Background: To date, no research has focused on the sonographic quantification of the degree of flexion of the fetal head in relation to the labor outcome in women with protracted active phase of labor., Objective: This study aimed to assess the relationship between the transabdominal sonographic indices of fetal head flexion and the mode of delivery in women with protracted active phase of labor., Study Design: Prospective evaluation of women with protracted active phase of labor recruited across 3 tertiary maternity units. Eligible cases were submitted to transabdominal ultrasound for the evaluation of the fetal head position and flexion, which was measured by means of the occiput-spine angle in fetuses in nonocciput posterior position and by means of the chin-to-chest angle in fetuses in occiput posterior position. The occiput-spine angle and the chin-to-chest angle were compared between women who had vaginal delivery and those who had cesarean delivery. Cases where obstetrical intervention was performed solely based on suspected fetal distress were excluded., Results: A total of 129 women were included, of whom 43 (33.3%) had occiput posterior position. Spontaneous vaginal delivery, instrumental delivery, and cesarean delivery were recorded in 66 (51.2%), 17 (13.1%), and 46 (35.7%) cases, respectively. A wider occiput-spine angle was measured in women who had vaginal delivery compared with those submitted to cesarean delivery owing to labor dystocia (126±14 vs 115±24; P<.01). At the receiver operating characteristic curve, the area under the curve was 0.675 (95% confidence interval, 0.538-0.812; P<.01), and the optimal occiput-spine angle cutoff value discriminating between cases of vaginal delivery and those delivered by cesarean delivery was 109°. A narrower chin-to-chest angle was measured in cases who had vaginal delivery compared with those undergoing cesarean delivery (27±33 vs 56±28 degrees; P<.01). The area under the curve of the chin-to-chest angle in relation to the mode of delivery was 0.758 (95% confidence interval, 0.612-0.904; P<.01), and the optimal cutoff value discriminating between vaginal delivery and cesarean delivery was 33.0°., Conclusion: In women with protracted active phase of labor, the sonographic demonstration of fetal head deflexion in occiput posterior and in nonocciput posterior fetuses is associated with an increased incidence of cesarean delivery owing to labor dystocia. Objective: Compare the issue of an operative vaginal delivery associated to the fetal presentation diagnosed by transperineal ultrasound. Three groups were formed: ≥55 mm, between 45 and 55 mm, and <45 mm., Methods: A monocentric prospective study on 108 patients has been conducted between April 2011 and August 2014. The distance between perinea and skull has been analyzed to compare the success of operative vaginal delivery considering the level of the fetal presentation in the pelvic cavity., Results: The failed operative vaginal deliveries are more frequent while the fetal head is above 55 mm (16.7%) or while the fetal head is between 45 and 55 mm (9.1%) than while the fetal skull is under 45 mm (1.8%) (P=0.04). However there is no significant difference for the fetal shoulder dystocia (5.6% vs 3.0% vs 3.5%, P=0.5), nor for the newborn outcomes (16.7 vs 15.2 vs 14; P=0.9)., Conclusion: Despite the high rate of failed operative vaginal delivery above 55 mm, it should be considered not to prohibit but send free to the obstetrician appreciation., (Copyright © 2015 Elsevier Masson SAS. Shoulder dystocia is one of the most feared obstetric emergencies due to related maternal and neonatal complications and therefore, the growing of medico-legal litigation that it entails. Although associated with risk factors such as fetal macrossomia, gestacional diabetes and instrumented delivery, the majority of cases are unpredictable. The lack of a consensus on shoulder dystocia diagnosis causes variations on its incidence and hampers a more comprehensive analysis. Management guidelines described for its resolution include several manoeuvres but the ideal sequence of procedures is not clearly defined in more severe cases. Hands-on and team training, through simulation-based techniques applied to medicine, seems to be a promising method to learn how to deal with shoulder dystocia having in mind a reduction in related maternal or neonatal morbidity and mortality. The main goal of this paper is to provide a comprehensive revision of shoulder dystocia highlighting its relevance as an obstetric emergency. A reflection on the management is presented emphasising the importance of simulation-based training.

Shoulder dystocia is defined as a complication of vaginal delivery that requires additional obstetric manoeuvres to release the shoulders of the baby. Reported incidence of shoulder dystocia is around 0.6-1.4%. Serious perinatal morbidity is associated with this type of complication. Brachial plexus injuries are one of the most important fetal complications of shoulder dystocia. Some cases results in permanent brachial plexus dysfunction. A high level of awareness and training for all birth attendants is recommended. The purpose of this article is to review the current evidence regarding the possible prediction and management of shoulder dystocia.

Shoulder dystocia is an unpredictable obstetric complication with the incidence of 0.15% to 2%. An increase in the incidence of shoulder dystocia has been recorded over the last 20 years, probably just because it has now been regularly registered at maternity wards as an obstetric complication. The risk factors for shoulder dystocia include fetal macrosomia, fetal malformations and tumors, maternal adiposity, excessive weight gain during pregnancy, diabetes mellitus, pathologic pelvis, multiparity, short maternal stature, advanced maternal age, postterm pregnancy, so-called midforceps delivery or vacuum extraction, prolonged delivery stage II, oxytocin labor induction, premature fetal expression according to Kristeller, and previous shoulder dystocia in macrosomatic children. The sequels of shoulder dystocia and obstetric maneuvers for incarcerated shoulder release include clavicular fracture, brachial plexus lesions, sternocleidomastoid muscle distension with or without hematoma, diaphragmatic paralysis, Horner's syndrome, peripartal asphyxia and consequential cerebral lesions (cerebral palsy), and peripartal death. Maternal complications due to shoulder dystocia are postpartal hemorrhage, cervical and vaginal lacerations, frequent infections during the puerperium, symphysiolysis and rupture of the uterus, and secondary cesarean section with related complications due to unsuccessful obstetric procedures or as continuation of Zavanelli's maneuver. McRoberts' maneuver (or Gaskin maneuver) is recommended as the initial procedure for shoulder release in case of shoulder dystocia. If it fails, other obstetric procedures such as Resnik's suprapubic pressure and Woods' grip with posteriorly placed arm release should be used, always with gross lateral episiotomy. The performance of all these obstetric procedures requires skilfull and highly experienced obstetrician and obstetric team as a whole.

The result of a literature survey on shoulder dystocia from year 1609 to 1860 are presented. Operative maneuvers for the resolution of dystocia were mainly examined, as well as the development of knowledge about the pathogenetic mechanism of this serious complication.

The Zavanelli maneuver is the manual replacement of a partially-born fetus due to severe shoulder dystocia. It is described in obstetrical textbooks as being among the last to be tried in a series of maneuvers to rescue the fetus with severe shoulder dystocia, as it is considered a very difficult and heroic maneuver. Few obstetricians have seen it and fewer have done it themselves. It is even more rare when a single obstetrician has done the Zavanelli maneuver repeatedly. Therefore, both experienced obstetricians and certainly young residents are fearful when they have to use this maneuver and can lose control in cases of shoulder dystocia. We have found descriptions of 93 cases of use of the Zavanelli maneuver in vertex presentations. We also describe a recent case in our experience. We conclude that this maneuver is safe and not too difficult to perform even without previous experience. Fetal and maternal complications are few, but there is of course a bias against reporting bad results. We recommend that every obstetrician become familiar with this maneuver so as to feel sure that it is safe for him to use in severe cases of shoulder dystocia.

Objective: To evaluate the applicability and safety of the Zavanelli maneuver (manual return to the vagina of the partially born but undeliverable fetus with subsequent extraction by cesarean)., Data Sources: MEDLINE computer database, Science Citation Index, and Index Medicus, 1985 through 1997 using the search terms Zavanelli maneuver, cephalic replacement, abdominal rescue, head entrapment, and shoulder dystocia., Method of Study Selection: Efforts to return 103 partially born but undeliverable fetuses to the uterus have been reported in 30 publications since the first case was reported in 1985. All cases were accepted for review, successful or not., Tabulation, Integration, and Results: Clinically relevant features were extracted from each case report. In 92 cases, fetuses presented by the vertex. Cephalic replacement was successful in 84 of those. In 11 cases, the fetus presented by the breech. Podalic replacement was successful in all of those. Seven women suffered tissue-disruptive trauma. There were no maternal deaths or fetal injuries ascribed to the maneuver., Conclusion: The 92% overall success rate is remarkable for a new procedure; even more noteworthy, in most cases it was applied by operators with no experience with it, and for cephalic replacement, it was used only after conventional maneuvers had failed. Using it early in the treatment of obstructed partial vaginal delivery is recommended.

Objective: Our goal was to examine and compare clinician-applied loads during simulated vaginal delivery., Study Design: We developed a birthing model and a microcomputer data acquisition system and used them to measure clinician-applied extraction forces, moment, and rates for three perceived categories of delivery. In 39 experiments, clinicians simulated delivery of the fetal shoulders during vaginal delivery for routine, difficult, and shoulder dystocia deliveries., Results: Clinicians averaged 84 N combined force and 473 N-cm neck-bending moment for routine deliveries, 122 N and 697 N-cm for difficult deliveries, and 163 N and 700 N-cm for shoulder dystocia deliveries (p < 0.002). No force or moment parameter was associated with clinician gender or experience. Force levels exceeding 100 N are reached for many clinicians (74% and 82%) for difficult and shoulder dystocia deliveries and for some clinicians (31%) for routine deliveries (p < 0.0001)., Conclusion: We conclude that simulating shoulder dystocia in the laboratory may be useful in measuring extraction forces and neck-bending moment and that birthing models can be used to train clinicians in force, moment, and rate perception.

Shoulder dystocia is a rare obstetric emergency with which few clinicians have adequate experience. A case which was successfully managed is presented and current management procedures reviewed.

Shoulder dystocia occurred in 98 women (0.9%) among 10,695 deliveries during the period of 1986-1988. The most frequent cause was overweight and strangulated umbilical cord. The following maneuvers were used: pressure on the uterine fundus, strong flexion of legs (maneuver of Knebel-Macroberts-Korokawa) and in 12 cases--extraction of the posterior hand. The head should be rotated to the opposite thigh of the parturient in cases with incorrect rotation of the head--the most frequent cause for low shoulder dystocia. Prophylactic measures are of special significance: cesarean section in women with large fetus, estimated by ultrasound, or with fetus with thoracic diameter larger than biparietal diameter with 1.5 sm. The delivery should be managed by the most experienced obstetricians and should not be protracted or precipitate without indications.
