Purpose: Those hospitalised with coronavirus disease 2019 (COVID-19) have recently been shown to have impaired right ventricular (RV) strain, but data about the course of heart function after discharge are limited. Our aim was to compare right ventricular strain and right atrial reservoir strain (RASr) associated with COVID-19 between acute disease (during hospitalisation) and follow-up (after discharge). Methods: In this retrospective single-center study, we analysed the echocardiograms of 43 patients hospitalised for non-severe COVID-19 between December 2020 and March 2021, undergoing echocardiography both during and after hospitalisation. In addition to conventional echocardiographic parameters, we applied 2-dimensional speckle tracking to obtain RV global longitudinal strain (RV-GLS), RV free wall strain (RV-FWS), and RASr. Results: Mean (standard deviation) age of the study population was 50 (9) years, and 18 (42%) of the participants were women. Median duration between exams was 6 months (range, 5-7 months). Both mean RV-GLS and mean RV-FWS significantly increased at follow-up (-20.8 [3.8] vs. -23.5 [2.8], p < 0.001 and -23.3 [4.2] vs. -28.2 [2.8], p < 0.001; respectively), and RASr significantly improved as well (-32.3 [6.6] vs. -41.9 [9.8], p < 0.001). Conclusion: In patients hospitalised for non-severe COVID-19 pneumonia, RV-GLS, RV-FWS, and RASr improved significantly between acute disease and 6 months after discharge. 35670268

Prediction of hospital mortality in patients with COVID-19 by the CHA2DS2VASc (M-CHA2DS2VASc) has been recently shown. Because COVID-19 patients with acute cardiac injury have higher mortality compared to those without, we assumed that this risk score may also predict acute cardiac injury in these patients.In this retrospective, single centre cohort study, we included 352 hospitalised patients with laboratory-confirmed COVID-19 and divided into three groups according to M-CHA2DS2VASc risk score which was created by changing gender criteria of the CHA2DS2VASc from female to male (Group 1, score 0-1 (As the M-CHA2DS2VASc risk score increased, acute cardiac injury was also significantly increased (Group 1, 11.3%; group 2, 48.6%; group 3, 76%;Admission M-CHA2DS2VASc risk score was associated with acute cardiac injury in hospitalised patients with COVID-19.

espanolAntecedentes: la formacion de trombos en el ventriculo izquierdo (VI) es una complicacion comun del infarto de miocardio anterior (ANT-MI). El objetivo de este estudio fue investigar la relacion entre la deformacion longitudinal apical (ELA) y el trombo apical del VI despues de ANT-MI. Metodos: El estudio transversal incluyo un total de 235 pacientes que fueron seguidos despues de una intervencion coronaria percutanea primaria realizada por ANT-MI y tenian una fraccion de eyeccion del VI (FEVI) reducida (≤ 40%). De estos pacientes, 24 fueron excluidos del estudio y los 211 pacientes restantes se incluyeron en el analisis. Los pacientes se dividieron en dos grupos segun la presencia (n = 42) o ausencia (n = 169) de trombo del VI detectado por ecocardiografia. La ELA se midio mediante ecocardiografia speckle-tracking. Resultados: Se detecto trombo en 42 de 211 pacientes. No hubo diferencia significativa entre los grupos en cuanto a edad o genero. El strain apical (AS), el strain longitudinal global (GLS), el grosor de la pared apical (AWT) y la FE fueron significativamente menores en los pacientes con trombo apical del VI en comparacion con aquellos sin trombo apical del VI (AS, –5,00 ± 2,30% vs. - 8,54 ± 2,48%, p Conclusion: la ELA es muy sensible y especifica para predecir el trombo del VI despues de ANT-MI. EnglishBackground: Left ventricular (LV) thrombus formation is a common complication of anterior myocardial infarction (ANT-MI). The aim of this study was to investigate the relationship between apical longitudinal strain (ALS) and LV apical thrombus after ANT-MI. Methods: The cross-sectional study included a total of 235 patients who were followed up after primary percutaneous coronary intervention performed for ANT-MI and had a reduced LV ejection fraction (LVEF) (≤40%). Of these patients, 24 were excluded from the study, and the remaining 211 patients were included in the analysis. Patients were divided into two groups based on the presence (n = 42) or absence (n = 169) of LV thrombus detected by echocardiography. ALS was measured using speckle-tracking echocardiography. Results: Thrombus was detected in 42 of 211 patients. There was no significant difference between the groups regarding age or gender. espanolAntecedentes: el sincope vasovagal (VVS) es una condicion clinica comun que involucra antecedentes geneticos. El papel de los betabloqueantes en el tratamiento es controvertido. Objetivo: El objetivo de este estudio fue investigar el efecto del polimorfismo del gen beta-1 en la terapia con betabloqueantes en pacientes con VVS. Metodos: se incluyeron 123 pacientes que fueron diagnosticados de VVS despues de la prueba de la mesa basculante. Buscamos el polimorfismo Arg389Gly (rs1801253) en el gen del adrenoceptor beta-1. Resultados: En general, 64 pacientes (52%) tenian el genotipo Arg389Arg y 59 pacientes (48%) tenian el genotipo Arg389Gly. Los episodios sincopales de los pacientes con genotipo Arg389Arg fueron mas frecuentes en comparacion con los pacientes con genotipo Arg389Gly (episodios sincopales totales [TSE], 7,9 ± 3,7 frente a 6,4 ± 3,0; p = 0,012). The role of beta-blockers in the treatment is controversial. Objective: The aim of this study was to investigate the effect of beta-1 gene polymorphism on beta-blocker therapy in patients with VVS. Methods: We included 123 patients who were diagnosed with VVS after the tilt-table test. We searched for the polymorphism Arg389Gly (rs1801253) in the beta-1 adrenoceptor gene. Results: Overall, 64 patients (52%) had Arg389Arg genotype and 59 patients (48%) had Arg389Gly genotype. The syncopal episodes of patients with Arg389Arg genotype were more frequent compared with patients having Arg389Gly genotype (total syncopal episodes [TSE], 7.9 ± 3.7 vs. 6.4 ± 3.0; p = 0.012).
