This study was planned as an open-label, single-centre trial with blinded evaluations by two independent radiologists, aimed at the intra-individual comparison of single-dose and double-dose Gd-DTPA-enhanced MRA in the renal arterial territory. Ten healthy volunteers were included in the study. Renal MRAs were carried out on a clinical 1.5-T MR system using a body phased-array coil. Images were acquired with three-dimensional fast spoiled gradient echo sequence. Contrast agent was injected with a power injector keeping the injection time constant for single dose and double dose. Both readers found at least 96% of vascular segments evaluable. Median overall image quality was rated excellent, and diagnostic confidence was rated confident. No statistically significant difference between the dosage groups could be demonstrated. Signal intensity, SNR and CNR were significantly higher for the double-dose group. Our results demonstrate that while a double dose of contrast agent increases SNR, it does not lead to further improvement in visual and perceptual image quality. A single dosage of approximately 0.1 mmol/kg bw Gd-DTPA may be the preferable dosage to demonstrate the renal arteries.

Purpose: To compare the intraindividual differences of single-dose (D(S)) and double-dose (D(D)) gadolinium diethylenetriamine pentaacetic acid (Gd-DTPA)-enhanced magnetic resonance angiography (MRA) in the carotid arterial territory, an open-label single-center trial with blinded evaluations by two independent radiologists was undertaken., Materials and Methods: A total of 11 healthy volunteers were included in the study. Carotid MRAs were carried out in a 1.5 T clinical scanner using a neurovascular phased-array coil. Images were acquired with three-dimensional (3D) fast spoiled gradient recalled echo (FSPGR) sequence. Contrast agent was injected with a power injector, keeping the injection time constant for D(S) and D(D)., Results: Both readers found at least 97% of vascular segments evaluable. Median overall image quality and diagnostic confidence were rated excellent and confident to very confident. No statistically significant difference between the dosage groups could be demonstrated. Variability based on a region of interest (ROI) signal intensity (SI) measurement was 7% for the D(S) and 5% for the D(D) group. SI, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR) were significantly higher for the D(D) group., Conclusion: Our results demonstrate that while a double dosing of contrast agent does increase SNR, it does not lead to further improvement in visual and perceptual image quality. A single dosage of approximately 0.1 mmol/kg body weight (bw) Gd-DTPA may be the preferable dosage in the carotid arteries.

Question: The availability of new MR contrast agents having either a protein binding effect or higher concentration leads to the question if they differ from standard compounds in their vascular contrasting properties., Methods: By intraindividual comparison five volunteers were examined (1.5 T MR system) with a multi-phasic MRA of the abdomen each receiving three different contrast agents (gadopentetate dimeglumine 0.5 M; gadobenate dimeglumine 0.5 M, and gadobutrol 1.0 M). The dose (0.15 mmol/kg body weight) and flow rate (3.0 ml/s) were kept identical. All images were quantitatively and qualitatively evaluated by blinded assessment., Results: Significantly higher maximum signal intensity was found in the arteries for the protein interacting gadobenate dimeglumine (p = 0.05). No significant difference in vascular enhancement was demonstrated in the comparison of gadobutrol to gadopentetate dimeglumine., Conclusion: Gadobenate dimeglumine was shown to be a favorable contrast agent for multi-phasic MRA. A higher concentrated Gd-chelate does not automatically lead to improved vascular contrast if standard imaging protocols are used.

Purpose: To assess the influence of the degree of contrast medium extravasation on different DSC EPI MR sequences for perfusion MR imaging., Material and Methods: 60 patients with cerebral gliomas were examined by either an FID EPI or an SE EPI DSC MR sequence. The acquired images were evaluated on a qualitative and quantitative basis. For qualitative assessment, the homogeneity of the signal time curve, image artifacts, the degree of signal drop and the degree of enhancement were evaluated. The quantitative assessment included the percentage of signal drop and the contrast-to-noise ratio of the different EPI sequences was analyzed., Results: FID EPI presented a more homogeneous signal time curve and a more pronounced susceptibility effect than the SE EPI sequence. Due to the lesser susceptibility effect, the SE EPI sequence was not as sensitive to contrast media extravasation. The signal returned to baseline in all patients. In patients with strongly enhancing lesions, the FID EPI sequence suffered from considerable T1 effects, causing problems in the quantification of perfusion data., Conclusion: FID EPI sequences were preferred for perfusion MR imaging in patients without strong enhancing lesions, e.g. in ischemia or tumors with intact blood-brain barrier. In patients with suspected strong enhancing lesions, an SE EPI sequence should be used.

Aim: Aim of this study was to demonstrate and compare different quantification techniques to assess contrast enhancement in dynamic MRI studies. The diagnostic potential of dynamic MRI studies is increasingly appreciated and already used in different organ systems., Method: A patient population of 314 histologically verified breast lesions (138 malignant, 176 benign) were evaluated using a high temporal resolved dynamic sequence. Different quantification techniques such as the use of a cutoff line, time dependent and pharmacokinetic assessment were comparatively evaluated., Results: Time dependent quantification methods revealed higher diagnostic potential which was further improved by in vivo normalization of the contrast availability in the vascular system. Significant differences in the enhancement characteristics were determined between malignant and benign as well within the different histological entities., Conclusion: Time dependent quantification methods enable an angiogenic characterization of lesions to improve diagnostic interpretation as well as monitoring during therapy. They are also the basis for automated, color-coded visualization of dynamic studies.

The purposes of the study was to assess intra-axial brain tumors by a blinded comparison of gadobenat-dimeglumine and Gd-DTPA. 27 patients with known cerebral gliomas or metastases were included into an intra-individual randomized double-blinded cross-over study. The protocol included T1 SE, T2 FSE and after contrast a series of five T1 SE sequences followed by T1 SE with MT, T1 SE, and 3D GRE sequences. Imaging data acquired at two centers were assessed on-site by the investigators and off-site by two experienced readers using quantitative and qualitative criteria. For a quantitative analysis tumor contrast and contrast-to-noise ratios were determined out of ROI in tumor, unaffected white matter, a region outside the head, and an external reference tube. For the qualitative assessment on- and off-site readers were asked to compare both MR scans for lesion contrast, lesion delineation and information upon the internal morphology and structure. In the quantitative analysis lesions examined with gadobenat-dimeglumine present a maximal 26% increase of the lesion contrast. In both, the on-site, as well as the off-site assessment the intensity of enhancement and the lesion contrast were found to be significantly better with gadobenat-dimeglumine enhanced MRI. There was a trend towards gadobenat-dimeglumine for the delineation of the lesion from the surrounding tissue and the internal lesion morphology. Based on our observations gadobenat-dimeglumine proved to be a safe and valuable contrast media for the assessment of CNS neoplasms. Compared with Gd-DTPA it provides a more intense contrast enhancement and a better tumor contrast which might be of importance for the further management of these patients.

Gd-DTPA kinetics in arterial blood was investigated by dynamic MRI in 47 patients with malignant and benign mammary tumors. Signal enhancement was monitored for 10 min after the beginning of a 1-min infusion of 0.1 mmol/kg Gd-DTPA. Kinetics in blood was biexponential with median half-lives of 21 sec and 11.1 min, respectively. Peak signal enhancement and the area under the signal enhancement-time curve varied 2.5- and 3.7-fold between patients. The shortest mean residence time in one of up to three tumor compartments, MRT*, was estimated using either the individual (reference) or a mean population (surrogate) arterial input function (AIF). MRT* (reference estimate) was 1.0 (0-1.5), 1.9 (1.5-2.3), and 2.5 (2.3-2.8) min in carcinomas, fibroadenomas, and mastopathies, respectively (median and interquartile distance). Surrogate estimates were unbiased but differed from the reference estimates 1.5-fold or more in 23% of cases. Background and Purpose: Reconstruction of first-pass bolus information to derive regional cerebral blood volume (rCBV) maps is commonly performed in many centers; however, various protocols with different doses of paramagnetic contrast injections have been reported. We evaluated the dose dependency of rCBV maps in a brain tumor population by using three different doses of gadodiamide injection to evaluate their diagnostic accuracy in blinded reader sessions., Methods: Eighty-three patients with intraaxial brain tumors (72 gliomas) were studied at three centers and randomized to receive a bolus injection of 0.1, 0.2, or 0.3 mmol/kg per body weight of gadodiamide. rCBV maps were generated from T2*-weighted gradient-echo echoplanar sequences at 1.5 T. Data processing was performed according to the indicator dilution theory., Results: The mean contrast-to-noise ratio (CNR) was significantly different between gadodiamide doses of 0.1 and 0.2 mmol/kg (CNR = 8.7 and 15.7) and between 0.1 and 0.3 mmol/kg (CNR = 17.7). No significant difference was found between doses of 0.2 and 0.3 mmol/kg. Sensitivity for the differentiation of benign and malignant brain tumors was 80%, 95%, and 91%, and specificity was 45%, 54%, and 43% by blinded readings at 0.1, 0.2, and 0.3 mmol/ kg, respectively, as compared with histologic findings. Nonblinded readings had a sensitivity of 83%, 100%, and 90% and a specificity of 82%, 100%, and 73% at 0.1, 0.2, and 0.3 mmol/kg, respectively., Conclusion: A dose of 0.2 mmol/kg of gadodiamide is recommended for reconstruction of rCBV maps if data are acquired with the T2*-weighted protocol described.

We describe a standard set of quantity names and symbols related to the estimation of kinetic parameters from dynamic contrast-enhanced T(1)-weighted magnetic resonance imaging data, using diffusable agents such as gadopentetate dimeglumine (Gd-DTPA). These include a) the volume transfer constant K(trans) (min(-1)); b) the volume of extravascular extracellular space (EES) per unit volume of tissue v(e) (0 < v(e) < 1); and c) the flux rate constant between EES and plasma k(ep) (min(-1)). The rate constant is the ratio of the transfer constant to the EES (k(ep) = K(trans)/v(e)). Under flow-limited conditions K(trans) equals the blood plasma flow per unit volume of tissue; under permeability-limited conditions K(trans) equals the permeability surface area product per unit volume of tissue. We relate these quantities to previously published work from our groups; our future publications will refer to these standardized terms, and we propose that these be adopted as international standards.

The blood-tissue exchange kinetics of gadopentetate were studied in 49 malignant and benign mammary tumors. Signal enhancement was monitored simultaneously in the aorta and in tumor for 10.5 minutes after the beginning of a 1 minute i.v. infusion of the contrast medium (CM). Kinetic analysis was based on a model with two compartments for systemic pharmacokinetics and up to three kinetically distinct compartments for tumor. Kinetic heterogeneity, ie, two or more compartments with different exchange rate constants in a given tumor, was found in 85% of carcinomas, 38% of fibroadenomas, and 14% of mastopathic tumors. The within-tumor average of CM exchange rates was 1.22 (0.62-1.65) min(-1) in carcinomas, 0.38 (0.26-0.60) min(-1) in fibroadenomas, and 0.16 (0. 12-0.20) min(-1) in mastopathies (median and interquartile distances). The area under the signal enhancement-time curve of the aorta varied 4.5-fold between individuals. It is concluded that individual CM kinetics in arterial blood should be taken into account when CM exchange rates between blood and tumor are to be determined and that a kinetic model for potentially malignant tumors should allow for kinetic heterogeneity.

While the diagnostic benefits of gadolinium (Gd)-chelate contrast agents are firmly established in magnetic resonance imaging (MRI) of tumors, the pathophysiologic basis of the enhancement observed and its histopathologic correlate remained vague. Tumor angiogenesis is fundamental for growth and metastasis and also of interest in new therapeutic concepts. By correlative analysis of a) histology; b) vascular density (CD31); and c) vascular permeability (vascular permeability factor/vascular endothelial growth factor [VPF/VEGF]), we found a) significantly (P < 0.001) faster exchange rates in malignant compared with benign breast lesions; b) distinct differences in enhancement characteristics between the histologic types (invasive ductal carcinoma, invasive lobular carcinoma, and ductal carcinoma in situ); and c) dependence of enhancement kinetics on the VPF/VEGF expression. The pathophysiologic basis for the differences in contrast enhancement patterns of tumors detectable by MRI is mainly due to vascular permeability, which leads to more characteristic differences than vascular density. MRI is able to subclassify malignant breast tumors due to their different angiogenetic properties.

Magnetic resonance imaging (MRI) is a noninvasive technique that complements computed tomography (CT), conventional X-ray, and bone marrow biopsies by sampling a large volume of musculoskeletal bone and providing information that aids the diagnosis, staging, and follow-up of various lesions. Although less sensitive to the mineral components of bones, the MRI appearance of physiologic bone marrow is mainly a reflection of the relative amounts of red marrow, yellow marrow, and trabecular bone. Therefore, use of T1-and T2-weighted MR sequences with or without fat suppression currently remains the most common approach to musculoskeletal bone lesion imaging. An additional imaging strategy to characterize various bone lesions is the application of contrast-enhanced dynamic MRI. This article examines semiquantitative and quantitative dynamic imaging, evaluation, and postprocessing techniques in various benign and malignant musculoskeletal lesions. Practical guidelines for performing a dynamic contrast-enhanced MR examination are proposed.

Purpose: To compare two different three-dimensional (3D) gadolinium-enhanced magnetic resonance (MR) angiographic techniques., Materials and Methods: In 26 patients suspected of having renal artery stenosis, results with fast multiphase 3D MR angiography were compared to those with standard 3D MR angiography in 37 patients. With both techniques, 31-second breath-hold acquisitions were performed. Multiphase angiography comprised five discrete 6.4-second acquisitions without bolus timing, and standard angiography comprised a single acquisition based on test-bolus timing. Two readers evaluated images obtained with both techniques in terms of image quality, artifacts, and vessel conspicuity. Accuracy of findings on the multiphase 3D MR angiograms for assessment of renal artery stenosis was determined by comparing them to digital subtraction angiograms and surgical findings., Results: In the early arterial phase, multiphase 3D MR angiograms showed no image degradation by venous overlay, whereas standard 3D MR angiograms depicted at least minor overlay in 53 of 83 renal arteries (P < .001). Less parenchymal enhancement in the early arterial phase resulted in a higher vessel conspicuity for the divisions and segmental arteries (P < .001). Both readers detected and correctly graded 18 of 20 stenoses on the multiphase angiograms with almost perfect interobserver agreement (kappa > 0.89)., Conclusion: Renal multiphase 3D MR angiography is an accurate technique requiring no bolus timing. The performance of early arterial phase imaging leads to improved depiction, particularly of the distal renovascular tree, compared to that with standard single-phase 3D MR angiography.

Purpose: To investigate multiphase (arterial, portal venous, and late venous phases) breath-hold three-dimensional (3D) gadolinium-enhanced magnetic resonance (MR) angiography for the detection and functional characterization of hepatic lesions., Materials and Methods: Breath-hold fast spoiled gradient-echo 3D gadolinium-enhanced MR angiography was performed in 18 patients with 35 hepatic lesions. Measurements of signal intensity were obtained for 27 seconds in each phase, with 23-second delays between the three phases. Lesion-liver visibilities at each phase on the MR angiographic, precontrast T1-weighted, T2-weighted, and postcontrast T1-weighted images were compared. The MR angiographic functional lesion characterization was based on the combined assessment of spatial variations and the evolution of contrast material enhancement in all three phases., Results: All 35 lesions were correctly characterized on the MR angiographic images, which is significantly (P < .01) better than the precontrast T1-weighted (n = 14 [40%]), T2-weighted (n = 23 [66%]), and postcontrast T1-weighted (n = 25 [71%]) imaging results. Analysis of the spatial variations and the evolution of contrast material enhancement significantly (P < .01) improved lesion characterization in 66% (23 of 35) of all lesions., Conclusion: Multiphase breath-hold 3D gadolinium-enhanced MR angiography is feasible and robust and significantly improves the morphologic detection of benign or malignant lesions during the early arterial phase. It further improves the functional characterization of hepatic lesions, combining an arterial, portal-venous, and late MR angiographic phase of contrast enhancement.

Purpose: The aim of this pilot study was to evaluate a magnetically labeled water perfusion imaging technique as a non-contrast-enhanced approach to demonstrate the uterine artery, its branches, and to assess the cervical uterine blood flow in healthy volunteers and in patients with advanced uterine cervical carcinoma (FIGO IIB-IVA)., Methods and Materials: Seven healthy volunteers (mean age, 29 years) and twenty-two patients (mean age, 52 years) with advanced cancer of the uterine cervix (FIGO IIB-IVA) were prospectively examined by magnetically labeled water perfusion imaging at different inversion delay times (300-900 ms). The magnetic resonance imaging (MRI) findings of all patients were matched to the findings of contrast-enhanced dynamic MRI and multiple biopsies (n = 5) and/or surgical whole mount specimens (n = 17), which were available in all patients., Results: The uterine artery was well visualized with short inversion delay times of 300-500 ms. It was characterized as single or multiple helical loops before dividing into its intracervical branches. The intracervical branching was observed at inversion delay times of 500-700 ms. With longer inversion delay times, arterial signal enhancement disappeared and cervical tissue enhancement was noted. Enhancement of benign tissue was observed at inversion delay times of 1100-1700 ms and in malignant tissue at shorter inversion delay times of 900-1300 ms. The maximum of this diffuse signal enhancement of benign tissue was seen at inversion delay times of 1500 ms (1100-1700 ms) in malignant tissue at significantly (p < 0.5) shorter inversion delay times of 1100 ms (900-1300 ms)., Conclusion: Our preliminary results show that the vascular supply and blood flow of the normal uterine cervix and of advanced cervical cancer can be assessed by magnetically labeled water perfusion imaging and that malignant cervical tissue is earlier and stronger perfused than normal cervical tissue.
