Background: Mild traumatic brain injury (mTBI) comprises a majority of traumatic brain injury (TBI) cases. While some mTBI would suffer neurological deterioration (ND) and therefore have poorer prognosis. This study was designed to develop the predictive model for the ND among mTBI using machine learning algorithms., Methods: mTBI patients recorded in the Medical Information Mart for Intensive Care-III were selected for the study. The ND was defined as a drop of Glasgow Coma Scale ≥ 2 within the first 7 day after admission. Eight machine learning algorithms were trained and validated with 5-fold cross validation including extreme gradient boosting, logistic regression, light gradient boosting machine, random forest, adaptive boosting, decision tree, complement naïve Bayes, and support vector machine. The value of eight machine learning algorithms was compared by the area under the receiver operating characteristic curve (AUC)., Results: 361 mTBI patients suffered the ND with the incidence of 30.7%. The ND group had higher 30-day mortality (p = 0.001). In the training cohort of mTBI patients, the random forest performed the best on predicting the ND with the AUC of 1.000. The XGBoost and AdaBoost had an AUC of 0.827 and 0.815, respectively. The logistic regression performed the best on predicting the ND in the validation cohort with the AUC of 0.741. The XGBoost, random forest and AdaBoost had an AUC of 0.729, 0.735, 0.736 in the validation cohort, respectively. After adjusting confounding effects, the multivariate logistic regression found only two independent risk factors for the ND including Sequential Organ Failure Assessment (SOFA) (p < 0.001) and hypertension (p = 0.001). The logistic regression predictive model composed of SOFA and hypertension had an AUC of 0.741., Conclusions: SOFA score and complicated hypertension are two independent risk factors for the neurological deterioration among mTBI patients. The logistic regression predictive model incorporating SOFA and hypertension is helpful to identify mTBI patients with the high risk of ND., (© 2024. Background: The number of geriatric traumatic brain injury (TBI) patients is increasing every year due to the population's aging in most of the developed countries. Unfortunately, there is no widely recognized tool for specifically evaluating the prognosis of geriatric TBI patients. We designed this study to compare the prognostic value of different machine learning algorithm-based predictive models for geriatric TBI. Methods: TBI patients aged ≥65 from the Medical Information Mart for Intensive Care-III (MIMIC-III) database were eligible for this study. To develop and validate machine learning algorithm-based prognostic models, included patients were divided into a training set and a testing set, with a ratio of 7:3. The predictive value of different machine learning based models was evaluated by calculating the area under the receiver operating characteristic curve, sensitivity, specificity, accuracy and F score. Results: A total of 1123 geriatric TBI patients were included, with a mortality of 24.8%. Non-survivors had higher age (82.2 vs. 80.7, p = 0.010) and lower Glasgow Coma Scale (14 vs. 7, p < 0.001) than survivors. The rate of mechanical ventilation was significantly higher (67.6% vs. 25.9%, p < 0.001) in non-survivors while the rate of neurosurgical operation did not differ between survivors and non-survivors (24.3% vs. 23.0%, p = 0.735). Among different machine learning algorithms, Adaboost (AUC: 0.799) and Random Forest (AUC: 0.795) performed slightly better than the logistic regression (AUC: 0.792) on predicting mortality in geriatric TBI patients in the testing set. Conclusion: Adaboost, Random Forest and logistic regression all performed well in predicting mortality of geriatric TBI patients. Prognostication tools utilizing these algorithms are helpful for physicians to evaluate the risk of poor outcomes in geriatric TBI patients and adopt personalized therapeutic options for them. Background: Patients suffered aneurysmal subarachnoid hemorrhage (aSAH) usually develop poor survival and functional outcome. Evaluating aSAH patients at high risk of poor outcome is necessary for clinicians to make suitable therapeutical strategy. This study is conducted to develop prognostic model using XGBoost (extreme gradient boosting) algorithm in aSAH. Methods: A total of 351 aSAH patients admitted to West China hospital were identified. Patients were divided into training set and test set with ratio of 7:3 to testify the predictive value of XGBoost based prognostic model. Additionally, logistic regression model was also constructed and compared with XGBoost based model. Area under the receiver operating characteristic curve (AUC), sensitivity and specificity were calculated to evaluate the value of XGBoost and logistic regression. Results: There were 74 (21.1%) non-survivors and 148 (42.1%) patients with unfavorable functional outcome. Non-survivors had older age (p=0.025), lower Glasgow coma scale (GCS) (p< 0.001), higher World Federation of Neurosurgical Societies WFNS score (p< 0.001), mFisher score (p< 0.001). The incidence of intraventricular hemorrhage (IVH) (p=0.025) and delayed cerebral ischemia (DCI) (p< 0.001) was higher in non-survivors than survivors. The AUC of XGBoost model for predicting mortality and unfavorable functional outcome were 0.950 and 0.958, which were higher than 0.767 and 0.829 of logistic regression model. Conclusion: XGBoost based model is more precise than logistic regression model in predicting outcome of aSAH patients.
