Objective Watching if long acting testosterone added to sildenafil can maintain improved sexual life, reflected by a longest erectile capability and sexual desire, in erectile dysfunction (ED) associated with metabolic syndrome (MetS). Design and method In 49 men with MetS (NCEP-ATP III criteria) ED was assessed (V1) using IIEF questionnaire and its domains: erectile function (EF) and sexual desire (SD). ED was treated by sildenafil citrate (S) in 2-4 tb. of 50 mg/week for 2 months. Group A = 24 participants receiving also a long acting testosterone (T) injection. Group B = 25 cases treated only with sildenafil. At 10 days (V2) and 30 days (V3) and immediate (V2) and at 30 days (V3) after S end usage EF and SD respectively were assessed. Results IIEF (V1): Gr. A vs. Gr. B = 44.16 vs. 43.67 points (p). EF domain scores: Gr. A: V1=16.2 p; V2=26.34 p (P=0.002); V3=20.89 p (P=0.07). Gr. B: V1=14.1 p; V2=23.3 (P=0.001); V3=15.9 (P=0.5). V2: Gr. A vs. Gr. B=26.34 vs. 23.3 (P=0.45). V3: Gr. A vs. Gr. B=20.89 vs. 15.9 (P=0.07). SD domain scores: Gr. A: V1=4.7 p; V2=8.9 p (P=0.001); V3=8.5 p (P=0.08). Gr. B: V1=5.7 p; V2=9.1 p (P=0.002); V3=4.9 p (P=0.15). V2: Gr. A vs. Gr. B=8.9 vs. 9.1 (P=0.6); V3: Gr. A vs. Gr. B=8.5 vs. 4.9 (P=0.001). Conclusions Long acting testosterone really improve sexual life by prolonging a satisfying prevalence of non phosphodiesteraze-5 inhibitor induced erections and maintaining sexual desire after sildenafil usage.
