On the example of the diagnosis of the structural basis of focal epilepsy in an adult patient, the possibilities of a multimodal and interdisciplinary approach to diagnosis, combining the latest methods of neuroimaging with the results of neurophysiological examinations, are considered. The interaction and high qualification of specialists in epileptology, neuroradiology and pathomorphology provide a high probability to determine the cause of the focal forms of epilepsy. Along with the introduction of super-inductive MR systems, it is important to use their capabilities correctly and optimize the scanning protocol for the individual characteristics of the patient. With a long-term pharmacoresistant course of focal epilepsy, accompanied by low quality of life, the progression of neurological deficits and the aggravation of cognitive and personality problems in patients, it becomes obvious that these patients, even with negative results of standard MRI, are candidates for a more in-depth comprehensive examination to identify the structural basis of epileptogenesis and subsequent epileptic surgery. However, the problem of epilepsy pseudoresistance should be considered, when a patient with uncontrolled seizures takes antiepileptic drugs for a long time in inadequate dosages. Modern comprehensive diagnostics offers new rational approaches to antiepileptic therapy indication on the part of the doctor, as well as to improve the patient's compliance to the treatment.На примере диагностики структурной основы фокальной эпилепсии у взрослого пациента рассматриваются возможности мультимодального и междисциплинарного подхода к диагностике, совмещающего новейшие методы нейровизуализации с результатами нейрофизиологических обследований. Взаимодействие и высокая квалификация специалистов в области эпилептологии, нейрорадиологии и патоморфологии с большой вероятностью позволяет установить причину развития фокальных форм эпилепсии. Наряду с внедрением супериндуктивных МР-систем важное значение имеет правильное использование их возможностей и оптимизация протокола сканирования под индивидуальные особенности пациента. При длительном фармакорезистентном течении фокальной эпилепсии, особенно на фоне низкого качества жизни, прогрессирования неврологического дефицита и усугубления когнитивных и личностных проблем у пациентов, становится очевидным факт, что эти пациенты даже при негативных результатах стандартной МРТ являются кандидатами для более углубленного комплексного обследования с целью идентификации структурной основы эпилептогенеза и последующей эпилептической хирургии. Однако следует отметить так называемую проблему псевдорезистентности эпилепсии, когда пациент с неконтролируемыми приступами длительное время принимает противоэпилептические препараты в неадекватных дозах. Современная комплексная диагностика позволяет предложить новые рациональные режимы противоэпилептической терапии, а также улучшить комплаентность пациента.

To study the therapeutic efficacy of two treatment modes of peptidergic nootropic medication cortexin in children with developmental dysphasia aged 3-4 years.Ninety-four children with developmental dysphasia were divided into three groups. In group 1 (27 patients), cortexin was administered once a day intramuscularly for 10 days. After this course, the children were not prescribed drug therapy, a second examination was carried out 2 months after the start of treatment. In group 2 (40 patients), two courses of cortexin were administered at 1-month intervals, and the children were also followed up for two months. Control group 3 (27 patients) did not receive medication, but was also followed up for two months. All the parents were provided with recommendations for the stimulation of speech development in children. Before the study and two months later, speech development was assessed with special scales and questionnaires for parents.The increase ratio of the active vocabulary volume by 2 times or more was observed in group 2 (two courses of cortexin treatment) in 80.0% of patients (significant differences with groups 1 and 3,The results of the study confirm the higher effectiveness of two courses of the peptidergic nootropic medication cortexin in the pharmacotherapy of developmental dysphasia in children, aged 3-4 years, conducted over two months, compared with the indication of one treatment course.Оценить терапевтическую эффективность двух режимов терапии пептидергическим ноотропным препаратом Кортексин при дисфазии развития у детей в возрасте 3—4 лет.Обследованы 94 ребенка с дисфазией, распределенных на три группы. В 1-й группе (27 пациентов) проводился курс лечения Кортексином: 1 раз в сутки внутримышечно ежедневно в течение 10 дней. После этого курса детям не назначалась лекарственная терапия, повторное обследование осуществлялось через 2 мес от начала курса Кортексина. Во 2-й группе (40 пациентов) проведено два курса Кортексина с интервалом 1 мес, дети также наблюдались в течение 2 мес. Третья группа сравнения (27 пациентов) не получала медикаментозной терапии, но также наблюдалась в динамике в течение 2 мес. Родители всех пациентов получали рекомендации по стимуляции речевого развития у детей. Перед началом лечения и через 2 мес оценивали состояние речи детей по шкалам речевого развития и специальным опросникам для родителей.Кратность увеличения объема активного словаря в 2 раза и более отмечена во 2-й группе (2 курса лечения Кортексином) у 80,0% пациентов (значимые различия с 1-й и 3-й группами

Inducing of pathological autoimmune response contributes to the development of Miller Fischer and Guillain-Barré syndrome. Hyperergic reaction with the generation of the so-called «cytokine storm» provokes multisystem hemorrhagic complications such as Kawasaki disease and acute necrotizing hemorrhagic encephalopathy. There is also a special form of COVID-19-associated stroke. To evaluate the level of adaptive functioning in patients with attention deficit hyperactivity disorder (ADHD).One hundred children and adolescents with ADHD (79 male and 21 female) were examined using M. Weiss functional impairment rating scale (WFIRS). The total score on the ADHD-DSM-IV was at least 24 and ADHD severity was rated as «Moderately ill» or «Markedly ill» on CGI-S-ADHD scale.Typical for ADHD difficulties with adaptive functioning in «Family», «Learning and school», «Life skills», «Social activities» domains are more pronounced in «Markedly ill» patients compared to «Moderately ill» and increase during the transition from childhood to adolescence. Comorbid disorders (oppositional defiant disorder and anxiety disorders) lead to a significant increase in adaptive functioning impairments in most domains of daily life, including «Family», «Learning and school», «Child's self concept», «Social activities» and «Risky activities».It is necessary not only to assess core symptoms in the follow-up of patients with ADHD, but also evaluate comorbid disorders and the level of functional impairment across the main domains of daily life.Определение уровня адаптивного функционирования у пациентов с синдромом дефицита внимания и гиперактивности (СДВГ).Обследовали 100 детей и подростков с СДВГ, 79 мужского и 21 женского полов, с помощью шкалы оценки функциональных нарушений М. Вейсс (WFIRS). Суммарный балл выраженности симптомов СДВГ по шкале ADHD DSM-IV в этих случаях был не менее 24 при умеренной или выраженной степени тяжести СДВГ по шкале общего клинического впечатления (CGI-ADHD-severity).Характерные для пациентов с СДВГ нарушения адаптивного функционирования в таких областях, как «семья», «учеба и школа», «базовые жизненные навыки», «общение и социальная активность», больше проявлялись при выраженной тяжести СДВГ по сравнению с умеренной и нарастали при переходе от детского к подростковому возрасту. Было установлено, что наличие у пациентов с СДВГ коморбидных расстройств (оппозиционно-вызывающее расстройство поведения и тревожные расстройства) приводит к значительному нарастанию нарушений адаптивного функционирования в большинстве сфер повседневной жизни, в том числе таких как «семья», «учеба и школа», «самооценка ребенка», «общение и социальная активность», «поведение, сопряженное с риском». Авторы подчеркивают необходимость динамического наблюдения за пациентами с СДВГ с оценкой не только основных симптомов СДВГ, но также проявлений коморбидных расстройств и показателей адаптивного функционирования в основных областях повседневной жизни.

To assess the formation of positive personality phenomena in patients with mild cognitive impairment and asthenic syndrome during the treatment with recognan (citicoline).Thirty-eight patients (17 men and 21 women), aged 18 to 45 years (mean age 27.8±12.1 years), with asthenic syndrome with mild cognitive impairment (ICD-10 F06.7) were examined. Patients were divided into two groups: 20 people in the main group and 18 people in the comparison group. The main group received recognan (orally, in solution, 100 mg in 1 ml) for 30 days, the daily dosage of the drug was 0.5 g (5 ml solution). The comparison group did not receive any medications. Adapted methods of positive personality psychology were used: the Fordyce Emotions Questionnaire, the Subjective Happiness Scale (SHS), the Adult Hope Scale (AHS), the Satisfaction with Life Scale (SWLS), M. Atkinson's Scale of Emotional Maturity, the projective technique 'Map of experiences'. The follow-up period was 30 days. All subjects were examined three times (at baseline, 15 and 30 days after treatment).After a month of treatment with recognan, there was an improvement of positive personality traits and a significant decrease in negative experiences, indicating the positive impact of the drug on the formation of positive personality manifestations and compensation for emotional disorders in patients with mild cognitive impairment and asthenic syndrome.Цель исследования. Оценка особенностей формирования позитивных личностных феноменов у пациентов с легкими когнитивными нарушениями и астеническим синдромом, а также их динамики в процессе использования препарата Рекогнан (цитиколин). Материал и методы. Обследовали 38 пациентов,17 мужчин и 21 женщину, в возрасте от 18 до 45 лет (средний возраст 27,8±12,1 года), у которых диагностирован астенический синдром с легкими когнитивными расстройствами (рубрика F06.7 по МКБ-10). Больные были разделены на две группы: 20 человек в основной группе и 18 человек в группе сравнения. Основная группа получала лечение препаратом Рекогнан (перорально, в растворе, 100 мг в 1 мл) в течение 30 дней, суточная дозировка препарата составляла 0,5 г (5 мл раствора). В группе сравнения лекарственную терапию не проводили. Для оценки состояния больных и личностных изменений применяли следующие психологические тесты и шкалы: 'Эмоциональный тест Фордайса', 'Шкала субъективного счастья', 'Шкала надежды', 'Шкала эмоциональной зрелости М. Аткинсона', 'Шкала удовлетворенности жизнью', проективная методика 'Карта переживаний'. Период наблюдения составил 30 дней. Все испытуемые были обследованы 3 раза (до начала исследования, на 15-й и 30-й дни). Результаты и заключение. После месячного курса лечения Рекогнаном отмечено улучшение показателей по шкалам, направленным на оценку позитивных изменений личности, а также достоверное уменьшение негативных переживаний, что свидетельствовало о положительном влиянии препарата на формирование у больных с легкими когнитивными нарушениями при астеническом синдроме позитивных личностных проявлений и компенсацию эмоциональных нарушений.

To study the therapeutic efficacy of peptidergic nootropic medication Cortexin during two-month follow-up in children with developmental dysphasia aged 3-4 years.Fifty-four children with developmental dysphasia were randomized into two equal groups. The cortexin group received the drug once daily intramuscularly during 10 days. After this course, children did not receive any pharmacotherapy and were examined two months after the beginning of treatment. The control group did not receive pharmacotherapy and was examined twice in two months interval. All the parents were provided with recommendations for the stimulation of speech development in dysphasic children. Before treatment and two months later, speech development was assessed by special scales and questionnaires for parents.After treatment with Cortexin, a significant improvement was achieved not only on the 'expressive language' scale but also on the 'attention to speech' and 'impressive language' scales. The volume of active vocabulary was increased by 2.3 times, the number of articulated phrases by 3.6 times. The evaluation of parents' questionnaires demonstrated the decrease of cerebrasthenic, psychosomatic problems, motor clumsiness, hyperactivity, attention deficit and problems with emotional control in children. The therapy contributed to the reduction of difficulties in communication and social adaptation. The positive effect of Cortexin was observed after the completion of treatment (the protracted effect).Цель исследования. Оценка терапевтической эффективности пептидергического ноотропного лекарственного препарата Кортексин при дисфазии развития у детей в возрасте 3-4 лет. Материал и методы. Обследовали 54 ребенка с дисфазией, рандомизированных на две группы по 27 пациентов. В 1-й группе проводили лечение Кортексином 1 раз в день внутримышечно в течение 10 дней; после этого лекарственную терапию не назначали, повторное обследование осуществляли через 2 мес от начала лечения. Во 2-й группе (контрольная) медикаментозную терапию не применяли, но детей также наблюдали в течение 2 мес. Родители всех пациентов получили рекомендации по стимуляции речевого развития у детей. Перед началом лечения и через 2 мес оценивали состояние речи у детей по шкалам речевого развития и специальным опросникам для родителей. Результаты и заключение. У детей с дисфазией развития, лечившихся Кортексином, в отличие от детей контрольной группы было достигнуто существенное улучшение показателей экспрессивной речи, а также речевого внимания и импрессивной речи. В 2,3 раза выросло число произносимых слов и в 3,6 раза - фраз. По данным анкетирования родителей, положительные результаты лечения Кортексином проявились также в регрессе церебрастенических симптомов, психосоматических нарушений, двигательной неловкости, гиперактивности, дефицита внимания, трудностей эмоционального контроля. Это способствовало улучшению у детей контактов с окружающими и социальной адаптации. Положительное влияние Кортексина отмечено и после окончания терапии (продолженный эффект).

To study clinical and neurophysiological data of early infantile epileptic encephalopathy type 14 caused by KCNT1 mutations.For the period 2017 to 2019, 3 non-relative girls with clinical characteristics of epilepsy of infancy with migrating focal seizures (EIMFS) and mutations in the KCNT1 gene are identified and studied. DNA sequencing was performed using the Hereditary epilepsy panel (Next Generation Sequencing on platform IlluminaNextSeq 500, USA). Dynamical video-EEG monitoring was done with "Encephalan-Video" RM-19/26 ("Medicom MTD", Russia).De novo KCNT1 mutations are identified and studied in three unrelated Russian girls: M.V., 3 years and 3 month old, T.V., 9 month old and M.A., 5 month old. M.V. has the previously unknown mutation in exon 12 (chr9:138656907CT) with amino acid substitution Arg356Trp. T.V. has the previously described mutation in chromosome 9: 138651532GA with amino acid substitution Gly288Ser (OMIM: 608167.0010). M.U. has the previously unknown mutation in exon 15 (chr9:138660712AG) with amino acid substitution Asp480Gly. M.V. has seizure onset at the age of 4 month with behavioral arrest seizures and tonic versive seizures. T.V. developed seizures at 4,5 month in the manner of behavior arrest and ophthalmo-clonic seizures with hyperemia of face. M.U. has neonatal seizures with bilateral tonic-clonic seizures, cyanosis and further development of status epilepticus of alternating hemiconvulsive seizures. Further all the girls develop polymorphic seizures of multiregional genesis up to migrating status epilepticus with typical electro-clinical pattern of EIMFS. Therefore, KCNT1 is likely to be a major gene causing this rare and severe epileptic syndrome.Цель исследования. Изучение клинико-электроэнцефалографических особенностей ранней младенческой эпилептической энцефалопатии (РМЭЭ) 14-го типа, обусловленной мутациями гена KCNT1. Материал и методы. За период 2017—2019 гг. обследованы 3 неродственные девочки (М.В. — 3 года и 3 мес, Т.В. — 9 мес и M.У. — 5 мес) с клинической картиной эпилепсии младенчества с мигрирующими фокальными приступами (ЭММФП) и идентифицированными мутациями в гене KCNT1. Секвенирование ДНК нового поколения (панель Наследственные эпилепсии) осуществлялось на платформе IlluminaNextSeq 500 (США). Мониторинг видео-ЭЭГ проводился посредством системы Энцефалан-Видео на базе РМ-ЭЭГ-19/26 ЭНЦЕФАЛАН-РМ (НПКФ «Медиком МТД», Россия). Результаты и заключение. У М.В. выявлена ранее не описанная мутация в 12-м экзоне гена KCNT1 (chr9:138656907CT) с аминокислотной заменой Arg356Trp. У Т.В. определена ранее известная мутация в 9 хромосоме 138651532GA с аминокислотной заменой глицина на серин в 288 позиции — Gly288Ser (OMIM: 608167.0010). У М.У. обнаружена ранее не описанная гетерозиготная мутация в 15-м экзоне гена KCNT1 (chr9:138660712AG), приводящая к аминокислотной замене в 480-й позиции белка (Asp480Gly). У М.В. дебют РМЭЭ был отмечен в возрасте 4 мес с гипомоторных диалептических и тонических версивных приступов. Эпилепсия у Т.В. дебютировала в возрасте 4,5 мес с гипомоторных приступов с офтальмоклониями и гиперемией лица. У М.У. отмечались неонатальные судороги с билатеральными тонико-клоническими приступами, цианозом, с последующим развитием эпилептического статуса альтернирующих гемиконвульсий. В дальнейшем у всех девочек развивались полиморфные приступы мультирегионального генеза вплоть до мигрирующего эпилептического статуса с характерной электро-клинической картиной ЭММФП. Таким образом, сделан вывод, что ген KCNT1 является, возможно, основным геном, детерминирующим развитие РМЭЭ.

Neurodevelopmental disorders (NDD) are characterized by disturbances of the formation of cognitive functions, communication skills, behavior characteristics and/or motor skills, which are caused by abnormalities in the course of the processes of neuroontogenesis. In the clinical practice of a pediatric neurologist and pediatrician, a significant part consists of patients with NDD without a general decrease in intelligence, primarily with speech development disorders, attention deficit hyperactivity disorder (ADHD), specific learning disorders (dyslexia, dysgraphia, dyscalculia). NDD represent a heterogeneous group of diseases, having multifactorial origin and a neurobiological nature, which are caused by genetic mechanisms and early (perinatal) brain damage. Among children with NDD, there is a higher occurrence of anxiety disorders compared to their peers. With NDD, early intervention is indicated, and its positive effect is possible during the period when the brain is most plastic and capable of changes. The published results of multicenter, double-blind, placebo-controlled, randomized clinical trials of pharmacotherapy with the medication "Tenoten for children" for ADHD, specific learning disorders, anxiety disorders and the consequences of perinatal damage to the central nervous system are reviewed.Расстройства нервно-психического развития (РНПР) — это нарушения формирования когнитивных функций, навыков общения, характеристик поведения и/или двигательных навыков, которые обусловлены отклонениями протекания процессов нейроонтогенеза. В клинической практике детского невролога и педиатра существенную часть составляют пациенты с РНПР без общего снижения интеллекта, прежде всего с нарушениями развития речи, синдромом дефицита внимания с гиперактивностью (СДВГ), специфическими трудностями обучения (дислексией, дисграфией, дискалькулией). РНПР представляют собой гетерогенную группу заболеваний, имеют мультифакториальный характер и нейробиологическую природу, обусловлены генетическими механизмами и ранними (перинатальными) повреждениями мозга. Среди детей с РНПР отмечается более высокая по сравнению с ровесниками частота встречаемости тревожных расстройств. При РНПР показано раннее вмешательство, и его положительный эффект возможен в тот период, когда мозг наиболее пластичен и способен к изменениям. Рассматриваются опубликованные результаты многоцентровых двойных слепых плацебо-контролируемых рандомизированных клинических исследований фармакотерапии препаратом Тенотен детский при СДВГ, специфических трудностях обучения, тревожных расстройствах, последствиях перинатального поражения центральной нервной системы.

The aim of the study was to study the effect of the drug recognan (citicoline) on the neurodynamic characteristics of mental activity in patients with mild cognitive impairment.A survey of 58 subjects (17 of them male and 41 female) aged 18-45 years (average age 27.2±12.5 years) was conducted. Clinical diagnosis according to ICD-10 «Mild cognitive impairment» (F06.7). The main subgroup included29 people received oral recognan therapy (in solution, 100 mg in 1 ml) for 30 days, with a daily dosage of 500 mg (5 ml of solution). In the control group (29 people) drug therapy was not performed. Tests were used: «Graphic sample», the sample on the reciprocal coordination of the hands (Ozeretsky test), test for the compression of fingers, the test «number series». The follow-up period was 30 days. All subjects were examined three times (initially, in the middle of the study - on day 15, at the end of the study - on day 30).The results obtained showed that the use of the drug recognan (citicoline) has a positive effect on the indicators of visual-motor coordination and spatial representations, neurodynamic characteristics of movement, and cognitive functions. After a 2-week treatment with recognan improved graphical sample, 84% of patients, with reliable improvement according to the Wilcoxon (Изучение влияния препарата Рекогнан (цитиколин) на нейродинамические характеристики психической деятельности у пациентов с легкими когнитивными нарушениями.Проведено обследование 58 пациентов (17 мужчин и 41 женщина) в возрасте 18—45 лет (средний возраст 27,2±12,5 года) с клиническим диагнозом по МКБ-10 «Легкое когнитивное расстройство» (F06.7). В основную группу вошли 29 пациентов, которые получали пероральную терапию Рекогнаном (в растворе, 100 мг в 1 мл) в течение 30 дней, при этом суточная дозировка препарата составляла 500 мг (5 мл раствора). В группе сравнения (29 человек) медикаментозная терапия не проводилась. Использовались тесты «Графическая проба», проба на реципрокную координацию рук (проба Озерецкого), проба на сжимание пальцев, тест «Числовые ряды». Все пациенты обследовались трижды (до начала терапии, на 15-й день терапии, после лечения — на 30-й день).Полученные результаты показали, что применение препарата Рекогнан (цитиколин) оказывает положительное влияние на показатели зрительно-моторной координации и пространственных представлений, нейродинамические характеристики движения, когнитивные функции. После 2 нед лечения Рекогнаном отмечалось улучшение выполнения графической пробы у 84% обследованных, с достоверным улучшением (

OBJECTIVE The aim of the observational program was to study the effect of the use of the drug recognan (citicoline) on the state of higher mental functions (memory, attention, visual-motor coordination, dynamic praxis,verbal thinking and imagination) in patients with mild cognitive impairment. MATERIAL AND METHODS A survey of 54 subjects (16 of them male and 38 female) aged 18-50 years (average age 28.5±10.5 years) was conducted with a diagnosis of «Mild cognitive impairment» («F06.7»). The group was randomized into 2 subgroups: the main subgroup (26 people) received oral therapy with the drug Recognan, for 30 days, with the daily dosage of the drug being 500 mg. In the control group (28 people), nootropic drug therapy was not performed. Standard psychometric techniques were used to study higher mental functions. All subjects were examined three times (initially, in the middle of the study - on day 15, at the end of the study - on day 30). RESULTS AND CONCLUSION After 2 weeks of treatment with recognan, there was an improvement in concentration in 81.9%, memory in 50% (p=0.008), verbal imagination productivity in 68.2% (p=0.015), counting functions in 60% (p=0.015), visual-motor coordination and dynamic praxis - in 86.4% (p=0.003), increased speed and efficiency of mental work (p=0.001). After a 30-day course of treatment with recognan, there was an improvement in memory in 58.3% of patients (p=0.007), an increase in concentration in 64%, an improvement in counting functions in 64.3% (p=0.011), verbal imagination productivity in 63.3%, visual-motor coordination and dynamic praxis in 86.4% (p=0.007), speed and efficiency of mental work (p=0.006), which indicates a complex positive effect of recognan on higher mental functions in patients with mild cognitive impairment cognitive impairment.

The data on the mechanisms of action of the new domestic drug prospecta, the results of its clinical trials in patients with VCD are presented.25—26 июня 2021 г. в Казани состоялся круглый стол с участием ведущих неврологов России, на котором обсуждались вопросы лечения пациентов с когнитивными нарушениями вследствие цереброваскулярных заболеваний. Когнитивные нарушения сосудистого генеза (СКН) широко распространены в популяции, являются частой причиной снижения качества жизни и ограничения повседневной активности. Причиной СКН являются как острые, так и хронические цереброваскулярные заболевания. Эффективный способ предупреждения СКН — контроль факторов сердечно-сосудистого риска, обеспечение достаточного уровня когнитивных и физических нагрузок на протяжении жизни. Исключительно важна роль медикаментозной терапии, направленной в том числе на нормализацию обменных процессов в головном мозге. Приведены данные о механизмах действия нового отечественного препарата Проспекта, результатах его клинических испытаний у пациентов с СКН.

Approximately 30% of individuals with autism spectrum disorder (ASD) experience developmental regression, the etiology of which remains largely unknown. We performed a complete literature search and identified 47 genes that had been implicated in such cases. We sequenced these genes in a preselected cohort of 134 individuals with regressive autism. In total, 16 variants in 12 genes with evidence supportive of pathogenicity were identified. They were classified as variants of uncertain significance based on ACMG standards and guidelines. Among these were recurring variants in GRIN2A and PLXNB2, variants in genes that were linked to syndromic forms of ASD (GRIN2A, MECP2, CDKL5, SCN1A, PCDH19, UBE3A, and SLC9A6), and variants in the form of oligogenic heterozygosity (EHMT1, SLC9A6, and MFSD8).

Traumatic brain injury (TBI) clinical course and outcomes in children have peculiarities as the damage impacts brain, which growth and maturation are continuing. Thus, TBI interferes into normal processes of neuroontogenesis leading to negative consequences on the cognitive functions development, school education, social skills acquisition. Cognitive and behavioral disorders in children and adolescents in the long-term period of TBI become more prominent in co-occurrence with paroxysmal disorders, including posttraumatic headaches, posttraumatic epilepsy and subclinical epileptiform activity on the EEG. Therapeutic and rehabilitation procedures in in the long-term period of TBI in children and adolescents should be conducted not only during the first 12 months after injury, when they are expected to be the most efficient, but also later on taking into consideration continuing processes of the CNS morphological and functional maturation along with the high neuroplasticity of the developing brain.Своеобразие клинического течения и исходов ЧМТ у детей обусловлено тем, что повреждение воздействует на мозг, рост и развитие которого еще не закончено. ЧМТ вмешивается в нормальный ход процессов нейроонтогенеза, что негативно отражается на развитии когнитивных функций, школьном обучении, формировании социальных навыков. Когнитивные и поведенческие нарушения у детей и подростков в отдаленном периоде ЧМТ значительно нарастают при наличии пароксизмальных расстройств: посттравматической головной боли, посттравматической эпилепсии, субклинической эпилептиформной активности на ЭЭГ. Терапевтические и реабилитационные мероприятия в отдаленном периоде ЧМТ травмы у детей и подростков должны интенсивно проводиться как в первые 12 месяцев после ЧМТ, когда от их применения правомерно ожидать наиболее значительных результатов, так и в дальнейшем, с учетом продолжающихся процессов морфо-функционального созревания ЦНС и высокой нейропластичности развивающегося мозга.

To evaluate the characteristics of neurodevelopment by the age of 5-8 years in children who were born very preterm with extremely low body weight (ELBW) and very low body weight (VLBW).Seventy-two patients, aged from 5 years to 8 years were examined. Patients were divided into group I (36 patients born preterm with ELBW (16 boys, 20 girls)) and group II (36 patients born with VLBW (16 boys, 20 girls)). The control group included 30 healthy peers (16 boys, 14 girls). All children were assessed by means of the Griffiths Mental Development Scales, Extended Revised: 2 to 8 years (GMDS-ER 2-8).The value of general quotient (GQ) in patients born with ELBW (73.4±2.1) was significantly lower compared to their healthy peers (80.9±2.1; p=0.036). The same tendency was found in patients born with VLBW who's GQ was decreased by 73.1±3.0 (p=0.101). Concurrently the patients demonstrated lower scores on all six scales, which achieved significant difference with the controls for the 'Locomotor' and 'Performance' scales in group I, and for the 'Performance' and 'Practical reasoning' scales in group II. The boys born with ELBW tended to demonstrate lower results on all six scales and received significantly lower scores on the 'Locomotor' and 'Language' scales. The boys born with VLBW demonstrated significantly lower scores on the 'Language', 'Eye and hand co-ordination' and 'Practical reasoning' scales. In girls significantly lower score on the 'Performance' scale only was shown for those, who were born with ELBW.Early diagnosis and characterization of developmental delays in children born very preterm determine the effectiveness of therapeutic measures based on the individual approach and comprehensive medical-psychological-pedagogical support.Цель исследования. Оценка нарушений нервно-психического развития у детей в возрасте 5-8 лет, родившихся глубоко недоношенными с экстремально низкой (ЭНМТ) или очень низкой (ОНМТ) массой тела. Материал и методы. Обследовали 72 пациентов в возрасте от 5 до 8 лет. Детей разделили на две группы: по 16 мальчиков и 20 девочек в каждой. В 1-ю группу вошли дети, которые были рождены глубоко недоношенными с ЭНМТ, во 2-ю - с ОНМТ. Группу контроля составили 30 (16 мальчиков, 14 девочек) здоровых, сопоставимых по возрасту. Для оценки нервно-психического развития всех детей применяли шкалу Гриффитс (GMDS-ER 2-8), состоящую из 6 субшкал. Результаты. Значения общего показателя нервно-психического развития (GQ) были статистически значимо более низкими у детей 1-й группы по сравнению с контролем (73,4±2,1 и 80,9±2,1 соответственно; р=0,036). У пациентов 2-й группы GQ также имел тенденцию к снижению и составил 73,1±3,0 (р=0,101). При дифференцированном анализе шкалы Гриффитс в 1-й группе снижение показателей достигало статистически значимых различий по субшкалам 'двигательная активность' и 'выполнение действий', во 2-й - по субшкалам 'выполнение действий' и 'практическое мышление'. У мальчиков 1-й группы статистически значимым было снижение баллов по субшкалам 'двигательная активность' и 'речь', 2-й группы - по субшкалам 'речь', 'зрительно-моторная координация', 'практическое мышление'. Среди девочек значимое снижение показателей было выявлено только в 1-й группе по субшкале 'выполнение действий'. Заключение. Ранняя диагностика и определение характера отставаний в развитии детей, родившихся глубоко недоношенными, определяет эффективность лечебно-реабилитационных мероприятий, при этом необходимы индивидуальный подход и комплексное медико-психолого-педагогическое сопровождение.

Speech and language development may be impaired in all forms of epilepsy involving specialized functional areas in the dominant cerebral hemisphere and their connections. The concept of epilepsy-aphasia clinical spectrum was recently proposed, but the notion of aphasia is quite conditional here as many of these patients demonstrate disorders of speech and language development from their infancy. Those forms of epilepsy are considered as continuum from the most severe Landau-Kleffner syndrome (LKS) and epilepsy with continuous spike-and-wave during sleep (CSWS) (also indicating as electrical status epilepticus during sleep - ESES) to intermediate epilepsy-aphasia disorders (with incomplete correspondence to diagnostic criteria of LKS and epilepsy with CSWS). The mild end of the spectrum is represented by benign childhood epilepsy with centrotemporal spikes (rolandic), which is often associated with speech and language disorders. The importance of genetic factors is discussed, including mutations in SRPX2, GRIN2A and other genes. The perspectives of individualized pharmacotherapy in epilepsy, co-morbid with neurodevelopmental disorders or impairments of speech and language development, are depending on the progress in genetic studies. In the new generation of antiepileptic drugs the positive influence on neuroplasticity mechanisms and higher cerebral functions are supposed for levetiracetam.Речь и ее развитие могут страдать при всех формах эпилепсии, затрагивающих специализированные центры в доминантном большом полушарии и их формирующиеся связи. В последние годы сформулирована концепция спектра расстройств 'эпилепсия-афазия', при этом указание на афазию представляется достаточно условным, поскольку у значительной части этих пациентов с раннего возраста отмечаются нарушения речевого развития. Соответствующие формы эпилепсии рассматриваются как клинический континуум, в котором самыми тяжелыми являются синдром Ландау-Клеффнера и эпилепсия с CSWS (также обозначается как эпилепсия с электрическим эпилептическим статусом медленноволнового сна - ESES), за ними следует промежуточная форма расстройства спектра 'эпилепсия-афазия' (с неполным соответствием критериям синдрома Ландау-Клеффнера и эпилепсии с CSWS), а наименьшей степенью тяжести обладает доброкачественная эпилепсия детства с центротемпоральными спайками (роландическая), при которой часто отмечаются речевые расстройства. Обсуждается роль генетических изменений при данных формах эпилепсии, в том числе мутаций генов SRPX2, GRIN2A и др. Перспективы индивидуализированной фармакотерапии эпилепсии, коморбидной с нарушениями нервно-психического развития и формированием речи, связаны с достижениями генетики. Среди современных эффективных противоэпилептических препаратов в аспекте положительного влияния на патогенез болезни выделен леветирацетам.

The problem of epilepsy comorbidity with autism spectrum disorders in children is discussed. The incidence data of autism spectrum disorders in epilepsy, epileptiform discharges on the EEG in autism spectrum disorders and epilepsy in autism spectrum disorders are reviewed. The following types of epilepsy and autism co-occurrence are discussed: both conditions are independent, have different causes and may co-occur by chance; epilepsy and autism are associated, both being independent consequences of the same genetic disorder or early cerebral damage; autism is caused by the epileptic process which interferes with the function of specific brain networks involved in the development of communication and social behavior; autism is a result of the withdrawal reaction in the epileptic child. The known genetic causes of epilepsy and autism spectrum disorders comorbidity are provided. The practical issues are discussed, in particular the rational indication of antiepileptic drugs to the children suffering autism spectrum disorders.Рассматривается проблема коморбидности расстройств аутистического спектра (РАС) с эпилепсией у детей. Приводятся данные о встречаемости РАС при эпилепсии, эпилептиформных разрядов на ЭЭГ при РАС, а также эпилепсии при РАС. Рассматриваются возможные варианты ассоциации эпилепсии и РАС: РАС и эпилепсия могут развиться одновременно, но независимо, и вызываются разными причинами; РАС и эпилепсия взаимосвязаны, являются проявлением одного и того же генетического дефекта или раннего повреждения ЦНС; РАС - следствие эпилепсии, начинающейся или распространяющейся на развивающиеся нейронные сети, ответственные за коммуникацию и социальное поведение; РАС - результат реакции на изоляцию у детей с эпилепсией. Приводятся примеры сочетания эпилепсии и РАС, которые имеют общую генетическую природу. Обсуждаются практические аспекты проблемы коморбидности эпилепсии и РАС, в частности целесообразность назначения антиэпилептических препаратов детям с РАС.

Prion diseases, or transmissible spongiform encephalopathies, are a group of neurodegenerative diseases with progressive dementia and movement disorders. There are three variants of prion diseases pathogenesis: direct contamination, genetic and sporadic forms. The following clinical forms are known: Creutzfeldt-Jakob disease (common type), variant Creutzfeldt-Jakob disease, Gerstmann-Straussler-Scheinker disease, variably protease-sensitive prionopathy, fatal insomnia and fatal familial insomnia, kuru, prion disease associated with diarrhea and autonomic neuropathy. Clinical characteristic of prion diseases, molecular-genetic aspects of their pathogenesis and current diagnostic approaches are discussed. Because of the lack of effective treatment, prevention of both alimentary prion infections (consumption of contaminated meat products) and transmissible iatrogenic infections (the use of biopreparations from animal tissues) is important. The safety of such biopreparations should be ensured by modern manufacturing technologies and specially developed procedures that meet international requirements and standards.Прионные заболевания, или трансмиссивные губкообразные энцефалопатии - группа нейродегенераций, характеризующихся быстро прогрессирующими деменцией и двигательными нарушениями. Существуют три варианта возникновения прионных заболеваний: прямое заражение, наследственные и спорадические (возникающие спонтанно) формы. Известны следующие клинические формы прионных заболеваний человека: болезнь Крейтцфельда-Якоба (основной тип), вариант болезни Крейтцфельда-Якоба, болезнь Герстманна-Штрауслера-Шейнкера, вариабельная протеаза-чувствительная прионопатия, фатальная бессонница и фатальная семейная бессонница, куру, прионное заболевание, ассоциированное с диареей и вегетативной нейропатией. Рассматриваются клинические особенности прионных заболеваний, молекулярно-генетические аспекты их патогенеза, принципы современной диагностики. В связи с отсутствием эффективного лечения важное значение имеет профилактика заражений прионами как алиментарным (употребление зараженных мясных продуктов), так и трансмиссивным ятрогенным (применение лекарственных биопрепаратов, получаемых из тканей животных) путями. Безопасность биопрепаратов от загрязнения прионами должна обеспечиваться современными технологиями производства и специально разработанными процедурами, соответствующими международным требованиям и стандартам.

To assess the efficacy and safety of сortexin in the treatment of children with cerebral palsy (CP) combined with epilepsy.Eighty-four patients (55 boys and 29 girls), aged from 1 to 11 years, with CP combined with epilepsy received cortexin together with antiepileptic drugs (AEDs). Cortexin was administered in doses of 5-10 mg depending on the patient's age and body weight intramuscularly during hospitalization.Cortexin as add-on to AEDs reduced for more than two times the number of seizures, along with improvement of motor function, in 31 (36.9%) patients. The improvement of motor function, but without a significant decrease in epileptic seizures, was achieved in 15 (17.8%) of the patients. Reduction of epileptic seizures frequency (2 times), but without a significant effect on motor function, was observed in 14 cases (16.7%). Twenty-three patients (27.4%) did not respond the therapy. The aggravation of epileptic seizures during cortexin therapy was observed in only 1 girl with West syndrome (1.2%), and this was significantly lower than the probability of seizures aggravation on AED. Polypeptide nootropic medication cortexin demonstrated efficacy and safety as adjunctive therapy in children with CP combined with epilepsy.Цель исследования. Анализ эффективности и безопасности применения кортексина у пациентов с детским церебральным параличом (ДЦП) и эпилепсией. Материал и методы. Курс кортексина в сочетании с терапией противоэпилептическими препаратами (ПЭП) получали 84 пациента (55 мальчиков и 29 девочек) в возрасте 1-11 лет с ДЦП и эпилепсией. Дозы препарата подбирали в зависимости от возраста и массы тела пациентов, вводили по 5-10 мг внутримышечно в условиях стационара. Результаты и заключение. Добавление кортексина к ПЭП более чем в 2 раза сократило число приступов в сочетании с улучшением моторных функций у 31 (36,9%) пациента. Улучшение моторных функций без существенного урежения эпилептических приступов достигнуто у 15 (17,8%) пациентов. Сокращение частоты эпилептических приступов более чем в 2 раза без существенного эффекта на моторные функции отмечено у 14 (16,7%) пациентов. Не ответили на терапию 23 пациента (27,4%). Аггравация эпилептических приступов на фоне курса кортексина отмечена у пациентки с синдромом Веста (1,2%), что значительно ниже вероятности аггравации приступов на терапии ПЭП. Полипептидный ноотропный препарат кортексин продемонстрировал эффективность и безопасность в применении в качестве дополнительной терапии у детей с ДЦП в сочетании с эпилепсией.

This review presents the recently published revised classifications of epilepsies and seizure types developed by the International League Against Epilepsy (ILAE). The Classification of Epilepsies includes several diagnostic levels (steps): 1) from seizure type to epilepsy type (generalized/focal/combined generalized and focal/unknown), 2) diagnosis of epilepsy syndrome 3) etiology (genetic/ structural/ infectious/ metabolic/ immune/unknown). A clinician can use any level of the classification. Operational classification of seizure types is replaced by the previous classification that was grounded on the anatomical basis. Seizures are classified by the onset (focal, generalized or unknown). All types of seizures can be motor or non-motor. Focal seizure may evolve to bilateral tonic-clonic (previously called secondary-generalized). Atonic, clonic, tonic, myoclonic seizures and epileptic spasms can be either of focal or generalized onset. Unclassified type of seizure was introduced. New types of seizures (absence with eyelid myoclonia, myoclonic absence, myoclonic atonic and clonic-tonic-clonic seizures) were added. New terminology, definitions and some concepts developed by ILAE are presented.Представлен обзор опубликованных в 2017 г. новых классификаций эпилепсий и типов эпилептических приступов, которые были разработаны Международной лигой по борьбе с эпилепсией. Новая классификация эпилепсий состоит из нескольких уровней: на первом определяется тип приступа, на втором - тип эпилепсии (фокальная, генерализованная, сочетанная фокально-генерализованная, неклассифицированная), затем следует определение эпилептического синдрома и в завершение - этиология эпилепсии (эпилепсия структурная, генетическая, инфекционная, метаболическая, иммунная, неизвестной этиологии). Клиницист может остановиться на любом уровне классификации эпилепсий. Новая классификация типов эпилептических приступов отличается от предыдущей, которая базировалась на анатомических коррелятах приступа. Все приступы делятся на генерализованные, фокальные, с неизвестным началом. Все они могут проходить с двигательными проявлениями (моторные) и без них. Фокальный приступ может переходить в билатеральный тонико-клонический (прежнее название - вторично-генерализованный). Атонические, клонические, тонические, миоклонические эпилептические приступы и эпилептические спазмы могут быть как фокальными, так и генерализованными. Введен новый термин - 'неклассифицированный' приступ. Добавлены новые генерализованные типы приступов - абсанс с миоклонией век, миоклонический абсанс, миоклонико-атонический приступ и миоклонико-тонико-клонический приступ. Обсуждаются новая терминология, определения и некоторые новые концепции, разработанные Международной лигой по борьбе с эпилепсией.

To assess the efficacy of noophen (γ-amino-β-phenylbutyric acid hydrochloride) in the preventive treatment of tension-type headache (TTH) in children and adolescents.The study included 30 patients with TTH, aged from 8 to 16 years, treated with noophen in dose of 15-20 mg/kg per day (2-3 times perorally) during 2 month. Before and during the treatment characteristics of headache, its influence on daily activities were assessed with HIT-6 and PedMIDAS, anxiety disorders were assessed with SCAS, the dynamics of sleep disturbances in children were evaluated.A significant decrease in the frequency, duration and intensity of TTH as well as positive changes in daily life activities and reduction of anxiety and sleep disorders manifestations, which are the risk factors for TTH, were demonstrated during the treatment with noophen. Starting from the first month of treatment, there was a significant decrease in the number of days completely lost because of headache and days with the reduced activity (a decrease in the productivity by50%).Цель исследования. Оценка эффективности препарата ноофен (γ-амино-β-фенилмасляной кислоты гидрохлорид) в профилактической терапии головной боли напряжения (ГБН) у детей и подростков. Материал и методы. Обследованы 30 пациентов 8-16 лет с ГБН, которые принимали ноофен в течение 2 мес в дозе 15-20 мг/кг в день перорально в 2-3 приема. Контрольную группу составили 30 пациентов того же возраста, которым была назначена дыхательная гимнастика. До и во время лечения оценивали характеристики головной боли, ее влияние на повседневную активность по методикам HIT-6 и PedMIDAS, проявления тревожных расстройств по шкале детской тревоги SCAS, динамику по детской шкале нарушений сна. Результаты и заключение. На фоне терапии препаратом ноофен наряду с существенным уменьшением частоты, продолжительности и интенсивности ГБН установлена положительная динамика в повседневной активности пациентов, а также регресс проявлений тревожности и расстройств сна, которые относятся к факторам риска ГБН. Начиная с 1-го месяца терапии наблюдалось значительное уменьшение количества полностью 'потерянных' из-за головной боли дней и дней со сниженной активностью (снижение продуктивности более чем на 50%).

Neurodevelopmental disorders, including intellectual disability, autistic-spectrum disorders, speech disorders, attention deficit hyperactivity disorder (ADHD), learning disabilities, are more prevalent in children with epilepsy compared with the general population. Marked developmental delay and regression of acquired skills are characteristic of epileptic encephalopathies. Conditions, in which neurodevelopmental disorders are associated with the marked epileptiform EEG activity, while clinical epileptic seizures are absent, represent a serious problem. The authors consider the features of epilepsy with electrical status epilepticus during slow-wave sleep, pseudo-Lennox syndrome, Landau-Kleffner syndrome, children autistic epileptiform regression, autosomal-dominant rolandic epilepsy with verbal dispraxy and a combination of epilepsy and subclinical epileptiform EEG activity with developmental dysphasia and ADHD. In addition to the optimization of basic treatment with antiepileptic drugs (AEDs), nootropic drugs which do not increase epileptiform activity (hopantenic acid), are recommended.У детей с эпилепсией значительно чаще, чем в общей детской популяции, встречаются нарушения нервно-психического развития: общая интеллектуальная недостаточность, расстройства аутистического спектра, нарушения развития речи, синдром дефицита внимания и гиперактивности (СДВГ), расстройства обучения. Выраженное отставание в развитии, а также регресс ранее приобретенных навыков характерны для эпилептических энцефалопатий. Серьезную проблему представляют состояния, при которых нарушения нервно-психического развития ассоциированы с выраженной эпилептиформной активностью на ЭЭГ, а клинические эпилептические приступы отсутствуют. Рассматриваются особенности проявлений таких состояний, как эпилепсия с электрическим эпилептическим статусом медленноволнового сна, синдром псевдо-Леннокса, синдром Ландау-Клеффнера, детский аутистический эпилептиформный регресс, аутосомно-доминантная роландическая эпилепсия с речевой диспраксией, а также сочетание с эпилепсией и субклинической эпилептиформной активностью на ЭЭГ дисфазии развития и СДВГ. Помимо оптимизации основной терапии антиэпилептическими препаратами (АЭП) в случаях сочетания эпилепсии с нарушениями развития рекомендуются ноотропные препараты, которые не вызывают нарастания эпилептиформной активности (гопантеновая кислота).

To analyze the images obtained during pre-surgical neuroimaging in patients with gangliomas for the presence of specific signs and verification of the neoplastic process.The results of presurgical MRI (3.0, 1.5 Tesla) of 20 patients with gangliomas were analyzed to identify specific signs of a neuronal-glial tumor and verify the neoplastic process based on the results obtained and a review of the literature. In addition to high-resolution MRI (HR MRI), various protocol modifications were applied to patients with epileptogenic pathological substrates of unclear etiology, including tractography (DTI) and contrast-free MR perfusion (ASL). In 5 cases, a multi-modal study was performed that combined the results of CT, routine MRI, HR MRI, functional MRI (fMRI) in various combinations and PET CT.In 17 cases, patients underwent epileptic surgery. Three patients without epilepsy were operated on for a tumor diagnosed by radiological examination. In all 20 cases, gangliogliomas were verified, including 1 anaplastic, 1 infantile desmoplastic, and another patient had histological samples showing signs of a composite tumor. Combination with FCD IIIb was observed in 3 cases. Two patients had a double pathology (cases of tumors combination with lissencephaly and neuronal heterotopia) and another had a composite neuronal-glial tumor. In 15 cases, gangliogliomas showed neuroradiological features typical for dysembryoplastic neuroepithelial tumor (DNT) such as multicystic, nodular, and diffuse (dysplastic) described in the literature. In addition, in 9 cases, they had significant signs of neoplastic process such as contrast enhancement, continued growth and remodeling of the underlying bone. Verification of the neoplastic process based on the results of neuroradiological studies was difficult in 6 cases. In 2 cases, it was not possible to confirm the presence of neoplasm by radiological methods, and in 1 patient, the verification of the tumor during differential diagnosis took more than 8 years. The most common differential diagnosis was performed with DNT and FCD type IIb, which have a number of similar neuroradiological features.Анализ изображений, полученных при прехирургической нейровизуализации у пациентов с ганглиоглиомами, на предмет наличия специфических признаков и верификации неопластического процесса.Проанализированы результаты прехирургических МРТ 20 пациентов с ганглиоглиомами на томографах 3,0, 1,5 тесла (Т) для выявления специфических признаков нейронально-глиальной опухоли и верификации неопластического процесса, основываясь на полученных результатах и обзоре литературы. Пациентам с эпилептогенными патологическими субстратами неясной этиологии дополнительно к МРТ с высоким разрешением (МРТ ВР) применялись различные модификации протокола с включением трактографии (DTI), бесконтрастной МР — перфузии (ASL). В 5 случаях проводилось мультимодальное исследование, совмещавшее в различных комбинациях результаты КТ, рутинной МРТ, МРТ ВР, функциональной МРТ (фМРТ) и позитронно-эмиссионной КТ.В 17 случаях пациенты были подвергнуты хирургическому лечению по поводу эпилепсии. Три пациента без эпилепсии были прооперированы по поводу опухоли, диагностированной с помощью радиологического обследования. Во всех 20 случаях были верифицированы ганглиоглиомы, из них 1 анапластическая, 1 инфантильная десмопластическая, еще у 1 пациента гистологические образцы имели признаки композитной опухоли. Сочетание с фокальной кортикальной дисплазией IIIb отмечено в 3 случаях. У 2 пациентов отмечалась двойная патология (случаи сочетания опухолей с лиссэнцефалией и нейрональной гетеротопией) и еще у 1 — композитная нейронально-глиальная опухоль. В 15 случаях ганглиоглиомы демонстрировали характерные для дизэмбриопластических нейроэпителиальных опухолей нейрорадиологические признаки: мультикистозный, узловой и диффузный (диспластический), описанные в литературе. Помимо этого, в 9 случаях они имели достоверные признаки неопластического процесса, такие как контрастное усиление, продолженный рост и ремоделирование подлежащей кости. Верификация неопластического процесса по результатам нейрорадиологических исследований была затруднена в 6 случаях. В 2 случаях не удалось подтвердить наличие опухоли лучевыми методами исследования, еще у 1 пациента верификация опухоли при проведении дифференциального диагноза заняла более 8 лет. Наиболее часто дифференциальный диагноз проводился с дизэмбриопластическими нейроэпителиальными опухолями и фокальной кортикальной дисплазией IIb типа.

To evaluate the efficacy and safety of tenoten for children in the treatment of specific developmental disorders of academic skills in children of 1-3 grades.Two hundred and forty children, aged 7-9 years, (Total set, Safety population) with verified specific reading disorder (F81.0), specific spelling disorder (F81.1), specific disorder of arithmetical skills (F81.2), mixed disorder of scholastic skills (F81.3; F81.2+F81.0, or F81.2+F81.1, or F81.2+F81.0+F81.1), diagnosed with the use of logopedic or psychological testing (15-35 scores in Fotekova T.A. and Akhutina T.V. reading and writing tests; 5-15 scores in arithmetical subscale of the Wechsler Intelligence Scale for Children) were enrolled in the study. CT was conducted in 10 clinical centers in Russian Federation in 2015- 2019. Patients were randomized into two groups. The first one (The mean total academic skills score increased by 18.55±15.87 points. The significant total difference between the median changes in the total score in the tenoten and placebo groups was 5 points. There was a trend towards positive changes in reading and spelling mean scores in tests that didn't reach statistical significance due to lack of normal distribution of points in samples. There were 73 adverse events (AEs) in 42 patients of the tenoten group and 95 AEs in 31 children of the placebo group. No serious or severe AEs were registered in the tenoten group. No AEs definitely related to the study treatment were registered. No negative drug interactions were observed in the tenoten group.Tenoten for children is an effective and safe treatment for specific developmental disorders of academic skills in primary school children. Tenoten for children is well tolerated. The treatment is characterized by a high level of adherence of children and their parents to therapy.Изучение эффективности и безопасности Тенотена детского в терапии специфических расстройств развития учебных навыков у учащихся 1—3-го классов.В многоцентровое двойное слепое рандомизированное плацебо-контролируемое клиническое исследование были включены 240 детей в возрасте 7—9 лет с диагностированными расстройствами чтения (F81.0), спеллингования (F81.1), арифметических навыков (F81.2), смешанными расстройствами учебных навыков (F81.3; расстройство, одновременно отвечающее критериям подрубрик F81.2+F81.0, либо F81.2+F81.1, либо F81.2+F81.0+F81.1), подтвержденными результатами логопедического и психолого-педагогического тестирования (15—35 баллов при оценке чтения и 15—30 баллов при оценке письма по методу Т.А. Фотековой, Т.В. Ахутиной; 5—15 баллов по результатам субтеста №3 «Арифметический» теста Векслера). Клиническое исследование проведено в 10 клинических центрах в РФ в 2015—2019 гг. Пациенты были рандомизированы в 2 группы. Первая группа (В результате лечения Тенотеном детским суммарный балл сформированности учебных навыков увеличился на 18,55±15,87 балла. Статистически значимая итоговая разница между медианными изменениями суммарного балла в группах Тенотена детского и плацебо составила 5 баллов. Отмечена тенденция к положительному влиянию терапии на средние баллы чтения и письма по результатам тестов, однако отсутствие нормального распределения баллов в выборках не позволило получить статистически значимые различия между группами. В ходе исследования было зарегистрировано 73 нежелательных явления (НЯ) у 42 пациентов группы Тенотена детского и 95 НЯ у 31 пациента группы плацебо. В группе Тенотена детского не выявлены НЯ тяжелой степени и серьезные НЯ. Не выявлено ни одного НЯ, достоверно связанного с применением исследуемого препарата, и не зарегистрировано случаев взаимодействия Тенотена детского с лекарственными средствами различных классов.Применение препарата Тенотен детский в лечении специфических расстройств развития учебных навыков у детей младшего школьного возраста является эффективным и безопасным, хорошо переносится и сочетается с высоким уровнем приверженности терапии детей и их родителей.

The author considers causes, clinical variants and diagnostic principles of speech development in children. A role of neurobiological factors in the genesis of speech disorders and developmental dysphasia including early organic CNS lesions due to perinatal pathology and hereditary predisposition is described. Outcomes of dysphasia in children reached school-age are discussed. A screening method of the identification of speech delay in early childhood is presented. Early diagnosis and treatment of speech disorders in children are of great importance. Timely treatment with nootropics facilitates the correction of speech disorders.В статье рассматриваются причины, клинические варианты, принципы диагностики развития речи у детей. Представлены данные о роли нейробиологических факторов в генезе расстройств развития речи и дисфазий развития (алалия), в том числе раннего органического повреждения ЦНС в связи с перинатальной патологией и наследственной предрасположенностью. Обсуждаются исходы дисфазии развития к школьному возрасту. Представлен скрининговый метод выявления отставания в развитии речи у детей раннего возраста. Важное значение имеют ранние диагностика и терапия нарушений развития речи у детей. To study clinical characteristics mechanisms and pathogenesis of the tension-type headache (TTH) in children and adolescents treated with nooclerin.The blind randomized placebo-controlled study of the efficacy of nooclerin (deanol aceglumate) indicated during 2 months as a preventive treatment of TTH was performed. Forty patients (15 male and 25 female), aged 9-16 years, were randomized into two equal groups to receive either nooclerin or placebo.Nooclerin significantly reduced the frequency, duration and intensity of TTH. The treatment resulted in positive dynamics in daily activities, regression of fatigue symptoms and normalization of the sleep-wake cycle.Цель исследования. Оценка эффективности препарата нооклерин в профилактической терапии головной боли напряжения (ГБН) у детей и подростков. Материал и методы. Проведено слепое рандомизированное плацебо-контролируемое исследование эффективности нооклерина (деанола ацеглумат), назначавшегося в течение 2 мес с целью профилактической терапии ГБН. Лечили 40 пациентов, 15 мужского и 25 женского пола в возрасте 9-16 лет, распределенных на две группы по 20 человек (препарат и плацебо). Результаты и заключение. Применение нооклерина привело к существенному уменьшению частоты, продолжительности и интенсивности ГБН, положительной динамике в повседневной активности пациентов, регрессу проявлений астении и нормализации цикла сон-бодрствование.

To assess the efficacy and safety of children tenoten in the treatment of children and adolescents with anxiety disorders.It was conducted a multicenter, double-blind, placebo-controlled trial of the drug tenoten children at a dose of 1 tablet 3 times a day for 12 weeks. The study included 98 patients (boys and girls from 5 to 15 years with a confirmed diagnosis of anxiety disorder), randomized into two groups: the first included 48 patients treated tenotenom children, in the second - 50 patients receiving placebo.Tenoten children has a strong anti-anxiety effect both on the results of self-assessment of patients, and on the reports of parents. This anxiolytic activity of the drug manifested most significantly in children aged 5 to 7 years. In addition, in patients 8-15 years of treatment spent tenotenom children to regress the symptoms of anxiety disorders by anxiety subscales SCAS «Separation anxiety», «panic attacks and agoraphobia» and «social phobia». Throughout the course of treatment tenoten children have been no adverse events.Цель исследования - оценка эффективности и безопасности применения тенотена детского у детей и подростков с тревожными расстройствами. Материал и методы. Было проведено многоцентровое двойное слепое плацебо-контролируемое исследование препарата тенотен детский в дозе по 1 таблетке 3 раза в день в течение 12 нед. В исследование были включены 98 пациентов (девочки и мальчики от 5 до 15 лет с подтвержденным диагнозом тревожного расстройства), рандомизированные в две группы: в 1-ю вошли 48 больных, лечившихся тенотеном детским, во 2-ю - 50 пациентов, получавшие плацебо. Результаты и заключение. Тенотен детский обладает выраженным противотревожным эффектом как по результатам самооценки пациентов, так и по отчетам родителей. При этом анксиолитическая активность препарата наиболее значимо проявилась у детей в возрасте от 5 до 7 лет. Кроме того, у пациентов 8-15 лет лечение тенотеном детским приводило к регрессу симптомов тревожных расстройств по подшкалам тревожности SCAS 'Сепарационная тревожность', 'Панические атаки и агорафобия' и 'Социальная фобия'. На протяжении всего курса лечения тенотеном детским не было зафиксировано нежелательных явлений.

To evaluate the efficacy and safety of hopantenic acid (Pantogam) in the complex treatment of prematurely born infants, aged 6-12 months, with psychomotor developmental delay due to hypoxic-ischemic encephalopathy.Eighty-seven patients were randomized into two groups: 44 received standardized treatment and pantogam for two months, 43 standardized treatment and placebo. Pantogam (syrup 100 mg/ml) or placebo were prescribed orally 15-30 minutes after feeding, twice a day, in a daily dosage of 30-50 mg/kg body weight. The assessment of psychomotor development from birth to two years was performed with the Griffiths Mental Development Scales (GMDS-ER) twice (before and after completion of therapy).The response to two month therapy determined as the reduction of developmental delay for more than 6% of the initial GMDS-ER general quotient (GQ) score was significantly better in the group I after pantogam treatment (63.6% of patients) compared to group II (36.4%, p=0.021). Group I demonstrated the significant decrease of the developmental delay in two domains ('Personal-Social' and 'Performance') and a trend to overcome the delay in three other domains: 'Locomotor', 'Hearing and Speech', 'Eye and Hand Coordination'. The improvement after pantogam treatment was more obvious in the subgroup of infants born late preterm (gestational age 34-36 weeks) compared to infants born moderate preterm (gestational age 32-33 weeks). The favorable safety profile of pantogam was confirmed, comparable to that of placebo.Pantogam is efficient and safe medication in the complex treatment of psychomotor developmental delay in preterm infants, aged 6-12 months.Цель исследования. Изучение эффективности и безопасности лекарственного препарата (ЛП) гопантеновой кислоты Пантогам в комплексном лечении рожденных недоношенными детей в возрасте 6-12 мес с задержкой психомоторного развития вследствие перенесенного гипоксически-ишемического поражения центральной нервной системы. Материал и методы. Обследовали 87 пациентов, рандомизированных на две группы. Пациентам 1-й группы (44 ребенка) в течение 2 мес назначена стандартная терапия и дополнительно ЛП Пантогам, пациентам 2-й группы (43 ребенка) - стандартная терапия плюс плацебо. Пантогам (сироп 100 мг/мл) или плацебо применяли внутрь через 15-30 мин после кормления 2 раза в день в суточной дозе 30-50 мг на 1 кг массы тела. Продолжительность терапии составила 67 дней. Всех пациентов дважды (до и по окончании курса лечения) обследовали с помощью шкал Гриффитс для оценки психомоторного развития детей от рождения до 2 лет (использовали дополненную и пересмотренную версию - GMDS-ER). Результаты. В 1-й группе после лечения отмечено улучшение психомоторного развития более чем на 6% (от исходной величины общего показателя по шкалам Гриффитс) у 63,6% пациентов, что значимо превысило аналогичный показатель во 2-й группе - 39,5% (p=0,021). В 1-й группе наблюдали значимое сокращение отставания в развитии по сферам 'Личностно-социальная' и 'Манипуляции с предметами', а также тенденцию к уменьшению отставания по трем остальным сферам: 'Двигательная активность', 'Слух и речь', 'Зрительно-моторная координация'. Более выраженная положительная динамика после лечения ЛП Пантогам определена в подгруппе пациентов с поздней недоношенностью (34-36 нед) по сравнению с детьми с умеренной недоношенностью (32-33 нед). Подтвержден благоприятный профиль безопасности ЛП Пантогам, сопоставимый с плацебо. Заключение. Пантогам является эффективным и безопасным средством в комплексном лечении задержки психомоторного развития у детей первого года жизни.

To study the efficacy of recognan (citicoline) in the treatment of cognitive, emotional, autonomic, asthenic disorders in patients with asthenic syndrome.Thirty-eight subjects (17 men and 41 women), mean age 27.75±12.05 years, with asthenic syndrome (ICD-10 F48.0 - Neurasthenia) were studied. The sample was randomized into 2 subgroups: the main group (n=20) received oral therapy with recognan (100 mg in 1 ml) for 30 days, while the daily dosage was 500 mg (5 ml). In the control group (n=18), no drug therapy was performed. Patient's state was assessed with a large battery of psychological tests and psychometric scales. The follow-up period was 30 days. All participants were examined three times (initially, in the middle of the study (day 15), in the end of the study (day 30)).Asthenic syndrome was detected in 100% of the patients (total asthenia was noted in 70%, decreased activity in 70%, decreased motivation in 40%, physical asthenia in 45%, mental asthenia in 50%). Recognan (citicoline), used for 2 weeks or one month, has a positive effect on the compensation of autonomic, asthenic cognitive and emotional disorders and increases stress resilience of the patients.Цель исследования. Изучение эффективности лекарственного препарата (ЛП) Рекогнан (Цитиколин) в коррекции астенических, когнитивных, эмоциональных и вегетативных нарушений у пациентов с астеническим синдромом. Материал и методы. Обследовали 38 больных неврастенией (по МКБ-10 F48.0), 17 мужчин и 21 женщину в возрасте 18-45 лет с астеническим синдромом. Больные были разделены на две группы. В основной группе (20 человек) проводили терапию ЛП Рекогнан (в растворе, 100 мг в 1 мл) в течение 30 дней, при этом суточная дозировка препарата составляла 500 мг (5 мл раствора). В контрольной подгруппе (18 человек) медикаментозную терапию не проводили. Психическое состояние больных оценивали с применением ряда шкал и тестов. Период наблюдения составил 30 дней. Все больные (как в основной, так и в контрольной группе) были обследованы трижды (исходно, в середине исследования - на 15-й день, в конце исследования - на 30-й день). Результаты и заключение. До лечения астенический синдром был выявлен у 100% обследованных (при этом общая астения отмечена у 70%, пониженная активность - у 70%, снижение мотивации - у 40%, физическая астения - у 45%, психическая астения - у 50%). Применение ЛП Рекогнан как коротким (2-недельным), так и месячным курсом способствовало коррекции астенических, когнитивных, эмоциональных, вегетативных нарушений и повышению стрессоустойчивости пациентов.

To assess the incidence of emotional and behavioral disorders in children and adolescents with frequent episodic or chronic tension type headaches (TTH).One hundred and fifty patients with TTH (75 boys and 75 girls), aged 8-16 years, were included in the study. The severity of emotional and behavioral problems was analyzed in comparison with their healthy peers by means of parents' interviewing with the 'Strengths and Difficulties Questionnaire' (SDQ).Total difficulties scores measured by SDQ were significantly higher in boys (16.2±0.7) and girls (14.3±0.7) with TTH compared with their peers (7.9±0.4 and 7.7±0.4, respectively, p0.001). Patients with TTH had significantly more prominent manifestations than their peers (p0.001) on the four SDQ scales, including Hyperactivity and Inattention, Conduct problems, Emotional symptoms, Peer problems. Clinical examination revealed in many pediatric patients with TTH the following disorders: attention deficit hyperactivity disorder (45.3% boys and 13.3% girls), oppositional defiant disorder (26.7% boys, 18.7% girls), with co-occurrence of both disorders in some patients (17.3% boys, 10.7% girls). Moreover, most patients with TTH had anxiety disorders (68.0% boys, 77.3% girls).Clinical features and duration of TTH may be dependent on the severity of co-morbid emotional and behavioral disorders. This should be taken into account for individualized indication of drug therapy and non-pharmacological treatment approaches in pediatric TTH.Цель исследования. Изучить частоту эмоциональных нарушений и расстройств поведения у детей и подростков с частыми эпизодическими и хроническими головными болями напряжения (ГБН). Материал и методы. Обследовали 150 пациентов (75 мальчиков и 75 девочек) в возрасте 8-16 лет с ГБН. Оценка выраженности эмоциональных и поведенческих проблем в сравнении со здоровыми ровесниками (контроль) была проведена с помощью анкетирования родителей пациентов по опроснику 'Сильные стороны и трудности' (ССТ) с оценкой в баллах. Результаты. Показатели ССТ оказались более высокими у мальчиков (16,2±0,7 балла) и девочек (14,3±0,7 балла) с ГБН, чем у их сверстников (7,9±0,4 и 7,7±0,4 балла соответственно, р0,001). У пациентов с ГБН были значительно более выраженными по сравнению с контролем (р0,001) проявления по таким шкалам опросника, как гиперактивность и невнимательность, проблемы поведения, эмоциональные нарушения, трудности отношений со сверстниками. У многих пациентов с ГБН был выявлен синдром дефицита внимания с гиперактивностью (45,3% мальчиков, 13,3% девочек), оппозиционно-вызывающее расстройство поведения (26,7% мальчиков, 18,7% девочек), в некоторых случаях (17,3% мальчиков, 10,7% девочек) указанные синдромы сочетались. Кроме того, у большинства пациентов (68,0% мальчиков, 77,3% девочек) имелись тревожные расстройства. Заключение. Особенности клинической картины и течения ГБН могут зависеть от степени выраженности коморбидных эмоциональных и поведенческих нарушений, что необходимо учитывать при индивидуальном выборе методов в нейропедиатрической практике.

The objective of the study was to compare the efficacy and tolerability of once-daily atomoxetine (or =1.8 mg/(kg day) with those of placebo in children and adolescents (aged 6-16 years) with attention-deficit/hyperactivity disorder [ADHD (DSM-IV)]. This randomized, placebo-controlled, double-blind trial was conducted in Russia. The primary efficacy measure was baseline-to-end point changes in Attention-Deficit/Hyperactivity Disorder Rating Scale-IV-Parent Version: Investigator-Administered and Scored (ADHDRS-IV-Parent:Inv) total score. Tolerability measures included treatment-emergent signs and symptoms (TESS), laboratory values and weight. Compared with patients in the placebo group (n = 33), patients treated with atomoxetine (n = 72) with a mean final dose of 1.4 mg/kg showed significantly greater improvement in ADHDRS-IV-Parent:Inv total score (least-squares mean: atomoxetine, -15.8; placebo, -11.4; p = 0.013). The most common TESS in the atomoxetine group included anorexia [atomoxetine, n = 13 (18.1%); placebo, n = 2 (6.1%)], somnolence, n = 11 versus n = 3 (15.3% vs. 9.1%, respectively), abdominal pain n = 9 versus n = 1 (12.5% vs. 3.0%, respectively) and nausea, n = 8 versus n = 1 (11.1% vs. 3.0%, respectively). Seven patients in the atomoxetine group and two in the placebo group experienced clinically important weight loss during the study (or =7% from baseline; mean change, kg: atomoxetine, -0.6; placebo, 0.1; p = 0.032). Atomoxetine is efficacious in improving ADHD symptoms in children and adolescents. Atomoxetine treatment may be associated with a numerically higher incidence of anorexia, somnolence, abdominal pain and nausea, as well as statistically greater losses in body weight.

Though the mechanisms of action of intravenous immunoglobulins (IVIG) are not completely understood, these drugs are widely used in treatment of autoimmune diseases. In this review, we have analyzed the literature on the use of IVIG in the treatment of autoimmune diseases of the nervous system in children and discuss the management of patients basing on the recommendation of the European Federation of Neurological Societies. The efficacy of IVIG in children has been shown as first line treatment in Guillain-Barre syndrome, chronic inflammatory demyelinating polyneuropathy, multifocal motor neuropathy, dermatomyositis as a second-line drug in the combination with prednisolone or immunosuppressors in patients refractory to treatment with corticosteroids and cytostatics, myasthenic crisis in myasthenia gravis, exacerbations and short-term treatment of severe forms, non-responsiveness to acetylcholinesterase inhibitors, multiple sclerosis as second or third line of treatment in patients with relapsing-remitting course with intolerance to standard immunomodulatory therapy, acute multiple encephalomyelitis with no response to the treatment with high doses of corticosteroids, paraneoplastic syndromes, pharmacoresistant epilepsy and autoimmune encephalitis. Because the right choice of the drug plays a key role, in particular, in children, that determines the efficacy and safety of the treatment, we present the main approaches to the choice of the drug and schemes of treatment of autoimmune diseases of the nervous system in children.Несмотря на то что до сих пор механизмы действия внутривенных иммуноглобулинов (ВВИГ) не раскрыты в полной мере, препараты данной группы нашли широкое применение в лечении аутоиммунных заболеваний. В данной обзорной статье приведен анализ имеющихся в медицинской литературе данных о применении ВВИГ в терапии аутоиммунных заболеваний нервной системы детей и обсуждается тактика ведения данных пациентов с учетом рекомендаций Европейской Федерации неврологических сообществ. Эффективность применения ВВИГ у детей была показана в качестве терапии первой линии при синдроме Гийена-Барре, хронической воспалительной демиелинизирующей полинейропатии, мультифокальной моторной нейропатии, а также при дерматомиозите в качестве препаратов второй линии в комбинации с преднизолоном или иммуносупрессантами у пациентов, резистентных к терапии кортикостероидами и цитостатиками; при миастении гравис для лечения миастенического криза, обострений и краткосрочной терапии тяжелых форм, при отсутствии ответа на ингибиторы ацетилхолинэстеразы; при рассеянном склерозе в качестве второй или третьей линии терапии у пациентов с рецидивирующе-ремиттирующим течением при непереносимости общепринятой иммуномодулирующей терапии; остром рассеянном энцефаломиелите при отсутствии ответа на лечение высокими дозами кортикостероидов; оптикомиелите (болезни) Девика при недостаточном ответе на кортикостероиды; при паранеопластических синдромах; при фармакорезистентной эпилепсии и аутоиммунных энцефалитах. Учитывая, что правильный выбор препарата, особенно в педиатрической практике, зачастую имеет ключевое значение, так как во многом определяет эффективность и безопасность проводимой терапии, в статье представлены основные подходы к выбору препарата и схемы терапии аутоиммунных заболеваний нервной системы у детей.

Chronic tics (CT) are observed in 3-4% population, Tourette syndrome (TS) in 0.1-3.0%. In most cases, tic disorders start at age 2-15 years, but frequently they are diagnosed late. Clinical presentations of tics and comorbid disorders are various and depend on child's age. Difficulties in treatment of CT are associated with their persistence and those for TS with the fluctuating course of tics and probable onset of the disease in the form of behavioral disorders. Treatment of CT and TS is individual. Methods of behavioral and psychotherapy are recommended for a certain period of time if tics do not hamper everyday life. Increase in tick frequency and severity indicate the necessity of using pharmacological treatment. The European recommendations on pharmacotherapy of tic disorders and the drugs available in Russia are considered. Results of the studies on the efficacy of antiepileptic drugs and tenoten children in CT and TS are presented.Хронические тики (ХТ) встречаются у 3-4% населения, синдром Туретта (СТ) - у 0,1-3,0%. Дебют тикозных расстройств в большинстве случаев приходится на возраст от 2 до 15 лет. но они часто диагностируются поздно. Клинические проявления тиков и сопутствующих им расстройств разнообразны и зависят от возраста ребенка. Трудности в лечении ХТ связаны с их персистированием, СТ - с флюктуирующим течением тиков, а также возможным дебютом заболевания поведенческих нарушений. Лечение ХТ и СТ носит индивидуальный характер: если тики не мешают пациентам в их повседневной жизни, то на определенный период могут рекомендоваться методы поведенческой и психотерапии; усиление и учащение тиков служат показаниями для назначения фармакотерапевтических средств. Рассматриваются Европейские рекомендации по фармакотерапии тикозных расстройств и доступные в России препараты. Приводятся данные исследований эффективности при ХТ и СТ противоэпилептических препаратов, а также препарата тенотен детский.

Opsoclonus-myoclonus syndrome (OMS) is a rare neurological disorder with unknown etiology and pathogenesis. There is not enough knowledge about psychological development of children with OMS.To study an influence of the time of disease onset and children's age and type of disease course on psychological development of children with OMS.The study included 26 children with OMS of age between 1 year 7 months and 13 years, 10 boys and 16 girls. The following methods were used: analysis of patient's development records, psychological interview with parents, assessment of emotional, behavioral, motion state and interpersonal relations.The disease onset at the early stage of ontogenesis (before 3 years) results into severe psychological disorders. Patients at the age between 3 and 5 years demonstrate the maximal number of developmental delays with possibilities of compensation. The article discusses a possible role of psychotraumatic and psychosocial factors in psychological disorders in children with OMS. The influence of age should be taken into account in planning of treatment and rehabilitation and evaluation of disease prognosis.Опсоклонус-миоклонус-синдром (ОМС), возникающий чаще всего в детском возрасте, является редким и малоизученным в отношении этиологии и патогенеза заболеванием. Актуальность исследования - недостаточность данных о психическом развитии детей при ОМС. Цель исследования. Анализ особенностей психического развития детей с ОМС при разном возрасте начала и разных типах течения заболевания. Материал и методы. Обследовали 26 детей с ОМС (10 мальчиков, 16 девочек) в возрасте от 1 года 7 мес до 13 лет. В процессе клинико-неврологического и психологического обследования анализировали данные историй болезни, проводили беседы с родителями, оценивали эмоциональное состояние детей, поведенческие двигательные проявления болезни, когнитивное развитие больных и особенности их межличностных отношений. Результаты и заключение. Установлено, что при начале заболевания в наиболее ранний этап онтогенеза (до 3 лет) отмечаются более выраженные нарушения психического развития. Выделен возраст пациентов (от 3 до 5 лет), в котором наблюдается максимальное количество задержек психического развития с возможностями компенсации и развитием, близким к нормативному. Обсуждаются вопросы возможной роли психотравматического и психосоциальных факторов в развитии нарушений психического развития у детей с ОМС с учетом возраста. Подчеркивается значение возрастного фактора для планирования лечебно-реабилитационных мероприятий и оценки прогноза заболевания.

To investigate the dynamics of behavioral indicators, attention and memory in children with attention deficit hyperactivity disorder (ADHD) treated with noofen (capsules 250 mg).In an open study, 50 patients were randomized to 2 equal groups: patients of group 1 were treated with noofen (15-20 mg/kg (500-700 mg) per day perorally in 2-3 doses); the control group received low doses of multivitamins. Duration of treatment was one month.The results of neuropsychological testing revealed the improvement of cognitive functions, including the indicators of self-control, sustained, directed and divided attention, acoustic-verbal memory, to the end of treatment.The initial positive changes may be the basis for obtaining better clinical results during long-term treatment.

Developmental dysphasia (alalia) represents a severe speech and language disorder in children. To assess the efficacy of treatment with cerebrolysin, we have examined 60 children with developmental dysphasia, aged from 3 to 4 years. Group 1 (30 patients) received cerebrolysin (monotherapy, daily dosage 0.1 ml/kg, in the morning hours, on each other day, i.m., 30 injections in total) during 2 months. Group 2 (controls, 30 patients) did not receive pharmacotherapy. A significant improvement of expressive, impressive speech and speech attention was observed in group 1 after the treatment. The active vocabulary increased by a factor of 3.5 and phrases number in colloquial speech by a factor of 5, versus 1.5 in the control group. According to parents' questionnaires, the treatment resulted in the decrease of psychasthenic, psychosomatic problems, motor clumsiness and hyperactivity along with the improvement of attention, emotional control and behavioral measures. The positive effect of cerebrolysin on the motor development was demonstrated by the assessment of motor milestones.

To analyze MR-images of patients with focal epilepsy using a method close to the protocol of epileptic scanning on a MRI-device with the magnetic field tension of 0.4 Tesla.MRI data of 50 children who underwent examination due to difficult-to-treat or drug-resistant forms of focal epilepsy were analyzed. MRI study was conducted using open-ended device of static magnetic field HITACHI 'APERTO' with the magnetic field tension of 0.4 Tesla. The thickness of the slices and the scan step was performed at 3.0 and 3.5 mm with the use of special positioning of slices in the coronal and axial projections, T2, T1, STIR, FLAIR weighted images (WI) perpendicular and parallel to the long axis of the hippocampus.Potentially epileptogenic structural changes were identified in 37 patients. Abnormalities of brain development of different severity were identified in 16 patients. In 21 cases, the changes were due to the consequences of cerebral vascular catastrophes, neuroinfections, brain traumas affecting the cortical plate. A method close to the epileptic scanning protocol allowed the identification of signs of acute cerebral catastrophes in two patients. In 8 patients, potentially epileptogenic changes after false-negative descriptions of the results of previous MRI studies were newly identified. The most difficult for the diagnosis was the visualization of small structural changes in mediobasal regions of the temporal lobes. Confirmation of this supposition according to the recommended thorough investigation was obtained only in 5 out of 12 patients with suspected small-bore pathological changes in mediobasal temporal regions.Low-tension technique approximated to the epileptic scanning protocol does not allow the reliable diagnosis of small and similar in signal characteristics changes in the visualization of mediobasal temporal lobes regions. It was not possible to reliably differentiate some cortical-subcortical neoplastic formations from various forms of focal cortical dysplasia.Цель исследования. Анализ результатов магнитно-резонансной томографии (МРТ) пациентов с фокальной эпилепсией с применением методики, приближенной к протоколу эпилептического сканирования (ПЭС), на аппарате с напряженностью магнитного поля 0,4 Тл. Материал и методы. Проанализированы данные МРТ 50 детей, проходивших обследование по поводу трудно поддающихся лечению или фармакорезистентных форм фокальной эпилепсии. Для выявления эпилептогенного поражения исследование проводили на томографе Aperto ('Hitachi'), напряженностью магнитного поля 0,4 Тл с толщиной срезов и шага сканирования 3,0 и 3,5 мм с применением специального позиционирования срезов в коронарной и аксиальной проекциях в Т2, Т1, STIR, FLAIR, перпендикулярно и параллельно длинной оси гиппокампа. Результаты. Потенциально эпилептогенные структурные изменения были выявлены у 37 пациентов, у 16 - обнаружены аномалии развития головного мозга различной степени выраженности. Еще в 21 случае изменения были представлены последствиями перенесенных нарушений мозгового кровообращения, нейроинфекций, травм с поражением корковой пластинки. При применении методики, приближенной к ПЭС, у 2 пациентов были выявлены признаки острого поражения ЦНС. У 8 обследованных были впервые выявлены потенциально эпилептогенные изменения после ложнонегативных описаний результатов предыдущих МРТ. Наибольшие трудности для диагностики представляли случаи визуализации мелких структурных изменений медиобазальных отделов височных долей. Из 12 пациентов с подозрением на наличие мелких изменений медиобазальных отделов височных долей только у 5 было получено подтверждение предположения по данным рекомендованного углубленного обследования. Обследование на низкопольном томографе по приближенному к ПЭС протоколу при визуализации медиобазальных отделов височных долей не позволяет достоверно диагностировать мелкие, сходные по сигнальным характеристикам изменения. Также не удалось достоверно отличить некоторые корково-субкорковые неопластические образования от различных форм фокальных корковых дисплазий.

To assess the efficacy and safety of hopantenic acid (pantogam) compared to placebo in the treatment of attention deficit hyperactivity disorder (ADHD) in children, aged from 6 to 12 years, during 4 month in the prospective multicenter comparative double-blind placebo-controlled study in parallel groups.One hundred patients enrolled in the safety assessment population were stratified into two equal pantogam and placebo groups. Eighty-nine patients who completed the study in according to the protocol were included in the efficacy assessment group: 45 in the pantogam group and 44 in the placebo group. Pantogam was administered in tablets (250 mg) in the therapeutic dose 30 mg/kg of body mass, divided into 2 doses, during 4 month. Patient's state was assessed by the total score on ADHD-DSM-IV, CGI-S WFIRS-P and results of the Toulouse-Piéron test for sustained attention.There was a trend towards an increase in the percentage of patients with positive changes (a decrease in the total ADHD-DSM-IV by ≥25%) in the end of the 3rd and 4th month in the pantogam group (treatment response was 66.7 and 68.9%, respectively) compared to the placebo group (treatment response was 52.3 and 61.4%, respectively). A significant decrease in disease severity assessed by the CGI-S was noted in the pantogam group compared to the placebo group. After 4 month of treatment with pantogam, the severity of functional disturbances was reduced by 4 out of 6 WFIRS-P domains: Family, School and learning, Child's self-concept and Risky activities. Pantogam improved the measures of sustained attention (accuracy and speed) in the Toulouse-Piéron test. The drug used in mean daily dose 30 mg/kg during 4 month had a favorable safety profile which did not differ from that of placebo.Цель исследования. Оценка эффективности и безопасности гопантеновой кислоты (пантогам) по сравнению с плацебо в терапии СДВГ продолжительностью 4 мес у детей от 6 до 12 лет в ходе проспективного многоцентрового сравнительного двойного слепого плацебо-контролируемого исследования в параллельных группах. Материал и методы. В исследование включены 100 пациентов, составивших популяцию оценки безопасности (50 в группе лечившихся пантогамом и 50 в группе с применением плацебо). 89 пациентов, завершивших исследование в соответствии с протоколом, вошли в популяцию оценки эффективности: 45 в группе пантогама (1-я группа) и 44 в группе плацебо (2-я группа). Пантогам назначали в таблетках по 250 мг в терапевтической дозе 30 мг/кг массы тела ребенка, разделенной на два приема, в течение 4 мес. При оценке состояния пациентов в динамике учитывались суммы баллов по шкалам СДВГ-DSM-IV и общего клинического впечатления CGI-S, функциональных нарушений WFIRS-P, результаты выполнения корректурной пробы (тест Тулуз-Пьерона). Результаты и заключение. Эффективность пантогама при СДВГ у детей 6-12 лет в сравнении с плацебо проявилась выраженной тенденцией к увеличению доли пациентов с положительной динамикой (уменьшением общего балла по шкале СДВГ-DSM-IV более чем на 25%) к окончанию 3-го и 4-го месяцев терапии: ответ на лечение достигался у 66,7 и 68,9% соответственно, тогда как в группе плацебо - у 52,3 и 61,4%. Одновременно при лечении пантогамом статистически значимо снижалась тяжесть заболевания по шкале CGI-S по сравнению с плацебо. Через 4 мес терапии пантогамом в сравнении с плацебо уменьшалась выраженность функциональных нарушений по 4 из 6 разделов шкалы WFIRS-P: 'Семья', 'Учеба и школа', 'Самооценка ребенка' и 'Поведение, сопряженное с риском'. Пантогам улучшал у детей с СДВГ также показатели поддерживаемого внимания в тесте Тулуз-Пьерона (качество и скорость выполнения) в сравнении с плацебо. При назначении пантогама в средней суточной дозе 30 мг/кг в течение 4 мес он имел благоприятный профиль безопасности, который не отличается от плацебо.

To assess the efficacy of treatment with encephabol, we examined 40 children, aged from 3 to 5 years, with developmental dysphasia. All patients were randomized into two equal groups: group 1 received encephabol (suspension form, daily dosage 200-250 mg, or 12-15 mg/kg) during 2 months; group 2 did not receive this medication. In the first group, there was a significant improvement of expressive and impressive speech and speech attention; the active vocabulary and a number of phrases in colloquial speech increased by a factor of 3 versus 1.5 in the control group. After the treatment with encephabol, the parents reported the decrease in motor disturbances, psychosomatic disorders, the improvement of attention and the emotional state of the children.

Comorbid anxiety disorders are observed in many patients with attention deficit hyperactivity disorder (ADHD): 25-33% compared to 6-10% in pediatric population. Therapeutic effects of Adaptol have been studied, indicated for children and adolescents with ADHD as monotherapy, in dosages of 25-35 mg/kg for 1-3 months. The positive influence of Adaptol on the core symptoms of ADHD was confirmed: reduction of hyperactivity and impulsivity was registered after 1st month, while clinical manifestations of the attention deficit were decreased on the 2nd-3rd months of the treatment. Anxiolytic activity became apparent from the 1st month and continued to increase on the 2nd-3rd months of the treatment. Simultaneously significant reduction of the oppositional-defiant disorder manifestations were observed.

Thirty-two patients with attention-deficit hyperactivity disorder (ADHD), 23 boys and 9 girls, aged 6-12 years, were examined in two months intervals during the long-term treatment (up to 6-8 months) with pantogam (homopantothenic acid) in daily dosages of 500-1000 mg. The treatment results were evaluated by the ADHD Rating Scale-DSM-IV and The Weiss Functional Impairment Rating Scale - Parent Report (WFIRS-P). While the core symptoms of ADHD were according ADHD-DSM-IV diminished after 2 months, the improvement of WFIRS-P parameters required the longer duration of medical treatment. Only after 4 months of treatment, the improvement was achieved in selfesteem and social activities, and after 6 months in learning and behavior at school as well as in the level of life skills along with the decrease of risky activities. Thus, getting over psychosocial adaptation problems needs the longer treatment duration than the decrease of ADHD core symptoms.

To assess objectively a dynamics of brain functional state, EEG spectral power and peak latency of the P300 component of cognitive auditory evoked potentials have been analyzed in adolescents during the course of nootropic therapy of residual asthenic consequences of traumatic brain injury (ICD-10 F07.2). The study included 76 adolescents, aged 12-18 years, who have undergone severe closed head trauma with brain commotion 1/2--5 years ago. Patients have been divided into 3 groups treated during one month with cerebrolysin, piracetam or magne-B6, respectively. After the end of the nootropic therapy, 77% of patients treated with cerebrolysin as well as 50% of patients treated with piracetam and magne-B6 have demonstrated the positive dynamics of their brain functional state that manifested itself in the appearance of occipital EEG alpha rhythm or in the increase of its spectral power; in the normalization of alpha rhythm frequency; in the decrease in the spectral power of slow wave (theta and delta) EEG activity, in the amount (up to the disappearance) of paroxysmal EEG activity, in the EEG response to hyperventilation and in the shortening of the P300 peak latency. Such positive changes of neurophysiological parameters have been associated with the improvement of clinical conditions of patients and correlated significantly with the dynamics of psychometric scores of attention and memory.

Frequency of comorbid disorders and neuropsychological state, executive functions (EF), were studied in two groups of patients aged from 5 to 14 years: 53 patients with attention-deficit hyperactivity disorder (ADHD) in the association with enuresis and 71 patients with ADHD without enuresis. The most cases of enuresis (50 out of 53 patients) were represented by primary nocturnal enuresis. The significant increase of total number of ADHD cases with comorbidity for oppositional-defiant disorder, anxiety disorder, tics or encopresis (77.7%) was found in the first group compared to the second one (60.6%). The presence of enuresis in ADHD was associated with the significant increase of frequency of anxiety disorders (54.7% versus 39.4%). Moreover, in the group of patients with ADHD and enuresis, the frequency of oppositional-defiant disorder and encopresis was higher in the age of 5-9 years while the frequency of obsessive-compulsive disorder and tics increased in the period of 10-14 years as compared to patients without enuresis. The assessment of executive functions with the Wisconsin Card Sorting Test did not reveal any differences between patients of two groups.

Parents and teachers of 342 school children, aged from 7 to 11 years, were questioned with theStrenghts and Difficulties Questionnaire(SDQ) using non-randomized method. The following comparative analysis of SDQ items measured in parents and teachers of children with ADHD and age-matched controls was carried out. It has been shown that the spectrum of disturbances characteristic of ADHD is not confined only to main symptoms of ADHD. The results of questionnaire of both parents and teachers indicated the marked intensity of emotional disturbances, behavioral problems and difficulties in interactions with peers as well as the underdevelopment of social trends in behavior in children with ADHD compared to the controls. The data obtained confirm the necessity of treatment extension beyond the core symptoms with considering more general parameters of the patients quality of life.

Forty children with dyslexia, aged 7-12 years, have been randomized into two groups in an open controlled study: 18 of them received Nootropil (piracetam) in the dose of 100 mg/kg daily for 2 months and 22 were examined as a control group. Nootropil exerted positive effects on reading and spelling skills in 56% of children with dyslexia who demonstrated the improvement of fluency and accuracy of reading along with a significant decrease of specific errors (dysphonemic and visuospatial) in dictation. In the control group, the improvement was seen only in 10% of children. These data corresponded to the results of neuropsychological testing that demonstrated the improvement of automatic lexical retrieval, phonological skills and sustained attention in dyslexic children treated with nootropil as compared to the control group. The dynamics of interhemispheric asymmetry in EEG alpha-theta-subbands (4-13 Hz) during the performance of incomplete figures recognition test in the nootropil treatment may be indicative of the formation of compensative mechanisms linked with the functional activation of the right hemisphere structures and associative anterior left hemisphere areas which contributed to the progress in reading and spelling skills.

The efficacy of piracetam in treating the sequelae of moderate and severe closed craniocerebral trauma (CCT) in adolescents was evaluated in studies of 42 patients aged 12–18 years who had suffered CCT 1.5–5 years prior to the study. Adolescents of the experimental group (20 individuals) received piracetam (Nootropil) at doses of 40–50 mg/kg (daily daily 1600–2400 mg) for one month; patients of group 2 (22 individuals) served as controls. Piracetam was found to have positive therapeutic effects on impairments to higher mental (memory, attention, executive) and motor (coordination) functions and on measures of the speeds of cognitive and motor operations.

Therapeutic effect of atomoxetine and piracetam has been assessed in the open controlled study included 42 patients with attention deficit hyperactivity disorder (ADHD), aged from 6 to 13 years. Group 1 (16 patients) received atomoxetine (strattera) in daily dosage 0,8-1,2 mg/kg as a monotherapy for 6 weeks. Patients of group 2 (14 children) received piracetam as a monotherapy in daily dosage 50-70 mg/kg for 6 weeks. No pharmacological therapy was conducted in group 3 (a control one) which included 12 patients with ADHD. The high effectiveness of both atomoxetine and piracetam has been shown. However, comparing to piracetam, the therapeutic effect of atomoxetine was reached earlier (two weeks after the beginning of therapy) and was more pronounced for all components of syndromes.

The efficacy of piracetam in the treatment of the consequences of moderate and severe closed traumatic brain injury was assessed in 42 patients, aged 12-18 years, who suffered traumatic disorders 1,5-5 years before this study. Adolescents from the main group (20 patients) received piracetam in dosage of 40-50 mg/kg (or 1600-2400 mg daily) during one month. 22 patients of the second group were examined as controls. The positive therapeutic effects of piracetam on cognitive (memory, attention, executive functions) and motor (coordination) functions as well as the speed of cognitive and motor performance were demonstrated in this study.

Attention deficit hyperactivity disorder (ADHD) is a common behavioral disorder found in children. Comorbid disorders, including oppositional/aggressive behaviors and emotional disorders, may be present in the majority of children diagnosed with ADHD thus complicating an optimal choice of pharmacotherapy. We evaluated the comorbid disorders in an out-patient sample of 76 children with ADHD (59 boys and 17 girls) aged 5-11 years using the Russian version of Kiddie-Shedule for Affective Disorders and Schizophrenia - Present and Lifetime Version. In all cases, a diagnosis met ICD-10 diagnostic criteria. Only 23 (30,3%) subjects had pure ADHD and 53 (69,7%) suffered from at least one comorbid disorder, i.e. oppositional defiant disorder (43,4%), anxiety disorder (39,5%), tics (9,2%), nocturnal enuresis (17,1%) and encopresis (5,3%). The recently published data of double-blind placebo controlled studies are reviewed indicating that in children and adolescents with ADHD and comorbid disorders atomoxetine is superior to placebo in treating ADHD and alleviating the symptoms of comorbid disorders.

Symptomatic focal epilepsy is frequently caused by supratentorial brain tumors that may be surgically removed. The authors studied outcomes of surgical treatment depending on the use of electrocorticography c (ECoG).Seventy-five children, aged 5-7 years, with supratentorial brain tumors were examined. Symptomatic epilepsy was found in 52 (69.3%) patients. Dysembryoplastic neuroepithelial tumors (DNET) and fibrillary astrocytomas were the most epileptogenic tumors.The outcomes on the Engel scale were as follows: in 27 patients with surgical intervention without ECoG: class I - 9 patients, class II - 7 patients, class III - 5 patients, class IV - 6 patients and in 25 patients operated with ECoG: class I - 19 patients, class II - 4 patients and class III - 2 patients. The significant difference (p0.01) between I+II Engel classes in comparison with III+IV Engel classes in operated patients demonstrated the necessity of ECoG during surgery in the resection of supratentorial brain tumors in patients with symptomatic epilepsy.Цель исследования. Симптоматические фокальные формы эпилепсии нередко обусловлены супратенториальными опухолями головного мозга, при которых возможно хирургическое лечение заболевания.
