Abstract

Structural and functional studies have provided numerous insights over the past years on how members of the nuclear hormone receptor superfamily tightly regulate the expression of drug-metabolizing enzymes and transporters. Besides the role of the farnesoid X receptor (FXR) in the transcriptional control of bile acid transport and metabolism, this review provides an overview on how this metabolic sensor prevents the accumulation of toxic byproducts derived from endogenous metabolites, as well as of exogenous chemicals, in coordination with the pregnane X receptor (PXR) and the constitutive androstane receptor (CAR). Decrypting this network should provide cues to better understand how these metabolic nuclear receptors participate in physiologic and pathologic processes with potential validation as therapeutic targets in human disabilities and cancers.

Abstract

Although diet has been clearly associated with human health many potential mechanisms remain undefined. For instance, although the intestinal bacterial microflora has long been postulated to contribute to human health, little is known about the effects of diet on the bacterial microflora composition and the specific contributions of the microflora to human health. Thus, we analyzed 1) changes in the fecal microflora composition by fluorescent in-situ hybridization (FISH) and denaturing gradient gel electrophoresis (DGGE) and 2) changes in the fecal bile acid profile, in a crossover feeding study that investigated the effects of black tea drinking on blood lipids in hypercholesterolemic volunteers. DGGE analysis shows that each study subject harbors a specific bacterial profile that exhibits little change over time. Change from a "free" living diet to the controlled study diet or to black tea drinking did not significantly change these bacterial profiles. FISH analysis revealed that even though black tea did not affect the specific bacterial groups that were analyzed, it did decrease the amounts of bacteria that were detected by the universal bacterial probe, but not by any of the specific probes. We did not detect any consistent effects of either diet or black tea drinking on the levels and proportions of fecal bile acids. Our results indicate that tea drinking affects some microflora components. Larger studies with well defined end points that control for the observed variation are needed to improve our understanding of the effects of diet on intestinal microflora and fecal bile acid profile. Abstract. Fat absorption and fasting duodenal bile acids were studied at 11 to 68 days of age in 66 healthy preterm infants, gestational age of 31 to 36 weeks and birth weight of 1,230 to 2,160 g. The infants fed human milk received pooled, expressed milk (55%), partly supplemented (35%) with their own mother's expressed milk. Approximately 10% of the milk was given by breastfeeding once per day. All expressed human milk not fed immediately was pasteurized at 62 Celsius for 30 minutes. The other three groups of infants received an adapted formula (Fl), F1 supplemented with taurine (F2), or F1 supplemented with taurine and cholesterol (F3). In each group, fat was provided at 6.75 g/kg/d. The formulas had a fat concentration of 4.5 g/100 mL, containing 57.6% of unsaturated fatty acids from vegetable oils. In the infants fed human milk, fat absorption was consistently high, and no correlation was found between fat absorption and postnatal age. In the formula-fed infants, fat absorption increased with postnatal age (r = .310 P < .05). When the duodenal concentration of total bile acids was above the median (3.6 mmol/L), fat absorption in the formulated infants exceeded 80%. There was a linear correlation between fat absorption and duodenal bile acids (r = .630, P < .001) in the formula-fed infants. In the infants fed human milk, such correlation was not observed, and their intraluminal concentration of bile acids always exceeded 3.6 mmol/L. The fats from human milk were absorbed more efficiently (93%) than those from the formulas (82%, P < .001). Abstract. Fasting duodenal bile acid concentrations and conjugation patterns were studied during the first 5 weeks of life in 65 low-birth-weight infants, 31 to 36 weeks of gestational age. One group was fed human milk. Approximately 55% of this milk was pooled, expressed, and pasteurized (62 Celsius for 30 minutes), 35% was similarly treated milk from the infant's own mother, and the remainder (10%) was provided by breast-feeding. The other infants, from 3 days of age, were fed one of three formulas: an adapted formula (Fl), F1 supplemented with taurine (F2), or F1 supplemented with taurine and cholesterol (F3). The fasting intraluminal concentration of conjugated bile acids was higher in the infants fed human milk than in the infants fed formulas (F = 30.03, p < .001) reflecting the higher concentrations of all individual bile acids. No significant increase over time was found in the concentration of total bile acids in any feeding group. Chenodeoxycholic acid concentrations, however, increased significantly over time in the infants fed human milk (r = .286, P < .05). Thus, in the infants fed human milk, the ratio of cholates to chenodeoxycholates changed from 2.03 to 1.29 (P < .001), whereas it remained stable (2.61) in the groups fed formula. Tauroconjugated bile acids predominated until at least 5 weeks of life in all the infants fed human milk, F2, or F3. In the infants fed Fl, the concentration of glycoconjugates increased and that of tauroconjugates remained stable. Consequently, the ratio of glycine- to taurine-conjugated bile acids increased twofold, from 0.64 to 1.40 at 5 weeks of age. No further differences due to supplementation of formula with taurine plus cholesterol were found. Abstract

International audience; Structural and functional studies have provided numerous insights over the past years on how members of the nuclear hormone receptor superfamily tightly regulate the expression of drug-metabolizing enzymes and transporters. Besides the role of the farnesoid X receptor (FXR) in the transcriptional control of bile acid transport and metabolism, this review provides an overview on how this metabolic sensor prevents the accumulation of toxic byproducts derived from endogenous metabolites, as well as of exogenous chemicals, in coordination with the pregnane X receptor (PXR) and the constitutive androstane receptor (CAR). Decrypting this network should provide cues to better understand how these metabolic nuclear receptors participate in physiologic and pathologic processes with potential validation as therapeutic targets in human disabilities and cancers.

Abstract

Context Four gamma-glutamyltransferase (GGT) fractions with different molecular weights (big-, medium-, small- and free-GGT) are detectable in human plasma. Objective Verify if liver cells can release all four GGT fractions and if the spatial cell organization influences their release. Methods Hepatoma (HepG2) and melanoma (Me665/2/60) cells were cultured as monolayers or spheroids. GGT released in culture media was analysed by gel-filtration chromatography. Results HepG2 and Me665/2/60 monolayers released the b-GGT fraction, while significative levels of s-GGT and f-GGT were detectable only in media of HepG2-spheroids. Bile acids alone or in combination with papain promoted the conversion of b-GGT in s-GGT or f-GGT, respectively. Conclusions GGT is usually released as b-GGT, while s-GGT and f-GGT are likely to be produced in the liver extracellular environment by the combined action of bile acids and proteases.

The article reports on the removal of gallstones of the Shah of Iran. It relates that monarch went to New York City to undergo operation after discovering that he has gallstones aside from cancer. It says that reoperation is the only treatment when the gall bladder failed to function or new gallstones are formed after the surgery. It discusses several options assessed by the physicians to eradicate the stones of the Shah without undergoing a surgery. It mentions the use of a channel formed by a rubber tube to be inserted through an opening in the abdomen, the process of flowing the bile acids, and endocsopic papillotomy. It states that the physicians haves chosen the first option, wherein there is no need for general anaesthesia in executing the surgery.
