Purpose: To compare objective and subjective parameters in image quality and radiation dose of two MDCTs (helical 64 detector CT vs. axial 256 detector CT) in paediatric lung CT., Methods: Radiation dose and image quality were compared between non-enhanced lung CT from a helical 64-slice multidetector CT (MDCT 1) and a 256-slice scanner (MDCT 2) with axial wide-cone acquisition and using deep learning image reconstruction. In 23 size-matched paediatric studies (age 2-18 years) from each scanner, the radiation exposure, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), image sharpness and delineation of small airways were assessed. Subjective image quality was rated by 6 paediatric radiologists., Results: While MDCT 2 provided higher SNR and CNR, subjective image quality was not significantly different between studies from both scanners. Radiation exposure was lower in studies from MDCT 2 (CTDIvol 0.26 ± 0.14 mGy, effective dose 0.23 ± 0.11 mSv) than from MDCT 1 (CTDIvol 0.96 ± 0.52 mGy, effective dose 1.13 ± 0.58 mSv), p < 0.001. Despite lower radiation dose for the scout images, the relative scout-scan-ratio increased from 2.64 ± 1.42 % in MDCT 1 to 6.60 ± 5.03 % in MDCT 2 (p = 0.001)., Conclusions: By using latest scanner technology effective radiation dose can be reduced to 0.1-0.3 mSv for lung CT in children without compromising image quality. Scout image dose increasingly accounts for substantial portions of the total scan dose and needs to be optimized. In children CT should be performed on state-of-the-art MDCT scanners with size-adapted exposure protocols and iterative reconstruction., (Copyright © 2021 The Author(s). Published by Elsevier B.V. Accurate tracking of organ motion during treatment is needed to improve the efficacy of radiation therapy. This work investigates the feasibility of tracking an uncontoured target using the motion detected within a moving treatment aperture. Tracking was achieved with a weighted optical flow algorithm, and three different techniques for updating the reference image were evaluated. The accuracy and susceptibility of each approach to the accumulation of position errors were verified using a 3D-printed tumor (mounted on an actuator) and a virtual treatment aperture. Tumor motion up to 15.8 mm (peak-to-peak) taken from the breathing patterns of seven lung cancer patients was acquired using an amorphous silicon portal imager at ~ 7.5 frames/s. The first approach (INI) used the initial image detected, as a fixed reference, to determine the target motion for each new incoming image, and performed the best with the smallest errors. This method was also the most robust against the accumulation of position errors. Mean absolute errors of 0.16, 0.32, and 0.38 mm were obtained for the three methods, respectively. Although the errors are comparable to other tracking methods, the proposed method does not require prior knowledge of the tumor shape and does not need a tumor template or contour for tracking. Purpose: The goal of this study was to assess the experimental feasibility of circular multistatic holography, a novel breast microwave radar reconstruction approach, using experimental datasets recorded using a preclinical experimental setup. The performance of this approach was quantitatively evaluated by calculating the signal to clutter ratio (SCR), contrast to clutter ratio (CCR), tumor to fibroglandular response ratio (TFRR), spatial accuracy, and reconstruction time., Methods: Five datasets were recorded using synthetic phantoms with the dielectric properties of breast tissue in the 1-6 GHz range using a custom radar system developed by the authors. The datasets contained synthetic structures that mimic the dielectric properties of fibroglandular breast tissues. Four of these datasets the authors covered an 8 mm inclusion that emulated a tumor. A custom microwave radar system developed at the University of Manitoba was used to record the radar responses from the phantoms. The datasets were reconstructed using the proposed multistatic approach as well as with a monostatic holography approach that has been previously shown to yield the images with the highest contrast and focal quality., Results: For all reconstructions, the location of the synthetic tumors in the experimental setup was consistent with the position in the both the monostatic and multistatic reconstructed images. The average spatial error was less than 4 mm, which is half the spatial resolution of the data acquisition system. The average SCR, CCR, and TFRR of the images reconstructed with the multistatic approach were 15.0, 9.4, and 10.0 dB, respectively. In comparison, monostatic images obtained using the datasets from the same experimental setups yielded average SCR, CCR, and TFRR values of 12.8, 4.9, and 5.9 dB. No artifacts, defined as responses generated by the reconstruction method of at least half the energy of the tumor signatures, were noted in the multistatic reconstructions. The average execution time of the images formed using the proposed approach was 4 s, which is one order of magnitude faster than the current state-of-the-art time-domain multistatic breast microwave radar reconstruction algorithms., Conclusions: The images generated by the proposed method show that multistatic holography is capable of forming spatially accurate images in real-time with signal to clutter levels and contrast values higher than other published monostatic and multistatic cylindrical radar reconstruction approaches. In comparison to the monostatic holographic approach, the images generated by the proposed multistatic approach had SCR values that were at least 50% higher. The multistatic images had CCR and TFRR values at least 200% greater than those formed using a monostatic approach.

The performance of this approach was quantitatively evaluated by calculating the signal to noise ratio, contrast to noise ratio, spatial accuracy, and reconstruction time., Methods: Six datasets were recorded, three corresponding to fatty cases and three containing synthetic dense tissue structures. Five of these datasets contained an 8 mm inclusion that emulated a malignant lesion. The data were acquired from synthetic phantoms that mimic the dielectric properties of breast tissues in the 1-6 GHz range using a custom experimental breast microwave radar system. The spatial accuracy and signal to noise ratio of the reconstructed was calculated for all the reconstructed images. The contrast to noise ratio of the reconstructed images corresponding to the datasets containing fibroglandular tissue regions was determined. This was done to evaluate the ability of the circular holographic method to provide images in which the responses from tumors can be distinguished from adjacent dense tissue structures. The execution time required to form the images was also measured to evaluate the data throughput of the holographic approach., Results: For all the reconstructed datasets, the location of the synthetic tumors in the experimental setup was consistent with its position in the reconstructed image. The average spatial error was 2.2 mm, which is less than half the spatial resolution of the data acquisition system. The average signal to noise ratio of the reconstructed images containing an artificial malignant lesion was 8.5 dB, while the average contrast to noise ratio was 6.7 dB. The reconstructed images presented no artifacts. The average execution time of the images formed using the proposed approach was 5 ms, which is six orders of magnitude faster than current state of the art breast microwave radar (BMR) reconstruction algorithms., Conclusions: The results show that circular holography is capable of forming accurate images with signal to noise levels higher than 8 dB in quasi real time. Compared to BMR reconstruction algorithms tested on datasets containing dense tissue structures, the holographic approach generated images of similar spatial accuracy with higher signal to noise ratios and an acceleration factor of one order of magnitude.

Breast CT is an emerging modality that reconstructs 3D linear attenuation coefficient (μ) images of the breast. Its tomographic nature reduces the overlap of structures and may improve tissue visualization. Current prototype systems produce large levels of scatter that could be used to reconstruct electron density (ρ &#95;{e}) images. This could potentially enhance diagnosis. We are developing a first generation bench top CT system to investigate the benefits of simultaneous imaging μ and ρ &#95;{e} of the intact breast. The system uses an algorithm capable of reconstructing ρ &#95;{e} images from single Klein-Nishina scatter. It has been suggested that this algorithm may be impractical since measurements include coherent, bound incoherent and multiple scatter. To investigate this, the EGSnrc Monte Carlo (MC) code was used to simulate scans using a first generation system. These simulations were used to quantify the dose per scan, to provide raw data for the ρ &#95;{e} reconstructions and to investigate corrections for multiple and coherent scatter since these can not be directly related to ρ &#95;{e}. MC simulations show that the dose coefficients are similar to those of cone beam breast CT. Coherent scatter is only ∼9% concentrated in scattering angles < 8°. Electron binding reduces the number of incoherently scattered photons but this reduction can be included in the quantification of scatter measured by the system. Multiple scatter was found to be the major source of errors and, if not corrected for, can result in an overestimation of ρ &#95;{e} by more than a factor of two. Empirical corrections, based on breast thickness or radiological path, can be used to reconstruct images where the variance in ρ &#95;{e} error is half of that found in images derived from primary photons only. Although some practical challenges remain in creating a laboratory system, this work has shown that it is possible to reconstruct scatter images of the breast with a 4 mGy dose and further experimental evaluation of this technique is warranted.

Regularization methods are used in microwave image reconstruction problems, which are ill-posed. Traditional regularization methods are usually problem-independent and do not take advantage of a priori information specific to any particular imaging application. In this paper, a novel problem-dependent regularization approach is introduced for the application of breast imaging. A real genetic algorithm (RGA) minimizes a cost function that is the error between the recorded and the simulated data. At each iteration of the RGA, a priori information about the shape of the breast profiles is used by a neural network classifier to reject the solutions that cannot be a map of the dielectric properties of a breast profile. The algorithm was tested against four realistic numerical breast phantoms including a mostly fatty, a scattered fibroglandular, a heterogeneously dense, and a very dense sample. The tests were also repeated where a 4 mm x 4 mm tumor was inserted in the fibroglandular tissue in each of the four breast types. The results show the effectiveness of the proposed approach, which to the best of our knowledge has the highest resolution amongst the evolutionary algorithms used for the inversion of realistic numerical breast phantoms.

In this paper, we describe a 2-D wideband microwave imaging system intended for biomedical imaging. The system is capable of collecting data from 3 to 6 GHz, with 24 coresident antenna elements connected to a vector network analyzer via a 2 x 24 port matrix switch. As one of the major sources of error in the data collection process is a result of the strongly coupling 24 coresident antennas, we provide a novel method to avoid the frequencies where the coupling is large enough to prevent successful imaging. Through the use of two different nonlinear reconstruction schemes, which are an enhanced version of the distorted born iterative method and the multiplicative regularized contrast source inversion method, we show imaging results from dielectric phantoms in free space. The early inversion results show that with the frequency selection procedure applied, the system is capable of quantitatively reconstructing dielectric objects, and show that the use of the wideband data improves the inversion results over single-frequency data.

Pencil beam kernels describing scattered photon fluence behind homogeneous water slabs at various air gap distances were generated using the EGS Monte Carlo code. Photon scatter fluence was scored in separate bins based on the particle's history: singly scattered, multiply scattered, and bremsstrahlung and positron annihilation photons. Simultaneously, the mean energy and mean angle with respect to the incident photon pencil beam were tallied. Kernels were generated for incident photon pencil beams exhibiting monoenergetic spectra of 2.0 and 10.0 MeV, and polyenergetic spectra representative of 6 and 24 MV beams. Reciprocity was used to generate scatter fractions on the central axis for various field sizes, phantom thicknesses, and air gaps. The scatter kernels were further characterized by full width at half-maximum estimates. Modulation transfer functions were calculated, providing theoretical estimates of the limit of performance of portal imaging systems due to the intrinsic scattering of photon radiation through the patient.

The energy spectra and the dose to a Cu plate/Gd2O2S phosphor portal imaging detector were investigated for monoenergetic incident beams of photons (1.25, 2, and 5 MeV). The Monte Carlo method was used to characterize the influence of the patient/detector geometry, detector material and design, and incident beam energy on the spectral distribution and the dose, at the imaging detector plane, of a photon beam scattered from a water phantom. The results show that radiation equilibrium is lost in the air gap and that, for the geometries studied, this effect led to a reduction in the exit dose of up to 40%. The finding that the effects of the air gap and field size are roughly complementary has led to the hypothesis that an equivalent field size concept may be used to account for intensity and spectral changes arising from air gap variations. The copper plate preferentially attenuates the low-energy scattered photons incident on it, while producing additional annihilation, bremsstrahlung, and scattered photons. As a result, the scatter spectra at the copper surface entrance of the detector differs significantly from that at the Cu/phosphor interface. In addition, the mean scattered photon energy at the interface was observed to be roughly 0.4 MeV higher than the corresponding effective energy for 2 MeV incident beams. A comparison of the dose to various detector materials showed that exit dosimetry errors of up to 24% will occur if it is assumed that the Cu plate/Gd2O2S phosphor detector is water equivalent.
