Recently the dogma that there is an inverse linear association between androgen receptor (AR) CAG and GGN polymorphisms and receptor activity has been challenged. We analysed the pattern of association between 21 male reproductive phenotypes and AR CAG/GGN repeat lengths in 557 proven-fertile men. A linear association was only found between sperm DNA fragmentation index (DFI) and CAG length, and between inhibin B and GGN length. Men with longer CAG then the reference (22-24), had higher oestradiol levels, whereas men with shorter CAG stretches had a higher DFI and a higher proportion of Fas-positive germ cells. Subjects with either short or long CAG had increased seminal levels of prostate-specific antigen and neutral α-glucosidase activity. Compared to men with the median GGN length of 23, those with shorter GGN repeats had higher levels of inhibin B, higher proportions of normal and progressive sperm, and a higher fraction of Fas-positive sperm, while men with longer GGN had higher oestradiol levels. These data indicate that at least for some markers of male reproductive function the association with CAG or GGN repeat length is curvilinear., (Copyright © 2013 Elsevier Ireland Ltd. Background: Recent studies indicate that not only women's but also men's obesity has adverse effects on fecundity and since fecundity is a couple concept, we examined fecundity in relation to overweight and obesity of the couple. We also examined the association between weight changes and fecundity over time., Methods: Between 1996 and 2002, 64 167 pregnant women enrolled in the Danish National Birth Cohort were interviewed during and 18 months after pregnancy. Information on body mass index (BMI) and waiting time to pregnancy (TTP) was available for 47 835 couples., Results: Among men and women with a BMI of 18.5 kg/m(2) or more, we found a dose-response relationship between increasing BMI group and subfecundity (a TTP of more than 12 months): Odds ratio (OR) = 1.32 (95% CI: 1.26-1.37) for women and OR = 1.19 (95% CI: 1.14-1.24) for men. Among 2374 women with an initial BMI of 18.5 kg/m(2) or more, who participated more than once in the Danish National Birth Cohort, each kilogram increment in weight between the two pregnancies was associated with a 2.84 (95% CI: 1.33-4.35) days longer TTP., Conclusions: Couples have a high risk of being subfecund if they are both obese.

Background: One in seven married couples is involuntarily infertile. Several chemical exposures in the work environment have been hypothesized to affect female reproduction, and some are present in products used in hairdressing and related trades. Recent Swedish findings indicate that employment in hairdressing poses a risk for female reproductive function. This study examined the possible association between work as a hairdresser and subsequent hospital contact due to female infertility., Methods: A cohort of all women in Denmark aged 20-44 years on 1 January 1998 (baseline) and registered as economically active hairdressers, according to national registers, was formed to calculate age-standardized risk ratios (RRs) for hospital contacts due to female infertility during a 5-year follow-up period. Hairdressers were compared to a standard population, that is, all economically active women in Denmark aged 20-44 years at baseline, and to women working as shop assistants., Results: Sixty-eight cases of hospital contact due to female infertility were observed among the female hairdressers. On the basis of the standard population, the expected number was 73.27, which gives an observed RR of 0.928 (95% CI: 0.72-1.18). Hairdressers and shop assistants exhibited similar rates of hospital contact due to female infertility (1.01; 95% CI: 0.77-1.29)., Conclusion: The findings are not corroborating the hypothesis that hairdressers are at increased risk of infertility, but small risks in the entire group or high risks in small subgroups may not be detected by the study.

Background: Persistent organochlorine pollutants (POP) may affect both the female and male reproductive system in animals as well as in humans., Methods: Blood samples were collected from pregnant women and their partners from Greenland, Warsaw and Kharkiv, and from a cohort of Swedish fishermen's wives. Blood samples were analysed for 2,2',4,4',5,5'-hexachlorobiphenyl (CB-153) and 1,1-dichloro-2,2-bis (p-chlorophenyl)-ethylene (p,p'-DDE). Information on the participants' fertility, measured as time to pregnancy (TTP), was collected. In total, 778 men and 1505 women were included in the analyses., Results: The data from Warsaw, Kharkiv and the Swedish fishermen's wives indicated no effect of either male or female exposure to POP on TTP. However, among men and women from Greenland, there seemed to be an association between serum concentrations of CB-153 and p,p'-DDE and prolonged TTP. Due to the strong intra-individual correlation between CB-153 and p,p'-DDE in the Greenlandic population, it was not possible to determine whether the risk was associated with CB-153 or p, p'-DDE or was an interaction between the two compounds., Conclusions: The overall results of the present study create a somewhat ambiguous pattern, but give some support to the idea that dietary POP exposure might be harmful for couple fertility.

The predictive value of sperm motility parameters obtained by computer-assisted semen analysis (CASA) was evaluated for the fertility of men from general population. In a prospective study with couples stopping use of contraception in order to try to conceive, CASA was performed on semen samples from 358 men. A recently developed CASA system, Copenhagen Rigshospitalet Image house sperm Motility Analysis System (CRISMAS) was used for assessment of motility parameters. This system has an editing function which allows correction of tracks made by the computer. Probably due to this function, the concentration assessment made by CRISMAS was very close to that made by the technician (median difference <5%) in all concentration ranges. Correlation between CASA parameters and fertility of normal couples (measured as probability of achieving pregnancy) was examined by the Cox regression model. In univariate models ln(sperm concentration) [beta = 0.331, risk ratio (RR) = 1.392, P = 0.0001], ln(total sperm count) (beta = 0.252, RR = 1.286, P = 0.0007) and percentage motile spermatozoa (beta = 0.014, RR = 1.014, P = 0.0004) were most significant predictors for fertility. In a multivariate analysis ln(sperm concentration) (beta = 0.268, RR = 1.307, P = 0.0016) and percentage motile spermatozoa (beta = 0.010, RR = 1.010, P = 0.011) but even more significantly the combined parameter, ln(concentration of motile spermatozoa) (beta = 0.329, RR = 1.389, P = 0.0001), were the only parameters of predictive value for fertility of men in the general population. In conclusion, these parameters obtained by CASA measurements can be used for prediction of fertility potential in normal men. This appears to be the first study showing the value of CASA in prediction of fertility in the general male population.

Objectives: This study examined the possibility that work in greenhouses with potential exposure to pesticides entails a risk for reduced fecundity in terms of increased time to pregnancy., Methods: Among 1767 female members of the Danish Gardeners Trade Union, telephone interview data were obtained on the 492 most recent pregnancies of women employed when they stopped contraception to get a child (the starting time). The pregnancies were classified according to job characteristics at the starting time. The ratio between the likelihood of pregnancy during a month for the exposed persons versus the referents (the fecundability ratio) was estimated by discrete proportional hazards regression., Results: The adjusted fecundability ratio for workers in flower greenhouses versus other union members was 1.11 [95% confidence interval (95% CI) 0.90-1.36]. Among workers in flower greenhouses the handling of cultures many hours per week, the spraying of pesticides, and the nonuse of gloves was related to reduced fecundability [adjusted fecundability ratio 0.69 (95% CI 0.47-1.03), 0.78 (95% CI 0.59-1.06), and 0.67 (95% CI 0.46-0.98), respectively]., Conclusions: The findings suggest that female workers in flower greenhouses may have reduced fecundability and that exposure to pesticides may be part of the causal chain. Additional studies of fertility among women working in greenhouses are highly warranted.

Research into environmental causes of impaired human fertility is still immature. Knowledge on the strengths and limitations of several research options, the main sources of bias, the occurrence of infertility and subfecundity in several countries, the patterns of contraceptive behavior, and the distribution of sperm counts at the population level has advanced, but the much-debated questions about temporal and regional trends in fertility and semen quality remain unsettled. Refined biological markers of male fecundity and markers feasible for large-scale studies of female fecundity are needed. The effects of several life-style factors have been characterized successfully, but the detrimental effects of environmental exposures have not. Most studies are either inconclusive or fail to demonstrate any important risks. If comprehensive knowledge on biological mechanisms is a criterion of maturity, epidemiologic and toxicologic fertility research is young. The new millennium will be challenged to determine the role played by the environment in developed countries, wrong exposure time windows, too crude or inappropriate measures of fertility, and overlooked critical exposures.

The aim of this study was to examine time to pregnancy among farmers who used pesticides (traditional farmers) and farmers who did not (organic farmers). A total of 904 men, selected from the Danish Ministry of Agriculture lists of traditional and organic farmers, participated in telephone interviews. The participation rate was 84%. Information was collected on "time to pregnancy" for the youngest child, exposure to pesticides and potential confounders. Using the discrete analogue of the Cox Regression Model (including potential confounders: male and female smoking, female age, parity and contraceptive method), the fecundability ratio between traditional farmers using pesticides and organic farmers was 1.03 (95% CI: 0.75-1.40). In the group of farmers who sprayed with pesticides, none of the characteristics related to the use of pesticides could account for the variation in time to pregnancy. No overall effect of pesticides on male fecundability could be demonstrated in this retrospective study among Danish farmers.

Objective: To assess the effect of psychological distress on time to first pregnancy., Design: A follow-up study of time to pregnancy with prospective data on distress, with controlling for potential confounding variables., Setting: Two university hospitals., Patient(s): Danish couples (n = 430) who were planning their first pregnancy and had no previous reproductive experience were followed for six menstrual cycles. Psychological distress was measured in each menstrual cycle by the General Health Questionnaire., Intervention(s): None., Main Outcome Measure(s): A clinically recognized pregnancy or a biochemical pregnancy detected in urine samples from each period of vaginal bleeding., Result(s): For cycles with the highest distress score (General Health Questionnaire score >80th percentile), the probability of conception per cycle was 12.8%, compared with 16.5% in other cycles (adjusted odds ratio [OR] 0.6; 95% confidence interval [CI] 0.4-1.0). The effect of distress was found almost exclusively among women with long menstrual cycles (OR 0.1; 95% CI 0.01-0.4 and OR 0.9; 95% CI 0.5-1.4 for women with cycles of > or =35 and <35 days, respectively). An increased incidence of early embryonal loss was also found among highly distressed women with long cycles, but was based on a small number of observations., Conclusion(s): Psychological distress may be a risk factor for reduced fertility in women with long menstrual cycles.

We performed a follow-up study of time to pregnancy in a population of first-time pregnancy planners without previous reproductive experience. The objective of this paper is to report and discuss a finding of a strong relationship between glycosylated haemoglobin (HbA1C) and fertility. A total of 165 Danish couples without previous reproductive experience was followed from termination of birth control until pregnancy for a maximum of six menstrual cycles. HbA1C and reproductive hormones were measured at entry. Questionnaire data were collected at entry and once in each cycle during follow-up. The odds ratio (OR) for pregnancy per cycle decreased with increasing concentration of HbA1C (OR per percent HbA1C 0.4, 95% CI 0.2-0.9 for all six cycles and 0.2, 95% CI 0.1-0.5 in the first three cycles). A high concentration of HbA1C was associated with a high concentration of testosterone and a low concentration of inhibin A. No association was found between HbA1C and psychosocial distress. The reduced fertility among women with high HbA1C may be due to an association with subclinical polycystic ovaries as indicated by the hormonal profile.

Objectives: To evaluate the impact of extremely low frequency (ELF) magnetic fields on markers of human fertility., Methods: A follow up study of time to pregnancy, semen quality, and reproductive hormones was conducted among couples planning first pregnancies (36 males were welders and 21 were non-welders). The male and the female partner were monitored for exposure to ELF magnetic field by personal exposure meters. As summary measures of exposure the median value was calculated together with the 75 percentile value and the proportion of measurements exceeding 0.2 and 1.0 microT, respectively. Each summary measure was divided in three categories: low, medium, and high., Results: Couples in which the man had a medium or a high proportion of measurements > 1.0 microT had a reduced probability of conception per menstrual cycle compared with the men with low exposure, but the result was only significant for the men with medium exposure. This finding was partly attributable to a high probability of conception per menstrual cycle among the men with low exposure compared with non-welders in the original cohort of 430 couples. Other summary measures were not related to probability of conception or other markers of fertility. A possible negative association was found between high probability of conception and female exposure measures based on the median, the 75 percentile, and the proportion of measurements > 0.2 microT, but no association was found with the proportion of measurements > 1.0 microT., Conclusions: The findings provide no consistent support for a hypothesis of a deleterious effect of low level ELF magnetic fields on markers of human fertility. However, due to the relatively small size of the studied population only large associations would be detected.

Objective: To characterize how the menstrual cycle pattern relates to fertility regardless of potential biases caused by inappropriate coital timing during the menstrual cycle or early embryonal loss., Design: Prospective follow-up study., Setting: Healthy couples recruited throughout Denmark., Patient(s): Two hundred ninety-five couples who were planning their first pregnancy were followed up from the discontinuation of birth control until a pregnancy was recognized within six menstrual cycles. Early embryonal losses were detected by changes in urinary hCG levels., Intervention(s): None., Main Outcome Measure(s): The probability of pregnancy occurring within one menstrual cycle (fecundity)., Result(s): In women who had a cycle length that differed by >10 days from the usual cycle length, fecundity was approximately 25% that of women who had no variation (odds ratio 0.25, 95% confidence interval 0.09-0.68). When the combined effect of cycle variation and cycle length was assessed, cycle variation was a persistent strong predictor of fecundity., Conclusion(s): The mechanisms of the present findings probably are female functional disturbances in ovulation, conception, implantation, or sustained pregnancy, linked with variable menstrual cycle length. Thus, identification of medical and environmental causes of abnormal menstrual cycle patterns may provide clues to the causes of infertility. Moreover, the menstrual cycle pattern also should be taken into consideration in the clinical decision-making process.

The objective of the studies was to evaluate infertility according to sperm count shifts. The distribution of the sperm count of 1024 Danish men (median 56 million/ml) served as reference. The data were transformed with multiplicative or additive models to create alternative distributions with median sperm count values changed by 25-100%. Sperm-count-specific fecundabilities were provided from a follow-up of first-pregnancy planners in a Danish population. The estimated average fecundability of the 1024 Danish men was 16.9% [95% confidence interval (95% CI) 16.7-17.2], and the proportion of cohabiting men with spouses pregnant within 1 year was 86.0% (95% CI 84.1-87.8). Simulations of alternative sperm count distributions indicated that the relationship between sperm count shift and fertility strongly depends on the median level of the sperm count at onset and the type of shift, a dramatic decline from a high level in a multiplicative model indicating a marginal change and a minor decline from a low level in an additive model representing a strong decrease in fertility. In some cases sperm count, therefore, may be an early warning of changes in fertility.

During 1992-1995, 430 Danish couples were recruited after a nationwide mailing of a letter to 52,255 trade union members who were 20-35 years old, lived with a partner, and had no children. The couples were enrolled into the study when they discontinued birth control, and they were followed for six menstrual cycles or until a clinically recognized pregnancy. At enrollment and each month throughout the follow-up, both partners completed a questionnaire that asked them about their smoking, alcohol consumption, and intake of caffeinated beverages. The effect of current smoking and smoking exposure in utero was evaluated by using a logistic regression model with pregnancy outcome of each cycle in a Cox discrete model calculating the fecundability odds ratio. After adjustment for female body mass index and alcohol intake, diseases in female reproductive organs, semen quality, and duration of menstrual cycle, the fecundability odds ratio for smoking women exposed in utero was 0.53 (95% confidence interval (CI) 0.31-0.91) compared with unexposed nonsmokers. Fecundability odds ratio for nonsmoking women exposed in utero was 0.70 (95% CI 0.48-1.03) and that for female smokers not exposed in utero was 0.67 (95% CI 0.42-1.06). Exposure in utero was also associated with a decreased fecundability odds ratio in males (0.68, 95% CI 0.48-0.97), whereas present smoking did not reduce fecundability significantly. It seems advisable to encourage smoking cessation prior to the attempt to conceive as well as during pregnancy.

Background: Semen analysis is part of the routine assessment of infertile couples. WHO defines a sperm concentration above 20x10(6) per mL seminal fluid as normal. We studied the association between semen quality and the probability of conception in a single menstrual cycle in Danish couples with no previous reproductive experience., Methods: In 1992-94, we invited 52,255 trades-union members aged 20-35 years, who lived with a partner and had no children to take part in the study; 430 couples agreed. The couples discontinued use of contraception, and were followed up for six menstrual cycles or until a pregnancy was verified within this period. Each man was asked to provide a semen sample at enrolment (which was analysed without freezing). Women kept a daily record of vaginal bleeding and sexual activity. The association between semen quality and likelihood of pregnancy was assessed by logistic regression, adjusted for sexual activity and female factors associated with low fertility., Results: There were 256 (59.5%) pregnancies among the 430 couples: 165 (65.0%) among those with a sperm concentration of 40x10(6)/mL or more and 84 (51.2%) among those with lower sperm concentrations. The probability of conception increased with increasing sperm concentration up to 40x10(6)/mL, but any higher sperm density was not associated with additional likelihood of pregnancy. The proportion of sperm with normal morphology was strongly related to likelihood of pregnancy independently of sperm concentration. Semen volume and motility were of limited value in pregnancy prediction., Interpretation: Our study suggests that the current WHO guidelines for normal semen quality should be used with caution. Some men with sperm counts above the lower limit of the normal range defined by WHO may in fact be subfertile.

Objectives: The association between fertility and job strain defined as high job demands and low job control has not previously been studied. A follow-up study was conducted with prospective collection of information on job strain among women, achievement of pregnancy, and potential confounding variables., Methods: A total of 297 Danish couples without previous reproductive experience was followed for a maximum of 6 menstrual cycles from termination of birth control until pregnancy. Job demand and job control were measured by a questionnaire developed by Karasek and his co-workers., Results: The odds ratio and 95% confidence interval (95% CI) for conception per menstrual cycle for women with high job strain was 0.9 (95% CI 0.5-1.5) when compared with that of women in low-strain jobs. Only in secondary analyses restricted to couples with no suspected competitive causes of reduced fertility was a statistically significant reduced odds found for women with high-strain jobs compared with all other jobs., Conclusions: The main finding of this study did not corroborate a hypothesis of a substantial detrimental effect of job strain on fecundability.

Fecundability has been defined as the ability to achieve a recognized pregnancy. Several studies on caffeine and fecundability have been conducted but have been inconclusive. This may be explained partly by lack of stratification by smoking. Furthermore, few researchers have tried to separate the effect of caffeine from different sources (coffee, tea, cola, and chocolate). Clearly, the relationship between caffeine and fecundability needs further research, given the high prevalence of caffeine intake among women of childbearing age. We examined the independent and combined effects of smoking and caffeine intake from different sources on the probability of conception. From 1992 to 1995, a total of 430 couples were recruited after a nationwide mailing of a personal letter to 52,255 trade union members who were 20 to 35 years old, lived with a partner, and had no previous reproductive experience. At enrollment and in six cycles of follow-up, both partners filled out a questionnaire on different factors including smoking habits and their intake of coffee, tea, chocolate, cola beverages, and chocolate bars. In all, 1596 cycles and 423 couples were included in the analyses. The cycle-specific association between caffeine intake and fecundability was analyzed in a logistic regression model with the outcome at each cycle (pregnant or not pregnant) in a Cox discrete model calculating the fecundability odds-ratio (FR). Compared to nonsmoking women with caffeine intake less than 300 mg/d, nonsmoking women who consumed 300 to 700 mg/d caffeine had a FR of 0.88 [95% confidence interval (CI) 0.60-1.31], whereas women with a higher caffeine intake had a FR = 0.63 (95% CI 0.25-1.60) after adjusting for female body mass index and alcohol intake, diseases of the female reproductive organs, semen quality, and duration of menstrual cycle. No dose-response relationship was found among smokers. Among males, the same decline in point estimates of the FR was present. Smoking women whose only source of caffeine was coffee (>300 mg/d) had a reduced fecundability odds-ratio (FR = 0.34; 95% CI 0.12-0.98). An interaction between caffeine and smoking is biologically plausible, and the lack of effect among smokers may be due to faster metabolism of caffeine. Our findings suggest that especially nonsmoking women who wish to achieve a pregnancy might benefit from a reduced caffeine intake.

This paper presents design and methods of a follow-up study of fecundability (defined as the probability of conception during one menstrual cycle) in first-pregnancy planners. We recruited 430 couples by postal letter among 52,255 members of four trade unions. Couples without earlier reproductive experience were enrolled when they discontinued contraception and were followed in six complete menstrual cycles or until a pregnancy was recognized. Data included daily recordings of coitus and menstrual bleeding. Blood, semen, and urine specimens were collected to assess endocrine profile, semen quality, occult embryonal loss, and occupational exposures. Information was obtained about 1661 menstrual cycles (94% of expected). The proportion of couples obtaining a clinical pregnancy during 6 months of follow-up was 65%, and the average probability to obtain a pregnancy across all cycles was 16% (95% CI 14-18%). Among all conceptions the proportion of early embryonal loss was 17% (95% CI 12-22%). This study demonstrates the feasibility of a prospective study of fertility and that the established study base can provide useful information on environmental risk to subfertility.

The aim of this study was to estimate whether male welding has an impact on couple fecundability (the probability of conceiving in a menstrual cycle). A sample of Danish couples without previous reproductive experience was recruited nationwide by postal letters to members of the union of metal workers and three other trade unions. Among 430 included couples, 201 males were metal workers and 130 were welders. The couples were followed for a maximum of six menstrual cycles from termination of birth control until a clinical pregnancy was detected. Compared with nonwelding metal workers the fecundability odds ratio (OR) of male exposure to welding was 0.86 (95% confidence interval [CI] 0.58-1.28). An interaction between male smoking and welding was found; within smokers the OR for welding was 0.40 (95% CI 0.17-0.95) and within nonsmokers it was 1.22 (95% CI 0.74-1.99). Previous welding exposure was negatively related to fecundability among smokers (OR 0.84 per year with mild steel welding, OR 0.76 per year with stainless steel welding). No significant results were found when comparing with an external group of nonmetal workers. Decreased fecundability among smoking welders attributable to both current and previous welding exposure is possible, but these findings were the results of subanalyses that were not part of the a priori hypothesis.

Background: Inorganic lead may impair male fecundity through its action on the germinal epithelium, the endocrine system or both, but information on possible impact on fertility in exposed populations is limited., Methods: Fertility was examined in 1349 male battery plant employees and in 9596 reference company employees over a follow-up of 25,949 and 183,414 person years respectively. The cohorts were identified by records in a national pension fund and information on births was obtained from the Danish Population Register. In a subset of the battery worker cohort, the average level of lead in blood was 35.9 micrograms/dl (4639 blood samples on 1654 person years in 400 workers; SD 13.0; range 3-125). The birth rate was analysed by logistic regression on occupational exposure and several extraneous determinants (age, parity, calendar year, and previous children)., Results: The birth-rate was not reduced in years at risk from exposure to lead in comparison with years not-at-risk; either in comparison with not-at-risk years within the battery plant cohort (odds ratio [OR] = 0.997, 95% confidence interval [CI]: 0.88-1.13), or in comparison with the external reference population (OR = 0.983, 95% CI: 0.87-1.11)., Conclusions: Inorganic lead seems not to impair fertility among Danish battery workers. This finding does not rule out that the time taken to achieve a pregnancy is increased among battery workers because most pregnancies in Denmark are planned.

These studies were initiated by the results of two Danish investigations of infertility clients, which indicated the reduced fecundity of male metal welders. The objective was to refute or corroborate the effects of welding on male reproductive capability and--if there was any effect--to identify the causal exposures. The initial hypothesis postulated reduced spermatogenesis, spontaneous abortion, congenital malformation and childhood malignancy following exposure to hexavalent chromium among stainless steel welders. Subsequently, a hypothesis concerned with the significance of exposure to radiant heat on reduced semen quality was put forward. These studies comprised a case-referent study of infertility, cross-sectional and longitudinal studies of semen quality and historical cohort studies of fertility, pregnancy outcome and cancer in offspring. Exposure to welding was reported with a higher frequency during periods of infertility than prior to conception in the case-referent study (OR 2.0, 95% CI 1.0-4.0). This finding is consistent with the main cross-sectional study showing reduced semen quality in welders [average reduction ranging from 8% (sperm penetration rate in eggwhite) to 28% (total sperm count)] and with the cohort study revealing reduced fertility in relation to welding (OR 0.89, 95% CI 0.83-0.97). However, reduced semen quality and fertility were not attributable to the welding of stainless steel but to the welding of mild steel; and no relationship was found between biological measures of exposure to chromium and parameters of semen quality. If the unexpected association between mild steel welding and reduced fecundity is causal, the biological mechanisms involved are obscure. A separate longitudinal study leaves little doubt that moderate radiant heat exposure may cause reversible deterioration of semen quality, but it is not justified to generalize this observation to the entire population of welders. Male-mediated effects on occurrence of congenital malformation and cancer in offspring from stainless steel welding are not indicated by the studies. Weak indications of an increased risk of spontaneous abortion among partners to stainless steel welders (OR 1.9, 95% CI 1.1-3.2) need to be refuted or corroborated in future studies. Suggested effects of mild steel welding on male fecundity should be corroborated by longitudinal controlled studies of semen quality examined before and during exposure and by prospective studies of fecundability in couples trying to conceive a child. On account of the present knowledge it is not possible to recommend rational preventative measures with the exception of elimination of radiant heat exposure in cases of infertility.(ABSTRACT TRUNCATED AT 400 WORDS)

During the past 15 years, approximately 50 occupational medical sperm quality investigations have been carried out in the world as a whole. The discovery of reduced testicular function among workers exposed to the chemical agent dibromochloropropane (DBCP) was an important incitment for the conduct of these investigations. These have not demonstrated new occupational medical influences with as dramatic an effect as DBCP but moderately reduced sperm quality has, however, been proved or suspected after occupational exposure to a series of other agents: certain cell poisons (ethylene dibromide, carbaryl, chlordecone), certain glycoethers (in paint, glue, printing inks, antifreeze solutions), certain organic solvents e.g. styrene (plastic casting), choroprene (plastic production), low exposure to lead, metal welding, thermal influences and high frequent electromagnetic fields (300 kHz-300 mHz). Only a few investigations illustrate the significance of the male factors for infertility and delay before deliberate pregnancy and there are still no well-proved examples of human paternal teratogenic agents or carcinogens. Our present knowledge only serves to prevent a limited proportion of reproductory failure in men. Reports of decreased sperm quality in the population and the influence of the environment on reproduction in domestic animals indicate that further investigations are necessary. Longitudinal investigations of sperm quality together with investigations of fertility or delay till deliberate pregnancy are proposed subjects for future strategy.

The aim of this study was to determine whether there was a relation between male exposure to pesticides and the amount of time needed to conceive (time to pregnancy) for farmers and agricultural workers in France and Denmark. The authors used retrospective studies to compare the time to pregnancy of couples in which the man was exposed to pesticides during the year before the birth of their youngest child with that of couples in which the man was not exposed. In 1995 and 1996, the authors studied 362 French rural workers (142 exposed to pesticides and 220 not exposed), 449 Danish farmers (326 conventional farmers exposed to pesticides and 123 nonexposed organic farmers), and 121 Danish greenhouse workers exposed to pesticides. The fecundability ratio for exposure to pesticides (Cox model, before and after adjustment for confounding factors) did not differ from 1 in any of the three populations. In France, the adjusted fecundability ratio was 1.17 (95% confidence interval (CI) 0.89-1.55) for exposed and nonexposed agricultural workers. In Denmark, it was 1.09 (95% CI 0.82-1.43) for exposed and nonexposed farmers and 0.83 (95% CI 0.69-1.18) for greenhouse workers and nonexposed farmers. Background: There is circumstantial evidence that human sperm count may have declined during past decades. The purpose of this study was to identify the association between semen quality and year of birth.Methods: The study comprised 8608 men consulting four Danish medical centres from 1968 to 1992 because of infertility. Data on semen quality and urogenital disorders were obtained from medical records while lifestyle data were collected from a subset of the population by a postal questionnaire (response 80%). Semen characteristics were analysed as a linear function of year of birth, centre, season and calendar year at time of semen examination, sexual abstinence and lifestyle factors. Effects of age were accounted for by restriction and stratified analysis.Results: The sperm count declined with increasing year of birth at two of the four centres, but this association disappeared when confounders were adjusted for. Within the subset of men born 1950-1970 we revealed a decrease in the average sperm count by 1.9 mill/ml (95% confidence interval [CI]: 1.45, 2.27) per one advancing year of birth. This finding was consistent across centres even after adjustment for effects of covariates. The proportion of morphologically normal sperm cells changed in parallel with the sperm count, while semen volume did not decline in any time periods.Conclusions: We found a birth cohort effect on sperm count and morphology among Danish infertile men born after 1950 but not in men born in the first part of the century.
