Background & Purpose: One of the most important united nation Millennium Development Goals is to control Tuberculosis (TB). This study aimed to identify the high-risk areas of TB and other related factors of it in an epidemiological study. Materials & Methods: In this retrospective study, the records of 1566 TB infected patients in healthcare centers of 17 cities of Mazandaran University of Medical Sciences were investigated during 2009-2013 years. Information of patients was gathered using a check list. The collected data was then analyzed using chi-square. Results: The mean age of the patients was 46.17± 20.6 years. The 5-year incidence rate was 12.14 in 100,000 populations. Cities in the east of Province (Sari, Behshahr, and Neka) had highest incidence rate (about 16 cases in 100,000 population). The incidence rate was different based on gender as, 15.3 % of the men and 10.2 % of the women were infected. The median delay time in the diagnosis was 34 days. Conclusions: The incidence rate of TB in Mazandaran was found to be almost similar to the previous reported rate in Iran. High incidence rate in east of province may be due to bordering to the Golestan Province as the immigrant province. The short delay time of TB diagnosis during this period indicated the well health services practices in this province. More attention should be paid to high risk areas for identification and giving education to those patients.

Background and purpose: Disasters and traffic accidents as the leading causes of disability and death throughout the world are the most significant health problems which have usually been predictable and, therefore, possible to prevent. The present study, as the first attempt, was conducted to calculate the burden of life years lost due to early death caused by traffic accidents in Mazandaran Province. Materials and Methods: The current study was cross-sectional, and the data was collected from the center of Mazandaran Legal Medicine. The number of years of life lost due to premature death was calculated by using the instructions GBD2010 age and gender composition of the province was taken in the last census in 2012 from the Statistical Center. Then the SPSS Software was used to key in all the collected information in order to perform the analysis. Results: Of the total population in 2015, 729 deaths were recorded due to car accidents with 77.9 percent of them being male. Mean age was 43.07±21.18 and 44.67±23.34 in women. The number of years of life lost due to premature death was 24972.7 years in men, 6965.3 years in women, and the total of two genders was 31938 years (10.6 years per thousand people) which were calculated, and it was the highest in the age group ranging from 20 to 24 years old. Discussion: According to the high rate of deaths from traffic accidents and damages resulted from it, and in order to reduce these losses, it is necessary to take appropriate preventive measures.

Introduction: Heavy metals are one of the pollutants which are found in minor amounts in natural water and high concentration in industry waste water. Heavy metals poisoning effects are increasing in marine organisms especially fishes due to disposal of industrial wastes to marine environment and their bio accumulation. The aim of this study is to determine of mercury, cadmium and lead levels in canned fish sold in Mazandaran province of Iran. Materials and Methods: This descriptive-cross sectional study was done on 12 types of canned tuna distributed, 3 numbers of each type and overall 36 sample. The concentration of 3 type heavy metals assessed with standard method by Atomic Absorption and then expressed based on µg/g of wet weight. The results were compared with WHO and Iran National Standard Organization. Results: The results show that, minimum and maximum of mercury level in 36 samples of canned tuna fish was 0.01 and 0.5 µg/g, cadmium 0.001 and 0.05 and lead 0.001 and 0.3 respectively. According to replicates experiments, the mean concentration of mercury in 12 such samples of canned tuna fish was from 0.106 to 0.31, cadmium from 0.006 to 0.022 and lead from 0.03 to 0.15µg/g. that there was a significant difference in samples(p

Considering the high cultured efficiency of the social networks effective in purposeful directing the life of the users’ lifestyle by emergence and significant development of the social networks. Major changes have taken place in informatics process. The social networks can play role in the production and development of the science and the science as a managed educational program be used in the educational centers. This research investigates the effects of the social networks on the educational relationship among the students and universities. This cross sectional study was performed on 1000 students at 7 medical universities of Iran in 2015. The data collection tool was questionnaire comprised three sections with approved validity. The durability of the α-cronbach coefficient for second part of the questionnaire was 80% and of the third section on the questions with the Likert Criteria was 75%. The obtained data were analyzed by descriptive statistic method of ANOVA, Tukey and X2 using SPSS-19. In this study 940 subjects were under study, of them 85% had the daily use of social networks. Telegram had the highest usage among the users, 52% preferred the image in exchange of view. We found that 85% believed that social networks have effective role on the social relationships. Also 69% of the study subjects believe that the effects of the networks in learning insignificant materials are high. Referring to the obtained data of the present study, it is proposed that the universities use the social networks officially and managed. Establish groups with definite context and help in improvement of the scientific relation level, to be effective in the prospectiveness, creation and consent of the individuals from their scientific situation, in direct procedure.

Introduction: The prevalence of type 1 diabetes in all countries throughout the world is different. In this study, an introduction is, therefore, provided for the development of type 1 diabetes in children in Mazandaran province by using all available resources.Methods: In this descriptive cross-sectional study, the prevalence of patients with initial diagnosis of type 1 diabetes mellitus, aging from 6 months to 18 years, was located through contacting and referring to all organizations of insurers and doctors or experts in children endocrinologist and adolescent diseases in Mazandaran province.Results: 289 patients with type 1 diabetes were identified, 138 of whom were male (47.8%), and 151 were women (52.2%) with an overall prevalence of 0.04%, and the rate of this prevalence was 48 cases among 100000 people (50 cases among 100000 girls and 36 cases among 100000 boys). The mean age of the patients was 11.5 years old. In the study of delivery method, 180 patients (62.3%) were diagnosed with cesarean section, and 109 (37.7%) were born through natural delivery method. The mean age of onset of diabetes was 7 years. The minimum and maximum ages were 7 months and 18 years, respectively. 97 (33.6%) of the patients were also hospitalized for the first time due to ketoacidosis, and were diagnosed with diabetes.Conclusion: Type 1 diabetes in Mazandaran province was more prevalent than the similar study in southern Iran and neighboring countries. It is essential to pay greater attention to this disease through studying it in the other parts of the country..

Background and Purpose: One of the most important conceptual and thematic priorities in twenty-year visions of the country has been to develop scientifically, and achieve high scientific progress and excellence and the optimum position in the various global relations. Since scientific production is one of the most important missions of universities, the aim of this study was to review the scientific production of Mazandaran University of Medical Sciences, before and after the implementation of the strategic planning. Method: This descriptive-analytical study was conducted on six research centers at Mazandaran University of Medical Sciences. The current research adopted the census method, and the data was collected through definition data collection form, and all statistical analyses were carried out using SPSS (V20.0) through descriptive statistics and inferential statistical methods, such as Kolmogorov-Smirnov, t-test, and Chi-square. Result: The results showed that the total score improved after the implementation of strategic planning (Sig. = 0.0001). In addition, there was a meaningful relationship between the total scores before and after the implementation of strategic planning in clinical centers and non-clinical research centers (Sig.=0.000). Conclusion: The Strategic planning in research organizations can act as a driving force along with other factors for the progress in all areas of science, and it was found that achieving comprehensive development without proper utilization of research findings will undoubtedly be impossible. Hence, it is necessary for academic centers, and especially for universities of medical sciences, to prepare their current planning with environmental assessment, and regularly pursue strategic opportunities to fulfill the role of research and technology organizations in the knowledge-based economy. &nbsp

Background: Genetic polymorphisms of cytochrome p450 in humans are the main cause of differences in the metabolism. The allele and genotype frequencies of CYP2C19 and CYP2C9 have been studied in some Iranian populations. The aim of present study was to examine the frequencies of CYP2C18m1, and CYP2C18m2, alleles in the Mazandarani ethnic group among Iranian Population. Materials and Methods: In this study, genomic DNA was extracted from leucocytes of one hundred unrelated healthy volunteers. The prevalence of the common variants CYP2C18 m1 and m2 alleles were studied by using high fidelity polymerase chain reaction (HF-PCR) - DHPLC methods. Results: The frequency of CYP2C18 m1 and m2 alleles were 0.0% and 3.0%, respectively. CYP2C18 genotypes wt/wt, wt/m1, wt/m2, m1/m1, m1/m2, and m2/m2 frequencies were 97 %, 0.0 %, 3.0%, 0.0 %, 0.0%, and 0.0%, respectively. Conclusion: The result of the current study shows that impaired CYP2C18 activity in 03% of our sample population may decrease extra hepatic metabolism of some important drugs such as Phenytoin. It may also affect transdemal delivery of drug substrate for this isoenzyme.

CYP2C19*2 and CYP2C19*3 are the most frequent identified defective alleles in Orientals and Caucasian poor metabolizers (PM). The aim of this study was to investigate the frequencies of CYP2C19*1, CYP2C19*2 and CYP2C19*3 alleles and CYP2C19 genotypes among Mazandarani ethnic group among Iranian Population. METHODS: The study was conducted on 103 unrelated healthy volunteers. DNA was extracted from leucocytes and analyzed by the PCR-RFLP protocol. The PCR product was digested with restriction enzymes (SmaI and BamH1) and then separated electrophoretically using polyacrylamide gel. RESULTS: Of the tested alleles, CYP2C19*1, and CYP2C19*2, but not CYP2C19*3, were detected. The frequencies for CYP2C19 alleles *1, *2, and *3 were 91%, 9.0%, and 0.0%, respectively. CYP2C19 genotypes *1/*1, *1/*2, *1/*3, *2/*2, *2/*3 and *3/*3 frequencies were 84%, 14%, 0.0%, 2.0%, and 0.0%, respectively. CONCLUTION: The result of the present study shows that the two inactive alleles of CYP2C19 accounted for 9.0% of CYP2C19 alleles in our sample versus 8.8 40.1% reported in other populations. Objective(s): To study health-related quality of life of a general population over 40 years old in Mazandaran province, Iran. Methods: This was a cross-sectional study. A sample of the general population aged 40 and over were studied. Quality of life was measured using the Iranian version of SF-36. Data were analyzed in a descriptive fashion. Results: In all data from 1183 individuals were analyzed. Of these, 46.87 % rated their health as excellent or very good. There were significant differences in quality of life scores between males and females and among urban and rural populations. However, vitality score was higher in those who were living in rural areas. Conclusion: The finding suggest that there are inequalities in health status among different genders and urban and rural populations. The need for policies to reduce this seems necessary.

Background and Objective: Cystic fibrosis (CF) is the most common inherited disorder in Caucasian populations caused by mutation in cystic fibrosis transmembrane conductance regulator (CFTR). The type of mutations and their distributions varies widely between different countries and/or ethnic groups. The aim of this study was to characterize mutations involved in this disease in Mazandaran province, Iran. Materials and Methods: In this descriptive study thirty unrelated Iranian cystic fibrosis patients were screened for deltaF508, N1303K, G542X, R347H and W1282X mutations in the CFTR gene using Reverse Dot Blot method during 2004-06. This technique uses biotinilated PCR products for simultaneous hybridization with several normal and mutant probes specific to known mutations fixed on Biodyne C membranes. Results: DeltaF508 mutation was found in 13 (21.66%) alleles. 6 patients were homozygote and one was compound heterozygote for this mutation. Conclusion: These findings reveal an important heterogeneity of CFTR gene mutations in Mazandaran Province. Thus regarding the relative low rate of detectable mutations, it is necessary to undertake larger studies for molecular diagnosis of cystic fibrosis in this province.

According to Kurtzke, Iran is considered to have a low prevalence. In this study, we wanted to estimate the prevalence period and Indicate Epidemiologic aspect of MS in Mazandaran Province, North of Iran.Materials and methods: A cross- sectional Registered case Study Conducted in 2007 on all definite MS Patients that live and reside within Mazandaran, as well az being a member of the Mazandaran MS Society. 582 difinite patients (161 men and 421 woman) were identified. The patients were Viu interview and questionaire to record demographic and Case related information. According to the national Sensius Reports, the population of Mazandaran was 2,893,087.Results: The mean (SD) age of the participants was 34.3 (9.4) years. The MS period pervalence was 20.1 per 100,000{95% confidence interval(CI) 18.7-22.1}. The Female/male ration was 2.6 and MS rates were highest among 3rd decade. Visual and sensory distorbancec were thw most common initial persontations with prevalence of 40.1 %( 95% CI: 38.5-42.9 ) and 34.2% ( 95% CI: 32.8 – 36.1), respictively.Conclusion: Mazandaran is in medium MS prevalence area. Background and purpose: Due to the high annual birth rate of thalassemia major in our country, its prevention by prenatal diagnosis is of important priority. Gene mutation remains unknown in 10-20% of thalassemia trait people in Iran. In these cases, linkage analysis using polymorphic sites which are located near or within the gene is necessary to follow the mutant or the normal chromosome. SspI polymorphic site which is studied for the first time in Iran is located in the second intron of beta globin gene. The aim of this study was to determine the polymorphism frequency of this site in Mazandaran province.Materials and Methods: Peripheral blood of 211 thalassemia trait patients living in Mazandaran province was collected. After DNA extraction and amplification of the beta globin gene region containing the SspI polymorphic site, the effect of SspI restriction enzyme was evaluated on agarose gel.Results: In 422 analyzed chromosomes, 20.6% were negative for SspI polymorphic site. Negative sites were almost equally associated with normal and mutant alleles (11.9% and 14.3% respectively).Conclusion: SspI site analysis can be applied to follow the normal or mutant alleles of beta globin gene.

Background and purpose: Leptospirosis is an uncommon widespread spirochetal zoonoses caused by the members of the genus leptospira. Human disease is acquired by coming into contact with urine or tissues of infected animals, or through contaminated water and soil, thus is an occupational disease particularly in farmers, slaughterhouse workers and fishermen. The goal of this study was to evaluate the clinical and Paraclinical findings of leptospirosis cases in Mazandaran.Materials and Methods: In this cross-sectional descriptive study, patients with high suspicion to leptospirosis presenting to Imam Khomeini and Razi hospitals of Sari and Qaemshahr from June to September 2004 with positive serology finding (MAT) were enrolled. Demographic, clinical and Paraclinical data for each patient were recorded.Results: From 63 patients suspicious for leptospirosis, 36 had positive serologic test. The mean age of the positive patients (25 male, 11 female) was 34.2 ± 13.77 (range 14-64 years old). Most cases (86.1%) were farmers and 83.4% villagers. 55.6% of the patients presented in July. Fever and chills were the main complaints in 42.9% of the cases and fever, weakness, headache, anorexia and mialgia were seen in 96.6%, 96.6%, 93.1%, 89.7% and 85% respectively.Thrombocytopenia and leucopenia were noticed in 45.7% and 33.4% of the patients respectively. AST & ALT in 51.7% & 55.2% of patients were above the normal upper limit. Weil's syndrome and ileus occurred in 7 (19.5%) and one (2.8%) cases respectively.Conclusion: The classic history of fever, weakness, headache, anorexia and mialgia in villagers and farmers in warm months should raise the possibility of leptospirosis.

Background and purpose : Malaria is one of the most fatal infectious diseases which annually kills more than 2 million people world wide. This parasitic infectious disease with global incidence rate of 300-500 million is one the most important complaints of our health system despite of strict preventive and treatment programs in IRAN. Because of special condition of Mazandaran climate we have both the sporadic and the endemic forms of this disease in different parts of our province. The Purpose of the present study was to define the epidemiologic condition of the disease through an incidence survey between 1999-2003 in Mazandaran province.Materials and methods : Present study is a descriptive study (old data) of 184805 files during a five years period between 1999-2003 in all of the 14 health centers of Mazandaran province. Demographic and other data were collected through a questionnaire consisting eleven questions. The data were then analyzed using descriptive statistical methods by SPSS software.Results : 518 patients were found infected with different kinds of malaria parasites, of whom 80.3% were the Afghan refugee, 13.7% were locally infected and 5% of them were passengers who came to Mazandaran from other Malaria endemic areas. Most of the cases (57.1%) belong to the age group (21-30 years) and 88.4% of them were male. Plasmodium Vivax was the most common parasite (96.9%). We noticed %150 increase in annuall parasite Incidence (API) during 1999-2003.Conclusion : Considering the high, Incidence rate of the disease in Mazandaran Province as well as high immigration rate from infected neighbour counties and special condition of Mazandaran climate, compaign against Malaria must continue. The incidence rate of malaria from 0.02% to 0.05% during these five years period. There for we need to enforce malaria control programs increased and improve the health system.

Background and purpose: Previous studies indicate an important genetic factor in the etiology of β-Thalassemia and bipolar mood disorder. There has been several case reports implicating a possible association between the two conditions. But the results of a cross sectional study was not reconfirming. Regarding different patterns of mutations in different geographical areas, this study was performed to evaluate the genetic association between these two conditions in Mazandaran province in which β–Thalassemia is prevalent.Materials and methods : This case-control study was conducted on 110 patients with bipolar I disorder and 118 persons without any history of mood disorders in them and their family. Hemoglobin electrophoresis was performed in all cases and controls that showed microcytosis in their CBC. Hemoglobin A2 between 3.4 to 7 was considered for diagnosis of β-Thalassemia minor.Results : 8.18% of cases and 4.24% of controls were diagnosed to be heterzygote for β-Thalassemia, the difference was not statistically significant.Conclusion : The reported genetic relationship between Bipolar disorders and heterozygous β-Thalassemia probably has been a “false association”. However, as there are different mutations for minor thalassemia in different populations, the authors are aware of low external validity of this study, and suggest farther studies on different populations.

Background and purpose : Serological studies revealed that toxoplasmosis has world wide distribution. It is a zoonosis and humans are infected through contact with infected animal, consuming infected food and congenital infection. Due to the different ways of transmission, prevalence of infection may vary between different groups of people. This study was carried out to determine the prevalence of toxoplasmosis in Mazandaran rehabilation centers.Materials and methods : 336 individuals (50% of population) were randomly selected from a total of 647 mental patients resided in 10 rehabilitation centers. Five milliliter (ml) blood were taken from each individual and sera were separated. Sera were diluted 1:50 and antitoxoplasma antibody detected using indirect immunofluorescense technique. Positive sera were diluted further to , , , , and the titer of antibody determined prevalence and its confidence interval (CI) were calculated. Results : 161 of the subjects were male and 175 female. 77.4% of them were positive for toxoplasma antibody (95% CI was 72.9-81.9). Prevalence in male was 73.3% (95% CI was 68.2-78.4) while in female it was 81.1% (a 5% CI was 72-88.2). The difference of prevalence between male and female was not statistically significant (P

Background and purpose: In Iran, wheat is one of the main cereals for human consumption. Wheat grains containing zearalenone can cause genital problems and reproductive disorders in domestic animals. In human, episodes of breast enlargement in young boys and sexual precocity have been reported. This toxin may therefore have a role in hormonal balance and mammary cancer in regions with high zearalenone ingestion. In this study the incidence of zearalenone in wheat samples from Mazandaran warehouses were investigated.Material and methods: A total of 118 wheat samples (1 kg each) were randomly collected from all 12 warehouses of Mazandaran province. Competitive microtitre plate enzyme immunoassay for the quantitative determination of zearalenone was used according the manufacturer instructions (Riedel-de Haen Sigma-Aldrich Laborchemikalien GmbH). Results: Zearalenone was found in 81% of samples and 64.4% of samples contained >200 μg/kg of this toxin. The minimum levels of zearalenone were 29 μg/kg. All of the warehouses were positive for the presence of zearalenone. 50-100% of samples in each of the warehouses had >200 μg/kg contamination of zearalenoneConclusion: Our results showed a high percentage and levels of contamination of wheat samples with zearalenone. With regard to proposed standards given by the Standard Administration of Iran, under which the content of zearalenone should not exceed 200 ppb in food products, 64.4% of samples meet these criteria.

Background: Growing national concern about distortions in the size, specially composition, and availability of the physician workforce -especially after "cultural revolution n- has evoked challenges in Iran. Purpose: To determine various factors that influence medical graduates choices for residency program. Methods: All applicants for residency program in Mazandaran university of Medical Sciences and Health Services completed the Medical School Graduation Questionnaire, and rated each factor using 0 to 4 Likert-type scale. Factors' ratings were also compared across applicants of different residency program, and demographic variables. Results: The top two factors rated as having strong influences were ones related to interest in helping peop1e (rated 3.07), and intellectual content of the specially (rated 3). Malpractice insurance cost has the least influence (rated 0.98). Most of men preferred independence, whereas most of women preferred predictable working hours. Opportunity to make differences in people's l(fe influenced the specially choices of usual participants. whereas those who used war veterans quota paid more attention to independence and exercise of social responsibility. Patient contact factors were less important to graduates who chose diagnostic speciafties. Also, there was a significant association between the participants' age and four factors. Conclusion: These graduates based their specially preference heavily on the opportunity that the specially affords to help people, and intellectual content of the specially. Knowing the hierarchy of influences on graduates' motivations should help education strategists determine what experiences and perceptions must change if a different mix of specially decision is to result. SummaryBackground and purpose : Acute poisoning due to suicide , is one of the most important factors providing social and problems in developing countries. This study was designed to determine the prevalence of attempted suicide by poisoning in the west of Mazandaran.Patients and Methods : A total of 301 intentional Poisoning cases referred to hospitals located the west of Mazandaran (Tonekabom and Pamsar) during 1994-1997 , were included in this study. Various parameters in relation to poisoning and patients been investigated retrospectively in these cases.Results : In this study , 97.1% of patients were outpatient and others were hospitalized. Data in these cases showed greater rate of poisoning in males(53%) than famales(47%) , but the mortality rate was higher in females (15 versus 2 cases). Most poisoning victims were at the age range of 16-25 years. Oral ingestion was the only route of intoxication. Most cases of intentional poisoning were referred to hospitals at the sfternoon. Attempt to suicide with drugs was more than pesticides (41.5% versus 31.5%) , and other agents such as chemicals (15.9%) , opium (7%) and unknown causes (4.1%). Among the drugs , benzodiazepine (18%) was the first, followed by cardiovascular agents (9%), antiepileptics (6%)antidepressants (5%) and neuroleptics (3%). In 49.3% of cases, specific antidotes were used. Finally, about 90% of the patients recovered without any sequelae, 3.3% were referred to other centers and 5.6% died.Conclusion : More careful consideration should be given to limit to limit and control the availability of drugs and chemicals (such as organophosphate pesticides). Attention should ne paid to special groups such as females and adolescents. Further attempts to establish poison information and control centers in different parts of this province is highly suggested.

SummaryBackground and purpose : Beta thalassemia is one of the most prevalent genetic disease in Iran and Mazandaran.This study was designed to determine the various mutatious in beta thalassemia gene in patients referred to Boo Ali Sina hospital during the year 1373.Materials and Methods : This descriptive study was done on 44 Thalassemic families (affected child and his/her parents). About 10 milliliter heparinized blood was collected from each case and sent to the Prenatal Diagnosis Center of London Universitu. Tlhe which blood cel DNA was extracted and studied by polymerase cgain reaction method (PCR) and amplification refractory mutation system ARMS.Results : 44families from different areas of the province were studied. 18 percent of them had consanguineous marriage. The patients age were 23 boys (52.3%) and 21 girls (47.7%). The patients aged between 0.8 to 25 (Mean 9) years. Ten different mutatvions were found ; The most prevalent one was IVII-I(G A) whch was detected in 68.7% of chromosomes, 24 cases were homozygote and 12 other cases were compound heterozygote. Mutations were not found in 8 patients(18.2%).Conclusion : Exact infection about brta thalassemia gene mutations in each area are necessary and useful for presentation of prental diagnostic services
