Objectives:To evaluate changes in bone mineral density (BMD) in the hands, hip and spine after 1 and 2 years of follow-up, in relation to antirheumatic and antiresorptive therapies and disease and demographic variables in patients with recent-onset rheumatoid arthritis (RA).Methods:Changes in BMD measured in metacarpals 2–4 by digital x-ray radiogrammetry and in the hip and spine by dual energy x-ray absorptiometry were assessed at baseline and after 1 and 2 years of follow-up in 218 patients with recent-onset RA from the BeSt study, who received one of four treatment strategies: sequential monotherapy (group 1); step-up combination therapy (group 2); initial combination therapy with tapered high-dose prednisone (group 3); or initial combination therapy with infliximab (group 4).Results:After 1 and 2 years, there was significant BMD loss in all locations, with significantly greater BMD loss in the hands than generalised BMD loss in the hip and spine. Initial combination therapy with prednisone or infliximab were associated with less hand BMD loss compared with initial monotherapy after 1 and 2 years (−0.9 and −1.6%, −0.6 and −1.4%, −1.7 and −3.3%, and −2.6 and −3.6% for group 4–1 after 1 and 2 years, overall p = 0.001 and p = 0.014, respectively).Progression in erosions was independently associated with increased BMD loss both in the hands and hip after 1 year. The use of bisphosphonates protected only against generalised BMD loss in the hip and spine.Conclusions:The association between joint damage progression and both hand and generalised BMD loss in RA suggests common pathways between these processes, with hand BMD loss occurring earlier in the disease course than generalised BMD loss.

The Sharp–van der Heijde method (SHS; range 0–448)1 is an often used and well-validated method to assess joint damage on x-rays in rheumatoid arthritis (RA). It is a comprehensive method that requires training, making it less feasible for clinical practice. The simplified erosion and narrowing score (SENS; range 0–86)2 scores the same joints as SHS, but without grading of damage per joint, making it quicker and easier to learn. Using SENS takes 7 min for seven sets of radiographs, compared with 25 min for seven sets using SHS.2 In earlier studies, SENS showed good reliability and responsiveness, but these studies either had no restrictions in disease duration or limited patient numbers.2–4 We aimed to compare the properties of SENS and SHS in a large group of early, intensively treated RA patients with limited joint damage during 5 years of follow-up in the BeSt (Behandel Strategieen [Treatment Strategies]) study. Objective. To determine the efficacy of minocycline in the treatment of rheumatoid arthritis (RA). Methods. Minocycline (maximal oral daily dose 200 mg) or placebo was administered in a 26-week, randomized, double-blind study to 80 patients with active RA, who were treated or had previously been treated with at least one disease-modifying antirheumatic drug. Results. There were 15 premature discontinuations: 6 (5 taking minocycline) because of adverse effects, 8 (all taking placebo) because of lack of efficacy, and 1 (taking placebo) because of intercurrent illness. There was a statistically significant improvement in the minocycline group over the placebo group. There was a pronounced improvement in laboratory parameters of disease activity; however, improvement in clinical parameters was less impressive. The observed adverse effects attributable to minocycline were mainly gastrointestinal symptoms and dizziness. Conclusion. The results of the present study suggest that minocycline is beneficial and relatively safe in RA patients.
