Purpose: The aim of the study was to evaluate and compare the long-term visual outcomes and higher-order aberrations (HOAs) between femtosecond laser-assisted in situ keratomileusis (FS-LASIK) and small-incision lenticule intrastromal keratoplasty (SMI-LIKE) in the correction of moderate to high hyperopia., Methods: In this study, 16 subjects (20 eyes) underwent FS-LASIK and 7 subjects (10 eyes) underwent SMI-LIKE. Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), manifest refraction, mean keratometry (Km), anterior asphericity (Q), and HOAs preoperatively and 2 years postoperatively were obtained in both procedures., Results: The efficacy indices of the FS-LASIK group and the SMI-LIKE group were 0.85 ± 0.14 and 0.87 ± 0.17, respectively. The safety indices of the FS-LASIK and SMI-LIKE groups were 0.99 ± 0.15 and 1.08 ± 0.24, respectively. No significant difference in safety index or efficacy index was found between the FS-LASIK and SMI-LIKE groups (all P > 0.05). The correlation coefficient of the attempted versus achieved spherical equivalent postoperatively was 0.69 ( P < 0.01) and 0.89 ( P < 0.01) in the FS-LASIK group and SMI-LIKE groups, respectively. The front Km, negative Q value, negative spherical aberrations (SAs), coma, and total HOAs significantly increased postoperatively in the 2 groups ( P < 0.05). The FS-LASIK group had greater changes in Q value and SA postoperatively than the SMI-LIKE group ( P < 0.01)., Conclusions: SMI-LIKE had similar safety and efficacy to FS-LASIK in the correction of moderate to high hyperopia. Background: Few studies have reported the visual outcomes of small-incision lenticule extraction (SMILE) and laser-assisted subepithelial keratomileusis (LASEK) for myopia correction. This study aims to compare the visual quality and corneal wavefront aberrations after SMILE and LASEK for low-myopia correction., Methods: In this prospective study, we included 29 eyes of 29 patients who received SMILE and 23 eyes of 23 patients who received LASEK between June 2018 and January 2019. The following measurements were assessed: uncorrected (UDVA) and corrected (CDVA) distance visual acuity, manifest refraction, corneal wavefront aberrations, and subjective visual quality. All patients were followed up for two years., Results: All procedures were uneventful. An efficacy index of 1.19 ± 0.17 was established in the SMILE group and 1.23 ± 0.20 in the LASEK group. No eyes lost more than two lines of CDVA. We found that 93% (27/29) of the treated eyes in the SMILE group and 91% (21/23) in the LASEK group had spherical equivalent (SE) within ± 0.25D. The increases in the total corneal spherical aberration and the corneal front spherical aberration were lower in the SMILE group than in the LASEK group (P < 0.01). In contrast, the increases in the total corneal vertical coma and the corneal front vertical coma in the SMILE group were greater than those in the LASEK group (P < 0.01)., Conclusion: Both SMILE and LASEK have good safety, stability, and patient-reported satisfaction for low myopia. Purpose: To investigate optical zone decentration following femtosecond laser-assisted lenticule intrastromal keratoplasty (FS-LIKE) or small incision lenticule intrastromal keratoplasty (SMI-LIKE) for correcting hyperopia., Methods: This study analyzed decentration values obtained from optical coherence tomography (OCT) and tangential topography difference maps of 23 eyes (18 patients) undergoing FS-LIKE (n = 12) or SMI-LIKE (n = 11) via the concentric centration method. Total higher order aberrations (HOAs) and component aberrations were measured preoperatively and 6 months postoperatively., Results: The mean optical zone decentration was 0.27 ± 0.08 and 0.39 ± 0.16 mm for the FS-LIKE and SMI-LIKE groups, respectively ( P = .039). A significant difference was noted in lenticule decentration between the two groups (0.18 vs 0.37 mm), whereas no significant difference was observed in stromal bed (pocket) decentration between the two groups (0.10 vs 0.12 mm). Six months after surgery, the spherical equivalent showed a reduction of 6.14 ± 2.44 and 6.10 ± 1.79 diopters (D) for the two groups, respectively ( P = .971), whereas the surgically induced astigmatism was 0.68 ± 0.49 and 1.56 ± 0.78 D for the two groups, respectively ( P = .004). Furthermore, induction of HOAs in the SMI-LIKE group was significantly larger than that in the FS-LIKE group ( P = .013)., Conclusions: FS-LIKE can yield improved treatment centration and less induction of total HOAs. Purpose: To investigate morphological changes in the cornea and the implanted lenticule in patients with corneal ectasia after laser in situ keratomileusis (LASIK) and implantation using a lenticule from small incision lenticule extraction (SMILE)., Methods: Eight eyes of 8 patients with ectasia after LASIK from 2015 to 2018 at the Eye and ENT Hospital of Fudan Unversity were analyzed. For recipient eyes, lenticules were transferred with a mean cryopreserved time of 20 ± 21 days extracted from SMILE donors to the exposed stromal bed after lifting the flaps and the flap was repositioned. Uncorrected and corrected distance visual acuity, manifest refraction, and corneal topography were performed. Patients were followed up for 3 years. Changes in lenticules and recipient corneas were analyzed using in vivo confocal microscopy., Results: Activated keratocytes in the anterior and posterior stroma were observed near the lenticule at the first 6 months. Elongated, deformed keratocyte nuclei were found in the implanted lenticules in postoperative year 1. The anterior and posterior interfaces showed an absence or decrease of keratocytes and the presence of small particles of varying brightness. Nerve fibers in the implanted lenticules were detected in one case. No obvious difference was detected between cryopreserved and fresh lenticules., Conclusions: The preliminary findings suggest that recipient keratocytes repopulate the lenticules and the keratocytes in lenticules gradually return to their normal morphology. Purpose: The aim of this study was to compare visual outcomes after small incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for high myopia., Methods: In this prospective, comparative study, a total of 52 eyes of 34 consecutive highly myopic patients with spherical equivalent within the range of -8.00 to -10.00 diopters were recruited. Twenty-three eyes of 16 patients were in the FS-LASIK group and 29 eyes of 18 patients were in the SMILE group. Visual outcomes and wavefront aberrations were analyzed preoperatively and 6 months postoperatively., Results: At the postoperative 6-month visit, 96.6% in the SMILE group and 91.3% in the FS-LASIK group achieved unchanged or better corrected distance visual acuity (CDVA). As for uncorrected distance visual acuity (UDVA), 96.6% in the SMILE group and 95.7% in the FS-LASIK group achieved UDVA of 20/20 or better. As for wavefront aberrations, high-order aberrations (HOAs) and spherical aberrations increased significantly after surgery in both groups relative to corresponding preoperative values (p < 0.001), and vertical coma increased after SMILE (p < 0.001). No statistically significant differences in changes of HOAs (p =0.90), spherical aberrations (p = 0.07), horizontal coma (p = 0.56), coma (p =0.08), horizontal trefoil (p =0.19), vertical trefoil (p = 0.90), and trefoil (p = 0.45) were detected between the 2 groups, except for vertical coma (p < 0.01)., Conclusions: SMILE is as effective as FS-LASIK in correcting high myopia, but attention should be paid to the induction of vertical coma in highly myopic patients following a SMILE procedure., (© 2019 S. Background: This study compares the clinical outcomes of femtosecond laser small-incision lenticule extraction (SMILE) for the correction of myopia and myopic astigmatism greater than - 10 D, and - 10 D or less respectively., Methods: 60 eyes/patients were equally selected into group 1 (myopia and myopic astigmatism of - 10 D or less) and group 2 (myopia and myopic astigmatism of over - 10 D), both of which were treated with SMILE. Visual and refractive outcomes, corneal higher-order aberrations, and Bowman's layer micro-distortions were evaluated preoperatively, 3 months, and 6 months postoperatively., Results: LogMAR corrected distance visual acuity (CDVA) of group 1 and group 2 was - 0.069 ± 0.047 and - 0.053 ± 0.073 6 months postoperatively (P = 0.48). 100% eyes in group 1 and 97% in group 2 were within 1 D of targeted correction (P = 0.45). Meanwhile, 100% eyes in group 1 and 97% in group 2 had an uncorrected distance visual acuity of 20/25 or better (P = 0.20). Changes in corneal higher-order aberrations root mean square, coma, and trefoil were similar between the two groups but spherical aberration was higher in group 2 (P < 0.01). Micro-distortions were observed in 53% in group 1 and 77% in group 2. More micro-distortions were observed in group 2 (3.40 ± 2.66) than in group 1 (2.07 ± 2.29) (P = 0.041). The total number of micro-distortions was not correlated with postoperative CDVA (P = 0.77)., Conclusions: Visual outcomes showed similar results of SMILE for myopic correction of > - 10 D and ≤ - 10 D. Refractive outcomes showed slightly under-correction in higher myopic eyes. Higher myopic treatment tends to induce more spherical aberrations. Micro-distortions had no impact in visual and refractive outcomes.

Purpose: To compare visual outcomes and aberration outcomes in small incision lenticule extraction (SMILE) versus femtosecond laser LASIK (FS-LASIK)., Methods: This prospective, comparative, nonrandomized clinical study included 68 eyes of 37 patients receiving SMILE and 55 eyes of 30 patients receiving FS-LASIK between December 2011 and January 2013 at the Fudan University Eye and ENT Hospital (Shanghai, People's Republic of China). Patients were followed up at 3, 6 months and 5 years after surgery. Main outcome measurements included uncorrected (UDVA) and corrected (CDVA) distance visual acuity, manifest refraction, central corneal thickness (CCT), total corneal refractive power (TCRP) and wavefront aberrations., Results: About 98% (49/50) of treated eyes in the SMILE group and 95% (39/41) in the FS-LASIK group had a postoperative logMAR UDVA of 0 or better. Spherical equivalent (SE) after 5 years was -0.01 ± 0.35 D in the SMILE group and -0.23 ± 0.41 D in the FS-LASIK group. A regression of -0.02 ± 0.39 D in the SMILE group and -0.12 ± 0.32 D in the FS-LASIK group was observed between 6 months and 5 years postoperative time-points. TCRP increased by 0.39 ± 0.38 D in the SMILE group and 0.45 ± 0.49 D in the FS-LASIK group between 6-month and 5-year time-points. However, no statistically significant difference was found between the SMILE and FS-LASIK groups in terms of ∆SE, ∆CCT and ∆TCRP between 6 months and 5 years postoperative time-points., Conclusion: Myopic regression was observed in terms of TCRP but not in subjective refraction. Background: To report a first case of lenticule addition and corneal cross-linking for post-LASIK ectasia with associated corneal thinning., Case Presentation: Lenticule addition followed by corneal cross-linking was performed on the left eye of a patient with post-LASIK ectasia. Postoperatively, the corneal power and elevation were stable with a remarkable improvement in corneal thickness, and the lenticule had merged with the adjacent corneal stroma at 30 months follow-up. The patient's corrected distance visual acuity gained two lines., Conclusions: This case provides a potential treatment option for patients with keratectasia and keratoconus in association with thin corneal thickness (less than 400 μm) and may provide the benefit of delaying or avoiding the need for keratoplasty, which has its own associated complications and is limited by the availability of donor corneas.

Purpose: To investigate the long-term changes in anterior corneal structure after surface ablation., Methods: In this retrospective study, patients who received surface ablation including laser-assisted subepithelial keratomileusis (LASEK) and epipolis laser in situ keratomileusis at the Department of Ophthalmology of Fudan University Eye and Ear, Nose and Throat (EENT) Hospital (Shanghai, People's Republic of China) were telephoned. Patients were asked to follow-up at the refractive center. Changes in the anterior cornea (from the epithelium to the anterior stroma) were examined by optical coherence tomography (OCT) and in vivo confocal microscopy., Results: Thirty-four eyes of 18 patients (10 years or more after operation), 16 eyes of 8 patients (4 years after operation), 12 eyes of 6 patients (1 year after operation), 8 eyes of 4 patients (6 months after operation), and 12 eyes of 6 patients (1 month after operation) were included. Under OCT, a smooth, continuous, and highly reflective demarcation line between the epithelial layer and the stroma was noted in all eyes that received surgeries more than 1 year previously. For eyes at 6 months after operation, the complete formation of this demarcation line was detected in 12.5% (1/8) of eyes and a partial formation of this demarcation line was observed in 87.5% (7/8) of eyes. A partial formation of this demarcation line was observed in 100% (12/12) of eyes in patients at 1 month after surgery., Conclusions: A demarcation line in the human cornea can be detected after corneal surface ablation. It was completely formed around postoperative 6 to 12 months. The functions and components of this structure merit investigation.

Objective: To investigate the factors associated with optical and visual quality of advanced surface ablation in high myopia., Design: A cross-sectional study of high myopic eyes treated with laser epithelial keratomileusis (LASEK)/epipolis laser in situ keratomileusis (Epi-LASIK). SETTING6: Eye and ENT Hospital of Fudan University in Shanghai., Methods: One hundred and thirty-eight high myopic eyes (138 patients) (myopia -6 D or more) were examined more than 12 months after LASEK or Epi-LASIK with advanced surface ablation on the MEL 80 excimer laser (Zeiss AG, Jena, Germany). Refraction, higher order aberrations (HOAs) and contrast sensitivity before and after surgery were evaluated. Factors including preoperative refraction, age, gender, central corneal thickness, pupil size, optical diameter, ablation depth and flap creation method were analysed for association with postoperative high-order aberration, contrast and glare sensitivities, and different analytic diameters., Results: HOAs increased significantly postoperatively (p<0.05), with the most significant change found in Z(spherical aberration). At a 5 mm analysis diameter, increased coma was associated with age; increased spherical aberration difference was associated with age, optical zone diameter and method of epithelial flap creation. At a 3 mm analysis diameter, none of the factors contributed to changes in HOAs. Higher preoperative refractive error was associated with decreased contrast and glare sensitivity at each spatial frequency., Conclusion: A larger optical zone diameter design is recommended to achieve better visual quality in advanced surface ablation for high myopia correction. Purpose: To describe a case in which post-LASIK corneal ectasia was treated using an allogeneic lenticule implanted into the recipient eye., Methods: A 29-year-old man presented with bilateral ectasia 10 years after LASIK. Manifest refraction was -1.50 -3.25 × 15° in the right eye and -13.50 -6.00 × 10° in the left eye. Because of contact lens intolerance in the left eye, the authors decided to implant an allogenic corneal lenticule, created by small incision lenticule extraction with the VisuMax laser (Carl Zeiss Meditec, Jena, Germany) in a donor eye with a refraction of +5.75 -0.50 × 15°. The intended peripheral thickness was 116 μm and the central thickness was 20 μm. The lenticule was placed under the original LASIK flap in the left eye., Results: Ten months after surgery, refraction of the left eye had improved to -3.25 -1.50 × 10° with a corrected distance visual acuity of 20/40. The front corneal flat and steep values were decreased by 6.20 and 5.90 diopters (D), respectively, relative to corresponding preoperative values. The cornea remained clear throughout the follow-up., Conclusions: Implantation of an allogenic lenticule may be an option in the treatment of advanced ectasia. Purpose: To investigate re-innervation in the implanted lenticule, as well as changes to the cornea, after correcting hyperopia with an autologous implantation of a lenticule obtained through small incision lenticule extraction (SMILE)., Methods: This study retrospectively analysed re-innervation in the implanted lenticule, as well as microscopic morphological changes in the corneal architecture of the recipient cornea in five patients (with myopia in one eye and hyperopia in the contralateral eye) who received SMILE in the myopic eye and femtosecond laser in situ keratomileusis (FS-LASIK) combined with lenticule implantation in the contralateral hyperopic eye. Re-innervation in the implanted lenticule, as well as microscopic morphological changes in corneal architecture, were evaluated by in vivo confocal microscopy., Results: One patient was examined at postoperative week six, two were examined at postoperative month two, one was examined at postoperative month nine, and one was examined at postoperative month 12. Partial subbasal nerve fibre regeneration was detected in two patients (Case 4 who was examined at postoperative month nine and Case 5 who was examined at postoperative month 12). Regenerated and branched nerve fibres were detected in the implanted lenticule in Case 5 who was examined at postoperative month 12. Both the anterior and posterior interfaces showed an absence or decrease of keratocytes and the presence of small particles of various brightnesses. Keratocytes in the implanted lenticule presented abnormal morphology in size and shape after surgery in all treated eyes, but showed partial morphological recovery in two patients (Case 4 who was examined at postoperative month nine and Case 5 who was examined at postoperative month 12)., Conclusions: These preliminary findings suggest that nerve fibres will regenerate into the implanted lenticule after autologous lenticule implantation. Background: To report a first case of late-onset diffuse lamellar keratitis (DLK) occurring 4 years after femtosecond laser-assisted small incision lenticule extraction (SMILE)., Case Presentation: A 41-year-old man who underwent SMILE 4 years prior developed DLK in the right eye 1 day after he was struck in the eye by a finger while playing with his son. Slim-lamp microscopy and anterior segment optical coherence tomography (AS-OCT) were used to evaluate the cornea of the right eye. Slit-lamp examination of the right eye revealed epithelial exfoliation and stage 3 DLK with diffuse, dot-like, granular haze in the interface between the cap and stromal bed. After intensive treatment with topical corticosteroids, the DLK resolved and corneal transparency was achieved., Conclusions: This case indicates that DLK can occur several years after SMILE. Ocular trauma may be a risk factor for the development of DLK. The prognosis is usually favorable with early diagnosis and treatment with topical corticosteroids.

Purpose: To compare the impact on corneal sensation after small incision lenticule extraction (SMILE) and femtosecond laser-assisted LASIK (femto-LASIK) in patients with myopia., Methods: In this prospective, nonrandomized comparative study, 71 subjects were enrolled. Thirty-eight eyes of 38 patients underwent SMILE and 33 eyes of 33 patients underwent femto-LASIK. Corneal sensation was tested with Cochet-Bonnet esthesiometry in five corneal areas preoperatively and at 1 week and 1, 3, and 6 months postoperatively. Comparison of corneal sensation was performed for the SMILE and femto-LASIK groups. Additionally, the correlations were evaluated between the postoperative corneal sensation, preoperative spherical equivalent, and ablation depth., Results: All tested areas within the cap or flap demonstrated corneal hypoesthesia immediately after both surgeries. SMILE-treated eyes showed less compromised corneal sensation than femto-LASIK-treated eyes at all postoperative visits in the central, inferior, nasal, and temporal areas at the 1-week and 1-month visits. In the SMILE group, the inferior, nasal, and temporal quadrants recovered faster than other areas. In the femto-LASIK group, the sensation over the flap did not recover to preoperative levels by postoperative 6 months. There was no correlation between postoperative corneal sensation, preoperative spherical equivalent, and ablation depth in both groups., Conclusions: The impairment of corneal sensation was less significant in the SMILE group than in the femto-LASIK group and was independent of preoperative spherical equivalent or ablation depth., (Copyright 2014, SLACK Incorporated.)

Purpose: To evaluate corneal reinnervation, and the corresponding corneal sensitivity and keratocyte density after small incision lenticule extraction (SMILE) and femtosecond laser in situ keratomileusis (FS-LASIK)., Methods: In this prospective, non-randomized observational study, 18 patients (32 eyes) received SMILE surgery, and 22 patients (42 eyes) received FS-LASIK surgery to correct myopia. The corneal subbasal nerve density and microscopic morphological changes in corneal architecture were evaluated by confocal microscopy prior to surgery and at 1 week, 1 month, 3 months, and 6 months after surgery. A correlation analysis was performed between subbasal corneal nerve density and the corresponding keratocyte density and corneal sensitivity., Results: The decrease in subbasal nerve density was less severe in SMILE-treated eyes than in FS-LASIK-treated eyes at 1 week (P = 0.0147), 1 month (P = 0.0243), and 3 months (P = 0.0498), but no difference was detected at the 6-month visit (P = 0.5277). The subbasal nerve density correlated positively with central corneal sensitivity in both groups (r = 0.416, P<0.0001, and r = 0.2567, P = 0.0038 for SMILE group and FS-LASIK group, respectively). The SMILE-treated eyes have a lower risk of developing peripheral empty space with epithelial cells filling in (P = 0.0005)., Conclusions: The decrease in subbasal nerve fiber density was less severe in the SMILE group than the FS-LASIK group in the first 3 months following the surgeries. The subbasal nerve density was correlated with central corneal sensitivity.

Purpose: To investigate the changes in dry eye symptoms and clinical signs and corneal sensitivity after small incision lenticule extraction (SMILE) and femtosecond LASIK (femto-LASIK)., Design: Prospective, non-randomized comparative study., Methods: The study included a total of 71 eyes of 71 patients; the SMILE group comprised 38 eyes of 38 patients, and the femto-LASIK group comprised 33 eyes of 33 patients. Ocular Surface Disease Index (OSDI), Tear film breakup time (TBUT), the Schirmer test without anesthesia (S1T), corneal fluorescein staining, and central corneal sensation were evaluated before surgery and at 1 week, 1 month, 3 months, and 6 months after surgery., Results: OSDI scores in both groups were increased immediately and returned to preoperative level at 1 month after surgeries. The TBUT values in both groups were reduced after surgeries relative to their preoperative scores. Patients in SMILE group were less likely to have corneal staining compared with those in the femto-LASIK group ([odds ratio] OR = 0.50, 95% [confidence interval] CI 0.28 to 0.93, P = 0.03). Central corneal sensitivity was decreased at all postoperative time points in both groups. However, the central corneal sensation scores in the SMILE group were greater than that in the femto-LASIK group at all of the postoperative time points (all P<0.05)., Conclusions: SMILE surgeries resulted in a short-term increase in dry eye symptoms, tear film instability, and loss of corneal sensitivity. Furthermore, SMILE surgeries have superiority over femto-LASIK in lower risk of postoperative corneal staining and less reduction of corneal sensation.

Purpose: To study microdistortions in Bowman's layer after femtosecond laser small incision lenticule extraction (SMILE) using Fourier-domain optical coherence tomography (OCT) and to investigate possible sources and potential visual impacts., Methods: A nonrandomized controlled prospective study enrolled 52 eyes of 29 consecutive patients undergoing SMILE, with spherical equivalent of -6.33 ± 1.88 diopters and 4.2 mm superior incision. The microdistortions in Bowman’s layer were counted at 1 day, 1 week, and 1 month postoperatively and at long-term follow-up. Another 38 eyes of 20 patients undergoing femtosecond laser-assisted LASIK (FS-LASIK) were examined at 1 day and long-term postoperatively as the control group., Results: Microdistortions in Bowman’s layer were observed by OCT, with no clinically significant corneal striae under slit-lamp microscopy, in 46 eyes (88.5%) on day 1 after SMILE and in 16 eyes (42.1%) after FS-LASIK, with more in SMILE eyes than in FS-LASIK eyes. The amount decreased at 1 week and then remained stable. There were more microdistortions in the center area than in the periphery in both groups. Microdistortions were more in the inferior than the superior quadrant on the first day after SMILE but there was no difference at long-term follow-up. Microdistortions after SMILE were associated with the refractive lenticule thickness and surgery order but had no significant impact on long-term visual outcomes or wavefront aberrations., Conclusions: Microdistortions in Bowman's layer after SMILE were associated with the refractive lenticule thickness and surgery order. The microdistortions remained stable after 1 week and had no impact on long-term visual performance.
