Introduction: People with serious mental illness (SMI) have low rates of primary care (PC) use and die years prematurely, mostly because of medical illnesses such as cardiovascular disease or cancer. To meet the needs of these individuals, a novel, specialized patient-centered medical home with care coordination ("SMI PACT") was developed and implemented in PC. This study qualitatively examined patients' experiences with this innovative care model. Method: After implementation of the medical home in 2018, one-on-one semistructured interviews were conducted with 28 patients (32% women, 43% Black, and 25% Hispanic). Interviews were professionally transcribed and coded prior to thematic analysis. Results: Patients overwhelmingly described positive experiences with SMI PACT because of the qualities of interpersonal communication displayed by SMI PACT staff (e.g., nonjudgment, good listening, patience), structural features of the SMI PACT collaborative care model (e.g., frequent follow-up communication), and other unique aspects of the SMI PACT model tailored for SMI, such as easy-to-understand language. For these reasons, most patients expressed a desire to continue care in SMI PACT. Patients also self-reported improved engagement with their healthcare and self-management of diet, exercise, blood pressure, and diabetes control as a result of SMI PACT participation. Discussion: Patients enrolled in a specialized PC medical home identified clinician characteristics and behaviors that informed an overwhelmingly positive impression of the program model. Their experiences can guide dissemination of specialized PC models and integrated services for people with SMI. Public Significance Statement: An innovative specialized primary care medical home for patients with serious mental illness was found to improve treatment quality and outcomes. Methods: Analysis of the degree of implementation of integrated social and health HCS perceived by social care services (SCS) and primary health care centers (PHCs) between December 2020 and June 2021 in two phases. First, the perception of integration by social workers within SCS and PHCs was assessed using a screening questionnaire. Then, SCS in counties with the highest integration scores received a customized questionnaire for an in-depth assessment. Results: A total of 105 (100%) SCS and 94 (25%) PHCs answered the screening questionnaire, and 48 (45.7%) SCS received a customized questionnaire. The most frequent barrier identified was the lack of shared protocols, with the most frequent facilitator being the recognition of the importance of integrated HCS. Conclusions: Our study showed that the degree of implementation of integrated health and social HCS offered by the Government of Catalonia was perceived as low. The identified barriers and facilitators can be used to facilitate such implementation. Further studies should include professionals other than social workers in PHC assessments. We investigated the impact of an IC programme in North East Hampshire and Farnham (NEHF), in southern England, on hospital utilisation among older adults over a 55 months period. Method: We used a Generalised Synthetic Control design to investigate the effect of implementing IC in NEHF between 2015 and 2020. For a range of hospital use outcomes, we estimated the trajectory that each would have followed in the absence of IC and compared it with the actual trajectory to estimate the potential impact of IC. Results: Three years into the programme, emergency admission rates started reducing in NEHF relative to its synthetic control, particularly those resulting in overnight hospital stays. By year 5 of the study overall emergency admission rates were 9.8% lower (95% confidence interval: -17.2% to -0.6%). We found no sustained difference in rates of emergency department (ED) visits, and average length of hospital stay was significantly higher from year 2. Conclusion: An IC programme in NEHF led to lower than estimated emergency admission rates; however, the interpretation of the impact of IC on admissions is complicated as lower rates did not appear until three years into the programme and the reliability of the synthetic control weakens over a long time horizon. Methods: We searched electronic databases (PubMED and Ovid Embase) using three main concepts: ‘(integrated care) AND (child population) AND (measurement)’, along with additional searches. Results: Fifteen studies describing 16 measurement instruments were eligible for inclusion. The majority of studies were conducted in the USA. There was a diversity of health conditions included in the studies. The most frequent type of assessment used was a questionnaire (11 identified), but interviews, patient data and healthcare records, and focus groups were also used. Integration outcomes assessed were quality of care coordination, quality of collaboration, continuity of care, completeness of care, structure of care, quality of communication, and local implementation of integrated care. Conclusion: A variety of instruments for the measurement of integration within CYP healthcare systems were identified. Further work on the standardisation of integrated care measures would be valuable; however, it is important that instruments and measures meet the needs of specific settings, populations and conditions being studied. This conceptual article integrates value co-creation concepts with dimensions of integrated care demonstrating how a marketing framework and a framework originated in health can achieve a beneficial fusion to enhance health outcomes. Using midwifery health care services as the context, we contend that integrated care models focus only on co-production overlooking the complex, value co-creation potential of value-in-use for improved health outcomes. We add four new dimensions of value-in-use: client–provider shared principles, client agency, empowerment, and relationship equality. Adopting an interdisciplinary approach, a value co-creation perspective advances our understanding of the activities and processes of integrated care at the various levels in the patient's lifeworld beyond the patient–carer interface. We argue that adding value-in-use dimensions to health care's integrated care model adds conceptual clarity and will improve service delivery and patient health care outcomes. [ABSTRACT FROM AUTHOR]

Description: The increased health needs related to the ageing society and epidemiological patterns in these countries also require funding needs to increase, rationing to be reformed, sectors to be integrated (the managed care approach), and an analytical information base to be developed if supervision of new technological approaches is to improve. The period of system transitions in CEE countries entailed significant changes in their health systems, including health care financing. Discussion: Large deficits in the public financing of health systems were just one of the challenges arising from the economic downturn of the 1990s, which was coupled with inflation, increasing unemployment, low salaries, a large informal sector and tax evasion in a number of CEE countries. During the communist period, there was universal access to a wide range of health services, proving it difficult to retain this coverage. As a result, many states sought to ration publicly funded health services - for example, through patient cost-sharing or decreasing the scope of basic benefits. Yet, not all of these reform plans were implemented, and in fact, some were rolled back or not implemented at all due to a lack of social or political consensus. Conclusion: CEE health systems had come to practice implicit rationing in the form of under-the-table payments from patients, quasi-formal payments to providers to compensate for lack of funding, and long waiting lists forcing patients to the private sector. The aim of this study was to investigate the impact of implementing IC in Norrtälje on the rate of admissions for ambulatory care sensitive conditions (ACSC). Method: Using interrupted time series analyses we investigated the effect of implementing IC in Norrtälje municipality in the northern part of Stockholm county, Sweden. The time period included 48 time points, from year 2000 to year 2011 with measurements before and after introducing IC in Norrtälje in 2006. In order to control for other extraneous events that could affect the outcome measure, but not related to the introduction of IC, we included a control population from Stockholm municipality. Results: After introducing IC in Norrtälje the rate of admissions for ACSC decreased. This decrease was greater in Norrtälje than in the matched control population, however the difference between the two areas was not statistically significant (p = 0.08). Conclusion: Introducing IC in Norrtälje may have had positive impact on admissions for ACSC for older people living in Norrtälje; however, the interpretation of the impact of IC on admissions for ACSC is complicated by intervening policy changes in health and social care during the study period.

Integrated care is a continuum of services delivered by a system organized around the health needs of people rather than diseases. People with chronic musculoskeletal pain often live with coexisting chronic health conditions. Current care for musculoskeletal pain remains narrowly focused on individual painful conditions, despite the complex health needs of patients. We explore the challenges of delivering integrated care to people with musculoskeletal pain and coexisting chronic health conditions. We discuss these challenges in relation to 3 areas: (1) the relationships between musculoskeletal conditions and other chronic health conditions, (2) the factors that impact the clinician's capacity to provide Integrated care for musculoskeletal conditions, and (3) system-level constraints that impact both the clinician's delivery of care and the patient's health care experience. We suggest ways by which clinicians and researchers may move toward better integrated care for musculoskeletal pain and coexisting chronic health conditions. [ABSTRACT FROM AUTHOR]

Integration of health care services has been promoted in several countries to improve the quality and coordination of care. We investigate the effects of such integration in a model where providers compete on quality to attract patients under regulated prices. We identify countervailing effects of integration on quality of care. While integration makes coordination of care more profitable for providers due to bundled payments, it also softens competition as patient choice is restricted. We also identify circumstances due to asymmetries across providers and/or services under which integration either increases or reduces the quality of services provided. In the absence of synergies, integration generally leads to increases in quality for some services and reductions for others. The corresponding effect on health benefits depends largely on whether integration leads to quality dispersion or convergence across services. If the softening of competition effect is weak, integration is likely to improve quality and patient outcomes. [ABSTRACT FROM AUTHOR]

The COVID-19 pandemic puts health and care systems under pressure globally. This current paper highlights challenges arising in the care for older and vulnerable populations in this context and reflects upon possible perspectives for different systems making use of nested integrated care approaches adapted during the work of the EU-funded project VIGOUR (“Evidence based Guidance to Scale-up Integrated Care in Europe”, funded by the European Union’s Health Programme 2014–2020 under Grant Agreement Number 826640).

Introduction: Since the early 2000s, Government policies in Finland have supported administrative and operative integration of health and social care to create larger authorities for organising services and to strengthen coordination of primary and specialised care, and social services. Nationally, the policies have led to attempts to reform the service system to support integration. At the local level, municipal collaboration has been launched to create regional joint health and social care authorities to boost administrative integration and to facilitate the implementation of innovative forms of integrated care. Of these local initiatives, we address two county level joint authorities which have established comprehensive integrated care organisations merging primary and specialised health care and social services under one management. Our aim is to evaluate the success and transferability of these initiatives. Methods: We focus on two vanguard counties, South and North Karelia, which have established joint health and social care authorities in Finland. In South Karelia the authority has operated since 2010 and in North Karelia since 2017. For the multimethod study, we collected intensively data on the operation of the joint authorities and health and social services provided particularly for client groups benefiting from integration. We compiled county policy and evaluation documents on health and social care, run an online survey among employees, and carried out individual and group interviews of management and employees. Results: The two counties have several common features including the population of 130,000-169,000 inhabitants, regions of a larger town with a specialised hospital and several rural municipalities. While both counties have intensively supported to care integration, they display differences in their approaches. South Karelia has gradually developed integrated services for focused client groups, such as homebased rehabilitation services for the elderly, integrated services for children and young people, and integrated psychosocial services for adults. In North Karelia, reorganised recently, integration has focused on integrated health and welfare centres bringing earlier separate primary and mental health care, and social and substance abuse services on the same premises. In both counties, preliminary experiences on adopting integrated care models are positive. Efficiency has improved and waiting time and costs decreased without deteriorating patient satisfaction. The early experiences also underline the importance of strengthened shared management to address smooth work processes between different professional groups. Discussion and conclusions: The establishment of the joint health and social care authorities with a strong emphasis on integrated care seems to be successful. The early experiences suggest a positive impact in terms of efficiency. Transferability particularly to larger urban municipalities is challenging to assess. The national health and social system reform currently prepared in Finland may include features, such as increasing privatisation and market-based services, which request changes in the integrated care models. Limitations: The study was based only on two county-based health and social care authorities with relatively similar integrated care models. Suggestions for future research: Due to limitations in case-studies, long-term broader follow-up studies are needed for assessing and comparing the impact and outcomes of different integrated care models.

Screening for social determinants of health allows health care teams to assess and address social factors that influence one's health, mental health, and access to care. These social factors include poverty, health literacy, social support, exposure to trauma, food insecurity, and housing instability. The objective of this study was to examine what screening tools for social determinants of health are being used, in what contexts, and with what populations. Findings suggest that health literacy is the most commonly screened for, followed by trauma history, social support, food insecurity and housing across diverse contexts and populations. The high prevalence of diabetes mellitus - third among the leading chronic non-communicable diseases, the large costs of its treatment, incl. and the complications that have occurred determine the need to look for mechanisms for better control of the disease and prevention of its complications. The significance of the problem is also determined by the social and health needs of people with poor quality of life and higher specific health requirements, such as diabetic patients. It focuses on the need to reform the existing model of care in order to improve it in order to achieve better quality, complexity and comprehensiveness of care and increase patient satisfaction. The proposed organizational structure is part of the development of an innovative model for the organization of health care in diabetes mellitus, which provides the opportunity for the implementation of integrated care and aims at achieving better quality, complexity and coverage of the care and increasing the satisfaction of patients. It allows the pooling of resources, information and knowledge, provides opportunities to move closer to the WHO model to provide modern care for patients with chronic illness and to integrate our country with those with "good practices" for providing integrated care and managing chronic diabetes mellitus. The planned organizational structure of the Specialized Center for the Control of Diabetes Mellitus and Prevention of its Complications is designed to serve the complex needs and needs of care for patients with diabetes mellitus in outpatient care.
