Objective. To provide 3D high-resolution cardiac T1 maps using model-based super-resolution reconstruction (SRR). Approach. Due to signal-to-noise ratio limitations and the motion of the heart during imaging, often 2D T1 maps with only low through-plane resolution (i.e. slice thickness of 6–8 mm) can be obtained. Here, a model-based SRR approach is presented, which combines multiple stacks of 2D acquisitions with 6–8 mm slice thickness and generates 3D high-resolution T1 maps with a slice thickness of 1.5–2 mm. Every stack was acquired in a different breath hold (BH) and any misalignment between BH was corrected retrospectively. The novelty of the proposed approach is the BH correction and the application of model-based SRR on cardiac T1 Mapping. The proposed approach was evaluated in numerical simulations and phantom experiments and demonstrated in four healthy subjects. Main results. Alignment of BH states was essential for SRR even in healthy volunteers. In simulations, respiratory motion could be estimated with an RMS error of 0.18 ± 0.28 mm. SRR improved the visualization of small structures. High accuracy and precision (average standard deviation of 69.62 ms) of the T1 values was ensured by SRR while the detectability of small structures increased by 40%. Significance. The proposed SRR approach provided T1 maps with high in-plane and high through-plane resolution (1.3 × 1.3 × 1.5–2 mm3). The approach led to improvements in the visualization of small structures and precise T1 values.

Background Cardiac PET has recently found novel applications in coronary atherosclerosis imaging using [18F]NaF as a radiotracer, highlighting vulnerable plaques. However, the resulting uptakes are relatively small, and cardiac motion and respiration-induced movement of the heart can impair the reconstructed images due to motion blurring and attenuation correction mismatches. This study aimed to apply an MR-based motion compensation framework to [18F]NaF data yielding high-resolution motion-compensated PET and MR images. Methods Free-breathing 3-dimensional Dixon MR data were acquired, retrospectively binned into multiple respiratory and cardiac motion states, and split into fat and water fraction using a model-based reconstruction framework. From the dynamic MR reconstructions, both a non-rigid cardiorespiratory motion model and a motion-resolved attenuation map were generated and applied to the PET data to improve image quality. The approach was tested in 10 patients and focal tracer hotspots were evaluated concerning their target-to-background ratio, contrast-to-background ratio, and their diameter. Results MR-based motion models were successfully applied to compensate for physiological motion in both PET and MR. Target-to-background ratios of identified plaques improved by 7 ± 7%, contrast-to-background ratios by 26 ± 38%, and the plaque diameter decreased by −22 ± 18%. MR-based dynamic attenuation correction strongly reduced attenuation correction artefacts and was not affected by stent-related signal voids in the underlying MR reconstructions. Conclusions The MR-based motion correction framework presented here can improve the target-to-background, contrast-to-background, and width of focal tracer hotspots in the coronary system. The dynamic attenuation correction could effectively mitigate the risk of attenuation correction artefacts in the coronaries at the lung-soft tissue boundary. In combination, this could enable a more reproducible and reliable plaque localisation.

In this work we reduce undersampling artefacts in two-dimensional ($2D$) golden-angle radial cine cardiac MRI by applying a modified version of the U-net. We train the network on $2D$ spatio-temporal slices which are previously extracted from the image sequences. We compare our approach to two $2D$ and a $3D$ Deep Learning-based post processing methods and to three iterative reconstruction methods for dynamic cardiac MRI. Our method outperforms the $2D$ spatially trained U-net and the $2D$ spatio-temporal U-net. Compared to the $3D$ spatio-temporal U-net, our method delivers comparable results, but with shorter training times and less training data. Compared to the Compressed Sensing-based methods $kt$-FOCUSS and a total variation regularised reconstruction approach, our method improves image quality with respect to all reported metrics. Further, it achieves competitive results when compared to an iterative reconstruction method based on adaptive regularization with Dictionary Learning and total variation, while only requiring a small fraction of the computational time. A persistent homology analysis demonstrates that the data manifold of the spatio-temporal domain has a lower complexity than the spatial domain and therefore, the learning of a projection-like mapping is facilitated. Even when trained on only one single subject without data-augmentation, our approach yields results which are similar to the ones obtained on a large training dataset. This makes the method particularly suitable for training a network on limited training data. Finally, in contrast to the spatial $2D$ U-net, our proposed method is shown to be naturally robust with respect to image rotation in image space and almost achieves rotation-equivariance where neither data-augmentation nor a particular network design are required., Comment: To be published in IEEE Transactions on Medical Imaging

Purpose: Respiratory motion‐compensated (MC) 3D cardiac fat‐water imaging at 7T. Methods: Free‐breathing bipolar 3D triple‐echo gradient‐recalled‐echo (GRE) data with radial phase‐encoding (RPE) trajectory were acquired in 11 healthy volunteers (7M\4F, 21–35 years, mean: 30 years) with a wide range of body mass index (BMI; 19.9–34.0 kg/m2) and volunteer tailored B1+ shimming. The bipolar‐corrected triple‐echo GRE‐RPE data were binned into different respiratory phases (self‐navigation) and were used for the estimation of non‐rigid motion vector fields (MF) and respiratory resolved (RR) maps of the main magnetic field deviations (ΔB0). RR ΔB0 maps and MC ΔB0 maps were compared to a reference respiratory phase to assess respiration‐induced changes. Subsequently, cardiac binned fat‐water images were obtained using a model‐based, respiratory motion‐corrected image reconstruction. Results: The 3D cardiac fat‐water imaging at 7T was successfully demonstrated. Local respiration‐induced frequency shifts in MC ΔB0 maps are small compared to the chemical shifts used in the multi‐peak model. Compared to the reference exhale ΔB0 map these changes are in the order of 10 Hz on average. Cardiac binned MC fat‐water reconstruction reduced respiration induced blurring in the fat‐water images, and flow artifacts are reduced in the end‐diastolic fat‐water separated images. Conclusion: This work demonstrates the feasibility of 3D fat‐water imaging at UHF for the entire human heart despite spatial and temporal B1+ and B0 variations, as well as respiratory and cardiac motion.

PURPOSE: Myocardial fat infiltrations are associated with a range of cardiomyopathies. The purpose of this study was to perform cardio-respiratory motion-correction for model-based water-fat separation to image fatty infiltrations of the heart in a free-breathing, non-cardiac-triggered high-resolution 3D MRI acquisition. METHODS: Data were acquired in nine patients using a free-breathing, non-cardiac-triggered high-resolution 3D Dixon gradient-echo sequence and radial phase encoding trajectory. Motion correction was combined with a model-based water-fat reconstruction approach. Respiratory and cardiac motion models were estimated using a dual-mode registration algorithm incorporating both motion-resolved water and fat information. Qualitative comparisons of fat structures were made between 2D clinical routine reference scans and reformatted 3D motion-corrected images. To evaluate the effect of motion correction the local sharpness of epicardial fat structures was analyzed for motion-averaged and motion-corrected fat images. RESULTS: The reformatted 3D motion-corrected reconstructions yielded qualitatively comparable fat structures and fat structure sharpness in the heart as the standard 2D breath-hold. Respiratory motion correction improved the local sharpness on average by 32% ± 24% with maximum improvements of 81% and cardiac motion correction increased the sharpness further by another 15% ± 11% with maximum increases of 31%. One patient showed a fat infiltration in the myocardium and cardio-respiratory motion correction was able to improve its visualization in 3D. CONCLUSION: The 3D water-fat separated cardiac images were acquired during free-breathing and in a clinically feasible and predictable scan time. Compared to a motion-averaged reconstruction an increase in sharpness of fat structures by 51% ± 27% using the presented motion correction approach was observed for nine patients.

Coronary artery disease (CAD) is caused by the formation of plaques in the coronary arteries and is one of the most common cardiovascular diseases. NaF-PET can be used to assess plaque composition, which could be important for therapy planning. One of the main challenges of NaF-PET is cardiac and respiratory motion which can strongly impair diagnostic accuracy. In this study, we investigated the use of a synergistic image registration approach which combined motion-resolved MR and PET data to estimate cardiac and respiratory motion. This motion estimation could then be used to improve the NaF-PET image quality. The approach was evaluated with numerical simulations and in vivo scans of patients suffering from CAD. In numerical simulations, it was shown, that combining MR and PET information can improve the accuracy of motion estimation by more than 15%. For the in vivo scans, the synergistic image registration led to an improvement in uptake visualization. This is the first study to assess the benefit of combining MR and NaF-PET for cardiac and respiratory motion estimation. Purpose To evaluate prospective motion correction using the pilot tone (PT) as a quantitative respiratory motion signal with high temporal resolution for cardiac cine images during free breathing. Methods Before cine data acquisition, a short prescan was performed, calibrating the PT to the respiratory-induced heart motion using respiratory-resolved real-time images. The calibrated PT was then applied for nearly real-time prospective motion correction of cine MRI through slice tracking (ie, updating the slice position before every readout). Additionally, in-plane motion correction was performed retrospectively also based on the calibrated PT data. The proposed method was evaluated in a moving phantom and 10 healthy volunteers. Results The PT showed very good correlation to the phantom motion. In volunteer studies using a long-term scan over 7.96 ± 1.40 min, the mean absolute error between registered and predicted motion from the PT was 1.44 ± 0.46 mm in head-feet and 0.46 ± 0.07 mm in anterior-posterior direction. Irregular breathing could also be corrected well with the PT. The PT motion correction leads to a significant improvement of contrast-to-noise ratio by 68% (P ≤ .01) between blood pool and myocardium and sharpness of endocardium by 24% (P = .04) in comparison to uncorrected data. The image score, which refers to the cine image quality, has improved with the utilization of the proposed PT motion correction. Conclusion The proposed approach provides respiratory motion-corrected cine images of the heart with improved image quality and a high scan efficiency using the PT. The PT is independent of the MR acquisition, making this a very flexible motion-correction approach.

SIRF is a powerful PET/MR image reconstruction research tool for processing data and developing new algorithms. In this research, new developments to SIRF are presented, with focus on motion estimation and correction. SIRF’s recent inclusion of the adjoint of the resampling operator allows gradient propagation through resampling, enabling the MCIR technique. Another enhancement enabled registering and resampling of complex images, suitable for MRI. Furthermore, SIRF’s integration with the optimization library CIL enables the use of novel algorithms. Finally, SPM is now supported, in addition to NiftyReg, for registration. Results of MR and PET MCIR reconstructions are presented, using FISTA and PDHG, respectively. These demonstrate the advantages of incorporating motion correction and variational and structural priors. Magnetic Resonance Fingerprinting (MRF) is a promising technique for fast quantitative imaging of human tissue. In general, MRF is based on a sequence of highly undersampled MR images which are analyzed with a pre-computed dictionary. MRF provides valuable diagnostic parameters such as the T 1 and T 2 MR relaxation times. However, uncertainty characterization of dictionary-based MRF estimates for T 1 and T 2 has not been achieved so far, which makes it challenging to assess if observed differences in these estimates are significant and may indicate pathological changes of the underlying tissue. We propose a Bayesian approach for the uncertainty quantification of dictionary-based MRF which leads to probability distributions for T 1 and T 2 in every voxel. The distributions can be used to make probability statements about the relaxation times, and to assign uncertainties to their dictionary-based MRF estimates. All uncertainty calculations are based on the pre-computed dictionary and the observed sequence of undersampled MR images, and they can be calculated in short time. The approach is explored by analyzing MRF measurements of a phantom consisting of several tubes across which MR relaxation times are constant. The proposed uncertainty quantification is quantitatively consistent with the observed within-tube variability of estimated relaxation times. Furthermore, calculated uncertainties are shown to characterize well observed differences between the MRF estimates and the results obtained from high-accurate reference measurements. These findings indicate that a reliable uncertainty quantification is achieved. We also present results for simulated MRF data and an uncertainty quantification for an in vivo MRF measurement. MATLAB® source code implementing the proposed approach is made available.

This paper presents a simulation framework for dynamic PET-MR. The main focus of this framework is to provide motion-resolved MR and PET data and ground truth motion information. This can be used in the optimisation and quantitative evaluation of image registration and in assessing the error propagation due to inaccuracies in motion estimation in complex motion-compensated reconstruction algorithms. Contrast and tracer kinetics can also be simulated and are available as ground truth information. To closely emulate medical examination, input and output of the simulation are files in standardised open-source raw data formats. This enables the use of existing raw data as a template input and ensures seamless integration of the output into existing reconstruction pipelines. The proposed framework was validated in PET-MR and image registration applications. It was used to simulate a FDG-PET-MR scan with cardiac and respiratory motion. Ground truth motion information could be utilised to optimise parameters for PET and synergistic PET-MR image registration. In addition, a free-breathing dynamic contrast enhancement (DCE) abdominal scan of a patient with hepatic lesions was simulated. In order to correct for breathing motion, a motion-corrected image reconstruction scheme was used and a Toft's model was fit to the DCE data to obtain quantitative DCE-MRI parameters. Utilising the ground truth motion information, the dependency of quantitative DCE-MR images on the accuracy of the motion estimation was evaluated. We demonstrated that respiratory motion had to be available with an average accuracy of at least the spatial resolution of the DCE-MR images in order to ensure an improvement in lesions visualisation and quantification compared to no motion correction. The proposed framework provides a valuable tool with a wide range of scientific PET and MR applications and will be available as part of the open-source project Synergistic Image Reconstruction Framework (SIRF).

In this paper we present a generalized Deep Learning-based approach for solving ill-posed large-scale inverse problems occuring in medical image reconstruction. Recently, Deep Learning methods using iterative neural networks (NNs) and cascaded NNs have been reported to achieve state-of-the-art results with respect to various quantitative quality measures as PSNR, NRMSE and SSIM across different imaging modalities. However, the fact that these approaches employ the application of the forward and adjoint operators repeatedly in the network architecture requires the network to process the whole images or volumes at once, which for some applications is computationally infeasible. In this work, we follow a different reconstruction strategy by strictly separating the application of the NN, the regularization of the solution and the consistency with the measured data. The regularization is given in the form of an image prior obtained by the output of a previously trained NN which is used in a Tikhonov regularization framework. By doing so, more complex and sophisticated network architectures can be used for the removal of the artefacts or noise than it is usually the case in iterative NNs. Due to the large scale of the considered problems and the resulting computational complexity of the employed networks, the priors are obtained by processing the images or volumes as patches or slices. We evaluated the method for the cases of 3D cone-beam low dose CT and undersampled 2D radial cine MRI and compared it to a total variation-minimization-based reconstruction algorithm as well as to a method with regularization based on learned overcomplete dictionaries. The proposed method outperformed all the reported methods with respect to all chosen quantitative measures and further accelerates the regularization step in the reconstruction by several orders of magnitude.

Respiratory and cardiac motion can strongly impair cardiac PET image quality and tracer uptake quantification. Standard gating techniques can minimize these motion artefacts but suffer from low signal-to-noise ratio because only a small percentage of the total data is utilized. Motion correction approaches have been proposed to overcome this problem but require accurate knowledge of such physiological motion. Here we present a joint PET-MR motion estimation approach which combines complimentary dynamic image information from simultaneously acquired MR and PET to ensure improved cardiac and respiratory motion estimation for motion-corrected image reconstruction (MCIR) of PET images. A 3D triple-echo Dixon MR scan is used both for calculation of MR-based attenuation correction (AC) maps and estimation of physiological motion. PET listmode data is obtained simultaneously to the MR acquisition which is used for a joint motion estimation and reconstruction of the final MCIR PET. In a first step, dynamic cardiac and respiratory motion resolved 4D MR and PET images are reconstructed. These image series are used in a joint image registration to estimate non-rigid cardiac and respiratory motion fields. In a second step, the motion fields are utilized in a MR MCIR to obtain cardiac and respiratory resolved dynamic MR-based AC maps. In the last step, the non-rigid motion fields and the dynamic AC maps are applied in a PET MCIR to obtain the final motion-corrected PET images. PET-MR data has been obtained in six patients without any known heart disease. Motion amplitudes were between 5.6 and 16 mm, with higher values in the basal compared to the mid-ventricular and apical segments. The proposed joint PET-MR motion estimation provided more accurate motion estimation than using either modality separately. The underestimation of PET uptake due to respiratory and cardiac motion artefacts in the AC maps was up to 17%. The average increase in uptake values using MCIR was 23% ± 10% (p < 0.0001), with values of 28% ± 11% (p < 0.0001) for basal, 21% ± 8% (p < 0.0001) for mid-cavity and 17% ± 7% (p < 0.0001) for apical segments. With the proposed scheme we could ensure high PET image quality and improve local PET uptake quantification by up to 30%. Attenuation correction and motion information was obtained from the same PET-MR raw data, which was obtained during free-breathing to minimize scan times and to increase patient comfort.

Quantification of myocardial perfusion by contrast-enhanced cardiovascular magnetic resonance imaging (CMR) aims for an observer independent and reproducible risk assessment of cardiovascular disease. Currently, the data used for the pixel-wise analysis of cardiac perfusion are either filtered prior to a fitting procedure, which inherently reduces the spatial resolution of data; or all pixels are considered without any regularization or prior filtering, which yields an unstable fit in the presence of low signal-to-noise ratio. Here, we propose a new pixel-wise analysis based on spatial Tikhonov regularization which exploits the spatial smoothness of the data and ensures accurate quantification even for images with low signal-to-noise ratio. The regularization parameter is determined automatically by an L-curve criterion. We study the performance of our method on a numerical phantom and demonstrate that the method reduces significantly the root-mean square error in the perfusion estimate compared to a non-regularized fit. In patient data our method allows us to recover the myocardial perfusion and to distinguish between healthy and ischemic regions.

Respiratory motion during cardiac PET acquisitions can cause image blurring and erroneous uptake quantification. In particular the misalignment of attenuation correction (AC) maps and PET emission data can lead to severe quantification errors, because the AC value of the heart is five times higher than of the surrounding lung tissue. Standard PET-MR approaches assume accurate alignment between breathhold MR-based AC maps and free-breathing PET emission data but cannot necessarily ensure it. Here we propose a 75 s free-breathing MR-acquisition, which provides respiratory-resolved AC maps (ACDyn) and non-rigid respiratory motion information. This approach ensures accurate AC for free-breathing PET data and the motion information can be utilized to reduce image blurring caused by respiratory motion. 3D multi-echo MR data was acquired during a 75 s free-breathing scan in six patients. Both a respiratory-resolved dynamic AC map (ACDyn) and a non-rigid respiratory motion field are provided by the MR scan. ACDyn yielded AC values for different breathing phases ensuring accurate AC for each respiratory phase of the free-breathing PET data. In addition, motion-corrected image reconstruction (MCIR) of MR and PET data was used to minimize breathing artefacts. Motion amplitudes in the left ventricle were 8.2 ± 2.9 mm with a dominant motion direction along the anterior-anterolateral and inferior-inferoseptal axis of the heart. The proposed ACDyn-MCIR technique led to significant signal recovery of PET tracer uptake by 24 ± 5% (p < 0.05). The maximum improvement was observed in patients with large misalignment between standard breathhold MR-based AC maps and PET emission data. PET image resolution was improved by 20 ± 12% (p < 0.05). We have presented an efficient MR-scan, which ensures accurate motion information and AC values to improve PET quantification for cardiac PET-MR scans. The short scan time of 75 s makes this free-breathing acquisition easy to integrate into standard clinical PET-MR protocols.

Cardiac PET is a versatile imaging technique providing important diagnostic information about ischemic heart diseases. Respiratory and cardiac motion of the heart can strongly impair image quality and therefore diagnostic accuracy of cardiac PET scans. The aim of this study was to investigate a new cardiac PET/MR approach providing respiratory and cardiac motion-compensated MR and PET images in less than 5 min. Methods: Free-breathing 3-dimensional MR data were acquired and retrospectively binned into multiple respiratory and cardiac motion states. Three-dimensional cardiac and respiratory motion fields were obtained with a nonrigid registration algorithm and used in motion-compensated MR and PET reconstructions to improve image quality. The improvement in image quality and diagnostic accuracy of the technique was assessed in simultaneous 18F-FDG PET/MR scans of a canine model of myocardial infarct and was demonstrated in a human subject. Results: MR motion fields were successfully used to compensate for in vivo cardiac motion, leading to improvements in full width at half maximum of the canine myocardium of 13% ± 5%, similar to cardiac gating but with a 90% ± 57% higher contrast-to-noise ratio between myocardium and blood. Motion correction led to an improvement in MR image quality in all subjects, with an increase in sharpness of the canine coronary arteries of 85% ± 72%. A functional assessment showed good agreement with standard MR cine scans with a difference in ejection fraction of 22% ± 3%. MR-based respiratory and cardiac motion information was used to improve the PET image quality of a human in vivo scan. Conclusion: The MR technique presented here provides both diagnostic and motion information that can be used to improve MR and PET image quality. Reliable respiratory and cardiac motion correction could make cardiac PET results more reproducible.

Knowledge of atrial wall thickness (AWT) has the potential to provide important information for patient stratification and the planning of interventions in atrial arrhythmias. To date, information about AWT has only been acquired in post-mortem or poor-contrast computed tomography (CT) studies, providing limited coverage and highly variable estimates of AWT. We present a novel contrast agent-free MRI sequence for imaging AWT and use it to create personalized AWT maps and a biatrial atlas. A novel black-blood phase-sensitive inversion recovery protocol was used to image ten volunteers and, as proof of concept, two atrial fibrillation patients. Both atria were manually segmented to create subject-specific AWT maps using an average of nearest neighbors approach. These were then registered non-linearly to generate an AWT atlas. AWT was 2.4 ± 0.7 and 2.7 ± 0.7 mm in the left and right atria, respectively, in good agreement with post-mortem and CT data, where available. AWT was 2.6 ± 0.7 mm in the left atrium of a patient without structural heart disease, similar to that of volunteers. In a patient with structural heart disease, the AWT was increased to 3.1 ± 1.3 mm. We successfully designed an MRI protocol to non-invasively measure AWT and create the first whole-atria AWT atlas. The atlas can be used as a reference to study alterations in thickness caused by atrial pathology. The protocol can be used to acquire personalized AWT maps in a clinical setting and assist in the treatment of atrial arrhythmias.

Respiratory motion is a complicating factor in PET imaging as it leads to blurring of the reconstructed images which adversely affects disease diagnosis and staging. Existing motion correction techniques are often based on 1D navigators which cannot capture the inter- and intra-cycle variabilities that may occur in respiration. MR imaging is an attractive modality for estimating such motion more accurately, and the recent emergence of hybrid PET/MR systems allows the combination of the high molecular sensitivity of PET with the versatility of MR. However, current MR imaging techniques cannot achieve good image contrast inside the lungs in 3D. 2D slices, on the other hand, have excellent contrast properties inside the lungs due to the in-flow of previously unexcited blood, but lack the coverage of 3D volumes. In this work we propose an approach for the robust, navigator-less reconstruction of dynamic 3D volumes from 2D slice data. Our technique relies on the fact that data acquired at different slice positions have similar low-dimensional representations which can be extracted using manifold learning. By aligning these manifolds we are able to obtain accurate matchings of slices with regard to respiratory position. The approach naturally models all respiratory variabilities. We compare our method against two recently proposed MR slice stacking methods for the correction of PET data: a technique based on a 1D pencil beam navigator, and an image-based technique. On synthetic data with a known ground truth our proposed technique produces significantly better reconstructions than all other examined techniques. On real data without a known ground truth the method gives the most plausible reconstructions and high consistency of reconstruction. Lastly, we demonstrate how our method can be applied for the respiratory motion correction of simulated PET/MR data.

To provide 3D multicontrast anatomical MR with high isotropic resolution and metabolic positron emission tomography (PET) images using a respiratory motion-compensated simultaneous PET-MR examination with high scan efficiency.Standard abdominal PET-MR examinations combine MR data obtained during multiple breath-holds with free-breathing PET acquisitions, limiting the achievable image resolution and potentially causing misalignment errors between breath-hold and free-breathing data. Here, a 3D free-breathing PET-MR acquisition is presented, yielding TThe proposed approach was evaluated in 11 oncology patients and provided respiratory motion information with an accuracy of 1.3 ± 0.1 mm. Sharpness of anatomical features was increased by 19 ± 13% compared with the uncorrected MR images in a 54 ± 26% shorter scan time than a gated MR acquisition. The MR-based motion information improved uptake values (75 ± 94%) and resolution (16 ± 27%) of simultaneously acquired PET images.The proposed method provides motion-compensated 3D high-quality MR and PET images in a comprehensive and highly efficient examination. Cardiac and respiratory motion cause image quality degradation in PET imaging, affecting diagnostic accuracy of the images. Whole-body simultaneous PET-MR scanners allow for using motion information estimated from MR images to correct PET data and produce motion-compensated PET images. This article reviews methods that have been proposed to estimate motion from MR images and different techniques to include this information in PET reconstruction, in order to overcome the problem of cardiac and respiratory motion in PET-MR imaging. MR-based motion correction techniques significantly increase lesion detectability and contrast, and also improve accuracy of uptake values in PET images.

Purpose. To visualize corneal microstructure such as tear film, epithelium, and Bowman’s layer in three dimensions with spectral domain optical coherence tomography (SDOCT) exhibiting 1.3 m axial resolution at 100,000 A-scans/s. This enables measurement of epithelial and Bowman layer thickness across an area of 8.4 mm 8.4 mm and measuring the tear film thickness at the central cornea. Methods. We designed a high-performance SDOCT system, which uses a broad bandwidth TiSapph Laser and a high-speed complementary metal-oxide-semiconductor detector technology, providing a resolution in tissue of 1.3 m and an acquisition speed of 100,000 A-scans/s. Such speed and resolution is a prerequisite if precise anatomy is to be determined. The high resolution gives access to corneal microstructure such as the epithelium layer as well as the boundaries of Bowman’s layer and stroma. Even more interestingly, the tear film can be distinguished on the surface of the cornea. The Bowman’s layer and epithelial thickness for both eyes of nine subjects have been measured out of which two subjects underwent photorefractive keratectomy treatment. Results. Three-dimensional volumes of the human cornea have been recorded in vivo at an A-scan rate of 100,000 scans/s. Epithelial thickness was measured to be 55.8 3.3 m and Bowman’s layer thickness 18.7 2.5 m in normal eyes. Epithelial thickness in the eyes after refractive surgery was measured to be 68.2 5.0 m. The Bowman layer was degenerated in these eyes. The average tear film thickness of four eyes was 5.1 0.5 m. Conclusions. Using a high-performance SDOCT system with high-imaging speed and ultrahigh resolution, we produced precise thickness maps of the epithelium and for the first time of the Bowman’s layer. Such a system will give insight into high-fidelity three-dimensional corneal microstructure helping to precisely plan refractive surgery. It may furthermore yield new perspectives on studying and understanding tear film dynamics. (Optom Vis Sci 2012;89:E795–E802)

Cardiovascular diseases, including arrhythmias and heart failure, are commonly treated with percutaneous procedures guided by X-ray fluoroscopy. The visualization of the targeted structures can be enhanced using preacquired respiratory-resolved anatomic data (dynamic roadmap), which is displayed as an overlay onto X-ray fluoroscopy images. This article demonstrates how dynamic roadmaps using an affine motion model can be obtained from one respiratory-resolved three-dimensional whole-heart acquisition using the previously introduced Radial Phase Encoding-Phase Ordering with Automatic Window Selection method. Respiratory motion in different regions of the heart was analyzed in 10 volunteers, and it was shown that the use of dynamic roadmaps can reduce misalignment errors from more than 10 down to less than 1.5 mm. Furthermore, the results suggest that reliable motion information can be obtained from highly undersampled images due to the advantageous undersampling properties of the radial phase encoding trajectory. Finally, results of a three-dimensional dynamic roadmap obtained from a patient before catheter ablation for atrial fibrillation treatment are presented.

High-resolution three-dimensional (3D) cardiovascular magnetic resonance (CMR) is a valuable medical imaging technique, but its widespread application in clinical practice is hampered by long acquisition times. Here we present a novel compressed sensing (CS) reconstruction approach using shearlets as a sparsifying transform allowing for fast 3D CMR (3DShearCS) using 3D radial phase encoding (RPE). An iterative reweighting scheme was applied during image reconstruction to ensure fast convergence and high image quality. Shearlets are mathematically optimal for a simplified model of natural images and have been proven to be more efficient than classical systems such as wavelets. 3DShearCS was compared to three other commonly used reconstruction approaches. Image quality was assessed quantitatively using general image quality metrics and using clinical diagnostic scores from expert reviewers. The proposed technique had lower relative errors, higher structural similarity and higher diagnostic scores compared to the other reconstruction techniques especially for high undersampling factors, i.e. short scan times. 3DShearCS provided ensured accurate depiction of cardiac anatomy for fast imaging and could help to promote 3D high-resolution CMR in clinical practice.

Doppler Fourier domain optical coherence tomography (D-FDOCT) can be used to visualise and measure ocular blood flow in the retina of the human eye. This has the potential to diagnose illnesses such as macula degeneration or diabetic retinopathy early on because these medical conditions cause a pathological change in the retinal vasculature and perfusion. Here we are presenting a method to separate quantitative blood flow information from static tissue. Our approach is demonstrated on retinal D-FDOCT volume scans taken at the optic nerve head (see figure) and near the fovea with a high speed CMOS-based FDOCT system. The advantage of this method is the small post-processing effort together with the immediate availability of segmented quantitative Doppler flow maps. The performance of the filtering technique is also compared to spatial frequency filtering methods.

Background Respiratory motion in positron emission tomography (PET) is an unavoidable source of error in the measurement of tracer uptake, lesion position and lesion size. The introduction of PET-MR dual modality scanners opens a new avenue for addressing this issue. Motion models offer a way to estimate motion using a reduced number of parameters. This can be beneficial for estimating motion from PET, which can otherwise be difficult due to the high level of noise of the data. Method We propose a novel technique that makes use of a respiratory motion model, formed from initial MR scan data. The motion model is used to constrain PET-PET registrations between a reference PET gate and the gates to be corrected. For evaluation, PET with added FDG-avid lesions was simulated from real, segmented, ultrashort echo time MR data obtained from four volunteers. Respiratory motion was included in the simulations using motion fields derived from real dynamic 3D MR volumes obtained from the same volunteers. Results Performance was compared to an MR-derived motion model driven method (which requires constant use of the MR scanner) and to unconstrained PET-PET registration of the PET gates. Respiratory motion poses significant challenges in image-guided interventions. In emerging treatments such as MR-guided HIFU or MR-guided radiotherapy, it may cause significant misalignments between interventional road maps obtained pre-procedure and the anatomy during the treatment, and may affect intra-procedural imaging such as MR-thermometry. Patient specific respiratory motion models provide a solution to this problem. They establish a correspondence between the patient motion and simpler surrogate data which can be acquired easily during the treatment. Patient motion can then be estimated during the treatment by acquiring only the simpler surrogate data. In the majority of classical motion modelling approaches once the correspondence between the surrogate data and the patient motion is established it cannot be changed unless the model is recalibrated. However, breathing patterns are known to significantly change in the time frame of MR-guided interventions. Thus, the classical motion modelling approach may yield inaccurate motion estimations when the relation between the motion and the surrogate data changes over the duration of the treatment and frequent recalibration may not be feasible. We propose a novel methodology for motion modelling which has the ability to automatically adapt to new breathing patterns. This is achieved by choosing the surrogate data in such a way that it can be used to estimate the current motion in 3D as well as to update the motion model. In particular, in this work, we use 2D MR slices from different slice positions to build as well as to apply the motion model. We implemented such an autoadaptive motion model by extending our previous work on manifold alignment. We demonstrate a proof-of-principle of the proposed technique on cardiac gated data of the thorax and evaluate its adaptive behaviour on realistic synthetic data containing two breathing types generated from 6 volunteers, and real data from 4 volunteers. On synthetic data the autoadaptive motion model yielded 21.45% more accurate motion estimations compared to a non-adaptive motion model 10 min after a change in breathing pattern. On real data we demonstrated the method's ability to maintain motion estimation accuracy despite a drift in the respiratory baseline. Due to the cardiac gating of the imaging data, the method is currently limited to one update per heart beat and the calibration requires approximately 12 min of scanning. Furthermore, the method has a prediction latency of 800 ms. These limitations may be overcome in future work by altering the acquisition protocol.

To evaluate two commonly used respiratory motion correction techniques for coronary magnetic resonance angiography (MRA) regarding their dependency on motion estimation accuracy and final image quality and to compare both methods to the respiratory gating approach used in clinical practice.Ten healthy volunteers were scanned using a non-Cartesian radial phase encoding acquisition. Respiratory motion was corrected for coronary MRA according to two motion correction techniques, image-based (IMC) and reconstruction-based (RMC) respiratory motion correction. Both motion correction approaches were compared quantitatively and qualitatively against a reference standard navigator-based respiratory gating (RG) approach. Quantitative comparisons were performed regarding visible vessel length, vessel sharpness, and total acquisition time. Two experts carried out a visual scoring of image quality. Additionally, numerical simulations were performed to evaluate the effect of motion estimation inaccuracy on RMC and IMC.RMC led to significantly better image quality than IMC (P's paired Student's t-test were smaller than 0.001 for vessel sharpness and visual scoring). RMC did not show a statistically significant difference compared to reference standard RG (vessel length [99% confidence interval]: 86.913 [83.097-95.015], P = 0.107; vessel sharpness: 0.640 [0.605-0.802], P = 0.012; visual scoring: 2.583 [2.410-3.424], P = 0.018) in terms of vessel visualization and image quality while reducing scan times by 56%. Simulations showed higher dependencies for RMC than for IMC on motion estimation inaccuracies.RMC provides a similar image quality as the clinically used RG approach but almost halves the scan time and is independent of subjects' breathing patterns. Clinical validation of RMC is now desirable.

Respiratory motion models have been proposed for the estimation and compensation of respiratory motion during image acquisition and image-guided interventions on organs in the chest and abdomen. However, such techniques are not commonly used in the clinic. Subject-specific motion models require a dynamic calibration scan that interrupts the clinical workflow and is often impractical to acquire, while population-based motion models are not as accurate as subject-specific motion models. To address this lack of accuracy, we propose a novel personalisation framework for population-based respiratory motion models and demonstrate its application to respiratory motion of the heart. The proposed method selects a subset of the population sample which is more likely to represent the cardiac respiratory motion of an unseen subject, thus providing a more accurate motion model. The selection is based only on anatomical features of the heart extracted from a static image. The features used are learnt using a neighbourhood approximation technique from a set of training datasets for which respiratory motion estimates are available. Results on a population sample of 28 adult healthy volunteers show average improvements in estimation accuracy of 20% compared to a standard population-based motion model, with an average value for the 50th and 95th quantiles of the estimation error of 1.6mm and 4.7 mm respectively. Furthermore, the anatomical features of the heart most strongly correlated to respiratory motion are investigated for the first time, showing the features on the apex in proximity to the diaphragm and the rib cage, on the left ventricle and interventricular septum to be good predictors of the similarity in cardiac respiratory motion.

Positron emission tomography (PET) is a highly sensitive medical imaging technique commonly used to detect and assess tumor lesions. Magnetic resonance imaging (MRI) provides high resolution anatomical images with different contrasts and a range of additional information important for cancer diagnosis. Recently, simultaneous PET-MR systems have been released with the promise to provide complementary information from both modalities in a single examination. Due to long scan times, subject nonrigid bulk motion, i.e., changes of the patient's position on the scanner table leading to nonrigid changes of the patient's anatomy, during data acquisition can negatively impair image quality and tracer uptake quantification. A 3D MR-acquisition scheme is proposed to detect and correct for nonrigid bulk motion in simultaneously acquired PET-MR data.A respiratory navigated three dimensional (3D) MR-acquisition with Radial Phase Encoding (RPE) is used to obtain T1- and T2-weighted data with an isotropic resolution of 1.5 mm. Healthy volunteers are asked to move the abdomen two to three times during data acquisition resulting in overall 19 movements at arbitrary time points. The acquisition scheme is used to retrospectively reconstruct dynamic 3D MR images with different temporal resolutions. Nonrigid bulk motion is detected and corrected in this image data. A simultaneous PET acquisition is simulated and the effect of motion correction is assessed on image quality and standardized uptake values (SUV) for lesions with different diameters.Six respiratory gated 3D data sets with T1- and T2-weighted contrast have been obtained in healthy volunteers. All bulk motion shifts have successfully been detected and motion fields describing the transformation between the different motion states could be obtained with an accuracy of 1.71 ± 0.29 mm. The PET simulation showed errors of up to 67% in measured SUV due to bulk motion which could be reduced to less than 10% with the proposed motion compensation approach.A MR acquisition scheme which yields both high resolution 3D anatomical data and highly accurate nonrigid motion information without an increase in scan time is presented. The proposed method leads to a strong improvement in both MR and PET image quality and ensures an accurate assessment of tracer uptake.

To present and validate a manifold learning (ML)-based method that can estimate both cardiac and respiratory navigator signals from electrocardiogram (ECG)-free free-breathing cardiac magnetic resonance imaging (MRI) data to achieve self-gated retrospective CINE reconstruction.In this work the use of the ML method is demonstrated for 2D cardiac CINE to achieve both cardiac and respiratory self-gating without the need of an external navigator or ECG signal. This is achieved by sequentially applying ML to two sets of retrospectively reconstructed real-time images with differing temporal resolutions. A 1D cardiac signal is estimated by applying ML to high temporal resolution real-time images reconstructed from the acquired data. Using the estimated cardiac signal, a 1D respiratory signal was obtained by applying the ML method to low temporal resolution images reconstructed from the same acquired data for each cardiac cycle. Data were acquired in five volunteers with a 2D golden angle radial trajectory in a balanced steady-state free precession (b-SSFP) acquisition. The accuracy of the estimated cardiac signal was calculated as the standard deviation of the temporal difference between the estimated signal and the recorded ECG. The correlation between the estimated respiratory signal and standard pencil beam navigator signal was evaluated. Gated CINE reconstructions (20 cardiac phases per cycle, temporal resolution ∼30 msec) using the estimated cardiac and respiratory signals were qualitatively compared against conventional ECG-gated breath-hold CINE acquisitions.Accurate cardiac signals were estimated with the proposed method, with an error standard deviation in comparison to ECG lower than 20 msec. Respiratory signals estimated with the proposed method achieved a mean cross-correlation of 94% with respect to standard pencil beam navigator signals. Good quality visual scores of 2.80 ± 0.45 (scores from 0, bad, to 4, excellent quality) were observed for the proposed approach in comparison with the conventional ECG-gated breath-hold images (visual score: 3.00 ± 0.71).Accurate respiratory and cardiac navigator signals can be estimated using the proposed framework from the acquired data itself, resulting in retrospective self-gated CINE reconstruction with high spatial and temporal quality.

Motion occurring during magnetic resonance imaging acquisition is a major factor of image quality degradation. Self-navigation can help reduce artefacts by estimating motion from the acquired data to enable motion correction. Popular self-navigation techniques rely on the availability of a fully-sampled motion-free reference to register the motion corrupted data with. In the proposed technique, rigid motion parameters are derived using the inherent correlation between radial segments in k-space. The registration is performed exclusively in k-space using the Phase Correlation Method, a popular registration technique in computer vision. Robust and accurate registration has been carried out from radial segments composed of as few as 32 profiles. Successful self-navigation has been performed on 2-D dynamic brain scans corrupted with continuous motion for six volunteers. Retrospective motion correction using the derived self-navigation parameters resulted in significant improvement of image quality compared to the conventional sliding window. This work also demonstrates the benefits of using a bit-reversed ordering scheme to limit undesirable effects specific to retrospective motion correction on radial trajectories. This method provides a fast and efficient mean of measuring rigid motion directly in k-space from dynamic radial data under continuous motion.

Purpose Electrocardiogram (ECG)-gated cine MRI provides highly accurate functional assessment of the heart. Nevertheless, reliable ECG signals are not always available due to patient's electrophysiology or due to high MR field strengths. Here, a novel framework for cardiac functional assessment using physiological information is presented, which is obtained from MR image data. Methods Multiple long-axis slices rotated around the center axis of the left ventricle are acquired using a 2D Golden Radial acquisition scheme. This sampling approach allows for both real-time data and retrospectively reordered cine images with different temporal resolutions. Functional information from the left ventricle is used for retrospective reordering of the data to reconstruct cine images without an external ECG signal. Afterward, individual 2D cine slices are synchronized using physiological information on the mitral valve closure. The proposed approach was assessed in 15 volunteers and applied in four patients for feasibility. Results Physiological gating signals obtained with our approach show great correlation with an ECG reference signal. Functional parameters determined with the presented method show a relative difference of less than 1.3% when compared with an ECG-gated approach. Conclusion It is successfully demonstrated that functional assessment of the heart is possible using physiological information obtained directly from MR image data. In image-guided cardiac interventions, respiratory motion causes misalignments between the pre-procedure roadmap of the heart used for guidance and the intra-procedure position of the heart, reducing the accuracy of the guidance information and leading to potentially dangerous consequences. We propose a novel technique for motion-correcting the pre-procedural information that combines a probabilistic MRI-derived affine motion model with intra-procedure real-time 3D echocardiography (echo) images in a Bayesian framework. The probabilistic model incorporates a measure of confidence in its motion estimates which enables resolution of the potentially conflicting information supplied by the model and the echo data. Unlike models proposed so far, our method allows the final motion estimate to deviate from the model-produced estimate according to the information provided by the echo images, so adapting to the complex variability of respiratory motion. The proposed method is evaluated using gold-standard MRI-derived motion fields and simulated 3D echo data for nine volunteers and real 3D live echo images for four volunteers. The Bayesian method is compared to 5 other motion estimation techniques and results show mean/max improvements in estimation accuracy of 10.6%/18.9% for simulated echo images and 20.8%/41.5% for real 3D live echo data, over the best comparative estimation method.

Compressed sensing (CS) has been demonstrated to accelerate MRI acquisitions by reconstructing sparse images of good quality from highly undersampled data. Motion during MR scans can cause inconsistencies in k-space data, resulting in strong motion artifacts in the reconstructed images. For CS to be useful in these applications, motion correction techniques need to be combined with the undersampled reconstruction. Recently, joint motion correction and CS approaches have been proposed to partially correct for effects of motion. However, the main limitation of these approaches is that they can only correct for affine deformations. In this work, we propose a novel motion corrected CS framework for free-breathing dynamic cardiac MRI that incorporates a general motion correction formulation directly into the CS reconstruction. This framework can correct for arbitrary affine or nonrigid motion in the CS reconstructed cardiac images, while simultaneously benefiting from highly accelerated MR acquisition. The application of this approach is demonstrated both in simulations and in vivo data for 2D respiratory self-gated free-breathing cardiac CINE MRI, using a golden angle radial acquisition. Results show that this approach allows for the reconstruction of respiratory motion corrected cardiac CINE images with similar quality to breath-held acquisitions.

Three dimensional (3D) whole-heart magnetic resonance imaging (MRI) has become an important imaging modality to assess cardiovascular diseases. The main challenges for 3D whole-heart MRI are long acquisition times, required to achieve high spatial resolution, and image artefacts due to physiological motion. Here we propose to overcome these problems by the combination of an interleaved Radial Phase Encoding trajectory and the Phase Ordering with Automatic Window Selection method. This Radial Phase Encoding-Phase Ordering with Automatic Window Selection approach yields fast 3D whole-heart imaging with a high isotropic resolution and high navigator efficiency even for extremely irregular breathing. Numerical simulations were performed and Radial Phase Encoding-Phase Ordering with Automatic Window Selection was implemented on a clinical scanner. A comparison between the proposed method and a respiratory gated 3D Cartesian approach was carried out. Radial Phase Encoding-Phase Ordering with Automatic Window Selection leads to a better depiction of coronary arteries and an increase in navigator efficiency. In addition to a high resolution image, this method also provides dynamic respiratory information without an increase in scan time.

Applying novel detector development based on CMOS technology to Fourier domain optical coherence tomography we achieve depth profile rates up to 200,000 scans/sec. This speed allows for dramatic improvement for imaging small retinal details, such as photo-receptors and capillaries. We demonstrate the impact of this achievable speed to Doppler tomography and discuss advantages as well as short-comings of high speed 3D Doppler imaging of the human retina. Experimental data of 3D static D-FDOCT sets from fovea and nerve head region are shown together with first 4D imaging of retinal capillary flow.

An experimental design for noninvasive assessment of neural retinal tissue function with enhanced sensitivity is presented. By matching the response detection to a defined flicker frequency stimulus similar to heterodyne detection, the response signal will be shifted out of the low-frequency noise and the specificity of response detection will be strongly enhanced. Optimal measurement parameters are discussed, such as the function and timing of the response function to a single flash stimulus. The results indicate responses on the order of 200 ms that have been probed with our frequency-encoded approach using 5 Hz flickering. Preliminary results indicate the feasibility of our measurement concept to assess small changes in reflectivity with enhanced sensitivity. A functional tomogram for response localization and quantification is introduced.
