3. [Chronic cough in China: progress and perspectives]

Abstract: Great progress has been made in regard of mechanism, etiological diagnosis and treatment of chronic cough since the beginning of the 20th century, and Chinese guidelines of diagnosis, management of cough was developed. Moreover, Chinese experts also participated in the formulation of international cough guidelines. Through the promotion of the guidelines, the level of diagnosis and treatment of chronic cough has been significantly improved in China. In the future, we should strengthen the studies on the epidemiology of chronic cough, the central regulation of chronic cough, and the treatment of refractory chronic cough, and promote the cough guidelines in primary community hospitals.中国于20世纪初开始了慢性咳嗽的发病机制、病因诊断与治疗方面的系统研究，取得了一系列成果；结合研究成果制订了中国《咳嗽的诊断与治疗指南》，并参与国际咳嗽指南的制订。通过指南的推广普及，国内慢性咳嗽的诊治水平有了显著的提高。展望未来，我们要加强慢性咳嗽的流行病学、慢性咳嗽的中枢调控、难治性慢性咳嗽的治疗等方面的研究，进一步在基层社区医院推广咳嗽指南。.

4. [Epidemiology of chronic cough in China: current status and future perspective]

Abstract: Chronic cough is one of the most common major complaints in medical outpatient. Chronic cough not only seriously affects quality of personal life, but also burdens public health. Large-scale and high-quality epidemiological study on chronic cough has not been carried out in China, and relevant reviews are also lacking. Therefore, based on the studies concerning epidemiology of chronic cough in China, we reviewed the prevalence, risk factors, etiology, quality of life and economic burden. In addition, future perspectives and reasonable suggestions for the development of epidemiology of chronic cough were also proposed.慢性咳嗽是内科门诊最常见的主诉之一，不仅严重影响患者生活质量，还给社会造成了巨大的公共卫生资源负担。目前我国仍未开展大规模的、高质量的慢性咳嗽流行病学研究，相关综述也比较缺乏。本文结合国内已发表的慢性咳嗽流行病学有关文献，分别从患病率、危险因素、病因分布、生活质量状况、经济负担5个方面进行了综述，并对慢性咳嗽流行病学学科发展进行了展望及建议。.

5. Reduced long noncoding RNA PGM5-AS1 facilitated proliferation and invasion of colorectal cancer through sponging miR-100-5p

Abstract: We aimed at investigating the expression of Long non-coding RNA (LncRNA) PGM5-AS1 and its facilitating effects on proliferation and invasion of colorectal cancer by sponging miR-100-5p.qRT-PCR was performed to detect the expressions of PGM5-AS1 and SMAD4 in human colorectal cancer tissues and cells. CCK-8 assay was performed to evaluate the SW403 cells proliferation and transwell assay was performed to evaluate the SW403 cells migration. The correlation between miR-100-5p and PGM5-AS1 was detected by statistical analysis. Bioinformatics prediction and Luciferase assay were performed to explore the interaction and binding site of PGM5-AS1 and miR-100-5p, miR-100-5p and SMAD4, respectively.We found that both PGM5-AS1 and SMAD4 were downregulated in human colorectal cancer tissues and cells. qRT-PCR and CCK-8 assay showed that PGM5-AS1 expression is associated with the proliferation of colorectal cancer cells. Transwell assay showed that PGM5-AS1 regulated the migration ability of colorectal cancer cells. The bioinformatics prediction and Luciferase assay demonstrated that by sponging miR-100-5p, PGM5-AS1 can serve as a molecular sponge to further regulate the expression of SMAD4.In this study, we found that lncRNA-PGM5-AS1 was low expressed in human colorectal cancer cells, which could promote tumor proliferation, migration and invasion by serving as a molecular sponge and by modulating the inhibitory effect of miR-100-5p on tumor suppressor gene SMAD4.

Abstract: Publisher's version (útgefin grein), Cancer is driven by genetic change, and the advent of massively parallel sequencing has enabled systematic documentation of this variation at the whole-genome scale1,2,3. Here we report the integrative analysis of 2,658 whole-cancer genomes and their matching normal tissues across 38 tumour types from the Pan-Cancer Analysis of Whole Genomes (PCAWG) Consortium of the International Cancer Genome Consortium (ICGC) and The Cancer Genome Atlas (TCGA). We describe the generation of the PCAWG resource, facilitated by international data sharing using compute clouds. On average, cancer genomes contained 4–5 driver mutations when combining coding and non-coding genomic elements; however, in around 5% of cases no drivers were identified, suggesting that cancer driver discovery is not yet complete. Chromothripsis, in which many clustered structural variants arise in a single catastrophic event, is frequently an early event in tumour evolution; in acral melanoma, for example, these events precede most somatic point mutations and affect several cancer-associated genes simultaneously. Cancers with abnormal telomere maintenance often originate from tissues with low replicative activity and show several mechanisms of preventing telomere attrition to critical levels. Common and rare germline variants affect patterns of somatic mutation, including point mutations, structural variants and somatic retrotransposition. Abstract: Blunt vertebral artery injury occurs frequently in forensic practice. However, injuries of the vertebral artery are easily ignored or overlooked because of its relatively deep location. Through literatures review, this paper finds that the manners of blunt vertebral artery injury are varied and one or more injury mechanisms may be involved simultaneously. Patients often undergo immediate or delayed cerebral apoplexy as well as compression and injury of surrounding structures, due to direct injury or secondary aneurysm or dissection, resulting in disability or death. Diseases such as, vertebral atherosclerosis and dysplasia can increase the disability and death risk and the difficulty of forensic identification. In forensic identification, the details of the case should be considered. For cases of suspected vertebral artery injury, in addition to routine examination of intracranial segment, attention should be paid to the examination of extracranial segment. Abstract: Aim: This study was to investigate the change of high-molecula-weight (HMW) adiponectin (APN) isoform, the association between type 2 diabetes mellitus (T2DM) and HMW APN isoform, the variation of Disulfide-bond A oxidoreductase-like protein (DsbA-L), the effect of HMW APN isoform on AMP-dependent protein kinase (AMPK) and endothelial nitric oxide synthase (eNOS) in Chinese T2DM. Method: 169 patients aged at (48.7±9.4) years and 107 healthy control subjects aged at (42.6±7.8) years took part in this study. Anthropometric measures of the characters were assayed and different APN isoforms, DsbA-L, AMPK and eNOS levels were determined. Results: Ln(sRAGE) and Ln(Adiponectin) were significantly lower and significantly higher for the other characteristics in T2DM. Ln(Adiponectin) was negatively and significantly correlated with WHR, Ln(triglycerides), fasting plasma glucose, HbA1c (%) in control subjects and T2DM patients. Plasma and adipose tissue total APN and HMW APN were significantly reduced in newly diagnosed T2DM patients. DsbA-L was markedly down-regulated in diabetic adipose tissue. HMW APN caused significant decreases in AMPK and eNOS phosphorylation levels of human umbilical vein endothelial cells (HUVECs). Conclusions: Our results demonstrated that total APN levels was closely related to the risk of T2DM and HMW APN reduction was involved in the diabetic vascular AMPK/eNOS signal pathway. The findings will provide insight into novel therapeutic approaches for reducing the elevated cardiovascular risk associated with T2DM.

Abstract: Bladder cancer is the 2nd most common reason for human genitourinary cancer-associated mortality. This study aimed to investigate the effects of Nanoscale bubble ultrasound contrast agents-mediated yeast-cytosine-deaminase-thymidine kinase/ganciclovir (YCD-TK/GCV) or YCD-TK/5-fluorocytosine (5-FC) suicide gene therapy system on BIU-87 cell growth.Targeted nanoscale bubble ultrasound contrast agents were prepared by utilizing thin-film hydration-sonication approach. Nanoscale bubble-LV5-YCD-TK/GCV(5-FC) was constructed and transfected to BIU-87 cells. Hematoxylin and eosin (HE) staining was used to evaluate inflammation. 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2-H-tetrazolium bromide (MTT) assay was used to examine cell viability. Cell-cycle distribution was analyzed with cell cycle assay. Flow cytometry assay was utilized to test apoptosis of BIU-87 cells. YCD-TK expression was examined using Western blot and quantitative Real Time-PCR (qRT-PCR), respectively.YCD-TK highly expressed in Nanoscale bubble mediated suicide gene therapy system. Nanoscale bubble-mediated suicide gene therapy system significantly induced inflammatory response and apoptosis compared to that of Nanoscale bubble group (p0.05). Nanoscale bubble mediated suicide gene therapy system significantly reduced cell viability compared to that of the Nanoscale bubble group (p0.05). Nanoscale bubble mediated suicide gene therapy system significantly inhibited cell cycle arrest compared to that of the Nanoscale bubble group (p0.05). Nanoscale bubble-LV5-YCD-TK/GCV/5-FC therapy system significantly reduced BIU-87 cell viability compared to that of the Nanoscale bubble-associated groups (p0.05).Nanoscale bubble-mediated suicide gene therapy system, bubble-LV5-YCD-TK/GCV/5-FC, acts as a novel therapeutic strategy for bladder cancer treatment.

Abstract: With the emergence of new technologies and devices including minimally invasive catheters and rotary couplers, the application of imaging technology such as traditional ultrasound and optical coherence tomography （OCT） is gradually expanded. In recent years, intravascular ultrasound （IVUS） and OCT have become increasingly mature as coronary intravascular imaging techniques, and therefore become an important complementary means of coronary angiography. Although studies on feasibility of clinical applications of IVUS and OCT have been confirmed in the evaluation of previous cadaver studies, these techniques have been neglected in forensic autopsy. This paper reviews the application value of IVUS and OCT in forensic autopsy, especially in the adjuvant evaluation of coronary artery disease. Abstract: Vascular calcification is one of the most important factors for high morbidity and mortality from cardiovascular and cerebrovascular diseases. The aim of this study is to investigate the effect and mechanism of miR-26a on vascular smooth muscle cell calcification. First, the VSMCs were induced by β-glycerol phosphate (β-GP) for 7d and 14d, and Alizarin Red S staining was performed to examine the mineralized nodule change; then real time RT-PCR and western blotting were performed to explore the expression of miR-26a, CTGF, OPG, RANKL and ALP in un-induced and β-GP-induced VSMCs; next, the VSMCs were transfected with miR-26a mimics, and Alizarin Red S staining was performed to examine the mineralized nodule change; finally, real time RT-PCR and western blotting were performed to explore the expression of miR-26a, CTGF, OPG, RANKL and ALP in un-transfected and miR-26a mimics transfected VSMCs. After β-GP treatment, β-GP promoted clear mineralized nodule changes, and miR-26a and OPG expression were signiﬁcantly decreased and CTGF, RANKL and ALP expression were increased in VSMCs. Overexpression of miR-26a inhibited VSMCs calcification induced by β-GP, and regulated the expression of CTGF, OPG, RANKL and ALP. Our findings suggested that up-regulation of miR-26a before β-GP treatment inhibits VSMCs calcification through targeting CTGF (Fig. 4, Ref. 18).

Abstract: We used clustered regularly interspaced short palindromic repeats/Cas9-mediated genomic modification to investigate B-cell receptor (BCR) signaling in cell lines of diffuse large B-cell lymphoma (DLBCL). Three manipulations that altered BCR genes without affecting surface BCR levels showed that BCR signaling differs between the germinal center B-cell (GCB) subtype, which is insensitive to Bruton tyrosine kinase inhibition by ibrutinib, and the activated B-cell (ABC) subtype. Replacing antigen-binding BCR regions had no effect on BCR signaling in GCB-DLBCL lines, reflecting this subtype's exclusive use of tonic BCR signaling. Conversely, Y188F mutation in the immunoreceptor tyrosine-based activation motif of CD79A inhibited tonic BCR signaling in GCB-DLBCL lines but did not affect their calcium flux after BCR cross-linking or the proliferation of otherwise-unmodified ABC-DLBCL lines. CD79A-GFP fusion showed BCR clustering or diffuse distribution, respectively, in lines of ABC and GCB subtypes. Tonic BCR signaling acts principally to activate AKT, and forced activation of AKT rescued GCB-DLBCL lines from knockout (KO) of the BCR or 2 mediators of tonic BCR signaling, SYK and CD19. The magnitude and importance of tonic BCR signaling to proliferation and size of GCB-DLBCL lines, shown by the effect of BCR KO, was highly variable; in contrast, pan-AKT KO was uniformly toxic. This discrepancy was explained by finding that BCR KO-induced changes in AKT activity (measured by gene expression, CXCR4 level, and a fluorescent reporter) correlated with changes in proliferation and with baseline BCR surface density. PTEN protein expression and BCR surface density may influence clinical response to therapeutic inhibition of tonic BCR signaling in DLBCL.

16. Serum human epididymis protein 4 vs. carbohydrate antigen 125 and their combination for endometrial cancer diagnosis: a meta-analysis

Abstract: Cancer antigen 125 (CA125) and Human epididymis protein 4 (HE4) appear to be promising predictors for endometrial cancer (EC). However, conflicting results exist in the diagnostic performance comparison among CA125 and HE4.A systematic review was conducted using PubMed, EMBASE and other databases till December 2015. All studies included were closely assessed with the QUADAS. Diagnostic value of HE4, CA125 and HE4+CA125 was systematically evaluated, and comparison among the predictive performances of HE4, CA125 were conducted. Sensitivity, specificity, DOR (diagnostic odds ratio), and area under the SROC curve were summarized with a random model. Meta-regression was used to explore the heterogeneity.8 studies including 1832 cases (1129 in the study group and 703 in the control group) were included in our meta-analysis. Mean estimates of HE4 and their 95% CIs were: sensitivity 0.53 (95% CI: 0.50-0.56), specificity 0.91 (95% CI: 0.89-0.93), DOR 17.01 (95% CI: 7.88-36.72). The area under the SROC curve of HE4 in the diagnosis of EC was 0.77. However, CA125 had lower sensitivity, specificity, DOR, and the area under the SROC in diagnosis of EC with sensitivity 0.26 (95% CI: 0.24-0.29), specificity 0.81 (95% CI: 0.78-0.84), DOR 2.61 (95% CI: 0.92-7.41), and the area under the SROC 0.37. In patients with EC diagnosed by HE4+CA125, the overall sensitivity was 0.58 (95% CI: 0.54-0.62) and a specificity of 0.92 (95% CI: 0.89-0.94) in predicting EC. DOR and the area under the SROC curve of HE4+CA125 for diagnosis of EC were 21.86 (95% CI: 11.08-43.15) and 0.83 respectively, which showed a higher level of diagnostic accuracy than HE4 alone.HE4 is helpful for distinguishing EC from healthy and benign disease. CA125 is not better than HE4 either for EC diagnosis. HE4+CA125 is promising a predictor of EC to replace He4, but its utilization requires further exploration.

Abstract: Internal carotid arterial system is the main source of blood supply in brain. In forensic identification practice, blunt injures of internal carotid arterial system usually cause complications such as thrombus, aneurysm and arteriovenous fistula, etc. The deaths following delayed cerebral infarction or intracranial haemorrhage are not rare. At present, literature of deaths caused by blunt injuries of internal carotid arterial system mainly consist of case reports in China. This paper reviews related literature and case reports at home and abroad, and summarizes forensic medical features and identification method of the deaths caused by such injuries. The results show that blunt injures of internal carotid arterial system are related to the direct or indirect force on head and neck, which can result in exceed physiological range traction of head and neck, incision following basicranial fracture, etc. Such injuries are common in the cases as mechanical asphyxia, cervical manipulation, traffic accident and fall, etc. The artery should be examined carefully, and the relationship among injury, disease and death should be analysed correctly when no cause of infarction and hemorrhage was found in routine examination of such cases in forensic pratice. Because of the difficulty for exposing the artery completely in autopsy, angiography can be used to infer the location when necessary for improving the scientificity and reliability of the appraisal conclusion.

Abstract: The exact role of follicle-stimulating hormone (FSH) and luteinizing hormone (LH) in ovarian epithelial carcinoma (OEC) development has not been yet characterized. This prompted us to identify particular proteins to better understand the underlying mechanism. Total proteins from ovarian epithelial tumor (OET) cells treated with gonadotropins were analyzed by proteomics. Western blot and immunohistochemistry were used to validate the target protein (prohibitin) and to detect its expression in human ovarian tissue of serous tumors. As the results, prohibitin was found to be significantly up-regulated by LH, with a maximum of 2.5-fold increase at the concentration of 200 mIU/mL. The expression of prohibitin was steadily decreased from benign serous cystadenomas to borderline tumors and serous carcinomas (P < 0.0001). The difference between any two groups was significant (P < 0.001). Collectively, data from this study indicate that prohibitin is one LH-associated protein and it may be protective of ovarian cancer development and progression, supporting that LH may play an inhibitory role in ovarian tumorigenesis.

20. Generation of mammalian cell lines with gene knock-down for human MCHR2

Abstract: Appetite regulating neuropeptide melanin-concentrating hormone (MCH) has been implicated in obesity. It functions through its two receptors MCHR1 and MCHR2. While MCH and MCHR1 have been studied more extensively, the function of MCHR2 remains largely unknown, due to the lack of suitable in vitro and in vivo models. To create an in vitro system of genetic knock-down of MCHR2 in mammalian cells, we constructed four small hairpin RNAs (shRNAs) against human MCHR2 in eukaryotic expression vector, and transfected the plasmids into CHO cells that stably express human MCHR2. Using the empty vector or a negative shRNA control plasmid, we show that MCHR2-shRNAs suppressed 45.8% - 66.4% of MCHR2 expression at both mRNA and protein levels. As the result, in cells carrying the MCHR2-shRNAs, binding of MCHR2 to MCH was decreased by 39.4% - 78.7% accompanied by a similar decrease in affinity of the receptor to ligand by 40.9% - 81.9%. These cells still respond to MCH treatment, but intracellular Ca2+ release as the downstream signaling event was also decreased by 114.8% - 822.4%. Together, this study generated a set of shRNAs and cell lines as valuable reagents for further study on MCHR2 functions. These results will ultimately help to advance our knowledge about appetite regulating neuropeptide receptors.

Abstract: 44 cases of adrenal hemorrhage (AH) were analyzed. The forensic autopsy showed that AH occurred mainly in male aged from 20 to 40 years old. The traumatic AH was more often seen in young men and the pathologic AH was mostly seen in elderly persons. AH were more likely found in bilateral adrenal corticomedullary junction. The pathological grade of AH was not related to the interval time of survival. The pathologic mechanism of AH was discussed as well.

Abstract: Statistics from a 64 case study showed that mucinous adenocarcinoma was apt to invade the intestinal wall and to metastasize to lymph nodes (P0.05). The activity of arylsulfatase and lysozyme of mucinous adenocarcinoma was stronger than that of the papillary and tubular adenocarcinoma (P0.05). In RR staining for electron microscopic observation, a significant decrease of proteoglycan granules was found in the surrounding matrix of mucinous adenocarcinoma, which correlated with the amount of arylsulfatase and lysozyme secreted by mucinous adenocarcinoma. These enzymes reduced the degree of sulfation in heparan sulfate and degraded proteoglycans. The proteoglycan structural barrier having been destroyed, facilitates mucinous adenocarcinoma to infiltrate and metastasize.

Abstract: The light emitting diode (LED) stimulator was used to record PVEP from 59 eyes (38 patients) with optic neuropathy and 46 eyes (23 patients) with cataract. The amplitude and the latency of P100 in LED PVEP were comparable to those in TV PVEP with insignificant differences (P0.05), suggesting that LED PVEP may be used in the evaluation of visual function and prognosis of patients with optic neuropathy or cataract.

Abstract: In order to clarify the relationship between hydrolases and the invasion of gastric carcinoma, both fibronectin and proteoglycan in pericancerous matrix and beta-galactosidase activity in gastric carcinoma were investigated by means of histochemical, immunohistochemical and ruthenium red electrocytochemical stains. The results showed that the activity of beta-galactosidase in mucous cell carcinoma was more intensive than that in well-differentiated and poorly-differentiated adenocarcinoma. RR granules and fibronectin were not found in the pericancerous matrix close to the mucous cell carcinoma, but were obtained in the region far from the mucous cell carcinoma. Nevertheless, both of them were present and intact near the well-differentiated adenocarcinoma. The result of this study suggests that mucous cell carcinoma may secrete beta-galactosidase into the surrounding matrix, inducing degradation of proteoglycan and fibronectin in favour of further infiltration and metastasis.

25. Reliability of compact electronic blood pressure monitors for hypertensive pregnant women

Abstract: Self-monitoring of blood pressure is an asset to managing hypertensive pregnant women on an outpatient basis. Many brands of electronic blood pressure devices are now available for use during pregnancy. An investigation was undertaken to compare the relative accuracy and acceptability of four of the least expensive and most compact units (Sunbeam III, Norelco, Lumiscope and Marshall). The reliability of the devices was compared with simultaneous measurements using a Dinamap 845A monitor on 22 pregnant hypertensive women. All measurements were performed without difficulty by a skilled observer and the patients. The accuracy varied, but each electronic device was considered as accurate as a mechanical aneroid unit available at the same retail stores. The advantages of these electronic units were ease of application and readability of the measurements without the need for a stethoscope or an assistant.
