Asthma does not appear to be a risk factor for developing COVID-19.The objective of the study was to analyze the role of asthma as a factor associated with COVID-19 among healthcare workers (HW).A crosssectional study was conducted in HW from a Mexican hospital. Data were obtained through an epidemiological survey that included age, sex, and history of COVID-19. Multivariate logistic regression analysis was performed to identify factors associated with COVID-19.In total, 2295 HW were included (63.1% women; mean age 39.1 years); and 1550 (67.5%) were medical personnel. The prevalence of asthma in HW with COVID-19 was 8.3%; for the group without COVID-19, the prevalence was 5.3% (p = 0.011). The multivariate analyses suggested that asthma was associated with COVID-19 (OR 1.59, p = 0.007).Our study suggests that asthma could be a factor associated with COVID-19 in HW.

Objective: To determine the frequency of anxiety and depression in adult patients with asthma and to identify factors associated with them.Methods: This cross-sectional study included 164 consecutively recruited patients with asthma aged ≥ 18 years. Participants were clinically assessed and the mental state was analyzed through of the Beck Anxiety Inventory (BAI), the Beck Depression Inventory II (BDI-II); the Asthma Control Test (ACT) was measured, and allergic sensitization and respiratory function were also evaluated. Binary logistic regression models to identify the factors related to anxiety or depression were carried out. Complementary, 95% confidence intervals (CI) were estimated.Results: Anxiety was present in 54.3% of patients (95% CI: 46.6%-61.7%) while depression was found in 50.6% (95% CI: 43.0%-58.2%). Depression increased the odds of anxiety in patients with asthma (OR: 26.00, p < 0.0001), whereas an ACT score ≥ 20 points and allergic asthma were associated with a lower odds than the reference group (OR: 0.29; p = 0.007 and OR: 0.29; p = 0.024, respectively). Depression was associated with anxiety and obesity (OR: 25.33, p < 0.0001 and OR: 3.66; p = 0.014, respectively).Conclusions: Overall, more than half of all asthmatic patients suffer from anxiety and depression. Well-controlled asthma and allergic sensitization decreased the likelihood of anxiety, while depression was associated with both obesity and anxiety.

Objective. This study aims to assess the association between asthma and severity of COVID-19 in the Mexican population.Methods. The data from a national database of confirmed patients diagnosed with COVID-19, who attended from February to June 2020, were analyzed in a retrospective cohort study. Patients with and without asthma were compared concerning hospitalization, pneumonia, endotracheal intubation, and death related to COVID-19. Other covariates (age, sex, indigenous group, and comorbidity) were included in various logistic regression models.Results. Asthma was associated with a lower risk of hospitalization (OR =0.71, 95% CI 0.66 to 0.76), lower risk of pneumonia (OR =0.75, 95% CI 0.69 to 0.81), and lower risk of endotracheal intubation (OR =0.79, 95% CI 0.63 to 0.98). In addition, asthma decreased the risk of death from COVID-19 (OR =0.73, 95% CI 0.65 to 0.82). In a subgroup analysis, the same association was observed in patients who required hospitalization (OR =0.79, 95% CI 0.69 to 0.90), while in non-hospitalized patients, associations were inconsistent according to the covariates introduced in the models. There was no association between asthma and death in patients admitted to the intensive care unit (ICU), however, asthma significantly reduced the risk of death in the hospitalized patients who did not require ICU.Conclusion. Our results suggest that patients with asthma are less likely to require hospitalization, develop pneumonia, need tracheal intubation or die from COVID-19 as compared to patients without asthma.

Introduction Young women under 30 years with breast cancer (BC) are an emerging challenge. The purpose is to identify prognostic factors for survival in young women under 30 years of age with BC. Material and methods A retrospective cohort study was conducted among women younger than or equal to 40 years with BC and who were treated at the State Cancer Center during the period 2012–2017. Overall survival was assessed using the Kaplan–Meier method and the log-rank test. Univariate and multivariate analysis assessed survival predictors using Cox proportional hazards regression model. Results 282 young women were included. The >30-year-old subgroup showed a significant association with excess weight ( P = .002) compared to the 5 cm HR = 2.3; 95% CI 1.03 to 5.1), and advanced clinical stages (HR = 6.6 95% CI 1.3 to 35.5) persisted as predictors. To determine the prevalence of blood eosinophilia in adults with chronic obstructive pulmonary disease (COPD) according to different cut-off points.A cross-sectional study was carried out in patients with COPD. The frequency of blood eosinophilia was determined by absolute (cells/ µL) and relative (%) eosinophil count. Multivariate methods were used in order to identify the associated factors.81 patients were included; the mean age was 71.9 ± 9.8 years; 46 (57%) of the patients were men. The prevalence of eosinophilia for the cut-off points of ≥ 100, ≥ 150, ≥ 200, ≥ 300, and ≥ 400 cells/µL was of 64.2%, 37.0%, 16.1%, and 9.9% respectively. Out of 81 patients, 34 (42%) had a relative eosinophil concentration of ≥ 2%; 21 (25.9%) ≥ 3%; 14 patients (17.3%) had ≥ 4%; and 10 patients (12.3%) had ≥ 5%. Eosinophilia of ≥ 100 cells/µL was associated with age of ≥ 80 years (OR = 6.04, p = 0.026), and with the exacerbation of COPD (OR = 9.40, p = 0.038); in contrast, eosinophilia of ≥ 2% was associated only with age of ≥ 80 years (OR = 3.73, p = 0.020). In addition, the eosinophil count of ≥ 100 and300 cells/µL was associated with the exacerbation of COPD (OR = 11.00, p = 0.026).Our results suggest that the frequency of eosinophilia in the context of COPD shows substantial variations according to the used definition.Objetivo: Establecer la prevalencia de eosinofilia en sangre en adultos con enfermedad pulmonar obstructiva crónica (EPOC) según varios puntos de corte. Métodos: Se realizó un estudio transversal en pacientes con EPOC. La frecuencia de eosinofilia en sangre se determinó a partir de la concentración absoluta (células/µL) y relativa (%) de eosinófilos. Fueron realizados modelos multivariados para identificar factores asociados. Resultados: En 81 pacientes incluidos, la edad promedio fue de 71.9 ± 9.8 años; de los cuales, 46 (57 %) fueron hombres. La prevalencia de eosinofilia para los puntos de corte ≥ 100, ≥ 150, ≥ 200, ≥ 300 y ≥ 400 células/µL fue de 64.2, 43.2, 37.0, 16.1 y 9.9 %, respectivamente. De 81 pacientes, 34 (42 %) tuvieron una concentración ≥ 2 %; 21 (25.9 %) ≥ 3 %; 14 (17.3 %) ≥ 4 %; y 10 (12.3 %) ≥ 5 %. The impact of obesity on the severity of asthma continues to be a cause of controversy.To compare the severity of asthma and asthma control in obese patients with non-obese patients.A cross-sectional study which included 188 patients with asthma, of ≥ 18 years of age, who were selected consecutively. The patients were categorized and compared based on the presence or absence of obesity. In order to establish an association between variables, logistic regression analyses were performed.In total, 111/188 of the patients had obesity; these patients were older than those without obesity (35.9 ± 15.3 years vs. 44.4 ± 13.8 years, p0.001). No significant difference was observed in the groups regarding sex, total IgE serum concentration, peripheral blood eosinophil count, personal history of atopic diseases, frequency of allergic sensitization, hospitalization for asthma, and asthma control. Overall, obesity was significantly associated with moderate-severe asthma (OR = 1.82, p = 0.047), but not with asthma control (p = 0.094).Our results suggest that patients with asthma and obesity are older and that their pulmonary function is worse, which is reflected in a greater level of severity of asthma.Antecedentes: El impacto de la obesidad sobre la gravedad del asma contiúa siendo motivo de controversia. Objetivo: Comparar la gravedad y el control del asma entre pacientes con obesidad y sin obesidad. Métodos: Estudio transversal que incluyó a 188 pacientes con asma, con edad ≥ 18 años, seleccionados consecutivamente. Los pacientes fueron categorizados y comparados de acuerdo con la presencia de obesidad o no. Para establecer asociación entre variables se realizó análisis de regresión logística. Resultados: En total, 111 pacientes tenían obesidad; su edad fue mayor que la de los pacientes sin obesidad (35.9 ± 15.3 años versus 44.4 ± 13.8 años, p0.001). No se observó diferencia significativa entre los grupos respecto a sexo, concentración sérica total de IgE, número de eosinófilos en sangre periférica, historia personal de enfermedad atópica, frecuencia de sensibilización alérgica, hospitalización por asma y control del asma. En general, la obesidad se asoció significativamente con el asma moderada o grave (RM = 1.82, p = 0.047), pero no lo hizo con el control del asma (p = 0.094). Conclusiones: Nuestros resultados sugieren que los pacientes con asma y obesidad tienen mayor edad y peor función pulmonar, la cual se reflejada en mayor gravedad del asma.

Objective To assess whether loss of energy and concentration difficulty are related to allergic rhinitis. Design A cross-sectional study. Setting Secondary care allergy outpatients. Participants A total of 100 patients with allergic rhinitis and 96 healthy subjects. Main Outcome Measure: Loss of energy and concentration difficulty were assessed through a survey using the Beck Depression Inventory-II (BDI-II). Peanut allergy among adults with respiratory diseases has seldom been studied within Mexico.To establish the prevalence of peanut allergy among adults that have been diagnosed with either asthma or allergic rhinitis; we will also be describing the symptoms that are associated with peanut allergy.We carried out a cross-sectional study through which we analyzed the corresponding data of 257 patients with allergic respiratory diseases, asthma or allergic rhinitis, all participants were 16 years of age or older, and were recruited in a consecutive manner. Peanut allergy was established by testing positive to a peanut skin-prick test; we also conducted a standard interview with each patient.From our sample of 257 patients, 18 tested positive to peanut sensitization, (7.0%; 95% CI = 3.9% to 10.1%); among these 18 participants, 7 were considered to be allergic to peanuts (2.7%; 95% IC: 0.7% to 4.7%). Predominant symptoms were oral, primarily affecting the pharynx and the palate, followed by swelling of the lips. When it came to respiratory discomfort, sneezing and rhinorrhea stood out, and lastly there were cutaneous symptoms. We did not detect any systemic reactions to the peanut.In our study, peanut allergy among adults with allergic respiratory diseases is not an uncommon occurrence.Antecedentes: La alergia al cacahuate en adultos con enfermedades respiratorias alérgicas pocas veces ha sido estudiada en México. Objetivo: Establecer la prevalencia de alergia al cacahuate en un grupo de adultos con asma o rinitis alérgica; también se describen los síntomas asociados con la alergia al cacahuate. Métodos: Se realizó estudio transversal en el que analizaron los datos correspondientes a 257 pacientes con enfermedades respiratorias alérgicas, asma o rinitis alérgica, con edad igual o mayor a 16 años; los pacientes fueron reclutados consecutivamente. La alergia al cacahuate se determinó por una prueba cutánea positiva al cacahuate y a través de una entrevista estandarizada. Se estimaron intervalos de confianza (IC) a 95 % para proporciones. Resultados: De los pacientes incluidos, 18 estuvieron sensibilizados al cacahuate (7.0 %, IC 95 % = 4.4-10.9), siete de ellos fueron considerados alérgicos al cacahuate, para una prevalencia de 2.7% (IC 95%: 1.2% - 5.6%). En los pacientes con alergia al cacahuate, los síntomas predominantes fueron los orales, principalmente el prurito en la faringe y en el paladar, seguidos de edema de los labios; entre las molestias respiratorias sobresalieron los estornudos y la rinorrea y, al final, los síntomas cutáneos. No se documentaron reacciones sistémicas al cacahuate. Conclusión: En nuestro estudio, la alergia al cacahuate en adultos con enfermedades respiratorias alérgicas no fue infrecuente.

In Mexico, there are very few studies aimed at establishing the prevalence of allergic rhinitis (AR) and atopic dermatitis (AD) in late adolescents.To determine the prevalence of allergic rhinitis and atopic dermatitis in late adolescents and compare it according to their gender.A cross-sectional study that included 1992 15 to 18 year old adolescents who were recruited through a stratified and randomized probabilistic sample by clusters. The prevalence of AR (allergic rhinitis) and AD (atopic dermatitis) was identified with the questionnaire of "The International Study of Asthma and Allergies in Childhood".1056 (53%) women and 936 (47%) men were included in the study. The prevalence of AR was 9.0% (95% CI = 7.8% - 10.4%); the frequency was higher in women (6.7% versus 11.1%, p = 0.001). The symptoms of rhinitis plus conjunctivitis predominated in women (16.0% versus 23.1%, p0.0001). The prevalence of AD was 5.2% (95% CI = 4.3% - 6.2%) and, once again, it was more frequent in women (7.7% versus 2.4%, p0.0001). The main factors associated with AR and AD were the female gender and a history of family atopic allergic diseases (p0.001).There was a significant difference in the prevalence of AR and AD according to gender in late adolescents; women were most affected. In addition, a history of family atopic allergic diseases was another risk factor that was linked to both diseases.Antecedentes: En México son escasos los estudios encaminados a establecer la prevalencia de la rinitis alérgica y la dermatitis atópica en adolescentes tardíos. Objetivo: Determinar la prevalencia de rinitis alérgica y dermatitis atópica en adolescentes tardíos y compararla conforme el sexo. Métodos: Estudio transversal de 1992 adolescentes de 15 a 18 años, reclutados mediante muestreo probabilístico por conglomerados, estratificado y aleatorizado. Las prevalencias de rinitis alérgica y dermatitis atópica se identificaron con el cuestionario propuesto en The Internacional Study of Asthma and Allergies in Childhood. Resultados: Se incluyeron 1056 mujeres (53 %) y 936 hombres (47 %). La prevalencia de rinitis alérgica fue de 9.0 % (IC 95 % = 7.8-10.4); la frecuencia fue mayor en las mujeres (6.7 % versus 11.1 %, p = 0.001). Los síntomas de rinitis más conjuntivitis predominaron en las mujeres (16.0 % versus 23.1 %, p0.0001). La prevalencia de dermatitis atópica fue de 5.2 % (IC 95 % = 4.3-6.2); fue más frecuente en las mujeres (7.7 % versus 2.4 %, p0.0001). Los principales factores asociados con rinitis alérgica y dermatitis atópica fueron sexo femenino y atopia familiar (p0.001). Conclusiones: Se registró diferencia significativa en la prevalencia de rinitis alérgica y dermatitis atópica conforme el sexo en los adolescentes tardíos; las mujeres fueron las más afectadas. La atopia familiar fue otro factor asociado.

Background The prevalence of food allergy is on the rise on a global scale. Objective To determine the prevalence of food hypersensitivity (FHS) and probable food allergy (PFA), as well as the foods and factors associated with these occurrences. Methods A cross-sectional study was carried out among 1992 adolescents (aged 15–18 years). Each adolescent answered a structured questionnaire. A multivariate analysis was used to identify the association between the variables. Results The prevalence of FHS was 10.6% (the most commonly associated foods were shrimp, cow's milk and avocado) and the PFA was 7.8% (shrimp, cow's milk and pecan). The prevalences of oral allergy syndrome, food-associated urticaria and systemic reaction were 4.9%, 3.6% and 1.5%, respectively. The following factors were associated with FHS: personal history of asthma (OR 1.63; 95% CI: 1.11–2.41), allergic rhinitis (OR 2.60; 95% CI: 1.75–3.87), atopic dermatitis (OR 2.07; 95% CI: 1.25–3.43), maternal history of asthma (OR 1.80; 95% CI: 1.02–3.16), atopic dermatitis (OR 6.11; 95% CI: 2.45–15.29), and female sex (OR 1.89; 95% CI: 1.38–2.59). PFA was associated with a personal history of asthma (OR 1.65; 95% CI: 1.06–2.56), allergic rhinitis (OR 2.46; 95% CI: 1.56–3.88), atopic dermatitis (OR 2.02; 95% CI: 1.15–3.54), paternal allergic rhinitis (OR 2.52; 95% CI: 1.15–5.51), maternal atopic dermatitis (OR 7.46; 95% CI: 2.93–19.00), and female sex (OR 1.89; 95% CI: 1.31–2.72). Conclusion The adverse reactions associated with foods among late adolescents are a frequent occurrence, and the most commonly associated factor is atopy.

Food allergy is considered a typical problem in the pediatric population, however, cases occur in adults more frequently.To determine the prevalence of food allergy in patients with allergic rhinitis and asthma; as well as the foods and symptoms most related to said problem.Through a cross-sectional study, the information of 257 adults with asthma or allergic rhinitis was analyzed. The information related to food allergy was obtained through a standardized interview; the confidence intervals (CI) were calculated for 95% proportions.The prevalence of food allergy was 17.5% (95% CI = 13.3% to 22.6%). The main foods that caused symptoms were soy, shrimp, almonds, avocado and peanuts. The majority of patients who were allergic to a single food were 35.6%, two foods were 17.8% and three foods were 17.8%. The most frequent affections of food allergy were oral (62.2%): itching of the pharynx, tongue and palate; respiratory (26.7%), dyspnea and sneezing; and skin (26.7%), itching and hives.Food allergy in adults with asthma or allergic rhinitis is a common problem. Oral symptoms are the main ailments in this type of patients and are mainly caused by soy.Antecedentes: La alergia a alimentos se considera un problema típico de la población pediátrica, sin embargo, los adultos se ven afectados cada vez con mayor frecuencia. Objetivo: Determinar la prevalencia de alergia a los alimentos en pacientes con rinitis alérgica y asma, así como los alimentos y los síntomas más relacionados con este problema. Métodos: Mediante un estudio transversal se analizaron los datos de 257 adultos con asma o rinitis alérgica. La información relacionada con la alergia a los alimentos se obtuvo por entrevista estandarizada. Se calcularon intervalos de confianza (IC) a 95 % para proporciones. Resultados: La prevalencia de alergia a alimentos fue de 17.5 % (IC 95 % = 13.3-22.6). Soya, camarón, almendra, aguacate y cacahuate fueron los alimentos que principalmente provocaron molestias; 35.6 % de los pacientes fue alérgico a un alimento, 17.8 % a dos y 17.8 % a tres. Las manifestaciones más frecuentes fueron las orales (62.2 %), prurito en faringe, lengua y paladar; las respiratorias (26.7 %), disnea y estornudos; y las cutáneas (26.7 %), comezón y ronchas. Conclusiones: La alergia a alimentos en adultos con asma o rinitis alérgica es un problema frecuente. Los síntomas orales son la principal molestia en este tipo de pacientes y principalmente son provocados por la soya.

The study assessed the association between the presence of type2 diabetes mellitus (T2DM) and mortality in women with breast cancer (BC).A matched pair case-control study was conducted at the State Cancer Center, which is located in Xalapa, Veracruz, Mexico. It was matched by age (±3 years) within a cohort of 1442 patients with BC. Descriptive statistics were performed. Analysis through paired odds ratio (OR and multivariate analyses were used to calculate the association between BC mortality and the variables studied.166 cases and 166 controls with confirmed diagnosis of BC were studied, with a mean age of 52.9 ± 11.9 years. The T2DM was associated with an increased mortality of women with BC (OR = 1.75 95 %CI 1.06-2.89). Similarly, metastasis (OR = 14.17 95 %CI 6.19-32.342), advanced clinical stage (OR = 3.04 95 %CI 1.45 - 6.38), and the molecular subtypes Her2 (OR = 2.0 95 %CI 1.02-3.92), and triple negative (OR = 3.54 95 %CI 1.72-7.32). There was no difference in mean glucose between cases and controls (208.9 ± 132 vs 194.4 ± 90.4 mg/dL, respectively).T2DM was found to be a relevant risk factor for BC mortality in this Mexican population. Thus, it is important to consider the presence and evolution of DM in the prevention programs, diagnostic algorithms and treatments established for BC.

Late adolescents with asthma are rarely studied, since generally they are not included in children or adult groups; in addition, studies assessing asthma trends are scarce in Mexico, and their results might differ from findings in developed countries.To determine the trends in asthma prevalence and its symptoms in late adolescents over a period of 7 years.The prevalence of asthma and its symptoms were compared in two cross-sectional population-based cohorts, one from 2009 and the other from 2016.The presence of wheezing sometime in life and during the previous year was significantly increased between 2009 and 2016 (12.8 % versus 20.1 % and 7.3 % versus 10.3 %, p0.001 and 0.002, respectively). The prevalence of asthma went from 7.8 % in 2009 to 12.7 % in 2016 (p0.0001). In addition, the prevalence of allergic rhinitis (4.5 % versus 9.0 %) and atopic dermatitis (3.8 % versus 5.2 %) also showed marked increases (p0.0001 and 0.051, respectively).The prevalence of asthma and some of its symptoms in late adolescents substantially increased in a 7-year period.Antecedentes: Los adolescentes tardíos con asma pocas veces son investigados, dado que generalmente no se incluyen en los grupos de niños ni adultos, además, en México, los estudios que evalúan la tendencia del asma son escasos y sus resultados pudieran diferir de los hallazgos en los países desarrollados Objetivo: Determinar la tendencia de la prevalencia del asma y sus síntomas en adolescentes tardíos en un periodo de siete años. Métodos: Se compararon las prevalencias de asma y sus síntomas de dos cortes transversales con base poblacional, uno en 2009 y otro en 2016. Resultados: La presencia de sibilancias alguna vez en la vida y durante el año previo se incrementó significativamente entre 2009 y 2016 (12.8 % versus 20.1 % y 7.3 % versus 10.3 %; p0.0001 y 0.002, respectivamente). La prevalencia del asma pasó de 7.8 % en 2009 a 12.7 % en 2016 (p0.0001). Adicionalmente, las prevalencias de rinitis alérgica (4.5 % versus 9.0 %) y dermatitis atópica (3.8 % versus 5.2 %) también sufrieron incrementos notorios (p0.0001 y 0.051 respectivamente). Conclusión: La prevalencia de asma y algunos de sus síntomas en adolescentes tardíos se incrementaron sustancialmente en los siete años previos.

Omalizumab, which is a monoclonal anti-IgE antibody, has recently been used as an option in the treatment of inducible urticaria.We describe the case of a 46-year-old woman who was referred to the Department of Allergy and Immunology of "Hospital Civil de Guadalajara, Dr. Juan I. Menchaca" due to a history of hives, body itching, changes in the color of the skin after exposure to water, and chest tightness after the intake of cold beverages; therefore, she used to limit her outdoor activities and personal hygiene. We conducted challenge tests with heat, soaked towels, treadmill walks, and dermographism; which were negative. The ice cube test was positive. To establish the speed of wheal formation, we established intervals of exposure to cold of one, three, five, and ten minutes; a positive result was obtained from the third minute. Due to the poor response to the drug treatment and to measures to avoid the cold, as well as to the poor quality of life, the high risk of anaphylaxis, and the advent of winter season, omalizumab was administered at monthly doses of 150 mg during the winter season. After the first dose, there were no reports of episodes of hives in areas exposed to cold; the ice cube test was negative before the second dose and in the following months, and the patient was able to ingest cold beverages and cold food. There were no adverse reactions that could be attributable to the use of omalizumab. Three years after the first dose, the patient was still asymptomatic.The described case is one of the first cases of cold urticaria with risk of anaphylaxis with a positive response to omalizumab, which was reflected in symptom control and the improvement in the quality of life.Antecedentes: Omalizumab, un anticuerpo monoclonal anti-IgE, recientemente es utilizado como una opción en el tratamiento de la urticaria inducible. Caso clínico: Describimos el caso de una mujer de 46 años referida al Servicio de Alergia e Inmunología del Hospital Civil de Guadalajara Dr. Juan I. Menchaca, por historia de urticaria, picazón en el cuerpo, cambios en el color de la piel tras la exposición al agua y opresión en el pecho posterior el consumo de bebidas frías; en consecuencia, ella limitaba sus actividades al aire libre y de higiene personal. Se realizaron pruebas de desafío con calor, toalla húmeda, caminata en banda sinfín y dermografismo, las cuales fueron negativas; la prueba del cubo de hielo fue positiva. Para establecer la velocidad de la formación de la roncha establecimos intervalos de exposición al frío durante uno, tres, cinco y 10 minutos; se obtuvo un resultado positivo desde el tercer minuto. Debido a la mala respuesta al tratamiento farmacológicos y a las medidas para evitar el frío, así como por la mala calidad de vida, el alto riesgo de anafilaxis y el advenimiento de la temporada de invierno, se administraron 150 mg/mes de omalizumab durante la temporada de invierno. Después de la primera dosis no se notificaron episodios de urticaria en zonas expuestas al frío; la prueba del cubo de hielo antes de la segunda dosis y en los meses siguientes fue negativa y la paciente pudo ingerir bebidas y alimentos fríos. No se produjeron reacciones adversas atribuibles al uso de omalizumab. Tres años después de la primera dosis, la paciente permanecía asintomática. Conclusión: El caso descrito constituye uno de los primeros de urticaria inducida por el frío con riesgo de anafilaxia con respuesta positiva a omalizumab, lo que se reflejó en el control de los síntomas y la mejoría en la calidad de vida.

Background The relationship between birth by cesarean section (CS) and allergic diseases' prevalence continues to be controversial. We aimed to investigate if being born by CS is associated with the prevalence of allergic diseases and their symptoms in schoolchildren. Methods This study included children between 6 and 7 years of age, selected by population-based sampling. We investigated the presence of allergic diseases and their symptoms, family history of asthma, smoking in parents, breastfeeding, exposure to pets, the season of birth, number of siblings, consumption of unpasteurized cow's milk, and mode of birth. Results We included 1003 subjects (526 girls), of which 44.2% were born through CS. The prevalence of allergic diseases did not differ according to birth mode. Asthma in either parent, current smoking by the mother, breastfeeding, and unpasteurized cow's milk consumption were associated with wheezing at some time in life. Conclusions No association between CS birth mode and allergic diseases and their symptoms was found.

Background: Recently, inflammatory cell ratios have gained importance as useful indicators in the categorization of asthma.Objective: We compared the concentration of white blood cells in peripheral blood, as well as their respective inflammatory cell ratios, between patients with asthma and a healthy control group.Methods: We performed cross-sectional analyses of the data obtained from 53 adult patients with asthma and 109 adult controls. In our study, we estimated and compared the following inflammatory cell ratios: Neutrophil-Lymphocyte Ratio (NLR), Eosinophil-Lymphocyte Ratio (ELR), Eosinophil-Neutrophil Ratio (ENR), Eosinophil-Monocyte Ratio (EMR), and Platelet-Lymphocyte Ratio (PLR). The magnitude of association was quantified with the odds ratio.Results: In both groups, the average age was 33 years. In asthmatic patients, we obtained the following results: eosinophils ≥ 400 cells/µl, accounted for 37.7%; basophils ≥ 110 cells/µl, comprised 37.7%; and monocytes < 320 cells/µl, reached 11.3%. In the control group, the results were as follows: 4.6%, 9.2% and 0.9%, respectively. Factors like ethnic origin and geographical area affect the frequency of cow's milk protein allergy (CMPA) and lactose intolerance (LI). Epidemiological information about the non-pediatric population is still missing.To determine the prevalence of CMPA and LI in university students.A cross-sectional study of 1200 students of 18 to 25-year-old. A structured questionnaire was applied in order to identify the clinical manifestations triggered by the intake of cow's milk (CM), and these were categorized as linked to CMPA or linked to LI.Thirty students met the criteria for CMPA (prevalence of 2.5 %; CI 95 % = 1.7-3.6 %) and 128 for LI (prevalence of 10.7 %, CI 95 % = : 9.0 % - 12.5 %). The frequency of personal history of food allergy and dust mite allergy was higher in students with CMPA than in students with LI. Oral pruritus, skin and respiratory discomforts were predominant in CMPA. Abdominal cramps and flatulence were predominant in LI.IL is more frequent than CMPA; which frequency was one in every 10 students; and the frequency of CMPA was one in 400.Antecedentes: El origen étnico y la zona geográfica influyen en la frecuencia de la alergia a la proteína de leche de vaca (APLV) y de intolerancia a la lactosa. Faltan datos epidemiológicos en población no pediátrica.Objetivo: Determinar la prevalencia de APLV e intolerancia a la lactosa en estudiantes universitarios.Métodos: Estudio transversal de 1200 estudiantes de 18 a 25 años. Se aplicó cuestionario estructurado para identificar manifestaciones clínicas desencadenadas por la ingesta de leche de vaca y fueron categorizadas como relacionadas con APLV o intolerancia a la lactosa.Resultados: 30 estudiantes cubrieron criterios para APLV (prevalencia de 2.5 %; IC 95 % = 1.7-3.6) y 128 para intolerancia a la lactosa (prevalencia de 10.7 %, IC 95 % = 9.0-12.5). Los estudiantes con APLV tuvieron mayor frecuencia de historia personal de alergia a alimentos y alergia a ácaros del polvo casero, comparados con los estudiantes con intolerancia a la lactosa. El prurito oral y las molestias cutáneas y respiratorias predominaron en la APLV. El cólico abdominal y las flatulencias predominaron en la intolerancia a la lactosa.Conclusiones: La intolerancia a la lactosa fue más frecuente que la APLV; se observó en uno de cada 10 estudiantes y la APLV en uno de cada 400.

BACKGROUND: It has recently been argued that asthma does not increase the risk of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection. If so, the prevalence of asthma in subjects diagnosed with COVID-19 should be lower than in the general population. OBJECTIVE: To determine the prevalence of asthma in Mexican children and adults with SARS-CoV-2 infection. METHODS: A public database of the Epidemiological Surveillance System for Viral Respiratory Disease in Mexico was analyzed. Those who underwent the real-time reverse transcriptase-polymerase chain reaction-SARS-CoV-2 (rtRT-PCR-SARS-CoV-2) test from February 27 to June 21, 2020, were included. In addition to the prevalence of asthma, some factors associated with it were investigated. RESULTS: Data from 417,366 subjects were analyzed. Asthma prevalence in children, adults, and global were 3.7%, 3.3%, and 3.3%, respectively. Although the asthma prevalence was lower in SARS-CoV-2 positive over negative patients, significant differences were only found in adults (2.8% vs. 3.7% respectively; odds ratio (OR) = 0.74; 95% confidence interval (CI): 0.71-0.77); but not in children (3.5% vs. 3.8%, respectively; OR = 0.91; 95%CI: 0.76-1.10). Multivariate analysis showed in younger than 18 years that girls and immunosuppression were factors associated with a decrease in the odds to develop asthma. In adults, asthma was positively associated with females, obesity, smoking, immunosuppression, chronic obstructive pulmonary disease, arterial hypertension, and cardiovascular disease. CONCLUSION: The prevalence of asthma in child and adult were lower than those previously reported. Our study seems to support the hypothesis that asthma patients have a lower risk of SARS-CoV-2 infection. Further studies are required to demonstrate the consistency of our findings.

Both breastfeeding and the moment at which introduction to solid food occurs have been associated with food allergy.To evaluate whether prolonged breastfeeding and the delayed introduction of whole cow's milk into an infant's diet are factors that can be associated with egg sensitization.This was a hospital-based case-control study, matched by age and sex: each study group comprised 97 atopic children. Additionally, logistic regression was used to identify the factors associated with egg protein sensitization.The most common type of allergic disease among both groups was allergic rhinitis. After adjusting for possible confounding variables, a delayed introduction to whole cow's milk decreased the odds of egg protein sensitization; OR=0.16 (95% CI: 0.07-0.36, p0.0001). Notably, breastfeeding during the first six months of life, regardless of whether it was the only milk an infant drank, increased the risk for sensitization to chicken eggs; OR=5.54 (95% CI: 2.41-12.7, p0.0001).Prolonged breastfeeding, regardless of whether it was the only milk an infant drank, greatly increased the risk of egg sensitization. Interestingly, a delayed introduction to whole cow's milk was associated with a reduced possibility of becoming sensitized to eggs. Further studies are required to elucidate these findings.

Prevalence of latex allergy in medical students is not known.To determine the prevalence of self-reported latex allergy and associated factors in medical students.Cross-sectional, analytical study of students with or without self-reported latex allergy. By means of a structured questionnaire, past personal and family history of allergic disease, time and frequency of exposure to latex gloves and symptoms after exposure to products or foods associated with latex allergy were inquired. Logistic regression models were carried out.Out of 854 subjects, 431 (50.5%) were females. Median age was 21 years. Overall prevalence of latex allergy was 4.3% (95% CI = 3.1 to 5.9). Associated risk factors were age (OR = 1.37; 95% CI = 1.05 to 1.79), personal history of atopic dermatitis (OR = 7.32; 95% CI = 3.14 to 17.08), use of gloves ≥ 15/week (OR = 2.59; 95% CI = 1.17 to 5.76), use of latex products (OR = 5.76; 95% CI = 2.15 to 15.49) and fruit allergy (OR = 3.24; 95% CI = 1.27 t o8.27).Four out of a hundred students reported latex allergy. Age, personal history of atopic dermatitis, higher frequency of exposure to latex gloves and history of fruit allergy were risk factors for self-report latex allergy.Antecedentes: La prevalencia de la alergia al látex se desconoce en los estudiantes de medicina. Objetivo: Determinar prevalencia y factores asociados con autorreporte de alergia al látex en estudiantes de medicina. Métodos: Estudio transversal de estudiantes con o sin autorreporte de alergia al látex. Mediante cuestionario estructurado se indagó historia personal y familiar de enfermedad alérgica; tiempo y frecuencia de exposición a guantes de látex y síntomas tras la exposición a productos o alimentos relacionados con alergia al látex. Se realizaron modelos de regresión logística. Resultados: De 854 sujetos, 431 (50.5 %) fueron mujeres. La mediana de edad fue de 21 años. La prevalencia global de alergia al látex fue 4.3 % (IC 95 % = 3.1 a 5.9). Los factores de riesgo relacionados fueron edad (RM = 1.37; IC 95 % = 1.05 a 1.79), historia personal de dermatitis atópica (RM = 7.32; IC 95 % = 3.14 a 17.08), uso de guantes ≥ 15/semana (RM = 2.59; IC 95 % = 1.17 a 5.76), uso de productos con látex (RM = 5.76; IC 95 % = 2.15 a 15.49) y alergia a frutas (RM = 3.24; IC 95 % = 1.27 a 8.27). Conclusión: Cuatro de cada 100 estudiantes reportaron alergia al látex. La edad, la historia personal de dermatitis atópica, la mayor frecuencia de exposición a guantes de látex y antecedente de alergia a frutos fueron factores de riesgo.

Introduction The commonly held notion that a rural environment decreases the frequency of allergic diseases has proven to be inconsistent amongst children. Objective Our objective was to contrast the prevalence of bronchial asthma (BA), allergic rhinitis (AR), and atopic dermatitis (AD) between children that live in a rural environment and those that live in urban areas. Methods We carried out a cross-sectional study amongst children aged six to seven; they were selected through probabilistic, stratified and conglomerated sampling. The prevalence of BA, AR, and AD was identified with the use of the questionnaire provided by The International Study of Asthma and Allergies in Childhood , additionally, we inquired about each child's family history of atopy, their exposure to farm animals, the intake of unpasteurised cow's milk, and the number of siblings related to every child. We used logistic regression and multivariate analysis to determine the correlation between asthma, allergic diseases, and rural environment. Results We included 189/1003 (18.8%) children from a rural environment, and 814/1003 (81.2%) from an urban area. BA and AR were associated to a family history of atopy (OR = 2.15, p = 0.001; OR = 2.58, p = 0.002, respectively). BA was more prevalent in males (OR = 1.92, p = 0.007). Notably, a higher number of siblings seems to protect against AR (OR = 0.45, p = 0.008). A paternal history of allergies was associated to AD. Conclusions In our study, we were unable to find protective factors in a rural environment that might decrease the prevalence of asthma or allergic diseases.

In our country, the prevalence of latex allergy in health personnel has rarely been studied.To determine the prevalence and associated factors to self-reported latex allergy in health care workers.A cross-sectional study was conducted among 1,292 health care workers of a second level hospital. All workers were included and they were required to answer a structured questionnaire aimed at identifying latex allergy, atopic personal and family history, exposure to latex gloves and surgical background. Odds ratio (OR) and 95 % confidence interval (95 % CI) were estimated using regression logistic to investigate factors associated to latex allergy.The female to male ratio was 2.4:1. The men age 38.4 ± 11.6 years. The prevalence of latex allergy auto-reported was 9.7 %, with a confidence interval of 95 % (95 % CI) 8.1 %-11.3 %. There was no statistical difference by workplace (p = 0.508). Factors associated with latex allergy included female gender (OR = 1.68; 95 % CI: 1.03-2.73, p = 0.037), personal history of atopy (OR = 4.82; 95 % CI: 3.19-7.26, p0.0001), family history of atopic dermatitis (OR = 4.33, 95 % CI: 1.20-4.41) and history of allergy to fruits (OR = 4.33; 95 % CI: 2.62-7.14, p0.0001).Up to 10 out to 100 health workers may have latex allergy. The main factors associated with latex allergy in this study were: being a female, personal or familiar atopy and allergy to fruits.Antecedentes: pocas veces ha sido estudiada la prevalencia de alergia al látex en personal de la salud en México. Objetivo: determinar la prevalencia de autorreporte de alergia al látex y los factores asociados en trabajadores de la salud. Métodos: estudio trasversal de 1292 trabajadores de la salud de un hospital de segundo nivel, a quienes se les aplicó un cuestionario estructurado para identificar alergia al látex, historia personal y familiar de atopia, exposición a guantes de látex y antecedente de cirugías. La búsqueda de asociaciones entre variables se realizó mediante regresión logística. Se calcularon razones de momios (RM) e intervalo de confianza a 95 % (IC 95 %) Resultados: la relación mujer:hombre fue de 2.4:1. Edad media de 38.4 ± 11.6 años. La prevalencia de alergia al látex fue de 9.7 %, IC 95 %, 8.1-11.3 %. No hubo diferencia estadística por área laboral (p = 0.508). Los factores asociados con la alergia al látex fueron sexo femenino (RM = 1.68; IC 95 %, 1.03-2.73), historia personal de atopia (RM = 4.82; IC 95 %, 3.19-7.26), historia familiar de dermatitis atópica (RM = 4.33; IC 95 %, 1.20-4.41) e historia de alergia a frutos (RM = 2.30; IC 95 %, 2.62-7.14). Conclusiones: hasta 10 % los trabajadores de la salud podría presentar alergia al látex; los principales factores asociados fueron sexo femenino, atopia personal o familiar y alergia a frutos.

Introduction The oral allergy syndrome (OAS) is a particular type of food allergy rarely explored in the paediatric population that is already considered an adult problem. Objective Identify the prevalence of OAS, symptoms and pollen species associated with its presence in children affected by allergic diseases. Methods A cross-sectional study was conducted. Consecutive sampling included children from 6 to 14 years who needed allergy treatment for the first time. A structured questionnaire was carried out to collect demographic and clinical data and history of OAS. Besides sensitisation to various allergens, the skin prick-by-prick test was performed to corroborate sensitisation to food related to OAS. Prevalence of OAS and its association with pollens was established following the covariate adjusted logistic regression. Results 267 subjects were included. Overall prevalence of OAS was 8.9% (95%CI 6.1–13.1%). Prevalence of OAS for allergic rhinitis and asthma were 8.8% and 9.1%, respectively. In patients sensitised to pollen, the prevalence ranged from 9.6% to 12.2% depending on the type of pollen. 62.5% of children with OAS were sensitive to pineapple. After adjusting for gender and family history of atopic disease, trees from the Quercus species showed an association with OAS (OR = 2.7, 95%CI 1.2–6.2). Conclusions OAS is not uncommon in our environment. Pineapple, a typical fruit from the region, was the main food related. Quercus sp., but not birch nor olive, was the pollen associated with this syndrome.

The prevalence of food sensitization in adults shows differences owing to geographic, regional and cultural circumstances.To identify the prevalence and factors associated with food sensitization in adults with allergic diseases.Cross-sectional study, where medical records of adult patients with any allergic disease and who tested positive to airborne allergens on at least one skin test were reviewed. Age, sex, underlying allergic disease and skin test result were recorded. Statistical analyses included food sensitization prevalence with the corresponding 95 % confidence intervals (CI), as well as a multivariate analysis to determine associated factors.We included 258 patients with a mean of 36 years of age; 75 % were of the female sex. The most common underlying condition was allergic rhinitis (59 %). The prevalence of food sensitization was 37 % (95 % CI, 31.5-43.3) and it did not vary significantly according to sex or allergic disease in question. The most sensitizing food was soybeans (44.8 %), and associated factors were sensitization to cat dander (OR = 1.8; 95 % CI, 1.03-3.2) and 5 or more positive skin tests (OR = 4.3, 95 % CI 2.3-8.1).The prevalence of food sensitization in people with allergic disease was 40 %. Further studies are required to determine its prevalence in the general population.Antecedentes: La prevalencia de sensibilización a alimentos en adultos presenta diferencias debido a las condiciones geográficas, regionales y culturales. Objetivo: Identificar la prevalencia y los factores asociados a la sensibilización a alimentos en adultos con alguna enfermedad alérgica. Métodos: Estudio transversal en el que se revisaron los expedientes clínicos de adultos con alguna enfermedad alérgica que presentaron al menos una prueba cutánea positiva a aeroalérgenos. Se registró edad, sexo, enfermedad alérgica de base y resultado de la prueba cutánea. El análisis es-tadístico incluyó prevalencia de sensibilización a alimentos e intervalos de confianza (IC) de 95 %, así como análisis multivariado para determinar los factores asociados. Resultados: Se incluyeron 258 pacientes con edad media de 36 años; 75 % fue del sexo femenino. La enfermedad de base más frecuente fue la rinitis alérgica (59 %). La prevalencia de sensibilización a alimentos fue de 37 % (IC 95 %, 31.5-43.3) y no varió significativamente según sexo o enfermedad alérgica. El alimento más sensibilizante fue la soya (44.8 %) y los factores asociados, la sensibil-ización al epitelio de gato (OR = 1.8; IC 95 %, 1.03-3.2) y 5 o más pruebas cutáneas positivas (OR = 4.3; IC95 %, 2.3-8.1). Conclusiones: La prevalencia de sensibilización a alimentos en personas con enfermedad alérgica fue de 40 %; para determinarla en la población general se requieren estudios adicionales.

Background In our country, the prevalence and the factors associated to peanut allergy are unknown, a health problem that has been emerging worldwide. Objective To establish the prevalence and the factors that are associated to peanut allergy amongst school children. Methods This is a population-based cross-sectional study. We included 756 children aged 6–7 years. The children's parents were questioned about their peanut intake habits. A structured questionnaire was applied, it included questions regarding peanut intake; family and personal history of asthma; rhinitis; and atopic dermatitis. Allergic reactions to peanuts were registered as: probable, convincing and systematic. The statistical analyses included logistical regression models to look for associated factors. Results Males were 356/756 (47.1%). Peanut allergy prevalence: probable reaction: 14/756 (1.8%), convincing reaction: 8/756 (1.1%) and systemic reaction: 3/756 (0.4%). Through multivariate analysis, the presence of symptoms of allergic rhinitis (OR = 4.2 95% CI 1.3–13.2) and atopic dermatitis (OR = 5.2; 95% CI 1.4–19.5) during the previous year, showed significant association to probable peanut reaction. The former year, the presence of atopic dermatitis was the only variable that was substantially associated to a convincing reaction (OR = 7.5; 95% CI 1.4–38.4) and to a systematic reaction (OR = 45.1; 95% CI 4.0–510.0), respectively. Conclusions The reported prevalence of peanut allergy was consistent with that found in previous studies; symptoms of allergic rhinitis and atopic dermatitis were identified as associated factors to peanut allergy.

To determine if the place of residence and the level of social marginalization are associated with prostate cancer survival.All patients diagnosed with prostate cancer (PC) in the period from 2013 to 2017 in a tertiary healthcare hospital in Veracruz, Mexico were included. Patients resided in rural and urban areas. Variables were collected according to clinical-epidemiological and histopathological characteristics. The Kaplan Meier method and the Log Rank test were used to measure survival. Prognostic factors were determined by calculating the adjusted hazard ratio (HRa) in a multivariate analysis using the Cox proportional risk method.A total of 186 PC cases were analyzed. Overall, after 5 years, 48.3% of the patients survived. Men living in urban areas had a higher probability of survival than those living in rural areas (HRa 1.67, 95% CI 1.16-2.41). Similarly, people living in areas classified as low- marginalization zones had a higher probability of survival than those living in areas with a high level of social marginalization (HRa 2.32, 95% CI 1.47-3.66).To reside in a rural place was identified as a negative prognostic factor for the survival of patients with PC regardless of other sociodemographic and clinical variables; patients living in high-marginalization places had an unfavorable survival prognosis.Determinar si el lugar de residencia y el grado de marginación se encuentran asociados a la supervivencia al cáncer de próstata.Se incluyeron a todos los pacientes diagnosticados con cáncer de próstata (CP) en el periodo 2013-2017 en un hospital de tercer nivel de atención de Veracruz, México. Los casos expuestos fueron los pacientes que habitualmente residían en zonas rurales, los no expuestos fueron los de zonas urbanas. Se recolectaron variables según características clínico epidemiológicas e histopatológicas. Para medir la supervivencia se utilizó el método de Kaplan Meier y la prueba de Log Rank. Los factores pronósticos fueron determinados calculando hazard ratio ajustado (HRa) en un análisis multivariado mediante el método de riesgos proporcionales de Cox.Se analizaron 186 casos de CP. La supervivencia global a 5 años fue de 48,3%. Los hombres que residían en zonas urbanas tuvieron una probabilidad de supervivencia mayor que quienes residían en zonas rurales (HRa 1,67; IC 95%: 1,16-2,41). Asimismo, las personas que vivían en lugares catalogados como zonas de baja marginación tuvieron una mayor probabilidad de supervivencia que quienes vivían en zonas de alta marginación (HRa 2,32; IC 95%: 1,47-3,66).El lugar de residencia rural se identificó como un factor de mal pronóstico para la supervivencia de pacientes con CP independientemente de otras variables sociodemográficas y clínicas; los pacientes que vivían en lugares con grados de marginación más elevados tuvieron un pronóstico desfavorable de supervivencia.

Background Exposure to pets can be a predisposing factor in the development of certain diseases, including allergic diseases. Objective We analyzed the role that exposure to indoor dogs and cats plays in the prevalence of allergic diseases. Methods We examined the cross-sectional data of 1056 women and 936 men aged 15 to 18 years; these individuals were selected through stratified and cluster random sampling. We asked all participants about their exposure to indoor dogs and cats during the year that preceded our study. The prevalence of allergic diseases was determined through core questions taken from The International Study of Asthma and Allergies in Childhood questionnaire. Results The prevalence was 12.7% (95% CI: 11.3%–14.2%) for asthma, 9.0% (95% CI: 7.8%–10.4%) for allergic rhinitis, and 5.2% (95% CI: 4.3%–6.2%) for atopic dermatitis. The multivariate analyses showed that exposure to indoor dogs, but not indoor cats, was associated with asthma prevalence (aOR 1.37; 95% CI: 1.03–1.83), as was male sex (aOR = 1.42; 95% CI: 1.08–1.86), a personal history of allergic rhinitis (aOR = 3.24; 95% CI: 2.25–4.66), and a maternal history of asthma (aOR = 3.06; 95% CI: 1.89–4.98). The population attributable risk for exposure to indoor dogs was 18%. Notably, neither allergic rhinitis nor atopic dermatitis was found to be associated with dog or cat exposure (p > 0.05). Conclusion Exposure to dogs in late adolescence is a factor associated with asthma, although its contribution to the development of asthma should be investigated in new studies.

This paper aims to assess diet and lifestyle by designing a healthy behavior index (HBI) related to the educational level and time of breast cancer (BC) diagnosis.241 female breast cancer survivors treated at Centro Estatal de Cancerología (State Cancer Center) in Mexico were assessed based on dietary pattern, physical activity, smoking, alcohol consumption, body size and shape, sleep disorders with increasing scores that represent less healthy characteristics. The odds ratios (OR) and quartiles of the healthy behavior index and the time of diagnosis were estimated. The regression model was used to assess the association between HBI and the BC covariates.The healthy behavior index for the two first quartiles was that of p 0.001. The graphic behavior of correspondence with the covariates (age, schooling and moderate physical activity) showed a different three-dimensional effect on healthy behaviors. According to diagnosis time ≤ three years, the following covariates were significant: socioeconomic status, (OR: 4.34 CI 95% 1.2-9.5), sleep disturbances (p = 020) and protective intake of fruits and vegetables (p = 0.001). In the multivariate analysis, survivors with a high level of education are significantly more likely to show a healthy behavior (OR: 0.3 95% CI 0.12 - 0.8); as well as the early clinical stages (OR: 0.4 95% CI 0.2-0.9).In breast cancer survivors, both the high level of education and early clinical stages were important healthy behavior modifiers.Since diets are potentially modifiable, the findings may have further implications to promote a careful dietary pattern to prevent breast cancer. These variables should be assessed as a strategy in cancer survivor preventive programs.

Obesity is characterized, among other features, by overeating, reduced physical activity and an abnormal accumulation of body fat. These features are thought to result, at least in part, from the individual's inability to self-regulate their eating and physical activity behaviors (EPaB). Self-regulation of the EPaB is a three-step sequential process: self-observation, self-evaluation and self-reaction. However, it is yet unclear whether deficient self-regulation of EPaB could predispose, facilitate and/or consolidate obesity. Unraveling this issue is fundamental in order to more precisely define the role of self-regulation of EPaB in the management of obesity.This research was focused on the question of whether or not self-regulation of EPaB is related to obesity in female undergraduate students. This population segment seems especially vulnerable to developing obesity since they undergo a significant shift of their EPaB upon their university enrollment. To address this question, a cross-sectional study with 108 female undergraduate students with normal weight (n = 80) or obesity (n = 28) was performed, in which self-regulation of eating habits and physical activity was measured by two validated scales and a personal data questionnaire.Female undergraduate students displaying lower EPaB self-reactions were consistently overweight or obese. In addition, a multivariate analysis identified high levels of self-reaction towards eating habits related to a minor presence of overweight issues or obesity.Self-regulation should be an essential component in the strategies for obesity prevention as an integral approach that must include orientation about healthy eating and physical activity behaviors. In addition, further studies on the effect of self-regulation in the treatment of the obesity are needed.

Introduction We assessed the association between overweight, obesity, and morbid obesity with the incidence of the most aggressive breast cancer subtypes in women. Methods and Materials A cross-sectional study was performed. We conducted a record review to identify the following aspects: body mass index, sociodemographic features, tumor characteristics, and reproductive and molecular aspects. Descriptive statistics and univariate analysis were performed to identify the association between the molecular subtypes and the study variables. In addition, we used multivariate analysis to identify the association between obesity and the presence of metastatic lymph nodes. Results We included 1446 women with an average age of 52.5 ± 12.1 years. Of the 1446 patients, 47% were premenopausal and 75% were overweight. Univariate analysis indicated a statistically significant association between obesity and advanced disease stage, as well as nulliparity and multiparity. Similar results were found for women with morbid obesity. Model 1 of the multivariate analysis showed an association between the presence of metastatic lymph nodes and obesity (odds ratio [OR], 1.6; P = .008) and histologic grade 2 or 3 (OR, 2.4; P = .003). Using model 2, an association was identified between an advanced disease stage and 2 factors: morbid obesity (OR, 1.9; P = .02) and positive human epidermal growth factor receptor 2 (OR, 1.8; P = .045). Conclusion We found that obesity is associated with the more advanced stages of breast cancer. Further studies are needed to evaluate the role of obesity in breast cancer progression in women.

Introduction The association regarding the exposure to pets, especially cats and dogs, and the prevalence of allergic diseases is inconsistent. Objective We analyzed the role played by early exposure to dogs or cats in the prevalence of allergic diseases amongst school-aged children. Method Through a cross-sectional study, we examined 756 children, aged 6–7; these candidates were selected through cluster sampling. We inquired about the exposure that these children had had to dogs and cats, and whether these pets spent most of their time indoors or outdoors during the first year of the child's life. In order to identify the prevalence of allergic diseases and their symptoms, each child's parent completed the International Study of Asthma and Allergies in Childhood questionnaire. Results Exposure to outdoor dogs was associated to nocturnal coughing, odds ratio (OR) 0.64, with a confidence interval of 95% (95% CI) 0.43–0.95 and with atopic dermatitis (OR: 0.39; 95% CI: 0.20–0.76). Interestingly, exposure to outdoor cats was associated to nocturnal coughing (OR: 0.51; 95% CI: 0.32–0.83) and current rhinitis symptoms (OR: 0.59; 95% CI 0.36–0.97). After carrying out the multivariate analyses, only exposure to dogs, both indoor and outdoor, was significantly associated to a decrease in the prevalence of atopic dermatitis OR 0.40 (95% CI: 0.20–0.79) and OR 0.38 (95% CI: 0.18–0.83), respectively. Conclusion Our findings suggest that exposure to dogs, whether they be indoor or outdoor pets, is associated to a decreased prevalence in atopic dermatitis.

Hypovitaminosis D has been associated with various chronic diseases such as infections, autoimmune diseases, chronic obstructive pulmonary disease, cancer and asthma Objective: The objective at hand is to determine the prevalence of vitamin D (VD) insufficiency and deficiency in adults with allergic asthma.Objective: The objective at hand is to determine the prevalence of vitamin D (VD) insufficiency and deficiency in adults with allergic asthma.Through a cross-sectional study, we analyzed corresponding data amongst 135 patients. VD concentration was categorized as sufficient (≥ 30 ng/mL), insufficient (21-29 ng/mL), and deficient (≤ 20 ng/mL). The level of VD deficiency was measured through chemo-luminescence. We estimated the prevalence of VD alterations and their respective confidence intervals at 95 % (CI 95 %).Within the analyzed population, there were 99/135 women (73.3 %); the mean age was 34.5 ± 10.3 years. The mean concentration of VD was 17.9 ± 6.9 ng/mL and the median was 17 ng/mL. The prevalence of VD insufficiency and deficiency was 25.2 % (CI 95 %, 18.6-33.2 %) and 71.1 % (CI 95 %, 62.9-78.1 %), respectively; VD concentrations ≤ 10 ng/mL had 13.3 % (CI 95 %, 8.5-20.2 %) and ≥ 30 ng/mL at 3.7 % (CI 95 %: 1.4-8.6 %). When we contrasted the men to the women, the median concentration of VD did not differ significantly (16 ng/mL vs. 18 ng/mL, p = 0.71).In this study, patients with allergic asthma had distinctively reduced VD concentration levels; future research will determine if and how VD affects the severity of asthma.Antecedentes: La hipovitaminosis D ha sido asociada con diversas enfermedades crónicas como infecciones, enfermedades autoinmunes, enfermedad pulmonar obstructiva crónica, cáncer y asma. Objetivo: Determinar la prevalencia de insuficiencia y deficiencia de vitamina (VD) en adultos con asma alérgica. Métodos: Estudio transversal en el que se analizaron los datos de 135 pacientes. La concentración de VD fue categorizada en suficiente (≥ 30 ng/mL), insuficiente (21-29 ng/mL) y deficiente (≤ 20 ng/mL). La concentración de VD se midió por quimioluminiscencia. Se estimaron las prevalencias de las alteraciones de la VD y sus respectivos intervalos de confianza a 95 % (IC 95 %). Resultados: En la población analizada, 99 fueron mujeres (73.3 %), con edad media de 34.5 ± 10.3 años. La concentración media de VD fue 17.9 ± 6.9 ng/mL (mediana de 17 ng/mL). La prevalencia de insuficiencia y deficiencia de VD fue de 25.2 % (IC 95 %, 18.6-33.2 %) y 71.1 % (IC 95 %, 62.9-78.1 %), respectivamente; las concentraciones de VD ≤ 10 ng/mL representaron 13.3 % (IC 95 %, 8.5-20.2 %) y ≥ 30 ng/mL, 3.7 % (IC 95 %, 1.4-8.6 %). Al contrastar hombres y mujeres, la concentración mediana de VD no difirió significativamente (16 ng/mL versus 18 ng/mL, p = 0.71). Con futuras investigaciones se podrá evaluar el papel de la VD en la gravedad del asma.

Versión pre-print: 24 páginas, 4 figuras, 4 tablas, OBJECTIVE: To evaluate the effect size of type 2 diabetes mellitus (T2DM) on tuberculosis (TB) treatment outcomes and multi drug resistance (MDR). METHODS: A cohort with 507 individuals with diagnosed TB included 183 with coexistence of T2DM and TB (TB-T2DM). Participants were identified at the time of TB diagnosis and followed during the course of TB treatment. Then we computed relative risks and adjustments by Cox proportional hazards for outcome variables (drug resistance, death, relapse, treatment failure), and the size of their effect as Cohen's-d. RESULTS: Patients with TB-T2DM were more likely to remain positive for acid-fast bacilli after two months of anti-TB treatment RR = [2.01 (95% CI: 1.3, 3.1)], to have drug resistant (DR) [OR 3.5 (95% CI: 1.8, 6.7)] and multi-drug resistant (MDR) TB [OR 3.5 (95% CI: 1.8, 7.1)]. The Cohen's-d for DR or MDR in T2DM was 0.69 when compared with non-DM subjects. The T2DM patients had higher odds of resistance to isoniazid (OR 3.9, 95% CI: 2.01, 7.9), rifampicin (OR 3.4, 95% CI: 1.6, 7.2) and pyrazinamide (OR 9.4, 95% CI: 2.8, 25.6), and their effect sizes were ≥0.67. Patients with TB-T2DM (versus no DM) were more likely to present with MDR TB (HR 3.1; 95% CI: 1.7, 5.8; p < 0.001), treatment failure (HR 2.04; 95% CI: 1.07, 3.8; p = 0.02) and relapse (HR 1.86; 95% CI: 1.09, 3.1; p = 0.02), with effect size ≥0.34. CONCLUSION: T2DM showed a substantial contribution to the presence of DR or MDR-TB and to adverse clinical outcomes during and after TB treatment. Our findings support the importance for routine screening of T2DM among newly-diagnosed TB patients in order to stratify them for immediate DR assessment, and highlight the need for clinical trials to evaluate variations to the standard TB treatment in TB-T2DM to prevent adverse treatment outcomes.

Introduction: Anxiety and depression are frequent disorders of chronic diseases, yet there is no conclusive information to their association with rhinitis. Objective: The objective is to determine the frequency of anxiety and depression and its possible association to allergic rhinitis (AR) and non-allergic rhinitis (NAR). Methods: This is a cross-sectional study in which procured subjects with AR (n = 111), NAR (n = 34) and a control group (n = 96) from the university hospital. The presence of anxiety and depression was considered when it reached a score > 13 based on The Beck Anxiety Inventory Test and The Beck Depression Inventory II Test, respectively. The association between AR and NAR with anxiety and depression was adjusted with the Mantel-Haenszel Method and logistic regression. Results: The frequency for anxiety in AR, NAR and the control group was 45.9%, 52.9%, 10.4%, respectively (p < 0.001); depression frequency was 38.7%, 47.1%, 16.6% (p = 0.0003), respectively. Both AR and NAR were associated to anxiety and depression in women, but not to men. After adjusting the sex: AR was associated to anxiety (OR = 5.7, p < 0.001) and depression (OR = 2.5, p = 0.015), while NAR was also associated to anxiety (OR = 7.8, p < 0.001) and depression (OR = 3.3, p < 0.014). In multivariate analysis it was identified that AR, NAR and the individual's sex (women) were factors associated to anxiety and depression. Results showed that age was only associated to anxiety. Conclusion: AR and NAR are diseases associated to anxiety and depression, at least in women. Resumo Introdução: Ansiedade e depressão são distúrbios frequentes em doenças crônicas, embora não haja informação conclusiva sobre sua associação com a rinite alérgica. Objetivo: Determinar a frequência de ansiedade e depressão e sua possível associação a rinite alérgica (RA) e rinite não alérgica (NRA). Método: Estudo transversal, no qual indivíduos com RA (n = 111), NRA (n = 34) e um grupo controle (n = 96) foram recrutados a partir do hospital universitário. A presença de ansiedade e depressão foi considerada quando atingiu uma pontuação > 13 no Teste do Inventário de Ansiedade de Beck e no Teste II do Inventário de Depressão de Beck, respectivamente. A associação entre a RA e RNA com ansiedade e depressão foi ajustada com o Método de Mantel-Haenszel e regressão logística. Resultados: A frequência de ansiedade na RA, RNA e grupo controle foi de 45,9%, 52,9%, 10,4%, respectivamente (p < 0,001); a frequência de depressão foi de 38,7%, 47,1%, 16,6% (p = 0,0003), respectivamente. Tanto a RA como a RNA foram associadas a ansiedade e depressão em mulheres, mas não para os homens. Depois de ajustar o sexo: RA foi associada à ansiedade (OR = 5,7, p < 0,001) e depressão (OR = 2,5, p = 0,015), enquanto a RNA foi associada a ansiedade (OR = 7,8, p < 0,001) e depressão (OR = 3,3, p < 0,014). Na análise multivariada identificou-se que a RA, RNA e o sexo do indivíduo (mulheres) foram fatores associados a ansiedade e depressão. Os resultados mostraram que a idade só foi associada à ansiedade. Conclusão: RA e RNA são doenças associadas a ansiedade e depressão, pelo menos em mulheres.

To evaluate impact of working days lost and factors for developing sick leave episodes in Mexicans workers with rheumatoid arthritis (RA). A prospective cohort of 123 patients with RA was followed for 1 year. Factors evaluated for sick leave episodes included: demographics, job characteristics, comorbidity, depressive symptoms, and clinical/therapeutic variables. Rates of sick leave episodes, working days lost, and permanent work disability (PWD) were identified. Statistical analysis included Cox regression models estimating hazard risks (HR) and their 95 % confidence intervals (95% CI). Cumulative time of follow-up for the cohort was 43,380 days, 24 % of workers had at least one episode of sick leave, with a mean of working days lost per patient-year of 18.36; 4.1 % developed PWD. Development of sick leave in the Kaplan-Meier analysis was associated with: age ≥40 years (p = 0.04), having a couple (p = 0.04), performing manual work (p = 0.03), suffering depressive symptoms (p = 0.04), limitations in functioning (p = 0.01), and poor global functional status ≥ III (p = 0.01). Cox regression models identified HAQ-Di ≥ 0.6 as the stronger predictor for sick leave (HR = 4.04, 95 % CI 1.41-11.58, p = 0.009) followed by age (HR = 1.05, 95 % CI 1.01-1.11, p = 0.04), ≥4 risk factors had a HR to 9.4 (95 % CI: 2.1-42.7) for sick leave. In this prospective cohort of Mexican workers with RA, we identified several factors associated with sick leave episodes and working days lost that should be potentially addressed by a multidisciplinary approach, being required to revaluate these strategies with the aim of increasing the work permanence of these patients.

The elevated prevalence of obesity as well as of asthma in preschool children has prompted investigators to speculate that obesity in childhood might be a causal factor in the development of asthma. The results obtained to date are debatable. We investigated the association between obesity and asthma in 1,160 preschool Mexican children. Diagnosis of asthma was performed using the International Study of Asthma and Allergy in Childhood (ISAAC) questionnaire. The body mass index (BMI) in units of kg/m2was determined, and children were categorized according to age- and gender-specific criteria, such as normal weight (5th-85thpercentile), overweight (ࣙ85thand thpercentile), and obesity (ࣙ95thpercentile). Power test for logistic regression model was calculated. We found no association between overweight (adjusted OR = 1.02; 95% CI = 0.66–1.58), obesity (adjusted OR = 0.94; 95% CI = 0.68–1.30), and wheezing during the last year as determined by logistic regression model adjusted. We did not find an association between overweight, obesity, and asthma-associated hospitalizations. Further longitudinal studies are required to provide a better understanding of the relationship between obesity and asthma in preschool children.

Healthy eating habits promote wellness and prevent disease, however, despite the intention to change a bad habit, people often fail in theirattempts. This is due, since the performance of a change requires self-regulation skills that allow to observe, to evaluate and to take an action, in a constant motivation during the all the process; not only theknowledge about proper nutrition. The objective of this study was to design and validate an instrument to evaluate the level of self-regulation for eating habits in college students.62 items were written and evaluated by four expert judges. Two applications of the instrument were performed to 487 subjects. An unweighted least squares factor analysis whit direct Oblimin rotation was performed. The items saturated in more than one factor were discarded, as well as those who had a loading factor less than 0.40 or commonality less than 0.30. It was obtained an instrument integrated by 14 items grouped into three factors, which explained the 46.9% of the variance: self-reaction, self-observation and self-evaluation. Cronbach's alpha yielded a high reliability coefficient (α = 0.874).The results show that the scale is a valid and reliable tool to measure of self-regulation of eating habits in college students. Its applications include the diagnostic of a population and the evaluation of interventions aimed to improving nutrition based on the assumption that the processes of change require sustained self-regulation skills in people protective effect against increases in both systolic and diastolic blood pressure.

The effect that breastfeeding and complementary feeding practices have on the prevalence of allergic diseases has shown inconsistent results.To assess the effect of breastfeeding and the initiation of complementary feeding on the prevalence of allergic disease.Analytical, crosssectional population-based study conducted in 6-12 year old children attending primary school and selected through a multistage sampling technique. A structured questionnaire was administered to parents or tutors to identify allergic diseases (asthma, allergic rhinitis, or atopic dermatitis), a history of prolonged breastfeeding and age at initiation of complementary feeding. A logistic regression analysis was used to establish associations among variables.A total of 740 children were included. The frequency of breastfeeding for6 months was 73.4%, and of complementary feeding at4 months old was 31.9%. Prolonged breastfeeding showed no effect on the prevalence of allergic diseases. A protective effect was observed on the frequency of atopic dermatitis when complementary feeding was initiated late, adjusted OR= 0.36, 95% confidence interval (CI): 0.1-0.8 (p 0.019).The protective effect of breastfeeding against the prevalence of allergic diseases has not been demonstrated. There is a reduction in the prevalence of atopic dermatitis when complementary feeding is started late.

No-reflow phenomenon is a common event in patients with acute myocardial infarction with ST elevation (STEMI) who underwent primary percutaneous coronary intervention. The objective is to determine the cumulative incidence of no-reflow phenomenon and some predictors related to its occurrence.We retrospectively analyzed the reports of 71 patients with STEMI. Subjects were categorized in two groups, those with no-reflow phenomenon and those without it; their clinical findings were compared. Predictive factors were identified by logistic regression analysis.We identified 20 patients with no-reflow phenomenon (with a cumulative incidence of 28.1 %) and 51 with adequate reperfusion. In the univariate analysis the following predictors were related to the no-reflow phenomenon: CK-MB = 160 UI/L, Killip class = II, TIMI flow 0, having more than two Q waves in ECG, the form of reperfusion therapy and the presence of intracoronary thrombus. The multivariate logistic regression analysis identified Killip class = II (OR = 9.3, p = 0.008), = two Q waves in ECG (OR = 1.8, p = 0.05), angioplasty as unique reperfusion therapy (OR = 19.9, p = 0.017) and the presence of intracoronary thrombus (OR = 11.9, p = 0.008) as predictors of no-reflow phenomenon.The early detection of predictors of no-reflow phenomenon will establish measures aimed to reduce its presentation.Introducción: el fenómeno de no reflujo es un evento común en sujetos con infarto agudo al miocardio con elevación del ST (IAM-CEST) que son sometidos a intervención coronaria percutánea primaria. El objetivo de este artículo es determinar la incidencia acumulada del fenómeno de no reflujo y algunos predictores relacionados con su presentación. Métodos: se analizaron retrospectivamente los reportes de 71 pacientes con IAM-CEST. Los sujetos fueron categorizados entre aquellos con fenómeno de no reflujo y quienes no lo presentaron; los hallazgos clínicos de ambos grupos fueron comparados. Los factores predictores fueron identificados mediante análisis de regresión logística. Resultados: se identificaron 20 pacientes con fenómeno de no reflujo (incidencia acumulada: 28.1 %) y 51 con reperfusión adecuada. En el análisis univariado se relacionaron con el fenómeno de no reflujo: CPK MB = 160 UI/L, clase de Killip = II, fl ujo TIMI 0, tener = 2 ondas Q en el ECG, método de reperfusión utilizado y presencia de trombo intracoronario. Los análisis de regresión logística multivariada identificaron como predictores a la clase de Killip = II (RM = 9.3, p = 0.008), = 2 ondas Q en el ECG (RM = 1.8, p = 0.05), angioplastia exclusiva como método de reperfusión empleado (RM = 19.9, p = 0.017) y presencia de trombo intracoronario (RM = 11.9, p = 0.008). Conclusión: la detección anticipada del fenómeno de no reflujo permitirá establecer medidas tendientes a disminuir su presentación.

The hepatitis B virus (HBV) represents a worldwide public health problem; the virus is present in one third of the global population. However, this rate may in fact be higher due to occult hepatitis B virus infection (OBI). This condition is characterized by the presence of the viral genome in the liver of individuals sero-negative for the virus surface antigen (HBsAg). The causes of the absence of HBsAg in serum are unknown, however, mutations have been identified that produce variants not recognized by current immunoassays. Epigenetic and immunological host mechanisms also appear to be involved in HBsAg suppression. Current evidence suggests that OBI maintains its carcinogenic potential, favoring the progression of fibrosis and cirrhosis of the liver. In common with open HBV infection, OBI can contribute to the establishment of hepatocellular carcinoma. Epidemiological data regarding the global prevalence of OBI vary due to the use of detection methods of different sensitivity and specificity. In Latin America, which is considered an area of low prevalence for HBV, diagnostic screening methods using gene amplification tests for confirmation of OBI are not conducted. This prevents determination of the actual prevalence of OBI, highlighting the need for the implementation of cutting edge technology in epidemiological surveillance systems.

Allergic diseases affect a great proportion of seniors.To determine the prevalence of allergen sensitization in this age group in the western of Mexico.A retrolective study related to skin prick tests with a common allergen panel was performed on the first consultation in elderly patients during a period of three years in Guadalajara, Mexico.We included 60 subjects, with an average of 67.7 years old; 83.3% were women. Diagnosis of allergic rhinitis was present in 60%, allergic rhinitis plus asthma in 30%, and asthma alone in 10%. Non atopic comorbidities were present in 56.7% of the cases. Median serum IgE was 124.95 UI/ml, the average of positive skin prick tests was 4 (1 to 21); 13.3% were sensitized to just 1 aeroallergen, and 55% were to more than three aeroallergens. Sensitization to pollens was predominant, even more to tree pollens. One by one, house dust mite, Fraxinus sp and Amaranthus palmeri were the most commonly found. Diabetes was associated to a lower positive prick test (p = 0.021).The most common sensitizing allergens are similar for other age groups from the same area. Elderly patients with diabetes had lower positive allergen skin tests. Itís recommended to identify the allergen sensitization in seniors with allergic disease symptoms, as part of their clinical evaluation.

The oral allergy syndrome (OAS) is a common cause of food allergy in adults.Our objectives were to determine its frequency and to describe its clinical characteristics among Mexican subjects with nasal pollinosis.The diagnosis of OAS was made using the clinical history and fresh food prick-by-prick tests. The sample to estimate the frequency consisted of 100 consecutive subjects with nasal pollinosis. The clinical features of OAS were described in thirty patients of this sample.The frequency of OAS among the subjects was 13%, the average age was 29.9 years and 26 subjects were women. The most common symptoms were oropharyngeal pruritus, followed by lip edema, starting mainly within the first minute after food ingestion. The median for the serum IgE levels was 160 UI/ml, while the mean for total eosinophils was 278. We observed that predominant sensitizing aeroallergens were trees, among them, oaks. A total of 23 different OAS-related foods was detected: peach (23 cases), apple (18 cases), pear (8 cases) and almond (7 cases). The OAS evolution time correlated significantly with the evolution time of allergic rhinitis (rho=0.49 P=0.006) and the duration of OAS symptoms (rho=0.37 P=0.05), whereas these last two variables showed an interdependent correlation (rho=0.52 P=0.003).The patients with nasal pollinosis have a considerable proportion of OAS. In subjects sensitized to pollen of oaks and/or alders, OAS should be suspected.

Design and develop a mobile messaging system called mSalUV, which reminds patients with type 2 diabetes about taking their medication and attending appointments and promotes healthy lifestyles, and explore patient opinion regarding use of the system.The study had three stages: 1) design and development of mSalUV; 2) design and development of text messages; and 3) exploration of patient opinion regarding use of mSalUV.The mSalUV system was created and around 40 text messages were designed. A total of 1 850 messages were sent during the 45 day intervention. Patients thought that mSalUV helped them to treat their disease and that it was easy to use, they were inclined to continue using the system in the future, and people close to them (family members and support networks) supported them using it. Furthermore, they perceived the system as "someone that cared about them."The results suggest that a system such as mSalUV would be well accepted by its potential users; in this case, people with type 2 diabetes. This presents a very interesting scenario that will enable taking advantage of new technologies for health. Further research is needed to evaluate the system's efficiency and effectiveness.

Introduction Given the number of changes that occur during late adolescence, it is possible that the behavior of asthma may also be different. The aim of our paper is to determine the prevalence of asthma in a population of late adolescents and its possible association with obesity, tobacco smoke exposure and family history of allergic disease. Methods and subjects In a cross-sectional, population-based analytical study design, we selected a stratified random sample of subjects aged 15–18. By modeling risk through logistic regression, we assessed the relationship between asthma and the following covariables: gender, obesity, excess weight, family history of allergic disease and tobacco smoke exposure. Results 1600 subjects were included, and the following prevalences were identified: asthma 7.8%, obesity 9.6%, active smoking in the father 2 9.8%, active smoking in the mother 18.6% and active smoking habit in the study subjects 15.1%. In the final model, a significant association was found between asthma and the following variables: (1) asthma in the mother (adjusted OR [aOR]=2.95, 95% CI, 1.55–5.6), (2) history of allergic rhinitis (aOR=4.66, 95% CI, 2.63–8.25), and (3) male sex (aOR=1.48, 95% CI, 1.02–2.15). No association was seen with obesity or tobacco smoking of the parents. Conclusion Our results suggest that maternal history of asthma, personal history of allergic rhinitis and male sex are related with asthma late adolescence, while smoking and being overweight are not.

Objective.We evaluated the utility of 6 generic and 2 specific risk indices for identifying low bone mineral density (BMD) or osteoporosis in women with rheumatoid arthritis (RA); and their correlation with 10-year probability of fractures as assessed with the World Health Organization fracture risk assessment (FRAX) tool.Methods.Mexican Mestizo women with RA were evaluated in this cross-sectional study using 6 generic indices [Simple Calculated Osteoporosis Risk Estimation (SCORE); Osteoporosis Risk Assessment Instrument (ORAI); Osteoporosis Self-Assessment Tool; Age, Body Size, No Estrogen; Osteoporosis Index of Risk (OSIRIS); and Guidelines of the US National Osteoporosis Foundation], 2 specific indices (Amsterdam and modified Amsterdam), and FRAX. BMD results on dual-energy x-ray absorptiometry (DEXA) at the lumbar spine and femoral neck were considered the “gold standard.” Sensitivity, specificity, and predictive values (PV) of the indices and their correlations with FRAX results were estimated.Results.Among 191 patients, 46 had osteoporosis (24.1%) and 119 had low BMD (62.3%). For predicting osteoporosis, SCORE showed the highest sensitivity (96%), whereas OSIRIS (87%) and ORAI (82%) showed the highest specificities. OSIRIS also had the greatest positive PV (92%). The specific indices had low sensitivity and low specificity (Amsterdam, 50% and 79%, respectively; modified Amsterdam, 56% and 70%). All the indices had a low but significant correlation with FRAX.Conclusion.These findings support the use of some generic indices to identify patients with RA who should undergo DEXA testing. Currently available specific indices did not perform satisfactorily. New specific risk indices for osteoporosis in RA should be developed to increase sensitivity and specificity for predicting osteoporosis.

Cutaneous test practice to identify atopy is recommended in persons who suffer persistent asthma.To estimate the prevalence of allergen sensitization in a group of adult asthma patients; plus, to investigate about the differences that exist between atopic and non-atopic asthmatic persons.Consecutive asthmatic outpatients were evaluated for the first time in an Allergology Service. They were analyzed through clinical history, physical examination, and allergic cutaneous reactivity to any of 38- allergen panel. IgE serum levels were quantified too.One hundred eighty one asthmatic patients were included. Mean age was 40 years; 75.1% were women; in 59.9%, asthma was accompanied by allergic rhinitis; 81.2% fulfilled the atopy criteria. On average, each patient had 7.2 positive skin tests, corresponding to house-dust mite the highest frequency, followed by cockroach. Subjects age correlated negatively with the amount of positive cutaneous test and IgE serum levels, and positively with severity of asthma. IgE serum level higher than 100UI/ml was identified as a predictive factor for atopic asthma (OR = 3.1, p = 0.03).A substancial amount of asthmatic patients were atopic. The most highly sensitizing allergens were house-dust mite and cockroach. Differences between atopic and non-atopic asthmatics depended on their age and IgE serum levels.

In Mexico, 20% of cases of tuberculosis (TB) are associated with diabetes mellitus (DM). However, the behavior of the factors related to this comorbidity is unknown, so the aim of this study was to estimate the risk factors and outcome for TB-DM in a population from the state of Veracruz, Mexico. We developed a double-design study: cases and controls for the estimation of risk factors, and a retrospective cohort for the outcome factors. The populations surveyed were 67 patients with the comorbidity TB-DB and 109 with TB. The risk factors for tuberculosis in the diabetic population studied were: age ≥ 35 with an OR of 2.5 (95% CI: 1.4-4.3) and IMC ≥ 25 with an OR of 8.5 (95% CI: 3.1-23.3). According to the outcome variables, the patients with TB-DM showed an increased risk of 2.8 (95% CI: 2.2-3.4) for the development of drug resistance against tuberculosis. In conclusion, age and overweight are important risk factors, and drug resistance is an important outcome factor for the binomial TB-DM in the population from Veracruz. This information will have important effects on the development of surveillance programs against TB, with emphasis on the characteristics of the diabetic population.

INTRODUCTION: The situation of tuberculosis (TB) is being modified by the human immunodeficiency virus (HIV), which is increasing the occurrence of new cases and the generation of drug resistant strains, affecting not only the people infected with HIV, but also their close contacts and the general population, conforming a serious public health concern. However, the magnitudes of the factors associated to this co-infection differ considerably in relation to the population groups and geographical areas. METHODS: In order to evaluate the prevalence and risk factors for the co-infection of tuberculosis (TB) in a population with human immunodeficiency virus (HIV+) in the Southeast of Mexico, we made the analysis of clinical and epidemiological variables and the diagnosis of tuberculosis by isolation of mycobacteria from respiratory samples. RESULTS: From the 147 HIV+ individuals analyzed, 12 were culture positive; this shows a prevalence of 8% for the co-infection. The only variable found with statistical significance for the co-infection was the number of CD4-T < 200 cells/mm³, OR 13 (95%, CI 2-106 vs 12-109). CONCLUSIONS: To our knowledge this is the first report describing the factors associated with tuberculosis co -infection with HIV in a population from Southern Mexico. The low number of CD4 T-cells was the only variable associated with the TB co-infection and the rest of the variables provide scenarios that require specific and particular interventions for this population group. Com isso, aumentou-se a ocorrência de novos casos de TB e a geração de cepas resistentes à droga, afetando não só as pessoas infectadas com HIV, mas também seus contatos próximos e da população em geral, gerando um sério problema de saúde pública. No entanto, a magnitude dos fatores associados à esta coinfecção diferem consideravelmente em relação aos grupos populacionais e áreas geográficas. MÉTODOS: Para avaliar a prevalência da comorbilidade e fatores de risco da coinfecção de tuberculose (TB) em uma população com o vírus da imunodeficiência humana (VIH+) no sudeste do México, nós fizemos a análise das variáveis clínicas e epidemiológicas e de isolamento da micobactérias através de cultura de amostras respiratórias. RESULTADOS: A partir de 147 indivíduos HIV+ analisados, 12 foram positivos na cultura, o que mostra uma prevalência de 8% para a coinfecção. A única variável com significância estatística encontrada para a coinfecção foi o número de células CD4-T
