Purpose: To investigate differences in the objective and subjective visual quality of patients with high myopia following implantable collamer lens V4c (ICL V4c) versus small incision lenticule extraction (SMILE) early postoperatively., Methods: This prospective analysis included 94 eyes of 57 patients (8 males) in the ICL V4c group (mean spherical equivalent (SE), -8.07 ± 1.03 dioptres (D)) and 103 eyes of 57 patients (13 males) in the SMILE group (mean SE, -7.85 ± 0.78 D). Refractive parameters and wavefront aberrations were recorded preoperatively and 6 months postoperatively. Subjective visual quality was scored with the Quality of Vision questionnaire, and operation satisfaction was evaluated 6 months postoperatively., Results: At 6 months postoperatively, the efficacy index was 1.17 ± 0.16 in the ICL and 1.09 ± 0.20 in the SMILE group; the safety index was 1.21 ± 0.18 in the ICL and 1.12 ± 0.16 in the SMILE group. A significant increase in total higher-order aberrations (HOAs) in both the ICL and SMILE groups was observed, but to a lesser degree in ICL (p < 0.05). In the ICL group, trefoil significantly increased (p < 0.05), whereas in the SMILE group, coma, trefoil and spherical aberration significantly increased (p < 0.05). Glare, haloes and blurred vision had the highest incidences after both types of surgery, and postoperative haloes were significantly more common following ICL; 96.1% of ICL-treated and 94.9% of SMILE-treated patients were satisfied with their visual outcomes., Conclusion: ICL V4c caused lower HOAs induction than SMILE. Objective: To evaluate the features of corneal nerve destruction after small incision lenticule extraction (SMILE) with different cap thicknesses (100 and 120 μm)., Methods: Nine consecutive patients (18 eyes) received SMILE with a 120-μm cap thickness and nine patients (18 eyes) with a 100-μm cap thickness were recruited. Confocal microscopy was used to evaluate the density and microscopic morphological changes of central corneal subbasal nerves preoperatively and at postoperative week 1., Results: The postoperative corneal subbasal nerve densities decreased significantly in both 120 μm and 100 μm groups. No statistical difference was detected in reduction of subbasal nerve density between two groups (P = 0.299). The number of subbasal nerve fibers significantly decreased in both groups. The reductions were not significantly different between two groups (P = 0.293)., Conclusions: Using a 120-μm cap thickness during SMILE preserves no more central corneal subbasal nerve fibers compared to a 100-μm cap thickness.

Purpose: The aim of this study was to compare visual outcomes after small incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for high myopia., Methods: In this prospective, comparative study, a total of 52 eyes of 34 consecutive highly myopic patients with spherical equivalent within the range of -8.00 to -10.00 diopters were recruited. Twenty-three eyes of 16 patients were in the FS-LASIK group and 29 eyes of 18 patients were in the SMILE group. Visual outcomes and wavefront aberrations were analyzed preoperatively and 6 months postoperatively., Results: At the postoperative 6-month visit, 96.6% in the SMILE group and 91.3% in the FS-LASIK group achieved unchanged or better corrected distance visual acuity (CDVA). As for uncorrected distance visual acuity (UDVA), 96.6% in the SMILE group and 95.7% in the FS-LASIK group achieved UDVA of 20/20 or better. As for wavefront aberrations, high-order aberrations (HOAs) and spherical aberrations increased significantly after surgery in both groups relative to corresponding preoperative values (p < 0.001), and vertical coma increased after SMILE (p < 0.001). No statistically significant differences in changes of HOAs (p =0.90), spherical aberrations (p = 0.07), horizontal coma (p = 0.56), coma (p =0.08), horizontal trefoil (p =0.19), vertical trefoil (p = 0.90), and trefoil (p = 0.45) were detected between the 2 groups, except for vertical coma (p < 0.01)., Conclusions: SMILE is as effective as FS-LASIK in correcting high myopia, but attention should be paid to the induction of vertical coma in highly myopic patients following a SMILE procedure., (© 2019 S. Background: This study compares the clinical outcomes of femtosecond laser small-incision lenticule extraction (SMILE) for the correction of myopia and myopic astigmatism greater than - 10 D, and - 10 D or less respectively., Methods: 60 eyes/patients were equally selected into group 1 (myopia and myopic astigmatism of - 10 D or less) and group 2 (myopia and myopic astigmatism of over - 10 D), both of which were treated with SMILE. Visual and refractive outcomes, corneal higher-order aberrations, and Bowman's layer micro-distortions were evaluated preoperatively, 3 months, and 6 months postoperatively., Results: LogMAR corrected distance visual acuity (CDVA) of group 1 and group 2 was - 0.069 ± 0.047 and - 0.053 ± 0.073 6 months postoperatively (P = 0.48). 100% eyes in group 1 and 97% in group 2 were within 1 D of targeted correction (P = 0.45). Meanwhile, 100% eyes in group 1 and 97% in group 2 had an uncorrected distance visual acuity of 20/25 or better (P = 0.20). Changes in corneal higher-order aberrations root mean square, coma, and trefoil were similar between the two groups but spherical aberration was higher in group 2 (P < 0.01). Micro-distortions were observed in 53% in group 1 and 77% in group 2. More micro-distortions were observed in group 2 (3.40 ± 2.66) than in group 1 (2.07 ± 2.29) (P = 0.041). The total number of micro-distortions was not correlated with postoperative CDVA (P = 0.77)., Conclusions: Visual outcomes showed similar results of SMILE for myopic correction of > - 10 D and ≤ - 10 D. Refractive outcomes showed slightly under-correction in higher myopic eyes. Higher myopic treatment tends to induce more spherical aberrations. Micro-distortions had no impact in visual and refractive outcomes.

Purpose: To compare visual outcomes and aberration outcomes in small incision lenticule extraction (SMILE) versus femtosecond laser LASIK (FS-LASIK)., Methods: This prospective, comparative, nonrandomized clinical study included 68 eyes of 37 patients receiving SMILE and 55 eyes of 30 patients receiving FS-LASIK between December 2011 and January 2013 at the Fudan University Eye and ENT Hospital (Shanghai, People's Republic of China). Patients were followed up at 3, 6 months and 5 years after surgery. Main outcome measurements included uncorrected (UDVA) and corrected (CDVA) distance visual acuity, manifest refraction, central corneal thickness (CCT), total corneal refractive power (TCRP) and wavefront aberrations., Results: About 98% (49/50) of treated eyes in the SMILE group and 95% (39/41) in the FS-LASIK group had a postoperative logMAR UDVA of 0 or better. Spherical equivalent (SE) after 5 years was -0.01 ± 0.35 D in the SMILE group and -0.23 ± 0.41 D in the FS-LASIK group. A regression of -0.02 ± 0.39 D in the SMILE group and -0.12 ± 0.32 D in the FS-LASIK group was observed between 6 months and 5 years postoperative time-points. TCRP increased by 0.39 ± 0.38 D in the SMILE group and 0.45 ± 0.49 D in the FS-LASIK group between 6-month and 5-year time-points. However, no statistically significant difference was found between the SMILE and FS-LASIK groups in terms of ∆SE, ∆CCT and ∆TCRP between 6 months and 5 years postoperative time-points., Conclusion: Myopic regression was observed in terms of TCRP but not in subjective refraction. Background: To report a first case of lenticule addition and corneal cross-linking for post-LASIK ectasia with associated corneal thinning., Case Presentation: Lenticule addition followed by corneal cross-linking was performed on the left eye of a patient with post-LASIK ectasia. Postoperatively, the corneal power and elevation were stable with a remarkable improvement in corneal thickness, and the lenticule had merged with the adjacent corneal stroma at 30 months follow-up. The patient's corrected distance visual acuity gained two lines., Conclusions: This case provides a potential treatment option for patients with keratectasia and keratoconus in association with thin corneal thickness (less than 400 μm) and may provide the benefit of delaying or avoiding the need for keratoplasty, which has its own associated complications and is limited by the availability of donor corneas.

Objective: To investigate the factors associated with optical and visual quality of advanced surface ablation in high myopia., Design: A cross-sectional study of high myopic eyes treated with laser epithelial keratomileusis (LASEK)/epipolis laser in situ keratomileusis (Epi-LASIK). SETTING6: Eye and ENT Hospital of Fudan University in Shanghai., Methods: One hundred and thirty-eight high myopic eyes (138 patients) (myopia -6 D or more) were examined more than 12 months after LASEK or Epi-LASIK with advanced surface ablation on the MEL 80 excimer laser (Zeiss AG, Jena, Germany). Refraction, higher order aberrations (HOAs) and contrast sensitivity before and after surgery were evaluated. Factors including preoperative refraction, age, gender, central corneal thickness, pupil size, optical diameter, ablation depth and flap creation method were analysed for association with postoperative high-order aberration, contrast and glare sensitivities, and different analytic diameters., Results: HOAs increased significantly postoperatively (p<0.05), with the most significant change found in Z(spherical aberration). At a 5 mm analysis diameter, increased coma was associated with age; increased spherical aberration difference was associated with age, optical zone diameter and method of epithelial flap creation. At a 3 mm analysis diameter, none of the factors contributed to changes in HOAs. Higher preoperative refractive error was associated with decreased contrast and glare sensitivity at each spatial frequency., Conclusion: A larger optical zone diameter design is recommended to achieve better visual quality in advanced surface ablation for high myopia correction. Purpose: To describe a case in which post-LASIK corneal ectasia was treated using an allogeneic lenticule implanted into the recipient eye., Methods: A 29-year-old man presented with bilateral ectasia 10 years after LASIK. Manifest refraction was -1.50 -3.25 × 15° in the right eye and -13.50 -6.00 × 10° in the left eye. Because of contact lens intolerance in the left eye, the authors decided to implant an allogenic corneal lenticule, created by small incision lenticule extraction with the VisuMax laser (Carl Zeiss Meditec, Jena, Germany) in a donor eye with a refraction of +5.75 -0.50 × 15°. The intended peripheral thickness was 116 μm and the central thickness was 20 μm. The lenticule was placed under the original LASIK flap in the left eye., Results: Ten months after surgery, refraction of the left eye had improved to -3.25 -1.50 × 10° with a corrected distance visual acuity of 20/40. The front corneal flat and steep values were decreased by 6.20 and 5.90 diopters (D), respectively, relative to corresponding preoperative values. The cornea remained clear throughout the follow-up., Conclusions: Implantation of an allogenic lenticule may be an option in the treatment of advanced ectasia. Purpose: To investigate early visual and refractive outcomes, corneal stability and optical quality after femtosecond laser small-incision lenticule extraction (SMILE) for treating myopia and myopic astigmatism over -10 D., Methods: Thirty eyes (30 patients) with myopia and myopic astigmatism of over -10 D were treated with VisuMax ® femtosecond laser (version 3.0; Carl Zeiss Meditec AG, Jena, Germany). Six months postoperative safety, efficacy and predictability were evaluated. Corneal Scheimpflug topography was measured preoperatively, 1 day, 3 months and 6 months postoperatively. Wavefront aberrations were measured preoperatively, 3 months and 6 months postoperatively., Results: Six months postoperatively, LogMAR uncorrected and corrected distance visual acuity (CDVA) were -0.013 ± 0.086 and -0.073 ± 0.069, respectively. 73% (97%) of eyes were within 0.5 (1) D of target refraction. No eyes lost CDVA, 43% (13 eyes) gained one line and 7% (two eyes) gained two lines. Mean corneal back curvature (KMB) and posterior central elevation (PCE) did not change significantly comparing preoperative and 6 months postoperative data (p = 0.91 and 0.77, respectively). Comparing 1 day with 6 months postoperative data, central corneal thickness (CCT), mean corneal front curvature (KMF), KMB and PCE did not change significantly (p = 0.27, 0.07, 0.52, 0.71, respectively). Total higher-order aberration (HOA), spherical aberration and coma increased significantly (p < 0.01) but trefoil remained stable (p = 0.49)., Conclusion: Our results indicate that SMILE can correct myopia and myopic astigmatism of over -10 D predictably. Purpose: To evaluate the surface characteristics of lenticules extracted by continuous curvilinear lenticulerrhexis (CCL) in small incision lenticule extraction., Methods: Twelve eyes treated with small incision lenticule extraction were included in the study and divided into two groups: the CCL and traditional groups. The extracted lenticules were examined with scanning electron microscopy. Both the anterior and the posterior surfaces of the lenticules were accessed. A scoring system was used to evaluate surface characteristics of the lenticules with a full mark of 16., Results: All of the lenticules in the CCL group exhibited smoothness and regularity on both sides and received scores of 14 or 15. The average score of anterior surfaces was 14.83 ± 0.41 for the CCL group and 14.00 ± 2.45 for the traditional group. The average score for the posterior surfaces was 14.50 ± 0.55 for the CCL group and 14.83 ± 0.41 for the traditional group., Conclusions: The surfaces of lenticules extracted by the CCL technique in small incision lenticule extraction are of good quality., (Copyright 2015, SLACK Incorporated.)

Purpose: To quantitatively analyze the characteristics of Bowman's layer microdistortions after femtosecond laser small incision lenticule extraction (SMILE) among different myopic corrections, and their correlations with stromal lenticule thickness., Methods: In this nonrandomized, prospective, controlled study, 63 eyes of 32 consecutive patients undergoing SMILE were divided into three groups according to the preoperative spherical equivalent (SE): greater than -6.00 D (20 eyes), -3.00 to -6.00 D (26 eyes), and -3.00 D or less (17 eyes). Measurement by slit-lamp microscope, uncorrected distance visual acuity (UDVA), manifest refraction, and Fourier-domain optical coherence tomography (FD-OCT) were conducted preoperatively and 1 day and 1 month postoperatively. Index M was calculated according to the OCT images to indicate the quantity of microdistortions in Bowman's layer., Results: Microdistortions in Bowman's layer were observed in 65.0% (13 of 20 eyes) and 30.8% (8 of 26 eyes) in the greater than -6.00 D and -3.00 to -6.00 D groups, respectively, 1 day postoperatively, and in 60% (12 of 20 eyes) and 23.1% (6 of 26 eyes) 1 month later. No Bowman's layer microdistortion was detected in the -3.00 D or less group postoperatively. The quantitative index M of microdistortions was higher in the greater than -6.00 D group than in the -3.00 to -6.00 D group at 1 day and 1 month (P < .05). There was no statistically significant difference between the indexes M on 1 day compared to 1 month. Index M was correlated with the preoperative SE and lenticule thickness (P < .05)., Conclusion: The incidence and range of microdistortions in Bowman's layer detected after SMILE are positively correlated with the myopic correction. Purpose: To develop and evaluate the new continuous curvilinear lenticulerrhexis (CCL) technique for small incision lenticule extraction., Methods: Thirty-one eyes of 20 patients with myopia were included in the study. The CCL technique for lenticule extraction was developed and used in 16 eyes of 10 patients (CCL group). The traditional technique of lenticule extraction was used in 15 eyes of 10 patients (traditional group). Uncorrected and corrected distance visual acuity, manifest refraction, lenticule quality, the duration of the extraction procedure, microdistorsions in Bowman's layer under optical coherence tomography (Optovue OCT; Optovue, Inc., Fremont, CA), and adverse events were evaluated at 1 day and 1 month postoperatively., Results: At the first postoperative month, all eyes in both groups had an uncorrected distance visual acuity of 1.0 or better. The safety indices were 1.12 and 1.09 for the CCL and traditional groups, respectively. The efficacy indices were 1.06 and 1.09 for the CCL and traditional groups, respectively. All lenticules in the CCL group had intact, round margins, whereas one lenticule in the traditional group had a microdefect margin. There was no statistically significant difference in duration of lenticule extraction or microdistorsions in Bowman's layer. No eyes in the CCL group developed diffuse lamellar keratitis, as compared to one eye in the traditional group., Conclusions: The CCL technique exhibited excellent safety and efficacy for myopia correction, suggesting CCL is a promising technique that merits further development and study., (Copyright 2015, SLACK Incorporated.)

Background: The novel Femtosecond lenticule extraction (FLEx) procedure has been considered safe, predictable, and effective in treating myopia and myopic astigmatism, with few complications. However, an enhancement procedure after FLEx may be required in some cases, but has not been reported in detail., Case Presentation: A 24-year-old woman who had undergone bilateral FLEx with the VisuMax femtosecond laser treatment for myopic astigmatism complained of double vision in her left eye after the operation. The manifest refraction was -0.50/-1.25 × 180°. The corneal topography showed a central-inferior steepened zone. The ocular wavefront measurements displayed a high value of total aberrations as well as coma. She was scheduled for an enhancement procedure and it was performed by relifting the primary FLEx flap in the left eye four months later. Ablation was made with the Mel-80 excimer laser. After retreatment, the corresponding aberrations were diminished and the corneal topography turned flattened. Her symptom resolved completely with good visual outcomes., Conclusion: This first detailed case report demonstrates the feasibility and efficacy of enhancement after FLEx for visual symptomatic eye after myopia correction. An analysis of more cases would be necessary to determine a more definite profile.

Purpose: To investigate objective optical quality and changes in light scatter after femtosecond laser small incision lenticule extraction (SMILE) for moderate to high myopia correction., Methods: Prospective clinical study of 66 eyes of 66 patients (23 males, 43 females) with a mean age of 28.67 ± 6.62 years, mean spherical refraction of -6.06 ± 1.57 diopters, and mean cylindrical refraction of -0.68 ± 0.46 diopters. Every patient had a 3-month follow-up after a standard SMILE procedure. An optical quality analysis system was used to measure modulation transfer function cutoff frequency (MTFcutoff), Strehl2D ratio, and objective scatter index (OSI)., Results: At 3 months postoperatively, mean spherical equivalent was 0.03 ± 0.24 diopters and corrected distance visual acuities of all patients were equal to or better than preoperative values, with a mean safety index of 1.12 ± 0.17 (range: 0.80 to 1.50) and mean efficacy index of 1.18 ± 0.21 (range: 0.80 to 1.50). Mean OSI increased from 0.75 preoperatively to 1.09 at 20 days postoperatively (P < .05), and gradually declined to 0.94 at 40 days and 0.82 at 3 months postoperatively (P > .05). MTFcutoff and Strehl2D ratio did not change significantly compared with preoperative values (P > .05). Multiple linear regression analysis revealed that preoperative OSI was significantly correlated with optical quality at 3 months. Patients with less preoperative OSI tended to acquire higher MTFcutoff (b = -8.61) and lower OSI (b = 0.65) (P < .05)., Conclusions: SMILE had little impact on retinal image quality after myopia correction. Purpose: To compare the impact on corneal sensation after small incision lenticule extraction (SMILE) and femtosecond laser-assisted LASIK (femto-LASIK) in patients with myopia., Methods: In this prospective, nonrandomized comparative study, 71 subjects were enrolled. Thirty-eight eyes of 38 patients underwent SMILE and 33 eyes of 33 patients underwent femto-LASIK. Corneal sensation was tested with Cochet-Bonnet esthesiometry in five corneal areas preoperatively and at 1 week and 1, 3, and 6 months postoperatively. Comparison of corneal sensation was performed for the SMILE and femto-LASIK groups. Additionally, the correlations were evaluated between the postoperative corneal sensation, preoperative spherical equivalent, and ablation depth., Results: All tested areas within the cap or flap demonstrated corneal hypoesthesia immediately after both surgeries. SMILE-treated eyes showed less compromised corneal sensation than femto-LASIK-treated eyes at all postoperative visits in the central, inferior, nasal, and temporal areas at the 1-week and 1-month visits. In the SMILE group, the inferior, nasal, and temporal quadrants recovered faster than other areas. In the femto-LASIK group, the sensation over the flap did not recover to preoperative levels by postoperative 6 months. There was no correlation between postoperative corneal sensation, preoperative spherical equivalent, and ablation depth in both groups., Conclusions: The impairment of corneal sensation was less significant in the SMILE group than in the femto-LASIK group and was independent of preoperative spherical equivalent or ablation depth., (Copyright 2014, SLACK Incorporated.)

Purpose: To investigate the changes in dry eye symptoms and clinical signs and corneal sensitivity after small incision lenticule extraction (SMILE) and femtosecond LASIK (femto-LASIK)., Design: Prospective, non-randomized comparative study., Methods: The study included a total of 71 eyes of 71 patients; the SMILE group comprised 38 eyes of 38 patients, and the femto-LASIK group comprised 33 eyes of 33 patients. Ocular Surface Disease Index (OSDI), Tear film breakup time (TBUT), the Schirmer test without anesthesia (S1T), corneal fluorescein staining, and central corneal sensation were evaluated before surgery and at 1 week, 1 month, 3 months, and 6 months after surgery., Results: OSDI scores in both groups were increased immediately and returned to preoperative level at 1 month after surgeries. The TBUT values in both groups were reduced after surgeries relative to their preoperative scores. Patients in SMILE group were less likely to have corneal staining compared with those in the femto-LASIK group ([odds ratio] OR = 0.50, 95% [confidence interval] CI 0.28 to 0.93, P = 0.03). Central corneal sensitivity was decreased at all postoperative time points in both groups. However, the central corneal sensation scores in the SMILE group were greater than that in the femto-LASIK group at all of the postoperative time points (all P<0.05)., Conclusions: SMILE surgeries resulted in a short-term increase in dry eye symptoms, tear film instability, and loss of corneal sensitivity. Furthermore, SMILE surgeries have superiority over femto-LASIK in lower risk of postoperative corneal staining and less reduction of corneal sensation.

Purpose: To study microdistortions in Bowman's layer after femtosecond laser small incision lenticule extraction (SMILE) using Fourier-domain optical coherence tomography (OCT) and to investigate possible sources and potential visual impacts., Methods: A nonrandomized controlled prospective study enrolled 52 eyes of 29 consecutive patients undergoing SMILE, with spherical equivalent of -6.33 ± 1.88 diopters and 4.2 mm superior incision. The microdistortions in Bowman’s layer were counted at 1 day, 1 week, and 1 month postoperatively and at long-term follow-up. Another 38 eyes of 20 patients undergoing femtosecond laser-assisted LASIK (FS-LASIK) were examined at 1 day and long-term postoperatively as the control group., Results: Microdistortions in Bowman’s layer were observed by OCT, with no clinically significant corneal striae under slit-lamp microscopy, in 46 eyes (88.5%) on day 1 after SMILE and in 16 eyes (42.1%) after FS-LASIK, with more in SMILE eyes than in FS-LASIK eyes. The amount decreased at 1 week and then remained stable. There were more microdistortions in the center area than in the periphery in both groups. Microdistortions were more in the inferior than the superior quadrant on the first day after SMILE but there was no difference at long-term follow-up. Microdistortions after SMILE were associated with the refractive lenticule thickness and surgery order but had no significant impact on long-term visual outcomes or wavefront aberrations., Conclusions: Microdistortions in Bowman's layer after SMILE were associated with the refractive lenticule thickness and surgery order. The microdistortions remained stable after 1 week and had no impact on long-term visual performance.

Purpose: To investigate the morphology of corneal caps in femtosecond laser small incision lenticule extraction (SMILE) and its relation to the refractive outcomes., Methods: A prospective study of fifty-four corneal caps created with VisuMax femtosecond laser were examined using an Fourier-domain optical coherence tomography at 1 day, 1 week, 1 month and 6 months after SMILE. The cap thickness at nine points on each of the four meridians (0°, 45°, 90°, 135°) and the diameter were measured. Cap morphology, changes over time and its correlation with refractive outcomes were assessed., Results: The mean achieved central cap thickness were (108.74±5.06) µm at 6 months and (107.32±4.81 ) µm at 1 month postoperatively, significantly thinner than that at 1 day (110.81±7.95) µm and 1 week (109.58±7.48 ) µm (P<0.05). The mean diameter on 0° meridian was (7.61±0.07) mm, significantly larger than that on 90° meridian (7.57±0.06) mm (P = 0.001). Cap morphology showed good regularity, except that the differences of points in two pairs were significant at 1 day postoperatively. The uniformity was consistent over time and the central cap thickness was thinner than those in the paracentral and peripheral areas. The refractive outcomes stabilized within 1 month. Uncorrected distance visual acuity (UDVA) was correlated to the central cap thickness at 1 day and 1 week (both rs  = 0.33, p<0.05). The uniformity index was correlated with UDVA (rs  = 0.34, p<0.05) and corrected distance visual acuity (rs  = 0.32, p<0.05) at 1 week postoperatively., Conclusions: Corneal caps of SMILE are predictable with good reproducibility, regularity and uniformity. Cap morphology might have a mild effect on refractive outcomes in the early stage. Further study should focus on the impact on the visual quality.
