Introduction: This study assessed the health screening behaviour of Singaporeans and evaluated factors associated with low uptake of screening tests., Materials and Methods: Data from the 2010 National Health Survey, which was conducted on Singapore citizens and permanent residents, was used in this analysis. Multivariate Cox regression was used to evaluate the relationship between sociodemographics and health screening behaviour for selected chronic diseases (hypertension, diabetes and hypercholesterolaemia) and cancers (cervical, breast and colorectal). National recommendations for age at which screening should be initiated and appropriate screening interval were used to define appropriate screening behaviour., Results: More respondents have had their last chronic disease screening done within the recommended time period compared to cancer screening. A total of 77.8%, 63.4% and 54.9% of the respondents had their last hypertension, diabetes and hypercholesterolaemia done within the recommended time period respectively, while less than 50% of the respondents had their cervical (45.8%), breast (32.9%) and colorectal (20.2%) cancer screenings done within the recommended time period. Respondents with higher household income or more years of education were more likely to have undergone screening within the recommended time period. Indians, who are at higher risk of chronic diseases such as diabetes and hypercholesterolaemia, were also more likely to have been screened. A total of 69.9% and 79.5% of the respondents with previously undiagnosed diabetes and hypertension had reported to have done diabetes and hypertension screenings respectively, within the recommended time period., Conclusion: Sociodemographic factors that could be associated with a lower uptake of screening tests include: 1) low household income, 2) low education level, and 3) Malay ethnicity. Health promotion programmes and outreach to these groups can be enhanced to further improve screening uptake.

Background: Health related quality of life (HRQoL) is an important dimension of individuals' well-being, and especially in chronic diseases like diabetes and hypertension. The objective of this study was to evaluate the contributions of disease process, comorbidities, medication or awareness of the disease to HRQoL in diabetes mellitus, hypertension and dyslipidemia., Methods: This was a cross-sectional study of 3514 respondents from the general community in Singapore, assessed for HRQoL, disease and comorbid conditions through self-report, clinical and laboratory investigations. HRQoL was assessed using SF-36 health survey version 2. For each condition, participants were categorized as having 1) no disease, 2) undiagnosed, 3) diagnosed, not taking medication, and 4) diagnosed, taking medication. Analysis used one-way ANOVA and multiple linear regression., Results: Diagnosed disease was associated with lower physical health component summary (PCS) scores across all three conditions. After adjustment for comorbidities, this association remained significant only for those not on medication in diabetes (-2.7±1.2 points, p = 0.03) and dyslipidemia (-1.3±0.4 points, p = 0.003). Diagnosed hypertension (no medication -2.6±0.9 points, p = 0.002; medication -1.4±0.5 points, p = 0.004) and dyslipidemia (no medication -0.9±0.4 points, p = 0.03; medication -1.9±0.5 points, p<0.001) were associated with lower mental health component summary (MCS) scores. Undiagnosed disease was associated with higher MCS in diabetes (2.4±1.0 points, p = 0.01) and dyslipidemia (0.8±0.4 points, p = 0.045), and PCS in hypertension (1.2±0.4 points, p = 0.004)., Conclusions: Disease awareness was associated with lower HRQoL across the diseases studied, with PCS associations partially mediated by comorbidities. Equally importantly, undiagnosed disease was not associated with HRQoL deficits, which may partly explain why these individuals do not seek medical care.

Background: The effect of "pre-hypertension" (pre-HTN) itself, and in combination with other cardiovascular disease (CVD) risk factors in relation to mortality has not been assessed in Asian populations., Methods: From three cross-sectional studies conducted in Singapore (baseline 1982-1995), 5830 persons were grouped into normotensive (NT), pre-HTN or hypertensive (HTN). Follow-up (median 12.0 yrs, IQR 12-19 yrs) was done by linkage to the National Death Register. Outcomes included all-cause and CVD (ischaemic heart disease (ICD-9 410-414) and cerebrovascular accidents (ICD-9 430-438)). Cox's proportional hazards model was used to obtain adjusted hazard ratios (HRs) for risk of mortality., Results: After adjustment, pre-HTN was not associated with a statistically significant increased risk of all-cause or CVD mortality. However, an increased risk for all-cause and CVD mortality, in the presence of diabetes (adjusted HR 1.8; 95%CI 1.0-2.9 and 4.4; 95%CI 1.9-10.4), smoking (adjusted HR 2.2; 95%CI 1.3-3.5 and 4.9; 95%CI 1.8-13.3), and especially, pre-existing CVD (adjusted HR 3.1; 95%CI 1.5-6.4 and 9.3; 95%CI 3.3-25.9) was found., Conclusions: Pre-HTN was not an independent risk factor for mortality. However an increased risk of mortality for pre-HTN in the presence of other CVD risk factors was observed. This finding supports current recommendations to screen for these risk factors in the pre-HTN Asian population.

Objectives: To investigate demographic and cardiovascular disease risk factors associated with awareness, treatment and control of hypertension in a multi-ethnic Asian population., Methods: Participants from four previous cross-sectional studies were invited for a repeat examination (2004--2007). Information of demographic details and cardiovascular disease risk factors was obtained using questionnaire, physical examination and blood tests. Odds ratios and 95% confidence intervals were calculated using multiple logistic regression models., Results: The final number of respondents was 5022 (response rate 49.7%). Although hypertension treatment (84.4% of those aware of hypertension) was high, awareness (51.8% of those having hypertension) and control (27.1% of hypertension on treatment) were low. Reduced awareness and treatment were associated with being younger, never married, and working adults with a higher education level. Low socioeconomic status individuals were more likely to be treated but had poorer control. A similar relationship was found for treatment and control for individuals with coexisting cardiovascular disease risk factors. The use of multiple drug classes was not associated with better control. Diuretic use for treatment of hypertension, as recommended by local and international guidelines, was not common (15.0% of all hypertension medications used)., Conclusions: The awareness, treatment, and control of hypertension in Singapore can be improved. There is a need to improve awareness through education and target screening and treatment in younger, working adults with higher levels of education and higher economic status, as well as Malays. Control of hypertension could be improved among older or diabetic hypertensive individuals.

We describe the centile trends of the blood pressure, glycemia and lipid profiles as well as renal function of a representative population who participated in the Singapore National Health Survey in 2010. Representative survey population was sampled in two phases, first using geographical/ residential dwelling type stratification, followed up ethnicity. 2,407 survey participants without any self-reported medical or medication history for diabetes mellitus, hypertension and dyslipidemia were included in this analysis. All biochemistry analyses were performed on Roche platforms. After excluding outliers using Tukey's criteria, the results of the remaining participants were subjected to lambda-mu-sigma (LMS) analysis. In men, systolic blood pressure increased linearly with age. By contrast, an upward inflection around late 40s was seen in women. The diastolic blood pressure was highest in men in the late 30s-50s age group, and in women in the late 50s-60s age group. All glycemia-related parameters, i.e. fasting and 2-hour plasma glucose and HbA1c concentrations increased with age, although the rate of increase differed between the tests. Total cholesterol and LDL-cholesterol concentrations increased with age, which became attenuated between the early 30s and late 50s in men, and declined thereafter. In women, total cholesterol and LDL-cholesterol concentrations gradually increased with age until late 30s, when there is an upward inflection, plateauing after late 50s. Our findings indicate that diagnostic performance of laboratory tests for diabetes may be age-sensitive. Unfavourable age-related cardiovascular risk profiles suggest that the burden of cardiovascular disease in this population will increase with aging population. [ABSTRACT FROM AUTHOR]

Recent studies suggest that no distinct glycemic threshold consistently differentiates individuals with or without retinopathy. The authors sought to determine whether the same was true for other microvascular complications. They studied 5,094 participants with fasting plasma glucose values and concurrent microvascular complications from 4 previous cross-sectional surveys carried out in Singapore (1982–1998) who attended a follow-up examination in 2004–2007. Peripheral neuropathy was diagnosed based on abnormal responses to a 10-g monofilament or neurothesiometer test. Chronic kidney disease was defined in various ways by using albuminuria (urine albumin:creatinine ratio >30 μg/mg) and estimated glomerular filtration rate, alone and in combination. Prevalence of peripheral neuropathy was 7.5%. For chronic kidney disease, prevalence of albuminuria only was 10.5%, estimated glomerular filtration rate of <60 mL/minute per 1.73 m2 only was 4.1%, and both was 2.1%. Prevalence of peripheral neuropathy and chronic kidney disease gradually increased in relation to fasting plasma glucose, beginning at levels below the existing diagnostic threshold for diabetes mellitus of 7.0 mmol/L (126 mg/dL). For chronic kidney disease, these associations persisted after adjustment for age, gender, ethnic group, and hypertension. Current diagnostic thresholds for diabetes mellitus have limited sensitivity for identifying individuals with these microvascular complications.
