We describe a late survey of 51 patients who had hemangiomas of the eyelid in infancy, together with the visual complications found and a program designed for prevention of the latter.

This article describes but the tip of the congenital vasoformative iceberg. Much has been left unsaid. The problem is very common, and it is hoped that with continued clinical prudence we can advance and improve our treatment modalities, particularly in those areas in which we fall so short!

The major findings in a study of 51 infants and children with infantile hemangioma of the eyelid were as follows: Visual complications occurred in 27 patients. The most common complications were amblyopia (in 22) and strabismus (in 17). Amblyopia of 6/60 (20/200) or less was probably caused by stimulus-deprivation, but amblyopia in the range of 6/30 (20/100) to 6/12 (20/40) was likely caused by anisometropia or strabismus. Eyelid occlusion of six months or more invariably resulted in amblyopia of 6/60 (20/200) or less. Occlusion for even one month carried a risk of amblyopia. Each child must be considered individually for therapy, which must be started as early as possible. Patients should receive careful follow-up from the beginning to prevent severe amblyopia. For difficult cases, we need more efficacious, safe methods to induce regression of these tumors.

Each section of this article has been discussed in severe brevity. Potential pitfalls have been expanded in an attempt to be helpful to the general surgeon who attends patients with these clinical problems. Making the relatives aware of a potential complication in its broadest sense is the responsibility of the surgeon and can only be done if there is cognizance of these problems. The author hopes that this article has accomplished this purpose, albeit in a brief format.

The port-wine stain, a relatively rare subgroup of capillary hemangiomas, usually involves the face and in children it may be associated with glaucoma. A series of 50 patients with facial port-wine stains had ocular examination at The Hospital for Sick Children, Toronto. When the facial areas supplied by both the ophthalmic and maxillary divisions of the sensory branch of the trigeminal nerve were involved (26 patients) there was about a 45% chance of diagnosing glaucoma, either as "true" glaucoma (with visual loss and raised intraocular pressure) or as "glaucoma suspect" (without visual loss). Involvement of the area supplied by either division alone was not associated with glaucoma.

It is always difficult to establish one preferred management program for all patients with any lesion, but serious consideration must be given to "no-touch" management of cavernous or capillary-cavernous hemangiomas of the nasal tip. This requires frequent consultations with the parents to furnish psychological support. If extreme pressure from the parents makes surgery inevitable, the operation should be limited to the immediate confines of the nasal tip.
