Children with severe sinusitis being treated with antibiotics may not need an additional nasal decongestant-antihistamine. Researchers tested 68 children with acute maxillary sinusitis being treated with oral amoxicillin by giving a nasal decongestant-antihistamine to half the children and a placebo to the other half. Both groups of children recovered at the same rate, with most symptoms vanishing by the third day of treatment. Also, x-rays of the maxillary sinuses using a Water's view may confirm a diagnosis of sinusitis even in one-year-olds.

Loracarbef, which is the first agent of the carbacephem class of [Beta]-lactam antibiotics to be developed, provides good activity against a broad spectrum of bacteria. A single-blinded, randomized, parallel clinical trial in 10 centers in the United States compared the efficacy and safety of loracarbef with that of amoxicillin/clavulanate potassium in the treatment of acute bacterial maxillary sinusitis. A 7-1-10-day regimen of loracarbef (400 mg twice daily) was as effective as amoxicillin/clavulanate (500/125 mg three times a day) and resulted in somewhat fewer side effects. The results of a European trial in Sweden, Finland, and Iceland showed that loracarbef was clinically more effective than doxycycline in the treatment of acute bacterial maxillary sinusitis.

A study was carried out at 13 medical centers to compare the effectiveness of azithromycin, a new azalide antibiotic, with that of amoxicillin for treating acute maxillary sinusitis (inflammation of the sinuses between the eyes and upper jaw). Seventy-eight patients were randomly assigned to receive either azithromycin (41 subjects) or amoxicillin (37 subjects): of these, 38 completed the study and were considered evaluable. The diagnosis of acute sinusitis was made when bacteria were identified in sinus fluid. After treatment started, patients were examined on days 6, 11, 18, and 30. Results showed a bacteriologic cure rate of 100 percent in both groups. Both treatment groups indicated only mild side effects, which were reported by 4.9 percent of the azithromycin patients and 8.1 percent of the amoxicillin patients. Azithromycin was given five times over a 5-day period, whereas 30 doses of amoxicillin were given over 10 days. The fact that the former drug was effective with fewer doses should improve patient compliance. Azithromycin seems to be a well-tolerated, effective drug for treating acute sinusitis. Patients with bacterial infections of the sinuses do not need antibiotic treatment unless their symptoms persist for longer than seven days. X-rays of the sinuses are also not recommended.

Subperiosteal hematoma of the orbit is a rare complication of ethmoidal sinusitis presenting most commonly in adults. The authors report a unique case who had subperiosteal hematoma of the orbit secondary to ethmoidal sinusitis. She was referred to our hospital with the diagnosis of orbital abscess and magnetic resonance imaging disclosed an extraconal mass in the upper part of the left orbit. A left frontal craniotomy was performed after aspiration of purulent material from ethmoid sinus using endoscopic approach, and a brownish black hematoma material was removed. This patient is the second reported case of subperiosteal hematoma of the orbit misdiagnosed as orbital abscess in the current literature. The presented case exemplifies the fact that early diagnosis and prompt intervention is very important to avoid possible complications often associated with this disease. The main goal of this article was to provide antibiotic treatment guidelines for acute bacterial rhinosinusitis (ABRS). It does so by using pathogen distribution, spontaneous resolution data, and pharmocodynamically derived susceptibilities of the major pathogens. The authors hope to educate the physician on discriminating between viral and bacterial rhinosunisitis and using the most effective antibiotics when bacterial disease is identified. Recommendations for children with mild disease without prior antibiotic treatment 4-6 weeks earlier are amoxicillin/clavulanate, amoxicillin (45-90 mg/kg per day), cefpodoxime, or cefuroxime. For patients with true immediate type 1-hypersensitivity reactions to Β-lactams, the recommendations are azithromycin, clarithomycin, erythromycin, or TMP/SMX. For children with mild disease and prior antibiotic treatment 4-6 weeks earlier or children with moderate disease, preferred regimens are amoxicillin/clavulanate, amoxicillin (80-90 mg/kg per day), cefpodoxime, or cefuroxime with the same recommendations for immediate type 1 hypersensitivity to Β-lactams. Treatment failure is defined as no improvement or worsening at 72 hours. The following principles of appropriate antibiotic use for adults with acute rhinosinusitis apply to the diagnosis and treatment of acute maxillary and ethmoid rhinosinusitis in adults who are not immunocompromised. 1. Most cases of acute rhinosinusitis diagnosed in ambulatory care are caused by uncomplicated viral upper respiratory tract infections. 2. Bacterial and viral rhinosinusitis are difficult to differentiate on clinical grounds. The clinical diagnosis of acute bacterial rhinosinusitis should be reserved for patients with rhinosinusitis symptoms lasting 7 days or more who have maxillary pain or tenderness in the face or teeth (especially when unilateral) and purulent nasal secretions. Patients with rhinosinusitis symptoms that last less than 7 days are unlikely to have bacterial infection, although rarely some patients with acute bacterial rhinosinusitis present with dramatic symptoms of severe unilateral maxillary pain, swelling, and fever. 3. Sinus radiography is not recommended for diagnosis in routine cases. 4. Acute rhinosinusitis resolves without antibiotic treatment in most cases. Symptomatic treatment and reassurance is the preferred initial management strategy for patients with mild symptoms. Antibiotic therapy should be reserved for patients with moderately severe symptoms who meet the criteria for the clinical diagnosis of acute bacterial rhinosinusitis and for those with severe rhinosinusitis symptoms--especially those with unilateral facial pain--regardless of duration of illness. For initial treatment, the most narrow-spectrum agent active against the likely pathogens, Streptococcus pneumoniae and Haemophilus influenzae, should be used. Ceftibuten effectively treats acute sinus infections in children. Ceftibuten eradicates bacteria that are resistant to amoxicillin and other older antibiotics.
