Early after THA, the Hb decreased significantly in the drainage group. There was no significant difference between non‐drainage and drainage groups in need for or volume of blood transfusions (9.6% vs 8.2%, P = 0.100; 3.8 units vs 2.9 units, P = 0.089, respectively). In the non‐drainage group, the incidence of superficial infection, ecchymosis and tension vesicles was significantly higher than in the drainage group (10.6% vs 2.4%, P = 0.031; 12.9% vs 3.6%, P = 0.026; 16.5% vs 4.8%, P = 0.013, respectively). In addition, the non‐drainage group had a greater volume of hematomas (P = 0.000). Patients in the non‐drainage group had smaller ROMs early after surgery but the final ROMs did not differ significantly between groups. No deep infection occurred in either group. CONCLUSION: Non‐drainage may reduce postoperative blood loss but has no benefits regarding blood transfusion or deep infection. It may cause more post‐operative complications because of restriction of early postoperative exercise by pain and swelling. Therefore we suggest routine use of drainage after THA.

OBJECTIVE: To assess whether computed tomography (CT)‐based 3‐dimensional (3D) computerized pre‐operative planning is accurate and reliable in patients with high‐riding dislocation developmental dysplasia of the hip (DDH) undergoing total hip arthroplasty (THA). METHODS: Between September 2009 and February 2011, a prospective study with an inbuilt means of comparing predictive techniques in 20 patients (20 hips) with high‐riding dislocation DDH was undertaken. All patients had pre‐ and post‐operative CT scans, data from which were transferred digitally to Mimics software. 3D pre‐operative planning to predict the acetabular component size, hip rotation center position and acetabular component coverage was performed using Mimics software. The results and post‐operative course were compared with those of the traditional acetate templating technique. RESULTS: Using 3D computerized planning, 14/20 components (70%) were predicted exactly and 6/20 (30%) within one size, whereas with the conventional acetate templating technique, 5/20 components (25%) were predicted exactly, 9/20 (45%) within one size and 6/20 (30%) within two or more sizes. There was a strong correlation between the 3D computerized planned acetabular component size, hip rotation center distance, acetabular component host coverage and that found postoperatively. Five patients were considered to need structural bone graft on the basis of 3D computerized planning; this was highly coincident with the intraoperative findings in all five cases. CONCLUSION: CT‐based 3D computerized pre‐operative planning using Mimics software is an accurate and reliable technique for patients with high‐riding dislocation DDH undergoing THA.

To compare the clinical efficacy and serum concentrations of cobalt, chromium metal ion in three different hard-on-hard bearings after total hip arthroplasty at 2-years postoperatively.Ninety (90) THA patients were divided into ceramic-on-ceramic (COC), ceramic-on-metal (COM), metal-on-metal (MOM) group (n = 30 in each group). At preoperative and 3, 6, 12, 24 months postoperative 5 time points, serum concentrations of cobalt and chromium metal ion were measured, Harris hip score was evaluated, X-rays and color doppler ultrasound examination of the ipsilateral hip also were observed.The excellent rates of Harris hip score were 100% in three groups. Continuous X-rays showed no radiolucent line around the acetabular component, no osteolysis, and no inflammatory pseudotumor. After the THA operation, the metal ion levels in COM and MOM groups increased rapidly, and stabilized at 12 months, then showed a downward trend, but the chromium ion level of MOM continued to rise at 24 months, with a significant difference when compared with that at 12 months (an increase of 0.48 microg/L, P = 0.021). The serum concentrations of metal ion in COC group were relatively constant at all time points, and the cobalt, chromium ion levels of MOM group were significantly higher than those of COC and COM group.The postoperative functional recovery of the three hard-on-hard bearings all were good, and no inflammatory pseudotumor and osteolysis were found. The serum levels of cobalt, chromium ion of COM were lower than those of MOM, but higher than those of COC.

To analyze the impact of hepatitis B virus (HBV) infection on liver function of patients after hematopoietic stem cell transplantation (HSCT), the transplantation outcome of 48 patients infected with HBV prior to transplantation among 185 patients received HSCT was investigated retrospectively. The results showed that during a follow-up for 6 months after HSCT, the alanine aminotransferase (ALT) peak average values of the patients with HBsAg(+), HBsAb(+) and control groups were (281.6 ± 414.6), (95.4 ± 79.9) and (65.1 ± 44.2) U/L, respectively. The incidences of abnormal liver function of the patients with HBsAg(+), HBsAb(+) and control groups were 61.54%, 40.00% and 30.23% respectively. There were no significant differences between any two groups (P0.05). The lethality of those patients at late period after transplantation was not related to HBV infection. The hepatocirrhosis and hepatocarcinoma caused by HBV infection have not become major problems in long-term survivors. It is concluded that in HBsAg(+) patients received HSCT, the damage of liver function is more severe than control group, possibly increasing the development of abnormal liver function. The measures against the liver function damage should be taken. The prophylactic administration of ganciclovir for virus may be effective to prevent the activation of HBV.

Background: It has been reported that the cluster of differentiation 14 (CD14) gene −159C/T variant may be associated with asthma risk. However, some studies yielded conflicting results. Therefore, a comprehensive meta-analysis was designed to assess the precise association. Methods: A systematic search in PubMed, Embase (Ovid), China National Knowledge Internet (CNKI), and Wan fang databases was conducted up to August 15, 2015. Odds ratio (OR) and 95% confidence interval (CI) were used to pool the effect size. We used I2 to assess heterogeneity, and a funnel plot and Egger test to assess publication bias. Results: In total, 34 studies involving 15,641 subjects were included in this meta-analysis. There was a statistically significant association between CD14 −159C/T polymorphism and asthma risk observed in dominant model (TT+TC vs CC: OR = 0.86, 95% CI = 0.77–0.97, P = 0.012) and codominant model (TC vs CC: OR = 0.88, 95% CI = 0.78–0.99, P = 0.035) in adults. However, there may be no significant association between CD14 159C/T and atopic and nonatopic asthma risk. Conclusion: In summary, the overall results suggested that the CD14 −159C/T variant may decrease the risk of asthma susceptibility in adults. However, no significant association between CD14 159C/T and atopic and nonatopic asthma susceptibility was identified. More studies with larger sample size are needed to validate the findings from this study.

To evaluate clinical and radiographic results and influencing factors of hybrid total hip arthroplasty (THA).Totally 126 patients (135 hips) from January 1999 to December 2001 accepted hybrid THA were followed up. Components migration, periprosthetic bone changes, the polyethylene wear rate were measured radiologically. Kaplan-Meier analysis was performed to evaluate the survival of the acetabular and femoral components. End point was obvious radiological loosening or revision either or both of the acetabular and femoral components for aseptic loosening, infection or osteolysis debridement surgery.A total of 79 cases (85 hips) had been given follow-up. Using revision as the end point, the survival rate of acetabular was 95.2%, cemented femoral components was 98.8%. Using loosening as the end point for failure, the survival rate of acetabular was 97.6%, cemented femoral components was 100%.The hybrid total hip prosthesis long-term survival rate was satisfactory, especially in the femoral side. As a result of third-cement technology, the long-term survival rate of the femoral components was close to the modern cementless prosthesis.

OBJECTIVE The objective of the study was to identify adults with symptomatic Kashin-Beck disease (KBD) and observe the efficacy and safety of diclofenac sodium, naproxen, and glucosamine hydrochloride in these adult patients in Rang-tang (Sichuan Province), China. SUBJECTS AND METHODS One hundred eighty-three adult patients with KBD were enrolled into this open study. Patients were randomized to receive diclofenac sodium 50 mg twice a day (BID), naproxen 300 mg BID, or glucosamine hydrochloride 750 mg BID for 6 weeks. The primary efficacy parameters evaluated were the visual analog pain scale, Western Ontario and McMaster Universities Osteoarthritis Index, and physical function subscores. Assessment of daily self-care activities and physician and patient global overall efficacy were also recorded. RESULTS Diclofenac sodium, naproxen, and glucosamine hydrochloride all reduced the joint pain and improved physical function and daily self-care activities in adult patients with KBD. Visual analog pain scale scores, Western Ontario and McMaster Universities Osteoarthritis Index pain scores, physical function scores, and daily self-care activities subscore differences were statistically significant compared with baselines (P < 0.05). Comparison studies among the 3 agents showed no statistically significant difference in efficacy. The incidences of gastrointestinal adverse reactions were 18% and 14% in the diclofenac sodium group and the glucosamine hydrochloride group, respectively, which tended to be lower than the naproxen group (29%). However, the differences were not statistically significant. CONCLUSIONS This report documents characteristic findings in these patients. Diclofenac sodium, naproxen, and glucosamine hydrochloride produced substantial improvements over baseline in pain relief, physical function, and daily self-care activities in these open observations of adult patients with KBD.

To investigate the clinical results of modified Sutherland pelvic osteotomy for developmental dysplasia of hip (DDH).Sutherland pelvic osteotomy were performed in 10 patients (11 hips) with DDH. Among them, there were 3 male (3 hips) and 7 female (8 hips) patients, aged (32 ± 8) years. During operation, arthroscopes were performed additionally to remove the existing hyperplasia tissue in the fossae ovalis and trimming acetabulum and glenoid labrum, thus to insure the better match between the femoral head and the realigned acetabulum. The change of imaging indexes were acquired by comparing the preoperative X-ray with the postoperative X-ray. The change of hip function and life quality were acknowledged according to contrast and analysis Harris hip score and Short Form 12-items Health Survey (SF-12) before and after osteotomy.All patients were followed up for a mean of (5.2 ± 2.3) years, the osteotomy were all union in 3 months. The acetabular head index was 71 ± 8 before operation, and 86 ± 4 after operation. The pre- and post-operative centre edge angle were (7 ± 9)° and (33 ± 9)°, sharp angle were (48 ± 4)° and (37 ± 5)°, acetabular index angle were (24 ± 8)° and (11 ± 5)° respectively. The average Harris score improved from 42 ± 13 preoperatively to 90 ± 5 postoperatively, with 100% excellent and good results. Every domains of SF-12 was improved in the different extents postoperatively, the improvement of physical component summary was more conspicuous than mental component summary. The imaging indexes, Harris and SF-12 were all improved with significant difference (all P0.05).The modified Sutherland pelvic osteotomy is effective. It could increase the load bearing capacity of hip, and improve the quality of life.

A 64-year-old woman presented with right hip pain of 6 years’ duration, accompanied by limping of 1 years’ duration. Preoperative pelvis radiograph showed narrowing of joint space, osteophyte formation, and cystis degeneration of the femoral head surrounded by bony sclerosis, without obvious collapse of the femoral head; the diagnosis was osteoarthrosis. The patient’s blood pressure on admission was 128/76 mm Hg. The patient had no special previous history. Preoperative blood and coagulation tests were normal. Total hip arthroplasty was performed via a posterolateral approach, and intraoperative bleeding was 400 mL. In the first 10 hours postoperatively, the drainage volume decreased slowly, the initial dose of 0.3 mL Fraxiparine was administered, and the blood pressure was 84∼92/45∼56 mm Hg. The drainage volume increased gradually, and 10 hours after administration of Fraxiparine, the drainage volume increased sharply and maintained at a high level. Protamine was applied and the drainage volume decreased sharply, furthermore, a high concentration of endogenous heparinoids were detected in the blood. Although no direct evidence were detected, it was clear that the massive hemorrhage was associated with administration of low-molecular-weight heparin (LMWH). The administration of LMWH combined with continuous low blood pressure caused by surgery and massive bleeding that resulted in low perfusion and inflammation in microcirculation, induced endogenous heparinoid synthesis in endothelial cells, and elevated concentration of endogenous heparinoids in blood led to more severe bleeding. Therefore, caution should be taken with administration of anticoagulant therapy in patients with massive hemorrhage, especially in patients with continuous low blood pressure.

To study the influence of mutation of -1997G--T of COL I A1 gene on the biochemical function of osteoblast, and the pathomechanism of BMD.Spongy bones were obtained to culture osteoblast primarily during total hip or knee replacements. The genotype of osteoblast was identified with PCR-RFLP. The amount of mRNA of COL I A1 and collagen type I were determined by RT-PCR and ELISA. The growth of osteoblast, the activity of bone ALP, and the amount of calcium in cell matrix and calcium nodus were measured.Three genotypes GG, GT and TT in osteoblast were successfully identified. Compared with GG and GT genotypes, lower expression of mRNA of COL I A1 gene, lesser collagen type I, calcium in cell matrix, and calcium nodus were found in the cells with TT genotype (P0.01). No significant differences were found between GG and GT genotype (P0.05). There were no significant differences in age, growth of osteoblast, and activity of bone ALP among the three genotypes (P0.05).Cells with TT genotype have low expression of mRNA of COL I A1 gene and less collagen type I , calcium in cell matrix and calcium nodus. The lower amount of collagen type I synthesized by osteoblast can decrease the matrix outside the bone cells and result in insufficient site for calcium deposition. This may be the cause of lower BMD in patients with TT genotype.

To study the changes of biomechanical properties and microarchitecture in callus during the healing process of osteoporotic fracture.Ovariectomy (OVX) was performed on SD rats to establish osteoporotic rat model. Osteotomy was performed on both groups of rats with OVX and sham operations to establish bone fracture. Bone mineral density (BMD), area of type I collagen, biomechanical properties, and microarchitecture of the fractured bones during healing process were measured.The BMD of both groups of rats were significantly lower at the 12th week than at the 6th week after fracture (P0.05). Both groups of rats had significantly higher biomechanical strength and area of type I collagen at the 12th week than at the 6th week after fracture (P0.05). The OVX group of rats had significantly lower structure model index (SMI) values than the sham group at both 6th and 12th week after fracture (P0.05). Higher values of SMI appeared at the 12th week than at the 6th week in both groups of rats (P0.05).During the healing of fracture, the BMD of the newly-formed bone reaches its peak value at the 6th week after fracture, which is earlier than the occurrence of peak biomechanical strength, area of type I collagen, and SMI. Although the collagen and aggravation of mineral determine the biomechanical properties, BMD by itself may not be able to determine the biomechanical strength because of the change of the 3-D structure of the callus. The combination of collagen and microarchitecture may act as a factor that determines the biomechanical strength of callus independent of BMD.

To retrospectively analyze the clinical and radiological results of the impaction morselized allografting combined with cementless long stem for femoral component revision in patients with significant bone loss.From July 2003 to June 2009, 27 patients (27 hips) underwent revision hip arthroplasty in femurs with bone defect using impaction allografting and cementless components. There were 15 male and 12 female with mean age of 67 years. According to the Paprosky classification system, the bone defects in femur were classified into type II in 3 cases, type III in 21 cases and type IV 3 cases. The patients were followed up postoperatively to observe the clinical and radiological results, and if there had the stem subsidence, periprosthetic fracture, infection and other complications.Twenty-three patients were followed up. The mean follow-up time was 26 months. Harris score improved from 43 points pre-revision to 83 points at the final follow-up. No femoral stem loosening and failure occurred. There were 4 stem with a mean 3.3 mm subsidence. One case had an acute infection after revision and treated successfully by debridement and drainage, antibiotics. No stem need further revision. Radiographic analysis showed all the revised stem was stable, and graft incorporation was seen in at least 1 zone in 100% of the femurs.The results of this study demonstrates that femoral support and vigorous impaction grafting combined with cementless long stem results in an good intermediated-term clinical effects.

To analyze the results of cortical windowing of the femoral diaphysis for well-fixed cement/plug removal during hip revision surgery.From May 2005 to June 2009, 14 patients (14 hips) were undergone revision total hip arthroplasty (THA), window was cut into the cortex of the femur, and the well-fixed cement/plug distal to the window was removed under the direct vision. After reimplanted the cementless revision stem, the cortical lids were replanted and fixed with 2 to 3 cerclage wires. Six patients who had suffered from osteoporosis were undergone morselized bone graft to the osteotomy site. Postoperatively, the patients were maintained at partial weight-bearing (touchdown) for 6 weeks and then advanced as they were able.The length of the cortical windows varied from 2.5 to 6.0 cm (mean, 3.4 cm), the width ranged from 0.8 to 1.4 cm (mean, 1.2 cm). In one patient the window was enlarged during the procedure to facilitate the cement/plug removal. The mean radiologically healing time for the windows was 19 weeks. There was no intraoperative femoral perforation during cement/plug removal. One femoral fracture during the revision stem was implanted. No postoperative periprosthetic fracture and other complications such as infection, implant subsidence occurred during the fellow-up. There was no femoral thigh pain or implant loosening with femoral window.The cortical windowing technique is very helpful to facilitate the well-fixed cement/plug distal to the prosthesis tip removal and the windows heal rapidly and decrease the femoral complications associated with revision THA.

To analyze the clinical and radiographic results of the extended trochanteric osteotomy in revision total hip arthroplasty procedure.The data of 27 patients (27 hips) of the clinical and radiographic results were reviewed between January 1998 and June 2005. There were 16 men and 11 women, and the mean age at the time of revision surgery was 63 years (range 42 to 78 years). Indications for use of the trochanteric osteotomy were removal of well-fixed femoral implants. Clinical evaluation included Harris and WOMAC scores. The radiological evaluation included stem subsidence, trochanteric fragment fractures and stem position change.Nineteen patients (19 hips) were followed up for an average of 5.3 years. All osteotomy sites healed by 16 months,with an average time to union of 16 weeks. No intraoperative fracture at the osteotomy site occurred. There was only one dislocation postoperatively. Three femoral components were subsidence at a mean of 3.4 mm (range 3-7 mm) in the first 6 weeks postoperatively; It united within 6 months without further migration.The extended trochanteric osteotomy facilitates the removal of well-fixed femoral implants, allows reliable reattachment of the trochanteric fragment and results an excellent rate of healing and implant stability in revision total hip arthroplasty during mid-term follow-up. Long-term follow-up is needed.

To assess the effect of perioperative administration of a selective cyclooxygenase 2 inhibitor (celecoxib) on pain management and recovery of function after total knee arthroplasty (TKA).Randomized, controlled trial conducted from January 2005 through February 2006, 60 patients underwent TKA for osteoarthritis or rheumatoid arthritis were randomly divided into group of perioperative, administration of celecoxib (Study group, n = 30) and postoperative administration of celecoxib (Control group, n = 30). Patients in Study group were given oral celecoxib 3 d before TKA, 200 mg twice daily, and extended to 5 d postoperatively; patients in Control group were given oral celecoxib 2 h after TKA, 200 mg twice daily, and extended to 5 d postoperatively. All operations were finished by the same surgeon group.The postoperative patient-controlled analgesia (PCA) consumption was significantly less in Study group than in Control group [(43 +/- 12) ml vs. (53 +/- 12) ml, P0.05]. The pain scores of postoperative 4, 8, 12 h, 1, 2 d in Study group were 6.1 +/- 1.2, 5.0 +/- 1.3, 4.3 +/- 1.1, 3.4 +/- 1.2, significantly less than in Control group (P0.05); There were no intergroup significant differences in the pain scores of postoperative 3, 4, 5 d (P0.05). There were no intergroup significant differences in respect to the side-effect occurrence, operation time and postoperative drainage, postoperative analgesic consumption (P0.05). The time to achieve 90 degrees knee flexion was significantly shorter in Study group than in Control group [(6.2 +/- 1.7) d vs. (8.6 +/- 1.8) d, P0.05].Perioperative administration of the selective Celecoxib holds the effect of preemptive analgesia. Compared with postoperative administration, perioperative administration of celecoxib can alleviate the early postoperative pain score, reduce the consumption of postoperative analgesic, accelerate the recovery of joint motion and thus increase the patient satisfaction.

To evaluate the patients with bone injury in Wenchuan earthquake.From May 12th to June 15th 2008 the data of 1410 patients with bone injury in Wenchuan earthquake were analyzed to evaluate clinical intervention and remedy-managing experience.The 1410 patients average age was from 4 to 103 years old. And 744 cases (52.7%) suffered from blunt injuries, 379 cases (26.9%) from buried injuries, 287 cases (20.4%) from falling injuries; And 1317 cases were with fracture, 93 with limbs soft tissue injuries; 261 patients combined with other parts of injuries including 45 cases with paralysis; 66 cases were with crush syndrome, 25 with gas gangrene, 76 with acute kidney failure, 26 with multiple organ failure. And 912 operations were performed including 402 fracture fixation, 224 debridement, 152 debridement and suture, 85 amputation, 29 implant skin, 8 fixation of joint dislocation, 5 surgical flaps transplantation, 4 nerve and tendon suture, 2 arthroscopes, 1 joint replacement. Among the 66 crush syndrome patients, 49 accepted continuous renal replacement therapy, in which 9 cases were bleeding from named arteries and 20 blood vessels were getting embolism. Among the 1410 cases, 1 died from multiple organ failure.Among the patients with bone injury in Wenchuan earthquake, the elderly patients are more than the youth; The injuries are always combined with other complications; Opened injuries are polluted severely; It is difficult to deal with the crush syndrome; Paraplegia cases are less, but the amputees are more.

To investigate the clinical manifestations and risk factors of the patients from developmental dysplasia of the hip(DDH) family.Detailed epidemiology investigation, physical examination, functional movement assessment, lab test and X-ray examination were applied to the whole members of a DDH family.In the family with 9 generations and 218 persons, the incidence of DDH was 31.03% in 145 survivors. Patients mainly manifested bilateral knee and hip joint pain, flexion contracture of hip, limitation in internal and external rotation of hip; a few had arthritic functional disorder, deformation, and limp. The radiography illustrated shallow acetabulum with increased inclination, which encompassed the femoral head badly. Deformation of the femoral head, narrow joint space and osteophyte were also found by X-ray examination. The main risk factors of DDH were genetic factors, gender, birth season etc. The son or daughter with one or two DDH parents had a higher risk for developing DDH than those with no DDH parents. Furthermore, first-degree relatives of the DDH patients also had a greater chance to develop DDH than second-degree relatives and third-degree relatives. The incidence among females was higher than males, and the family member who was given birth in winter had a highest risk for developing DDH. However, there was no difference between incidence of DDH in children and youths and in adults; the incidence of DDH in the immigrants with no blood relationship also did not differ from the incidence of DDH in the family member.The genetic factors play an important role in the development of DDH, so do the environmental factors.

Ceramic-on-ceramic hip replacements might stress the bone interface more than a metal-polyethylene because of material stiffness, microseparation, and sensitivity to impingement. To ascertain whether this potentially increased stress caused an increased cup migration we compared a ceramic-on-ceramic with a metal-on-polyethylene implant for cup migration. Sixty one patients (61 hips) undergoing THA for osteoarthritis were randomized to ceramic on ceramic (Ce/Ce) or cobalt-chromium on cross-linked polyethylene bearings (PE) in the same uncemented cup shell. Migration was followed with RSA. At 2 years we observed similar mean cup translations in the 3 directions (0.07-0.40 mm vs. 0.05-0.31 mm, Ce/Ce vs. PE), as well as similar rotations around the 3 axes (0.31-0.92 degrees vs. 0.57-1.40 degrees). WOMAC and SF-36 scores were also similar and no radiolucent lines or osteolysis found. The large migration seen in some cups in both implant groups will require close monitoring to ascertain the reasons. Mean proximal wear of the polyethylene liners measured 0.016 mm between 2 and 24 months. Our data suggest there is no increased cup migration in the ceramic-on-ceramic implant compared with the metal-on-polyethylene, and they seem an equally safe choice. However, the low wear measured with the more versatile and less expensive cross-linked polyethylene makes it a strong contender.Therapeutic Level I. To observe clinical results for reconstruction of periprosthetic fractures of hip with cortical bone plates allografts by deep-freezing and ethylene oxide treatment.Seven patients with periprosthetic fractures of hip underwent cortical bone plates allografts by deep-freezing at -70 degrees C after being treatment of 48 degrees C ethylene oxide. And evaluate clinical outcome by examining T lymphocytes, Harris scores, X-rays photograph, and bone scintigraphy.There were not activity of immune rejection and infection in all patients. Harris scores of patients increased 21, 32, 40, 40 scores at 3, 6, 12, 24 months after surgery. T-lymphocytes, antibody and immunocomplex in blood was normal postoperation. X-ray film indicated that fracture was healed at 3 months and there was partially bone conjunction between allograft strut and host bone. There was incorporation of 85% allograft strut to host bone, and 15% allograft strut was partially absorbed at 12 months after surgery. The size of femur of host was added 3 mm to 5 mm, averaged 4.3 mm at 12 months postoperation. Density of 80% allograft plates was as same as host bone after remodeling and the absorbtion of 10% allograft plates stopped at 24 months after surgery. There was thick of nuclein in the area of allograft cortical bone plates by bone scintigraphy examination at 3 months postoperation, and the thick of nuclein was stronger at 6, and 12 months after surgery.Allograft cortical bone plates by deep frozen at -70 degrees C after being treatment of 48 degrees C ethylene oxide is suitable for mechanical fixation and biological bone transplantation, and it can increase bone reservation, augment strength of femur once the allograft strut incorporates to host bone, and avoid removing metal implant in second operation when being applied into reconstruction femoral fracture in joint replacement.

To observe the cell apoptosis after tractive spinal cord injury in rats, determine expression of apoptosis correlative genes, and study the molecular mechanism of cell apoptosis.The T(13)-L(2) spinal cord of rats was injured by traction after the amplitude of P1-N1 wave decreased to 70% in postoperation than in preoperation through cortical somatosensory evoked potential (CSEP) monitor. Then rats were killed in 30 min, 6 h, 1, 4, 7, 14 and 21 d respectively after operation (n = 4). Cell apoptosis was examined by the flow cytometer and terminal deoxynucleotidyl transferase-mediated DUTP-biotin nick end labeling (TUNEL) reaction, the expression of p53, bax and bc1-2 genes was tested with immunohistochemistry.The flow cytometer test and TUNEL method showed that the apoptosis cell ratio raised in 6 h and reached at peak in 7 d after injury, and then declined till 21 d, they showed significant difference (P0.05, 0.01). TUNEL method showed that injured group had a large number of apoptosis glial cells in white matter. Immunohistochemical staining showed that the positive expression of p53, bax and bc1-2 protein raised at 6 h, expression of p53 protein reached at peak in 4 d, bax and bc1-2 protein reached at peak in 7 d after injury. Compared with control group and laminectomy group, the injured group showed significant difference (P0.05, 0.01).There is cell apoptosis phenomenon after tractive spinal cord injury in rats. Morphology indicates that apoptosis includes neurons and glialcytes, which is an important form of cell death and pathological changes in secondary lesion period after tractive spinal cord. There exist high expression of apoptosis correlative gene p53 and bax after spinal cord injury, they may play an important role in reduction of cells to apoptosis.

To establish an animal model of tractive spinal cord injury in rats in order to investigate its pathophysiological changes and clinical significance.T(12)-L(3) spines were tracted longitudinally with a special spinal retractor that was put on the proccessus transverses of T(12)-L(3) vertebrae of the rat after exposing T(13)-L(2) spinal cord via dual laminectomy. At the same tine, the spinal cord function was monitored by cortical somatosensory evoked potential (CSEP). Rats were randomly divided into four groups according to the amplitude of CSEP P(1)-N(1) wave, the amount of the decreasing P(1)-N(1) wave was 30% (the 30% group), 50% (the 50% group) and 70% (the 70% group), respectively. After traction, the changes of the neural behavioral function in rats were observed and the morphological structure of the spinal cord was analyzed quantitatively with image analysis system of computer.With traction of spine, compared with the control group, the 30% group had no marked difference in combined behavioral score (CBS), neuron count, section area of neuron and Nissl body density, but the 50% and 70% groups had marked difference (P0.01). Light microscope showed that the neuron volume was slightly small and the Nissl body was reduced lightly in the 30% group; the neuron space was enlarged and the neuron was degenerative, reductive, and dissolved, and the spinal cord structure was destroyed in the 50% and 70% groups.The animal model of tractive spinal cord injury in rats is a reproducible, graded and clinic mimic. The model in this article provides a valuable assistance in further understanding etiopathology and screening effective measures of therapy and prophylaxis of the injury.
